阿伦膦酸盐对绝经后骨质疏松妇女骨密度的影响   总被引:1,自引:0,他引:1  
为了解阿伦膦酸盐对骨密度的影响及其安全性和耐受性,对20名绝经后骨质疏松的妇女中进行阿伦膦酸盐(alendronate)10mg/天和安慰剂的随机、双盲、前瞻性研究,为期一年。结果显示,1年后阿伦膦酸盐组与安慰剂组相比,骨密度平均增长率:椎骨分别为4.87%与-0.23%;股骨颈分别为6.89%与-1.84%,(P<0.05)。副反应仅为轻微胃肠道反应。结论:阿伦膦酸盐能有效增加骨密度,且药物安全,耐受性好  相似文献

目的 评价福善美 (Fosamax)治疗绝经后骨质疏松症妇女的近期及中期疗效 ,以及改善骨量作用。方法 绝经 1年以上女性 80名 ,分成 4组 ,A组 :14例 ,年龄 5 4 . 97± 5. 5 1(47~ 6 2 )岁 ,绝经年限≤10年 ,疗程 6个月~ 1年 ;B组 :2 3例 ,年龄 5 5 . 5 5± 3. 6 6 (5 0~ 6 2岁 ) ,绝经年限≤ 10年 ,疗程 1年以上 ;C组 :18例 ,年龄 6 8. 18± 5 . 5 9(5 9~ 78岁 ) ,绝经年限 >10年 ,疗程 6个月~ 1年 ;D组 :2 5例 ,年龄6 7. 2 5± 6 .19(5 2~ 80岁 ,绝经年限 >10年 ,疗程 1年以上。患者每天接受口服福善美 10mg和元素钙5 0 0mg,疗程 6个月~ 2 8年。治疗前、后应用双能X线吸收仪 (HologicQDR 2 0 0 0型 )进行骨密度(BMD)测定。结果  4组不论绝经年限长短 ,通过 6个月~ 2年以上福善美治疗 ,腰椎BMD平均增加百分率 3 74 %~ 5 4 5 % ,较基础值均有明显增加 (P <0 . 0 0 1) ,4组间差异无统计学意义 (P >0 . 0 5 )。股骨颈部位治疗后BMD平均增加百分率为 0 84 %~ 4 2 1% ,其中绝经年限相同时 ,疗程长者高于短者 ,即B组高于A组 ,D组高于C组 ;疗程相同时 ,绝经年限长者高于短者 ,即C组高于A组 ,D组高于B组 ,但 4组间差异无显著性 (P >0 . 0 5 )。大转子部位治疗后BMD增加平均变化百分率 1 4. 2 %~  相似文献

Summary This retrospective study examined bone mineral density (BMD) for discrimination of female patients with fractures. Bone densitometry was done in 146 patients over the age of 50 years at radius, lumbar spine, and proximal femur sites using single and dual photon absorptiometry. The patients were divided into three groups: (A) no osteoporotic fractures (n=92); (B) mild spine fractures with >15% compression (n=38); and (C) hip fractures (n=16). Groups B and C did not differ significantly from each other in BMD, but these groups differed significantly from group A for spine and femur BMD. No significant differences between groups were found for the radius. Receiver operating characteristic (ROC) analysis showed that the BMD of the proximal femur had the highest diagnostic sensitivity for both spine and femur fractures; the radius had the lowest overall sensitivity, and the spine was intermediate.  相似文献

 目的 观察唑来膦酸盐(5 mg, 单次)治疗绝经后骨质疏松症妇女骨密度和跌倒风险的作用。方法 采用随机对照研究, 观察期为1 年。91 例绝经后骨质疏松症妇女经知情同意后, 随机分为两组。唑来膦酸盐组45 例院唑来膦酸盐5 mg(30 min 静脉滴注, 1 次), 骨化三醇0.25 ug 和钙剂600 mg 及维生素D 125 ID(1 次/d, 1 年); 对照组46 例院骨化三醇0.25 滋g 和钙剂600 mg 及维生素D 125 ID(1 次/d, 1 年)。用药前和用药12 个月后测量腰椎尧髋部及股骨颈骨密度和跌倒风险, 并进行患者不良反应和随访情况进行比较。结果 干预1 年后, 两组各有41例患者得到随访。与干预前自身比较, 唑来膦酸盐组患者腰椎尧髋部总量和股骨颈骨量均明显增加, 分别为5.8%, 3.9%和2.9%, 差异均有统计学意义; 对照组患者腰椎骨量与干预前自身比较有明显增加, 达4.4%。两组患者经治疗后跌倒风险较治疗前均明显降低, 组间比较差异无统计学意义。唑来膦酸盐组患者未见无法耐受的不良反应。结论 唑来膦酸盐(5 mg, 单次)治疗绝经后骨质疏松症可明显提高腰椎尧髋部和股骨颈骨密度, 联合应用活性维生素D 能进一步降低跌倒风险。唑来膦酸盐(5 mg)是临床骨质疏松症长期治疗疗效得以保证的重要手段。  相似文献

Introduction Osteoporosis is associated with increased risk for fracture. However, most postmenopausal women have bone mineral density (BMD) within the normal or osteopenic range. The aim of this study was to determine the proportion of the population burden of fragility fractures arising from women at modest risk for fracture.Methods We measured baseline BMD in a population-based random sample of 616 postmenopausal women aged 60–94 years and followed these individuals for a median of 5.6 years (IQR 3.9–6.5) to determine the incidence of fractures according to age, BMD and the presence of a prior fracture.Results Based on WHO criteria, 37.6% of the women had normal total hip BMD, 48.0% had osteopenia and 14.5% had osteoporosis. The incidence of fracture during follow-up was highest in women with osteoporosis, but only 26.9% of all fractures arose from this group; 73.1% occurred in women without osteoporosis (56.5% in women with osteopenia, 16.6% in women with normal BMD). Decreasing BMD, increasing age and prior fracture contributed independently to increased fracture risk; in a multivariate model, the relative risk for fracture increased 65% for each SD decrease in BMD (RR=1.65, 95%CI 1.32–2.05), increased 3% for every year of age (RR=1.03, 95%CI 1.01–1.06) and doubled with prevalent fracture (RR=2.01, 95% CI 1.40–2.88). A prevalent fracture increased the risk for fractures such that women with osteopenia and prevalent fracture had the same, if not greater, risk as women with osteoporosis alone.Conclusions Reducing the population burden of fractures requires attention to women with osteopenia, as well as osteoporosis, because over half of the fragility fractures in the population arise in these individuals, and women with osteopenia plus a prevalent fracture have the same fracture risk as women with osteoporosis.  相似文献

We examined the effect of exercise training and detraining on bone mineral density (BMD) in postmenopausal women with osteoporosis. Thirty-five postmenopausal women with osteoporosis, aged 53–77 years, were randomly assigned to three groups: a control group (n = 20), a 2-year exercise training group (n = 8), and an 1-year exercise training plus 1-year detraining group (n = 7). Exercise training consisted of daily brisk walking and gymnastic training. Calcium lactate, 2.0 g, and 1α-hydroxyvitamin D3, 1 μg were supplied daily to all subjects. No significant differences in initial lumbar BMD, measured by dual-energy X-ray absorptiometry (DXA) were found among the three groups. The mean percent change in BMD compared with the baseline was significantly higher at 1 and 2 years in the exercise training group and at 1 year in the detraining group than in the control group, and did not differ significantly at 2 years between the detraining and control groups. These findings indicate that our exercise training program led to a significant increase in lumbar BMD in postmenopausal women with osteoporosis compared with the control, but that the BMD reverted toward a level that was not significantly different from the control with detraining. To assess the bone turnover abnormalities which characterize postmenopausal osteoporosis with vertebral fractures (PMOp), a transiliac bone biopsy was performed after double labeling of the mineralizing front with tetracycline in 50 untreated PMOp patients who were compared with 13 healthy age-matched volunteer females. The analysis of bone remodeling and structure parameters demonstrated that PMOp is a disease affecting both the cancellous and the endocortical envelopes and characterized by increased resorption and by a marked decrease in the osteoblastic apposition rate due to a reduced duration of bone formation. This induces a decrease in the width of both individual osteons and trabeculae. In PMOp, the wide spectrum of bone turnover as compared with the controls, associated with the typical bimodal distribution of cancellous osteoid perimeter, allowed us to identify two subsets, one with normal turnover (NT) and one with high turnover (HT) representing 30% of the cases. When compared to NT, HT was characterized by increased osteoclast number, lower bone volume, thinner osteons, increased formation at the tissue-level and markedly decreased duration of formation. In HT the marked decrease in the duration of activity of osteoblasts and the markedly increased number of osteoclasts induced a greater decrease in bone volume, despite the increase of bone formation at the tissue level. These subsets could not be distinguished by any clinical or biochemical parameter except for serum bone gla protein (osteocalcin) which was significantly higher (as a group) in HT than in NT. The underlying cause for these two subsets is unknown. We conclude that PMOp affects the cancellous and the endocortical bone. Bone loss results from a wide spectrum of bone turnover abnormalities, with two distinct subsets, one with normal turnover and one with high turnover.  相似文献

A group of Northern Ireland women aged 40–75 years of age with low-trauma forearm fracture were studied to determine the incidence of such fractures and the prevalence of osteoporosis in this fracture population. A total of 1,147 subjects were identified in 1997 and 1998 throughout Northern Ireland following low-trauma forearm fractures, as well as 699 residents in the Eastern Health and Social Services Board (EHSSB), enabling calculation of the annual incidence rate of new low-trauma forearm fractures at 2.69/1,000 population aged 40–75. A total of 375 participants consented to have bone mineral density (BMD) measurements undertaken at the femoral neck, spine, and forearm using a Lunar Expert bone densitometer. Osteoporosis at the femur was present in 14% of women, at the spine in 29%, and at the forearm in 32%. A total of 45% were osteoporotic at one or more measured sites, but only 18% were on treatment for osteoporosis. Additional significant risk factors identified included an early menopause in 24.5% and current or previous corticosteroid use in 13%. Only 1.6% received information on treatment of osteoporosis at the time of fracture. Increased awareness is needed in both primary and secondary care including fracture services to improve treatment of women with low-trauma fracture.  相似文献

BACKGROUNDOsteoporosis and type 2 diabetes (T2D) have been recognized as a widespread comorbidity leading to excess mortality and an enormous healthcare burden. In T2D, bone mineral density (BMD) may underestimate the risk of low-energy fractures as bone quality is reduced. It was hypothesized that a decrease in the trabecular bone score (TBS), a parameter assessing bone microarchitecture, may be an early marker of impaired bone health in women with T2D.AIMTo identify clinical and body composition parameters that affect TBS in postmenopausal women with T2D and normal BMD.METHODSA non-interventional cross-sectional comparative study was conducted. Potentially eligible subjects were screened at tertiary referral center. Postmenopausal women with T2D, aged 50-75 years, with no established risk factors for secondary osteoporosis, were included. BMD, TBS and body composition parameters were assessed by dual-energy X-ray absorptiometry. In women with normal BMD, a wide range of anthropometric, general and diabetes-related clinical and laboratory parameters were evaluated as risk factors for TBS decrease using univariate and multivariate regression analysis and analysis of receiver operating characteristic (ROC) curves.RESULTSThree hundred twelve women were initially screened, 176 of them met the inclusion criteria and underwent dual X-ray absorptiometry. Those with reduced BMD were subsequently excluded; 96 women with normal BMD were included in final analysis. Among them, 43 women (44.8%) showed decreased TBS values (≤ 1.31). Women with TBS ≤ 1.31 were taller and had a lower body mass index (BMI) when compared to those with normal TBS (Р = 0.008 and P = 0.007 respectively). No significant differences in HbA1c, renal function, calcium, phosphorus, alkaline phosphatase, PTH and 25(ОН)D levels were found. In a model of multivariate linear regression analysis, TBS was positively associated with gynoid fat mass, whereas the height and androgen fat mass were associated negatively (all P < 0.001). In a multiple logistic regression, TBS ≤ 1.31 was associated with lower gynoid fat mass (adjusted odd ratio [OR], 0.9, 95% confidence interval [CI], 0.85-0.94, P < 0.001), higher android fat mass (adjusted OR, 1.13, 95%CI, 1.03-1.24, P = 0.008) and height (adjusted OR, 1.13, 95%CI, 1.05-1.20, P < 0.001). In ROC-curve analysis, height ≥ 162.5 cm (P = 0.04), body mass index ≤ 33.85 kg/m2 (P = 0.002), gynoid fat mass ≤ 5.41 kg (P = 0.03) and android/gynoid fat mass ratio ≥ 1.145 (P < 0.001) were identified as the risk factors for TBS reduction.CONCLUSIONIn postmenopausal women with T2D and normal BMD, greater height and central adiposity are associated with impaired bone microarchitecture.  相似文献

The hypothesis that a history of one or more weight reductions and regains (weight cycling) is associated with lower site-specific bone mineral density (BMD) was examined in 169 premenopausal women, aged 29–46 years. Data on the previous 10-years' weight cycling history, present weight-bearing physical exercise, number of deliveries, present use of contraceptive pills or hormone-releasing coils, age at menarche and present menstrual status were collected by a self-administered questionnaire. Dietary intake was calculated from food records. The areal BMD (g/cm2) was measured with dual-energy X-ray absorptiometry (norland XR-26). The lumbar spine (L2–4) BMD, adjusted to weight and age at menarche (ANCOVA), was 0.062 g/cm2 (95% confidence interval: 0.015 to 0.011 g/cm2;p=0.01) higher in the non-cyclers (n=68) than in subjects with reported weight-cycling history (n=101). The corresponding difference for femoral neck BMD was 0.019 g/cm2 (–0.018 to 0.056;p=0.30), for trochanter BMD 0.013 g/cm2 (–0.025 to 0.05 g/cm2;p=0.50) and for distal radius BMD 0.022 g/cm2 (0.006 to 0.397 g/cm2;p=0.008). A pairwise comparison of 34 weight-matched subjects (non-cycler vs cycler) gave similar BMD differences as found in the above (ANCOVA) analyses. The results suggest that weight cycling might be associated with lower spine and distal radius BMD.  相似文献

Postmenopausal fractures are associated with low bone mass; however, the role of low peak bone mass in young adults in determining subsequent osteoporosis suggests that premenopausal fractures may also be relevant. We therefore sought to determine whether a self-reported previous history of premenopausal wrist and nonwrist fractures could also be associated with bone density and therefore be used to predict osteoporosis. We recruited 453 volunteer women with a median age of 64 years (range 50-83 years), with no metabolic bone disease, previous femoral neck fracture, or prevalent vertebral fracture. Bone density at the femoral neck (FN) and lumbar spine (LS) was measured using a Lunar DPX-L. As expected, the 319 women who did not report any fracture had a higher T score at LS (-0.93 +/- 1.44) than the 134 women who reported a previous fracture at any site and at any age (T score -1.60 +/- 1.21, p < 0.001). The findings for the FN were similar. Compared with fracture-free women, the women who reported a first wrist fracture before menopause now had a lower LS T score (-1.77 +/- 1.20, n = 15, p < 0.05), whereas those who reported a nonwrist fracture showed no significant decrease in their LS T score (-1.26 +/- 1.00, n = 36). When both wrist and nonwrist fractures had occurred after menopause, the T score was significantly lower. Twenty percent of the fracture-free women were osteoporosis patients. After adjusting for body weight, age, hormonal replacement therapy (HRT), and hip fracture in the family, the relative risk (RR) of osteoporosis for premenopausal wrist fractures was 2.7 (95% confidence interval 1.4-4.3) vs. 1.2 (0.7-2.4) for women with premenopausal nonwrist fractures. We conclude that self-reported premenopausal wrist fractures, but no other fractures occurring before menopause, are likely to be associated with osteoporosis at 65 years of age, and therefore constitute strong grounds for screening.  相似文献

目的 探讨沈阳地区绝经后妇女不同部位骨密度值与亚洲人骨质疏松自我筛查工具(OSTA)得分、体表面积(BS)、体重指数(BMI)的关系,判断OSTA评分与绝经后妇女髋关节骨折风险的相关性。方法 采用双能X线骨密度仪测定沈阳地区670例绝经后妇女的骨密度值( BMD),并与OSTA 得分、体表面积、体重指数( BMl)、绝经年限进行Pearson相关性回归分析。应用WHO骨折风险因子评估工具(FRAX)评估绝经后妇女髋关节骨折风险,采用线性图表及Pearson相关性回归分析比较OSTA评分与绝经后妇女髋关节骨折风险是否存在相关性。结果 骨密度与OSTA评分、体表面积、体重指数呈线性正相关,相关性由大到小分别为OSTA评分、体表面积、体重、身高、BMI、年龄、绝经期、肥胖度;骨密度与绝经年限呈线性正相关趋势;OSTA评分与髋关节骨折风险呈负相关。结论 OSTA得分能较好的反映出绝境后妇女骨密度减低的趋势及程度,同时也能预测绝经后妇女髋关节骨折风险的程度,在临床中应对OSTA评分较低患者应采取必要的相关性治疗及干预。  相似文献

The objective of this cross-sectional study was to determine whether habitual physical activity such as daily walking at work affects bone mineral density (BMD) in healthy premenopausal women. Thirty-one letter or newspaper carriers and 30 sedentary (non-exercising) office workers were screened out from 167 subjects working in the public post office and a private newspaper publishing company. BMD was measured with a dual-energy X-ray densitometer at the lumbar spine (L2–4), femoral neck, distal femur, patella, proximal tibia, calcaneus and distal radius. In addition, maximal isometric strength, cardiorespiratory fitness, anthropometry, 4-day dietary record and daily occupational work load were assessed. During one work shift the carriers' mean walking distance was 5926 m, with 68 flights of stairs walked, and their mean heart rate was 105 beats/min (114 beats/min during the delivery). The corresponding figures for the office workers were significantly lower: 1895 m, 10 flights and 82 beats/ min, respectively. Neither the BMD values adjusted for body mass index (kg/m2) and calcium intake nor the indices of physical performance capacity showed significant differences between the groups. Consequently, habitual daily walking and stair climbing by healthy premenopausal women appeared to be insufficient exercise stimulus to increase considerably the BMD or aerobic and muscular fitness above the values found in a comparable group of sedentary office workers.  相似文献

The relationship between vitamin D and bone density was studied in 150 selected, mature (45–74), postmenopausal women with a lumbar spine Z score below 0. Vitamin D status was evaluated using calcidiol serum levels. Serum calcitriol and parathyroid hormone (PTH) values were also evaluated in some subjects. Bone mass was evaluated by ascertaining bone density and Z and T scores in the lumbar spine and femur region. The reference group consisted of 25 premenopausal women. The postmenopausal group was divided into subgroups according to age, i.e., under or over 60 years old. Additionally, the whole group was also subdivided according to their lumbar spine Z scores into group I (Z>-1), group II (Z<-1; >-2), and group III (Z<-2). Group III of postmenopausal women had higher PTH and lower calcitriol levels than premenopausal women. Calcidiol serum levels were lower in postmenopausal women groups II or III than in the group I and premenopausal women. Calcidiol serum levels and the bone mass values for the lumbar spine were correlated positively in all the postmenopausal women; in the women over 60 years of age, calcidiol levels also correlated with the bone mass values expressed as the bone density in three femur regions: femoral neck, trocanter, and Ward's triangle. In conclusion, mature postmenopausal woman showed high PTH levels and low calcidiol and calcitriol values. Calcidiol status is significantly related to bone mineral density in the lumbar spine and in women over 60 years, calcidiol levels also correlated with bone density in the femur regions.  相似文献

目的 观察利塞膦酸钠防治绝经后骨质疏松症的疗效。方法 绝经后骨质疏松症48例,对照组24例,服安慰剂,实验组24例,服利塞膦酸钠5 mg/d,两组每日均服凯思立D 1片,2组共观察6月。结果 利塞膦酸钠组腰椎、股骨颈及大粗隆的骨密度均明显升高(P<0.05),实验组总有效率为80.95%,明显高于对照组的45.45%(P<0.01)。结论 利塞膦酸钠能明显提高绝经后骨质疏松症患者的骨密度,副作用轻。  相似文献

30例绝经后妇女口服尼尔雌醇(CEE3),每2周2mg共1年。治疗前后均采用单光子吸收法(SPA)进行桡骨骨密度(BMD)测量,进行血骨钙素(BGP)、血降钙素(CT)、血雌二醇(E2)、血卵泡刺激素(FSH)及空腹尿钙/尿肌酐(Ca/Cr)测定。结果显示CEE3治疗1年后治疗组骨BMD增加1.1%,而对照组下降3.3%;血BGP显著下降(P<0.01);尿Ca/Cr下降。本文提示,CEE3能降低骨转换率,有效防止绝经后妇女的骨质疏松  相似文献

Summary The study retrospectively evaluated risk factors for osteopenia in anorexia nervosa (AN) patients. Sixty-nine outpatient, female anorexia nervosa patients (age range 20–40 years, mean 27.5) at varying stages of recovery, and 31 controls had lumbar spine trabecular bone density assessed with single energy computed tomography (CT) scans. An investigator-based, semistructured interview assessed weight and menstrual histories from age 10. Current exercise and dietary calcium levels were categorized and lifetime durations in categories were estimated for each subject. Bone density was significantly lower in the patient group (mean 120 mg/cm3) than in the controls (mean 148 mg/cm3,P<0.001). Bone density correlated significantly with duration of illness, amenorrhea, and weight histories but not with measures of dietary calcium or exercise histories. The most important predictor of bone loss on stepwise multiple regression analysis was duration of AN (23% of variance,P<0.001), and duration of an inadequate calcium intake in adolescence explained a further 5% of the variance (P=0.052). There was no evidence that regular exercise and adequate dietary calcium prevented bone loss related to prolonged periods of low weight and amenorrhea in these subjects.  相似文献

本文通过对60~70岁绝经后妇女20例健康对照组、28例骨质疏松组、18例骨质疏松伴骨折组骨密度及尿吡啶啉(Pyridinoline,PYD)、血清Ⅰ型前胶原肽(Carboxy\|terminalpropeptideoftypeIprocollagen,PICP)的测定,经过比较分析结果显示46例骨质疏松患者的骨密度显著低于健康对照组,18例骨质疏松伴骨折组患者的股骨颈部位的骨密度显著低于28例骨质疏松无骨折组.46例骨质疏松患者的尿PYD、血PICP均显著高于健康对照组.由此提示绝经后妇女测量骨密度尤其是股骨上端的骨密度,结合骨吸收及骨形成的生化指标尿PYD、血PICP,能更好地预测骨质疏松及提示骨折发生的危险性.  相似文献

目的 明确绝经后妇女血脂水平与骨密度有否联系。方法 测定85例绝经后妇女的甘油三酯和胆固醇水平,双能X线法测定腰椎k2-4和股骨颈的骨密度(BMD)。结果 与未绝经妇女相比,绝经后妇女甘油三酯(t=3.74,P=0.000)和胆固醇(t=3.75,P=0.000)水平均显著升高;而BMD腰椎k(t=2.43,P=0.017)和股骨颈(t=3.32,P=0.002)均显著降低;绝经后妇女甘油三酯水平与股骨颈的BMD经BMI校正后有显著负相关,偏相关系数r=-0.324,P=0.003;而与腰椎k2-4 BMD无相关。胆固醇水平则与股骨颈及腰椎k2-4的BMD均无相关性。结论 绝经后妇女甘油三酯水平的升高可能是绝经后骨量减少及骨质疏松发生的因素之一。  相似文献
