Ostéome ostéoïde du pouce : localisation sous-périostée

The case of a 19 year old man with a painful and swollen thumb is reported. Xrays showed a thickening of the cortex of the phalanx, and CT scan led to the diagnosis of osteoid osteoma, showing a typical aspect of a nidus. Pain stopped immediatly after surgical procedure, and the volume of the thumb became normal after 3 monthes. While the hand is not a frequent location for osteoid osteoma, the thumb, especially in the periostal region, is very rare. Contrary to the medullary spongy bone location, Xrays signs are modest, and the CT scan is of great value in diagnosis and specifies the localization.

     

L’ostéolyse massive idiopathique : les aspects extrêmes de deux observations

Résumé Les auteurs rapportent 2 cas d’ostéolyse massive idiopathique encore connue sous le nom de maladie de Jackson-Gorham. Ces observations se caractérisent par leurs aspects diamétralement opposés. La première est une forme cubitale acquise dans les suites d’une fracture chez un sujet de 28 ans, sans retentissement fonctionnel. La seconde, au contraire constitue une lésion pelvienne massive connue depuis l’enfance chez un patient de 17 ans, avec une atteinte fonctionnelle majeure et des complications infectieuses à distance. Une revue de la littérature permet de discuter les particularités cliniques, pathogéniques, et thérapeutiques de cette maladie rare. Les auteurs mettent en garde contre les traitements aggressifs dans certaines localisations où la maladie reste bénigne.
