目的:探讨糖耐量低减(IGT)的老年急性冠状动脉综合征(ACS)非ST段抬高病人临床特点。方法:96例ACS患者根据血糖和口服葡萄糖耐量试验(OGTT)被分为:糖尿病(DM)组(32例).糖耐量减低(IGT)组(32例)和糖耐量正常(NGT)组(32例)。所有病人检测血糖,血脂,心电图和作冠状动脉(冠脉)造影,并进行各组间相互比较。结果:DM组和IGT组的血糖、甘油三酯水平比NGT组高(P<0.05~<0.01),而。DM组的血糖水平比IGT组升高更为明显(P<0.05~<0.01)。DM组和IGT组的心电图缺血和冠状动脉造影显示的狭窄比NGT 组重。P<0.01。结论:IGT组和DM组的血脂代谢异常,心肌缺血和血管狭窄程度均明显较NGT组重。故对IGT 的老年病人应警惕冠心病病变。  相似文献

目的探讨不同糖耐量者血清超敏C-反应蛋白(hs-CRP)的表达及与胰岛素抵抗的相关性。方法对2004年10月至2005年4月中国医科大学附属第一医院体检中心90名体检者(男53名,女37名),根据标准75g口服葡萄糖耐量试验(OGTT)分为正常糖耐量(NGT)组、单纯空腹血糖受损(IFG)组、单纯糖耐量异常(IGT)组、同时合并IFG/IGT组及2型糖尿病(T2DM)组,采用酶联免疫吸附(ELISA)法分别检测各组血清hs-CRP,并与稳态模型胰岛素抵抗指数(HOMA-IR)作相关分析。结果IFG组、IFG/IGT组及T2DM组hs-CRP均明显高于NGT组,并与糖代谢指标及HOMA-IR呈正相关。结论T2DM患者早在IFG阶段就已经存在炎症状态,炎症可能参与了T2DM的发生与发展。  相似文献

Impaired fasting glucose (IFG) is a subgroup of impaired glucose regulation exhibiting an elevated fasting glucose levels without elevated 2-h glucose levels on oral glucose tolerance test (OGTT). Diabetes mellitus with isolated fasting hyperglycemia (DM/IFH) is a similar subgroup of diabetes having higher fasting glucose levels with 2-h glucose levels within the non-diabetic range. The aim of this study is to profile the characteristics of these subgroups to estimate the factors involved in the development from normal glucose tolerance (NGT) via IFG to DM/IFH. Five hundred and sixty seven Japanese males were classified on the basis of 75 g OGTT into four groups, NGT, IFG, DM/IFH, and isolated impaired glucose tolerance (isolated IGT). Insulin secretion was evaluated by insulinogenic index, insulin sensitivity was evaluated by ISI composite, and insulin secretory patterns were compared additionally. IFG and DM/IFH subjects exhibited both lower insulin secretion and lower insulin sensitivity than NGT subjects. There was an insulin peak in NGT, IFG, and DM/IFH at 60 min, which did not occur in isolated IGT. Impaired early-phase and basal insulin secretion and decreased insulin sensitivity both are estimated as factors in progression from NGT via IFG to DM/IFH in these subjects. IFG and DM/IFH subjects have definite fasting hyperglycemia in contrast to isolated IGT subjects, 2-h glucose levels being maintained within the non-diabetic range partly by the insulin peak at 60 min.  相似文献

BACKGROUND: Increased prevalence of diabetes mellitus (DM) in primary hyperparathyroidism (PHPT) is established, but not glucose intolerance (GI), nor benefit from parathyroidectomy on GI. We determined these during management of a continuous series of patients with PHPT routinely followed after surgery. PATIENTS AND METHODS: WHO criteria classified 75 g oral glucose tolerance tests (OGTT) in 51/54 consecutively proven PHPT patients, into normal glucose tolerance (NGT), DM, impaired glucose tolerance (IGT) or impaired fasting glucose (IFG); GI was derived by adding those with DM and IGT/IFG. OGTT were repeated after parathyroidectomy (mean follow up 2.4 +/- SD 1.6 years). Paired student t tests were used to compare fasting and 2-h plasma glucose (PG). RESULTS: At presentation 32/54 patients (59%) had NGT, 10 IGT/IFG (19%) and 12 type 2 DM (22%), nine newly diagnosed. Before parathyroidectomy 17/35 patients had NGT (49%), 18 GI (51%), 12 DM (34%) and 6 IGT/IFG (17%). Five out of six patients with IGT/IFG had NGT, one with NGT developed IGT. At completion 23 patients (66%) had NGT, 12 GI (34%), 4 IGT/IFG (11%) and 8 DM (23%). After parathyroidectomy fasting and 2-h. PG fell in 30/34 normocalcaemic patients not on hypoglycaemic agents, 5.6 +/- 1.0 to 5.4 +/- 0.8 mmol/l, 7.2 +/- 3.0 to 6.3 +/- 3.1 mmol/l (p < 0.05, p < 0.01). CONCLUSIONS: 1.At presentation with PHPT, OGTT commonly identifies Type 2 DM and GI.2.After successful parathyroidectomy fasting and 2-h. PG fall significantly (p < 0.05, p < 0.01). DM and IGT/IFG often ameliorates to IGT or NGT, persistently.  相似文献

目的 比较糖耐量正常 (NGT)、单纯空腹血糖受损 (I IFG)、单纯糖耐量低减 (I IGT)、糖耐量低减合并空腹血糖受损 (IGT/IFG)、新诊断的 2型糖尿病 (2型DM ) 5种不同糖代谢状态的尿白蛋白排泄率 (UAE)和微量白蛋白尿 (MAU )患病率。方法 根据 75g口服葡萄糖耐量试验 (75gOGTT)结果 ,将 2 93 4例受试者分为 :NGT组 13 3 2例、I IFG组 186例、I IGT组 470例、IGT/IFG组 2 3 6例、新诊断的 2型DM组 710例。用放射免疫法测定过夜 12h尿白蛋白。UAE在 2 0~ 2 0 0μg/min之间定义为MAU。 结果  (1)UAE水平 [中位数 (四分位数 ) ] ,在新诊断的 2型DM组为8 50 (4 89~ 15 95) μg/min、IGT/IFG组为 6 93 (4 85~ 10 89) μg/min、I IGT组为 6 51(4 0 9~10 74) μg/min ,均高于I IFG组的 5 56(3 70~ 9 2 3 ) μg/min(P值均 <0 0 1) ;I IFG组与NGT组的 5 2 6(3 50~ 8 12 ) μg/min比较差异无显著性 (P >0 0 5) ;MAU的患病率在新诊断的 2型DM组为 2 0 7%、IGT/IFG组为 13 1%、I IGT组为 11 7%、I IFG组为 5 8%、NGT组为 5 6% ,同样呈现上述变化规律。(2 )多元逐步回归分析显示 :UAE与OGTT 2h血糖、舒张压、体重指数呈现独立正相关。logistic回归分析显示 ,导致MAU危险性增加的因素有OGTT 2h血糖、舒张  相似文献

目的 探讨糖调节受损人群发生糖尿病的危险性及其影响因素.方法 2002年6月至8月,采用抽样分层横断面现场调查方法于上海市浦东新区社区中检出192例糖调节受损者.2007年同期进行随访,观察单纯糖耐量受损、单纯空腹血糖受损、糖耐量受损合并空腹血糖受损、糖尿病人群比例,变化.使用t检验、方差分析、卡方检验进行统计学分析.结果 5年中死亡5例,存活的187例中79例发生糖尿病,糖尿病年转化率为8.4%(79/187/5年).基线单纯糖耐量受损者糖尿病年转化率为8.2%(47/114/5年),基线单纯空腹血糖受损者精尿病年转化率为6.3%(12/38/5年),基线糖耐量受损合并空腹血糖受损者糖尿病年转化率为11.4%(20/35/5年).正常糖耐量年转化率为6.3%(59/187/5年).单纯糖耐量受损者和糖耐量受损合并空腹血糖受损者发生糖尿病的风险分别是单纯空腹血糖受损者的1.4倍(95%可信区间为0.753~2.508,P=0.29)和107倍(95%可信区间为1.021~2.80,P=0.028).转化为糖尿病者较未转化为精尿病者高血压、肥胖的比例增高,基线年龄、体重指数、腰围、腰臀比和2 h血精明显升高,随访结束时体重指数和收缩压较基线时增高.结论 上海市浦东新区糖耐量受损合并空腹血糖受损者糖尿病转化率最高.多种代谢异常可促进糖调节受损人群向糖球病转化.  相似文献

BACKGROUND: Impaired glucose tolerance (IGT) and impaired fasting glucose (IFG) are both intermediate stages that exist between normal glucose tolerance and overt type 2 diabetes. Epidemiological studies demonstrated that the two categories define distinct populations. In this study, we examined the contributions of insulin resistance and beta-cell dysfunction to both states in overweight subjects of Arab origin. METHODS: Twelve subjects with isolated IFG, 10 with isolated IGT, and 20 with IFG and IGT (combined glucose in tolerance-CGT) were compared with 30 subjects with normal glucose tolerance (NGT) subjects; all were of Arab origin and were overweight or obese. Different indices for insulin resistance and beta-cell function were calculated from oral glucose tolerance (OGTT) values. RESULTS: Subjects with isolated IFG and CGT were more obese and had significantly higher values of insulin resistance than subjects with isolated IGT and NFG. There was no significant difference between the insulin resistance in subjects with isolated IGT and that in subjects with NGT. Indices of beta cell function were severely reduced among subjects with isolated IGT and CGT when compared with those with both isolated IFG and NGT, while subjects with isolated IFG had similar beta-cell indices to subjects with NGT. CONCLUSION: These data demonstrate that beta-cell dysfunction and insulin resistance contribute differently to the pathogenesis of IFG and IGT among overweight Arab subjects.  相似文献

CONTEXT: Autonomic dysfunction is present in diabetes mellitus (DM), but no study is available on alteration in cardiac autonomic function (CAF) across different glycemic statuses including normal glucose tolerance (NGT), isolated impaired fasting glucose (IFG), impaired glucose tolerance (IGT), and DM. OBJECTIVE: Our objective was to examine whether CAF is altered in subjects with IGT and isolated IFG. DESIGN AND SETTING: The study was a stratified systematic cluster sampling design within the general community. PARTICIPANTS: A total of 1440 subjects were classified as NGT (n = 983), isolated IFG (n = 163), IGT (n = 188), and DM (n = 106). MAIN OUTCOME MEASURE: CAF was determined by 1) standard deviation of normal-to-normal (SDNN) or RR interval, power spectrum in low and high frequency (LF, 0.04-0.15 Hz; HF, 0.15-0.40 Hz), and LF/HF ratio in supine position for 5 min; 2) ratio between 30th and 15th RR interval after standing from supine position (30/15 ratio); and 3) average heart rate change during breathing of six deep breaths for 1 min (HR(DB)). RESULTS: Univariate analysis showed significant differences in SDNN, 30/15 ratio, HR(DB), HF power, and LF/HF ratio among subjects with NGT, isolated IFG, IGT, and DM. In multivariate analysis, none of the indices of CAF was related to isolated IFG in the reference group of NGT. IGT and DM were negatively associated with 30/15 ratio and HF power but positively associated with LF/HF ratio. In addition, DM was also related to a lower SDNN. CONCLUSIONS: DM and IGT subjects had an impaired CAF independent of other cardiovascular risk factors. The risk of altered CAF is not apparent in subjects with isolated IFG.  相似文献

Impaired fasting glucose (IFG) like impaired glucose tolerance (IGT) has increased risk of progressing to diabetes mellitus (DM). The aim of the study was to evaluate prevalance of IGT and type 2 DM with oral glucose tolerance test (OGTT) in Turkish patients who had fasting glucose of 110 and 125 mg/dl. Hundred and forty-eight (67.3%) women and 72 (32.7%) men (30-65 years old with mean age of 51.3 +/- 8.7 year) who had fasting glucose range 110-125 mg/dl were evaluated with OGTT. Seventy-two patients had IGT (32.8%), 74 (33.6%) patients had type 2 diabetes and 74 (33.6%) patients had normal glucose tolerance (NGT). Mean fasting glucose and insulin levels were higher in the IGT group than in the NGT group. Mean level of total cholesterol was higher in DM than that in NGT and IGT groups. Mean triglyceride (TG) (P = 0.476), high-density lipoprotein (HDL) (P = 0.594), low-density lipoprotein (LDL) (P = 0.612), Apoproteine A (P = 0.876), Apoproteine B (P = 0.518), uric acid (P = 0.948) and ferritin (P = 0.314) were found higher in diabetic patients. Lipoproteine a (P = 0.083), fibrinogen (P = 0.175) and hsCRP (P = 0.621) levels were higher in IGT. Mean HOMA S% levels of NGT, IGT and DM were found to be 65.0 +/- 13.0%, 60.9 +/- 16.0% and 50.1 +/- 11.1%, respectively. HOMA B% levels were measured to be 80.4 +/- 29.1% in NGT, 85.3 +/- 14.59% in IGT and 60.1 +/- 10.1% in DM. Significant difference was found between IFG and DM (P = 0.043) groups. The prevalences of diabetes and IGT were found to be 33.63 and 32.7% in IFG, respectively.  相似文献

To determine the prevalence of diabetes, glucose intolerance and impaired fasting glucose in Mexican patients with acromegaly and establish associations with clinical, anthropometric and biochemical variables. 257 patients with acromegaly were evaluated by a 75 g-oral glucose tolerance test with measurements of both GH and glucose (0, 30, 60, 90 120 min) as well as baseline IGF-1. Normal glucose tolerance (NGT), impaired fasting glucose (IFG), impaired glucose tolerance (IGT) and diabetes (DM) were defined based on the 2003 ADA criteria. NGT, IFG, IGT and DM were found in 27.6, 8.9, 31.6 and 31.9% of the subjects, respectively; 42 of the DM patients were unaware of the diagnosis. Patients with diabetes were older than subjects in the other 3 categories (P = 0.001), and the proportion of women was significantly higher in the DM (74%) and IGT (68%) groups than in the NGT group (52%) (P = 0.004). Odds ratio for the development of DM was 3.29 (95% CI 3.28–3.3). GH and IGF-1 levels were comparable among the different groups. In a multivariable analysis DM was significantly associated with age, presence of a macroadenoma, disease duration and a basal GH > 30 μg/dl. DM and probably IGT are more prevalent in acromegaly than in the general Mexican population. DM was more frequent in females of all ages, in subjects with severely elevated GH concentrations, in patients with macroadenomas, and long-standing disease duration. The odds ratio for DM in our subjects with acromegaly is more than 3 times higher than in the general population.  相似文献

BACKGROUND: Impaired glucose tolerance (IGT) is associated with an increased cardiovascular disease risk. Less is known about cardiovascular disease risk among subjects with impaired fasting glucose (IFG) or with combined IFG and IGT. AIMS: To compare body composition, body fat distribution, plasma glucose-insulin homeostasis and plasma lipid-lipoprotein profile between pre-menopausal women having either a normal glucose tolerance (NGT), isolated IFG, isolated IGT or combined IFG and IGT. METHODS: Three hundred and thirty-four women with NGT, 11 women with IFG, 35 women with IGT and 10 women with both IFG and IGT were studied. RESULTS: Women with IFG were characterized by a higher visceral adipose tissue (AT) accumulation than women with NGT (P < 0.05). Also, they were characterized by a higher subcutaneous AT area and by higher body fat mass than NGT and IGT women (P < 0.05). However, their lipid-lipoprotein profile was comparable with that of NGT women, except for reduced HDL-cholesterol concentrations (P < 0.05). After adjustment for visceral AT, women with IFG had lower total cholesterol, LDL-cholesterol and apolipoprotein B (apoB) levels than the three other groups. They also had lower HDL(2)-cholesterol than NGT women and lower total cholesterol/HDL-cholesterol ratio than IGT women. Women with IGT showed higher triglyceride and apoB concentrations and a higher total cholesterol/HDL-cholesterol ratio than women with NGT (P < 0.05). Overall, women with combined IFG and IGT showed body fatness characteristics and alterations in their metabolic risk profile which were essentially similar to women with isolated IGT. CONCLUSIONS: These results indicate that there are significant differences in anthropometric and metabolic variables between pre-menopausal women with IFG vs. IGT and that the association between body fatness-body fat distribution indices and the metabolic profile may differ between IFG and IGT women.  相似文献

Aims To investigate changes in insulin action and insulin secretion in obese subjects with different categories of impaired glucose regulation (IGR): impaired glucose tolerance (IGT), impaired fasting glucose (IFG), and combined IFG/IGT (CGI). Methods A total of 222 subjects underwent an oral glucose tolerance test and a frequently sampled intravenous glucose tolerance test (FSIGTT); 100 had normal glucose tolerance (subdivided into 32 lean NGT, 68 obese NGT), and 122 were obese with IGR (82 IGT, 14 IFG and 26 CGI). The insulin sensitivity index (SI) was assessed by Bergman's minimal model method with FSIGTT; insulin secretion was determined by acute insulin response to glucose (AIRg). The disposition index (DI), the product of AIRg and SI, was used to determine whether AIRg was adequate to compensate for insulin resistance. Results SI was similar in NGT and IGR obese subgroups. AIRg was significantly increased in obese NGT as compared with lean NGT, significantly reduced in IGT, and further reduced in IFG and CGI subjects as compared with obese NGT subgroups. DI was reduced in NGT obese individuals. Within the obese IGR subgroups, IFG and CGI subjects had even lower DI value than IGT subjects. Conclusions Obese Chinese subjects with IGR have a similar degree of insulin resistance but differ in insulin secretion. Subjects with IFG and CGI have a more prominent deficiency in insulin secretion than subjects with IGT.  相似文献

目的 分析中老年正常糖耐量人群血糖水平与胰岛B细胞分泌功能的关系.方法 选择上海市部分社区流行病学调研2095例居民,根据口服葡萄糖耐量试验(OGTT)、空腹血糖(FPG)和餐后2 h血糖(2 hPG)结果,将受检者分为正常糖耐量、糖耐量减低(IGT)、空腹血糖受损(IFG)、糖耐量减低合并空腹血糖受损(IFG/IGT)及糖尿病组.再将正常糖耐量者按年龄及空腹血糖值进行分组,观察稳态胰岛β细胞功能指数(HBCI),并对各组年龄、血糖与胰岛β细胞分泌功能指标进行统计学分析.结果 (1)随着年龄的增长,FPG逐渐升高,40~49岁、50~59岁及60~69岁组分别为(5.00±0.47)mmol/L、(5.09±0.44)mmol/L及(5.17±0.48)mmol/L,50~59岁组与40~49岁组比较(t=2.727,P<0.01)、60~69岁组与50~59岁组比较(t=2.303,P<0.05),均差异有统计学意义,但空腹胰岛素(FINS)值变化无明显规律;(2)随着年龄的增长,HBCI值呈下降趋势(F=33.75,P<0.05);(3)FPG≥5.0 mmol/L组较<5.0 mmol/L组HBCI值下降,分别为4.39±0.58和4.22±0.70,差异有统计学意义(t=2.974,P<0.05).结论 中老年正常糖耐量者随着年龄增长,空腹血糖增高;当空腹血糖≥5.0 mmol/L时,可能存在胰岛β细胞分泌功能异常.  相似文献

按口服葡萄糖耐量试验结果,将研究对象分为正常糖耐量(NGT)组、单纯空腹血糖受损(IFG)组、单纯糖耐量受损(IGT)组、IFG与IGT并存(IFG/ICT)组及2型糖尿病(DM)组。采用酶联免疫吸附法(ELISA)分别检测上述人群的血清可溶性细胞间黏附分子1(SICAM-1)、可溶性血管细胞黏附分子1(SVCAM-1)和超敏C反应蛋白水平(hr-CRP),并与糖脂代谢指标和稳态模型胰岛素抵抗指数(HOMA- IR)作相关性分析。结果显示,血清SICAM-1、SVCAM-1和hs-CRP水平按分组顺序逐组升高,提示2型DM的发病过程与内皮细胞损伤和炎症状态有关。  相似文献

OBJECTIVE: The aim of the present study was to determine the effects of glucose intolerance on oxidative stress in patients with coronary artery disease (CAD). METHODS: The patients were divided into 3 groups, diabetes mellitus (DM), IGT or normal glucose tolerance (NGT) according to the criteria of the American Diabetes Association. PATIENTS: The present study consisted of 178 consecutive patients who underwent diagnostic coronary arteriography and a 75-g glucose tolerance test. RESULTS: The level of plasma thioredoxin, a marker of oxidative stress was measured in every patient during the fasting state. The levels of plasma thioredoxin were significantly higher in the DM and IGT groups than the NGT group. Furthermore, we found that there was a positive association between thioredoxin levels and glycosylated hemoglobin (sigma=0.225, p=0.018). In multivariate logistic regression analysis, glucose intolerance (DM or IGT) was only independently associated with the high levels of thioredoxin. The levels of plasma thioredoxin were significantly higher in the CAD group compared to the non-CAD group. In multivariate logistic regression analysis, high levels of thioredoxin, male, age and hypertension were independently associated with the presence of CAD. CONCLUSION: Glucose intolerance was associated with the high levels of thioredoxin. High levels of thioredoxin were related to the presence of CAD. The measurement of thioredoxin as the marker of oxidative stress may be useful for monitoring the development of the cardiovascular diseases.  相似文献

目的用口服葡萄糖耐量试验中各点血糖和胰岛素的值来计算反映胰岛素敏感性及β细胞功能的参数,回顾性研究住院高血压病人糖代谢情况。方法根据WHO和美国糖尿病协会标准计算血糖分布情况,去除新诊断的糖尿病病人后,分成正常血糖(NGT)、单纯性空腹血糖升高(IFG)、单纯性餐后血糖升高(IGT)和空腹、餐后血糖均升高(IFG,/IGT)组进行比较。再分别以口服75g葡萄糖后30min或60min血糖正常值为标准对NGT组和IGT组进行分组。用HOMA-IR和Composite胰岛素敏感性指数(ISI)计算胰岛素敏感性,HOMA-B和△I/AG计算β细胞功能。结果1193例住院的原发性高血压病人中,新诊断的糖尿病病人为11.1%,其中57.9%仅有餐后血糖升高。IGT、和IFG/ICT组的HOMA-IR高于NGT组,Composite ISI和AI/AG低于NGT组。无论是否30min或60min血糖升高,IGT组的Composite ISI均低于30min和60min血糖正常的NGT组。30min和(或)60min血糖升高的NGT组△I/AG低于30min和60min血糖正常的NGT组。结论IGT或IFG/IGT的高血压患者同时存在空腹和总体胰岛素敏感性的下降和糖负荷后早期β细胞分泌功能的受损。30min和(或)60min血糖升高的NGT高血压病人存在糖负荷后早期β细胞分泌功能的受损。  相似文献

胰岛素抵抗对非ST段抬高急性冠脉综合征患者的影响   总被引:1,自引:1,他引:1  
目的 探讨胰岛素抵抗 (IR)对老年非ST段抬高急性冠脉综合征 (ACS)患者临床表现的影响。 方法  70例老年非ST段抬高的ACS患者 ,分为血糖正常 (NGT)组、糖耐量低减 (IGT)组及糖尿病 (DM )组。所有患者详细询问病史 ,检测血肌钙蛋白 (CTnT)、血糖、血脂、空腹血胰岛素等 ,计算胰岛素敏感性指数 (IAI)。冠脉病变用冠状动脉造影 (CAG)分析 ,心功能用超声心动图测左室射血分数 (LVEF)评定。  结果 IGT组与DM组间除空腹血糖及餐后 2h血糖、IAI有显著差异外 (P <0 0 1) ,其他均无显著差异 ;IGT组及DM组 (IR相对严重组 )与NGT组 (IR相对较轻组 )比较 :多支冠脉病变的发生率 (71 4%、67 7%比 2 5 0 % )、冠脉远端病变率 (4 2 9%、42 9%比 14 3 % )、心功能减退 (71 4%、76 2 %比 3 2 1% )、3 0d内心脏事件发生率 (3 3 3 %、2 8 6%比 3 6% )、CTnT阳性率 (4 7 6%、5 7 1%比 2 5 0 % ) ,均有显著差异 (P <0 0 1) ,而IGT与DM组间各项均无显著差异。  结论  IR相对较重患者多支病变的发生率明显升高 ,病变多呈弥漫性 ,远端血管病变发生率高 ,CTnT阳性率高 ,心功能较差 ,预后不良。  相似文献
