The scarcer resources are in a hospital, the more carefully the quality of the services rendered--the type and the extent--must be ensured by means of resolute control. We must not reach a point where sparse resources lead to a poorer quality in services or where patients do not receive the treatment required. The purpose of this paper is to explore some aspects of a general theoretical model within which research on the organizational impacts of quality assurance programs in hospitals may be examined. Quality assurance is conceptualized as an organizational control mechanism, operating primarily through increased formalization of structures and specification of procedures. Organizational effectiveness is discussed from the perspective of the problem-solving theory of organizations, wherein effective organizations are those which maintain at least average performance in all four system problem areas simultaneously (goal-attainment, integration, adaptation and pattern-maintenance). It is proposed that through the realization of mutual benefits for both professionals and the bureaucracy, quality assurance programs can maximize such effective performance in hospitals.  相似文献

The use of programs to help administrators and staff of health care facilities assess and monitor the quality of services provided is not new. For years, quality assurance (QA) programs have provided generalized sets of standards against which hospitals and other agencies measure their performance. Recently, there has been a trend towards quality improvement (QI) programs which advocate a more proactive response to quality concerns and emphasize a decentralized use of assessment and evaluation findings to continuously improve the care provided. QI is recognized as holding great opportunities for the enhancement of professional nursing practice and the improvement of patient care. The oversight of clinical research in the UK is currently in a state of flux. Discusses the quality assurance problems that have arisen in the management of research and the protection of the rights of human participants. Contrasts clinical governance and regulatory approaches to research quality assurance and performs a critical analysis of the Department of Health (England) Research Governance Framework (RGF) to see where it falls within the continuum. Highlights the implications for UK hospitals engaged in clinical research through the presentation of a case study in implementing the RGF. External quality assurance in medical rehabilitation is characterized by the programs of Germany??s two largest providers of rehabilitation (the German pension insurance and the statutory health insurance). The programs were developed with considerable input from scientific institutes. Their strengths lie in measuring several relevant dimensions of quality, the comprehensive implementation for nearly all illnesses treated, integration of the patient perspective, and realization of prospective study designs with follow-up. Weaknesses include low relevance for patient allocation and compensation, the absence of systematic evaluations, and a lack of published patient information on the results of the quality measurements. Internal quality management in medical rehabilitation is regulated by sweeping legal guidelines. Since 2009, a statutory agreement has been in effect which includes a provision that all inpatient rehabilitation centers must be certified by October 2012. Areas for potential improvements in internal quality management processes include an improved theoretical basis, higher specificity for rehabilitation, and a more transparent, methodically improved structure of certification procedures.  相似文献
