Vitamin D deficiency not only causes low bone mass but also may lead to neuropsychiatric disorders. In the present case, vitamin D supplementation reduced obsessive–compulsive disorder (OCD) symptoms associated with streptococcal infection in a 7‐year‐old boy. Sudden onset of symptoms, including excessive hand washing and fear of touching anything, had occurred 1 month before presentation. Although there are few studies on a possible causal relationship between vitamin D and neuropsychiatric disorders, the present report; together with previous data, suggest an etiological role of vitamin D‐related immune processes.  相似文献

Vitamin D deficiency and insufficiency are increasingly recognized in the general population, including healthy children. There is also an increasing emphasis on the importance of vitamin D status following pediatric liver transplantation and specifically its relationship to metabolic bone disease and growth retardation. Vitamin D insufficiency has also been associated with multiple immunological and metabolic disorders in adults. To our knowledge, this has not been systematically evaluated in children undergoing liver transplantation to date. Between October 2004 and August 2008, serum 25‐(OH)‐vitamin D levels were measured in 199 children who had undergone liver transplantation at Birmingham Children's Hospital. Potential factors contributing to vitamin D levels were evaluated. Additionally, we evaluated a possible relationship between vitamin D levels and immunological phenomena and metabolic complications. Median 25‐(OH)‐vitamin D level was 19.5 ng/mL (range: 4.4–71.4 ng/mL). A total of 105 children (53%) had insufficient vitamin D levels and 28 children (14%) showed vitamin D deficiency. The only factors found to be associated with vitamin D deficiency were season of sample, ethnicity, and PTH levels. Vitamin D deficiency was more prevalent during the first year after transplantation. We did not find a significant relationship between vitamin D levels and graft function or any other immunological and metabolic complications. Vitamin D insufficiency and deficiency are common in children after liver transplantation, especially in winter and spring and in non‐white patients. Initial post‐transplant period and high PTH are significantly associated with vitamin D deficiency. Vitamin D status should be monitored following pediatric liver transplantation and vitamin D supplementation provided as required.  相似文献

Vitamin D deficiency is associated with adverse health outcomes, including impaired bone growth, gingival inflammation and increased risk for autoimmune disease, but the relationship between vitamin D deficiency rickets in childhood and long‐term health has not been studied. In this study, we assessed the effect of early vitamin D deficiency on growth, bone density, dental health and immune function in later childhood to determine if children previously diagnosed with rickets were at greater risk of adverse health outcomes compared with healthy children. We measured serum 25‐hydroxyvitamin D, calcium, parathyroid hormone, bone mineral density, anthropometric measures, dietary habits, dental health, general health history, and markers of inflammation in 14 previously diagnosed rickets case children at Children's Hospital Oakland Research Center. We compared the findings in the rickets cases with 11 healthy children selected from the population of CHO staff families. Fourteen mothers of the rickets cases, five siblings of the rickets cases, and seven mothers of healthy children also participated. Children diagnosed with vitamin D deficiency rickets had a greater risk of fracture, greater prevalence of asthma, and more dental enamel defects compared with healthy children. Given the widespread actions of vitamin D, it is likely that early‐life vitamin D deficiency may increase the risk of disease later in childhood. Further assessment of the long‐term health effects of early deficiency is necessary to make appropriate dietary recommendations for infants at risk of deficiency.  相似文献

Rickets: Part I     
Rickets is characterized by impaired mineralization and ossification of the growth plates of growing children caused by a variety of disorders, the most frequent of which is nutritional deficiency of vitamin D. Despite ample knowledge of its etiology and the availability of cost-effective methods of preventing it, vitamin D deficiency rickets remains a significant problem in developing and developed countries. This two-part review covers the history, etiology, pathophysiology and clinical and radiographical findings of vitamin D deficiency rickets. Other less frequent causes of rickets and some of the disorders entering into the differential diagnoses of rickets are also considered. Controversial issues surrounding vitamin D deficiency include determination of what constitutes vitamin D sufficiency and the potential relationship between low levels of vitamin D metabolites in many individuals and unexplained fractures in infants.  相似文献

Vitamin D deficiency is associated with poor bone health, colorectal cancer, type 1 diabetes and multiple sclerosis. Two national health-related societies in Canada have made recommendations for vitamin D supplementation, yet little research has been reported on the vitamin D status of Canadians. Lifestyle changes, such as sunscreen use, spending less time outdoors and insufficient intake of vitamin D-containing foods as well as northern latitude, may be affecting human vitamin D status. A cross-sectional analysis of 25-hydroxyvitamin D [25-(OH)D] was conducted in pregnant women, newborns (umbilical cord blood) and children. Samples were analysed by liquid chromatography mass spectrometry. Published ranges for 25-(OH)D were used to determine vitamin D status. The prevalence of 25-(OH)D deficiency for the three groups studied revealed most concentrations in the 25-(OH)D deficiency or insufficiency ranges. There were significant differences in all groups studied between seasons, with the exception of maternal blood and female cord blood samples. 25-(OH)D insufficiency was common in all groups for winter and summer, more so in winter. 25-(OH)D insufficiency was common in the three groups studied. The Newfoundland and Labrador population may be at increased risk for vitamin D insufficiency because of factors such as northern latitude and lifestyle issues. Further research on the vitamin D status of this population is important, considering the potential adverse health-related outcomes and the recommendations on supplementation being made.  相似文献

We have read, with great interest, the recent article by Lee. In this excellent study, the authors investigated the association between vitamin D deficiency and anemia in a nationally representative sample of Korean children and adolescents. They concluded that vitamin D deficiency is associated with increased risk of anemia, especially iron deficiency anemia, in healthy female children and adolescents. We appreciate and congratulate the authors for having addressed such an important issue. However, we have some concerns regarding this report, which we would like to share with you. As a result, further studies are needed for the association between vitamin D deficiency and anemia. Factors affecting Vitamin D status and anemia such as measurement method, nutrition, medications, and infections should be considered to conclude an association between vitamin D and anemia. Therefore, we think that considering these confounders would add value to this well-written article.  相似文献

Calcium and phosphorus are, respectively, the fifth and sixth most abundant elements in the body; both play vital roles in a multitude of physiologic systems. Because the great bulk of these elements is found in the skeleton, a large part of the discussion of calcium and phosphorus metabolism focuses on skeletal disorders, the impact of which falls heavily on young children. This article reviews the physiology of calcium and phosphorus, the skeletal and systemic consequences of disorders of vitamin D nutrition and metabolism, and the metabolic bone disease of prematurity.  相似文献

Vitamin D is important for calcium homeostasis, muscle, and bone health. It has also immunomodulatory capacities in vivo and in vitro. Regulatory T cells (Treg) have been found to suppress a number of T cell-mediated immune disorders, including allergic responses and autoimmune diseases. This study aimed to investigate the correlation between 25-hydroxyvitamin D (25(OH)D) levels and the regulatory T cells in cord blood. The study group is comprised of 101 full-term newborn infants. Umbilical cord 25(OH)D levels and number and percentage of T lymphocyte, T helper, and Treg cells were measured. Infants were grouped according to 25-hydroxyvitamin D levels (25(OH)D <12 ng/ml and 25(OH)D >12 ng/ml) (converting factor of 25OHD level into SI unit, 2.6). Severe vitamin D deficiency (25(OH)D <12 ng/ml) was observed in 32% of the infants. There was no significant correlation between 25-hydroxyvitamin D levels and T cell number and percentages. There were also no significant differences in white blood cell, total lymphocyte count, T helper, and Treg cell percentage and number between groups. These results suggest that the serum level of 25-hydroxyvitamin D is not crucially involved in the correlation between vitamin D status and T cell regulation in cord blood.  相似文献

Background: The aim of this study was to estimate the prevalence of vitamin D deficiency among healthy infants and toddlers, as well as its associated factors, in Irbid, Jordan. Methods: A total of 275 subjects (136 infants and 139 toddlers) aged 6–36 months participated in this study. Information concerning sociodemographic characterastics and early feeding patterns was collected using a self‐guided questionnaire. Plasma vitamin D, calcium, phosphorous, and alkaline phosphatase activity were measured. Results: The prevalence of vitamin D deficiency was 28% (16.7% for severe vitamin D deficiency and 11.3% for vitamin D deficiency) and vitamin D insufficiency was 28.4%. Plasma calcium and alkaline phosphatase levels showed no correlation with the vitamin D status of the study population. For both age groups, a significant association was found between vitamin D status and sun exposure (P < 0.001). A significant association between infant feeding practices and vitamin D status was found (P < 0.001). Infants who were exclusively breast‐fed had higher risk for vitamin D deficiency and vitamin D insufficiency than those who were bottle‐fed. Multivariate logistic regression analyses results showed that female sex, low sun exposure and exclusive breast‐feeding were the main determents of vitamin D levels. Conclusion: The prevalence of vitamin D deficiency is considered to be high among northern Jordanian infants and toddlers. Sun exposure of less than 30 min daily and exclusively breast‐feeding are the main factors for developing vitamin D deficiency.  相似文献

Recent studies suggest that vitamin D insufficiency or deficiency is not only a problem of older generations anymore but also an important health concern among younger generations. However, comprehensive data are lacking in Korean adolescents. We investigated the vitamin D (25-hydroxy vitamin D [25(OH)D]) status, the prevalence of vitamin D insufficiency or deficiency, and the association between vitamin D levels and insulin resistance and lipid profiles in a sample of 188 Korean adolescents aged 12-13?years who participated in a general health check-up at a tertiary hospital. Vitamin D deficiency was considered as serum concentrations <20?ng/mL (50?nmol/L); a level of 21-29?ng/mL (52-72?nmol/L) was considered to indicate vitamin D insufficiency, whereas a level of 30?ng/mL or greater (>75?nmol/L) was considered sufficient or optimum. In this cross-sectional study, vitamin D insufficiency or deficiency was found in 98.9?% of boys and 100?% of girls, whereas only 1.1?% of boys and 0?% of girls had a serum 25(OH)D level of greater than 30?ng/mL. In multivariate linear regression analysis, HOMA-IR, triglyceride, and LDL cholesterol were inversely associated with 25(OH)D concentrations. We found that vitamin D insufficiency or deficiency is a very common health problem in Korean adolescents, particularly in girls, and that serum 25(OH)D levels are inversely associated with insulin resistance and lipid profiles. These results suggest that more time spent in outdoor activity for sunlight exposure and higher vitamin D intake may be needed in younger adolescents in South Korea.  相似文献

目的 通过研究维生素D受体(VDR)基因多态性与维生素D缺乏性佝偻病易感性的相关性,探讨维生素D缺乏性佝偻病的遗传易感因素。方法 利用限制性内切酶FokI,应用聚合酶链反应-限制性片段长度多态性(PCR—RFLP)分析、基因测序等技术测定48例维生素D缺乏性佝偻病患儿(病例组)和92名正常儿童(对照组)的VDR基因多态性,比较两组VDR基因型和等位基因的分布频率,并计算基因型优势比(OR)。结果 在48例佝偻病患儿中FF、Ff和ff基因型分布频率分别为46%、33%和21%;而在92名正常儿童中FF、Ff和ff基因型分布频率分别为22%、52%和26%。两组VDR基因型的分布频率差异有显著性(x^2=8.912,P=0.012),病例组中FF基因型占明显优势(OR=3.046)。两组VDR基因等位基因的分布频率差异也有显著性(x^2=5.451,P=0.020),病例组中F等位基因分布频率高于对照组。结论 VDR基因多态性与维生素D缺乏性佝偻病有相关性,提示VDR基因多态性可能在决定个体维生素D缺乏性佝偻病遗传易感性方面有重要作用。  相似文献

目的研究维生素D受体基因起始密码子(VDRSC)多态性在晚发性佝偻病组、维生素D缺乏状态组及正常对照组中分布频率的差异,探讨晚发性佝偻病的遗传易感因素。方法用聚合酶链反应——限制性长度多态性(RFLP)分析晚发性佝偻病组30例、维生素D缺乏状态组35例以及正常对照组60例VDRSC多态性的分布频率。结果三组VDRSC基因型分布频率差异有统计学意义(χ^2=13.184,P=0.010);等位基因分布频率差异也有统计学意义(χ^2=8.975,P=0.011)。组间两两比较晚发性佝偻病组VDRSC基因型和等位基因与其他两组比较差异有统计学意义,其FF型频率(56.7%)明显高于正常对照组(21.7%,P=0.006),也明显高于维生素D缺乏状态组(22.9%,P=0.002);晚发性佝偻病组F型(70.0%)明显高于正常对照组(48.3%,P=0.006),也明显高于维生素D缺乏状态组(47.1%,P=0.009)。多项分类Logistic回归分析结果显示,在调整了其他危险因素后,FF型仍是晚发性佝偻病的危险因素,相对危险度(OR)=3.120。结论VDRSC多态性可能决定晚发性佝偻病的遗传易感性。  相似文献

The prevalence of obesity among children and adolescents has been rapidly increasing in recent years. Obese individuals are at risk for vitamin D deficiency. The aim of this study was to investigate the relation of vitamin D deficiency with puberty and insulin resistance in obese children and adolescents. A total of 106 children and adolescents (48 prepubertal and 58 pubertal) between 8 and 16 years of age were included in the study. Fasting blood glucose, insulin, lipid profile, calcium, phosphorus, alkaline phosphatase, parathyroid hormone, 25-hydroxyvitamin D [25(OH)D] levels, as well as blood glucose and insulin concentrations at 120 min of oral glucose tolerance test were measured. Insulin resistance was calculated using the homeostasis model assessment. Daily vitamin D intake was questioned. Serum 25(OH)D level was normal in only 3.8%, insufficient in 34.0%, and deficient in 62.2% of the subjects. There was a statistically significant rate of 25(OH)D deficiency in the pubertal group compared with that in the prepubertal group. Those subjects with 25(OH)D deficiency were found to have greater insulin resistance. Vitamin D deficiency is common among obese children and adolescents. Low vitamin D levels in obese individuals may accelerate the development of metabolic syndrome, type 2 diabetes mellitus, and cardiovascular disease by further increasing insulin resistance.  相似文献
