目的 探讨LASIK后弥漫性板层角膜炎的病因、临床特征及防治措施.方法 选择2007年3月至2007年12月期间行LASIK手术患者199例(398只眼)术后发生弥漫性板层角膜炎(DLK)者11例(17只眼)作为观察组,对手术室、器械消毒、术前结膜囊清洁及角膜上皮保护等方面改进后,2007年12月至2008年4月期间行LASIK手术患者148例(296只眼)发生DLK者5例(7只眼)作为对照组,分析DLK各期病变发生情况、临床特征及治疗过程.结果 观察组11例(17只眼)发生DLK发生率为4.27%,其中Ⅰ、Ⅱ期病变发生率分别为3.01%和0.5%,Ⅳ期2例(3只眼)发生率为0.75%,对照组5例(7只眼)发生率为2.36%,均为Ⅰ、Ⅱ期病变,发生率分别为1.68%和0.67%.Ⅰ、Ⅱ期病变发生于术后1~3d治疗后7~10d全部吸收,视力恢复正常,Ⅳ期病变发生于术后3~7d裂隙灯下见角膜中央层间灰白色较致密混浊合并角膜瓣融解,愈合后遗留高度远视,5~6月远视部分回退,视力恢复至0.6~1.0.结论 LASIK术后弥漫性板层角膜炎的发生与多因素相关,对术中及手术前后各环节严格把关能预防DLK发生和发展,及时治疗其预后较好. 相似文献

The incidence rate of DLK in control group was 2.36%,among which,stage Ⅰ was 1.68% and stage Ⅱ was 0.67%. DLK of stage Ⅰ and stage Ⅱ happened 1~3 days after LASIK,it absorbed entirely after 7~10 days'treatment, and vision rocovered to normal. DLK of stage Ⅳ happened 3~7 days after LASIK, relatively white dense opacity under the lamellar of central cornea and flap melt could be seen through slit lamp. High hyperme-tropia was left after healing. Hypermetropia decreased partly after 3~6 months, and vision recovered to 0.6~1.0. Conclusion The occurrence of DLK after LASIK is related with multifactors. DLK can be prevented by the strict checks on each link of the operation and cured better if it is treated in time. 相似文献
