Objective To compare the effects of Celsior (CS) solution vs University of Wisconsin (UW) solution on preservation of non-heart- beating donor (NHBD) livers in rats. Methods The healthy male SD rats were taken as donors and recipients. The thoracic descending aorta and supradiaphragmatic inferior vena cava were cross-clamped for 10 min to induce ischemia as a NHBD model. After flushed with the cold CS or UW solution, NHBD livers were stored at 4℃ for 8 h or 16 h. Then they were transplanted using the two-cuff technique with arterial reconstruction. Accordingly, the recipients were divided into four groups: CS 8 h, UW 8 h, CS 16 h and UW 16 h groups. The bile discharge, liver enzymes (ALT and AST), ET-1, IL-1 and TNF-alpha levels in serum at 1, 3, and 6 h after reperfusion were measured. Histopathological examination was carried out by HE staining, and the 7-day survival rate was observed. Results As compared with a homogenous appearance in the NHBD livers perfused with CS solution, the NHBD livers perfused with UW solution showed a heterogenous appearance, with more or less mottling. In the CS 8 h group there were more bile discharge at 1 h, lower ALT and AST levels at 1, 3 and 6 h, and lower serum ET-1, IL-1 and TNF-alpha ? levels at 1 and 3 h after reperfusion than the UW 8 h group (P<0.05). Histopathological examination showed a well-preserved hepatic architecture, milder portal congestion and inflammatory infiltration in CS 8 h group as compared with UW 8 h group. The 7-day survival rate in CS 8 h and UW 8 h groups was 58.3% and 25.0% respectively (P<0.05). Objective To study the effects of TAT-HO-1 fusion protein,HIV-1 transactiviting protein (TAT) and heme oxygenase-1 (HO-1),on hepatic cell apoptosis of rat donors in cold storage stage.Methods Forty-eight male SD rats were randomly divided into two groups.Rat livers were flushed and preserved with 4℃ HTK solution containing(group P) or uncontaining(group C) 50 mg/L of TAT-HO-1.The preserved solution and hepatic tissue were collected at 0,6,12,18 h of cold storage stage.TAT-HO-1 transducing into liver,alanine aminotransferase(ALT) level in preserved solution,hyaluronic acid(HA) level and the expression of caspase-3 in hepatic tissue,and the apoptotic index (AI) of hepatocytes and sinusoidal endothelial cells (SECs) were measured or detected.Results ALT level in preserved solution,HA level and the expression of caspase-3 in hepatic tissue,and the AI of hepatocytes and SECs increased time-dependently in cold storage stage in both groups (P<0.05),with lower increasing extent in group P than that in group C (P<0.05) at 6h,12h and 18h of cold storage stage.A stronger accumulation of HO-1 staining was also detected at the same time-points in group P than that in group C(P<0.05).Conclusion TAT-HO-1 may transduce efficiently into rat livers,exerting protective effects on both hepatocytes and SECs during cold storage stage.Protein transduction technology may be a novel therapeutic means to reduce donor liver injury in preservation period for transplantion.  相似文献

目的 比较F-Z液与Uw液在细胞水平的保存效果.方法 1.测定内皮细胞经保存-复温后上清中乳酸脱氢酶(LDH)浓度,比较保存一复温后损伤;2.以MTT法比较保存-复温后的内皮细胞活力.结果 1.保存时间24 h内,UW液组上清中乳酸脱氧酶(LDH)浓度与F-Z液组无明显差别;2.保存时间48 h内,UW液组MTT结果与F-Z液组无明显差别.结论 在细胞水平,于减轻保存-复温后损伤和维持被保存细胞活力两个方面,F-Z液均达到UW液相似的保存效果,因而具进一步开发研究价值.  相似文献

INTRODUCTION: This study compared the safety and efficacy of University of Wisconsin solution (UW) and Celsior solution (C) in pancreas transplantation (PTx). METHODS: A retrospective review of 154 PTx performed over a 61-month period included 77 grafts preserved with UW and 77 with C. The two groups were comparable for both donor and recipient characteristics. RESULTS: After a mean cold ischemia time of 624 minutes (range 360 to 945 minutes) for UW versus 672 minutes (range 415 to 1005 minutes) for C (P = NS), no primary endocrine nonfunction occurred. Delayed endocrine function was diagnosed in two grafts in the UW group (2.6%) versus none in the C group (P = NS). After a minimum follow-up of 4 months (mean 26.5 +/- 15.2 months), 22 recipients (UW = 11 vs C = 11; P = NS) required relaparotomy. Overall, 18 pancreata were lost due to either patient death with functioning graft (UW = 4 vs C = 1; P = NS) or graft loss due to other reasons (UW = 8 vs C = 5; P = NS). Actuarial 1- and 5-year patient survival rates were 93.5% and 86.8% for UW compared with 98.7% and 98.7% for C (P = .04). Actuarial graft survival rates at the same times were 88.3% and 75.0% for UW compared with 90.4% and 90.4% for C (P = NS). CONCLUSIONS: Within the range of cold ischemia times reported in this study, UW and C show similar safety and efficacy profiles for PTx.  相似文献

BACKGROUND: A safe and effective preservation solution is a precondition for successful orthotopic liver transplantation (OLT). This study compared University of Wisconsin (UW) and histidine-tryptophan-ketoglutarate (HTK) solutions in OLT. PATIENTS AND METHODS: We retrospectively reviewed the medical records of 137 primary cadaveric. OLT performed between January 2003 and December 2006 at our institution. Sixty-eight grafts were harvested using UW and 69 using HTK. Recipients were managed similarly in regard to operative techniques and immunosuppression. We collected donor data including serum transaminases, serum sodium, ICU stay and assessed macroscopic liver quality. Recipient serum transaminases were collected on postoperative days 1, 7, 14, and 30. We compared biliary and vascular complications, as well as patient and graft survivals. RESULTS: Mean serum bilirubin levels were slightly higher in the HTK group at 1,7,14, and 30 days after transplantation, whereas transaminases were higher in the UW group. Primary nonfunction occurred in 1 patient in each group. Retransplantation was performed in 5 patients in the UW and in 9 patients in the HTK group. Biliary complication rates were similar in the UW and HTK groups (22% and 17%, respectively). Six arterial complications occurred in the HTK (8.7%) and 2 in the UW group (2.9%; P < .05). Mean follow-up was 25 months. Graft survival at 1, 12, and 36 months was 90%, 78%, and 75% versus 90%, 71%, and 71% in the UW versus HTK groups, respectively. One-, 12-, and 36-month patient survival rates were 93%, 78%, and 75% versus 93%, 78%, and 78% in the UW versus HTK groups, respectively. CONCLUSIONS: There were no significant differences in graft and patient survivals between the 2 groups. Whereas the biliary complication rates were comparable in both groups, the arterial complications were clearly higher in the UW group (8.7% vs 2.9%; P < .05%). UW and HTK solutions seemed to be equally safe and effective in the preservation of liver grafts. The high incidence of arterial complications in the UW group requires further prospective studies.  相似文献

All solutions studied were infused at 1 ml·kg−1·min−1 for 90 min after base excess (BE) reached about −13 mEq·l−1 by maintaining the mean blood pressure (MBP) at 40 mmHg. MBP, renal blood flow (RBF), vertebral blood flow (VBF), and urinary output significantly increased after the start of infusion of AR, LR, R, and AR-D. The VBF and urinary output were particularly improved with AR-D. The arterial blood level of HCO 3 and BE were also increased after the start of infusion of AR, LR, and AR-D but not of R. AR infusion improved BE more effectively than LR. Although AR-D, AR, and LR increased HCO 3 , the blood pH did not increase in AR-D. The value of plasma acetate increased after the start of infusion of AR and AR-D but not of LR, and R. On the other hand, plasma lactate and pyruvate levels were higher with LR than with AR. The increase in the lactate/pyruvate ratio induced by LR was larger than that by AR. The plasma norepinephrine and epinephrine levels decreased after the start of all infusions. Plasma insulin and glucose levels were markedly increased after the start of AR-D infusion but were not affected by AR, LR, and R. These results indicate that the effectiveness of various infusion solutions such as AR, LR, R, and AR-D during canine hemorrhagic shock varies. AR-D may be useful for increasing both peripheral blood flow and urine output. AR may also be useful for improvement in metabolic acidosis and surgical diabetes induced by hemorrhagic shock.  相似文献
