目的 探讨脑血管意外的病变性质及不同部位对心电图及心肌酶谱的影响。方法 对264例急性脑血管意外患者脑部病变的不同性质及部位引起心电图、心肌酶谱异常现象进行分析。结果 264例急性脑血管意外患者,60.6%(160/264)心电图改变,32.6%(86/264)心肌酶谱改变。急性脑血管意外(脑出血或脑梗死)性质对心电图及心肌酶谱的影响差异无显著性(P〉0.05)。引起心肌酶谱升高,尤以CPK及CPK—MB升高明显,但病变部位对心肌酶谱的影响差异无差异性(P〉0.05)。结论 急性脑血管意外可引起心肌损害,需常规检查心电图,检测心肌酶谱,临床予以重视。  相似文献

现将我院 1998- 12~ 2 0 0 2 - 12收治的急性脑卒中并有完整记录的 12 0例急性期 ECG改变分析如下。1 对象和方法本组均符合全国第四次脑血管病学术会议制定的诊断标准 ,并经头颅 CT证实。发病前均无心脏病史。ECG在住院 2 4~4 8h内检测。其中脑出血 6 5例 ,蛛网膜下腔出血 2 5例 ,脑梗塞30例。年龄 39~ 85岁 ,平均 6 3.14岁。2 结果本组 12 0例中合并 ECG异常 77例 ,占 6 4 .17%。其中脑出血 5 2例 ,蛛网膜下腔出血 2 3例 ,脑梗塞 2例。 ECG异常类型 :ST段水平下移和 /或 T波低平、双向、倒置 4 5例 (占 5 8.4 4 % ) ,窦性心动…  相似文献

目的:观察电惊厥大鼠癫痫发作时及发作后心电图变化,并检测动态血清心肌肌钙蛋白Ⅰ的变化。方法:实验于2004-10/12在重庆医科大学附属第一医院中心实验室进行。选20只SD大鼠,用电惊厥仪致痫动物。刺激条件:最大电休克用交流电500Hz,50mA,0.3s。刺激后大鼠100%惊厥发作,以双后肢强直为指标,用标准Ⅱ导联记录发作前、发作时以及发作后的心电图变化,用化学发光法动态检测大鼠惊厥前及惊厥后6,12,24h血清心肌肌钙蛋白水平的变化。结果:20只大鼠均进入结果分析。①血清心肌肌钙蛋白水平:惊厥6h即增高,12h达高峰,24h下降,均高于惊厥前[(9.51&;#177;3.68),(21.7&;#177;6.53),(7.26&;#177;1.95),(0.48&;#177;0.31)μg/L,P〈0.01]。②心电图表现:表现为伴有窄QRS复合波的窦性心动过速。异常主要表现为ST段下降、ST段抬高、T波倒置以及心房纤颤。这种异常改变在癫痫发作停止后仍持续两三分钟。③心率:惊厥时较惊厥前明显增快[(467.6&;#177;56.8),(373.5&;#177;41.6)次/min,t=5.39,P〈0.01],平均增加86.6次/min。结论:电惊厥大鼠癫痫发作可引起心电图异常改变。心肌肌钙蛋自在惊厥后的升高说明癫痫发作可引起心脏结构性损害。  相似文献

目的观察急性脑血管意外患者心电图的变化情况。方法216例急性脑血管意外患者入院后做心电图。结果216例急性脑卒中患者174例有心电图异常(80.6%),出血性卒中的心电图异常率为92.31%,高于缺血性卒中组(71.09%)。治疗后,部分患者的心电图有不同程序的恢复。结论急性脑血管意外患者应加强心脏功能的监护,监测心电图,使可逆性的心肌损害得到恢复。  相似文献

目的评价心肌酶谱及肌钙蛋白对有机磷农药中毒患者心肌损害的诊断价值。方法检测60例不同急性有机磷中毒患者及30例健康对照组的心肌酶谱及肌钙蛋白水平,并进行比较。结果不同程度的中毒患者血清肌钙蛋白和心肌酶谱随病情的加重而增加。结论心肌酶谱及肌钙蛋白反映急性有机磷农药中毒患者的心肌损害,尤以肌钙蛋白为佳。  相似文献

目的探讨D-二聚体(D-D)、肌钙蛋白T(cTnT)及心肌酶谱检测在急性心肌梗死(AMI)临床诊断中的价值。方法选取AMI患者36例(AMI组)和体检健康者20例(健康对照组)作为研究对象,检测两组血浆D-D、cTnT、心肌酶谱水平及其灵敏度、特异度。结果 AMI组D-D、cTnT、肌酸激酶(CK)、肌酸激酶同工酶MB(CK-MB)、天门冬氨酸氨基转移酶(AST)、乳酸脱氢酶(LDH)水平均明显高于健康对照组,差异均有统计学意义(P0.05)。AMI患者中,cTnT、D-D、心肌酶谱的阳性率分别为91.67%、69.44%、66.67%,cTnT阳性率明显高于D-D和心肌酶谱(P0.05)。血浆cTnT对AMI的诊断灵敏度和特异度明显高于D-D和心肌酶谱(P0.05)。结论与D-D和心肌酶谱相比,cTnT对AMI的诊断灵敏度和特异度更好,临床上可多项联合检测提高检测准确性。  相似文献

ObjectiveTo externally validate a clinical decision rule incorporating heart fatty acid binding protein (h-FABP), high-sensitivity troponin (hs-cTn) and electrocardiogram (ECG) for the detection of acute myocardial infarction (AMI) on presentation to the Emergency Department. We also investigated whether this clinical decision rule improved identification of AMI over algorithms incorporating hs-cTn and ECG only.Design and methodsThis study included data from 789 patients from the Brisbane ADAPT cohort and 441 patients from the Christchurch TIMI RCT cohort. The primary outcome was index AMI. Sensitivity, specificity, positive predictive value and negative predictive value were used to assess the diagnostic accuracy of the algorithms.Results1230 patients were recruited, including 112 (9.1%) with AMI. The algorithm including h-FABP and hs-cTnT had 100% sensitivity and 32.4% specificity. The algorithm utilising h-FABP and hs-cTnI had similar sensitivity (99.1%) and higher specificity (43.4%). The hs-cTnI and hs-cTnT algorithms without h-FABP both had a sensitivity of 98.2%; a result that was not significantly different from either algorithm incorporating h-FABP. Specificity was higher for the hs-cTnI algorithm (68.1%) compared to the hs-cTnT algorithm (33.0%). The specificity of the algorithm incorporating hs-cTnI alone was also significantly higher than both of the algorithms incorporating h-FABP (p < 0.01).ConclusionFor patients presenting to the Emergency Department with chest pain, an algorithm incorporating h-FABP, hs-cTn and ECG has high accuracy and can rule out up to 40% of patients. An algorithm incorporating only hs-cTn and ECG has similar sensitivity and may rule out a higher proportion of patients. Each of the algorithms can be used to safely identify patients as low risk for AMI on presentation to the Emergency Department.  相似文献

目的建立大鼠急性心肌梗死模型,观察心肌梗死后心室重塑过程中肌浆网钙转运ATP酶(Ca-2+ATPase,SERCA)和受磷蛋白(PLB)的表达变化。方法雄性SD大鼠,应用前降支动脉结扎法建立大鼠心肌梗死模型,术后4周观察血流动力学参数,SERCA mRNA和PLB mRNA的表达。结果大鼠心肌梗死后4周,心肌梗死组左心室舒张末压(LVEDP)显著大于假手术组,左心室内压最大上升和下降速率(±dp/dtmax)显著低于假手术组。心肌梗死组心肌组织中SERCA mRNA表达下调,PLB mRNA表达上调。结论大鼠心肌梗死4周后心室收缩和舒张功能已降低,进入了心力衰竭阶段,SERCA mRNA和PLB mRNA的表达变化与心力衰竭的发生发展相关,可能是心肌梗死后心肌收缩和舒张功能失调的重要机制。  相似文献

Cardiac troponin T in diagnosis of acute myocardial infarction   总被引:26,自引:0,他引:26  
Troponin T is a structurally bound protein found in striated muscle cells. We tested concentrations of its cardiac-specific isotype in peripheral venous blood samples serially drawn from 72 patients with confirmed myocardial infarction. Fifty-nine patients received thrombolytic treatment with intravenous streptokinase, urokinase, or recombinant tissue-type plasminogen activator; because of contraindications, the remaining 13 patients did not. Concentrations of troponin T in plasma, measured by an enzyme-linked immunosorbent assay, started increasing within a few hours after the onset of symptoms (median, 4 h; range, 1-10 h). The sensitivity of troponin T for detecting myocardial infarction was 100% from 10 to 120 h after the onset of symptoms; sensitivity on the seventh day after admission was 84%. Concentrations were increased for up to three weeks in some patients with late or high peak values. Successful reperfusion in Q-wave infarction obviously influences the release of troponin T into plasma, with all such cases showing peak values less than or equal to 26 h (median, 14 h) after the onset of symptoms. Troponin T concentrations in these patients returned to within the reference interval more rapidly than in nonreperfused subjects. In the 13 patients without fibrinolytic therapy, troponin T tended to peak approximately 48 h (median) after the onset of chest pain. Troponin T concentrations in patients for whom thrombolysis was unsuccessful resembled those in patients without fibrinolytic therapy. The specificity of the assay was 96% as tested in samples of 96 emergency-room patients. The reference interval (less than 0.5 micrograms/L) was established from samples of 100 healthy blood donors. Troponin T measurements are a specific and sensitive method for the early and late diagnosis of acute myocardial infarction and could, therefore, provide a new criterion in laboratory diagnosis of its occurrence.  相似文献

目的:探讨心肌肌钙蛋白I(cTnI)在急性心肌梗死(AMI)时的检测意义。方法:比较102例冠心病患者的cTnI和心肌酶谱的测定结果。结果:AMI患者的阳性率分别为:cTnI 87.5%、CK-MB62.5%、CK56.3%、AST46.9%、LDH40.6%。结论:AMI患者胸痛后10h血清cTnI的敏感性最高,血清cTnI对AMI诊断的特异性明显高于心肌谱(CK、Cl-MB、AST和LDH),对诊断AMI有重大临床价值。  相似文献
