抗CCP抗体和RF联检在RA诊疗中的临床价值   总被引:1,自引:0,他引:1  
目的:研究抗环瓜氨酸肽(anti-cyclic citrullinated peptide,Anti-CCP)(抗CCP抗体)和RF的检测在类风湿关节炎(RA)诊疗中的临床价值。方法:分别用酶联免疫吸附试验(ELISA)、BeckMan全自动蛋白分析仪同时检测早期RA组(病程〈1年)42例,RA组(病程〉1年)40例,非RA对照组40例患者血清抗CCP抗体和RF。结果:早期RA组、RA组的抗CCP抗体、RF阳性率显著高于非RA对照组(P〈0.05)RA组抗CCP水平显著高于早期RA组(P〈0.01),两者RF无显著差别(P〉0.05)。RA组与早期RA组CCP抗体与RF二者无相关性。结论:联检抗CCP抗体、RF有助于类风湿的早期诊断和预测病情的进展。  相似文献

目的:探讨类风湿因子(Rheum atoid factor,RF)、抗角质蛋白抗体(antikeratin antibody,AKA)及抗环瓜氨酸肽(anti-cyc lic c itru llinated peptide,CCP)抗体对类风湿关节炎(rheum atoid arthritis,RA)的临床意义和早期应用价值。方法:对40例类风湿关节炎患者、30例系统性红斑狼疮和30名正常健康体检者进行RF、AKA、抗CCP抗体检测,应用速率散射比浊法测定RF,间接免疫荧光法检测AKA,ELISA法测定抗CCP抗体。结果:40例RA患者血清中,RF灵敏度和特异性分别为70.0%、90.0%,AKA灵敏度和特异性分别为35.0%、96.7%,抗CCP抗体灵敏度和特异性分别为85.0%、93.3%,联检RF、AKA及抗CCP抗体灵敏度和特异性分别为97.07%、99.8%。结论:RF、AKA和抗CCP抗体可作为诊断RA比较特异的血清学指标,三项指标联检可在一定程度上弥补RF对RA的诊断不足,提高RA的阳性诊断率,且有助于疾病的预后判断。  相似文献

抗环瓜氨酸肽抗体在类风湿性关节炎诊断中的应用   总被引:7,自引:0,他引:7  
目的 探讨抗环瓜氨酸肽 (CCP)抗体检测在类风湿性关节炎 (RA)诊断中的意义。方法 根据cDNA序列人工合成的CCP为抗原 ,用ELISA方法检测 2 18例 (RA 112例 ,非RA 76例及正常人 30例 )血清中的抗CCP抗体 ,并比较抗CCP抗体与类风湿因子 (RF)的相关性。结果 抗CCP抗体在RA组患者血清中的阳性率 4 9.1% ,明显高于非RA组 ( 3.94 % )和正常对照组 ( 0 .0 % ) ,经 χ2 检验P <0 .0 5。抗CCP抗体对诊断RA的敏感性和特异性分别为 4 9.1%、96 .1% ,阳性预测值和阴性预测值为 94 .8%、4 3.8%。在 112例确诊为RA患者的血清中抗CCP抗体与RF重叠阳性率为 85 .4 % ,两者含量呈正相关。结论 抗CCP抗体对RA具有很高的特异性 ,可视为新的RA血清学诊断指标  相似文献

目的为寻找敏感性高、特异性强的诊断类风湿关节炎(RA)的实验指标,选择抗环瓜氨酸肽(CCP)抗体、葡萄糖6-磷酸异构酶(GPI)和类风湿因子(RF)并探讨对RA的诊断价值。方法抗CCP抗体和GPI试验采用酶联免疫吸附(ELISA)法,RF用胶乳凝集法,检测了125例RA患者(RA组)、56例其他风湿性疾病患者(对照组)和36例健康体检者(正常组)血清中上述3者的浓度。结果抗CCP抗体、GPI及RF,在RA组分别是:(83.6±45.9)RU/ml、(2.8±2.3)μg/ml和(320.4±208.6)IU/ml;在对照组分别是:(36.2±15.3)RU/ml、(0.19±0.06)μg/ml和(36.3±12.5)IU/ml;在正常组分别是:(19.2±8.6)RU/ml、(0.16±0.08)μg/ml和(19.2±6.5)IU/ml。在RA组中,3者的敏感度分别是64.8%、73.6%和69.6%,特异性分别是92.0%、72.8%和74.4%。结论 RA患者血清抗CCP抗体、GPI和RF显著高于其他疾病患者(P<0.01)。抗CCP抗体和GPI有可能成为诊断RA的新指标、RA的血清标志物,对提高诊断RA的敏感性和特异性,具有良好的临床研究和推广价值。  相似文献

目的 比较抗环瓜氨酸肽抗体(抗CCP抗体)和抗角蛋白抗体(AKA)在类风湿关节炎诊断中作用,探讨RA的早期诊断方法.方法 对已确诊的85例RA患者、74例非RA的自身免疫病患者同时测定抗CCP抗体(ELISA法)、AKA(间接免疫荧光检测).结果 抗CCP抗体对诊断RA的灵敏度和特异性分别为75.3%和93.2%;AKA对RA诊断的灵敏度和特异性分别为89.4%和85.14%.抗CCP抗体的灵敏度与AKA灵敏度差异无统计学意义(P>0.05),特异性差异有统计学意义(P<0.05);抗CCP抗体或AKA与二者联合检测的灵敏度差异有统计学意义(P<0.05),特异性差异有统计学意义(P<0.05).阳性预测值抗CCP抗体较高,阴性预测值以二者联合检测较好,Youden index二者联合检测比单独检测抗CCP抗体或AKA高.抗CCP抗体和AKA在检测RA组中抗CCP抗体和AKA同时阳性检出58例;抗CCP抗体或AKA阳性共检出85例.抗CCP抗体或AKA阳性率(96.5%)比二者同时阳性率(68.2%)大大提高.结论 抗CCP抗体、AKA对RA具有较好的灵敏度和高度的特异性,联合检测抗CCP抗体和AKA可作为早期RA患者及RF阴性RA患者的早期诊断指标.  相似文献

目的比较类风湿关节炎(rheumatoid arthritis,RA)患者血清中抗肽酰基精氨酸脱亚氨酶4(PADI4)抗体水平和其它风湿病组及正常对照组间的差异,以评估其在RA诊断中的价值。方法采用酶联免疫吸附法(enzyme-linked immunosorbentassay,ELISA)检测患者血清中抗PADI4抗体水平,并研究其与RA患者DAS28评分、抗CCP抗体、抗角蛋白抗体(anti-keratinantibody,AKA)、类风湿因子(rheumatoid factor,RF)等指标的相关性。结果RA患者血清中抗PADI4抗体阳性率为47%,明显高于其它风湿性疾病患者和健康对照组(P<0.05)。抗PADI4抗体对RA诊断的敏感性为43%,特异性为92%。相关性分析显示抗PADI4抗体水平与DAS28评分、抗CCP抗体无相关性(r=-0.025,P=0.82;r=-0.058,P=0.60)。RF阴性患者PADI4抗体阳性50%;AKA阴性的患者PADI4抗体阳性43%;抗CCP抗体阴性的患者抗PADI4抗体阳性60%。结论RA患者血清中抗PADI4抗体有较高的诊断价值,特别有助于RF、抗CCP...  相似文献

目的探讨血清中抗环胍氨酸肽抗体(A-CCP)、类风湿因子(RF)与C反应蛋白(CRP)联合检测在类风湿关节炎(RA)早期诊断中的意义。方法选取2016年10月至2018年5月在我院就诊的RA患者110例(RA组),同时选取其他风湿病患者97例(非RA组),健康体检者95例(健康组)的临床样本为试验材料。采用北京九强试剂定量检测A-CCP、RF及CRP水平,比较3种指标在类风湿关节炎(RA)早期诊断中的价值。结果RA组A-CCP、RF及CRP的阳性率显著高于非RA组及健康组,三者联合检测的灵敏性和特异性与单独检测时比较,差异有统计学意义(P<0.05)。结论抗CCP抗体、RF和CRP三者联合检测可大大提高早期诊断RA的灵敏度和特异性,对早期诊断RA具有重要意义。  相似文献

目的探讨抗CCP抗体、抗MCV抗体和类风湿因子在早期类风湿关节炎中的诊断价值。方法选择于2016年1月至2017年1月期间在我院就诊的早期类风湿关节炎(RA)患者及其他骨关节病患者各40例,并选择40名同期在我院体检的健康人作为研究对象。其中风湿关节炎患者设为RA组,其他骨关节病患者为疾病对照组,健康人为正常对照组。采用免疫透射比浊法、酶联免疫吸附法(ELISA)、免疫散射比浊法分别测定三组的抗环瓜氨酸肽(CCP)抗体、抗突变型瓜氨酸波形蛋白(MCV)抗体及IgM型类风湿因子(IgM-RF)水平,比较抗CCP抗体、抗MCV抗体、IgM-RF单独检测及三个指标联合检测对早期类风湿性关节炎的诊断价值。结果①RA组的抗CCP抗体、抗MCV抗体、IgM-RF水平均明显高于疾病对照组与正常对照组,差异具有统计学意义(P<0.05);疾病对照组与正常对照组间差异无统计学意义(P>0.05)。②RA组的抗CCP抗体、抗MCV抗体、IgM-RF及联合检测的阳性率均明显高于疾病对照组与正常对照组,差异具有统计学意义(P<0.05);疾病对照组与正常对照组间差异无统计学意义(P>0.05)。③联合检测的灵敏度及阴性预测值均明显高于抗CCP抗体、抗MCV抗体、IgM-RF单独检测,差异具有统计学意义;抗CCP抗体与抗MCV抗体的特异性及阳性预测值均明显高于IgM-RF及联合检测,差异具有统计学意义(P<0.05)。结论早期RA患者的血清中存在抗CCP抗体、抗MCV抗体、IgM-RF的高表达,且其灵敏度及特异性均较高,临床上可联合检测抗CCP抗体、抗MCV抗体及IgM-RF,以提高RA的早期诊断率。  相似文献

为了评价类风湿因子(RF)和抗CCP抗体阴性的关节炎患者就诊时血清轻链λ水平对类风湿关节炎(RA)的诊断价值,回顾性分析我院住院RA患者的临床资料,获取患者初诊的RF、抗环瓜氨酸肽(CCP)抗体、免疫球蛋白(Ig)和轻链等数据。根据RF和抗CCP抗体水平,RA患者分为自身抗体阴性组和阳性组。将血清阴性RA患者作为实验组,与非RA关节炎患者构成的对照组进行比较,用受试者工作特征曲线(ROC)和多元Logistic回归分析轻链λ对RA患者诊断的临床价值。结果显示,实验组较对照组关节炎患者的轻链λ水平升高,差异具有统计学意义(P=0.026);ROC分析表明轻链λ对关节炎患者发展为RA具有预测价值,曲线下面积(AUC)达到0.665[95%CI(0.518,0.812)];以620.5mg/dl为临界值,预测RA的敏感性为0.565(0.345,0.768),特异性为0.804(0.661,0.906);多元Logistic回归校正后轻链λ仍然与关节炎患者发生RA相关(OR=3.551,P=0.037)。研究结果表明,血清轻链λ水平对关节炎患者发展为RA具有一定的预测价值。  相似文献

The aim of this study was to investigate the effect of three bioceramics in the CaO-SiO(2)-MgO systems with different composition on the in vitro degradation, bioactivity, and cytocompatibility. The degradation was evaluated through the activation energy of Si ion release from ceramics and the weight loss of the ceramics in Tris-HCl buffers. The in vitro bioactivity of the ceramics was investigated by analysis of apatite-formation ability in the simulated body fluid (SBF). The cytocompatibility was evaluated through osteoblast morphology and proliferation. The results showed that the activation energy of Si ion release increased and the degradation decreased from bredigite to diopside ceramics with the increase of Mg content, and the apatite-formation ability in SBF decreased. The Ca, Si, and Mg containing ionic products from three ceramics could stimulate cell proliferation at lower concentration, and inhibit cell proliferation with the increase of ion concentrations. Furthermore, osteoblasts could adhere, spread, and proliferate on three ceramic disks, and cell proliferation on diopside was more obvious than that on other two ceramic disks.  相似文献

This review presents the current taxonomy of the genera Proteus, Providencia, and Morganella, along with the current methods for the identification of each species within the three genera, incorporating both conventional biochemical and commercial methods. While all of these organisms are ubiquitous in the environment, individual case reports and nosocomial outbreak reports that demonstrate their ability to cause major infectious disease problems are presented. Lastly, anticipated antimicrobial susceptibility patterns are reviewed. Many of these organisms are easily controlled, but the advent of newer and more powerful antimicrobial agents has led to some problems of which laboratorians need to be aware.  相似文献

Summary By ultracentrifugation and red cell adsorption experiments it was proved, that hemagglutinating activity of adenovirus types 20, 25, and 28 against monkey blood cells is associated with the virus particles; while no soluble hemagglutinins were found for type 20 and 25, the soluble hemagglutinating activity of type 28 against rat blood cells was located in a dodecon. The length of the fibres, as observed on the virus surface or in pentons, was 115 to 120 Å for type 20, 25, and 28, coinciding with a reference virus (type 9–15) of subgroup II.These data support the previous suggestion, that adenovirus 20, 25, and 28 belong to subgroup II.Aided by grants of the Deutsche Forschungsgemeinschaft.  相似文献

Effects of nicotine and caffeine, separately and in combination, were assessed in 12 male habitual smokers in a repeated-measures design. Caffeine (0-mg vs. two 150-mg doses administered in a decaffeinated/sugar-free cola drink post-baseline and 90 min later) was crossed with nicotine (ad libitum own dosing vs. 1.0-mg machine-delivered dose vs. 0.05-mg machine-delivered dose). Participants smoked a total of five cigarettes at 30-min intervals over a 2-hr period. Caffeine and nicotine had large effect sizes on electroencephalogram (EEG) power; however, these effects were modulated by the eyes open versus closed condition, the other drug, and electrode site. EEG effects of open versus closed eyes tended to be of the same size and direction as those of nicotine and caffeine. However, whereas nicotine increased EEG power in some higher frequency bands in some conditions, caffeine decreased EEG power across almost all conditions. Serum cortisol concentration, vigor, and pleasantness were increased by nicotine, but not by caffeine. Level of depressive mood depended on an interaction of caffeine and nicotine. Vigilance performance was enhanced significantly by caffeine and was increased almost significantly by nicotine. The findings were interpreted in terms of common and differential mechanisms of the two drugs.  相似文献

A thermosensitive, injectable, and bioresorbable polymer hydrogel, poly(N-isopropylacrylamide-co-dimethyl-γ-butyrolactone acrylate-co-acrylic acid) [poly(NDBA)], was synthesized by radical copolymerization with 7.00 mol % dimethyl-γ-butyrolactone acrylate in tetrahydrofuran. The chemical composition was determined by acid titration in conjunction with (1) H NMR quantification. The molecular weight and polydispersity were determined by gel permeation chromatography in conjunction with static light scattering. The degradation properties of the polymer hydrogel were characterized using differential scanning calorimetry, percentage mass loss, cloud point test, and swelling ratio over time. It was found that the initial lower critical solution temperature (LCST) of the polymer is between room temperature and body temperature and that it takes about 2 weeks for the LCST to surpass body temperature under physiological conditions. An indirect cytotoxicity test indicated that this copolymer has relatively low cytotoxicity as seen with 3T3 fibroblast cells. The in vivo-gelation and degradation study showed good agreement with in vitro-degradation findings, and no detrimental effects to adjacent tissues were observed after the complete dissolution of the polymer. ? 2011 Wiley Periodicals, Inc. Working independently, three laboratories had recognized considerable similarity among certain strains of dysgonic, fermentative, capnophilic, surface translocating, gram-negative bacilli referred to as Capnocytophaga, Bacteroides ochraceus, and Center for Disease Control biogroup DF-1. To determine the relationship among these groups, 21 strains were exchanged and independently characterized by the three laboratories. Additionally, a fourth laboratory examined the deoxyribonucleic acid homologies of the same strains. Using methods common to dental microbiology, eight of the strains had been isolated from the gingival sulcus and periodontal lesions and identified as Capnocytophaga. Three strains isolated from blood and transtracheal aspirate had been characterized by conventional anaerobic methods and recorded as B. ochraceus. Ten strains isolated from sputum, blood, throat, spinal fluid, and tracheal aspirate had been identified as DF-1 with the methods of E. O. King and a buffered single-substrate technique. All strains were similar in respect to colonial and microscopic morphology, surface translocation, biochemical features, gas-liquid chromatograms of metabolic end products, and deoxyribonucleic acid composition. Summary: Throughout much of the world, Trichinella spp. are found to be the causative agents of human trichinellosis, a disease that not only is a public health hazard by affecting human patients but also represents an economic problem in porcine animal production and food safety. Due to the predominantly zoonotic importance of infection, the main efforts in many countries have focused on the control of Trichinella or the elimination of Trichinella from the food chain. The most important source of human infection worldwide is the domestic pig, but, e.g., in Europe, meats of horses and wild boars have played a significant role during outbreaks within the past 3 decades. Infection of humans occurs with the ingestion of Trichinella larvae that are encysted in muscle tissue of domestic or wild animal meat. Early clinical diagnosis of trichinellosis is rather difficult because pathognomonic signs or symptoms are lacking. Subsequent chronic forms of the disease are not easy to diagnose, irrespective of parameters including clinical findings, laboratory findings (nonspecific laboratory parameters such as eosinophilia, muscle enzymes, and serology), and epidemiological investigations. New regulations laying down rules for official controls for Trichinella in meat in order to improve food safety for consumers have recently been released in Europe. The evidence that the disease can be monitored and to some extent controlled with a rigorous reporting and testing system in place should be motivation to expand appropriate programs worldwide.  相似文献

This project investigated the impact of daily events, varying in source of causation, on reports of physical and mental well-being in a multiple-measurement design (5 days). Subjects responded daily to health and mood instruments and to instruments assessing event occurrence, responses to events, and outcomes of those responses. Events were either environment-initiated “demands” or self-initiated “desires.” Positive and negative components of well-being are found to be differentially impacted by types of events and by the different event properties. The procedure was sensitive to the particular “ecology of events” representative of this particular subject sample. The results suggest the validity of a multicomponent analysis of both events and well-being in a dynamic, multiple-measurement approach. Implications for future research employing this model are then briefly discussed.  相似文献
