Objective The aim of this study was to investigate the relationship between obesity [measured by body mass index(BMI), waist circumference(WC), waist-to-hip ratio(WHR)]and arterial stiffness on community populations in Beijing area. Methods In a cross-sectional study of 2664 subjects(1379 men and 1285 women)aged(53.19 ± 15.73 years, mean±standard deviation), BMI, WC, WHR and other cardiovascular risk factors, were measured and carotidfemoral pulse wave velocity(cfPWV)was measured to assess the arterial stiffness. All the data were analyzed with linear correlation analysis, univariate analysis and stepwise regression method to explore the relationship between obese indexes and arterial stiffness. Results We noticed that a showed the existence of positive correlations between BMI(r=0.0829,P<0.01)and cfPWV, WC(r=0.2659, P<0.01)and cfPWV, WHR(r=0.2749, P<0.01)and cfPWV, in univariate analysis,cfPWV was associated with WC≥85 cm(male)or ≥80 cm(famale)(P<0.01), WHR≥0.90(male)or≥ 0.85(famale)(P< 0.01). Stepwise regression analysis revealed that WC was an independent risk factor of cfPWV, other than age, gender, systolic blood pressure, HDL-C, OGTT2h etc. Conclusion Our findings indicated that obesity was an independent risk factor of cfPWV, which was an early marker of cardiovascular and renal diseases, among community population in Beijing area. There were different relationships between obesity measurement parameters(BMI, WC, WHR)and cfPWV.  相似文献

目的 探讨社区人群肥胖及其不同测量指标与动脉僵硬度的相关性.方法 北京市4个社区参加体检的2664名经知情同意的研究对象(男性1379名,女性1285名),平均年龄(53.19±15.73)岁,测量身高、体重、腰围(WC)、臀围,并计算其体重指数(BMI)、腰围臀围比(WHR),应用自动PWV分析仪测定颈-股动脉脉搏波传导速度(cfPWV),分析不同肥胖指标(BMI、WC、WHR)与cfPWV的关系.结果 2664名受试者中,BMI(r=0.0829,P<0.01)、WC(r=0.2659,P<0.01)、WHR(r=0.2749,P<0.01)与cfPWV均呈正相关.单因素分析结果显示:将研究对象分为A组(cfPWV≤12 m/s)、B组(cfPWN>12 m/s),与A组相比,B组的WC、WHR高于A组,差异有统计学意义(P<0.05).BMI在A组与B组之间差异无统计学意义(P≥0.05).逐步回归分析显示:WC是cfPWV的危险因素,独立于性别、年龄、收缩压、高密度脂蛋白胆固醇、餐后2h血糖因素存在.结论 该社区人群中肥胖是动脉僵硬度的独立危险因素,其中不同的肥胖测量指标与动脉僵硬度的相关性不同.
Abstract:

Objective To observe the effect of p53 protein on smooth muscle cell(VSMC)in rabbit artery balloon injury.Methods Restenosis model of carotid artery after balloon injury was established in rabbits.30 rabbits were divided into 2 groups,the sham group(n = 6)and the vascular injury group(n = 24).With H.E.staining and automatic image analysis system,we investigated artery morphology alteration and measured the area of arterial intima and media.The expressions of p53 protein were detected by immunohistochemical analysis.Results With H.E.staining and automatic image analysis and immunohistochemistry,the results showed that the expression of p53 was significantly reduced and the intima area was increased in model group compared with the sham group(P <0.01).But the expression of p53 in media was remarkably reduced compared with intima(P < 0.01).Conclusions The possible mechanism of preventing arterial restenosis in the balloon injury might be related with p53,which may be through inhibiting neointimal proliferation in arterial restenosis .  相似文献

儿童肥胖对高血压发病率影响的随访研究   总被引:1,自引:0,他引:1  
目的 探讨儿童肥胖状态及肥胖状态的改变对其高血压发病率的影响.方法 采取前瞻队列研究的方法,抽取2004年北京市儿童青少年代谢综合征队列人群中2189名6~16岁血压正常儿童,于2010年12月对其随访,进行身高、腰围(WC)、体重和血压的测量.分别以体质指数(BMI)和WC作为评价超重、肥胖、腹型肥胖的指标.以不同肥胖状态组设置哑变量,以随访时点是否高血压作为结局变量,进行非条件logistic回归分析,分析基线肥胖状态及肥胖状态的改变与高血压发病率之间的关系,并计算相应的OR值及95%CI值.结果 共完成了1184名在校学生的随访,6年间高血压累积发病率为19.9%(236/1184).男性高血压发病率(23.2%,149/643)高于女性(16.1%,87/541) (χ2=9.257,P=0.002).基线非超重组、超重组、肥胖组随访期间高血压累积发病率分别为8.7%(45/519)、19.3%(35/181)、32.4%(156/484)(χ2=9.332,P<0.001),基线非肥胖组与腹型肥胖组随访期间高血压累积发病率分别为10.3%(63/613)、30.7%(173/567) (χ2=77.753,P<0.001).基线肥胖组的高血压发病危险高于基线非超重组(BMI:OR=4.9,95%CI:3.4~7.0)和基线非肥胖组(WC:OR=3.9,95%CI:2.8~5.3);基线时BMI、WC水平相同时,随访时BMI、WC水平增加,高血压发病危险增加.控制年龄、性别,基线BMI和WC每增加1 kg/m2和1 cm,高血压发病风险分别增加0.21和0.07倍,OR(95%CI)值分别为1.21(1.16~1.26)和1.07(1.05~1.09);随访时BMI和WC改变量每增加1 kg/m2和1 cm,高血压发病风险分别增加0.16和0.05倍,OR(95%CI)值分别为1.16(1.11~1.22) 和1.05(1.03~1.07).结论 儿童肥胖及肥胖水平升高的改变会增加其高血压发病风险.
Abstract:
Objective To explore the impact of obesity level and the level change in childhood on hypertension incidence.Methods A perspective cohort study was conducted.As part of Beijing Child and Adolescent Metabolic Syndrome Study,2189 aged 6-16 year non-hypertensive children was followed up in December,2010.In this study,height,weight,waist circumference (WC) and blood pressure was measured at follow-up,and body mass index (BMI) and WC was respectively used to assess overweight,obesity and abdominal obesity.Non-conditional logistic regression was used to evaluate the association between baseline obesity status, change of obesity status and hypertension incidence. OR and 95%CI were computed in the model using obese status as dummy variable and hypertension at follow up visit as dependent variable.Results The total hypertension incidence of 1184 subjects during 6 years follow-up was 19.9%(236/1184).The hypertension incidence in male (23.2%,149/643) was higher than that in female (16.1%,87/541) (χ2=9.257,P=0.002).The hypertension incidence of non-overweight,overweight and obese children at baseline was 8.7%(45/519),19.3%(35/181) and 32.4%(156/484)respectively (χ2=9.332,P<0.001),and the incidence of non-obese and abdominal obese children at baseline was respectively 10.3%(63/613) and 30.7%(173/567) (χ2=77.753,P<0.001).Hypertension incidence in the baseline obesity group was higher than the non-overweight (BMI:OR=4.9,95%CI:3.4-7.0) and non-obese group (WC:OR=3.9,95%CI: 2.8-5.3).The hypertension incidence increased with the follow-up BMI/WC level,based on the same baseline level of BMI and WC.The hypertension risk increased to 0.21 and 0.07 times respectively with elevation of baseline BMI level by 1 kg/m2 and WC level by 1 cm,and OR(95%CI) were 1.21(1.16-1.26) and 1.07(1.05-1.09),respectively.Similarly,the hypertension risk increased 0.16 and 0.05 times respectively with the elevation of BMI level change by 1 kg/m2 and 1 cm,and OR (95%CI) were 1.16(1.11-1.22) and 1.05(1.03-1.07),respectively.Conclusion Obesity and increased obesity level change in childhood can increase the risk of incident hypertension.  相似文献

Objective To investigate the incidences of nutrition risks, malnutrition ( undernutrition),overweight, and obesity, and nutrition support in tertiary hospitals in Xinjiang Uigur Autonomous Region. Methods Using fixed-point consecutive sampling, we collected the clinical data of inpatients in 6 departments of five tertiary hospitals in Xinjiang. According to the Nutrition Risk Screening 2002 (NRS 2002 ) published by European Society for Parenteral and Enteral Nutrition, patients were graded as at nutritional risk when their NRS 2002 scores ≥3 and as malnutrition when the body mass index (BMI) was < 18.5 kg/m2 (or albumin < 30 g/L). NRS 2002 screening was performed on the next morning after a patient was admitted. The nutrition supports within 2 weeks after admission were also investigated. The relationship between nutrition risks and nutrition support was analyzed.Results A total of 4036 inpatients were investigated, among them 3913 patients received NRS 2002 screening.The malnutrition (undernutrition) rate and the proportion of patients at nutritional risk were 8.4% and 34. 2%, respectively. The percentage of nutrition support was 10. 2%, which included parenteral nutrition (8. 5% ) and ena simple, fast and convenient tool for the investigation of nutrition risks and can provide a basis for reasonable nutrition support Therefore, it should be widely applied in clinical practice.  相似文献

ObjectiveTo explore the alterations of serum lipid m etabolism of Type 2 diabetes mellitus(DM) and its relationship with cardiovascul ar diseases.MethodsA 1∶1 case-control study in 100 newly diagnose d cases of Type 2 diabetes mellitus and 100 controls was conducted,8 indices of serum lipid metabolism were measured.ResultsThe means of TG and apo B in the group of obesity and overweight were significantly higher than those i n the normal weight group,and the means of HDL-C and Lp(a) were the reverse.BMI was significantly positively correlated with TG and apo B,but negatively correl ated with HDL-C and Lp(a).WHR was significantly positively correlated with TC,T G,LDL-C and apo B,whereas negatively correlated with HDL-C.The group with high er TC,LDL-C and apo E had a significantly higher proportion of hypertension,and the group with higher HDL-C had a significantly lower proportion of hypertensi on and CAD.ConclusionsThis study indicated that the dyslipidemia of type 2 diabetes mellitus(especially with obesity),such as low HDL-C,high TG,LD L-C and apo E might be a major cause of AS and cardiovascular diseases.  相似文献

Objective To investigate the prevalence of the nutritional risks, undernutrition, overweight,and obesity, and the application of nutritional support in hospitalized patients in middle and small hospitals of Guizhou province. Methods Adult patients in the departments of general surgery, chest surgery, neurology, gastroenterology, respiratory medicine, and nephrology in four hospitals in Guizhou Province were consecutively enrolled from Feberary 2008 to March 2009. Nutrional Risk Screening 2002 (NRS2002) was performed on the next morning after admission and nutritional support was evaluated on the 14 day of admission or on the discharge day.The relationship between nutritional risk and nutritional support was analyzed. Nutritional risk was defined as NRS 2002 score≥3 and under-nutrition was defined as body mass index < 18.5 kg/m2. Results A total of 1668 patients were enrolled and underwent, NRS 2002 scoring. The nutritional risk was 30. 2% and the prevalence of malnutrition was 12.2%. Although 54 patients ( 11.7 % ) with NRS 2002 ≥ 3 received nutrition support, only parenteral nutrition was applied. Conclusions NRS 2002 is a feasible nutritional risk screening tool for patients in middle and small hospitals. The application of nutrition support is still at a low level, with only parenteral nutrition is applied. Evidence-based nutrition support guidelines are required to standardize the application of nutrition support.  相似文献

目的 探讨中老年男性雄激素部分缺乏(PADAM)患者的骨密度(BMD)特点和骨质疏松的发生率.方法 采用双能X线骨密度仪对56例PADAM患者(PADAM组)和与之年龄、体重指数(BMI)相匹配的51例健康中老年人(对照组)进行腰椎及股骨颈等部位的BMD测定,采用MES-01S20肌肉功能分析仪测定肌肉分布和力学特性,并收集相关生化指标、激素水平及部分骨代谢指标进行多元逐步回归分析.结果 与对照组比较,PADAM组腰椎BMD无显著改变(P>0.05),而股骨颈、Ward三角区和大转子的BMD显著降低(P<0.01),且其抗骨折能力和下肢最大肌力显著降低(P<0.01).PADAM组和对照组的骨量减少发生率分别为48.2%(27/56)和35.3%(18/51),骨质疏松患病率分别为30.4%(17/56)和21.6%(11/51),两组比较差异有统计学意义(P<0.05).PADAM患者的腰椎BMD与BMI呈正相关,而股骨颈、Ward三角区、大转子的BMD与年龄呈负相关,与BMI、血清总睾酮呈正相关(P<0.05).结论 PADAM患者的BMD及抗骨折能力明显降低,存在骨质疏松性骨折的潜在危险,加强PADAM患者骨折的预防和治疗有重要意义.
Abstract:
Objective To explore the characteristics of bone mineral density (BMD) and the incidence of osteoporosis in partial androgen deficiency in aging male (PADAM) patients.Methods Fifty-six PADAM patients (PADAM group) and 51 healthy persons (control group) were selected according to their age and body mass index (BMI),and measured their BMD in the second to fourth lumber and the neck of femur with the dual-energy C X-ray BMD measuring instrument.MES-01S20 muscle function analyzer was used to determinate the distribution and mechanical properties of muscle.The biochemical and bone metabolic markers and sexual hormones were collected and observed by multivariate stepwise regression analysis.Results Compared with control group,BMD significantly decreased in the Ward triangle,femoral neck and big rotor (P < 0.01 ) and no significant change in lumbar in PADAM group (P > 0.05 ),and the fracture strength of femoral neck (FS) and the lower-limb muscular strength (MS) also significantly decreased (P < 0.01 ).The incidence of osteopenia were 48.2% (27/56) and 35.3% ( 18/51 ),osteoporosis were 30.4% (17/56) and 21.6% ( 11/51 ) respectively in PADAM group and control group.There was significant difference between two groups (P<0.05).The BMD was positively correlated to BMI at the first to fourth lumber and negatively correlated to age,positively correlated to BMI and serum level of andrusol at the proximal left femur in the patients with PADAM.Conclusion BMD and the resistance to fracture aresignificantly lower in PADAM patients,and aging lower BMI and androgen deficiency are the risk factors of low BMD in PADAM patients.There is the potential of osteoporotic fracture risk,and it is important to strengthening the prevention and treatment of fractures in elderly PADAM patients.  相似文献

Objective To analyze the region cluster and risk factors of hypertension in the Chinese adult population and to explore the application of multilevel regression model in the risk factors of hypertension. Methods Multi-stage random sampling technique was used to choose 15 540 individuals aged 35-74 years from 10 regions in China. Two-level logistic regression models were fitted under MLwiN 2.02 software. Results The region cluster of hypertension existed and variance portion coefficient was 3.1%. After adjusting for the age and gender, overall obese people (BMI≥28 kg/m2) were 4.50(95%CI: 4.00-5.06) times, overweight people (BMI=24-27.9 kg/m2) were 2.26 (95%CI: 2.07-2.46) times more likely to be hypertensive as compared with those of normal BMI (18.5-23.9 kg/m2), and those centrally obesive people (Waist circumference≥85 cm in male or 80 cm in female) were 2.62 (95%CI: 2.42-2.83) times more likely to be hypertensive as compared with those of normal WC. The age-and gender-adjusted odds ratios (Ors) of triglyceride (TG), serum total cholesterol (TC), glucose, low-density lipoprotein cholesterol (LDL-C) , high-density lipoprotein cholesterol (HDL-C) and drinking alcohol were 2.10 (95% CI: 1.89-2.33) , 2.08 (95% CI: 1.84-2.35) , 1.85 (95% CI: 1.60-2.14) , 1.58 (95% CI: 1.38-1.81), 1.49(95%CI: 1.32-1.69) and 1.15(95%CI: 1.05-1.27), respectively. Conclusion The prevalence of hypertension was not only affected by individual risk factors, such as obesity, drinking alcohol, abnormal glucose and serum lipids profile, but also affected by the geographic environment where people resided in. Population-and risk factors targeted strategies, proved a promising way to reduce individual risk of hypertension in the primary prevention of hypertension.  相似文献
