目的 探讨亚临床肝性脑病(SHE)早期诊断的方法。方法 对46例肝硬化无肝性脑病临床表现的患者进行P300、脑干听觉诱发电位(BAEP)、数字连接测验(NCT)、简明智力状况检查量表(MMSE)及脑电图(EEG)检测和随访,并与43例非肝病患者进行比较。结果 肝硬化组P300潜伏期、NCT时间较对照组明显延长(均P〈0.01);BAEP异常率明显增加(P〈0.01);随肝功能损害程度加重,P300潜伏期逐级显著延长、波幅显著降低,NCT时间显著延长,BAEP异常率显著增加(P〈0.05—0.01)。随访发现P300、BAEP或NCT异常的肝硬化患者HE发生率间差异无统计学意义。结论 P300、BAEP及NCT可作为早期诊断SHE的检测方法。 相似文献

BACKGROUND: Electroencephalogram (EEG) and brainstem auditory evoked potential (BAEP) are objective non-invasive means of measuring brain electrophysiology. OBJECTIVE: To analyze the value of EEG and BAEP in early diagnosis, treatment and prognostic evaluation of central coordination disorder. DESIGN, TIME AND SETTING: This case analysis study was performed at the Rehabilitation Center of Hunan Children's Hospital from January 2002 to January 2006. PARTICIPANTS: A total of 593 patients with severe central coordination disorder, comprising 455 boys and 138 girls, aged 1-6 months were enrolled for this study. METHODS: EEG was monitored using electroencephalography. BAEP was recorded using a Keypoint electromyogram device. Intelligence was tested by professionals using the Gesell scale. MAIN OUTCOME MEASURES: (1) The rate of abnormal EEG and BAEP, (2) correlation of abnormalities of EEG and BAEP with associated injuries, (3) correlation of abnormalities of EEG and BAEP with high risk factors. RESULTS: The rate of abnormal EEG was 68.6% (407/593 patients), and was increased in patients who also had mental retardation (P 〈 0.05). The rate of abnormal BAEP was 21.4% (127/593 patients). These 127 patients included 67 patients (52.8%) with peripheral auditory damage and 60 patients (47.2%) with central and mixed auditory damage. The rate of abnormal BAEP was significantly increased in patients who also had mental retardation (P 〈 0.01 ). Logistic regression analysis showed that asphyxia (P 〈 0.05), jaundice, preterm delivery, low birth weight and the umbilical cord around the neck were closely correlated with abnormal EEG in patients with central coordination disorder, lntracranial hemorrhage, jaundice (P 〈 0.05), low birth weight and intrauterine infection (P 〈 0.05) were closely correlated with abnormal BAEP in patients with central coordination disorder. 目的分析多发性硬化(multiple sclerosis,MS)模式翻转视觉诱发电位(pattern reversal evoked po-tential,PRVEP)、脑干听觉诱发电位(brainstem auditory evoked potential,BAEP)和体感诱发电位(somatosenso-ry evoked potential,SEP)等三种诱发电位(evoked potential,EP)的临床特点。方法对83例确诊MS患者进行回顾性分析,根据有无相应临床症状、病程及功能残障程度对EP进行分层研究,探讨其变化规律。结果三种EP的异常率在有临床症状组〔PRVEP、BAEP及下肢短潜伏期体感诱发电位(SLSEP)异常率分别为88.00%、66.67%、100%〕与无临床症状组(PRVEP、BAEP及下肢SLSEP异常率分别为60.61%、31.71%、79.63%)间比较均存在统计学差异(均P<0.05)。PRVEP的峰潜伏期(PL)延长及侧间峰潜伏期差值(ILD)增加的异常率之和与病程呈正相关(r=1.0,P<0.05);病程在20年以内时BAEP异常率与病程呈正相关(r=1.0,P<0.05);SLSEP下肢未引出率与病程呈正相关(r=1.0,P<0.05)。PRVEP异常率与EDSS分值呈正相关(r=1.7,P<0.01);SLSEP上肢异常率及下肢未引出率也与EDSS分值呈正相关(分别r=1.8,P<0.01;r=1.6,P<0.01)。结论三种EP的异常率与有无相应临床症状相关,且与病程及功能残障程度在一定范围内呈正相关。 相似文献

目的 观察帕金森病早期体感诱发电位(SL-SEP)、脑干听觉诱发电位(BAEP)和视觉诱发电位(PR-VEP)的异常特征.寻找敏感可靠的诊断指标。方法 对一组52例Yahr分期为Ⅰ期的帕金森病人和一组40例年龄匹配的健康对照组进行三种诱发电位观测。结果帕金森病早期SLSEP异常率为69.2%;BAEP的异常率为32.7%:PR-VEP的异常率为30、7%,病例组SL-SEP各主波峰潜伏期、峰间潜伏期、P25和N30波幅与对照组比较差异有湿著性意义;病例组BAEPⅠ—ⅢIPL、Ⅲ—ⅤIPL、Ⅰ一Ⅲ/Ⅲ一Ⅴ比值和PR-VEP的P100PL与对照组比较差异无显著性意义.结论 在帕金森病的早期,皮层感觉处理过程已受到干扰:体感诱发电位中央前成份以及顶成份P25是一个敏感的诊断指标。 相似文献

Sensory evoked potentials were studied in 15 patients with Wilson's disease. Thirteen patients were investigated with pattern reversal visual stimulation. A prolonged P 100 latency of the VEP was present in 7 patients. Brain stem auditory responses were evoked in 12 patients. Prolongation of III-V and I-V interpeak latency was found in 8 patients. The evoked potential studies demonstrated subclinical disturbances in optic and caudal brainstem auditory pathways. Further studies are in progress to evaluate the role of these techniques in monitoring the therapy of newly diagnosed cases. 相似文献

A 2-yr-3-mo-old girl with metachromatic leukodystrophy (MLD) was examined using serial multiple electrophysiological procedures. Sensory nerve conduction velocity was delayed earlier and more severely than motor nerve conduction velocity. Visual evoked potentials (VEPs) showed prolonged latency of wave IV. Auditory brainstem responses (ABRs) showed prolonged latency of waves I and V, and the I-V interval. As to the interpeak latency of somatosensory evoked potentials (SEPs), the P9-P14 and the P14-N20 intervals were prolonged on admission. Two months later, both intervals were more prolonged, but the prolongation of the P9-P14 interval was the most prominent. The demyelination in our case may have started in the cerebral white matter, progressed to the peripheral nerves, and at last via the spinal root reached the brainstem. An electrophysiological follow-up study may be valuable in the understanding of the progressive pathological changes and in the evaluation of therapeutic measures. 相似文献

Visual and brainstem auditory evoked potentials (VEPs, BAEPs) were recorded in 23 patients with neurosarcoidosis. Eight patients (35%) had abnormal BAEPs, and 10 (43%) had abnormal VEPs. Four of the 8 patients with abnormal BAEPs had facial paresis, one had impaired memory and only 3 had symptoms and signs compatible with brainstem lesion. Seven of the patients with abnormal VEPs had no visual symptoms. These findings suggest that BAEP and VEP can reveal subclinical nervous system involvement in sarcoidosis and can also help in the early diagnosis of neurosarcoidosis. Successive recordings of 5 patients showed that BAEP and VEP were useful in the follow-up of these patients. 相似文献

目的探讨大鼠慢性间断性缺氧后,脑诱发电位与慢性缺氧性脑损伤的关系,探讨脑诱发电位对缺氧性脑病的评估价值。方法在建立慢性间断性缺氧大鼠模型的基础上,雄性SD大鼠随机分成缺氧组(hypoxia group,H组)和对照组(normal group,N组),H组随机分为缺氧2周组(H2组)和缺氧4周组(H4组)2个亚组;N组随机分为正常2周组(N2组)和正常4周组(N4组)2个亚组。测定各组大鼠的脑干听觉诱发电位和短潜伏期体感诱发电位。结果BAEP的检测结果,H4和H2组大鼠、、波峰潜伏期,-、-波峰间期与N4和N2组相比显著延长(P<0.05);H4组、波PL,-、-波IPL与H2组相比显著延长(P<0.05);SLSEP的检测结果,H4和H2组N1、P1波PL与N4和N2组比较显著延长(P<0.05);H4组N1、P1波PL与H2组相比显著延长(P<0.05)。结论BAEP和SLSEP的改变与慢性缺氧性脑损伤及损伤的程度明显相关,是反映缺氧性脑损伤比较敏感的指标。 相似文献

The aims of this study were evaluate motor, somatosensory, visual and auditory brainstem evoked potential (MEP, SEP, VEP, ABER) changes in Wilson disease (WD) and correlate these with magnetic resonance imaging (MRI) and clinical findings.
Neurologic WD diagnosed on the basis of clinical, ceruloplasmin and Kayser–Fleischer ring were evaluated including pedigree charting, hepatic, renal, hematologic and osteoarticular manifestations. Blood counts, serum chemistry, MRI, MEP to tibialis anterior, tibial SEP, VEP and ABER were performed. Evoked potential (EP) changes were correlated with clinical and MRI findings.
Eighteen WD patients were recruited from 17 families whose mean age was 16 years. Movement disorders were present in 14, cognitive decline in 12 and pyramidal signs in 12 patients. MRI revealed involvement of basal ganglia in 80%, thalamus in 40%, brain stem in 46.7% and subcortical white matter in 53.3%. MEP was abnormal in 35.7%, SEP in 30.8%, VEP in 57% and ABER in 61.5% patients; the latter three EP changes were subclinical. Frequency and number of EP abnormalities were higher with increasing severity of illness.
SEP, VEP and ABER reveals subclinical abnormality and MEP helps in documenting both clinical and subclinical abnormalities. Number of EP abnormalities increases with increasing clinical severity of WD. 相似文献

Pentobarbital-induced changes in the mouse brainstem auditory evoked potential as a function of click repetition rate and time postdrug 总被引:1,自引:0,他引:1
The effects of pentobarbital (80 mg/kg, i.p.) and saline injections on the BAEP were studied in 10 adult female BDF1 mice. Pentobarbital anesthesia induced statistically significant increases in the amplitudes and latencies of various BAEP components relative to preinjection and saline control values. These pentobarbital-induced changes were maximal shortly after drug administration and dissipated over time in a pattern similar to a drug elimination curve. Since pentobarbital and fast stimulus repetition rates are considered to be 'synaptic stressors', it was predicted that pentobarbital anesthesia and click repetition rate would have combined effects on the BAEP. This prediction was partially supported in that pentobarbital-induced changes in P4 amplitude and latency were significantly dependent on click repetition rate. The pentobarbital-induced effects on earlier BAEP components, however, proved to be largely independent of click rate. Pentobarbital-associated changes in the BAEP were not due to such factors as core temperature changes, circadian variations, and stress. The importance of anesthetic-induced changes in the BAEP for clinical and experimental studies is discussed. 相似文献

The yield of magnetic resonance imaging (MRI) and electrophysiological studies in detecting brainstem lesions was assessed in 35 patients suffering from traumatic brain injury (Glasgow Coma Scale, 3-10). As an inclusion criterion, all patients had brainstem trauma as revealed by early MRI or electrophysiological studies. Of the 35 cases, 7 (20%) had brainstem lesions detected by MRI only, whereas in 10 patients (29%), electrophysiological examination disclosed impairment of brainstem function with normal MRI. In 18 (51%) subjects, both diagnostic techniques revealed brainstem lesions. The midbrain was the most common location of lesions. Masseter reflex recording had the highest yield (93%) of abnormal findings. No mismatch with respect to site and side of abnormality occurred between MRI and electrophysiological studies. Outcome analysis indicated an unfavorable course for the vast majority (83%) of patients, regardless of the diagnostic means disclosing traumatic brainstem injury. Therefore, both techniques are effective in disclosing traumatic brainstem injury, with diagnostic overlap in about 50% of cases. In contrast to MRI, electrophysiological investigation is easily performed and repeated at low cost in the setting of an intensive care unit, where such patients are typically hospitalized after trauma. In addition to electrophysiological assessment of brainstem function, MRI is recommended in each case having normal electrophysiological findings when brainstem injury is suspected. 相似文献

ObjectiveTo investigate how functional integrity of ascending sensory pathways measured by visual and somatosensory evoked potentials (VEP & SEP) is associated with abnormal glycemia and brain injury in newborns treated with hypothermia for hypoxic-ischemic encephalopathy (HIE).MethodsFifty-four neonates ≥ 36 weeks gestational age with HIE underwent glucose testing, VEPs, SEPs, and magnetic resonance imaging (MRI) the first week of life. Minimum and maximum glucose values recorded prior to evoked potential (EP) testing were compared with VEP and SEP measures using generalized estimating equations. Relationships between VEP and SEP measures and brain injury on MRI were assessed.ResultsMaximum glucose is associated with decreased P200 amplitude, and increased odds that N300 peak will be delayed/absent. Minimum glucose is associated with decreased P22 amplitude. Presence of P200 and N300 peaks is associated with decreased odds of brain injury in the visual processing pathway, with delayed/absent N300 peak associated with increased odds of brain injury in posterior white matter.ConclusionsDeviations from normoglycemia are associated with abnormal EPs, and abnormal VEPs are associated with brain injury on MRI in cooled neonates with HIE.SignificanceGlucose is a modifiable risk factor associated with atypical brain function in neonates with HIE despite hypothermia treatment. 相似文献

We have investigated brainstem and cortical auditory responses (BAERs and CAERs) in 16 cases of Friedreich's ataxia (FA) and have compared the findings with those obtained in 2 cases of familial spastic paraplegia (FSP), in 5 cases of Charcot-Marie-Tooth disease (CMTD), and in 6 cases of atypical FA of uncertain classification. BAERs could not be elicited in 11 FA patients and constantly disappeared at a higher intensity threshold than in normal subjects in the remaining 5 patients. BAERs were normal or only slightly abnormal in FSP and CMTD patients. CAERs were normal in all 29 patients. BAERs tended to disappear with the progression of FA and BAER thresholds were correlated with the Inherited Ataxias Clinical Rating Scale score, which is an index of the severity of illness. BAERs contributed to the diagnosis, or exclusion of FA in patients with an atypical picture. It is suggested that in FA myelinated fibers in the spiral ganglion are partially affected, resulting in the decrease of wave amplitude such as occurs for peripheral sensory potentials. 相似文献

目的:探讨孤独症谱系障碍(ASD)患儿婴儿期体格、神经心理发育及脑干听觉诱发电位(BAEP)特征。方法:收集21例2~6岁ASD患儿(ASD组)在其1、3、6及12月龄时体格发育资料(体质量、身长和头围)、神经心理发育评估资料[包括智力发展指数(MDI)和精神运动发展指数(PDI)]及BAEP检测结果(包括Ⅰ、Ⅲ、Ⅴ波潜伏期及I-Ⅲ、Ⅲ-Ⅴ、Ⅰ-Ⅴ峰间期),并与63名同龄正常儿童(对照组)比较。结果:两组婴儿期各月龄体格发育水平比较差异无统计学意义(P>0.05)。神经心理发育评估显示,12月龄时ASD组MDI和PDI明显低于对照组(P均<0.01)。BAEP检测结果显示,1及3月龄时ASD组BAEPⅠ波、Ⅲ波、Ⅴ波潜伏期、Ⅰ-Ⅲ峰间期、Ⅲ-Ⅴ峰间期及Ⅰ-Ⅴ峰间期明显长于对照组(P<0.05或P<0.01)。ASD组异常频率最多的是V波潜伏期延长(47.2%)。结论:婴儿期早期(1~3月龄)持续V波潜伏期延长、12月龄时神经心理发育迟缓可能为ASD患儿早期特征性改变,可作为ASD早期预警指标之一。 相似文献

目的:探讨瞬目反射及脑干听觉诱发电位(BAEP)对糖尿病患者颅神经亚临床损害的诊断价值。方法:对150例糖尿病患者及50名健康志愿者进行双侧瞬目反射和BAEP检查。结果:瞬目反射和BAEP均检查200人、400侧次,两项检查在糖尿病患者中总的阳性率为31.3%,远高于临床症状、体征的阳性率(6%)。瞬目反射中R2、R2′潜伏期和R1-R2波间期延长最为明显,BAEP的异常则以V波潜伏期延长最常见。结论:瞬目反射与BAEP检查能够可靠的检测糖尿病患者颅神经和脑干受损的早期及亚临床损害。 相似文献

Contrast (pattern-reversal) visual evoked potentials (VEPs) were examined in the awake rabbit to test the possibility that carotid occlusion would disrupt sensory transmission in the visual pathways. Rabbits were operated to ligate one carotid and to chronically transplant another one into a skin flap. VEPs to checkerboard stimuli reversing with a rate of 2/s were recorded from the visual cortex. No significant changes in latency, amplitude and waveshape of VEPs were noted after 3-30 min of carotid occlusion. Apparently, blood flow from the basilar system can be easily shunted via the circle of Willis to the territory of the internal carotid. 相似文献

目的:探讨脑干听觉诱发电位(BAEP)对前庭系统性眩晕的定位价值。方法:对138例临床确诊的前庭系统性眩晕患者进行了BAEP的检测。结果:138例前庭系统性眩晕患者行BAEP检查有98例异常,异常率为71%,其中内耳型13/98例(13%),表现为Ⅰ、Ⅱ波各项指标的改变;脑干型48/98例(49%),表现为Ⅰ、Ⅱ波正常,Ⅲ、Ⅳ、Ⅴ波各项指标存在异常;混合型37/98例(38%),各波指标均出现改变。结论:BAEP有助于鉴别前庭周围性眩晕与前庭中枢性眩晕,在前庭系统性眩晕的临床诊断中有定位价值。 相似文献
