目的:观察哮喘儿童变应原免疫治疗(AIT)前后血浆内皮素-1(ET-1)与血清IgE水平变化,以探讨二者在哮喘变应原免疫治疗中的意义和关系.方法:应用放射免疫分析检测46例哮喘儿童变应原免疫治疗前后血浆ET-1与血清IgE水平,并与32例正常对照组进行比较.结果:治疗有效组(35例)治疗前血浆ET-1与血清IgE水平分别为(59.1±11.7)pg/ml和(11.1±3.2)IU/ml,均显著高于正常对照组(P<0.01),AIT治疗后显著降低,分别为(52.7±11.1)和(8.2±2.4)IU/ml(P<0.01).治疗无效组(11例)治疗前后ET与IgE水平无显著性变化.结论:治疗后血浆ET-1与血清IgE水平降低可能是哮喘患儿AIT治疗有效的重要机理.  相似文献

目的 分析佛山乐从地区家具工人变应性鼻炎变应原阳性分布情况.方法 收集2014年1月至2016年6月就诊于本院五官科的变应性鼻炎患者500例为观察组,职业均为家具工人;另选取同期本院就诊的非家具工人变应性鼻炎患者500例为对照组.进行患者血清总IgE水平与血清特异性变应原检测及变应原皮肤点刺试验,探讨两组患者吸入性变应原的阳性分布情况,分析两种变应原检测方法的一致性.结果 观察组和对照组患者血清总IgE阳性率分别为95.6%(478/500)、92.8% (464/500),组间比较差异无统计学意义(x2=3.59,P>0.05).血清特异性变应原检测和变应原皮肤点刺试验结果显示,两组患者螨类(粉尘螨、屋尘螨、热带螨)变应原阳性率均最高,且观察组螨类、德国小蠊、交链孢菌属、多主枝孢菌属、树木、杂草、霉菌变应原阳性率均显著高于对照组(均P<0.05).两种变应原检测方法的一致性较好(均Kappa值>0.6).结论 相对于非家具工人,家具工人变应性鼻炎的螨类等多种变应原阳性率均升高.  相似文献

目的探讨呼吸道变态反应性疾病儿童特异性免疫球蛋白IgE(SIgE)在各年龄分布及其与总IgE(TIgE)的关系。方法采用荧光酶联免疫检测系统,对73例哮喘、过敏性咳嗽患儿,按年龄分为3组,进行14项SIgE检测,包括食人组和吸入组,同时检测TIgE。结果①患儿TIgE检出率,三组检出率没有显著差异。②患儿SIgE检出率有随年龄增加而增高的趋势。③患儿〈3岁组以食物多价Fmea以及Dx最高,各为5例;而3—6岁组以Fmea最高,为15例;〉6岁组Mxl最高14例。④SIgE和TIgE检测结果有明显相关,但两者检出率无明显差异。如以SIgE阳性和(或)TIgE阳性合并统计,患儿IgE阳性检出率为95.89%。结论①患儿〈3岁组的SIgE和TIgE检出率并不低。②患儿〈3岁婴幼儿最常见的变应原为食入性Fmea、F2、F4。③3~6岁学龄前儿童仍以食人性变应原为主,最高为Fmea,其次为F2,但吸入性变应原开始增多,Dx、Mxl以及wx;④〉6岁组学龄儿童则以室内吸入性变应原为主,Mxl最高,其次为El/E5、Dx,还有食入性Fmea。⑤SIgE与TIgE有很好的相关性,SIgE联合TIgE检测能够提高检出率,更有效地诊断儿童呼吸道变态反应疾病。  相似文献

目的 研究分析233例慢性荨麻疹患者过敏原分布及其临床意义.方法 选取2015年7月至2016年9月在我院就诊的慢性荨麻疹患儿233例,根据过敏原分为吸入组和食入组,采用免疫印迹法对患儿血清进行特异性IgE定性检测,观察分析两组患儿过敏原结果和对单一或合并多种过敏原的结果.结果 233例慢性荨麻疹患儿中有140例血清IgE检测为阳性,占比为60.01%,其中吸入组阳性例数为115例,占比为(49.35%),食入组阳性例数为60例,占比为25.76%,总共有35例为两组共同阳性,吸入组过敏原阳性率高于食入组,两组差异具有统计学意义(P<0.05);吸入组过敏原中尘螨组合的占比最高(15.02%),艾蒿和律草的占比最低(0.85%);食入组过敏原中蟹和虾的占比最高(4.72%),鸡蛋白和黄豆的占比最低(1.29%);140例阳性患儿中,对1种过敏原呈阳性的患儿55例,对2种过敏呈阳性的患儿34例,对3种及以上的过敏原呈阳性的患儿51例,单种过敏原阳性患儿例数明显多于多种过敏原阳性患儿例数.结论 对慢性荨麻疹患儿进行过敏原检测,为治疗和预防提供有效依据,具有重要的临床意义.  相似文献

目的 :探讨青霉素类过敏反应与特异性IgE抗体的关系。方法 :采用放射过敏原吸附试验 (RAST)测定 3 97例青霉素过敏病人血清中 8种抗原决定簇(BPO PLL、PVO PLL、APO PLL、AXO PLL、BPA PLL、PVA PLL、APA PLL、AXA PLL)特异性IgE抗体。结果 :3 97例过敏病人中 ,特异性IgE抗体阳性率为 5 8.9% (2 3 4例 )。其中 ,男性组抗体阳性率显著高于女性组 (P <0 .0 5 )。在所检测的 8种特异性IgE抗体中 ,BPA IgE抗体阳性率 2 5 .44% (1 0 1例 )最高 ,PVA IgE抗体 2 5 .1 9% (1 0 0例 )次之 ,而APA IgE抗体 9.82 % (3 9…  相似文献

目的研究屋尘螨(Dermatophagoides pteronyssinus)引起中国人群过敏的特异性变应原组分,为我国屋尘螨变应原的标准化提供依据。方法采用常规方法制备屋尘螨粗浸液,以48例螨过敏病人血清为探针进行Westen blot检测粗浸液中的致敏组分,并利用BandScan5.0软件分析血清阳性反应带(致敏带)结合特异性IgE的相对含量。结果免疫印迹杂交检测到屋尘螨的10条致敏带,相对分子质量(Mr)依次为103、99、68、62、57、37、33、29、25、15×103,其中致敏带1、2、4、5、6、7、8的阳性反应率超过95%;致敏带10的阳性反应率为47.9%,其结合特异性IgE的能力最强。结论屋尘螨的主要致敏组分既包括小Mr的变应原,也包括大Mr的变应原,现有的屋尘螨变应原的国际标准品不能普遍应用于中国人群,必须开发适合我国人群的尘螨变应原疫苗,本研究为开发适合我国人群的尘螨标准化疫苗提供理论依据。  相似文献

广州地区呼吸道疾病患儿16种常见变应原检测分析   总被引:1,自引:0,他引:1  
目的 了解广州地区呼吸道疾病患儿对16种常见变应原的过敏情况,同时探讨不同年龄组之间的变应原阳性率差异,并比较致敏程度.方法 选择2007年8月至2008年3月广州医学院第一附属医院呼吸科门诊和儿科门诊初诊为支气管哮喘、过敏性鼻炎、呼吸道感染等患儿320例为研究对象,其中婴幼儿组(≤3岁)214例,儿童组(>3岁)106例.采用德国FOOKE ALLERG-O-LIQ-SYSTEM变应原检测系统,通过免疫捕获法检测血清中常见的16种变应原特异性免疫球蛋白E(SIgE)抗体.结果 在所有患儿中,其中261例(81.56%)SIgE呈阳性.59例(18.44%)呈阴性.16种变应原SIgE阳性率为屋尘螨(36.88%)、粉尘螨(35.31%)、热带螨(24.06%)、狗毛(6.56%)、猫毛(8.75%)、德国小蠊(16.56%)、蜜蜂毒素(0.94%)、屋尘(45.63%)、全蛋(45.94%)、牛奶(46.25%)、小麦面粉(6.88%)、玉米粉(1.25%)、花生(9.06%)、大豆(5.94%)、螃蟹(2.81%)、虾(4.38%).婴幼儿组SIgE的阳性率为81.78%,以食物性变应原(牛奶、全蛋)为主;儿童组的阳性率为81.13%,以吸人性变应原(尘螨)为主(X2=0.02,P=0.89).儿童组尘螨的过敏阳性率均高于婴幼儿组(P<0.05),且儿童组屋尘螨和粉尘螨的过敏程度多在3级以上.婴幼儿组各级阳性率比较平均.两组对热带螨的过敏程度均较低,多在3级以下.婴幼儿组牛奶和全蛋过敏阳性率高于儿童组(P<0.05),两组的过敏程度均在3级以下.结论 儿童组与婴幼儿组对常见变应原的总阳性率差异无统计学意义,只是对不同变应原阳性率及过敏程度的差异.儿童组主要是对吸人性变应原产生过敏,婴幼儿组主要是对食物性变应原产生过敏,应以不同年龄段来分析儿童变应原检测结果.  相似文献

目的了解食物过敏儿童常见食物变应原的种类及其构成比、一般体液免疫及细胞免疫状况,为临床采取合理的诊疗措施提供科学依据。方法对246例食物过敏儿童和192例健康儿童血清的14种食物过敏原特异性IgE、血清免疫球蛋白总IgE、IgG、IgA、IgM、补体C3、C4进行检测,并进行外周血淋巴细胞亚群流式细胞术检测,对两组结果进行统计学分析。结果变应原阳性率前5位的分别是牛奶(21.11%),牛肉(17.20%),全蛋(16.49%),黄豆(9.73%),芝士8.77%。食物过敏组患儿总IgE水平、CD19+B淋巴细胞水平明显高于健康对照组(P〈0.05),两组间IgG、IgA、IgM、补体C3、C4水平差异无统计学意义(P〉0.05),各类型T淋巴细胞(包括CD3+T淋巴细胞、CD3+CD4+辅助T细胞、CD3+CD8+抑制T细胞、CD3-CD16+CD56+NK细胞水平差异无统计学意义(P〉0.05)。结论广州地区儿童食物过敏的变应原中最常见的是牛奶、牛肉和全蛋,血清总IgE水平、外周血CD19+B淋巴细胞水平等反映体液免疫功能指标的检测在儿童食物过敏诊断和治疗中的应用值得进一步研究。  相似文献

This study was aimed to investigate the sensitization pattern to a range of common allergens in young Singaporean children. A cross-sectional study involving 75 children aged below 3 years was carried out. They presented between December 1995 and April 2000 with symptoms of asthma, rhinitis, eczema, or food allergy. Their levels of allergen-specific serum IgE to a panel of foods (egg white, milk, soy protein, shrimp, wheat and peanut), pet dander, dust mites and cockroaches were measured with Pharmacia CAP System radioallergosorbent test kits. Serum IgE levels greater than 0.35 kU/l represented a positive result. Four children could not be tested with the complete panel because of insufficient serum. The prevalence of sensitization was highest for cow's milk (45.9%) followed by egg white (38.7%), dust mites Dermatophagoides pteronyssinus (31.4%) and Blomia tropicalis (25.5%). Sensitization to ingested allergens was significantly more prevalent in children aged 1 year or younger than in the older children (70.4% of those below 1 year, and 50% of those aged 1-3 years; p < 0.02). Sensitization to inhaled allergens, such as dust mites, was more likely to manifest as respiratory symptoms (allergic rhinitis and asthma), while ingested allergens were associated with gastrointestinal symptoms and eczema (p < 0.001). It was concluded that infants and young children are at high risk of sensitization to common environmental substances. Allergen avoidance is therefore important even in the very young. The prevalence of sensitization to food allergens is higher compared to inhalant allergens in young children.  相似文献

The nonstinging house ant, Monomorium pharaonis (pharaoh ant), was recently identified as a cause of respiratory allergy. This study was performed to evaluate the extent of sensitization to pharaoh ant, and its clinical significance in asthmatic patients. We carried out skin prick tests in 318 patients with asthma. Specific IgE (sIgE) to pharaoh ant was measured by ELISA, and cross-reactivity was evaluated by ELISA inhibition tests. Bronchial provocation testing was performed using pharaoh ant extracts. Fifty-eight (18.2%) of 318 patients showed positive skin responses to pharaoh ant, and 25 (7.9%) had an isolated response to pharaoh ant. Positive skin responses to pharaoh ant were significantly higher among patients with non-atopic asthma than among those with atopic asthma (26.0% vs. 14.9%, p<0.05). There was significant correlation between sIgE level and skin responses to pharaoh ant (rho=0.552, p<0.001). The ELISA inhibition tests indicated that pharaoh ant allergens had various pattern of cross-reactivity to house dust mites and cockroaches. Bronchial provocation tests to pharaoh ant were conducted for 9 patients, and eight showed typical asthmatic reactions. In conclusion, pharaoh ant is an important source of aeroallergens, and it should be included in the skin test battery for screening the causative allergens in patients with asthma.  相似文献

BACKGROUND: The diagnosis of allergic disease is performed by skin prick tests (SPT) or through the demonstration of specific IgE in a blood sample via an in vitro test. The measurement of IgE concentration against allergens provides critical information in clinical allergy. Standardized and reproducible methods contribute to the quality of diagnosis and treatment of allergic disease. METHODS: In this study we evaluated the performance of a new specific IgE method, developed by ALK-Abellò for Bayer Diagnostics to run on their ADVIA Centaur immunoassay system. One hundred and fifty-one children with allergic diseases (both food and inhalant allergies) were tested for specific IgE (sIgE) via SPT and in vitro tests (UniCAP system, Pharmacia, and ADVIA Centaur immunoassay system, Bayer Diagnostics) and the test results were correlated with the clinical data. RESULTS: Statistical analysis revealed no significant difference between the two in vitro tests compared with clinical history. The sensitivities and specificities are similar, but the UniCAP system method has higher sensitivity. In the children with cow's milk allergy, the UniCAP system has sensitivity of 91% and specificity of 70%; the ADVIA Centaur immunoassay has sensitivity of 82% and specificity of 74%. In hen's egg allergy, UniCAP system has 94% sensitivity and 64% specificity, and the ADVIA Centaur system has 88% sensitivity and 52% specificity. In inhalant allergies, the two methods show statistically similar performances for both grass pollen allergies (UniCAP sensitivity 100%, specificity 73%; ADVIA Centaur sensitivity 95%, specificity 79%) and in the dust mites allergies (UniCAP sensitivity 91%, specificity 62%; ADVIA Centaur sensitivity 86%, specificity 64%). In cat allergies, the systems showed equivalent results (UniCAP sensitivity 100%, specificity 71%; ADVIA Centaur sensitivity 100%, specificity 70%). Using the UniCAP system, the geometric mean of sIgE values in children with clinical allergy is significantly higher than in sensitized ones. The ADVIA Centaur system shows a similar trend with the exclusion of cow's milk and Dermatophagoides farinae allergens. With this last method the mean value of sIgE is higher in sensitized than in symptomatic children. CONCLUSION: The new ADVIA Centaur method compares favorably with the results obtained on the UniCAP system. If other studies continue to confirm this data, then the advantages are numerous: the use of only a small quantity of serum (25 micro l per allergen), rapid turnaround time, minimal hands-on time, and no interference from IgG.  相似文献

The prevalence of asthma among schoolchildren in Costa Rica is very high -at the level of 20-30% - and the reason is still unknown. A group of children from our previous epidemiologic study was randomly selected in order to establish the relation between asthma symptoms and allergy sensitization to common allergens. Serum samples from ciiildren with and without asthma were analyzed for the presence of IgE antibodies to 36 different allergens, for the presence of IgE antibodies to a pool of 10 common allergens, and for total serum IgE. The most prevalent IgE antibodies were those to mite, cockroach, dog, and house-dust allergens with MAST pipettes for the serologic measurements. Positive reactions to house dust, mite, cat, and the two molds (Aiternaria and Ciadosporium), and food allergens such as egg white, peanut, and shellfish were significantly more prevalent among the asthmatics than the nonasthmatics. Sensitization was equally prevalent at different ages, but the house-dust, mite, cat, dog, cockroach, Aiternaria, and egg-white allergens had sensitized boys more often than girls (P<0.01). The result of the analysis of IgE antibodies to a pool of 10 common allergens by Phadiatop supported the MAST pipette results, showing allergen sensitization in 57.7% of the asthmatic children and 42.3% in the nonasthmatic group. The concentration of IgE was significantly higher among the asthmatic children (372.2 kU/1) than among the nonasthmatic children (249.1 kU/1) (P < 0.00001). Parasitic infestations were not examined i n this study, but in most of Costa Rica these have largely been eliminated and could not explain the high total IgE levels. Our data indicate that the very high prevalence of bronchial asthma in Costa Rican schoolchildren can be related to sensitization, especially to airborne indoor allergens such as those of mites, cockroaches, and dogs.  相似文献

BACKGROUND: The atopy patch test (APT) was proposed to evaluate IgE-mediated sensitizations in patients with atopic eczema (AE). OBJECTIVE: The prevalence and agreement with clinical history and specific IgE (sIgE) of positive APT reactions was investigated in six European countries using a standardized method. METHODS: A total of 314 patients with AE in remission were tested in 12 study centers on clinically uninvolved, non-abraded back skin with 200 index of reactivity (IR)/g of house dust mite Dermatophagoides pteronyssinus, cat dander, grass, and birch pollen allergen extracts with defined major allergen contents in petrolatum. Extracts of egg white, celery and wheat flour with defined protein content were also patch tested. APT values were evaluated at 24, 48, and 72 h according to the European Task Force on Atopic Dermatitis (ETFAD) guidelines. In addition, skin-prick test (SPT) and sIgE and a detailed history on allergen-induced eczema flares were obtained. RESULTS: Previous eczema flares, after contact with specific allergens, were reported in 1% (celery) to 34% (D. pteronyssinus) of patients. The frequency of clear-cut positive APT reactions ranged from 39% with D. pteronyssinus to 9% with celery. All ETFAD intensities occured after 48 and 72 h. Positive SPT (16-57%) and elevated sIgE (19-59%) results were more frequent. Clear-cut positive APT with all SPT and sIgE testing negative was seen in 7% of the patients, whereas a positive APT without SPT or sIgE for the respective allergen was seen in 17% of the patients. APT, SPT and sIgE results showed significant agreement with history for grass pollen and egg white (two-sided Pr > /Z/ < or = 0.01). In addition, SPT and sIgE showed significant agreement with history for the other aeroallergens. With regard to clinical history, the APT had a higher specificity (64-91% depending on the allergen) than SPT (50-85%) or sIgE (52-85%). Positive APT were associated with longer duration of eczema flares and showed regional differences. In 10 non-atopic controls, no positive APT reaction was seen. CONCLUSION: Aeroallergens and food allergens are able to elicit eczematous skin reactions after epicutaneous application. As no gold standard for aeroallergen provocation in AE exists, the relevance of aeroallergens for AE flares may be evaluated by APT in addition to SPT and sIgE. The data may contribute to the international standardization of the APT.  相似文献

The aim of this study was to investigate the clinical characteristics of children who showed sensitization to any type of seafood and to classify the 10 seafood allergens based on IgE reactivities by a cluster analysis. In children with bronchial asthma (BA) and/or atopic dermatitis (AD), we defined the 'seafood' group as 23 patients having any type of seafood specific IgE antibody (CAP system). We analyzed the clinical features, the serum total IgE and each allergen specific IgE level. In addition, ten seafood allergens were also classified by a cluster analysis. Three patients revealed immediate hypersensitivity to some seafood. The frequency of patients with AD and the total IgE in the seafood group were high and the patients in this group were tend to be sensitized to multiple allergens. Seafood allergens were classified into 4 groups, 1) salmon, sardine, horse mackerel and mackerel, 2) cod and tuna, 3) octopus and squid, and 4) crab and shrimp, by a cluster analysis. These findings corresponded to the biological classification and the classification by the reported common allergens among various types of seafood. Based on our findings, this classification is therefore considered to be useful when selecting allergens to screen for sensitization to seafood.  相似文献

目的 回顾性分析南京医科大学附属儿童医院过敏性紫癜(Henoch-Schonlein purpura,HSP)患儿的过敏原检测情况,为临床诊治提供参考。方法 采用德国MEDIWISS过敏原体外检测系统,检测本院最近四年内接诊的1026例HSP患儿的血清过敏原特异性免疫球蛋白E(specific immunoglobulin E,sIgE)的含量。结果 432例HSP患儿血清sIgE检出为阳性,阳性率为42.11%(432/1026),该432例HSP患儿血清过敏原sIgE分布以1~2级为主,学龄组患儿血清过敏原sIgE阳性率占总人数的65.74%(284/432),远高于婴幼儿组(27例)和学龄前组(121例)(F值分别为7.71和4.05,P值均<0.05)。结论 患儿体内sIgE水平不同程度的增高对HSP的诊治具有一定意义。控制患儿与过敏原接触是防治HSP的关键。  相似文献

In a representative cohort of 55 asthmatic schoolchildren the progress of the allergy per se was followed up prospectively for 8 years. Judged by clinical data, skin prick tests and RAST, a large majority of the children retained their allergies to pollen and animal danders. These allergens were predominant, whereas allergy to mites and moulds was less frequent. Serum IgE levels showed a strong tendency to remain high. Positive RAST and skin prick tests were also found in a substantial number of children with normal serum IgE concentrations.  相似文献

Background Better knowledge of the accuracy of a skin prick test (SPT) and specific IgE (sIgE) levels to egg allergens would help to identify persistent egg‐allergic children, avoiding unnecessary risky challenges. This study was designed to assess the accuracy of a SPT and sIgE levels to egg allergens in order to determine persistent egg allergy in IgE‐mediated allergic children after an egg‐free diet. Methods Children below 16 years were prospectively and consecutively recruited. Inclusion criteria were: allergy to egg proteins (children with a positive clinical case of IgE‐mediated egg allergy and a positive SPT to egg allergens and/or positive sIgE levels), and strict egg avoidance diet followed for at least 6 months. Clinical histories were recorded and all patients underwent SPTs, sIgE levels to egg allergens and the gold standard ‐a double‐blind placebo‐controlled egg challenge (DBPCFG). DBPCFG was interpreted without knowledge of the results of the other tests and vice‐versa. A SPT and sIgE levels' ROC curves analysis was performed to compare the diagnostic performance of the different tests. Results Finally, 157 children were included in the study. One hundred out of these 157 children (63.7%) had a positive oral challenge. Ninety‐six were male (61%), and the median age was 2.5 years. One hundred and three (66.9%) had atopic dermatitis. A 7 mm egg white prick test had a positive likelihood ratio (+LR) of 6.7, and a level of 1.3 KU/L egg white‐sIgE had a +LR of 5.1. A 7 mm egg white SPT had a positive predictive value of 92.3% (95% CI 85.1–99.5), and for a 9 mm egg white SPT this value was 95.6% (95% CI 87.3–100.0). For egg white‐sIgE, 1.5 KU/L had a positive predictive value of 90.4% (95% CI 82.4–98.4) and for 25 KU/L it was 100.0% (95% CI 100.0–100.0). SPTs with ovotransferrin and lysozyme showed the lowest accuracy, followed by yolk and ovalbumin SPTs. Conclusion This study is the first to evaluate both tests (SPT and sIgE levels) and all egg allergens to determine the persistence of egg allergy in IgE‐mediated allergic children. Measuring the SPT and sIgE levels is useful to predict persistent allergy in these children, especially with the egg white complete extract. An oral challenge should not be performed in egg allergic paediatric patients with either an egg white prick test above 7 mm or a white egg‐sIgE determination above 1.3 KU/L, because there is a 90% probability of remaining allergic.  相似文献
