PURPOSE: To assess the long-term refractive, visual acuity, binocular vision, and quality-of-life outcomes of photorefractive keratectomy (PRK) and laser-assisted subepithelial keratectomy (LASEK) in children. SETTING: Non-hospital surgical facility with follow-up in a hospital clinical setting. METHODS: In this retrospective review, 56 eyes of 39 patients had PRK or LASEK under general anesthesia. Patients were examined preoperatively and postoperatively at 2 and 6 months and 1 year and then annually for a minimum of 3.5 years. Recorded variables included demographics, refractive error, best corrected visual acuity (BCVA), stereopsis, corneal haze, and quality of life. RESULTS: The mean age at surgery was 6.5 years (range 1.0 to 17.4 years). At the final postoperative examination (mean 5.15 years), the mean spherical equivalent was -1.73 diopters (D) in all patients, -3.20 D in PRK patients, and -1.37 D in LASEK patients. Refraction and corneal clarity were stable over the long term in all eyes. In 28 eyes that were measurable preoperatively, visual acuity improved by a mean of 1.6 lines (range 0 to 7 lines). Seven patients (18%) had measurable stereopsis before surgery and 19 (49%), after PRK or LASEK. No patient had reduced BCVA or loss of binocular fusion postoperatively. On a quality-of-life questionnaire, no family recorded negative opinions of the procedure or negative social or functional outcomes postoperatively. CONCLUSION: Photorefractive keratectomy and LASEK were effective and stable surgical alternative treatments in children with refractive errors who were unable to tolerate or who failed conventional methods of treatment.  相似文献

PURPOSE: To compare laser subepithelial keratomileusis (LASEK) and photorefractive keratectomy (PRK) in different eyes of the same patients in terms of visual acuity, refractive error, and complications over 1 year. METHODS: This prospective, randomized, double-masked study comprised 30 active-duty military personnel with myopia who underwent LASEK in one eye and PRK in the other eye. RESULTS: Twenty-eight patients' results were available for 1-year follow-up. The primary outcome measures were visual acuity and refractive error. The mean visual acuity for the LASEK group was 1.56 and 1.67 for the PRK group (z = -0.18, P = .15). The mean spherical equivalent refraction for the LASEK group was -0.007 D and +0.124 D for the PRK group (t = 0.982, P = .40). No significant differences were noted in visual acuity or refractive error in the eyes that had LASEK versus the eyes that had PRK. CONCLUSIONS: After 1-year follow-up, LASEK and PRK show similar levels of visual acuity and refractive error.  相似文献

OBJECTIVE: To describe the safety, effectiveness, and predictability of laser in situ keratomileusis (LASIK) for correcting residual myopia after primary photorefractive keratectomy (PRK). DESIGN: A retrospective, noncomparative case series. PARTICIPANTS AND INTERVENTION: Thirty-six consecutive eyes of 30 patients underwent LASIK after primary PRK. A Multiscan Schwind excimer laser was used for LASIK enhancement. MAIN OUTCOME MEASURES: Uncorrected visual acuity (UCVA), best spectacle-corrected visual acuity, refraction, videokeratography, and complications were determined before and after LASIK retreatment. Follow-up was at least 12 months. RESULTS: Before LASIK, 11.11% of eyes showed a UCVA of 20/40 or better. This increased to 94.44% 12 months after LASIK. A UCVA of 20/25 or better was achieved in 0% before and in 72.22% after retreatment. Refraction +/-0.5 diopters (spherical equivalent) represented 0% of eyes before and 77.78% of eyes after enhancement. Before LASIK, two eyes had significant haze. Haze remained in these two eyes and appeared in another eye. CONCLUSIONS: Laser in situ keratomileusis proved to be safe and effective for treating residual myopia after PRK. Care must be taken when considering LASIK retreatment in patients with significant haze after primary PRK.  相似文献

PURPOSE: To compare the release of tear fluid transforming growth factor-beta1 (TGF-beta1) of 15 patients who underwent photorefractive keratectomy (PRK) in one eye and laser subepithelial keratomileusis (LASEK) in the other eye. METHODS: Tear fluid samples were collected with scaled microcapillary tubes preoperatively (day 0) and on the first, second, and seventh postoperative days. We calculated the release of TGF-beta1 (tear fluid flow-corrected concentrations of TGF-beta1) by multiplying the concentration by capillary tear fluid flow and also evaluated corneal haze at 1, 3, and 6 months after PRK or LASEK. RESULTS: The median TGF-beta1 release values were: Day 0: 50.6 pg/min (range 0 to 101.6 pg/min) for PRK and 56.7 pg/min (range 0 to 121.8 pg/min) for LASEK (P=.496); Day 1: 240.6 pg/min (range 56.6 to 1120.2 pg/min) for PRK and 155.2 pg/min (range 45.1 to 480.4 pg/min) for LASEK, which showed a significant difference between the two procedures (P=.041); Day 2: 395.7 pg/min (range 92.3 to 1150.7 pg/min) for PRK and 185.2 pg/min (range 41.4 to 872.5 pg/min) for LASEK (P=.004); and Day 7: 91.3 pg/min (range 0 to 225.4 pg/min) for PRK and 74.3 pg/min (range 0 to 187.1 pg/min) for LASEK, (P=.244). The mean corneal haze score at 1 month after PRK was 0.88 +/- 0.50 and 0.53 +/- 0.29 after LASEK, and a statistically significant difference was noted between the two groups (P=.005). At 3 months (P=.083) and 6 months (P=.157) after the procedures, there were no statistically significant differences. CONCLUSIONS: A lower amount of tear fluid transforming growth factor-beta1 was released in the early postoperative days following LASEK than in PRK. The lower grade of corneal haze seen in LASEK than PRK in the early postoperative period may come from a decreased release of TGF-beta1, which may have a role in wound healing.  相似文献

目的 研究准分子激光角膜上皮瓣下磨镶术 (LASEK)与准分子激光屈光性角膜切削术(PRK)在治疗 - 8 0 0D以下近视的异同。方法  4 6例近视度数在 - 1 75~ - 8 0 0D的患者在双眼同时治疗时 ,其中一只眼行PRK治疗 ,另一只眼行LASEK治疗 ,观察上皮愈合时间 ,记载疼痛程度 ,对比术后 6个月内视力、屈光状态及角膜上皮下雾状混浊 (Haze)程度。结果 LASEK组上皮愈合时间平均为 3 4 9d ,PRK组为 2 87d ,两者差异有显著意义 (P <0 0 5 ) ;平均疼痛指数LASEK组为 2 0 4 ,PRK组为 2 4 5 ,差异有显著意义 (P <0 0 5 ) ;术后 6个月内两组间视力及屈光度恢复的差异无显著意义 (P>0 0 5 ) ,并发症的发生无差异。LASEK组术后Haze程度明显低于PRK组 (P <0 0 5 )。结论 LASEK可以和PRK一样安全有效地治疗低于 - 8 0 0D的近视 ,并且与PRK相比 ,可以降低早期疼痛水平 ,减少Haze并发症的发生。  相似文献

目的评价准分子激光角膜原位磨镶术(LASIK)矫治高度近视准分子激光屈光性角膜切削术(PRK)后屈光回退的疗效。方法回顾性临床研究。对PRK术后2年以上、屈光回退且屈光度稳定的患者8例14眼行LASIK,对LASIK术后术眼进行评价。患眼PRK术前屈光度为-6.25~-12.50 D,PRK术后屈光度为-1.50~-6.25 D。随访观察LASIK矫正1年后术眼的裸眼视力、最佳矫正视力、屈光度、上皮下雾状混浊(haze)形成和角膜厚度的变化。结果所有患眼术后主观症状较轻。LASIK术后1年平均球镜度数为(-0.62±0.94)D。LASIK术后0.5≤裸眼视力〈0.8者4眼,≥0.8者9眼,1眼(7.1%)最佳矫正视力下降2行。4眼术后出现不同程度的haze,包括2级haze 3眼、3级haze 1眼。应用氟米龙滴眼液1个月后,haze及屈光回退减轻;术后1年,1级haze 2眼,2级haze 2眼,3级haze 1眼。LASIK术前角膜厚度为(467±38)μm,术后为(422±21)μm。结论高度近视PRK术后屈光回退行LASIK矫治是一种可行的方法,但少数患者术后可出现haze,仍需治疗。  相似文献

PURPOSE: To evaluate the efficacy and safety of laser in situ keratomileusis (LASIK) for myopic regression and undercorrection after photorefractive keratectomy (PRK). SETTING: The Eye Institute, Sydney, Australia. METHODS: Fifty eyes of 32 patients were treated by LASIK for residual myopia following primary PRK. The mean spherical equivalent refraction (SEQ) was -2.92 diopters (D) +/- 1.57 (SD) (range -0.75 to -7.88 D). The mean refractive cylinder was 0.96 +/- 0.74 D (range 0 to 3.50 D). For analysis, the eyes were divided into 2 groups: those with 0 or low corneal haze (Group 1) and those with severe corneal haze (Group 2). In Group 1, the SEQ was -1.99 +/- 0.79 D (range -0.75 to -3.75 D) and in Group 2, -3.77 +/- 1.62 D (range -0.75 to -7.90 D). The procedure was performed using the Chiron Automated Corneal Shaper and the Summit Apex Plus laser. The mean interval between PRK and LASIK was 25 months (range 9 to 59 months). The following parameters were studied before and after LASIK retreatment: SEQ, mean refractive cylinder, uncorrected visual acuity (UCVA), and best corrected visual acuity (BCVA). Complications after LASIK retreatment were evaluated. RESULTS: Six months after LASIK, the mean SEQ in all eyes was -0.65 +/- 0.86 D (range +1.50 to -3.35 D); 70.0% of eyes were within +/-1.00 D of emmetropia and the UCVA was 6/12 or better in 72.5%. The mean SEQ in Group 1 was -0.22 +/- 0.55 D (range -0.88 to -1.50 D) and in Group 2, -0.97+/- 0.92 D (range 0.12 to -3.25 D); the UCVA was 6/12 or better in 94.0% of eyes in Group 1 and in 56.0% in Group 2. No statistically significant between-group difference was found in lines of Snellen acuity lost or gained at 6 months. No eye lost more than 1 line of BCVA. CONCLUSIONS: Laser in situ keratomileusis appears to be a safe, effective, and predictable procedure for treating eyes with 0 or low haze with residual myopia after PRK. 目的对比分析准分子激光上皮下角膜磨镶术(LASEK)、碰光性角膜切削术(PRK)、激光原位角膜磨镶术(LASIK)治疗中低度近视临床疗效。方法LASEK组58例(112眼)随访3-9月,PRK、LASIK组各120眼随访1a以上,观察分析术后疼痛症状,裸眼视力及角膜混浊情况。结果术后1d出现疼痛症状:LASEK组:81眼(72.3%),PRK组120 眼(100.0%),LASIK组13眼(10.8%);术后3d裸眼视力≥0.8:LASEK组24眼(21.4%),PRK组27眼(22.5%),LASIK组113 眼(94.2%);术后3月存在角膜Haze:LASEK组31眼(27.6%)、PRK组94眼(78.3%),LASIK组0眼。结论LASEK后疼痛症状、角膜Haze明显减轻,疗效优于PRK;LASEK与LASIK相比没有角膜瓣相关并发症,早期视力恢复较慢。  相似文献

PURPOSE: To evaluate the results of laser in situ keratomileusis after photorefractive keratectomy. METHODS: Eighty eyes of 80 patients with residual myopia after photorefractive keratectomy were reoperated with laser in situ keratomileusis. The study was retrospective. Laser in situ keratomileusis was performed using the automated corneal shaper microkeratome and Chiron Technolas 217-C d1 excimer laser. Data measured after laser in situ keratomileusis included uncorrected visual acuity, best-corrected visual acuity, refraction, haze, pachymetry, and keratometry. The follow-up was at least 12 +/- 1.6 months (range, 12 to 15 months). RESULTS: After laser in situ keratomileusis the mean spherical equivalent was -0.24 diopters +/- 0.78. (range, -3 to +1.5) at 12 months, and the mean uncorrected visual acuity was 0.76 diopters +/- 0.24 (range, 0.1 to 1). Sixty-five eyes (81.3%) had various degrees of haze after laser in situ keratomileusis. One eye (1.2%) lost 2 lines of best-corrected visual acuity. CONCLUSIONS: Laser in situ keratomileusis enhancement may be a good alternative to correct residual myopia and astigmatism after primary photorefractive keratectomy. Corneal haze is a common problem in these eyes, and the treatment after laser in situ keratomileusis enhancement should be the same as the treatment after primary photorefractive keratectomy.  相似文献

PURPOSE: To evaluate the safety and efficacy of laser in situ keratomileusis (LASIK) to enhance refractive status following other corneal surgical procedures. SETTING: Clinical office-based practice. METHODS: Seventy-one eyes of 57 patients had LASIK for refractive errors following radial keratotomy (n = 22), astigmatic keratotomy (n = 13), photorefractive keratectomy (n = 18), and penetrating keratoplasty (n = 18). A Moria LSK-1 microkeratome was used with a Visx S2 or Wavelight Allegretto excimer laser. Data were acquired by retrospective chart review of all appropriately qualified patients. RESULTS: The mean preoperative manifest refractive spherical equivalent (MRSE) was -3.93 diopters (D) +/- 2.83 (SD) in myopic eyes and +1.43 +/- 1.79 D in hyperopic eyes. The mean time from the initial corneal surgical procedure to LASIK was 65.0 months. The mean post-LASIK follow-up was 9.40 months (range 1 to 42 months). Postoperatively, the mean MRSE was -0.85 +/- 1.42 D in myopic eyes (P<.0001) and -0.16 +/- 1.09 D in hyperopic eyes (P<.0001). Enhancement by LASIK was required in 14% of eyes. CONCLUSION: In eyes that have had a variety of previous corneal surgeries, LASIK offers a safe and predictable method for enhancing refractive results.  相似文献

PURPOSE: To investigate the results of photorefractive keratectomy (PRK) and laser in situ keratomileusis (LASIK) procedures performed by novice ophthalmologists and compare the results with those of experienced refractive surgeons. SETTING: Doheny Eye Institute, University of Southern California, Los Angeles, and Department of Ophthalmology, University of California, Irvine, California, USA. METHODS: In this retrospective case series, data were examined from the first PRK procedures by 33 consecutive ophthalmologists and the first LASIK procedures by 19 consecutive ophthalmologists. Preoperative and postoperative uncorrected visual acuity (UCVA) and best spectacle-corrected visual acuity (BSCVA), as well as intraoperative and early postoperative complications, were recorded. RESULTS: In the PRK group, 33.3% of eyes achieved a UCVA of 20/20 and 87.8%, 20/40 or better; 54.5% were within +/-0.5 diopter (D) of emmetropia and 87.8%, within +/-1.0 D. PURPOSE: To study the role of photorefractive keratectomy (PRK) in the management of laser in situ keratomileusis (LASIK) flap complications. METHODS: A retrospective analysis was performed of seven patients (14 eyes) who had bilateral, simultaneous LASIK (single surgeon [TGA], Summit Apex Plus laser, Hansatome microkeratome in six patients [12 eyes], Automated Corneal Shaper keratome in one patient [two eyes]). One eye of each patient received PRK for a flap complication. The seven patients (four women, three men) had a mean age of 39.2 years (range, 22 to 64 yr). The uncomplicated LASIK eyes had moderate to high myopia (-2.25 to -8.75 D) and the eyes with LASIK flap complications had mild to moderate myopia (-1.50 to -5.75 D). Immediate transepithelial PRK was performed in all eyes after repositioning the defective flap. RESULTS: All seven patients had excellent visual outcome at 6 months follow-up; one patient required an enhancement (astigmatic keratotomy). Six of the seven LASIK eyes had best spectacle-corrected visual acuity of 20/20 or better and one LASIK eye had 20/25. In the transepithelial PRK eyes, six of the seven eyes had best spectacle-corrected visual acuity of 20/20 or better and one eye had 20/30. CONCLUSIONS: Immediate transepithelial PRK treatment of irregular and incomplete LASIK flaps at the time of surgery is an excellent therapeutic option to delayed surgical treatment.  相似文献

目的探讨准分子激光角膜上皮瓣下磨镶术(LASEK)手术和准分子激光屈光性角膜切削术(PRK)手术后角膜上皮下雾状浑浊(haze)的发生率。方法分别对120例(240眼)左右眼分别行LASEK和PRK手术,观察12个月,计算出低中度近视组(等于或低于-6.00D)和高度近视组(-6.00D以上)haze的发生率。结果低中度近视组和高度近视组LASEK手术后1,3,6,12个月haze的发生率均低于PRK手术后1,3,6,12个月haze的发生率,其中低中度近视组和高度近视组LASEK手术后1,3个月1级haze的发生率和PRK手术后1、3个月1级haze的发生率比较差异均有统计学意义(P〈0.05),低中度近视组LASEK手术后12个月1级haze的发生率和PRK手术后12个月1级haze的发生率比较差异有统计学意义(P〈0.05)。结论 LASEK手术后haze形成情况显著好于PRK手术。  相似文献

PURPOSE: To compare keratocyte loss in the corneal stroma after laser-assisted subepithelial keratectomy (LASEK) and photorefractive keratectomy (PRK) in rabbits. SETTING: Department of Ophthalmology, University of Essen, Essen, and the Institute of Anatomy, University of Bochum, Bochum, Germany. METHODS: Laser-assisted subepithelial keratectomy and PRK were performed in rabbits and studied 1, 3, 10, and 20 days after surgery. Excimer photoablation was done unilaterally with a 6.0 mm ablation zone and an 80 microm depth, equivalent to -6.0 diopters. Keratocyte death was analyzed using DNA fragmentation-detecting terminal deoxynucleotidyl transferase deoxy-UTR-nick end labeling (TUNEL) assay and transmission electron microscopy. RESULTS: Numerous TUNEL-positive keratocytes occurred 1 day after PRK; the number decreased significantly after 3 days. After LASEK, significantly fewer TUNEL-positive keratocytes were noted at the early time points (P<.001 at 1 day; P< or =.05 at 3 days). At 10 days, the number of TUNEL-positive keratocytes decreased in both groups but remained significantly higher after PRK than after LASEK (P<.001). Twenty days after both procedures, no significant signs of keratocyte death were found in the corneal stroma. Transmission electron microscopy revealed few apoptotic keratocytes after LASEK. After PRK, apoptotic keratocytes, characterized by chromatin condensation, apoptotic bodies, and cell shrinkage, were scattered in the stroma. The ultrastructural findings confirmed the results obtained with the TUNEL assay. CONCLUSIONS: Laser-assisted subepithelial keratectomy induced significantly less apoptotic keratocyte death than PRK and promoted wound healing in the acute phase after photoablation. This procedure may offer the possibility of treating higher myopia with a decreased risk for developing wound healing-related complications known to occur after PRK.  相似文献

PURPOSE: To assess low-contrast visual acuity (LCVA) after photorefractive keratectomy in relation to ocular higher-order wavefront aberration and corneal subepithelial haze. DESIGN: Prospective, cross-sectional analysis. METHODS: Photorefractive keratectomy was performed in 51 eyes of 27 subjects with myopic refractive error of -2.0 to -10.5 diopters. Ocular higher-order wavefront aberrations for a 4-mm pupil were measured using Topcon Hartmann-Shack wavefront aberrometer, and the extent of corneal subepithelial haze was quantified with Nidek TSPC-3 hazemeter before and 1 month after photorefractive keratectomy. Low-contrast visual acuity was recorded with Vector Vision CSV-1000LanC10% chart. Total higher-order, third-order (coma-like), and fourth-order (spherical-like) aberrations of the eye were determined. The influence of wavefront aberration and corneal subepithelial haze on LCVA was analyzed. RESULTS: Total higher-order, third-order, and fourth-order aberrations significantly increased by surgery (P < .001, Wilcoxon signed rank test). Photorefractive keratectomy induced a significant increase in corneal haze (P < .01), but no case presented severe corneal haze (grade 3 or greater by Fantes grading). By surgery, LCVA was reduced significantly (P < .001). The logarithm of the minimal angle of resolution LCVA showed a significant correlation with total higher-order aberration (Spearman rank correlation coefficient, r(s) = 0.642, P < .0001). Both third-order (r(s) = 0.618, P < .0001) and fourth-order aberrations (r(s) = 0.552, P < .0001) also significantly correlated with logarithm of the minimal angle of resolution LCVA. There was no correlation between the degree of corneal haze and logarithm of the minimal angle of resolution LCVA (r(s) = 0.094, P = .523). CONCLUSIONS: In eyes with mild to moderate corneal haze after photorefractive keratectomy, deterioration of LCVA is mainly attributable to increases in wavefront aberration, and not to corneal haze.  相似文献
