Ovarian fibromas and cystadenofibromas are neoplasms that share a similar distinctive tissue component of dense fibrous tissue. We sought to describe the MRI features of these neoplasms and to determine if the fibrous component shows distinctive characteristics. Fourteen patients in whom MR images performed with multicoil and fast-spin-echo images and who subsequently underwent surgery for resection of ovarian fibromas or cystadenofibromas were identified from two institutions. Five patients had ovarian fibromas, and nine patients had fourteen cystadenofibromas. 1.5-T MR studies used T1-weighted spin echo and multiplanar T2-weighted fast-spin-echo images, with fat saturation gadoliniumenhanced fast multiplanar gradient-echo images in seven patients. Studies were reviewed for findings of low (≈ skeletal muscle) signal intensity solid components on T2-weighted images, characteristics of gadolinium enhancement, and associated endometrial findings. Images were obtained ex vivo from three adnexal surgical specimens with an 8-cm field of view and correlated with histology. All five of the fibromas showed predominantly very low signal intensity, similar to skeletal muscle, on T2-weighted images. Two of five fibromas were in patients with endometrial polyps and increased amounts of fluid in the pelvis. Thirteen cystadenofibromas were multicystic masses with bands of very low signal intensity ranging from 2 to 20 mm in the wall of the mass, and one was predominantly solid fibrous tissue. Pathologic correlation with specimen images showed that the low signal intensity material was the subepithelial fibrous component of the Cystadenofibromas. Fibrous components of ovarian fibromas and cystadenofibromas are demonstrable by MR as solid components representing fibrous tissue of very low signal intensity on T2-weighted images.  相似文献

BACKGROUND AND PURPOSE: Rathke's cleft cysts often may be difficult to differentiate from other intrasellar or suprasellar masses on radiologic studies. The purpose of this study was to describe the significance of intracystic nodules, a diagnostic characteristic found in Rathke's cleft cysts, on MR images. METHODS: A retrospective review of MR studies was conducted for 13 patients who, after pathologic analysis, were diagnosed as having Rathke's cleft cyst. These patients underwent unenhanced and contrast-enhanced T1- and T2-weighted axial and coronal spin-echo sequential imaging. The signal intensity and incidence of the intracystic nodules on T1- and T2-weighted images were analyzed. The signal intensity of the nodule was compared with that of white matter and surrounding cyst fluid. The signal intensity of cyst fluid was compared with the intraoperative appearance of the cyst fluid. Biochemical and pathologic analyses of the intracystic nodules were conducted in two cases. RESULTS: An intracystic nodule having high signal intensity on T1-weighted images and low signal intensity on T2-weighted images was observed in 10 (77%) of the cases. At surgery, intracystic nodules were yellow, waxy, solid masses. Pathologic analysis showed this nodule to be a mucin clump. Biochemical analysis of the intracystic nodules showed cholesterol and proteins as the main constituents. In the Rathke's cleft cyst with intracystic nodules, cyst fluid revealed low signal intensity to isointensity relative to the intensity of the nodules on T1-weighted images, and isointensity to high signal intensity on T2-weighted images. Intracystic nodules were clearly visible on T2-weighted images. CONCLUSION: Because cyst fluid of Rathke's cleft cysts shows variable intensities on MR images, the specific diagnosis is often difficult when based on MR signal intensity values alone. The presence of an intracystic nodule with characteristic signal intensities on MR images may be indicative of the diagnosis of Rathke's cleft cyst.  相似文献

OBJECTIVE: The objective of our study was to define the MRI features of tuberculous infection of the wrist. MATERIALS AND METHODS: We present the MRI findings of eight patients with tuberculous infection of the wrist. Spin-echo T1-weighted, gradient-echo T2(*)-weighted, and fast spin-echo T2-weighted sequences were performed for all patients. Gadolinium-enhanced MR images were obtained in seven patients. All images were evaluated for the characteristics of tuberculous infection of the wrist, including the presence of synovial thickening around the joints and tendons, signal intensity of the thickened tenosynovium and synovium on the T2-weighted images, synovial fluid collection in the tendon sheath, small low-signal and nonenhanced foci in the synovial fluid, bone erosion, osteomyelitis, and encasement of the median nerve. RESULTS: The tuberculous infection involved the right (n = 6) and left (n = 2) wrists. All patients had synovial thickening around the flexor and extensor tendons with synovial fluid collection in the tendon sheath. The thickened tenosynovium and synovium revealed low signal intensity on T1-weighted images, intermediate to low signal intensity on T2- and T2(*)-weighted images, and enhancement on contrast-enhanced MR images. The synovial fluid showed intermediate to low signal on T1-weighted images and homogeneous or heterogeneous high signal intensity on T2- and T2(*)-weighted images. Multiple small foci of low signal intensity and nonenhancement scattered in the synovial fluid were present in seven patients. Bone erosion occurred in seven patients, osteomyelitis was seen in six patients, and encasement of the median nerve was found in three patients. CONCLUSION: Characteristic MRI findings of tuberculous infection of the wrist include synovial thickening around the flexor and extensor tendons and synovial fluid collection that contains small low-signal and nonenhanced foci in the tendon sheath. Bone erosion, osteomyelitis, and median nerve encasement are also frequently present. These characteristic manifestations are helpful in diagnosing this disease entity.  相似文献

Magnetic resonance images of cranial bone marrow in 238 patients (246 examinations) less than 25 years old were reviewed to establish normal age-related standards. Bone marrow in the clivus and calvaria had uniformly low signal intensity (grade 1) on T1-weighted images in most infants less than 1 year old. The number of patients with grade 1 marrow decreased rapidly in early childhood, while the number of patients with marrow of low and high signal intensity (grade 2) and uniformly high signal intensity (grade 3) gradually increased with age. A grade 1 marrow was no longer observed in either the clivus or calvaria after age 7. Most patients had a grade 3 marrow by age 15. Because bone marrow in certain pathologic conditions has decreased signal on T1-weighted images and therefore resembles grade 1 or 2 appearances of normal marrow in children, these results may be useful for differentiating normal and abnormal bone marrow signal intensities in infants and children.  相似文献

Magnetic resonance imaging of myositis ossificans: analysis of seven cases   总被引:7,自引:0,他引:7  
Myositis ossificans typically presents as soft tissue swelling with progressive ossification on radiographs. Since magnetic resonance imaging (MRI) is commonly used to evaluate soft tissue masses, we analyzed eight MR examinations in seven patients with myositis ossificans to determine if typical patterns were present. One acute lesion had homogeneous intermediate signal intensity on T1-weighted images and high signal intensity on T2-weighted images. Two subacute lesions had low signal intensity margins with slightly increased signal intensity centers on T1-weighted images and very high signal intensity on T2-weighted images. Five chronic lesions had two different patterns. All five were well-defined with low signal intensity borders. Three had signal intensity patterns characteristic of fat on T1-weighted and T2-weighted images. The other two lesions had intermediate signal intensity on T1-weighted images and slightly increased signal intensity on T2-weighted images.We conclude that typical MR appearances of myositis ossificans do exist. A low signal intensity rim is a common finding. However, these patterns are not unique to myositis ossificans and resemble those that have been reported in other lesions. It is important to be aware of the spectrum of MR findings of myositis ossificans when considering the differential diagnosis of a soft tissue mass.  相似文献

The magnetic resonance (MR) imaging features of Brodie abscess have not yet been fully evaluated. Ten patients with Brodie abscess, eight of long bone and two of vertebra, were studied with MR imaging. Long bone abscess had a characteristic “target” appearance with four layers: (a) a center with low signal intensity on T1-weighted images and high signal intensity on T2-weighted and STIR (short-inversion-time inversion recovery) images, (b) an inner ring isointense to muscle on T1-weighted images and with high signal intensity on T2-weighted and STIR images, (c) an outer ring hypoin-tense on all images, and (d) a peripheral halo hypointense on T1-weighted images. In six of eight cases, a soft-tissue mass was found. The two vertebral abscesses had a less specific appearance, with low signal intensity on T1-weighted images and high signal intensity on T2-weighted and STIR images. Only the peripheral halo was clearly identified in both cases.  相似文献

Objective. To quantify image artifact reduction using a new technique (MARS – metal artifact reduction sequence) in vitro. Design. Coronal T1-weighted MR images were obtained through two metal phantoms (titanium/chromium-cobalt and stainless steel femoral prostheses) immersed in water. Comparison of artifact volume was made with images obtained using conventional and modified (MARS) T1-weighted sequences. Signal intensity values outside a range of ±40% the average signal intensity for water were considered artifact and segmented into low or high signal artifact categories. Considering the arbitrary selection of this threshold value, volumetric calculations of artifact were also evaluated at ±50%, 60%, 70%, and 80% the mean signal for water. Results. Conventional T1-weighted images produced 87% more low signal artifact and 212% more high signal artifact compared with the MARS modified T1-weighted images of the stainless steel prosthesis. Conventional T1-weighted images of the titanium prosthesis produced 84% more low signal artifact and 211% more high signal artifact than the MARS modified sequence. The level of artifact reduction was essentially uniform for the various threshold levels tested and was greatest at ±20% the global signal intensity average for water. Conclusion. The MARS technique reduces the volume of image signal artifact produced by stainless steel and titanium/chromium-cobalt femoral prostheses on T1-weighted spin-echo images in a tissue phantom model. Eight patients with intracranial tuberculomas were studied with CT and magnetic resonance (MR) imaging. Large, ring enhancing, solid lesions on CT showed low intensity on T2-weighted images and intermediate intensity on T1-weighted images. Small lesions, with ring enhancement on CT, showed central bright signal on T2-weighted images with a peripheral low intensity rim surrounded by high intensity edema. The MR imaging features of the tuberculomas were found to be distinct from those of abscesses, metastases, and gliomas.  相似文献

Rathke cleft cyst: MR and biomedical analysis of cyst content   总被引:17,自引:0,他引:17  
PURPOSE: At least one type of Rathke cleft cyst has unique MR findings, specifically, high intensity on T1-weighted images and iso- to low intensity on T2-weighted images relative to white matter. To clarify the influence of cyst content on MR images, we analyzed the cyst content by biomedical methods after surgical removal. METHOD: We studied five patients diagnosed with Rathke cleft cyst, whose MR images showed high intensity on T1-weighted images and iso- to low intensity on T2-weighted images. After surgery, total protein and cholesterol levels were quantified, and correlations of protein and cholesterol content with T1 and T2 signal intensities were performed in vitro. RESULTS: All five cysts had very high concentrations of protein (11,700-26,600 mg/dl, mean 17,940 mg/dl) with nearly no cholesterol (at most 2.0 mg/dl). Along with increases in protein concentration in vitro, the signal intensity of T1-weighted images increased, while that of T2-weighted images decreased. In contrast, the cholesterol concentration sequence influenced the signal intensity of neither T1- nor T2-weighted images. CONCLUSION: The unique MR finding of Rathke cleft cysts--high signal intensity on T1-weighted images and low signal intensity on T2-weighted images--might depend mainly on protein concentration, not on cholesterol.  相似文献

目的 评价MRI诊断颅咽管瘤的价值。方法 回顾性分析 5 2例颅咽管瘤的MRI表现 ,并与病理比较。结果  37例( 71.2 % )的颅咽管瘤位于鞍上。颅咽管瘤囊液的MRI信号在T2 加权像上一般呈较均一的高信号 ,而在T1加权像上则呈低、等、高等多种表现 ,反映了囊液内不同病理成分的存在。结论 直接的多维成像能力及敏感的信号变化 ,使MRI成为诊断颅咽管瘤的有效方法  相似文献

Fifty-seven magnetic resonance (MR) imaging examinations were obtained at 0.5 T in 19 patients before and after percutaneous ethanol injection (PEI) for 23 hepatocellular carcinoma (HCC) lesions less than 3.5 cm in diameter. Seventeen patients also underwent MR imaging 6 months after completion of therapy. In 11 patients, computed tomography was performed before and after treatment. After PEI, fine-needle biopsy specimens were obtained in all cases. Before treatment, HCC lesions had low signal intensity on T1-weighted images in 13 cases, had the same signal intensity as normal liver parenchyma in six, and had high signal intensity in four; all 23 tumors had high signal intensity on T2-weighted images. After treatment and at 6-month follow-up, all 21 lesions that contained no malignant cells at fine-needle biopsy had high signal intensity on T1-weighted images and had low signal intensity on T2-weighted images. The remaining two HCC lesions in which tumor necrosis was not achieved with PEI displayed a different MR pattern, since the residual neoplastic tissue showed no change in signal intensity on either T1- or T2-weighted images. The authors conclude that MR imaging may be useful for evaluating the effectiveness of PEI in achieving tumor regression.  相似文献

OBJECTIVE: To describe the MR imaging features of tuberculous osteomyelitis. DESIGN AND PATIENTS: MR imaging features of 11 patients (14-65 years) with proven extra-spinal tuberculous osteomyelitis were reviewed. Osseous and adjacent soft-tissue changes were analyzed. RESULTS: On the basis of the signal intensity characteristics compared with the normal marrow fat, two kinds of lesions were observed: (a) predominantly intermediate to low signal intensity lesions on T2-weighted images with low signal intensity on T1-weighted images, and (b) lesions which had a discrete peripheral zone of marginally higher signal intensity than the center on T1-weighted images and surrounding edema and lower signal intensity than the fatty bone marrow with variable signal intensity on T2-weighted images. Soft-tissue abscesses and marrow edema were each noted in eight cases. Soft-tissue edema was noted in most cases. CONCLUSION: An osseous lesion with intermediate to low signal intensity on T2-weighted images and associated soft-tissue abscess may be suggestive of tuberculous osteomyelitis. Lesions with a rim of mildly increased signal intensity on T1-weighted images, a non-specific indicator of an infective process, may also be seen.  相似文献

A case of chronic osteomyelitis in the occipital bone associated with underlying chronic epidural abscess was diagnosed by skull radiography, CT, and MR. Skull radiography showed diffuse sclerosis of the occipital bone and dural calcifications. Computed tomography demonstrated thickened sclerotic changes of the osteomyelitic area, dural calcifications, and epidural fluid collection. The area of chronic osteomyelitis displayed, essentially, no signal intensity in the T1-weighted and T2-weighted MR images. The chronic epidural abscess had heterogeneous signal intensity on T1-weighted images and good enhancement after Gd-diethylenetriamine pentaacetic acid administration. On T2-weighted images the abscess had relatively homogeneous high signal intensity.  相似文献

OBJECTIVE: We evaluated the ability of superparamagnetic iron oxide (SPIO)-enhanced MRI to differentiate solid metastatic tumors and nonsolid benign lesions by clarifying the characteristic signal-intensity pattern of each lesion on SPIO-enhanced T2-weighted and heavily T1-weighted gradient-echo images. MATERIALS AND METHODS: SPIO-enhanced MRI was performed using a 1.5-T system in 33 consecutive patients without cirrhosis who had 81 focal hepatic lesions (42 cysts, 13 hemangiomas, 26 metastatic tumors). The relative signal intensity of lesions on SPIO-enhanced heavily T1- and T2-weighted gradient-echo images was classified into one of the following three categories: high intensity, isointensity, or low intensity relative to the surrounding liver parenchyma. The diagnostic accuracy for differentiating solid metastatic tumors from nonsolid benign lesions (cysts or hemangiomas) was determined. RESULTS: A combination of the relative signal intensity of the lesion on T2- and heavily T1-weighted gradient-echo images could be classified into the following five categories: high intensity and high intensity (category 1), high intensity and isointensity (category 2), high intensity and low intensity (category 3), isointensity and isointensity (category 4), and isointensity and low intensity (category 5). According to these categories, category 1 contained two hemangiomas, category 2 had 11 hemangiomas, category 3 had 25 metastatic tumors and two cysts, category 4 had three cysts, and category 5 had 37 cysts and one metastatic tumor. When a tumor with a relative signal intensity of categories 1 or 2 was considered to be a hemangioma (category 3 metastatic tumors and categories 4 and 5 cysts), diagnostic accuracy for characterizing such hepatic lesions was 96% (78/81). CONCLUSION: When evaluating metastatic liver tumors on SPIO-enhanced MRI, we recommend that heavily T1- and T2-weighted gradient-echo images be obtained with our parameters to exclude hemangiomas or cysts.  相似文献

目的:探讨阔韧带平滑肌瘤的MR表现及诊断价值.方法:回顾性分析13例经手术和病理证实为阔韧带平滑肌瘤的MR表现.结果:13例中肿块位于子宫右侧4例,左侧8例,两侧1例.肿块呈圆形、类圆形10例,不规则形或分叶状3例.肿块大于5 cm占84.6%(11/13).T1WI肿块均呈等或略低信号;T2WI为中等信号,其中11例肿块中见不均匀斑点状、梭条状高信号影,2例见大片状液化坏死灶.注射钆喷替酸葡甲胺(Gd-DTPA)后扫描肿块实质部分明显强化,显示包膜完整,与子宫等周围脏器分界清晰10例,包膜不清,与子宫有粘连且分界不清3例;包膜不强化且显示更清晰.结论:阔韧带平滑肌瘤的MR表现具有一定的特征性:盆腔内、子宫外大而有完整包膜的实性或实性为主的肿块,T1WI呈等或略低信号,T2WI呈中等信号,其内常可见不均匀斑点状、梭条状高信号影,增强扫描肿瘤强化明显.MR具有良好的软组织分辨率、多方位成像的特点,有利于肿瘤结构、成份与包膜显示,对明确诊断有肯定的价值.  相似文献

We report a case of parasellar dermoid tumor with intra-tumoral hemorrhage. It is rare for a dermoid tumor that hemorrhage was detected as high attenuation on the initial CT. In the present case, the tumor content included a little fat component and mostly cholesterin-rich fluid which resulted in extremely low signal intensity on T2-weighted and high signal on T1-weighted MR images. In addition to this, hemosiderin accumulation in the tumor could be the reason for low signal intensity on T2-weighted images. Background and purpose: Although imaging features of cerebellar pilocytic astrocytoma and medulloblastoma have been described in many texts, original comparisons of magnetic resonance intensity between these two tumours are limited. In the present study the results of magnetic resonance imaging (MRI) were reviewed, focusing especially on the signal intensity of the solid portion of these neoplasms. Methods: MR images of ten cerebellar pilocytic astrocytomas and ten medulloblastomas were reviewed. The signal intensities of the solid components were graded on a scale of 1 to 5, with higher scores indicating a signal intensity closer to that of water. The degree of enhancement, tumour cysts and peripheral oedema were evaluated on MR images. When the solid portion was heterogeneous (i.e. mixed signal intensity or degree of enhancement), the dominant area was selected for evaluation. Results: On T2-weighted images, the signal intensity of the solid portion was equal to that of cerebrospinal fluid (CSF) in 50% of pilocytic astrocytomas. No medulloblastomas showed such hyperintensity. Most medulloblastomas (80%) were isointense to grey matter. On T1-weighted images, the signal intensity varied widely in pilocytic astrocytomas; however, all medulloblastomas were iso- or hypointense to grey matter. The MR enhancement pattern, cystic component and peripheral oedema all varied in both tumour types and no specific features were identified. Conclusion: A signal intensity of the solid portion isointense to CSF on T2-weighted images was characteristic of cerebellar pilocytic astrocytomas; this was not observed in medulloblastomas. Attention to T2-weighted imaging of the solid portions of a tumour is easy and helpful in differentiating between cerebellar pilocytic astrocytoma and medulloblastoma.  相似文献

BACKGROUND AND PURPOSE: MR imaging can now be used safely in extremely preterm infants. The aim of this study was to compare the MR imaging appearance of the immature brain with neuropathologic findings at postmortem examination. METHODS: Seven extremely sick preterm infants, born at a median of 24 weeks' gestation, were studied using T1- and T2-weighted MR sequences. Infants died at a median of 3 days after initial MR imaging, and postmortem examinations were carried out. RESULTS: The cortex and germinal matrix were seen as areas of low signal intensity on T2-weighted images, which corresponded to their highly cellular histologic appearance. The periventricular and subcortical layers of white matter had a high signal intensity, corresponding to high fiber and relatively low cellular density; the intermediate layer of low signal intensity corresponded to a dense band of migrating cells. Regions of acute hemorrhage were seen as low signal intensity and regions of infarction as high signal intensity on T2-weighted images. One infant with mild periventricular leukomalacia had some low signal intensity on T1-weighted images, but no focal changes on T2-weighted images. Regions of neuronal mineralization, seen in association with infarction and capillary proliferation, within the basal ganglia and thalami were characterized by very low signal intensity on T2-weighted images and by very high signal intensity on T1-weighted images. There were no imaging abnormalities detected in regions with more subtle histologic abnormalities, such as increased glial or apoptotic cells. CONCLUSION: MR imaging can be used to observe normal developing brain anatomy in extremely premature infants; it can detect areas of hemorrhage and infarction within the developing brain, but conventional MR imaging may not detect more subtle histologic abnormalities.  相似文献

We retrospectively reviewed 330 T1-weighted sagittal images, 80 T2-weighted sagittal images, and 83 gadopentetate-dimeglumine-enhanced scans of normal adults to determine the MR appearance of the normal adult clivus. MR images of 21 patients with an abnormal clivus (19 with tumor invasion and two with marrow reconversion) were also evaluated restrospectively and compared with those of the control group to assess MR features distinguishing the two groups. Our study revealed that a normal adult clivus consisted of low- and high-intensity portions mixed in various proportions on T1-weighted images. The low-intensity portion was isointense or hyperintense relative to the pons and always contained foci of bright signal intensity. The low-intensity tumor of a pathologic clivus tended to be hypointense relative to the pons (17/19), and was completely devoid of foci of bright signal intensity. The normal adult clivus was approximately isointense relative to the pons on T2-weighted images. Clival tumors were grossly hyperintense relative to the pons on T2-weighted images in 11 of 17 patients. In the remaining six patients, either a portion of or the entire lesion was isointense relative to the pons and, therefore, was not detectable on T2-weighted images. A normal adult clivus can enhance to some degree (19/83). Clival tumors were found to enhance intensely. A clivus of very low signal intensity (signal void) on T1- or T2-weighted images was always abnormal. The clivus with marrow reconversion was uniformly hypointense relative to the pons on T1-weighted images and isointense relative to normal marrow on T2-weighted images.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

The magnetic resonance (MR) findings in three patients with malignant pleural mesothelioma are described. All patients had a circumferential pleural mass surrounding the lung on the affected side. These tumors had a signal of intermediate intensity on T1-weighted images. The T2-weighted images showed a slight increase in signal intensity of the mass, with focal areas of very high signal intensity due to pleural fluid. Adenopathy was demonstrated by CT and MR in two patients. The extent of the tumor and its effects on adjacent structures were well appreciated on the coronal MR images.  相似文献
