目的分析上颌中切牙与同侧尖牙同时阻生病例的临床特征。方法选择上颌中切牙与同侧尖牙同时阻生病例21例,分析上颌中切牙阻生类型与尖牙阻生情况,测量阻生侧及对侧侧切牙发育或萌出异常情况。结果与中切牙阻生同时发生的尖牙76.2%为唇侧阻生,其中完全易位与不完全易位占唇侧阻生尖牙的68.8%,尖牙唇侧阻生病例中侧切牙根远中倾斜和伴牙冠近中倾斜者占87.5%,尖牙唇侧或腭侧阻生病例中,出现锥形或过小等畸形侧切牙的比例分别为31.25%和40.00%。上颌中切牙与尖牙不同阻生类型患者,前牙反牙合比例都超过40%,且多为牙型反牙合。结论与中切牙阻生同时发生的尖牙阻生多为唇侧阻生,与中切牙阻生类型无关,与侧切牙根远中倾斜高度相关,尖牙完全易位与过小或锥形等畸形侧切牙有关。  相似文献

The occurrence of tooth anomalies in association with failure of the first and second molars to erupt was assessed in a sample of 1520 nonsyndromic subjects with uncrowded dental arches (mean age, 14 years 4 months) and compared with the prevalence rate calculated in a matched control group of 1000 subjects. The tooth anomalies examined included infraocclusion of deciduous molars, palatal displacement of maxillary canines, rotation of maxillary lateral incisors, aplasia of second premolars, and small size of maxillary lateral incisors. Associations among arrested eruption of first and second permanent molars and anomalies in tooth eruption and position (infraoccluded deciduous molars, palatally displaced canines, rotated maxillary lateral incisors) were highly significant (P <. 001). No significant association was found among the occurrence of molar eruption disturbances, aplasia of premolars, and small-sized laterals. These findings point to a common biologic cause for the appearance of failure of eruption of molar teeth and other disturbances in tooth eruption and position, most likely under genetic influence.  相似文献

OBJECTIVE: To evaluate the dental characteristics of patients subjected to a protocol that included early secondary gingivoalveoloplasty (ESGAP). DESIGN: Panoramic radiographs of 87 patients with unilateral cleft lip and palate (UCLP) and 29 with bilateral cleft lip and palate (BCLP) were evaluated. Missing and supernumerary teeth were also quantified on the cleft and noncleft side and in the maxilla and mandible. Crown and root malformations and tooth rotations were quantified. A subsample in permanent dentition was extrapolated to analyze canine eruption patterns. RESULTS: A total of 48.8% of the UCLP patients presented with missing permanent lateral incisors in the cleft area and 6.1% contralaterally. A total of 4.9% presented with missing second maxillary premolars on the cleft site and 1.2% contralaterally. A total of 7.3% presented with supernumerary lateral incisors, and 45% of the BCLP cleft sites presented with missing lateral incisors, while 25% of the cleft sites presented second maxillary premolars agenesis. Five percent of the cleft sites presented with supernumerary lateral incisors. Evaluation of the subsample in permanent dentition showed that 15.5% had a canine retention and 4.4% of the canines had to be surgically exposed. A significant association was observed between canine inclination and retention but not with absence of the lateral incisor. CONCLUSIONS: The frequency of dental anomalies in this sample was similar to other cleft populations. As surgical trauma has been suggested to damage forming teeth, the results of this study indicated that ESGAP has no detrimental influence on subsequent dental development.  相似文献

Transposition is an uncommon dental anomaly involving positional interchange of two teeth. The maxillary canine is the tooth more frequently transposed in man. Maxillary canine-first premolar appears to be the most common type of tooth transposition, followed by maxillary canine-lateral incisor transposition. Maxillary canine transpositions are frequently associated with other dental abnormalities such as agenesis and pegshaped incisors. This report describes the presence of transposed canines in one sister and two brothers. The female showed bilateral maxillary canine-first premolar transposition with the left canine fully mesial to its neighboring first premolar, and the right canine blocked-out facially between the first and second premolar. One of the brothers showed full maxillary left canine-lateral incisor transposition. The other brother showed maxillary canine-first premolar transposition and agenesis of maxillary lateral incisors, with the left canine blocked-out facially between the first and second premolar. Findings from this case report and other previously published cases provide strong evidence that maxillary canine transpositions are a disturbance of tooth order and eruptive position resulting from genetic influences within a multifactorial inheritance model.  相似文献

A sample of 85 dental transpositions in 75 subjects (27 male, 48 female; mean age at diagnosis 12.25 years) involving both maxillary and mandibular arches was analysed using dental panoramic radiographs and clinical records. Transposition affected the maxillary dentition (76 per cent) more frequently than the mandibular dentition (24 per cent). Unilateral transposition accounted for 88 per cent of cases, with the maxilla being involved more commonly than the mandible. Overall, the most common transposition involved the maxillary canine and first premolar (58 per cent). Considering the jaws in isolation, the canine and first premolar were the most commonly affected teeth in the maxilla (83 per cent) whilst in the mandible, the canine and lateral incisor teeth were most commonly transposed (73 per cent). No significant difference in symmetrical distribution of the unilateral transposition sample occurred. There was evidence of associated hypodontia in 41 per cent of the sample; however, if third molars were excluded, this figure decreased to 25 per cent. Peg-shaped maxillary lateral incisors were judged to be present in 27 per cent of subjects, whilst 41 per cent had retained primary teeth; all of these, except one, were primary canines. Overall, the majority of the sample (76 per cent) demonstrated at least one of the dental anomalies under investigation. Multivariate analysis showed associations between unilateral transposition, gender, and the presence of peg-shaped maxillary lateral incisors; whilst bilateral transposition was more closely associated with gender and the presence of retained primary teeth. There was a poor association between both unilateral and bilateral transposition and hypodontia. Together, these results suggest a mutifactorial aetiology to this disorder, with both genetic and environmental factors playing an important role.  相似文献

STATEMENT OF PROBLEM: Dimensions of teeth have been available for a century. Some significant and clinically relevant aspects of dental esthetics, however, such as the crown width/length ratios, have not been presented in tooth morphology sources until recently. PURPOSE: The purpose of this study was to analyze the anatomic crowns of 4 tooth groups (central incisors, lateral incisors, canines, and first premolars) of the maxillary dentition with respect to width, length and width/length ratios and determine how these parameters are influenced by the incisal edge wear. MATERIAL AND METHODS: Standardized digital images of 146 extracted human maxillary anterior teeth from white subjects (44 central incisors, 41 lateral incisors, 38 canines, 23 first premolars) were used to measure the widest mesiodistal portion "W" (in millimeters) and the longest inciso-cervical/occluso-cervical distance "L" (in millimeters). The width/length ratio "R" (%) was calculated for each tooth. A 1-way analysis of variance was used to compare the mean values of W, L, and R for the different groups ("unworn" and "worn" subgroups, except for premolars). Multiple least significant difference range tests (confidence level 95%) were then applied to determine which means differed statistically from others. RESULTS: There was no influence of the incisal wear on the average value of W (width) within the same tooth group. The widest crowns were those of central incisors (9.10 to 9.24 mm) > canines (7.90 to 8.06 mm) > lateral incisors (7.07 to 7.38 mm). Premolars (7.84 mm) had similar width as canines and worn lateral incisors. The L-value was logically influenced by incisal wear (worn teeth were shorter than unworn teeth) except for lateral incisors. The longest crowns were those of unworn central incisors (11.69 mm) > unworn canines (10.83 mm) and worn central incisors (10.67 mm) > worn canines (9.90), worn and unworn lateral incisors (9.34 to 9.55 mm), and premolars (9.33 mm). Width/length ratios also showed significant differences. The highest values were found for worn central incisors (87%) and premolars (84%). The latter were also similar to worn canines (81%), which constituted a homogeneous group with worn lateral incisors (79%) and unworn central incisors (78%). The lowest ratios were found for unworn canines and unworn lateral incisors (both showing 73%). CONCLUSIONS: Along with other specific and objective parameters related to dental esthetics, average values for W (mesiodistal crown dimension), L (inciso-cervical crown dimension), and R (width/length ratio) given in this study for white subjects may serve as guidelines for treatment planning in restorative dentistry and periodontal surgery.  相似文献

OBJECTIVE: Oculo-facio-cardio-dental (OFCD) syndrome is a very rare condition that requires comprehensive dental management because of associated multiple dental anomalies such as canine radiculomegaly, delayed dentition, oligodontia, persistent primary teeth, microdontia, and macrodontia. This report presents a case of OFCD in a Japanese girl (13 years 1 month old). We analyzed cephalograms, panoramic roentgenograms, and dental casts and discuss our integrated orthodontic-prosthodontic treatment. DESIGN: The sizes of the tooth crown and root as well as lateral cephalograms were compared with those from a Japanese control group. The outcome of orthodontic treatment was evaluated by comparing cephalograms taken before and after treatment. RESULTS: Radiculomegaly was found not only in the upper and lower canines but also the upper central incisors and lower first premolars. Macrodontia was found in the upper central incisors, upper canines, lower canines, and lower first premolars. Microdontia was found in the upper lateral incisor. Lateral cephalometric analysis showed a remarkable hypoplastic midface in both the sagittal and vertical dimensions, coupled with a significantly decreased cranial base length and an increased gonial angle. Serial lateral cephalograms during orthodontic treatment from 13 to 23 years of age demonstrated only slight maxillary growth and significant downward mandibular growth with clockwise rotation in addition to pronounced labial tipping of the upper central incisors. CONCLUSIONS: Characteristic dental anomalies together with a unique craniofacial dysmorphology were clarified. Successful oral rehabilitation was achieved by integrated orthodontic-prosthodontic treatment.  相似文献

Dens invaginatus (dens in dente) is a common dental anomaly with a reported prevalence of between 0.04% and 10%. It typically affects permanent maxillary lateral incisors, central incisors and premolars. These developmental lesions are less common in mandibular teeth and are extremely rare in canines and molars. This report describes a rare case of dens invaginatus (Oehlers type II) in a permanent mandibular canine. The tooth was mature with a closed apex and showed apical pathosis. The tooth was treated endodontically using a non-surgical technique with hand endodontic files, and then followed up after a period of 8 months. A follow-up radiograph showed some healing of the lesion.  相似文献

The published literature on tooth transposition includes only a few studies that have involved more than 50 subjects. The aim of the present study was to investigate the prevalence of true maxillary tooth transposition and possible associated dental anomalies in a larger sample of children. The dental records and radiographs of children who had been diagnosed as having true maxillary tooth transposition at a School Dental Clinic in Hong Kong were studied retrospectively. Data were analyzed for sex and side distribution, as well as for associated dental anomalies. Trends of differences were analyzed statistically using the Fisher exact or chi-squared test. A total of 69 cases of true maxillary tooth transposition were identified and studied; its prevalence in Hong Kong Chinese children was 0.81%. More females than males were affected, and the difference between the sexes was statistically significant (P < 0.05). The prevalence of congenitally missing teeth, microdontia of the maxillary lateral incisors or dental impaction was higher in patients with maxillary tooth transposition than in the general population (P < 0.05, P < 0.0005, and P < 0.0001, respectively). The fact that patients with maxillary tooth transposition were more likely to have congenital absence or microdontia of the maxillary lateral incisors lent further support to the contention that a developmental field defect plays a role in the pathogenesis of maxillary tooth transposition.  相似文献

Seventy-three children with submucous cleft palate (38 girls and 35 boys), mean age 8.2 years (range 7.7-9.5), were studied retrospectively from orthopantomograms. Dental abnormalities in permanent dentition were found in 26 patients (36%). Missing teeth, mainly lower 2nd premolars, upper lateral incisors, and upper 2nd premolars, were found in 12 patients (16%). Most of the patients had 1 or 2 missing teeth, 2 had 3 missing teeth. In 5 patients hypodontia was associated with another dental abnormality. Other dental abnormalities included peg-shaped lateral incisors in 7 patients (10%), ectopic eruption of upper 1st molars in 6 patients (8%), transposition of upper canines and 1st premolars in 3 patients (4%), supernumerary teeth in 2 patients (3%), and palatally displaced upper canines in 1 patient (1%). As children with submucous cleft palate have a tendency towards increased frequency of missing teeth and other dental abnormalities, the need for thorough clinical and radiological dental examination is emphasized.  相似文献

We performed computed tomography (CT) on 107 children and adolescents aged 9-15 years with 176 unerupted maxillary canines (152 erupting ectopically and 24 erupting normally) to determine whether there is an association between widened dental follicles of the maxillary canines and resorption of the adjacent incisors during eruption. Contiguous axial (transverse) CT scans were obtained through the maxilla in the region of the canines. The width and shape of the dental follicles were recorded, as were any contacts between the follicles and the crowns of the maxillary canines and neighboring incisors. Fifty-eight lateral incisors (38%) and 14 central incisors (9%) had some type of root resorption. The position of the maxillary canine in relation to the root of the lateral incisor varied greatly, as did the width and shape of the canine dental follicle. Follicle width ranged from 0.5 mm to 7.0 mm. The mean +/- SD width of dental follicles was, on average, larger for the ectopically positioned canines (2.9 +/- 0.8 mm) than for the normally erupting canines (2.5 +/- 0.8 mm) (P < or = .01). We found that during eruption, the follicle of the erupting maxillary canine frequently resorbed the periodontal contours of adjacent permanent teeth but not the hard tissues of the roots. We concluded that the dental follicle did not cause root resorption of permanent teeth. Resorption of neighboring permanent teeth during maxillary canine eruption was most probably an effect of the physical contacts between the erupting canine and the adjacent tooth, active pressure during eruption, and cellular activities in the tissues at the contact points, all of which are part of the eruptive mechanism. The findings also confirm an association between root resorption of deciduous canines and the dental follicles of erupting permanent canines.  相似文献

The aims of this study were to produce odontometric data for a Moroccan population, to test Tanaka and Johnston and Moyers methods, to derive regression equations and elaborate a specific orthodontic prediction table for Moroccan children. 50 dental casts of Moroccan students (25 males, 25 females, mean age 22.6 years sd 1.35) were used to perform measurements of the greatest mesiodistal widths of all mandibular and maxillary canines and premolars and mandibular incisors with an orthodontic calliper. Significant sexual dimorphism was found in tooth sizes (p = .001). The study revealed close relationships between the total mesiodistal widths of the mandibular permanent incisors and that of the maxillary and mandibular canines and premolars. The correlation coefficients (r) and the coefficient of determination (R2) were better sexes combined for the maxilla (r = 0.60 ; R2 = 0.36 ) and for the mandible (r = 0.61 ; R2= 0.37). The regression equations elaborated for males and females were used as a basis for establishing an orthodontic prediction table for Moroccan children.  相似文献

目的 经回顾性研究了解非综合征性单侧完全性唇腭裂患者恒牙发育异常的情况.方法 收集244例植骨期非综合征性单侧完全性唇腭裂患者的影像学资料,记录和统计分析恒牙发育异常.结果 在本研究中各种恒牙异常的发生率为:先天缺失163例(269颗)(163/244,66.8%),最常累及的牙位是上颌侧切牙;畸形牙82例(85颗)(82/244,33.6%),绝大多数为上颌侧切牙的小牙畸形;额外牙12例(12颗)(12/244,4.9%),多发生于裂区,表现为畸形小牙.牙位发育异常的性别比较发现,非裂侧上颌仅侧切牙缺失,男性高于女性(P<0.05).裂侧的先天缺牙、额外牙和畸形牙的发生率均高于非裂侧,差异有统计学意义(P<0.01),上下颌均有相同发现.结论 非综合征性单侧完全性唇腭裂患者各类牙齿发育异常的患病率均高于健康人群.裂侧的牙齿发育异常较非裂侧更为多见.
Abstract:
Objective To investigate the prevalence of dentM anomalies in Chinese children with complete unilateral cleft lip and palate(UCLP).Methods Dental histories and radiographs of 244 Chinese children with UCLP were collected.The diagnosis of dental anomalies waft based on panoramic radiographs before alveolar bone grafting.All patients were over 8 years old.Results In the UCLP group,66.8% of the patients was presented with hypodontia.The maxillary lateral incisors were the most common teeth affected,followed by maxillary second premolars,mandibular incisors and mandibulax second premolars.A total of 33.6%the patients Was presented with dental malformation,most were mierodontic laterel incisors.A total of 4.9% the patients was presented with hyperdontim The supernumerary teeth were more frequently found in the cleft region.The prevalence of missing maxillary lateral incisor in the noneleft side was statistically different between genders,which was higher in male(P<0.050).This group of Chinese children with UCLP demonstrated significantly higher prevalence of hypodontia,hyperdontia,and malformation on the cleft side than on the noncleft side(P<0.01).Conclusions Hypodontia is the most common type of dental anomalies.The prevalence of dental anomalies is higher in the UCLP patients than in the normal population.Dental anomalies occur more frequently on the cleft side than on the nonclefi side.  相似文献

The aims of this retrospective computed tomography (CT) study were to determine the occurrence of severe root resorption involving the pulpal canal of adjacent permanent teeth associated with ectopically erupting canines, and to verify the existence of related factors. The sample consisted of 255 consecutive patients (159 females and 96 males, mean age 18.4 and 16.8 years, respectively). Three hundred and thirty-four ectopic maxillary canines and adjacent teeth were analysed using CT images. Statistical significance was evaluated with chi-square and Fisher's exact tests. The results showed that severe root resorption of adjacent permanent teeth occurred in 17.7 per cent of ectopic canines and was equally common in females and males. Severe root resorption affected 12.6 per cent of the lateral incisors, 4.8 per cent of the first premolars, and 2.1 per cent of the central incisors. No relationship was found between the type or side of ectopic eruption, inclination of the longitudinal axis of the ectopic canine and the occurrence of severe root resorption. A significant relationship was found between a bucco-lingual position of the ectopic canine and root resorption (P < 0.05). Root resorption mainly occurred in the apical third (57.6 per cent) and apical and middle thirds (27.1 per cent). A significant relationship existed between the occurrence of root resorption and complete loss of space for the erupting canine (P < 0.05). No association was found between alignment of the upper permanent incisor and root resorption. A widened dental follicle occurred in 15 per cent of ectopic canines but did not cause root resorption of the adjacent permanent teeth. Since root resorption is asymptomatic, early detection by radiographic examination is essential for correct diagnosis and treatment.  相似文献

This study aimed to examine the eruption status of unerupted maxillary permanent canines in patients who also had an impacted central incisor. Seventy-five unilateral cases of the incisor anomaly were retrospectively examined. Using the initial panoramic films, we assessed the positions of the ipsilateral and contralateral canines and the lateral incisors and the timing (eruption progress) of the canines. The final position of the canines was evaluated from patient records and progress photographs. The data showed a significant increase in prevalence and severity of displaced canines in the ipsilateral side (41.3%) compared with the contralateral side (4.7%). Palatally displaced canines occurred in 9.5%, buccally displaced canines in 30.2%, and canine-lateral incisor transposition in 1.6% of the patients. Half of the buccally displaced canines in the ipsilateral side were pseudotransposed with the adjacent lateral incisor. The eruption timing of ipsilateral undisplaced canines was either similar or delayed, relative to the contralateral tooth, but never accelerated. The ipsilateral lateral incisor was strongly angulated because of a more distally located apex (5 mm) rather than a mesially tipped crown. The ipsilateral canine was relatively more superiorly positioned (undererupted). Patients who had trauma had a significantly higher prevalence and severity of canine displacement and more distally displaced lateral incisor apices. Initial height of incisor impaction did not influence the results. These data provide evidence of a significant environmental influence of an impacted maxillary central incisor in delaying and altering the eruption path of the ipsilateral maxillary canine.  相似文献

The 3-dimensional (3-D) inclination of the facial axis of the clinical crown (FACC) and the size of the clinical crowns were measured in 100 white northern Italians. The subjects consisted of 22 girls and 21 boys, ages 13-15 years (adolescents), and 31 women and 26 men, ages 16-26 years (adults), all with a complete permanent dentition and Class I dental relationships. The 3-D coordinates of dental landmarks were obtained with a computerized electromagnetic digitizer. Clinical crowns heights and FACC inclinations in the anatomical frontal and sagittal planes relative to 2 reference planes, maxillary and mandibular (between the incisive papilla and the intersection of the palatal/lingual sulci of the first permanent molars with the gingival margin), were calculated. Ages and sexes were compared by ANOVA. On average, the frontal plane FACCs of most teeth converged toward the midline plane of symmetry. In contrast, the incisors diverged from the midline plane or were nearly vertical. Within each quadrant, the inclinations of the postincisor teeth progressively increased. In the sagittal plane, most teeth had a nearly vertical FACC. FACC inclinations showed sex- and age-related differences (P < .05). In the frontal plane, the canines, premolars, and molars were more inclined in adolescents than in adults. In the sagittal plane, a large within-group variability was observed. Clinical crown height was significantly larger in males than in females in all maxillary and mandibular canines, premolars, second molars, maxillary central incisors, and first molars. With age, some degree of dental eruption was found in maxillary and mandibular canines, maxillary second premolars, and molars. The age-related decrease in FACC inclination may be the effect of a progressive buccal and mesial drift.  相似文献

目的调查青年人恒牙牙齿形态和数目异常的状况。方法2002至2004年对3所北京高校6453名17~21岁学生进行恒牙数目和形态异常的调查。对不同性别和不同牙位发育异常的患病率进行卡方检验,并对2种牙齿异常伴发的情况进行卡方检验。结果牙齿数目和形态异常的总患病率为16.07%。女性锥形牙或桶状牙、畸形中央尖和先天缺牙的患病率高于男性,差异有统计学意义(P〈0.05)。牙内陷、锥形牙或桶状牙好发于上颌侧切牙,畸形中央尖好发于下颌第二前磨牙,融合牙和先天缺牙好发于下颌切牙,差异均有统计学意义(P〈0.01)。锥形牙或桶状牙与先天缺牙伴发的比率高,差异有统计学意义(P〈0.01)。结论各种牙齿数目和形态异常的患病率、性别差异和好发牙位与以往的研究基本相符。但锥形牙或桶状牙、畸形中央尖的女性患病率均高于男性。  相似文献

AIM: To investigate the prevalence of various concomitant dental anomalies in Hong Kong children with true talon cusps on the permanent maxillary incisors. METHODS: Dental records and radiographs of a group of Hong Kong Chinese primary schoolchildren with true talon cusps (half crown height or more) on one or more permanent maxillary incisors were selected and studied retrospectively. The prevalence of various dental anomalies in this group of children was compared with that of the general population of Hong Kong Chinese children of similar age. RESULTS: A total of 11,537 records were reviewed and 58 children with true talon cusps on one or more permanent maxillary incisors were identified. A total of 69 permanent maxillary incisors were affected, of which all except one were lateral incisors. Dens evaginatus on premolars, supernumerary teeth in the anterior maxilla, and hypodontia were found in 5 (8.6%), 5 (8.6%), and 5 (8.6%) cases respectively. The prevalence of supernumerary teeth was significantly higher in children with true talon cusps as compared with the results of two previous general studies of Chinese children of similar age (P<0.05, Fisher's exact test). CONCLUSION: Children with true talon cusps on the permanent maxillary incisors were more frequently affected by supernumerary teeth in the anterior maxilla. Further studies with a larger sample are needed to confirm a true association.  相似文献
