AIM: The purpose of the investigation was to examine in field conditions the acute effects of passive stretching after a general warming-up bout as well as the effects of passive stretching alone. METHODS: Eighteen adolescent team soccer players participated in this study performing 3 different flexibility-training protocols in separate training sessions. In the first treatment stretching protocol a general warm-up was performed where subjects jogged for 20 minutes. The second treatment stretching protocol consisted of the same general warm-up followed by passive stretching of the lower extremities and the trunk, whereas the third and final treatment stretching protocol consisted of passive stretching alone, without any jogging. Passive range of joint motion was examined in hip flexion, hip extension, hip abduction, ankle dorsiflexion, knee flexion and trunk flexion using a goniometer and a flexometer. RESULTS: The general warming-up session induced a significantly increased range of motion only at the ankle dorsiflexion joint (P<0.05). Results also suggest that passive stretching alone and passive stretching after a general warming-up bout both induced a significantly increased range in all lower extremity joints and trunk flexion (P<0.001). CONCLUSIONS: Improvements in flexibility are observed after passive muscle elongation, irrespective of warming-up.  相似文献

AIM: The purpose of the investigation was to examine the acute effects of static stretch on the range of motion (ROM) of the lower extremities and trunk in elderly women, when stretching is performed with and without warming-up exercises. METHODS: Twenty-two sedentary subjects 65-85 years old (mean age: 76.5 years) with normal ROM without joint abnormalities took part in the study, and performed 3 different flexibility protocols in non-consecutive randomized sessions. The first stretching protocol comprised of a general warming-up for 20 min, the second of the same general warming-up followed by static stretching of the lower extremities and the trunk, whereas the third and final stretching protocol consisted of static stretching alone. Passive ROM was examined at the lower extremity joints and trunk flexion, using a goniometer and a flexometer. RESULTS: Static stretching alone and static stretching after a general warming-up bout, significantly increased the range of all lower extremity joints and trunk flexion (P<0.001). The general warming-up session included a significantly increased ROM only at the ankle dorsiflexion joint (P<0.001). CONCLUSIONS: The results reflect immediate changes in flexibility via acute stretching exercises, in sedentary elderly women, when muscles undergo static elongation, irrespective of the performance of warming-up exercises.  相似文献

Objective

To establish the relation between handball playing, passive hip range of motion (ROM), and the development of radiological hip osteoarthritis (OA) in former elite handball players. Two related issues are addressed: (a) the relation between long term elite handball playing and the incidence of hip OA; (b) the relations between hip ROM, OA, and pain.

Methods

Data on 20 former elite handball players and 39 control subjects were collected. A questionnaire yielded personal details, loading patterns during physical activity, and previous lower limb joint injury. Bilateral radiographs were analysed to diagnose and classify hip OA. Passive hip ROM was measured bilaterally with a goniometer.

Results

A close relation was found between long term elite handball practice and the incidence of hip OA: 60% of the handball players were diagnosed with OA in at least one of the hip joints compared with 13% of the control subjects. Passive ROM measured in the handball players was significantly lower for hip flexion and medial rotation and higher for abduction, extension, and lateral rotation than the control values. The handball players with OA reported less pain in the hip joints during daily activities than the control subjects with OA.

Conclusion

The risk of developing premature hip OA seems high for retired handball players and significantly greater than for the general population. Pain and discomfort represent two difficult diagnostic challenges to the sports physician, as the repetitive nature of movements that are specific to handball can lead to alterations that are rarely seen in the general population.  相似文献

Forty-eight players from four senior, male soccer teams were tested for ranges of motion (ROM) in the lower extremity before, immediately after, and 24 h after different forms of soccer training. The players were tested after regular soccer training (A), after soccer training with contract-relax stretching prior to beginning the session (B), and after soccer training with stretching added at the end of the program (C). Each training session lasted 1.5 h. In group A all six ROMs were decreased 24 h after the training. In group B the only change in ROM noted was an increase in knee flexion directly after the training. In group C there was an increase in hip extension, hip flexion, and knee flexion directly after the training.  相似文献

Purpose

Team handball is associated with a high risk of severe knee injury that needs to be reduced, particularly at the youth level. The purpose of this study was to show how an injury-prevention programme effectively reduces severe knee injury in adolescent team handball players.

Methods

Of 23 adolescent handball teams of both sexes, 13 were randomly allocated into the intervention group (168 players) and 10 into the control group (111 players). Players of the intervention group regularly participated in an injury-prevention programme for one season. Handball exposure and sustained injuries were documented for both groups on a monthly basis. The primary outcome parameter of the injury-prevention programme was the incidence of severe knee injury.

Results

Of the 279 included players, 68 (24%) sustained 82 injuries yielding an overall incidence of 1.85 injuries per 1000 h handball exposure (intervention group: 50 injuries/incidence: 1.90/1000 h; control group: 32 injuries/incidence: 1.78/1000 h). Knee injury was the second most frequent injury in adolescent team handball. The primary outcome parameter, severe knee injury occurred significantly more often in the control group [mean age (SD) 15.1 (1.0), injury incidence 0.33/1000 h] than in the intervention group [mean age (SD) 14.9 (0.9), injury incidence 0.04/1000 h]. The odds ratio was 0.11 (95% CI 0.01–0.90), p?=?0.019. Other injuries to the lower extremities showed no significant difference between the two groups.

Conclusions

Frequent neuromuscular exercises prevent severe knee injury in adolescent team handball players and should thus be included in the practical routine as well as in the education of team coaches.

BACKGROUND: Neuromuscular training that includes both plyometric and dynamic stabilization/balance exercises alters movement biomechanics and reduces ACL injury risk in female athletes. The biomechanical effects of plyometric and balance training utilized separately are unknown. HYPOTHESIS: A protocol that includes balance training without plyometric training will decrease coronal plane hip, knee, and ankle motions during landing, and plyometric training will not affect coronal plane measures. The corollary hypothesis was that plyometric and balance training effects on knee flexion are dependent on the movement task tested. STUDY DESIGN: Controlled laboratory study. METHODS: Eighteen high school female athletes participated in 18 training sessions during a 7-week period. The plyometric group (n = 8) performed maximum-effort jumping and cutting exercises, and the balance group (n = 10) used dynamic stabilization/ balance exercises during training. Lower extremity kinematics were measured during the drop vertical jump and the medial drop landing before and after training using 3D motion analysis techniques. RESULTS: During the drop vertical jump, both plyometric and balance training reduced initial contact (P = .002), maximum hip adduction angle (P = .015), and maximum ankle eversion angle (P = .020). During the medial drop landing, both groups decreased initial contact (P = .002) and maximum knee abduction angle (P = .038). Plyometric training increased initial contact knee flexion (P = .047) and maximum knee flexion (P = .031) during the drop vertical jump, whereas the balance training increased maximum knee flexion (P = .005) during the medial drop landing. CONCLUSION: Both plyometric and balance training can reduce lower extremity valgus measures. Plyometric training affects sagittal plane kinematics primarily during a drop vertical jump, whereas balance training affects sagittal plane kinematics during single-legged drop landing. CLINICAL RELEVANCE: Both plyometric and dynamic stabilization/balance exercises should be included in injury-prevention protocols.  相似文献

BACKGROUND: Little data are available on changes that occur with age in joint range of motion in dancers and nondancers. HYPOTHESIS: In dancers, joint range of motion will increase with age, whereas it will decrease in nondancers, independent of the joint studied. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: The study population included 1320 female dancers, aged 8 to 16 years, who participated in different types of dancing classes (classical ballet, modern dance, jazz, etc) and 226 nondancers of similar age. Range of motion was measured for the hip, knee, ankle, foot, and spinal joints. RESULTS: The pattern of differences in range of motion with age varied in different joints and types of movement. (1) For combined ankle and foot plantar flexion (pointe), ankle plantar flexion, and hip external rotation, there was no change in range of motion in dancers, whereas range of motion diminished with age in the nondancers. (2) For ankle dorsiflexion, neither group showed any change with age, and range of motion was significantly greater in the nondancer group. (3) For knee flexion, hip flexion, and hip internal rotation, range of motion decreased with age in both groups. (4) For hip abduction, range of motion decreased with age in dancers and remained constant in the nondancers. (5) For hip extension, range of motion increased in both groups. (6) For lower back and hamstrings, range of motion increased among dancers with age and remained constant among nondancers. CONCLUSION: Dancers and teachers should realize that passive joint range of motion is unlikely to improve with age. Therefore, the major goal of a dancing program should focus on exercises that retain the natural flexibility of the dancers' joints rather than trying to improve them.  相似文献

A total of 114 healthy female European team handball players in the elite to second division with no previous ACL injury, and the uninjured legs of 22 females with a unilaterally reconstructed ACL (the injury sustained while playing handball; mean time from reconstruction 15 months) were examined with a Biodex isokinetic testing device. Five extensions and flexions at lower speed (60°/s) and 15 repetitions at higher speed (240°/s) were performed. Gravity-corrected peak torque values for flexion and extension were obtained, and hamstrings-to-quadriceps ratios (H/Q) were calculated at both speeds. Compared with the control group, the uninjured legs of the injury group showed 8% weaker quadriceps muscles and 14% higher H/Q ratios at higher speed. The other between-group comparisons were not significant. These differences could have been developed by strength reduction after the injury in the noninjured limb due-to-insufficient rehabilitation, by a weaker musculature in the patient group already initially, or by both mechanisms. If the second option is true, the finding may indicate that weak quadriceps musculature is one of the risk factors for anterior cruciate ligament injuries. The study establishes additional normative data on hamstrings and quadriceps torque on high-level female handball players.  相似文献

BACKGROUND: Several authors have analyzed the incidence of handball injuries in amateur players but information of handball injury in top-level players is very limited. The aim of the study was to analyze the incidence, circumstances and characteristics of handball injuries during major international tournaments. METHODS: Injuries during six international handball tournaments were analyzed, using an established injury report system. The physicians of all participating teams were asked to report all injuries after each match on a standardized injury report form. The response rate was on average 87%. RESULTS: The incidence of injury was on average 108 injuries/1000 player hours (95% confidence interval (CI): 98-117) or 1.5 injuries/match (95% CI: 1.4-1.6). The injuries affected most frequently the lower extremity (42%), followed by injuries of the head (23%), upper extremity (18%) and trunk (14%). The most frequent diagnosis was contusion of head (14%) or ankle sprain (8%). The majority of injuries were caused by contact with another player. The incidence of time loss injuries was on average 27/1000 player hours (95% CI: 22-32), and significantly higher in men than in women. CONCLUSION: Preventive program proven effective should be implemented. Fair Play is an essential aspect of injury prevention. Therefore, close cooperation with the referees is also necessary to make handball a safer sport.  相似文献

INTRODUCTION/PURPOSE: Muscles of the trunk, hip, and knee influence the orientation of the lower extremity during weight bearing activities. The purpose of this study was threefold: first, to compare the orientation of the lower extremity during a single leg (SL) squat among male and female athletes; second, to compare the strength of muscle groups in the trunk, hips, and knees between these individuals; and third, to evaluate the association between trunk, hip, and knee strength and the orientation of the knee joint during this activity. METHODS: Twenty-four male and 22 female athletes participated in this study. Peak isometric torque was determined for the following muscle actions: trunk flexion, extension, and lateral flexion, hip abduction and external rotation, and knee flexion and extension. The frontal plane projection angle (FPPA) of the knee during a 45 degrees SL squat was determined using photo editing software. RESULTS: Males and females moved in opposite directions during the SL squat test (F(1,42) = 5.05, P = 0.03). Females typically moved toward more extreme FPPA during SL squats (P = 0.056), while males tended to move toward more neutral alignment (P = 0.066). Females also generated less torque in all muscle groups, with the exception of trunk extension. The projection angle of the knee during the SL squat test was most closely associated with hip external rotation strength. CONCLUSION: Using instruments suitable for a clinical setting, females were found to have greater FPPA and generally decreased trunk, hip, and knee isometric torque. Hip external rotation strength was most closely associated with the frontal plane projection angle.  相似文献

ObjectivesTo assess the effect of mid-flight trunk flexion and extension on the movements of body segments and lower extremity joints and subsequent landing mechanics during a jump-landing task.DesignParticipants performed three jump-landing conditions in a randomized order.MethodsForty-one participants completed jump-landing trials when performing three different mid-flight trunk motion: reaching forward, reaching up, and reaching backward. Whole-body kinematic and ground reaction force data were collected.ResultsThe reaching backward condition resulted in a more posteriorly positioned upper body center of mass (COM) and more anteriorly positioned pelvis COM, legs COM, hip, and knee joint positions relative to the whole-body COM in flight and at initial contact of landing. The reaching backward condition showed the least hip flexion and ankle plantarflexion angles at initial contact as well as the least hip and knee flexion angles and the greatest ankle dorsiflexion angles at 100 ms after landing. The reaching backward condition also demonstrated the greatest peak posterior ground reaction forces, peak and average knee extension moments, peak and average hip flexion moments, and peak knee varus moments within the first 100 ms after landing. Opposite changes were observed for the reaching forward condition.ConclusionsMid-flight trunk extension resulted in body postures that predisposed individuals to land with increased knee extension and varus moments and decreased knee flexion angles, which are indirectly associated with increased ACL loading. These findings may help to understand altered trunk motion during certain ACL injury events and provide information for developing jump-landing training strategies.  相似文献

The effects of general warming up, massage, and stretching on ranges of motion (ROM) and strength of quadriceps and hamstring muscles were measured in eight male volunteers. Thigh muscle strength was not influenced by the experimental procedures. Stretching resulted in a significantly increased range of hip flexion/extension, hip abduction, knee flexion, and ankle dorsiflexion; the effect was significantly greater than that obtained by massage and warming up separately or combined. Only ankle dorsiflexion was influenced by massage or warming up, whereas stretching affected all muscle groups tested. Stretching was, therefore, superior to the other methods tested for increasing flexibility in the lower extremity.  相似文献

We aimed to examine the prevalence of injury after physical therapy intervention for muscle tightness and injury prevention in male high school soccer players. A randomized controlled trial was conducted. Participants comprised 124 players from two high schools who competed in national tournament soccer games held from April 2018 to March 2019. Players were randomly divided into intervention (with a 12-week stretching intervention by physical therapists) and control groups (without the intervention). Players and coaches provided written information regarding injuries and daily training and match times; physical therapists visited each team weekly to collect data and review documentation. Muscle tightness and injury incidence, number, type, location, circumstances, situations, severity, and contents during the 12-week intervention period and a subsequent 40-week observation period were compared between groups. Injuries were significantly lower with intervention during the 40-week observation period (P < .01) but not during the 12-week intervention period (P = .44). Injury types mainly included disorder, non-contact, lower-limb/trunk, and muscle/tendon injuries. Significant interactions were observed for all tightness-test measurement items. The intervention group showed significant improvements in heel-buttock distance, and straight leg-raise and hip rotation angles (pre-intervention < 12 weeks < 52 weeks), as well as significant improvements in ankle dorsiflexion angles at 12 and 52 weeks (relative to pre-intervention values). Instructed stretching exercises, personally designed by physical therapists to address muscle tightness, improved the range of motion and trunk flexibility, with a positive effect on the injury rate in male high school soccer players, especially for non-contact disorder injuries during training.  相似文献

OBJECTIVE: To quantify the prevalence of osteoarthritis and the severity of pain in the lower limb joints of players retired from English professional soccer. METHOD: An anonymous self administered questionnaire was distributed to 500 former players registered with the English Professional Footballers' Association. The questionnaire was designed to gather information on personal details, physical activity loading patterns, history of lower limb joint injury, and current medical condition of the lower limb joints. RESULTS: Of 500 questionnaires distributed, 185 (37%) were returned. Nearly half of the respondents (79: 47%) retired because of injury; 42% (33) were acute injuries and 58% (46) chronic injuries. Most of the acute injuries that led to early retirement were of the knee (15: 46%), followed by the ankle (7: 21%) and lower back (5: 15%). Most of the chronic injuries that led to early retirement were also of the knee (17: 37%), followed by the lower back (10: 22%) and the hip (4: 9%). Of all respondents, 32% (59) had been medically diagnosed with osteoarthritis in at least one of the lower limb joints. More respondents had been diagnosed with osteoarthritis in the knee joints than either the ankle or the hip joints. Significantly (p<0.001) more respondents reported pain in one lower extremity joint during one or more daily activities than those who did not (joint pain: 137, 80%; no joint pain: 35, 20%). CONCLUSION: The risk for professional soccer players of osteoarthritis in at least one of the lower extremity joints is very high and significantly greater than for the general population. The results support the suggestion that professional soccer players should be provided with health surveillance during their playing career.  相似文献

PURPOSE: This study investigated the influence of static stretching exercises on specific exercise performances. METHODS: Thirty-eight volunteers participated in this study. The stretching group (STR) consisted of 8 males and 11 females whose activity was limited to a 10-wk, 40-min, 3-d.wk(-1) static stretching routine designed to stretch all the major muscle groups in the lower extremity. The control group (CON) consisted of 8 males and 11 females who did not participate in any kind of regular exercise routine during the study. Each subject was measured before and after for flexibility, power (20-m sprint, standing long jump, vertical jump), strength (knee flexion and knee extension one-repetition maximum (1RM)), and strength endurance (number of repetitions at 60% of 1RM for both knee flexion and knee extension). RESULTS: STR had significant average improvements (P < 0.05) for flexibility (18.1%), standing long jump (2.3%), vertical jump (6.7%), 20-m sprint (1.3%), knee flexion 1RM (15.3%), knee extension 1RM (32.4%), knee flexion endurance (30.4%) and knee extension endurance (28.5%). The control group showed no improvement. CONCLUSION: This study suggests that chronic static stretching exercises by themselves can improve specific exercise performances. It is possible that persons who are unable to participate in traditional strength training activities may be able to experience gains through stretching, which would allow them to transition into a more traditional exercise regimen.  相似文献

ObjectivesTo determine if there is evidence of abnormal hip joint range of motion (ROM) in youth and senior team professional footballers compared with matched controls.DesignA case control study design was used.Participants40 professional footballers (20 youth and 20 senior team) and 40 matched control subjects.Main outcome measuresBilateral measurements of passive hip internal rotation (IR), external rotation (ER), flexion, abduction and extension were made together with Faber's test and the hip quadrant.ResultsYouth and senior footballers had significantly less IR and Faber's range and significantly higher abduction than their respective controls (p < 0.001). Senior footballers also had significantly reduced IR (p < 0.05) and Faber's (p < 0.001) than the youth team. A higher proportion of senior footballers had positive hip quadrants (45% of all hips) compared to all other groups. No significant difference in hip ROM was found between dominant and non-dominant legs.ConclusionsA specific pattern of hip ROM was found in professional footballers which appeared to be different from controls. These changes may demonstrate the early stages of hip degeneration to which it has been shown ex-professional players are prone to. Hip joint ROM exercises may be necessary in these players to restore normal movement and prevent the onset of hip osteoarthritis (OA).  相似文献

This study evaluated changes in range of motion (ROM) in competitive male and female junior tennis players participating in a specific conditioning program over a two-year period. Subjects were also compared to an age and sex-matched control group of players not performing the exercises. Fifty-one tennis players, 29 male (mean age 13.6) and 22 female (mean age 13.2) entered the study. Baseline ROM measurements were performed on all players upon entering the study. The conditioning program employed standardised stretch-hold-relax flexibility exercises for all the areas tested. These exercises were taught to the players and each athlete was given a video of the exercises. Compliance with the program was assessed by exercise logs, the parents, and the athletes. High compliers were compared to low compliers. Mean changes in the combined ROM measurements over time indicate improvement in flexibility in most anatomical areas after one year of participation, with small improvements in the second year. With the exception of forearm supination, there were no differences between high compliers and low compliers. Compared to controls, the experimental group showed significant improvement in sit and reach, dominant and nondominant shoulder internal rotation, dominant and nondominant shoulder external rotation, nondominant gastrocnemius, dominant iliotibial band, dominant and nondominant hip internal rotation, dominant and nondominant hip external rotation, dominant and nondominant forearm pronation, and dominant wrist flexion. The areas of greatest significant change were in the shoulder and back, which correspond to the areas of most clinical risk of Injury. In conclusion, joint ROM can be positively influenced with a specific conditioning program.  相似文献

The purpose of the study was to determine the effects of kinesio taping (KT) on trunk flexion, extension, and lateral flexion. Thirty healthy subjects with no history of lower trunk or back issues participated in the study. Subjects performed two experimental measurements of range of motion (with and without the application of KT) in trunk flexion, extension, and right lateral flexion. A dependent t test was used to compare the range of motion measurements before and after the application of KT. Through evaluation of the sum of all scores, KT in flexion produced a gain of 17.8 cm compared with the non-kinesiotape group (t(29)=2.51, p<0.05). No significant difference was identified for extension (-2.9 cm; t(29)=-0.55, p>0.05) or lateral flexion (3 cm; t(29)=-1.25, p>0.05). Based on the findings, we determined that KT applied over the lower trunk may increase active lower trunk flexion range of motion. Further investigation on the effects of KT is warranted.  相似文献
