A 59-year-old man showed a saccular aneurysm due to a penetrating atherosclerotic ulcer, as well as a small type B aortic dissection located in the proximal descending aorta. The lesion was treated by the implantation of a stent-graft. On release, the stent-graft dislocated into the aortic arch. Intraoperative angiogram showed free perfusion of the brachiocephalic trunk and left common carotid artery; however, an overstenting of the carotid artery was apparent. Computed tomographic scan exhibited a complete covering of the supra-aortic vessels, and conventional, open aortic arch surgery was inevitable. A partial resection of the proximal part of the stent-graft was performed. 相似文献

A 66-year-old patient underwent emergency endovascular repair of a descending thoracic aneurysm because of suspected aortic rupture. Two weeks later, a small saccular aneurysm of the aortic arch was treated with open surgery. An unexpected intraoperative finding was retrograde dissection of the aortic arch and of the ascending aorta that was not seen on the postprocedural computed tomographic scans after endografting. The ascending aorta, the aortic arch, and the proximal part of the descending thoracic aorta were successfully replaced with a Dacron graft with deep hypothermia, circulatory arrest, and retrograde cerebral perfusion. Awareness that this life-threatening complication that necessitates extensive cardiovascular surgery can occur not only during or immediately after endovascular stenting of the thoracic aorta but also as much as several days or perhaps even weeks after the procedure is important. 相似文献

Endovascular intervention is an alternative form of treatment for patients with thoracic aortic aneurysms. Coexistent cardiovascular diseases may adversely influence the postoperative course and affect the long-term prognosis. The case of a 76-year-old man with severe coronary artery disease and a thoracic aortic aneurysm is reported. A single-stage procedure of off-pump coronary artery revascularization and endoluminal exclusion of the descending thoracic aortic aneurysm was performed. The patient was treated first with off-pump coronary artery bypass graft (left internal mammary artery on the left anterior descending coronary artery and two single venous grafts from ascending aorta to obtuse marginal artery and posterior descending artery). After heart revascularization, two Thoracic Excluder endovascular grafts (34 x 100 and 37 x 100 mm) were implanted to treat the descending thoracic aortic aneurysm. Follow-up with computed tomography angiography showed successful exclusion of the thoracic aneurysm 12 months after the procedure. The patient is well and free of symptoms 18 months later. 相似文献

Although transluminal stent-graft implantation is less invasive for treating an aneurysm in the thoracic descending aorta, this technique is not always successful. Here, we report two cases of a perigraft leak identified after stent-graft deployment, and complete occlusion of the leak could not be attained by additional stent-graft deployment. Open surgical repair using a stented graft was performed through a median sternotomy without opening the left pleura. This procedure should be considered as one of the less-invasive techniques for treating a perigraft leak after endovascular stent-graft emplacement. 相似文献

INTRODUCTION: Thoracic aortic aneurysms (TAA) are the most common condition of the thoracic aorta requiring surgical treatment. Despite significant improvement in anesthetic, surgical techniques and postoperative care, the mortality and morbidity rate in TAA-repair remains high. The aim of this study was to assess the morbidity and mortality rate after endovascular stent-graft treatment of atherosclerotic (non-dissecting) TAAs. METHODS: Thirty-one patients, ASA-classification III-IV, with symptomatic or expanding atherosclerotic TAAs underwent endovascular stent-graft repair between May 1997 and August 2003. Procedures were performed on an emergency basis in 13 patients and elective in 18 patients. Patients were assessed postoperatively by routine CT-scan within 48 h. Further follow up investigations were performed after 3, 6, 12 months and annually thereafter. RESULTS: Stent-graft placement was successful in all but one patient in whom the stent-graft procedure had to be postponed due severe hemodynamic instability. Perioperative mortality rate was 19% including three haemorrhages, two cardiac events and one respiratory failure (6/31). Technical success rate was 55% (17/31). Completion CT scans performed in 30 patients within 2 days of stent-graft procedure showed type I leaks in seven patients (23%), type II leaks in four patients (13%) and type III leaks in two patients (6%). Further complications included one stroke, one paralysis, one spinalis anterior syndrome and five relevant access related complications. New onset endoleaks, all type I, were observed in seven patients (23%) occurring after 3, 4, 7, 8, 17, 25 and 26 months. Mean follow-up was 15 months (range 2-69 months). CONCLUSION: Thoracic aortic atherosclerotic aneurysm stent-grafting is feasible but not without significant morbidity and mortality. 相似文献

Aortoesophageal fistula secondary to thoracic aneurysm is rare and is usually fatal without prompt surgical intervention. A 79-year-old man with significant comorbidities and previous cancer surgery was admitted on an emergency basis because of the suspicion of a ruptured thoracic aortic aneurysm. Computed tomographic scan followed by angiography demonstrated a ruptured thoracic aneurysm with aortoesophageal fistula. An endovascular stent graft repair was performed with successful exclusion of both aneurysm and fistula. On postoperative day 6, dyspnea and an isolated episode of hemoptysis occurred. Endoscopy revealed the presence of a bronchoesophageal fistula, which necessitated double exclusion of the esophagus and feeding jejunostomy. At 6 months, clinical, bronchoscopic, and computed tomographic scan follow-up showed complete sealing of the aneurysm and resolution of the bronchoesophageal fistula. At 9 months, the patient was still alive but refused to undergo substernal gastric bypass in an attempt to restore oral feeding. Endovascular repair seems promising as an emergent and palliative treatment of aortoesophageal fistula. To the best of our knowledge, this is the first case in which a bronchoesophageal fistula developed after successful endovascular repair of aortoesophageal fistula. The pathogenesis of this complications remains unclear. 相似文献

Endovascular aneurysm exclusion represents a valuable alternative treatment for descending thoracic aortic aneurysms. Although the minimally invasive character of this procedure is obvious, major complications are possible. We report a 77-year-old male who developed acute retrograde dissection of the aortic arch and ascending aorta during endovascular stent-grafting of a descending aortic aneurysm. 目的:探讨脑脊液引流在胸降、胸腹主动脉瘤腔内修复术中对截瘫的预防保护作用。方法:回顾性分析我科于2007年9月至2009年12月期间的32例胸降、胸腹主动脉瘤行腔内修复术病人,在术中及术后予以脑脊液引流的效果。结果:所有病人痊愈出院,随访1个月~1年。4例病人术后出现轻瘫症状,经脑脊液引流后治愈;其余病人围手术期间及随访期间未出现截瘫表现,治疗期间无严重并发症。结论:脑脊液引流可在胸降、胸腹主动脉瘤腔内修复术中有效地预防及治疗轻瘫及截瘫。 相似文献

In 2005, the findings of the multicenter Gore Tag study led to United States Food and Drug Administration approval for endovascular repair of thoracic aortic aneurysms (TEVAR). TEVAR provides a therapeutic option for patients who have thoracic aortic aneurysm and for the treatment of type B aortic dissection with malperfusion. Spinal cord ischemia and stroke are recognized neurologic complications of TEVAR. Identification of high-risk patients combined with targeted anesthetic and perioperative management may decrease the risk of neurologic complications after TEVAR. 相似文献

PURPOSE: To facilitate electrocardiography (ECG)-guided central venous catheter placement by observing the shape and size of the P wave at specific locations of a central venous catheter (CVC) tip. METHODS: We evaluated 54 patients for whom central venous catheterization was planned as part of routine care for their elective surgery. The junction of the superior vena cava (SVC) and the right atrium (RA) was defined as the superior border of the crista terminalis by transesophageal echocardiography. The RA ECGs were recorded while withdrawing the CVC into the SVC or advancing it into the RA at 1-cm intervals. Saline was used as an electrical conductor via the distal lumen of the CVC. RESULTS: The tallest peaked and biphasic P waves [median (interquartile range)] were observed when the CVC tip was located at positions 0.0 cm (-1.0 to 0.0) and -4.0 cm (-5.0 to -3.0) below the SVC/RA junction, respectively. The P wave returned to a normal shape and size at 4.0 cm (3.0 to 4.0) above the SVC/RA junction. Overshoot P waves were observed at - 4.0 cm (-5.0 to -3.0) below the SVC/RA junction in 22 patients, when the CVC tip appeared to be contacting or in close proximity to the RA wall. CONCLUSIONS: During ECG-guided central venous catheterization, the tallest peaked P wave may be used to place the CVC tip at the SVC/RA junction, the normally-shaped P wave identifies the mid to upper SVC, and biphasic P waves identify RA localization. 相似文献
