AIM: To evaluate the usefulness of urodynamic study in young men with lower urinary tract symptoms (LUTS). METHODS: We reviewed the charts of 50 men with LUTS aged 50 years and below. Those with neurological diseases, urethral trauma or strictures were excluded. All underwent multichannel urodynamic studies (UDS). The pre- and post-UDS diagnoses and treatment modalities were compared. RESULTS: Mean age was 38.1 years (17-49). The main pre-UDS diagnoses included prostatitis in seven (14%), overactive bladder in seventeen (34%) and benign prostatic hyperplasia in nine (18%). Pre-UDS management ranged from anticholingeric agents for thirteen (26%), alpha-adrenergic antagonists for nine (18%), antibiotics for six (12%). Abnormal UDS were noted in 36 (72%), including detrusor overactivity in 9 (18%), detrusor underactivity/acontractility in 5 (10%) and bladder outlet obstruction in 21 (42%). Fourteen (28%) had primary bladder neck dysfunction and five (10%) had benign prostatic hyperplasia. Post-UDS management included anticholingeric agents for ten (26%), alpha-adrenergic antagonists for seventeen (34%), catheterization for four (10%), behavioral therapy for three (6%), surgery for three (6%). None were prescribed antibiotics. Following UDS, the diagnosis had to be updated in 40 (80%) and concomitant change in management was required in 34 (68%). CONCLUSION: Young men presenting with LUTS have different underlying etiologies. Clinical diagnosis and treatment are often empiric and inaccurate. Urodynamic study is useful in the evaluation of this group of patients as it aids in arriving at an accurate diagnosis and guides treatment therapy.  相似文献

PURPOSE: In a prospective manner we sought to determine the cause of lower urinary tract symptoms in young men and whether noninvasive testing and symptoms scores are useful in deciding which patients to evaluate with videourodynamics. MATERIALS AND METHODS: We evaluated 85 men 18 to 45 years old with lower urinary tract symptoms. Patients with a history of known neurological disease or urethral stricture were excluded from the study. Patients were evaluated with the American Urological Association (AUA) symptom index, noninvasive uroflowmetry post-void residual and videourodynamics, and classified by specific urodynamic diagnoses. Noninvasive uroflowmetry (normal versus abnormal), post-void residual and AUA symptom index (total, voiding and storage scores) were evaluated as predictors of urodynamic abnormalities. RESULTS: Mean patient age was 35.1 (range 18 to 45) and mean symptom duration was 53.8 months. Mean AUA scores were total 19.3, voiding 10.8 and storage 8.5. Videourodynamic diagnoses were primary bladder neck obstruction in 40 (47%) cases, dysfunctional voiding in 12 (14%), impaired contractility in 8 (9%), sensory urgency in 7 (8%), detrusor instability alone in 5 (6%), detrusor instability and impaired contractility in 1 (1%), external detrusor-sphincter dyssynergia in 1 (1%) and normal in 5 (6%). Of these patients, 9 could not void during urodynamics and in 6 (7%) no urodynamic diagnosis was made. Videourodynamics were not considered helpful in patients with a normal or nondiagnostic study or sensory urgency only (group 1) but were helpful or diagnostic in the remaining patients (group 2). Only 5 of 18 patients (28%) in group 1 had an abnormal uroflow compared to 56 of 67 (84%) in group 2 (p <0.0001). Mean post-void residual volumes were not different between the 2 groups (40.3 versus 40.0 ml.). Mean total and storage symptom scores were also not different between the 2 groups but voiding scores were significantly higher in group 2 (11.5 versus 8.3, p <0.03). CONCLUSIONS: Lower urinary tract symptoms in young men have a variety of underlying causes. Videourodynamics is an extremely helpful diagnostic test especially in men with abnormal uroflow and high voiding scores.  相似文献

目的评估尿流动力学在女性下尿路症候群(LUTS)分类和治疗中的作用。方法采用尿动力测定仪对127例女性下尿路症候群患者进行尿流动力学检查,并根据检查结果进行分类及相关治疗,治疗后观察临床症状变化并复查尿流动力学各项参数进行对照。结栗不稳定膀胱30例(26.8%),低顺应性膀胱10例(8.9%),逼尿肌无力12例(10.7%),尿道压增高89例(79.5%),尿动力学无明显异常15例(11.8%)。112例尿动力学异常患者经相应治疗后有效97例,有效率为86.6%。结论尿流动力学检查在女性LUTS的诊断分型及指导治疗上具有重要意义。  相似文献

目的 探讨女性下尿路症状原因及临床意义。 方法  72例下尿路症状女性患者 ,年龄 8~ 86岁 ,平均 4 8岁。其中排尿困难 4 6例 ,尿失禁 14例 ,尿频 10例 ,夜间遗尿 2例。患者均行尿动力学检测。 结果  4 6例排尿困难者膀胱容量 6 0~ 80 0ml,其中 2 0例膀胱逼尿肌收缩力为 0 ,2 6例9~ 16 0cmH2 O(1cmH2 O =0 .0 98kPa)。Linp URR图示为Ⅲ级以上梗阻者 19例。尿道静态分布阻力2 5~ 130cmH2 O。膀胱逼尿肌与尿道外括约肌协调性良好者 5例 ,协同失调者 33例。 10例尿频患者膀胱容量 97~ 4 5 0ml,2例排尿期膀胱逼尿肌收缩力为 0 ,8例 31~ 110cmH2 O。Linp URR图示显示Ⅲ级以上梗阻者 2例。 1例急迫性尿失禁者膀胱过度敏感。 6例压力性尿失禁者腹压漏尿点压力 4 4~76cmH2 O。 3例充盈性尿失禁者排尿期膀胱逼尿肌压力为 0 ,尿道静态分布阻力 32~ 4 2cmH2 O。真性尿失禁 4例尿道外括约肌压力 0~ 16cmH2 O ,平均 (8.5± 5 .7)cmH2 O。 2例遗尿患者存在不稳定性膀胱表现 ,排尿期膀胱收缩乏力。 结论 尿动力学检查对女性下尿路症状疾病诊断、治疗、预后判断具有重要指导意义。  相似文献

目的:了解下尿路症状(LUTS)男性人群的勃起功能障碍(ED)患病情况,探讨LUTS与ED之间的相关性。方法:2011年11月~2012年8月,抽取1 000例40~80岁、有固定性伴侣的男性人群,采用国际前列腺症状评分(IPSS)及国际勃起功能指数(IIEF-5)评估LUTS和ED的严重程度,单因素Logistic回归分析LUTS与ED的相关性。结果:40~80岁男性人群的LUTS患病率为42.81%(426/995),ED患病率为76.18%(758/995)。其中426例有LUTS症状患者的ED患病率为82.16%(350/426),569例无LUTS症状的ED患病率为71.70%(408/569)。随着LUTS严重程度的增高,ED的患病率明显升高。Logistic回归分析显示,年龄、LUTS严重程度与ED的关联存在统计学显著意义(P0.01)。结论:LUTS患者存在很高的ED发生率。年龄越大,LUTS症状愈严重者患ED的风险更高。  相似文献

OBJECTIVE: To estimate the frequency of conditions associated with lower urinary tract symptoms (LUTS, typically included when assessing benign prostatic hyperplasia, BPH), as other causes of LUTS should be excluded when diagnosing BPH, using data from the Olmsted County Study of Urinary Symptoms and Health Status among Men. SUBJECTS AND METHODS: During 1989-91, Caucasian men aged 40-79 years were randomly selected from the Olmsted County population. Before contact, eligibility was determined by reviewing the community medical records. Baseline exclusion criteria included comorbid pre-existing conditions or treatments, e.g. prostate, bladder or lower back surgery, bladder neck contracture or cancer, diabetes with lower extremity amputation, and neurological diseases, including Parkinson's disease, amyotrophic lateral sclerosis, multiple sclerosis, tabes dorsalis and stroke. Men with these conditions were excluded from the Olmsted County Study at baseline, because these conditions are potentially associated with LUTS. RESULTS: Of the 5100 randomly sampled men, 13.4% met at least one of the pre-existing exclusion criteria. Individually, the frequency of exclusions was 7.8% for prostate cancer or surgery, 4.8% for back surgery, 1.3% for bladder surgery and 1.4% for neurological conditions. All other conditions represented <1.0% of the study exclusions. Older men were more likely to meet at least one of the exclusion criteria, with men in their fifth to eighth decade having a total exclusion frequency of 1.4%, 5.4%, 8.5% and 32.8%, respectively. The most common reason for men in their fifth decade to be excluded was lower back surgery (0.9%), whereas the most common reason in the eighth was prostate surgery (21.8%). CONCLUSIONS: In men, conditions that may contribute to LUTS, other than BPH, are prevalent in the community and increase in frequency with age. It is important that other conditions associated with LUTS be excluded before a definitive diagnosis of BPH. Any oversight in this initial evaluation can potentially result in misclassification bias, misdiagnosis and incorrect treatment of patients.  相似文献

ObjectivesLower urinary tract symptoms (LUTS) are highly prevalent in aging men. In this study we examined the relationship between age, total prostate volume (TPV), and videourodynamic study findings.MethodsWe retrospectively analyzed a total of 971 men ≥ 40 years of age referred to us for investigation of LUTS. We analyzed the distribution of the different videourodynamic study diagnoses in male LUTS by correlating their age and prostate size.ResultsThe most common diagnosis in the bladder outlet obstruction (BOO) group differed significantly by age and poor relaxation of the external sphincter (PRES) in those aged < 50 years; bladder neck dysfunction in those aged 50–69 years, and benign prostatic obstruction in those ≥ 60 years. Detrusor overactivity was the most common diagnosis in all ages in the bladder dysfunction group, and the cases of hyperactivity with impaired contractility (DHIC) increased with age. In patients < 50 years of age, PRES was the most common diagnosis in the BOO group in both those with small prostates (total prostate volume ≤ 40 mL) and large prostates (total prostate volume > 40 mL). In patients aged 50–69 years, the most common diagnosis in those with BOO and a small prostate was bladder neck dysfunction, and that in those with BOO and a large prostate was benign prostatic obstruction. Similar results were observed in patients aged ≥ 70 years. In all age groups, the majority of patients with detrusor overactivity, hypersensitive bladder, detrusor underactivity, and DHIC had a small prostate.ConclusionIn male LUTS, the diagnoses in the BOO group differed by age and prostate volume. In young patients with BOO, the leading diagnosis was PRES, and the contribution of prostate volume to BOO increased with age. As age increased, the bladder function became more complex with an increased percentage of patients with DHIC. Both bladder outlet and bladder functions were affected by age.  相似文献

目的了解成年男性人群下尿路症状(LUTS)的年龄分布与变化规律.方法采用国际前列腺症状评分调查问卷(IPSS)调查武汉市东湖周边地区自然村、街道居委会、工厂、行政机关和高校男性2811人,占其成年男性人数的71.7%.年龄20~82岁,以10岁为一年龄段,分为6个年龄组,评估被调查者LUTS.结果各组LUTS发生情况20岁~组798人中有轻度(IPSS 1~7分)LUTS仅5人(0.6%);30岁~组453人中有轻度LUTS 55人(12.1%);40岁~组537人中有LUTS者143人(26.6%),其中轻度118人,中度(8~19分)20人,重度(20~35分)5人;50岁~组465人中有LUTS者244人(52.5%),其中轻度109人,中度117人,重度18人;60岁~组366人中有LUTS 240人(65.6%),其中轻度78人,中度138人,重度24人;≥70岁组192人中有LUTS者162人(84.4%),其中轻度47人,中度68人,重度47人.不同年龄组男性LUTS发生率与年龄相关(x2=998.385,P<0.001).各年龄组间LUTS IPSS评分比较差异有显著性意义(F=40.472,P<0.001).IPSS评分与LUTS困扰评分呈正相关(r=0.829,P<0.001).结论LUTS在成年男性中普遍存在,年龄越大,发病率越高,症状越重.  相似文献

目的 研究以下尿路症状为主诉的女性患者的尿动力学变化及其临床意义。方法女性患者.年龄6-89岁,临床表现为储尿期(刺激性)和(或)排尿期(梗阻性)症状,采用尿动力学方法检查尿流率、同步膀胱压力容积流率及肛门括约肌肌电图测定、部分同步透视下行影像尿动力学检查。结果 以排尿症状为主者86例,以储尿症状为主者197例。不稳定膀胱57例,感觉性尿急30例。神经原性膀胱尿道功能障碍32例,其中逼尿肌反射低下26例。逼尿肌反射亢进7例。逼尿肌收缩力低下26例。逼尿肌外括约肌协同失调1例。下尿路梗阻30例,其中膀胱颈梗阻6例,尿道远端狭窄11例,非神经原件逼尿肌外括约肌协同失调13例。压力性尿失禁68例,其中Ⅰ型19例,Ⅱ型12例。Ⅲ型14例,Ⅱ/Ⅲ型23例;TF常40例。结论 尿动力学检查能了解膀胱的功能状况和膀胱出口梗阻的部位,为临床治疗的选择提供了可靠的依据。  相似文献

ObjectiveThe aim of this study was to investigate the predictive factors of prostate volume (PV) by analyzing potential predictors in a population of middle-aged men with bothersome lower urinary tract symptoms (LUTS) and use a prediction model for PV estimation to compare with digital rectal examination (DRE) alone.Materials and methodsPatients between the ages of 40 years and 64 years who underwent transrectal prostate ultrasound as part of a self-paid medical check-up were enrolled. Participant demographics, medical history, and voiding symptoms were assessed by the International Prostate Symptoms Score (IPSS) questionnaire. A multiple linear regression with stepwise selection was used to analyze the correlations between PV and all potential predictors.ResultsTwo hundred and twenty-eight men with bothersome LUTS (IPSS > 7) were enrolled as study participants at a mean age of 56.4 years. Patients with PV > 25 mL were significantly older and had higher serum prostate-specific antigen (PSA) levels and scores for total IPSS, storage, urgency items, and nocturia items. DRE, serum PSA, age, and urgency score were independent predictors for PV, especially for men with PV > 25 mL, for which the standardized regression equation was PV = 0.74 × (DRE estimation) + 0.10 × (age) + 0.12 × (serum PSA) + 0.079 × (urgency score) (adjusted R2 = 0.80).ConclusionIn the current study, we confirmed that serum PSA, age, and urgency score are significant predictors of PV. The prediction model including DRE, PSA, age, and urgency score was a better method to estimate PV than DRE alone, especially for men with a larger prostate (PV > 25 mL).  相似文献

AIM: The aim of this study was to compare voiding parameters by using urodynamic investigations in men and women with lower urinary tract symptoms (LUTS). METHODS: A total of 164 individuals (76 men and 88 women) completed the International Prostate Symptom Score (IPSS) and underwent a detailed urodynamic investigation. Patients were stratified by voiding function, which included bladder voiding efficiency (BVE) of < 80% and of >or= 80%. RESULTS: IPSSs were similar in the men and women except for voiding symptoms for those with a BVE of >or= 80% (12 for men vs. 9 for women, P=0.016). When voiding parameters were compared according to BVE in men and women, respectively, maximum flow rate (Q(max)) (P=0.002), average flow rate (Q(ave)) (P=0.003), voided volume (P=0.037), post-void residual (PVR) (P<0.001), and bladder outlet obstruction index (P=0.016) were different for men with a BVE of < 80% and men with a BVE of >or= 80%. However, for women with a BVE of < 80% or >or= 80%, PVR (P<0.001), detrusor pressure at maximum flow rate (P=0.022), and opening detrusor pressure (P=0.024) were different. CONCLUSIONS: The voiding parameters of women differ from those of men according to voiding function. The present results show that a pressure-flow study may represent a difference according to BVE in women as well as in men. Our findings suggest that opening detrusor pressure in women reflects the urethral and detrusor during voiding phase accurately as detrusor pressure at maximum flow rate does.  相似文献

PURPOSE: We determine the prevalence and bothersomeness of erectile and ejaculatory dysfunction in men with lower urinary tract symptoms suggestive of bladder outlet obstruction in real life practice, and analyze predictors of sexual dysfunction in this population. MATERIALS AND METHODS: Sexual function of 1,274 European men with lower urinary tract symptoms was assessed by the DAN-PSSsex questionnaire. The relationship between sexual dysfunction and selected clinical characteristics was analyzed. RESULTS: The proportion of sexually active men decreased from 91% in those 60 years old or younger to 60% in those 70 years old or older. Erectile dysfunction, reduced ejaculation and pain/discomfort on ejaculation were reported by 62%, 63% and 23% of patients, respectively. Erectile dysfunction strongly related with age (40% younger than 60 years, 80% 70 years old or older), lower urinary tract symptom severity (55% mild, 70% severe) and body mass index, hypertension and concomitant treatment with calcium channel antagonists. Reduced ejaculation was significantly related to age (42% younger than 60 years, 82% 70 years old or older), lower urinary tract symptom severity (55% mild, 68% severe) and previous benign prostatic hyperplasia surgery. Pain/discomfort on ejaculation was only related to lower urinary tract symptom severity (7% mild, 31% severe). Erectile dysfunction, reduced ejaculation and pain/discomfort on ejaculation were considered a problem by 96%, 82% and 91% of patients younger than 60 years, respectively. These 3 sexual symptoms were still highly bothersome in patients 70 years old or older. CONCLUSIONS: Erectile dysfunction and reduced ejaculation are highly prevalent in men with lower urinary tract symptoms, and are strongly related to increasing age and lower urinary tract symptom severity. Both these aspects of sexual dysfunction are also highly bothersome, even in advanced age. Sexual function should be carefully assessed in the initial evaluation of patients with lower urinary tract symptoms and in deciding on treatment options, as it may have a negative impact on sex life.  相似文献

OBJECTIVE

To investigate whether a mechanism of action of α‐blockers on lower urinary tract symptoms (LUTS) involves improved perfusion of the LUT.

PATIENTS, SUBJECTS AND METHODS

The accuracy of perfusion measurements using transrectal colour Doppler ultrasound (TRCDUS) and colour pixel density (CPD) was initially confirmed in a porcine model. Following this confirmation, measurements were taken from four healthy male volunteers and 19 patients with LUTS. The urinary bladder was filled slowly (50 mL/min) with 0.2 m KCl, which resembles the osmolarity of concentrated urine, and evaluated by cystometry. In parallel, TRCDUS and measurement of the CPD of the LUT were performed. The patients with LUTS were then treated with daily α‐blocker (0.4 mg tamsulosin) for 5 weeks and urodynamic variables as well as perfusion were evaluated again.

RESULTS

In the healthy men, perfusion of the LUT increased considerably (157%) during filling of the bladder to a mean (sd ) maximum cystometric capacity (Cmax) of 481 (28.9) mL. All the patients with LUTS had a reduced mean Cmax during filling with KCl at 322.4 (58.5) mL. The mean CPD in the urinary bladder and the prostate were only increased by 58.4% during filling with KCl. After α‐blocker therapy the mean Cmax during filling with KCl rose to 382.5 (42.9) mL; furthermore, perfusion of the LUT measured by CPD was significantly increased (132.8%).

CONCLUSIONS

The present data strongly suggest that LUTS are associated with chronic ischaemia of the prostate and urinary bladder. α‐blockers increase perfusion in the LUT and Cmax. These results might explain the therapeutic effects of α‐blockers on LUTS.  相似文献

AIM: The aim of the present study was to determine the frequency of lower urinary tract symptoms (LUTS), assess the impact of LUTS on quality of life (QOL) and compare the results with recent reports from other population-based studies. METHODS: A total of 266 men participated in the study. The men were stratified into 10-year age groups between 40 and 79 years. All participants were asked to complete a questionnaire that included a Turkish translation of the International Prostate Symptom Score (IPSS) with QOL questions, and void into a uroflowmeter to obtain voided urine volume, peak and mean flow rate. RESULTS: While 14.8% of men had no symptoms (IPSS = 0), 24.9% had moderate to severe symptoms (IPSS> 7). Severity of symptoms increased with age (P = 0.0018). There was a strong relationship between bother score and IPSS (rs = 0.79, P = 0.0001). Fifty-five percent of moderately symptomatic and 78% of severely symptomatic men reported poor QOL (QOL score >/= 3). The results of the survey provide a general picture of the symptomatology and urinary flow profiles of elderly men living in Turkey. CONCLUSION: The prevalence of LUTS in the Turkish community is fairly high, it increases with age and has an impact on QOL that is not negligible.  相似文献

目的 了解成年男性下尿路症状(LUTS)的发生情况、年龄分布与变化规律.方法 采用国际前列腺症状评分问卷表(IPSS)调查2008年参加健康体检的成年男性1 048人,年龄30~80岁,以10岁为一年龄段,分为5个年龄段,评估被调查者的LUTS发生情况.结果 各组LUTS发生的情况为:30岁~为347人,平均IPSS评分(2.48±0.38);40岁~为31 1人,平均IPSS评分(3.80±0.53);50岁~为114人,平均IPSS评分(4.78±1.07):60岁~为207人,平均IPSS评分(5.33±0.68);70~79岁为69人,平均IPSS评分(6.80±1.66).其中各年龄组储尿期症状平均评分分别为:1.20,1.66,2.24,2.57,3.20;排尿期症状平均评分分别为:1.28,2.14,2.54,2.76,3.59;生活质量平均评分为:(1.78±1.42),(1.84±0.30),(1.89±1.04),(2.03±1.08),(2.07±1.92).不同年龄组男性LUTS发生率及症状严重程度与年龄相关(J-T统计量为239743.50,P<0.001),IPSS总评分、排尿期症状评分及储尿期症状评分与生活质量评分呈正相关,其中储尿期症状比排尿期症状对生活质量影响更大.结论 LUTS症状在成年健康男性中较普遍存在,发病率随年龄增大而增加.LUTS症状越重,对生活质量影响越大,其中储尿期症状较排尿期症状对生活质鼍影响更大.  相似文献
