目的总结高位双前臂缺失异体移植术后手功能重建及康复的临床效果。方法在应用新型免疫抑制剂及综合抗排斥方案的条件下,采用一体化康复计划,对1例高位双前臂缺失患者术前组织配型、异体移植手术及术后功能康复等治疗,随访4年。结果双前臂、手外形好,感觉基本正常,两点辨别觉2.5~4.0 cm,手指总屈伸度(TAM)均为优良,生活完全自理,心理健康。结论异体双前臂移植术后手功能康复效果满意。  相似文献

Five patients with rheumatoid arthritis or osteoarthrosis of the wrist joint were followed up for 15 (14–17) years after wrist-joint arthroplasty with semiconstrained artificial joint mechanisms that had been anchored to bone using the osseointegration principle. They were fixed by one titanium screw introduced into the radius, and two or more titanium screws introduced distally into the metacarpal bones. In four cases a screw was also introduced into the ulna thereby constituting one component in a distal radio-ulnar (DRU) joint mechanism. The titanium screw introduced into the radius and the distal metacarpal screws osseointegrated in all cases. In three cases the mechanism of the wrist joint or parts of it were replaced with new components that could be attached to the screws that were already osseointegrated. We conclude that wrist joint prostheses can be anchored to the surrounding bone using osseointegration and that they remain fixed for at least 15 years. The principle allows replacement of the joint mechanism, if needed, with maintenance of the osseointegrated anchoring elements.  相似文献

Revision after failed femoral components may be technically demanding due to loss of peri-prosthetic bone. This retrospective study evaluated the long-term results of femoral revision using the cementless Wagner Self-Locking stem. Between 1992 and 1998, 68 consecutive hips in 66 patients underwent femoral revision using this implant. A total of 25 patients died from unrelated causes without further revision; the remaining 41 hips in 41 patients (12 men and 29 women) with a mean age of 61 years (29 to 80) were reviewed at a mean follow-up of 13.9 years (10.4 to 15.8). A transfemoral approach was used in 32 hips. A total of five stems required further revision because of infection in two, progressive subsidence in two and recurrent dislocation in one. Four hips had dislocated and eight stems had subsided ≥?10?mm. The mean Harris hip score improved from 33 points pre-operatively to 75 points at final follow-up (p < 0.001). In all, 33 stems (91.7%) showed radiological signs of stable bone fixation. The cumulative survival rates at 15.8 years with femoral revision for any reason and for stem failure as the endpoints were 92.0% (95% confidence interval (CI) 86.0% to 98.4%) and 96.6% (95% CI 92.2% to 100%), respectively. The survivorship with revision and ≥ 10 mm migration of the stem as the endpoint was 83.6% (95% CI 76.6% to 91.4%). This study shows quite good survival and moderate clinical outcome when using a monoblock tapered titanium stem for supporting the regeneration of bone in complex revision hip surgery.  相似文献

Ten girls and 13 boys with mean age 11 years when sustaining a fracture of the olecranon were examined at a mean of 19 years after the injury. Ten fractures were displaced less than 2 mm, three 2-3 mm, eight more than 3 mm and two were multifragmental. The treatment consisted of mobilization in three cases, plaster treatment in nine and open reduction and internal fixation in 11 cases. At follow-up, 21 children had no subjective complaints while two had occasional mild pain. The upper arm circumference was thinner in the former injured extremities than in the uninjured (P<0.05). No other objective deficits were found. None had developed non-union or elbow osteoarthritis. Olecranon fractures during growth have an excellent long-term outcome.  相似文献

A ten- to 15-year follow-up of the cementless spotorno stem   总被引:6,自引:0,他引:6  
We followed the first 354 consecutive implantations of a cementless, double-tapered straight femoral stem in 326 patients. Follow-up was at a mean of 12 years (10 to 15). The mean age of the patients was 57 years (13 to 81). At follow-up, 56 patients (59 hips) had died, and eight (eight hips) had been lost to follow-up. Twenty-five hips underwent femoral revision, eight for infection, three for periprosthetic fracture and 14 for aseptic loosening. The overall survival was 92% at 12 years (95% CI 88 to 95). Survival with femoral revision for aseptic loosening as an endpoint was 95% (95% CI 92 to 98). The median Harris hip score at follow-up was 84 points (23 to 100). Radiolucent lines (< 2 mm) in Gruen zones 1 and 7 were present in 38 (16%) and 34 hips (14%), respectively. Radiolucencies in zones 2 to 6 were found in five hips (2%). The results for mid- to long-term survival with this femoral component are encouraging and compare with those achieved in primary cemented total hip arthroplasty. The high rate of loosening of the cup and the high rate of pain are, however, a source of concern.  相似文献

Two patients with double inlet left ventricles were successfully treated by ventricular septation. Patient 1, a 38-year-old woman, underwent operation in 1971. This patient has been a full-time worker. Patient 2, a 2-year-old boy, had a relatively small left ventricle with an estimated volume of 134% of normal value. This patient underwent ventricular septation in 1995. He is now in New York Heart Association class II and enjoying school life.  相似文献

The authors present an in-depth clinical, radiographic, and pathologic analysis of a 62-year-old man with massive bone reabsorption around the proximal femur in a total hip arthroplasty (THA). The THA was revised 13 years after implantation. Thirty millimeters of calcar resorption was noted radiographically. Evidence of stem bending was present on examination of the femoral implant, and marked wear of the acetabular cup was noted. The pathologic evaluation of removed calcar bone and cement revealed a histiocytic mass invading the bone. Intracellular and extracellular polymethylene debris was noted within the invasive mass. Evidence of fragmented methylmethacrylate cement was also present. The bone-cement interface in the excised calcar region contained segments that showed active bone remodelling around the cement without an interposed membrane. It is possible that this case of calcar resorption began with histiocytic activation and recruitment by polyethylene wear debris followed by active bone lysis. The process may be perpetuated by the fragmentation of cement, as motion occurred at the calcar bone-cement interface, and may represent an extreme example of a process occurring in cases of calcar resorption in general.  相似文献

The influence of intramedullary (IM) and extramedullary (EM) femoral cutting guides on survivorship of total knee arthroplasty was studied in 6726 total knee arthroplasty guided by either an IM (4993 knees) or EM (1733 knees) system. Fifteen-year survivorship of the 2 cohorts showed no statistically significant difference (EM 97.9% vs IM 98.5%; P = .2500, log rank). Medial bone collapse comprised the highest proportion of all failure modes for both groups (0.35% vs 0.40%, respectively, P = .6731, Cox regression). Mean tibiofemoral (overall) anatomical alignment was statistically more accurate in the IM group (IM 4.6° [±2.2°] valgus vs EM 5.1° [±3.1°] valgus; P < .0001). The mean tibial alignment was 90.5° (±3.0) and 90.3° (±2.2) (P = .0077). The EM group had a significantly larger tibial component alignment variance (SD2) than the IM group. No statistical difference in postoperative Knee Society scores, pain, or stair-climbing abilities was found. The choice of either alignment system should be determined by the patient's anatomy; however, the overall alignment is not as precise using the extramedullary system.  相似文献

Introduction

Vertical banded gastroplasty (VBG) has been the procedure of choice for bariatric surgeries since the 1980s. However, long-term results of VBG have been reported with different opinions, and new restrictive procedures have been innovated and showing variable results. The aim of this study is to analyze the long-term results of our VBG patients.

Patient and methods

Between June 1998 and May 2002, 652 morbidly obese patients received VBG, with the initial 40 patients having open procedures and the subsequent 612 patients using a laparoscopic approach. Operative complications, weight loss, and late complications were followed and compared with groups of laparoscopic adjustable gastric banding (LAGB) and sleeve gastrectomy (LSG).

Results

Mean age, preoperative weight, and body mass index (BMI) were 30.96 years, 108.83 kg, and 40.63 kg/m2, respectively. The overall early postoperative complication rate was 3.4 % (22/652). The excess weight loss percentages at 1, 2, 5, and 10 years were 61.04, 59.70, 51.11, and 42.0 %, respectively. BMI at 1, 2, 5, and 10 years were 29.64, 29.71, 31.33, and 31.73 kg/m2, respectively. This result is inferior to the 67 % excess weight loss in the LSG group, but is higher than the 38 % excess weight loss of the LAGB group. The revision rate is 13.19 % (86/652) up to now. Revision surgery was required in 28 (14.0 %) patients in the LAGB group and 8 (1.3 %) in the LSG group.

Conclusion

VBG was an operation with acceptable outcome for treating morbid obesity and metabolic disorders. It sets a standard for new restrictive procedures.

A cohort of 74 patients with rheumatoid arthritis was monitored prospectively for 15 years. At the end of the study 148 shoulders were radiographed with a standard method. The subacromial space was examined from the radiographs with a method where the acromiohumeral interval was measured from the dense cortical bone marking the inferior aspect of the acromion to a point directly above the head of the humerus. The smallest distance was recorded, and negative values were used when the original articular surface of the humerus exceeded the inferior surface of the acromion. Destruction of the glenohumeral (GH) joints was assessed by the Larsen method on a scale of 0 to 5. The relation of subacromial space measurement to the grade of destruction of GH joints was examined. The mean subacromial space was 6.7 (SD 4.4), range from -13 to 12 mm: 6.1 mm (SD 5.6) in men and 6.9 mm (SD 4.0) in women. The mean of nonaffected (Larsen grade 0 or 1) shoulders (n = 77) was 8.6 mm (SD 1.5), and the corresponding mean of the affected (Larsen grade > or =2) shoulders (n = 71) was 4.6 mm (SD 5.5). Previously reported pathologic criterion (<6 mm) indicating rotator cuff involvement was fulfilled in 30 (20%) of 148 shoulders: in 8 (22%) of 36 shoulders in men and in 22 (20%) of 112 shoulders in women. All the shoulders with severe rheumatoid destruction (Larsen grade 4 or 5) fulfilled the pathologic limit. The subacromial space had a significant negative correlation with the GH joint destruction (Larsen grade) in both sides: right r = -.63 (95% CI -.75 to -.47), left r = -.71 (95% CI -.81 to -.58). Progressive upward migration is an inevitable consequence of rheumatoid destruction in the GH joint. A significant step in this process occurred between the Larsen grades of 3 and 4, where the mean distance turned negative, indicating rotator cuff disease. A patient with rheumatoid arthritis and painful shoulder and upward migration of the humerus on the shoulder radiograph should be evaluated by an orthopaedic surgeon. In indistinct cases with subacromial space diminution, imaging techniques like ultrasonography or magnetic resonance imaging may be required to determine the exact pathologic condition of the rotator cuff and to select optimal treatment.  相似文献

BACKGROUND: Xenotransplantation offers a solution to the shortage of organ donors and may offer resistance to human-specific pathogens. Baboons are resistant to productive infection with HIV-1. A baboon bone-marrow transplant (BMT) was performed in an attempt to reconstitute the immune system of a patient with advanced AIDS. The aims of this pilot study were to evaluate the safety of the procedure and develop an approach to prevent and monitor for xenozoonoses. METHODS: A source animal was selected on the basis of infectious disease surveillance protocols. Baboon bone marrow, engineered to remove graft-versus-host-disease-producing mature lineages, but to retain hematopoietic stem cells and facilitating cells, was infused into the patient after nonmyeloablative conditioning. Serial clinical, virologic, immunologic, and hematologic evaluations were performed. RESULTS: A 38-year-old male with advanced AIDS, who had failed to respond to triple-drug antiretroviral therapy, underwent baboon BMT in 1995. The patient tolerated the procedure without complication. Baboon cells were detected in the peripheral blood on days 5 and 13 after transplantation. Baboon endogenous virus (BaEV) was detected on day 5 but not subsequently. Antibody to BaEV was not detected. HIV-1 viral load declined 1.5 log and remained low until 11 months. The patient improved clinically, and no adverse events occurred. The patient is alive 8 years after the procedure. CONCLUSIONS: Baboon BMT to treat AIDS was attempted using nonmyeloablative conditioning and resulted in transient microchimerism and clinical and virologic improvements. Long-term improvement was not achieved; however, no adverse events occurred, and no evidence of transmission of xenogeneic infections was found.  相似文献

A review is presented of 39 patients with spinal cord injury who were diverted by ureteroileostomy. Because of a significant complication rate and the fact that some normal renal units may deteriorate, the procedure is reserved for the patient with irreversible upper tract changes that fail to respond to more conservative management.  相似文献
