Abstract Periapical pathology indicating endodontic infection, when present in marginal periodontitis-affected teeth, has recently been shown to be an aggravating factor in progression of marginal destruction. This has been associated with patency of dentinal tubules in the tooth cervix, an area normally devoid of cementum following periodontal therapy. These studies are, however hampered by that only circumstantial evidence such as presence of periapical destruction have been applied as criteria of endodontic infection. The purpose of the present investigation was to study to what extent a predefined selection of endodontic pathogens inoculated in the root canal can influence periodontal pathology and healing in areas of the root covered by or devoid of cementum, using root resorption as a histomorphometric marker. Exposed dentine surfaces, in the present study showed significantly larger areas of resorption in infected roots compared to non-infected roots, while cementum surfaces showed an almost identical distribution of tissue reactions regardless of root canal infection or not. It was concluded that endodontic pathogens or their products were not able to penetrate the cementum barrier. The significantly larger areas of resorption on exposed dentine surfaces in infected roots compared to non-infected roots indicated that endodontic pathogens or their products could spread through dentinal tubules to a root surface void of cementum. Extrapolated to the marginal situation this indicated that endodontic pathogens in the root canal might be able to aggravate marginal infection in areas of the root devoid of cementum.  相似文献

Abstract Large experimental defects with marginal communication, and small isolated experimental defects were created on the root surfaces of extracted monkey lateral incisors with either open or closed apices. The pulp tissue was either infected or removed, and calcium hydroxide placed in the root canal. The teeth were then replanted and the healing pattern evaluated histomorphometrically after 20 weeks. From the results, it was concluded that (1) an intrapulpal infection promotes marginal epithelial down-growth on a denuded dentin surface irrespective of tooth developmental stage, and that (2) the periodontal healing potential after calcium hydroxide-treatment appears to be higher in teeth with open apices compared with teeth with closed apices, where ankylosis was promoted as opposed to teeth with open apices where significantly more reparative cementum was found.  相似文献

Abstract The purpose of the present investigation was to compare clinical periodontal healing in periodontally involved teeth with and without pulpal pathosis. The investigation was conducted as a retrospective study on a consecutive referral population. The periapical conditions in endodontically involved single-rooted teeth from a selected patient sample were evaluated and correlated with their periodontal healing pattern. Multiple regression analysis of the registered variables showed that initial mean pocket depth and time elapsed after treatment significantly influenced change in pocket-depth. Non-surgical treatment of periodontal pockets exceeding 2.5 mm in teeth with horizontal marginal defects, over the observation period, showed significantly reduced mean pocket depth reduction in teeth with periapical pathology compared to teeth without periapical pathology. It was, furthermore, evident that proximal restorations, abutments for fixed bridges and root fillings with and without dowels did not significantly influence pocket depth reduction in the present material. It was concluded, based on the present results, that a root-canal infection, evident as a periapical radiolucency, if left untreated may in the long term perspective result in retarded or impaired periodontal healing following periodontal therapy and, consequently, should be given appropriate consideration when coordinating endodontic therapy and periodontal treatment.  相似文献

Abstract The purpose of the present study was to compare cementum surfaces after etching at neutral or low pH in both healthy monkey teeth and periodontitis-affected human teeth. 16 monkey teeth and 16 human periodontitis-affected teeth were used. Etching with phosphoric and citric acids as well as EDTA was performed on the following surfaces: healthy monkey cementum, human cementum surfaces coronal and apical to the level of periodontal breakdown as well as exposed human dentin surfaces. Results indicate a profoundly higher capacity of EDTA to selectively expose collagen fibers in both healthy cementum surfaces and periodontitis-affected dentin surfaces compared to agents operating at low pH which seemed to erode the surfaces to varying degrees. Variable results were seen on cementum surfaces which had been exposed to the environment of the periodontal pocket or the oral cavity. In view of this, it would seem preferable to mechanically remove the superficial layer of “diseased” cementum prior to the etching procedure. In conventional periodontal surgery, etching may be of limited value. However, in regenerative procedures, exposure of an intact collagenous matrix provides a matrix for retention of implants of biologically active substances such as growth factors, in addition to serving as a biocompatible surface for periodontal ligament cell colonization.  相似文献

Abstract – Despite the established anatomical relationship between the periodontal and pulpal tissues, bacterial migration between endodontium and periodontium is still under discussion. The objective of this study was an investigation of profiles of periodontal pathogens in pulpal and periodontal diseases affecting the same tooth by means of 16S rRNA gene directed polymerase chain reaction (PCR). 31 intact teeth with both pulp and marginal infections were investigated. The diagnosis was based on clinical and radiological examination. Samples were taken from the gingival sulcus or periodontal pocket, respectively, with sterile paper points before trepanation of the teeth. After trepanation sterile paper points and Hedstroem files were used for taking samples from the root canal. Specific PCR methods were used to detect the presence of the following pathogens: Actinobacillus actinomycetemcomitans, Bacteroides forsythus, Eikenella corrodens, Fusobacterium nucleatum, Porphyromonas gingivalis, Prevotella intermedia and Treponema denticola. In addition, quantitative competitive PCR was used to determine the total bacterial count of the samples. The investigated pathogens were proven to be present in the endondontium in all disease categories. Particularly in endodontic samples of "chronic apical periodontitis" and "chronic adult periodontitis" profiles of the periodontal pathogens were found. The results confirmed that periodontal pathogens often accompany endodontic infections and supported the idea that the periodontic-endodontic interrelationships should be considered as critical pathways which might contribute to refractory courses of endodontic or periodontal diseases.  相似文献

The present investigation was designed to evaluate the potential for reformation of connective tissue attachment on exposed and planed root surfaces by preventing the dentogingival epithelium and the gingival connective tissue from interfering with healing following periodontal surgery. Following the elevation of soft tissue flaps, the buccal and proximal alveolar bone of 24 teeth (48 roots) was removed to mid-root level in 6 monkeys and the exposed root surfaces were carefully planed in order to remove the root cementum. Before the flaps were repositioned and sutured, a membrane (Millipore® filter) was placed over the denuded part of the root surfaces of 16 teeth (test teeth) in order to prevent the epithelium and the gingival connective tissue from interfering with healing. The membrane was adjusted to cover the tooth surfaces from midcrown level to approximately l mm apical to the bone crest. No membranes were placed around the remaining 8 teeth (control teeth) before flap repositioning. The animals were sacrificed 6 months after surgery. The jaws were removed and histological sections of test and control teeth including their buccal periodontal tissues were produced. Nine of the test teeth had to be excluded from examination due to technical failures in the surgical procedure or tissue preparation. New cementum with inserting collagen fibers was observed on all remaining 14 test roots. The length of this newly formed fibrous attachment corresponded to approximately 50% of the distance from the apical extension of root planing to the cemento-enamel junction. In the majority of the control teeth no new attachment had formed but a “long” junctional epithelium was lining the root surfaces to the apical extension of root planing. In 3 control roots a small amount of new cementum with inserting collagen fibers was found in the most apical area of root planing. The results showed that the reformation of a connective tissue attachment was considerably favored by the placement of membranes which prevented the dentogingival epithelium and the gingival connective tissue from interfering with healing.  相似文献

Abstract The aim of the present study was to examine if new cementum and new attachment may form during healing of a wound prepared in such a way that preference is given to periodontal ligament ceils to repopulate the wound area adjacent to a root which has been surgically deprived of its periodontal ligament and cementum layer. The maxillary lateral incisors and mandibular canines in three monkeys were used for experimentation. Following elevation of a mucoperiosteal flap, the buccal and approximal alveolar bone was removed within an area extending from the mid-root level to a level 2 mm apical to the marginal bone crest. Following bone removal, the root surfaces were curetted in order to remove the cementum layer. Notches were prepared in the roots to demarcate the denuded root portion. Prior to repositioning of the tissue flap a millipore filter was placed over the treated area in order to prevent the gingival connective tissue from coming into contact with the root surface during healing. The animals were sacrificed 6 months after surgery. The jaws were removed and histological sections of the experimental teeth and surrounding periodontal tissues were produced. New cementum with inserting collagen fibers was observed on the curetted root surfaces. However, this result of healing did not consistently occur along the entire length of the curetted root portion. In the coronal part of the wound, healing was frequently characterized by connective tissue adhesion to the root surface without signs of cementum formation and fibrous attachment. The results of the experiment suggest that the periodontal ligament cells possess the ability to reestablish connective tissue attachment.  相似文献

The effect of the endodontic status of the tooth on periodontal reattachment after the raising of a mucogingival flap was studied with the use of 48 teeth in four old baboons. Root canal therapy was performed on 26 front teeth. Tubliseal, AH26, N2, and Diaket were used as sealers with gutta-percha. The pulps of 5 teeth were exposed and left infected; 17 teeth with intact pulps served as a control. The teeth and their surrounding structures were studied histologically for reattachment with new cementum formation 2 to 17 days, 1 to 2 months, and up to 1 year after the operation. In the teeth examined 1 month postoperatively or more, if epithelium had not covered the denuded surface, new cementum was present, regardless of the endodontic status.  相似文献

An enamel matrix protein derivative (Emdogain) has been recently shown to promote periodontal regeneration in experimentally created recession-type defects. However, only limited histologic data from human material are available concerning the healing of intrabony periodontal defects following treatment with Emdogain. The aim of the present study was therefore to present the clinical and histologic results following the application of Emdogain in intrabony defects. Two patients with marginal periodontitis and deep intrabony defects adjacent to teeth scheduled for extraction were treated with Emdogain. The postoperative healing phase was uneventful in both cases. At 6 months following treatment, newly formed cementum with inserting collagen fibers was found in both specimens. In one case, the new attachment formation was also accompanied by bone neoformation. The results of this human histologic study indicate that Emdogain possesses the potential to stimulate new connective tissue attachment formation in human intrabony defects.  相似文献

Abstract The purpose of the present investigation was to explore possible relationships between clinical periodontal status in periodontally involved teeth with and without endodontic infection. The investigation was conducted as a retrospective study on a consecutive referral population. The periapical conditions in endodontically-involved single-rooted teeth from a selected patient sample were evaluated and correlated to their periodontal status. There was a significant correlation between periapical pathology and vertical bony destructions. An intra-individual comparison between pocket depth in teeth with and without periapical pathology showed that periapical pathology was significantly correlated to an increased pocket depth in the absence of a vertical bony destruction. It was concluded that an endotontic infection, evident as a periapical radiolucency. promotes periodontal pocket-formation on an instrumented marginal root surface and, consequently, should be regarded as a risk factor in periodontitis progression and be given appropriate consideration in periodonlal treatment planning.  相似文献

An immunohistochemical technique was used to investigate the distribution of cementum-associated LPS in 14 human teeth extracted because of periodontal disease. Autologous affinity purified anti-LPS antibodies were obtained against a preparation of oral LPS types bound to an epoxy-activated Sepharose 6B-polymyxin B column. The concentration of immunoglobulin classes G. M and A in the prepared sera was assayed by rocket immunoelectrophoresis and the reactivity of the sera was checked using an ELISA to the LPS preparation. Ground sections of the teeth were prepared and incubated with prepared autologous antiserum, followed by detection of Ig binding by a rabbit PAP system. LPS was detected on cementum which had been previously exposed to a periodontal pocket and extended to within I mm of the previous level of connective tissue attachment. LPS was only detected on the surface of cementum and on no occasion was LPS seen penetrating into sub-surface cementum. These results suggest that extensive removal of sub-surface cementum is not necessary in order to render root surfaces free of bacterial endotoxins during root planing procedures.  相似文献

In this study the bacterial invasion in root cementum and radicular dentin of periodontally diseased, caries-free human teeth was examined. In addition, structural changes in these tissues, which could be related to the bacterial invasion, were reported. Twenty-one caries-free human teeth with extensive periodontal attachment loss were studied by light and scanning electron microscopy. At the base of the gingival pocket, bacteria were found in the spaces between remnants of Sharpey's fibers and their point of insertion in the cementum. In teeth that had been scaled and root planed, most of the root cementum had been removed. Bacterial invasion was found in the remaining root cementum. The invasion seemed to start as a localized process, often involving only one bacterium. In other areas bacteria were present in lacunar defects in the cementum. These lacunae extended into the radicular dentin. In 11 teeth bacteria had invaded the dentinal tubules. Most bacteria were located in the outer 300 microns of the dentinal tubules, although occasionally they were found in deeper parts. In two of the nontreated teeth, bacteria were detected on the pulpal wall. No correlation was found between the presence of bacterial invasion and the absence of radicular cementum. No bacteria were found in the portion of the root located apically to the epithelial attachment. These data are in agreement with our results from cultural studies of the bacterial flora in these structures. It was also demonstrated that in spite of meticulous scaling and root planning and personal oral hygiene, bacterial plaque remained present on radicular surfaces. Both the invaded dentinal tubules and the lacunae could act as bacterial reservoirs from which recolonization of treated root surfaces occurs. From these reservoirs bacteria could also induce pulpal pathoses. Since these bacterial reservoirs are not eliminated by conventional mechanical periodontal treatment, it seems appropriate to combine mechanical periodontal therapy with the use of chemotherapeutic agents.  相似文献

BACKGROUND: Periodontal disease is marked by inflammation and damage to tooth-supporting tissues. In particular, damage occurs to factors present in cementum that are thought to have the ability to influence the regeneration of surrounding tissues. Bone sialoprotein and osteopontin are major non-collagenous proteins in mineralized connective tissues associated with precementoblast chemo-attraction, adhesion to the root surface, and cell differentiation. The purpose of this investigation was to determine whether the expression and distribution of bone sialoprotein and osteopontin on root surfaces affected by periodontitis are altered compared to healthy, non-diseased root surfaces. METHODS: Thirty healthy and 30 periodontitis-affected teeth were collected. Following fixation and demineralization, specimens were embedded in paraffin, sectioned, and exposed to antibodies against bone sialoprotein and osteopontin. Stained sections were assessed using light microscopy. RESULTS: Bone sialoprotein was not detected in the exposed cementum (absence of overlying periodontal ligament) of diseased teeth. In most areas where the periodontal ligament was intact, bone sialoprotein was detected for healthy and diseased teeth. For teeth reactive for bone sialoprotein, the matrix of the cementum just below the periodontal ligament was moderately stained. A similar immunoreactivity pattern for osteopontin was observed. CONCLUSIONS: The absence of bone sialoprotein and osteopontin staining along exposed cementum surfaces may be due to structural and compositional changes in matrix components associated with periodontal disease. This may influence the ability for regeneration and new connective tissue attachment onto previously diseased root surfaces.  相似文献

The effects of pulpal infection and intrapulpal application of calcium hydroxide and antibiotics were evaluated with special reference to cell and tissue reactions in the marginal hard/soft tissue interface in monkeys. Results indicate the pulpal conditions and endodontic dressings influence marginal healing and repair, provided an endodontic-periodontic pathway such as patent dentin tubules is present. These results suggest that permanent endodontic treatment, with an inert material, should be performed before the protective cementum layer is removed. Pulpally aggravated periodontal lesions should therefore not be overlooked in periodontal diagnosis and treatment.  相似文献

This study was designed to investigate human repair and reattachment of principal periodontal fibers in areas of resorption on anchor premolar root surfaces following rapid maxillary expansion. Maxillary first premolar teeth were obtained from patients requiring rapid maxillary expansion. Extraction of the teeth was scheduled after periods of retention varying between 14 and 53 weeks. The roots of the teeth were examined via light microscopy and scanning electron microscopy. Extensive root resorption characterized the buccal surfaces of anchor premolars. Repair of the resorptive defects was found to occur exclusively with cellular cementum. Anchor teeth retained for longer periods, up to the maximum of 53 weeks of retention in this study, generally demonstrated more advanced repair. Topographically, Sharpey fiber holes indicative of principal periodontal fiber insertion were found in repair cementum. However, these depressions were neither numerous nor consistent in their presence and location. In human teeth, periodontal attachment to resorbed and repairing surfaces was shown to be present. SEM studies of histologic sections revealed that periodontal fibers and fiber bundles inserted directly into the repair cellular cementum matrix, irrespective of the site of the lesion on the root.  相似文献

This report presents histologic results in humans following a laser-assisted new attachment procedure (LANAP) for the treatment of periodontal pockets. Six pairs of single-rooted teeth with moderate to advanced chronic periodontitis associated with subgingival calculus deposits were treated. A bur notch was placed within the pocket at the clinically and radiographically measured apical extent of calculus. All teeth were scaled and root planed with ultrasonic and hand scalers. One of each pair of teeth received treatment of the inner pocket wall with a free-running pulsed neodymium:yttrium-aluminum-garnet (Nd:YAG) laser to remove the pocket epithelium, and the test pockets were lased a second time to seal the pocket. After 3 months, all treated teeth were removed en bloc for histologic processing. LANAP-treated teeth exhibited greater probing depth reductions and clinical probing attachment level gains than the control teeth. All LANAP-treated specimens showed new cementum and new connective tissue attachment in and occasionally coronal to the notch, whereas five of the six control teeth had a long junctional epithelium with no evidence of new attachment or regeneration. There was no evidence of any adverse histologic changes around the LANAP specimens. These cases support the concept that LANAP can be associated with cementum-mediated new connective tissue attachment and apparent periodontal regeneration of diseased root surfaces in humans.  相似文献

The present investigation was undertaken to examine whether a new connective tissue attachment will form on previously periodontitis-involved roots when reduced but healthy periodontal ligament tissue persists following periodontal treatment and the epithelium is prevented from migrating into the wound. In each of 4 monkeys, periodontal tissue breakdown was induced around one maxillary and one mandibular second premolar or first molar by placing orthodontic elastics around the teeth. The elastics were kept in situ until about 50% of the supporting tissues had been lost. 3 months following removal of the elastics, the crowns of the teeth were resected. The pocket epithelium and subjacent granulation tissue were excised and the cementum of the periodontitis-involved part of the roots was removed using a diamond bur. The roots were covered with a mucosal flap. The animals were sacrificed after 3 months of healing. The jaws were removed and histological sections of the experimental roots including their surrounding periodontal tissues were produced. The microscopic analysis disclosed that in all roots, new cementum with inserting collagen fibers had formed in the apical portion of the previously exposed root surfaces. It was always in continuity with the original cementum layer apical to the instrumental part of the root and was thickest in its apical portion, becoming gradually thinner in the coronal direction. In the roots, which for the entire length of the study remained covered by the oral mucosa, the extension in the coronal direction of this newly formed fibrous attachment amounted to an average of 1.0 mm with a range from 0.1 mm to 2.6 mm. These findings were interpreted to mean that new attachment is formed by coronal migration of cells originating from the periodontal ligament.  相似文献

The oral cavity has long been considered a potential reservoir for respiratory pathogens. The mechanisms of infection could be aspiration into the lung of oral pathogens capable of causing pneumonia, colonization of dental plaque by respiratory pathogens followed by aspiration, or facilitation by periodontal pathogens of colonization of the upper airway by pulmonary pathogens. Several anaerobic bacteria from the periodontal pocket have been isolated from infected lungs. In elderly patients living in chronic care facilities, the colonization of dental plaque by pulmonary pathogens is frequent. Notably, the overreaction of the inflammatory process that leads to destruction of connective tissue is present in both periodontal disease and emphysema. This overreaction may explain the association between periodontal disease and chronic obstructive pulmonary disease, the fourth leading cause of death in the United States. These findings underline the necessity for improving oral hygiene among patients who are at risk and those living in long-term care institutions.  相似文献

Abstract – The purpose of the study was to evaluate periodontal healing after replantation of intentionally rotated teeth with healthy and denuded root surfaces. Ten teeth with hopeless prognoses because of extensive alveolar bone loss and deep pockets extending to the apexes only on one or two surfaces, but with a healthy periodontal apparatus on the other surfaces, were selected. A mucogingival flap was elevated and the teeth were extracted carefully so as not to damage the healthy remnant of the periodontal membrane remaining on the root surfaces. Thorough extra‐oral debridement was performed on the contaminated root surface and the remnant was carefully conserved. Retrofilling was also done to eliminate an endodontic cause of attachment loss. The teeth were horizontally rotated and then replanted so that the healthy root would face the connective tissue at the initially periodontally involved sites, and the root planed surfaces, which had been periodontally involved, would face the surfaces of the alveolar sockets at initially healthy sites. The teeth were splinted with adjacent teeth. Clinical parameters and radiographic examination were performed pre‐operatively, and at 3 or 6 months, 1, 2, and 3 years postoperatively. One tooth was extracted 1.5 years postoperatively due to reduced support and the treatment strategy of the patient. The other nine teeth were well maintained without any discomfort for the whole maintenance period of 3 years. In all teeth, areas that previously had no pocketing but were now against denuded root surfaces maintained the previous pocket depth readings. In areas where a deep pocket was present but now had a root surface with healthy periodontium, a distinct decrease of mean pocket depth was observed at the first reexamination 3 months postoperatively. The mean radiographic alveolar bone level increased from 0.3% to 45.3% in 2 years and was thereafter maintained for the entire observation period. Loss of periodontal space and possible root resorption were observed in only one case without other ankylotic symptoms. These results suggest that the healing without ankylosis of an extensive denuded root surface may occur by mechanisms other than the maintenance of a viable periodontal ligament on the root surface.  相似文献
