The aim of this study was to evaluate the clinical relevance of coronary artery calcifications detected by spiral CT, congruence with fluoroscopy (FS) and coronary angiography, and comparison with studies reporting on application of double-helical CT and ultrafast CT. Forty patients underwent spiral CT (2-mm slice thickness, table feed 3 mm/s), coronary angiography, and FS (performed in the usual manner). Stenosis and calcifications were evaluated semiquantitatively. Nineteen patients suffering from a stenosis ≥ 75 % were verified at coronary angiography. All had coronary artery calcification on spiral CT. Fluoroscopy did not detect 8 of 19 patients with a stenosis ≥ 75 % (1 vessel: n = 1; 2 vessels: n = 3; 3 vessels: n = 4). In spiral CT sensitivity was 100 % and specificity was 33 % (FS: 58 and 48 %). Positive predictive value was 83 % for spiral CT (FS: 50 %), and negative predictive value was 100 % (FS: 56 %). A significant linear increase in the calcification score was found for increasing maximal stenosis (p < 0.005). Spiral CT is more sensitive than FS in the recognition of hemodynamic relevant stenoses using the detection of coronary artery calcifications. Statistical parameters are comparable to ultrafast-CT. Spiral CT is a suitable non-invasive diagnostic technique in coronary heart disease. Coronary calcifications found incidentally in symptomatic patients at chest CT should be reported to the referring physician for further cardiological workup. Multislice CT imaging of anomalous coronary arteries   总被引:21,自引:0,他引:21  
The purpose of the present study was to evaluate the role of 16 multislice computed tomography (MSCT) to identify the origin of anomalous coronary arteries and to confirm their anatomic course in relation to the great vessels. Accuracy of coronary artery disease (CAD) detection was a secondary aim and was tested with conventional angiograms (CA) serving as standard of reference. Two hundred and forty-two consecutive patients referred for noninvasive coronary CT imaging were reviewed for the study. Sixteen patients (6.6%) with anomalous coronary arteries were detected and included as the study group. MSCT and CA images were analyzed in a blinded fashion for accuracy of anomalous artery origin and path detection. Results were compared in a secondary consensus evaluation. Accuracy ratios to detect CAD with MSCT in all vessels were calculated. Coronary anomalies for all 16 patients were correctly displayed on MSCT. CA alone achieved correct identification of the abnormality in only 53% (P=0.016). Sensitivity and specificity of MSCT to detect significantly stenosed vessels was 90 and 92%. 16-MSCT is accurate to delineate abnormally branching coronary arteries and allows sufficiently accurate detection of obstructive coronary artery disease in distal branches. It should therefore be considered as a prime non-invasive imaging tool for suspected coronary anomalies.  相似文献

Purpose: To compare the use of a new 64-slice computed tomography (CT) scanner with 16-slice CT in the visualization of coronary artery stent lumen.

Material and Methods: Eight different coronary artery stents, each with a diameter of 3 mm, were placed in a static chest phantom. The phantom was positioned in the CT gantry at an angle of 0° and 45° towards the z-axis and examined with both a 64-slice and a 16-slice CT scanner. Effective slice thickness was 0.6 mm with 64-slice CT and 1 mm with 16-slice CT. A reconstruction increment of 0.3 mm was applied in both scanners. Image quality was assessed visually using a 5-point grading scale. Stent diameters were measured and compared using paired Wilcoxon tests.

Results: Artificial lumen reduction was significantly less with 64-slice than with 16-slice CT. Average visible stent lumen was 53.4% using 64-slice CT and 47.5% with 16-slice MSCT. Most severe artifacts were seen in stents with radiopaque markers. Using 64-slice CT, image noise increased by approximately 30% due to thinner slice thickness.

Conclusion: Improved spatial resolution of 64-slice CT resulted in superior assessment of coronary artery stent lumen compared to 16-slice CT. However, a relevant part of the stent lumen is still not assessable with multi-slice CT.  相似文献

To identify patients with increased risk of having coronary artery disease (CAD), electron-beam computed tomography (EBCT) was used for years for quantifying calcifications of the coronary arteries. The first direct comparison between EBCT and conventional CT was performed to determine the reliability of widely available conventional CT for the assessment of the coronary calcium score. Fifty male patients with suspected CAD were investigated with both modalities, EBCT and conventional 500-ms non-spiral partial scan CT. Scoring of the coronary calcification was performed according to the Agatston method. Forty-two of these patients underwent coronary angiography for the assessment of significant luminal narrowing. The correlation coefficient of the score values of both modalities was highly significant (r = 0.982, p < 0.001). The variability between the two modalities was 42 %. Mean calcium score in patients with significant coronary luminal narrowing (n = 37) was 1104 ± 1089 with EBCT and 1229 ± 1327 with conventional CT. In patients without luminal narrowing (n = 5) mean calcium score was 73 ± 57 with EBCT and 26 ± 35 with conventional CT. Although images of the heart from conventional CT may suffer from cardiac motion artifacts, conventional CT has the potential to identify patients with CAD with accuracy similar to EBCT. The objective of this study is to evaluate multidetector CT (MDCT) in detecting and characterizing anomalous coronary arteries. Forty-four patients with anomalies of the coronaries were selected from a total of 1758 individuals examined with ECG-gated 4- and 16-row MDCT including thinMIP, MPR and VRT post-processing. Twenty-eight patients showed origin and course anomalies of the central coronary segments, and in this subgroup 13 were judged as malignant because of interarterial courses between the aortic root and the pulmonary trunk, either of the right coronary artery (n=11) or the left coronary artery (n=2). Twelve non-hemodynamic anomalies were found, affecting the coronary origins only (n=10) or the peripheral vessels courses (n=2). Four arteriovenous fistulas were present, all of them with complex arterial feeders. Regardless of vessel anatomy, coronary opacification was always possible by means of the systemic contrast agent, and the aberrant coronary arteries were visualized synoptically in direct relation to the great mediastinal vessels. In contrast to MDCT, selective cannulation and final diagnosis was possible in only 11 of the 20 catheter angiograms performed (accuracy of 55.0%). In conclusion, its non-invasiveness and precise visualization makes MDCT the standard of reference for evaluating anomalous coronary arteries.  相似文献

Multi-detector-row computed tomography (MDCT) has emerged as a rapidly developing method for non-invasive imaging of the heart. An understanding of ECG synchronization, contrast material administration, patient preparation and image post-processing is needed to optimize image quality. The basic technical principles and essentials of these technical basics are described here. Correctly applied cardiac MDCT allows imaging of the coronary arteries including coronary anatomy and stenosis detection. The same is true for evaluation of coronary artery bypass grafts and, to some extent, coronary artery stents. While quantification of total calcified plaque burden has been long established, coronary MDCT allows assessing plaque morphology and constitution. Recent approaches go beyond the coronaries and include evaluation of left ventricular function at rest and myocardial viability. In combination with experimental approaches for assessing aortic valve function and myocardial perfusion imaging, cardiac MDCT offers the potential for a comprehensive examination of the heart using a single breath-hold examination.  相似文献

目的:报告电子束CT检测冠状动脉钙化(CAC)的方法和初步研究结果。材料和方法:将经ImatronC-150电子束CT机检查冠状动脉的90例分为三组:(1)非冠心组;(2)可疑组;(3)冠心组。对各例冠状动脉的走行进行观察;对比分析各组CAC的发生率和钙化定量积分。结果:电子束CT能清晰地显示冠状动脉主干及主要分支。CAC发生率冠心组为95.0%(19/20例),可疑组为48.6%(17/35例),非冠心组为22.9%(8/35例),差别具有显著性(p<0.001)。发生CAC的44例中,冠心组的定量总分显著高于可疑组和非冠心组(P值分别<0.05和<0.01)。结论:电子束CT检测CAC可为早期冠心病的诊断、预测和防治提供依据。  相似文献

The purpose of this study is to asses the impact of small field-of-view (FOV) sizes on the detection of coronary artery calcifications using multislice-spiral computed tomography (MSCT). First, a static chest phantom containing calcium inserts was scanned 10 times using a standardized scan protocol. Secondly, 50 patients (28 male, 63.6±10.6 years) underwent cardiac MSCT using the same protocol. Images were reconstructed with three different FOV sizes (180×180, 220×220, 380×380 mm2). Coronary calcium scoring and risk stratification were performed for each image series. In the phantom study, the Agatston score calculated with a FOV size of 180×180 mm2 was 657.80±20.05. At a FOV of 220×220 mm2 and 380×380 mm2, the corresponding values were 657.04±21.36 and 655.04±20.74, respectively. The corresponding values in the patient study were 541.65±869.87, 541.91±872.57 and 536.61±867.81. No statistically significant differences in the calcium score were found comparing different FOV sizes. Significantly more lesions (p=0.00149) were detected in the patient study. Comparing the different FOV sizes of 180×180 mm2 and 220×220 mm2 (380×380 mm2), four (six) patients had to be assigned to different risk groups. The use of small FOV sizes resulted in an improved detection of coronary calcifications influencing the risk stratification for further cardiac events in MSCT coronary calcium scoring.  相似文献

The purpose of this study was to compare the diagnostic value of MR angiography (MRA) with conventional contrast angiography in coronary artery disease. Thirty-five patients underwent MRA and coronary angiography within 4 hours. Of these, three patients were investigated twice: once before and once after balloon angioplasty. The pulse sequence was a cardiac-triggered, single-slab, three-dimensional gradient-echo sequence, employing a spin-echo navigator echo measurement to track the variation of the diaphragm during the scan. The following segments of the coronary arteries were included in this prospective study: left main coronary artery, proximal and middle left anterior descending, proximal and middle left circumflex, proximal and middle right coronary artery, and intermediate branch, if present. In total, 176 segments were classified as normal or having a stenosis of less than 50% and as having a stenosis of more than 50%. Five patients were excluded because of lack of cooperation. Over all, 45 of 54 stenoses were detected and interpretable by MRA. Sensitivity, specificity, and positive and negative predictive values of MRA for detecting significant stenoses were 83%, 94%, 87%, and 93%, respectively. MRA identified significant stenoses within the major coronary arteries with a high degree of accuracy. Sensitivity and specificity are higher compared with exercise tests or scintigraphy or top of the precise localization.  相似文献

RATIONALE AND OBJECTIVES: The authors performed this study to evaluate the effect of window level and gray-scale threshold on the demonstration of coronary artery lumina at three-dimensional electron-beam computed tomographic (CT) angiography. MATERIALS AND METHODS: Forty-four coronary artery branches in postmortem pigs were evaluated with electron-beam CT angiography, and the findings were compared with those from conventional angiography. Images from electron-beam CT angiography were reconstructed with maximal intensity projection (MIP), multiplanar reformation (MPR), and shaded-surface display (SSD). Four categories of window level and gray-scale threshold were evaluated. RESULTS: Three-dimensional electron-beam CT angiography accurately depicted the luminal diameters of the coronary arteries compared with conventional angiography (r = 0.83-0.90, P < .0001). The length of lumina visualized at electron-beam CT angiography was significantly shorter than that visualized with conventional angiography (P < .001). The use of MPR enabled visualization of longer segments of coronary arteries than did the use of MIP or SSD (P < .05). The higher the window level and gray-scale threshold used, the smaller the coronary luminal diameters measured (P < .05). The most accurate window level and gray-scale threshold (82.6 HU +/- 29.8 and 89.5 HU +/- 29.7, respectively) were found to correspond to the attenuation of the lumina (275.8 HU +/- 58.8). Results of simple linear regression showed a strong correlation between luminal attenuation and window level (r = 0.89, P < .0001) or gray-scale threshold (r = 0.95, P < .0001). CONCLUSION: Electron-beam CT angiography shows promise in the visualization of coronary artery lumina. For accurate display of lumina, a proper window level and gray-scale threshold for three-dimensional rendering techniques should be determined and used on the basis of the attenuation of the target vessel.  相似文献

目的:研究CT冠状动脉成像在冠心病早期诊断冠状动脉狭窄定性定量判读的作用。方法:对34例临床未发生急性冠脉综合征的冠心病患者,先后进行冠状动脉造影、128排双源CT冠状动脉成像。以冠状动脉造影为"金标准",计算CT冠状动脉成像敏感性、特异性、阳性预测值、阴性预测值。结果:①与冠状动脉造影相比,CT冠状动脉成像的敏感性为68%,特异性为97%,阳性预测值为89%,阴性预测值为90%。②CT冠状动脉成像有65个血管段图像质量差,约占12%,造成图像质量差的原因主要为钙化,心跳、呼吸伪影,少部分为管腔显示不良。结论:冠心病早期诊断中,CT冠状动脉成像可用作冠状动脉造影前筛选,CT冠状动脉成像阴性的患者不必行冠状动脉造影检查;CT冠状动脉成像阳性的患者,可行冠状动脉造影进一步确认病变。  相似文献

目的:探讨多层螺旋CT冠状动脉成像技术的应用和临床价值。材料和方法:对32例(包括3例冠状动脉搭桥术后和1例冠状动脉成形术及支架置入术后病人)进行了MSCT冠状动脉成像检查,并进行CT图像重建后处理,重建出各主要血管,对冠状动脉搭桥术后的病人重建出桥血管及含支架血管。分析MSCT对冠状动脉的显示能力,并分析是否有管壁钙化和软性斑块,评价管腔狭窄及其程度。对桥血管和支架的显示及通畅性进行评价。结果:MSCT冠状动脉成像对冠状动脉近中段显示清晰,显示率达90%以上;对冠状动脉钙化和软性斑块显示良好,对冠状动脉狭窄显示较佳。7根搭桥血管其中有1根可见约75%-90%的狭窄,其余均通畅,支架显示良好。结论:MSCT冠状动脉成像可作为冠状动脉粥样硬化疾患筛选手段,对血管搭桥术和支架放置并进行术后评价有很高的价值。  相似文献

Coronary artery fistula (CAF) is a congenital condition characterized by a pathological communication between a coronary artery and a systemic vein or one of the cardiac chambers. Iatrogenic CAFs were reported to develop secondary to the rupture of coronary aneurysm. Instances of acquired CAF draining into the cardiac chambers have been described after acute myocardial infarction. Angiography is the gold standard in diagnosing CAF. We describe the case of a patient who developed a fistula draining into the middle cardiac vein on the posterior interventricular sulcus, after acute myocardial infarction, revealed by CT scan.  相似文献

双源CT诊断冠脉狭窄的价值—与冠状动脉造影对照分析   总被引:2,自引:1,他引:1  
目的:探讨双源CT冠脉成像评价冠脉中度及中度以上狭窄的准确性及可行性。方法:入选60例临床高度怀疑或已确诊冠脉疾病的患者,行双源CT扫描,扫描过程中患者心率平稳,未出现心率不齐及心律失常。患者于1周内行经皮选择性冠状动脉造影,并以冠状动脉造影作为标准,从冠脉节段及冠脉分支角度分别评价双源CT诊断冠脉中度及中度以上狭窄的准确性、敏感性、特异性、阳性预测率及阴性预测率。结果:以冠脉节段为基础分析,双源CT诊断冠脉明显狭窄的敏感性、特异性、阳性预测率、阴性预测率、准确性分别为81.14%、97.57%、85.34%、96.74%、95.13%;以冠脉分支为基础分析,双源CT诊断冠脉明显狭窄的敏感性、特异性、阳性预测率、阴性预测率、准确性分别为84.21%、93.79%、89.88%、90.06%、90.0%。双源CT与选择性冠脉造影对发现冠脉狭窄节段及检查冠脉分支病变的能力进行卡方检验,χ2分别0.625、1.041;P0.05,双源CT与选择性冠脉造影比较在发现冠脉明显狭窄差异上无统计学意义。结论:双源CT在诊断冠脉明显狭窄时有很高的准确性,可作为无创性评价冠脉狭窄及疾病的手段。  相似文献

RATIONALE AND OBJECTIVES: The R wave of the electrocardiogram is used widely as a trigger for cardiac imaging. This study was designed to determine the optimal interval between the R wave and end systole for triggering of electron-beam computed tomography (CT) in a group of patients with various heart rates who are undergoing assessment for coronary artery calcification. MATERIALS AND METHODS: A total of 862 consecutive asymptomatic patients referred for screening with electron-beam CT for coronary artery calcification were enrolled in the study. Patients' R-R, RT, and PR intervals were measured by using the software of the CT console computer. Correlation coefficients were computed and linear regression analyses were performed for all intervals measured. Results were analyzed according to patient age (three subgroups), sex (two subgroups), and heart rate (nine subgroups). Separate formulas for calculating the length of RT intervals in men and in women were developed. RESULTS: After correction for heart rate, a significant difference was found in mean RT and PR intervals between women and men, with the mean intervals in women being longer (P < .001). No significant difference was found in these intervals within the three age-defined subgroups (< or = 40, 41-60, and >60 years; P > .05). However, significant negative correlations were found between heart rates and the lengths of all measured intervals. The results of statistical analysis indicate that most of the variation in the R-R interval with different heart rates occurred in diastole and that the duration of systole was relatively constant. CONCLUSION: For optimal cardiac imaging, triggering should take place in late systole, avoiding the RT interval variability that occurs in diastole.  相似文献

目的 探讨冠状动脉多层螺旋CT成像的临床应用价值。资料与方法 38例冠状动脉进行多层螺旋CT血管造影,利用多种重建方法进行重建,分析冠状动脉的成像质量及其显示率,以及冠状动脉疾病的影像特点。结果 图像质量的优良率:优28.9%(11/38),良60.5%(23/38),差10.5%(4/38),优良率为89.5%(34/38);冠状动脉分段显示率为:右冠状动脉近段100%(38/38),中段76.3%(29/38),远段60.5%(23/38);左冠状动脉前降支近中段100%(38/38),远段81.6%(31/38),左回旋支近段100%(38/38),远段55.3%(21/38)。发现冠状动脉管壁欠光滑18例,钙化8例,管腔充盈缺损2例,冠状动脉仿真内镜发现冠状动脉夹层1例,管腔狭窄3例。结论 多层螺旋CT冠状动脉成像可以作为冠心病的一种筛选手段。  相似文献

The purpose of this study was to investigate a 3D coronary artery segmentation algorithm using 16-row MDCT data sets. Fifty patients underwent cardiac CT (Sensation 16, Siemens) and coronary angiography. Automatic and manual detection of coronary artery stenosis was performed. A 3D coronary artery segmentation algorithm (Fraunhofer Institute for Computer Graphics, Darmstadt) was used for automatic evaluation. All significant stenoses (>50%) in vessels >1.5 mm in diameter were protocoled. Each detection tool was used by one reader who was blinded to the results of the other detection method and the results of coronary angiography. Sensitivity and specificity were determined for automatic and manual detection as well as was the time for both CT-based evaluation methods. The overall sensitivity and specificity of the automatic and manual approach were 93.1 vs. 95.83% and 86.1 vs. 81.9%. The time required for automatic evaluation was significantly shorter than with the manual approach, i.e., 246.04±43.17 s for the automatic approach and 526.88±45.71 s for the manual approach (P<0.0001). In 94% of the coronary artery branches, automatic detection required less time than the manual approach. Automatic coronary vessel evaluation is feasible. It reduces the time required for cardiac CT evaluation with similar sensitivity and specificity as well as facilitates the evaluation of MDCT coronary angiography in a standardized fashion.  相似文献

Objective

Congenital coronary artery anomalies are generally incidental, uncommon and asymptomatic. Some can cause severe potentially life threatening symptoms. The common mode of studying the coronary arteries is Conventional Coronary Angiogram. ECG-gated-multidetector CT is a non invasive modality. The objective of our study was to identify rare congenital coronary artery anomalies and discuss their clinical significance.

Material and methods

A total number of 900 MDCT coronary angiograms were carried out at our institution between the period of April 2006 and October 2010. Patients with coronary artery anomaly constituted the subject of study.

Results

The incidence of anomalous anatomical origin and course of the coronary arteries in our study was 1.55%. Hemodynamical significance was seen in five patients. 3 cases of single coronary artery originating from right coronary sinus were seen. 1 case of anomalous left coronary artery arising from main pulmonary artery was seen. 4 cases of anomalous RCA arising from left aortic cusp, 6 cases of absent LMCA with separate origin of LAD and LCX were seen.

Conclusion

Multidetector row CT is a noninvasive modality in cardiac imaging. It provides superior resolution of coronary tree and its variant. No projectional vascular overlap is seen. Various postprocessing techniques outclass catheter angiography imaging. Definition of ostia and proximal course of the coronary arteries by Multidetector CT is better than catheter angiography.  相似文献

Coronary artery fistula, defined as an abnormal communication between the coronary arteries and a cardiac chamber (most commonly) or a thoracic great vessel, may result in hemodynamically significant problems due to vascular shunting in children. Echocardiography, cardiac catheterization, cardiac MRI, and cardiac CT may be used to evaluate coronary artery fistula in children. Recently, CT has played a pivotal role for the accurate diagnosis of coronary artery fistula in children. Surgical or interventional treatment is performed for hemodynamically significant coronary artery fistulas. In this pictorial review, the detailed imaging findings of coronary artery fistula in children are described.  相似文献
