Fracture-dislocations of all five carpometacarpal joints: a case report   总被引:1,自引:0,他引:1  
Injuries to all five carpometacarpal joints are rare. Only four cases have been reported in the English literature. A case of fracture-dislocations of all five carpometacarpal joints is presented. An excellent result has been obtained 2 years after open reduction and internal fixation. This case is the only one in the English literature so treated and with prolonged follow-up.  相似文献

Simultaneous subluxation of the metacarpophalangeal joints of all four fingers is rare, and we have found no cases in the literature. A 45-year-old construction worker fell and dislocated the metacarpophalangeal joints of all the fingers of his left hand. Closed reduction by manipulation was successfully accomplished, and the patient returned to work within 5 months of his accident. He had a full range of motion 1 year later.  相似文献

Twenty patients who had a dislocation of one or all of the medial four carpometacarpal joints were followed for an average of 6.5 years (range, 1.5 to 20.5 years). Fifteen patients were treated with open reduction and internal fixation during the first three weeks after injury and the long-term result was excellent in thirteen of them. Three of the four unsatisfactory results were in patients who had injuries to the normally rigid second and third carpometacarpal joints or had a concomitant ulnar-nerve injury.  相似文献

A case of multiple carpometacarpal joints volar dislocation in a young man of 25 years of age is reported. He presented with pain and deformity of his left hand immediately after road traffic accident. He was unable to move his left hand. X-rays of the affected hand showed volar dislocation of all four medial carpometacarpal joints. Dislocated joints were reduced, fixed with K-wires and immobilized in slab. K-wires were removed after 8 weeks. Hand function is satisfactory on the follow-up.  相似文献

From 1997 through 2005, 118 patients aged from seven through sixteen years with unstable dysplastic hip joints (126 hip joints) were operated upon by an acetabular transposition technique after triple pelvic osteotomy. The dynamics of principal roentgenological values were studied. Based on CT and 3D reconstructions a blood supply in the hip joint region and the birth canal measurements were evaluated. In all patients a satisfactory clinical result has been obtained. All the patients have attained a social and professional adaptation. Neither blood supply disorders around the hip joint nor affections of birth-canal were noted.  相似文献

The results of a new subcapital shortening osteotomy for correction of metacarpophalangeal joint deformity in patients with rheumatoid arthritis of the hands are presented. Seven patients (16 joints) were followed up for a mean of 33.5 months. The mean shortening of the metacarpal bone was 4.6 mm (range, 4-8 mm), and seven joints had additional intrinsic release. Only four (25%) joints held the correction of the deformity; all other joints had recurrence of palmar subluxation with or without additional ulnar drift. The range of motion of the joints with preserved correction after surgery was 80 degrees compared with 28 degrees of the joints with recurrent deformity. The possible mechanism of failure was analyzed. The results of the current series suggest that subcapital shortening osteotomy may not be indicated for treatment of severe metacarpophalangeal joint deformity in patients with rheumatoid arthritis.  相似文献

Owing to the limited blood supply in the midfoot, multiple arthrodesis attempts in midfood joints may result in severe osteonecrosis, leading to a difficult scenario for bone reconstruction. This article describes the use of fibula flaps (two free and one pedicled flap) to reconstruct bone defects in three cases of persistent nonunion in midfoot joints. Before admission, all patients (aged 32–56 years old) had undergone multiple arthrodesis attempts (range 3–4) aimed at treating joint diseases associated with flatfoot or Müller–Weiss syndrome. All inserted flaps were stable, and bone fusion was confirmed by X‐ray examination at month 4. After the follow‐up period (range 1–4 years), all patients showed normal, painless gait. No foot deformities or further complications were observed. Our report suggests that fibula flap transfer may be a good alternative for treating persistent nonunion in midfoot joints involving bone loss, and may prevent the osteonecrosis associated with limited blood supply. © 2016 Wiley Periodicals, Inc. SUMMARY: Since arthroplasty, prosthetic replacement and non-vascularised articulation autografting do not normally produce very satisfactory results for ankylosis of metacarpophalangeal and interphalangeal joints, the authors performed reconstruction of phalangeal articulations of the hand using vascularised phalangeal articulations of the foot in 11 patients with ankylosis of the metacarpophalangeal and interphalangeal joints of hand due by trauma. Procedures included reconstruction of 9 hand metacarpophalangeal joints with vascularised grafting of pedal metatarsophalangeal joints in six patients, reconstruction of the hand metacarpophalangeal joints with grafting of vascularised proximal interphalangeal joints of foot in two patients and reconstruction of the hand proximal interphalangeal joints with grafting of vascularised proximal interphalangeal joints of foot in three patients. Early functional exercise was encouraged in all cases post-operatively. Follow-up ranged from 3 to 10 years and revealed that 9 cases had normal appearance and length of recipient area, 1 had slightly clumsy dorsal skin in the hand and 1 had slight dorsal angulation of a metacarpal bone. Recovery of joint range of motion was satisfactory. Radiographic, gross and sensation examinations also showed good operative outcomes. The authors believe that vascularised pedal metatarsophalangeal joints, with a rich blood supply, can be grafted to effectively reconstruct the finger joints with good function. A low rate of degeneration results because pedal and hand metatarsophalangeal joints are similar in anatomy and physiological function.  相似文献

This study presents cases in which postburn scar contractures of finger joints and interdigital spaces have been reconstructed using a scar band rotation flap. The scar band rotation flap is a subcutaneous pedicled skin island flap consisting of a contracture-causing scar band, the graft surface and normal skin. After flap rotation, the normal skin in the flap divides the scar band, thus releasing the scar contracture. In the past 10 years, we have used this reconstruction method to treat seven patients (three males and four females) ranging in age from 4 to 72 years. In all patients, the cause of the scar contractures was marginal scarring of a life-saving skin graft that was used to treat extensive burns. We reconstructed four finger joints and eight interdigital space contractures and obtained favorable results with no flap loss. We have found this method to be useful for the reconstruction of small scar contractures in locations such as finger joints and interdigital spaces after skin grafting to treat extensive burns.  相似文献

A series of 20 patients who had bilateral hip and knee replacement are reviewed and analyzed. Early surgical opinion should be sought before all four joints are irreparably destroyed and the patient is nonambulant so that the joints can be replaced one at a time, as necessary to maintain mobility. This considerably reduces stress to the patient, the surgeon, the nursing staff, and the rehabilitation team. The follow-up period ranged from 2 to 13 years. The early postoperative complications were few and the reoperation rate for aseptic loosening in this study was 3.75%.  相似文献

Synovioma, a benign neoplasm arising from pigmented villonodular synovium or synovitis, is uncommon. It involves the tendons and joints of the finger, ankle and foot. In this study four cases of villonodular synovioma of spinal joints are reported. The authors studied 555 cases of patients operated for sciatica due to herniated disk. Histopathology showed that in four out of 555 cases sciatica was found to be due to synoviomas of the spinal joints. The patients, three women and one man, were between 54 and 70 years of age and each had had a long history of low back pain with episodes of sciatica. Clinical evaluation at the time of surgery showed moderate severity of signs and symptoms. Roentgenographic studies of all four cases revealed osteoarthritic changes of vertebral apophyses, with two cases of spondylolisthesis. The pathogenesis of synovioma remains controversial and its histopathological characterization -hyperplasia VS/true tumours- has not yet been determined. It is possible that the number of recorded cases of sciatica due to synoviomas is small because surgical and pathological studies in patients with sciatica associated with osteoarthritic changes have not been adequately performed. Surgery consists of a large resection of the capsule and ligament including resection of intraspinal extension. Because this study involves only four cases, the first of which occurred in 1977, the possibility of recurrence cannot be evaluated.  相似文献

目的 :探讨造成髋关节发育不良的病理发病机制。方法 :将 18只幼兔的左髋关节屈曲膝关节伸直位石膏固定 4周 ,比较双侧髋关节的X线片及病理组织学变化。结果 :髋关节屈曲膝关节伸直位石膏固定 4周造成了髋关节发育不良的动物模型。髋臼变浅、股骨头变小、向外上方移位。X线片可见髋臼指数增大 ,由固定前的 (2 8.44± 3 .91)°增大为 (3 3 .17± 3 .65 )° ;臼头指数减小 ,由固定前的 (75 .2 0± 4.41) %减小为 (5 8.71± 8.2 1) %。组织学观察可见关节软骨退行性改变。结论 :髋关节屈曲膝关节持续伸直位状态 ,容易造成髋关节发育不良。随着时间的延长 ,病理变化加重。及早复位 ,髋关节可以恢复正常  相似文献

Background

Hand injuries affect a person’s ability to engage successfully in activities of daily living (ADLs). Video motion capture (VMC) facilitates measurement of dynamic movement. No study to date has used VMC as a means of quantifying the simultaneous movement patterns of all joints of all digits of the hand during active purposeful movement.

Method

The purpose of this study was to analyze all joints of all five digits during active completion of the lateral and pulp pinches. VMC data were collected from four participants during completion of two pinches. Joint angles were plotted to facilitate identification of movement patterns.

Results

Range of motion recorded in all joints with VMC, excluding flexion of the thumb carpometacarpal of both pinches, coincided with the normative goniometric data. Three phases were observed: initiation, preshaping, and pinch phases. Patterns of movement in all digits were identified for the two pinches.

Conclusion

VMC is a feasible and valid method for objectively quantifying dynamic movement of multiple joints simultaneously. The results provide new insight to the dynamics of hand movement as well as a basis for subsequent evaluations of movement patterns performed in ADLs and instrumental ADLs.  相似文献

目的比较类风湿性关节炎(RA)与骨性关节炎(OA)行人工全膝关节置换术(TKA)时的失血量,为制定适宜的输血方案提供参考依据。方法实施TKA的RA(30例)与OA(62例)共92个膝关节为研究对象,筛选时制定了严格的剔除方案,比较两组行TKA时总失血量。结果RA组TKA的平均失血量为(334.33±221.87)ml,OA组TKA的平均失血量为(495.97±242.83)ml。两组失血量的比较差异有显著性(P〈0.05)。结论不同膝关节疾病行TKA时失血量有差异,因而在制定输血方案时应因病制宜。  相似文献

Four cases of synovial cyst (ganglion) arising from the facet joints of the lumbar spine are reported. A typical presenting feature was exacerbation of pain on standing or walking, mimicking vascular claudication. MRI proved in all four cases to be the definitive investigation and surgery the treatment of choice, producing excellent results.  相似文献

The sacroiliac joints of seven pelvic specimens were examined to determine functional, morphologic, and histopathologic aspects. The movements were measured in four intact pelvises (from two men and two women). The joint surfaces of all pelvises (from three men and four women) then were examined topographically by means of a photogrammetric method. After this, they were examined histologically to characterize any effects on function. The morphologic investigation revealed sex-specific differences. All joint surfaces from the female pelvises showed circular contours, the centers of which coincided with the iliac tuberosities. These morphologic characteristics were not discernible in the joint surfaces from the male pelvises; these had interlocking irregularities without a topographic pattern. As expected, this configuration involved distinct differences in mobility. Rotation of the sacrum was markedly less in the sacroiliac joints of men than in those of women.  相似文献

目的 研究分析软骨的营养和生长特点,探讨外伤性软骨缺损后软骨移植的可能性及提高手术疗效的处理对策。方法 对健康狗的掌指关节行全关节及半关节的肌肉内植及原位植入,大体及光镜下观察软骨的生长和变性情况;对18例手掌指关节及近侧指间关节外伤性软骨缺损行第二跖趾关节软骨及全关节移植,并对部分手术细节予以改进,随访分析。结果 动物实验中植入肌肉组较原位植入组软骨变性明显,后者在弹性软骨细胞数量及基质分泌虽较正常对照组减少,但均在4周左右建立循环,其中半关节组优于全关节组。18例患者经6月至8年随访,关节活动度平均较术前增加43.5度,5例出现疼痛,6例出现关节间隙狭窄,余关节形态及结构正常。结论 早期的规律的关节活动与关节的营养供应对软骨的存活同等重要,而匹配良好的关节软骨面,可靠的内固定,韧带重建,供区尽量薄的软骨面和少的骨量及适宜的生化,力学环境是提高手术疗效的关键。  相似文献

A treatment plan for pyogenic arthritis has been devised in which the accessory collateral ligaments are resected through radial and ulnar midaxial incisions. The surgical wound is debrided and copiously irrigated with an antibiotic solution, and intravenous antibiotics are instituted before operation and continued after operation. Physical therapy is begun within 24 hours of surgery and includes active range of motion (ROM) of all joints, dynamic splinting, and hand therapy to ensure both full flexion and extension. Forty patients were treated with this method within a 6-year period. Of five interphalangeal (IP) joints of the thumb, four regained 0 degree to 60 degrees full ROM and one gained 15 degrees to 65 degrees limited ROM. Of the two distal IP joints, one regained full ROM and the other became fused. Among the 33 proximal IP joints, 13 regained full ROM of 110 degrees and 13 regained limited ROM. Good functional results were obtained with this treatment plan in lieu of amputation, which previously was the most likely treatment.  相似文献

Total knee joint transplantation has been performed in animal models and humans. This study investigates the impact of this operation on knee joint function in a dog model. Therefore, replantation was compared to transplantation during a 6-month follow-up period in four dogs in each group. The peak vertical ground reaction force normalized in all legs undergoing replantation and in two of four after transplantation. A third transplant recipient reduced loading from the 4th month due to a local complication, and the fourth succumbed to sepsis 3 months postoperatively. A weight-bearing index (WBI), defined as loading of the grafted divided by loading of both hindlimbs decreased from 0.48 ± 0.08 preoperatively to 0.13 ± 0.10 by 1 month after replantation and from 0.53 ± 0.07 to 0 after transplantation. After 6 months, weight-bearing of all replant recipients was restored, but reduced in two transplant recipients with graft function. Full recovery after replantation, but impaired function after transplantation, was also reflected in the histological results: normal histological pictures of blood vessels, cartilage, bone and soft tissues were found in all replant recipients, but infiltrative vasculopathy indicating chronic rejection was found in the transplanted joints. The results of this animal study confirm that the procedure can lead to satisfactory functional results but also emphasize the need for perfect control of immunosuppression. 骨质疏松症(OP)以骨量减少和骨的微细结构破坏为主要特征,并伴随骨质脆性增加和骨折危险性升高.由于骨的结构破坏,可引发脊柱退行性变,使椎体高度丢失应力改变及使人体成骨、破骨细胞代谢紊乱,发生脊柱尤其是腰椎小关节退变.但由骨质疏松症引起脊柱小关节退变尤其是腰椎小关节退变原因发病机制文献报道较少,目前认为骨质疏松对脊柱小关节退变与成骨,破骨细胞代谢影响,及发生椎体内微骨折,改变椎体高度及软骨下骨血供应微环境,增加小关节应力,减少其血供营养有关.  相似文献
