麦考酚酸酯在肾移植中的应用 总被引:4,自引:2,他引:2
目的:观察麦考酚酸酯( mymcophenolatemofetil,MMF)预防移植肾急性排斥的安全性及有效性。方法:40例肾移植病人分为2组,20例[男性12例,女性 8例,年龄(43 ± s 10) a]为治疗组,给予MMF1g, po, bid;另 20例[男性 13例,女性 7例,年龄(45±11) a]为对照组,给予硫唑嘌呤(Aza) 150mp,po, qn。共 6 mo。结果: MMF组急性排斥反应发生率为0,Aza组为2例,2组间差异无显著意义(P>0.05)。2组病人的消化道反应、肝中毒、感染等并发症的发生率的差异无显著意义(P>0.05)。结论:MMF是一种能预防移植肾急性排斥反应的安全、有效的免疫抑制剂。 相似文献

利用“单因素多浓度”和“均匀设计法”对环抱菌素生产菌株茄病镰刀菌的发酵条件和发酵培养基组分进行优化.并在5000L发酵罐上对菌株的发酵动力学规律作了初步的探索。实验结果表明。5.5%玉米粉,0.5%葡萄糖,0.5%干酪素.0.05%KCl,0.05%MgSO4,0.015%KH2PO4和0.3%CaCO3为最佳摇瓶发酵培养基组成;5000L发酵罐的发酵动力学规律表明,菌体浓度发酵培养至80h左右达到最大.产素水平在116h左右达到最大。 相似文献

目的:探讨肾移植患者中影响麦考酚酸(MPA)游离分数的因素。方法:采集23名肾移植术后3个月内患者12h内11个时间点的血浆样本,HPLC法测定血浆中游离和总的MPA及其代谢物葡萄糖醛酸结合物(MPAG)浓度。多元线性回归法考察受试者生理、病理特征和MPA游离分数(FUMPA)的相关性。结果:MPA游离分数的自然对数[In(FUMPA)]与白蛋白(ALB),血红蛋白的自然对数[In(Hb)]呈负相关,与MPA游离浓度AUC(MPAfAUG0-12),MPAG的游离分数(FUMPAG)呈正相关,相关系数r分别为-0.552,-0.459,0.781和0.458;P值分别为0.006,0.028,0.000和0.028。其关系式为:In(FUMPA)=-0.0554(ALB)-1.924,r=-0.552;ln(FUMPA)=1.128MPAf+2.181FUMPAG-5.171,r=-0.850。结论:ALB升高,可致FUMPA呈指数增加;MPAG可与MPA竞争性结合蛋白。 相似文献

In vitro and in vivo studies have shown that building-associated Penicillium spores and spore extracts can induce significant inflammatory responses in lung cells and animal models of lung disease. However, because spores and spore extracts comprise mixtures of bioactive constituents often including toxins, it is impossible to resolve which constituent mediates inflammatory responses. This study examined dose-response (0.5 nM, 2.5 nM, 5.0 nM, 12.5 nM/g body weight (BW) animal) and time-course (3, 6, 24 and 48 h post instillation (PI)) relationships associated with inflammatory and cytotoxic responses in mouse lungs intratracheally instilled with pure brevianamide A, mycophenolic acid, and roquefortine C. High doses (5.0 nM and/or 12.5 nM/g BW animal) of brevianamide A and mycophenolic acid, the dominant metabolites of P. brevicompactum, and roquefortine C, the dominant metabolite of P. chrysogenum, induced significant inflammatory responses within 6 h PI, expressed as differentially elevated macrophage, neutrophil, MIP-2, TNF, and IL-6 concentrations in the bronchioalveolar lavage fluid (BALF) of intratracheally exposed mice. Macrophage and neutrophil numbers were maximal at 24 h PI; responses of the other inflammatory markers were maximal at 6 h PI. Except for macrophage numbers in mycophenolic acid-treatment animals, cells exhibited significant dose-dependent-like responses; for the chemo-/cytokine markers, dose dependency was lacking except for MIP-2 concentration in brevianamide A-treatment animals. It was also found that brevianamide A induced cytotoxicity expressed as significantly increased LDH concentration in mouse BALF, at concentrations of 12.5 nM/g BW animal and at 6 and 24 h PI. Albumin concentrations, measured as a nonspecific marker of vascular leakage, were significantly elevated in the BALF of mice treated with 12.5 nM/g nM brevianamide A/animal from 6 to 24 h PI and in > or =5.0 nM/g mycophenolic acid-treated animals at 6 to 24 h PI. These results suggest that these three toxins from Penicillium species common on damp materials in residential housing provoke compound-specific toxic responses with different toxicokinetics. Moreover, that these toxins can stimulate significant inflammatory responses in vivo might help explain some of the indoor effects associated with Penicillium spore exposures in indoor environments. 相似文献

目的:建立HPLC测定人血浆中麦考酚酸(MPA)及其代谢物葡糖醛酸结合物(MPAG)浓度的方法。方法:以安定为内标,血浆经乙腈蛋白沉淀后直接进样。采用Zorbax Eclipse XDB C_(18)(150 mm×4.6 mm,5μm),柱温:30℃,流动相:甲醇-0.1%三氟乙酸(55:45,pH=2.2),流速:1.0 mL·min~(-1),检测波长215 nm。结果:血浆内源性杂质和常用合并用药对样品测定无干扰,MPA和MPAG血药浓度分别在0.5-40μG·mL~(-1)和2.5—200μg·mL~(-1)范围内线性关系良好。MPA和MPAG的日内、日间RSD均小于8%,样品提取后在24 h内及3次冻融后稳定性良好。结论:该法快速,简便,准确,可用于MPA和MPAG的体内分析及常规的治疗药物监测。 相似文献

目的研究肾移植受者的霉酚酸(MPA)药代动力学。方法 采用高效液相色谱法监测19名肾移植受者术后多剂量口服霉酚酸酯(MMF)后的MPA血药浓度,采用非线性混合效应模型法计算多剂量口服达稳态的MPA药代动力学参数。结果 MPA达峰时间Tmax为1.0 h,峰浓度Cmax为8.3μg/ml,谷浓度Cmin为2.6μg/ml,药时曲线下面积AUC0~12 h为44.9μg.h/ml。MPA药动学参数群体值CL=27.7 L/h,Vd=85.3 L,个体间变异分别为22.0%2、5.7%。结论本研究建立的群体药动学模型可较准确地描述MPA在肾移植受者的药动学特征,其预测能力尚需进一步评估。 相似文献

The potential bioavailability improvement of mycophenolic acid (MPA), 1, through ester derivatization was evaluated in monkeys at a dose of 20 mg/kg in this study. The acetyl solketal ester 3 was found to have excellent partition properties but poor aqueous solubility. Thus, even though it can be converted rapidly to MPA by plasma and liver enzymes, it showed poor oral bioavailability (56% of MPA) in monkeys. The bioavailability of the morpholinoethyl ester 4 and the acetyl morpholinoethyl ester 5, on the other hand, was found to be 236 and 150% that of MPA, respectively. Since ester 5 has greater aqueous solubility, but similar chemical stability and enzymatic hydrolysis rates compared to ester 4, the better bioavailability of ester 4 may result from its greater partitioning into the gastrointestinal membranes. 相似文献

目的建立反相高效液相色谱法,同时测定人血浆中霉酚酸(mycophenolic acid,MPA)、酚化葡萄糖醛麦考酚酸(phenol glucuronide metabolite,MPAG)、酰基化MPAG(acyl-MPAG,AcMPAG)的浓度。方法用蛋白沉淀法对样品进行处理。固定相为Zorbax Eclipse XDB C18柱(4.6 mm×250 mm,5μm),流动相:甲醇:20 mmol-L-1 NaH2PO4(用20%磷酸调至pH 3.0)为55∶45,流速:1.2 mL-min-1,检测波长:304 nm,柱温:45℃。结果 MPA、MPAG、AcMPAG浓度在0.2~50μg-mL-1(r=0.999 7)、2.8~531μg-mL-1(r=0.999 9)、0.3~24μg-mL-1(r=0.999 4)内呈良好的线性关系。MPA及其代谢物的绝对回收率均大于80%,MPA、MPAG、AcMPAG的相对回收率分别为94.0%~101.4%,98.4%~101.9%和96.1%~104.2%。日内及日间RSD远低于15%。结论采用反相高效液相色谱法测定人血浆中MPA、MPAG、AcMPAG的浓度和进行药物代谢动力学研究,方法灵敏度高、重复性强。 相似文献

从12株产麦角固醇(1)的酵母菌中,筛选出的一株1产量较高的酵母菌YN2,并对其产1发酵条件进行了优化.结果表明,最适培养基配方(%)为:酵母粉1,牛肉膏2.5,葡萄糖8,K2HPO4 0.3,MgSO4 0.15.最适培养条件为:培养温度28℃,起始pH6.5,发酵时间72h.在优化的试验条件下,进行摇瓶发酵,1含量可达生物量的2.2%,100ml发酵液中1产量达25.3mg. 相似文献
