目的 研究中国汉族健康人群中脊髓小脑性共济失调(spinocerebellar ataxias,SCA)1、2、3、6、7、8、10、12、17亚型和齿状核红核苍白球路易体萎缩(dentatorubral-pallidoluysian atrophy,DRPLA)亚型致病基因多核苷酸正常重复次数的范围,以助于SCA各亚型基因诊断平台的建立和诊断标准的确认.方法 应用荧光聚合酶链反应、毛细管凝胶电泳等技术对300名健康个体进行SCA 1、2、3、6、7、8、10、12、17亚型和DRPLA亚型致病基因多核苷酸正常重复次数的检测分析.结果 在300名健康个体中CAG三核苷酸在SCA1亚型中为17~35次,在SCA2亚型中为14~28次,在脊髓小脑性共济失调3型/Machado-Joseph病,SCA3/Machado-Joseph disease,SCA3/MJD亚型中为13~41次,在SCA6亚型中为4~16次,在SCA7亚型中为5~17次,在SCA12亚型中为5~21次,在SCA17亚型中为23~41次,在DRPLA亚型中为12~33次;在SCA8亚型中CTA/CTG三核苷酸的重复次数为12~43次;在SCA10亚型中ATTCT五核苷酸的重复次数为9~32次.其中SCA8的12次CTA/CTG三核苷酸重复,SCA10的9次ATTCT五核苷酸重复,SCA17的23次CAG三核苷酸重复,为目前国内外报道的3种亚型多核苷酸正常重复次数的最小次数.SCA3/MJD亚型的41次CAG三核苷酸重复,SCA10亚型的32次ATTCT五核苷酸重复为目前报道的2种亚型多核苷酸正常重复次数的最大次数.结论 SCA各亚型多核苷酸正常次数范围存在种族差异,与遗传和种族背景相关;首次建立了中国汉族人群10种不同SCA亚型正常核苷酸重复次数范围,可为SCA的诊断提供参考标准.  相似文献

目的 对东北地区 110名汉族正常人 SCA1及 SCA3/ MJD基因 (CAG) n拷贝进行检测 ,探讨其正常变异范围 ,并对临床诊断为遗传型脊髓小脑共济失调的 8个家系的 2 5例患者和 6个散发病例进行基因分型评价和症状前及产前诊断。方法 应用荧光 - PCR方法测定不同基因型片段长度 ,并进行 DNA序列分析。结果  SCA3/ MJD基因 (CAG) n正常变异范围为 14~ 38个拷贝 ,集中于 14个拷贝 ,其等位基因频率为 39.5 5 % ,杂合频率为 78.18% ,共 13种等位基因。检出一个家系先证者携带有 (CAG) 6 8的 SCA3/ MJD基因 ,并对该家系成员进行了症状前诊断 ,没有发现 (CAG) n拷贝异常突变 ;SCA1基因内 (CAG) n正常变异范围 2 0~ 39拷贝 ,集中于 2 6及 2 7次 ,等位基因频率分别为 34.0 9%和 2 0 .91% ,杂合频率为 84 .5 5 % ,共 13种等位基因 ;散发病例未检出 CAG扩展性突变。结论  SCA1及 SCA3/ MJD基因中 (CAG) n正常变异范围存在地区和种族差异 ,SCAs基因分型是该病症状前及产前诊断的首选策略。  相似文献

目的 探讨齿状核红核苍白球路易氏体萎缩症(dentatorubral-pallidoluysian atrophy,DRPLA)患者的临床特征和基因突变特点.方法 应用基于荧光标记的毛细管电泳片段分析法对708个常染色体显性遗传脊髓小脑共济失调(spinocerebellar ataxias,SCA)家系的先证者和119例临床拟诊为SCA的散发患者进行DRPLA基因CAG重复次数分析.结果 共检出3例患者存在DRPLA基因CAG重复扩展突变.片段分析显示其CAG重复次数分别为16/58、16/58和14/54次,长片段重复次数达到异常范围.3个家系的先证者均为成年起病,以共济失调为主要症状,患者可伴发抽搐、颈部扭转等表现.结论 在827例共济失调病例中仅发现3例DRPLA,说明该病在中国人群中较为罕见.DRPLA的临床表现复杂多样,存在变异.对DRPLA患者的临床特征和突变特点的细致分析有助于同其他SCA类型进行鉴别诊断.  相似文献

目的 研究中国汉族人群脊髓小脑性共济失调(spinocerebellar ataxia,SCA)1、2、3、6、7、12、17亚型致病基因的CAG三核苷酸病理重复次数范围.方法 应用聚合酶链反应、琼脂糖凝胶电泳、T载体克隆重组DNA技术并结合直接测序等技术对559例临床诊断为SCA的患者(363例常染色体显性遗传先证者,196例散发患者)进行SCA1、SCA2、SCA3/马查多-约瑟夫病(Machado-Joseph disease,MJD)、SCA6、SCA7、SCA12和SCA17致病基因CAG三核苷酸病理重复次数突变分析.结果 在559例SCA患者中,共检测出SCA1患者23例,CAG病理重复次数范围39~60次,平均(51.09±4.88)次;SCA2患者32例,CAG病理重复次数范围36~51次,平均(40.34±4.40)次;SCA3/MJD患者305例,CAG病理重复次数范围49~86次,平均(73.84±5.07)次;SCA6患者9例,CAG病理重复次数范围23~29次,平均(25.56±1.94)次;SCA7患者27例,CAG病理重复次数范围38~71次,平均(58.22±10.90)次;SCA12患者3例,CAG病理重复次数范围51~52次,平均(51.33±0.58)次;SCA17患者2例,CAG病理重复次数范围53~55次,平均(54.00±1.41)次.结论 SCA1的39次CAG病理重复、SCA3/MJD的49次CAG病理重复和SCA12的51次CAG病理重复为国内或国外报道的最小CAG病理重复次数;SCA3/MJD的86次CAG病理重复为国内外报道的最大CAG病理重复次数;SCA17为国内首次发现的SCA亚型;首次建立中国汉族人群不同SCA亚型CAG三核苷酸病理重复次数范围标准.  相似文献

脊髓小脑性共济失调的分子遗传学诊断与临床应用   总被引:5,自引:1,他引:4  
目的研究分析脊髓小脑性共济失调(spinocerebellar ataxlas,SCA)的分子遗传学诊断及临床应用策略。方法收集临床诊断为SCA的36个家系43例患者、38例散发患者、60名健康家系成员,以及44名正常对照共185份标本,采用聚合酶链反应对三核苷酸重复(trinucleotide repeats,TNR)片段进行扩增.聚丙烯酰胺凝胶电泳并图像分析软件计算其长度,推算所有正常和异常扩增等位基因内TNR重复次数。结果我国南方汉族人群中SCA3是最常见的类型,占42.0%,其余分别为SCA2占7.4%,SCA1占4.9%,SCA7占3.7%,SCA6占2.5%.SCA12约占1.2%,未检出SCA8、SCA10、SCA17、齿状核红核苍白球路易体萎缩患者。结论分子遗传学检测可为SCA提供准确的分型诊断和症状前遗传学诊断。  相似文献

目的 研究中国正常人群脊髓小脑性共济失调(spinocerebellar ataxia,SCA)8型(SCA8)致病基因ATXN8OS(CTA/CTG)n正常变异范围,以及AIXN8OS基因(CTA/CTG)n扩展突变在中国大陆SCA患者中的分布.方法 应用荧光PCR、8%变性聚丙烯酰胺凝胶电泳和毛细管凝胶电泳等方法对132例已经排除SCA1、SCA2、SCA3、SCA6、SCA7、SCA12、SCA17和齿状核红核苍白球路易体萎缩(dentatorubral-pallidoluysian atrophy,DRPLA)的SCA患者及261名正常汉族对照人群进行ATXN8OS基因三核苷酸重复次数分析.结果 未发现ATXN8OS基因(CTA/CTG)n核苷酸扩展突变;132例SCA患者中ATXN8OS基因(CTA/CTG)n变异范围为17~47次,平均重复次数(24.20+4.57)次,18次重复最常见,纯合子35例,纯合率为26.5%;261名正常对照人群ATXN8OS基因(CTA/CTG)n正常变异范围为12~43次,均重复次数(24.04+4.53)次,18次重复最常见,纯合子70人,纯合率为26.8%.结论 SCA8在中国大陆为罕见的SCA亚型,中国大陆SCA8发病率低可能和正常人群较大(CTA/CTG)n重复次数的等位基因少见有关.  相似文献

目的探讨一个遗传性脊髓小脑型共济失调(SCA)大家系的遗传特点和基因突变分析。方法对一个遗传性脊髓小脑型共济失调(SCA)大家系进行家系调查,绘制系谱图,抽取家系成员外周血,采用聚合酶链反应和毛细管电泳对致病基因进行分析检测。结果该家系的遗传性脊髓小脑型共济失调(SCA)为常染色体显性遗传,6代45人中有15人为SCA患者,4人为携带致病基因的无症状患者。患者ATX3基因的CAG三核苷酸重复65-73次。结论该家系为常染色体显性遗传的SCA3型(SCA/MJD),患者基因突变检测分析显示异常扩增的CAG突变数与发病年龄呈明显的负相关。基因突变检测在疾病诊断和早期发现无症状患者方面有重要作用,从遗传生殖角度阻断该病的遗传有重要意义。  相似文献

目的 探讨浙江沿海脊髓小脑性共济失调的基因突变检测与临床表现.方法 对该家系18例患者的临床表现、头颅MRI等辅助检查资料分析,并与10名家系中未发病成员及12名非血缘的健康人进行SCA31MJD基因CAG三核苷酸重复数目比较.结果 家系18例患者均为SCA3/MJD型,同时检测出家系中未发病对照组有2例为SCA31MJD型基因携带者.产物测序结果家系对照组与健康对照组CAG重复数为14~27次;SCA患者CAG重复数为67~82次;SCA3/MJD携带者CAG重复数为28~45次.在现存三代18例患者中,每代均有患者,男女均受累,起病年龄平均38岁,以行走不稳、动作笨拙和言语含糊为突出表现,MRI检测结果小脑、脑干明显萎缩.结论 在我国沿海存在SCA3/MJD家系遗传.临床均以共济失调和构音障碍为突出,CAG重复数目检测可为基因诊断和症状前诊断提供依据.  相似文献

目的 探讨中国人遗传性痉挛性截瘫(hereditary spastic paraplegia,HSP)spastin基因的突变特点,为该病的基因诊断提供依据。方法 应用聚合酶链反应—单链构象多态性(PCR—single strand conformation polymorphism,PCR—SSCP)结合DNA序列分析方法,对22个常染色体显性遗传HSP家系的先证者和9例散发性HSP患者的spastin基因进行研究,对发现异常SSCP条带的家系内成员进行突变研究。结果 在22例常染色体显性遗传HSP家系的先证者和9例散发性HSP患者中发现异常SSCP条带6例,进行DNA序列分析,共发现3种spastin基因突变,为外显子8的T1258A和A1293G,外显子14的1667delACT或1668delCTA或1669delTAC,均未见报道,突变位点均位于spastin基因功能区域,其中两个家系存在同一种突变(T1258A),各突变家系内患者存在同样的异常SSCP条带。结论 中国人遗传性痉挛性截瘫患者存在spastin基因突变,该基因在中国人常染色体显性遗传的遗传性痉挛性截瘫家系中的突变率较低(18.2%),点突变是主要的突变形式,外显子8可能是中国人spastin基因的突变热点。  相似文献

Summary The autosomal dominant cerebellar ataxias are a heterogeneous group of neurodegenerative disorders characterized by slowly progressive cerebellar ataxia. Recently, among the ataxias, spinocerebellar ataxia type 1 (SCA1), Machado-Joseph disease (MJD) and dentatorubral-pallidoluysian atrophy have been found to be caused by expansion of a CAG trinucleotide repeat in the coding region of the disease genes. We have analyzed the CAG repeats of 67 patients from 47 families with dominantly inherited ataxia who lived in the Kinki area of Japan. The following results were obtained. First, 31 patients from 22 families were found to be positive for the MJD repeat expansion, indicating that MJD is the most common dominantly inherited ataxia in the Kinki area of Japan. Second, no SCA1 repeat expansion was found among the families studied. This presents a striking contrast to the fact that there are many families with SCA1 in Hokkaido and the Tohoku area of Japan. These findings suggest geographic variation in autosomal dominant cerebellar ataxias in Japan.  相似文献

Expansions of trinucleotide repeats have been discovered in spinocerebellar ataxia (SCA) types 1, 2, 6, 7, 12, and 17, Machado-Joseph disease (MJD/SCA3), and dentatorubropallidoluysian atrophy (DRPLA). However, the frequency of familial SCA in Japan remains unclear. The number of trinucleotide repeats was determined for 1,286 patients. Three hundred and thirty families (523 cases) were autosomal dominant group (A), and 165 families were positive for family history but not autosomal dominant group (B), while the remaining 598 cases were the sporadic group (C). The frequency of SCA subtypes in autosomal dominant group was: 1) 5.5% for SCA1; 2) 2.4% for SCA2; 3) 27.6% for MJD/SCA3; 4) 25.5% for SCA6; 5) 0.3% for SCA17; and 6) 7.3% for DRPLA. Abnormal expansion of SCA12 was not detected. Another 31.5% of the patients in the autosomal dominant group had unknown genetic abnormalities. Within group B, SCA6 was the most prominent and within the sporadic group MJD/SCA3 and SCA6 were the most common subtypes observed. The disease-free survival curve of SCA6 was different from that of other SCAs and the mean age at onset for SCA6 was found to be later than that of the other types. Regional differences were observed in the relative rate of SCA subtypes. MJD/SCA3 appears more common in the Kanto and Kyushu districts of Japan, whereas SCA6 is most common in the Chugoku district. In order to establish an effective social welfare system for SCA patients, clinical course and regional differences in the prevalence of SCA subtypes must be taken into consideration.  相似文献

Using a molecular diagnostic approach, we investigated 101 kindreds with autosomal dominant cerebellar ataxias (ADCAs) from the central Honshu island of Japan, including spinocerebellar ataxia type 1 (SCA1), spinocerebellar ataxia type 2 (SCA2), Machado–Joseph disease (MJD), dentatorubral and pallidoluysian atrophy (DRPLA) and spinocerebellar ataxia type 6 (SCA6). In our unselected series, MJD was the most common type of ADCA, accounting for 33.7% followed by DRPLA (19.8%), SCA2 (5.9%) and SCA6 (5.9%). No SCA1 mutations were identified. We analysed the clinical features of six molecular confirmed SCA6 kindreds: in each family, there was an expanded allele in the α1A-voltage dependent calcium channel comprising between 23 and 25 CAG repeats. The mean age at onset of symptoms was 43±13 years. The clinical features consisted predominantly of cerebellar ataxia, dysarthria and horizontal nystagmus, which was generally consistent with ADCA type 3. However several new clinical features were found in some patients: dramatic anticipation, rapid disease progression, severe ataxia associated with action tremor or action myoclonus, and very early onset, which are not described as the classical features of ADCA type 3.  相似文献

The spinocerebellar ataxias (SCAs) are clinically and genetically a heterogeneous group of neurodegenerative disorders. To date, eight different loci causing SCA have been identified: SCA1, SCA2, Machado-Joseph disease (MJD)/SCA3, SCA4, SCA5, SCA6, SCA7, and dentatorubropallidoluysian atrophy (DRPLA). Expansion of a CAG repeat in the disease genes has been found in five of these disorders. To estimate the relative frequencies of the SCA1, DRPLA, MJD, SCA2, and SCA6 mutations among Portuguese ataxia patients, we collected DNA samples from 48 ataxia families and performed polymerase chain reaction (PCR) amplification of the CAG repeat mutations on chromosomes 6p, 12p, 14q, 12q, and 19p, respectively. Fifty-five individuals belonging to 34 dominant families (74%) had an expanded CAG repeat at the MJD gene. In five individuals from two kindreds with a dominant pattern of inheritance (4%), an expanded CAG repeat at the SCA2 gene was found. In MJD patients, the normal allele size ranged from 13 to 41, whereas the mutant alleles contained 65 to 80 repeats. For the SCA2 patients, normal alleles had 22 or 23, while expanded alleles had between 36 and 47 CAG units. We did not find the SCA1, DRPLA, or SCA6 mutations in our group of families. The MJD mutation remains the most common cause of SCA in Portugal, while a small number of cases are caused by mutations at the SCA2 gene, and 22% are due to still unidentified genes. Am. J. Med. Recently, moderate (CAG)>20 repeat expansions in the alpha1A-voltage- dependent calcium channel gene (CACNL1A4) have been identified in a previously unmapped type of SCA which has been named SCA6. We investigated the (CAG)n repeat length of the CACNL1A4 gene in 733 patients with sporadic ataxia and in 46 German families with dominantly inherited SCA which do not harbor the SCA1, SCA2, or MJD1/SCA3 mutation, respectively. The SCA6 (CAG)n expansion was identified in 32 patients most frequently with late manifestation of the disease. The (CAG)n stretch of the affected allele varied between 22 and 28 trinucleotide units and is therefore the shortest trinucleotide repeat expansion causing spinocerebellar ataxia. The (CAG)n repeat length is inversely correlated with the age at onset. In 11 parental transmissions of the expanded allele no repeat instability has been observed. Repeat instability was also not found for the normal allele investigating 431 meioses in the CEPH families. Analyzing 248 apparently healthy octogenerians revealed one allele of 18 repeats which is the longest normal CAG repeat in the CACNL1A4 gene reported. The SCA6 mutation causes the disease in approximately 10% of autosomal dominant SCA in Germany. Most importantly, the trinucleotide expansion was observed in four ataxia patients without obvious family history of the disease which necessitates a search for the SCA6 (CAG)n expansion even in sporadic patients.   相似文献

DNA tests in normal subjects and patients with ataxia and Parkinson's disease (PD) were carried out to assess the frequency of spinocerebellar ataxia (SCA) and to document the distribution of SCA mutations underlying ethnic Chinese in Taiwan. MJD/SCA3 (46%) was the most common autosomal dominant SCA in the Taiwanese cohort, followed by SCA6 (18%) and SCA1 (3%). No expansions of SCA types 2, 10, 12, or dentatorubropallidoluysian atrophy (DRPLA) were detected. The clinical phenotypes of these affected SCA patients were very heterogeneous. All of them showed clinical symptoms of cerebellar ataxia, with or without other associated features. The frequencies of large normal alleles are closely associated with the prevalence of SCA1, SCA2, MJD/SCA3, SCA6, and DRPLA among Taiwanese, Japanese, and Caucasians. Interestingly, abnormal expansions of SCA8 and SCA17 genes were detected in patients with PD. The clinical presentation for these patients is typical of idiopathic PD with the following characteristics: late onset of disease, resting tremor in the limbs, rigidity, bradykinesia, and a good response to levodopa. This study appears to be the first report describing the PD phenotype in association with an expanded allele in the TATA-binding protein gene and suggests that SCA8 may also be a cause of typical PD.  相似文献
