Objective To study the immediate conservative breast surgical reconstruction by trans-ferring the same lateral latissimus dorsal myocutaneous flap (LDM) for the treatment of stage Ⅱ and stage Ⅲ breast cancer, combined with neoadjuvant chemotherapy. Methods Breast cancer patients in stage Ⅱ and Ⅲ with pathological diagnosis by core needle biopsy, had undergone immediate conservative breast reconstruction surgery with transferring the latissimus dorsal myocutaneous (LDM). We scored the reconstructed breast twice at one month after operation and completion of radiation. The final score was decided using the mean value of the twice evaluating score. Kaplan-Meier survival was used to analyze the survival of 18 cases with traditional mastectomy. Results The tumor size ranged from 30 mm to 55 mm before neoadjuvant chemotherapy, and ranged from 25mm to 45 mm after neoadjuvant chemotherapy. The median weight of the tumor specimens in breast conservative operation was 140 g (90 g to 220 g). A-mong the 18 patients, 16 cases had more than 3 scores with satisfied cosmetics (93.33 % ). Dorsal subcu-taneous seroma in donation area was observed in 5 of 18 patients (27.78 %) and dorsal incision dehiscence was observed in 2 of 18 patients (11.11%). None of the patients developed recurrence except that 1 pa-tient (5.55 %) had distance metastasis (bone metastasis} in a mean 22-months follow-up. The Kaplan-Meier survival curve in breast conservation reconstruction patients had no different from that of traditional mastectomy patients at the same time. 目的 探讨保留乳头乳晕复合体(NAC)的乳腺癌改良根治术即刻胸大肌包裹假体植入乳房重建的可行性.方法 对28例0、I、II期乳腺癌行保留皮肤的乳腺癌改良根治术后,即刻于胸大肌后方植入硅胶假体重建乳房,并根据冰冻切片结果决定是否保留NAC.结果 28例早期乳腺癌均保留了NAC,术后随访2~18个月(中位随访期:15个月),外观良好,双侧乳房对称,优良率达96.5%;均无局部复发或远处转移,无明显术后并发症.结论 保留NAC的乳腺癌改良根治术后用硅胶假体行即刻乳房重建,能达到满意的乳房美容效果,是治疗早期乳腺癌安全可行的方法.  相似文献

Background Recent published series demonstrate the safety and effectiveness of skin-sparing mastectomy (SSM) with immediate reconstruction for the treatment of early-stage breast carcinoma. Although several reports have retrospectively evaluated outcomes after breast reconstruction for locally advanced disease (stages IIB and III), no study has specifically considered immediate breast reconstruction after SSM for locally advanced disease. Methods From 1996 to 1998, 67 consecutive patients with breast carcinoma underwent SSM with immediate reconstruction and were prospectively observed. From this group of patients, those with locally advanced disease (stage IIB, n=12; stage III, n=13) were analyzed separately. Tumor characteristics, adjuvant therapy, type of reconstruction, operative time, complications, hospital stay, and incidence of local recurrence and distant metastasis were noted. Results Breast reconstruction consisted of a transverse rectus abdominis myocutaneous flap (n=22) or a latissimus flap plus an implant (n=4). The median operative time was 5.5 hours; the average hospital stay was 5.2 days. Complications required reoperation in three patients (12%): partial skin flap necrosis in two and partial abdominal skin necrosis in one. Surgery on the opposite breast for symmetry was required in one patient (4%). Postoperative adjuvant therapy was not significantly delayed (median interval, 32 days). With a median length of follow-up of 49.2 months (range, 33–64 months), local recurrence was present in only one patient (4%), with successful local salvage treatment, and distant metastasis was present in four patients (16%). Conclusions SSM with immediate reconstruction seems safe and effective and has a low morbidity for patients with advanced stages of breast carcinoma. Local recurrence rates and the incidence of distant metastasis are not increased compared with those of patients who have had modified radical mastectomies without reconstruction.  相似文献

目的 研究乳腺癌改良根治术联合扩大背阔肌肌皮瓣进行即时乳房再造的方法及治疗效果.方法 自2000年1月至2007年12月选择162例乳腺癌行乳腺全切除(包括传统的改良根治术32例,保留皮肤的乳房切除术51例,保留乳头乳晕的乳房切除术79例),单纯应用扩大背阔肌肌皮瓣进行即时乳房再造.结果 手术成功161例,失败1例.术后再造乳房外形良好,患者自我评定"优良"率达到91.93%(148/161).术后6例出现乳头部分坏死,5例胸部皮肤挫伤、表皮脱落,自行愈合.11例出现背部血清肿,8例穿刺抽液愈合,2例刮除窦道假膜愈合,1例切除纤维囊愈合.3例切缘皮肤局部坏死,1例背部供区部分坏死,二次手术愈合.1例移植物缺血坏死,予以切除.随访时间7~90个月,失访23例,失访前均无病生存.2例骨转移,1例肺转移,1例锁骨上淋巴结转移,无局部复发. 结论改良根治术联合Ⅰ期扩大背阔肌肌皮瓣乳房再造,安全有效,尤其适合中、小体积乳房.  相似文献

目的 总结保留乳头乳晕复合体( nipple-areola complex,NAC)的乳腺癌改良根治术经验,以期提高手术成功率。方法 对1998年1月至2009年12月行保留NAC乳腺癌改良根治术的159例患者的临床病理资料进行回顾。结果 159例患者中成功实施保留NAC乳腺癌改良根治术141例(88.68%)。术中放弃保留NAC而改变为乳腺癌改良根治术或根治术14例(8.81%),其中4例(2.52%)为术中发现NAC缺血,5例(3.14%) NAC有癌浸润,2例(1.26%)为重度不典型增生,3例(1.89%)因术中发现Ⅲ级淋巴结肿大而更改术式;术后乳头完全坏死导致手术失败4例(2.52%)。术后并发症:乳头部分缺血坏死16例(10.06%),皮瓣坏死、感染11例(6.92%),皮下积液10例(6.29%)。患者均获得随访,随访时间15 ~96个月(中位随访时间51个月)。术后局部复发5例(3.14%),远处转移2例(1.26%),NAC及其基底部未见复发。结论 保留NAC乳腺癌改良根治术在严格入组条件和手术流程的情况下是安全的术式,导致改变术式的常见原因为NAC癌浸润、NAC缺血坏死及术中分期改变等。  相似文献

目的探讨用硅胶假体植入法修复乳腺癌改良根治术后乳房缺失的疗效。方法2005年6月至11月,10例患者行保留皮肤的乳腺癌改良根治术后,同期于胸大肌后植入硅胶假体再造乳房,并根据冰冻结果决定是否保留乳头乳晕复合体。结果本组病人术后无伤口积液感染,皮肤坏死及异物反应等。10例患者美观效果均满意,其中有4例保留乳头乳晕复合体。所有病人进行术后随访。随访时间为1~6个月。未发现有远处转移和局部复发。无上肢水肿及功能障碍。结论保留皮肤的乳腺癌改良根治术后用硅胶假体行一期乳房再造,具有创伤小、安全、简单、恢复快的特点;再造后乳房美观,效果满意。  相似文献

目的:探讨改良根治术全乳切除联合一期扩大背阔肌肌皮瓣乳房再造术治疗乳腺癌的临床疗效。 方法:回顾性分析2008年3月—2013年3月行乳腺全切除的150例乳腺癌患者的临床资料,其中传统的改良根治术28例,保留皮肤的全乳切除术47例,保留乳头乳晕的全乳切除术75例,均同期行扩大背阔肌肌皮瓣乳房再造术。 结果:手术成功149例,失败1例。再造乳房外观评价“优良”率达91.27%(136/149)。术后发生乳头部分坏死4例,发生胸部皮肤挫伤、表皮脱落3例,未经特殊处理,自行愈合;出现背部血肿8例,其中6例经穿刺抽液愈合,1例经切除纤维囊愈合,1例经刮除窦道假膜愈合;切缘皮肤局部坏死2例,背部供区部分坏死2例,经二次手术愈合;移植物缺血坏死1例,给予切除处理。随访8~73个月(失访18例,失访前均无瘤生存),骨转移2例,肺转移1例,锁骨上淋巴结转移1例,无局部复发患者。 结论:乳腺癌改良根治术乳腺全切除联合I期扩大背阔肌肌皮瓣乳房再造术,安全有效,尤其适用于中小体积乳房的患者。  相似文献

目的:探讨保留乳头乳晕乳腺癌改良根治术后即刻扩展型背阔肌乳房再造术的安全性与疗效。方法:回顾性分析蚌埠医学院第一附属医院肿瘤外科32例行保留乳头乳晕乳腺癌改良根治术后即刻扩展型背阔肌乳房再造术患者(观察组)和34例行传统乳腺癌改良根治术患者(对照组)临床资料,比较两组患者并发症、预后,并分析观察组重建乳房外形评分情况。结果:两组年龄、肿瘤大小、病理分期方面无统计学差异(均P0.05);观察组与对照组术后总并发症发生率差异无统计学意义(37.5%vs.32.4%,P0.05)。所有患者随访18~60个月,中位时间44个月,观察组局部复发2例,远处复发3例;对照组局部复发3例,远处转移3例,观察组与对照组3年无瘤生存率差异无统计学意义(87.5%vs.91.2%,P0.05)。观察组乳房外形评分优良率为90.6%。结论:保留乳头乳晕乳腺癌改良根治术后即刻扩展型背阔肌乳房再造术安全有效,重建的乳房外观满意,且不增加并发症与局部复发、远处转移风险。  相似文献

目的:探讨保留乳头乳晕复合体的乳腺癌改良根治术后行即时以扩展型背阔肌肌皮瓣行乳房再造的安全性和手术技巧。方法:28例早期乳腺癌患者行保留乳头乳晕复合体乳腺癌改良根治术,术后即刻使用扩展型背阔肌肌皮瓣再造乳房。术中改进:注意保护乳房皮肤的感觉神经、保留胸背神经及二级分支,采取皮瓣翻转法乳房塑型,行腋窝填塞和腋前皱襞的重建以及切取皮瓣的组织量大于切除的20%~50%腺体量等。结果:28例即时乳房再造全部获得成功。随访12~36个月,均生存,无局部复发,其中1例术后10个月骨转移,1例2年后肺转移;美学效果:优18例,良6例,一般2例,差2例;再造乳房皮肤的感觉、乳头的竖起功能存在,自然下垂,外形对称,腋窝饱满,患侧肢体运动范围较常规改良根治术明显增大。结论:对于选择合适的病例,保留乳头乳晕复合体的乳腺癌改良根治术后应用即时扩展型背阔肌肌皮瓣再造术安全、有效,术中几点合理的技术改进可以明显提高再造乳房的质量和自然度。  相似文献
