Objective: There is increasing demand in the hearing research community for the creation of laboratory environments that better simulate challenging real-world listening environments. The hope is that the use of such environments for testing will lead to more meaningful assessments of listening ability, and better predictions about the performance of hearing devices. Here we present one approach for simulating a complex acoustic environment in the laboratory, and investigate the effect of transplanting a speech test into such an environment. Design: Speech reception thresholds were measured in a simulated reverberant cafeteria, and in a more typical anechoic laboratory environment containing background speech babble. Study sample: The participants were 46 listeners varying in age and hearing levels, including 25 hearing-aid wearers who were tested with and without their hearing aids. Results: Reliable SRTs were obtained in the complex environment, but led to different estimates of performance and hearing-aid benefit from those measured in the standard environment. Conclusions: The findings provide a starting point for future efforts to increase the real-world relevance of laboratory-based speech tests.  相似文献

Objective: The purpose of this study was to assess perceived listening effort and speech intelligibility in reverberant and noisy conditions for hearing-impaired listeners for conditions that are similar according to the speech transmission index (STI). Design: Scaled listening effort was measured in four different conditions at five different STI generated using various relative contributions of noise and reverberant interferences. Intelligibility was measured for a subset of conditions. Study sample: Twenty mildly to moderately hearing-impaired listeners. Results: In general, listening effort decreased and speech intelligibility increased with increasing STI. For simulated impulse responses consisting of white Gaussian noise exponentially decaying in time, a good agreement between conditions of different relative contributions of noise and reverberation was found. For real impulse responses, the STI slightly overestimated the effect of reverberation on the perceived listening effort and underestimated its effect on speech intelligibility. Including the average hearing loss in the calculation of the STI led to a better agreement between STI predictions and subjective data. Conclusion: Speech intelligibility and listening effort provide complementary tools to evaluate speech perception over a broad range of acoustic scenarios. In addition, when incorporating hearing loss information the STI provides a rough prediction of listening effort in these acoustic scenarios.  相似文献

Objective: The purpose of this study was to evaluate the effects of directional microphone use on laboratory measures of sentence recognition, listening effort and localisation. An additional purpose of this study was to evaluate the effects of asymmetric directional microphone use on the same laboratory measures. Design: Three hearing aid conditions were evaluated: (1) bilateral omnidirectional microphones, (2) bilateral directional microphones and (3) asymmetric microphones (directional microphone for only one hearing aid). Sentence recognition performance was evaluated using a connected speech test. Listening effort was evaluated using a dual-task paradigm with a response time-based secondary task requiring word categorisation. Localisation was examined using a complex task requiring localisation and recall of speech originating from one of four loudspeakers in the horizontal plane (?60°, ?45°,?+45°,?+60°). Study sample: Eighteen adults (M?=?61.8 years) with symmetrical, moderate-to-severe hearing loss participated. Results: Performance on each task was analysed separately using a repeated measures analysis of variance. Results revealed directional benefits for sentence recognition and listening effort, but microphone setting did not affect localisation. Performance was equivalent with symmetric and asymmetric directional configurations. Conclusions: Bilateral and asymmetric directional microphone configurations equally improved sentence recognition and listening effort; neither affected localisation or recall.  相似文献

Objective: The purpose of this study was to evaluate the reliability and validity of four subjective questions related to listening effort. A secondary purpose of this study was to evaluate the effects of hearing aid beamforming microphone arrays on word recognition and listening effort. Design: Participants answered subjective questions immediately following testing in a dual-task paradigm with three microphone settings in a moderately reverberant laboratory environment in two noise configurations. Participants rated their: (1) mental work, (2) desire to improve the situation, (3) tiredness, and (4) desire to give up. Data were analysed using repeated measures and reliability analyses. Study sample: Eighteen adults with symmetrical sensorineural hearing loss participated. Results: Beamforming differentially affected word recognition and listening effort. Analysis revealed the same pattern of results for behavioural listening effort and subjective ratings of desire to improve the situation. Conversely, ratings of work revealed the same pattern of results as word recognition performance. Ratings of tiredness and desire to give up were unaffected by hearing aid microphone or noise configuration. Conclusions: Participant ratings of their desire to control the listening situation appear to reliable subjective indicators of listening effort that align with results from a behavioural measure of listening effort.  相似文献

Abstract

The speech intelligibility benefit of visual speech cues during oral communication is well-established. Therefore, an ecologically valid approach of auditory assessment should include the processing of both auditory and visual speech cues. This study describes the development and evaluation of a virtual human speaker designed to present speech auditory-visually. A male and female virtual human speaker were created and evaluated in two experiments: a visual-only speech reading test of words and sentences and an auditory-visual speech intelligibility sentence test. A group of five hearing, skilled speech reading adults participated in the speech reading test whereas a group of young normal-hearing participants (N?=?35) was recruited for the intelligibility test. Skilled speech readers correctly identified 57 to 67% of the words and sentences uttered by the virtual speakers. The presence of the virtual speaker improved the speech intelligibility of sentences in noise by 1.5 to 2?dB. These results demonstrate the potential applicability of virtual humans in future auditory-visual speech assessment paradigms.  相似文献

In this study the potential benefit of hearing instruments with multi-microphone technology was investigated in laboratory and in field tests for users with severe-to-profound hearing loss. Twenty-one experienced hearing aid users were fitted with high-power multi-microphone hearing instruments (Phonak PowerZoom P4 AZ). The following evaluations were performed: (i) adaptive speech test (SRT for HSM sentence test) in quiet and in noise with their own instrument and the test instrument in the omnidirectional (basic program) and directional mode (party noise profound+zoom algorithm). (ii) Paired comparisons of loudness, sound quality and speech intelligibility for both the omni and zoom program. (iii) Questionnaires on satisfaction and self-assessment of communication in different listening conditions (Oldenburg Inventory). Only 10 subjects achieved 50% correct (SRT) on the sentence test in noise (speech 0 degrees/noise 180 degrees) with both their own instrument and the test instrument in the omnidirectional mode. However, 15 subjects succeeded in the SRT measurement in the directional mode. The average SRT improvement of the directional over the omnidirectional mode was 13.7 dB. Loudness was judged 'medium loud' for both listening programs. Sound quality and intelligibility were rated significantly better for the zoom program. Compared to their own instrument users' satisfaction with the test instrument was significantly higher, especially in noisy listening situations.  相似文献

OBJECTIVE: To evaluate the improvement in speech intelligibility in noise obtained with an assistive real-time fixed endfire array of bidirectional microphones in comparison with an omnidirectional hearing aid microphone in a realistic environment. DESIGN: The microphone array was evaluated physically in anechoic and reverberant conditions. Perceptual tests of speech intelligibility in noise were carried out in a reverberant room, with two types of noise and six different noise scenarios with single and multiple noise sources. Ten normal-hearing subjects and 10 hearing aid users participated. The speech reception threshold for sentences was measured in each test setting for the omnidirectional microphone of the hearing aid and for the hearing aid in combination with the array with one and three active microphones. In addition, the extra improvement of five active array microphones, relative to three, was determined in another group of 10 normal-hearing listeners. RESULTS: Improvements in speech intelligibility in noise obtained with the array relative to an omnidirectional microphone depend on noise scenario and subject group. Improvements up to 12 dB for normal-hearing and 9 dB for hearing-impaired listeners were obtained with three active array microphones relative to an omnidirectional microphone for one noise source at 90 degrees . For three uncorrelated noise sources at 90 degrees, 180 degrees, and 270 degrees, improvements of approximately 9 dB and 6 dB were obtained for normal-hearing and hearing-impaired listeners, respectively. Even with a single noise source at 45 degrees, benefits of 4 dB were achieved in both subject groups. Five active microphones in the array can provide an additional improvement at 45 degrees of approximately 1 dB, relative to the three-microphone configuration for normal-hearing listeners. CONCLUSIONS: These improvements in signal-to-noise ratio can be of great benefit for hearing aid users, who have difficulties with speech understanding in noisy environments.  相似文献

Objectives: The purpose of this study was to investigate speech recognition in noise and listening effort among a group of adults with cochlear implants (CIs). Two main research questions were addressed. First, what are the effects of omni versus directional microphone configuration on speech recognition and listening effort for noisy conditions? Second, what is the effect of unilateral versus bimodal or bilateral CI listening on speech recognition and listening effort in noisy conditions?

Design: Sixteen adults (mean age 58 years) with CIs participated. Listening effort was measured using a dual-task paradigm and also using a self-reported rating of difficulty scale. In the dual-task measure, participants were asked to repeat monosyllabic words while at the same time press a button in response to a visual stimulus. Participants were tested in two baseline conditions (speech perception alone and visual task alone) and in the following experimental conditions: (1) quiet with an omnidirectional microphone, (2) noise with an omnidirectional microphone, (3) noise with a directional microphone, and (4) noise with a directional microphone and with a second sided CI or hearing aid. When present, the noise was fixed with a +5?dB signal-to-noise ratio. After each listening condition, the participants rated the degree of listening difficulty.

Results: Changing the microphone from omni to directional mode significantly enhanced speech recognition in noise performance. There were no significant changes in speech recognition between the unilateral and bimodal/bilateral CI listening conditions. Listening effort, as measured by reaction time, increased significantly between the baseline and omnidirectional quiet listening condition though did not change significantly across the remaining listening conditions. Self-perceived listening effort revealed a greater effort for the noisy conditions, and reduced effort with the move from an omni to a directional microphone.

Conclusions: Directional microphones significantly improve speech in noise recognition over omnidirectional microphones and allowed for decreased self-perceived listening effort. The dual task used in this study failed to show any differences in listening effort across the experimental conditions and may not be sensitive enough to detect changes in listening effort.  相似文献

Objective: Binaural beamformers are super-directional hearing aids created by combining microphone outputs from each side of the head. While they offer substantial improvements in SNR over conventional directional hearing aids, the benefits (and possible limitations) of these devices in realistic, complex listening situations have not yet been fully explored. In this study we evaluated the performance of two experimental binaural beamformers. Design: Testing was carried out using a horizontal loudspeaker array. Background noise was created using recorded conversations. Performance measures included speech intelligibility, localization in noise, acceptable noise level, subjective ratings, and a novel dynamic speech intelligibility measure. Study sample: Participants were 27 listeners with bilateral hearing loss, fitted with BTE prototypes that could be switched between conventional directional or binaural beamformer microphone modes. Results: Relative to the conventional directional microphones, both binaural beamformer modes were generally superior for tasks involving fixed frontal targets, but not always for situations involving dynamic target locations. Conclusions: Binaural beamformers show promise for enhancing listening in complex situations when the location of the source of interest is predictable.  相似文献

Objective: The National Acoustic Laboratories Dynamic Conversations Test (NAL-DCT) is a new test of speech comprehension that incorporates a realistic environment and dynamic speech materials that capture certain features of everyday conversations. The goal of this study was to assess the suitability of the test for studying the consequences of hearing loss and amplification in older listeners. Design: Unaided and aided comprehension scores were measured for single-, two- and three-talker passages, along with unaided and aided sentence recall. To characterise the relevant cognitive abilities of the group, measures of short-term working memory, verbal information-processing speed and reading comprehension speed were collected. Study sample: Participants were 41 older listeners with varying degrees of hearing loss. Results: Performance on both the NAL-DCT and the sentence test was strongly driven by hearing loss, but performance on the NAL-DCT was additionally related to a composite cognitive deficit score. Benefits of amplification were measurable but influenced by individual test SNRs. Conclusions: The NAL-DCT is sensitive to the same factors as a traditional sentence recall test, but in addition is sensitive to the cognitive factors required for speech processing. The test shows promise as a tool for research concerned with real-world listening.  相似文献

Abstract

Objective: A recent study suggested that placebo effects are a source of bias in non-blinded hearing-aid trials. Given the potential impact of this finding on the interpretation of non-blinded trials and design of future research trials, the objective of the present study was to investigate the reliability of this effect. Design: Using the same procedure as an earlier study, participants were told that they were taking part in a trial of new hearing-aid technology. Participants compared two devices that were acoustically identical, except one was described as “new” and the other as “conventional”. Participants completed a speech-in-noise test, sound quality ratings, and rated overall personal preference for both hearing aids. Study sample: Sixteen adult hearing-aid users. Results: Participants had significantly better mean speech-in-noise performance (70.9% versus 66.8%, Z = 2.30, p = 0.02, effect size Pearson's r = 0.15) and sound quality ratings for the “new” hearing aid (8.1 versus 7.4, Z = ? 2.99, p = 0.003, r = 0.28). A significant proportion of participants (75%) expressed an overall preference for the “new” hearing aid (p = 0.001, effect size φc = 0.66). Conclusion: Placebo effects reliably impact on hearing-aid trials. In order to control for placebo effects, double-blind methodology is optimal. However, when double-blinding is not possible other strategies may be appropriate.  相似文献

In this study speech intelligibility in background noise was evaluated with 10 binaural hearing-aid users for hearing aids with one omnidirectional microphone and a hearing aid with a two-microphone configuration (enabling an omnidirectional as well as a directional mode). Signal-to-noise ratio (SNR) measurements were carried out for three different types of background noise (speech-weighted noise, traffic noise and restaurant noise) and two kinds of speech material (bisyllabic word lists and sentences). The average SNR improvement of the directional microphone configuration relative to the omnidirectional one was 3.4 dB for noise presented from 90 degrees azimuth. This improvement was independent of the specific type of noise and speech material, indicating that one speech-in-noise condition may yield enough relevant information in the evaluation of directional microphones and speech understanding in noise.  相似文献

Objectives: The goal of this study is to investigate whether prior exposure to reverberant listening environment improves speech intelligibility of adult cochlear implant (CI) users.

Methods: Six adult CI users participated in this study. Speech intelligibility was measured in five different simulated reverberant listening environments with two different speech corpuses. Within each listening environment, prior exposure was varied by either having the same environment across all trials (blocked presentation) or having different environment from trial to trial (unblocked).

Results: Speech intelligibility decreased as reverberation time increased. Although substantial individual variability was observed, all CI listeners showed an increase in the blocked presentation condition as compared to the unblocked presentation condition for both speech corpuses.

Conclusion: Prior listening exposure to a reverberant listening environment improves speech intelligibility in adult CI listeners. Further research is required to understand the underlying mechanism of adaptation to listening environment.  相似文献

ObjectivesThe purpose of the current study was to examine the relation between speech intelligibility and prosody production in children who use cochlear implants.MethodsThe Beginner's Intelligibility Test (BIT) and Prosodic Utterance Production (PUP) task were administered to 15 children who use cochlear implants and 10 children with normal hearing. Adult listeners with normal hearing judged the intelligibility of the words in the BIT sentences, identified the PUP sentences as one of four grammatical or emotional moods (i.e., declarative, interrogative, happy, or sad), and rated the PUP sentences according to how well they thought the child conveyed the designated mood.ResultsPercent correct scores were higher for intelligibility than for prosody and higher for children with normal hearing than for children with cochlear implants. Declarative sentences were most readily identified and received the highest ratings by adult listeners; interrogative sentences were least readily identified and received the lowest ratings. Correlations between intelligibility and all mood identification and rating scores except declarative were not significant.DiscussionThe findings suggest that the development of speech intelligibility progresses ahead of prosody in both children with cochlear implants and children with normal hearing; however, children with normal hearing still perform better than children with cochlear implants on measures of intelligibility and prosody even after accounting for hearing age. Problems with interrogative intonation may be related to more general restrictions on rising intonation, and the correlation results indicate that intelligibility and sentence intonation may be relatively dissociated at these ages.Learning outcomes: As a result of this activity, readers will be able to describe (1) methods for measuring speech intelligibility and prosody production in children with cochlear implants and children with normal hearing, (2) the differences between children with normal hearing and children with cochlear implants on measures of speech intelligibility and prosody production, and (3) the relations between speech intelligibility and prosody production in children with cochlear implants and children with normal hearing.  相似文献

Abstract

Objective: The acceptable noise level (ANL) test is used for quantification of the amount of background noise subjects accept when listening to speech. This study investigates Danish hearing-aid users’ ANL performance using Danish and non-semantic speech signals, the repeatability of ANL, and the association between ANL and outcome of the international outcome inventory for hearing aids (IOI-HA). Design: ANL was measured in three conditions in both ears at two test sessions. Subjects completed the IOI-HA and the ANL questionnaire. Study sample: Sixty-three Danish hearing-aid users; fifty-seven subjects were full time users and 6 were part time/non users of hearing aids according to the ANL questionnaire. Results: ANLs were similar to results with American English speech material. The coefficient of repeatability (CR) was 6.5–8.8 dB. IOI-HA scores were not associated to ANL. Conclusions: Danish and non-semantic ANL versions yield results similar to the American English version. The magnitude of the CR indicates that ANL with Danish and non-semantic speech materials is not suitable for prediction of individual patterns of future hearing-aid use or evaluation of individual benefit from hearing-aid features. The ANL with Danish and non-semantic speech materials is not related to IOI-HA outcome.  相似文献

ObjectiveHearing aid amplification rationales have typically been developed by using global averages of the long-term average speech spectrum (LTASS) from Western European languages. However, there are few reports on hearing-aid amplification based on acoustic-phonetic characteristics of the Japanese language. This study’s objective is to investigate the LTASS for Japanese, and to compare a typical amplification rationale originally developed mainly for Western European languages with an amplification rationale specifically adjusted to the LTASS for Japanese.MethodsLTASS for two speech materials provided by four Japanese talkers were analyzed using 1/3 octave bandwidth filters. The speech was recorded with different levels of vocal effort, yielding three LTASS for “soft”, “moderate” and “loud” speech. From these results, a gain offset of the hearing-aid amplification for Japanese was obtained as compared to ANSI S3.5. Speech intelligibility for an amplification rationale for Western European languages and the newly-developed Japanese version was obtained for presentation levels of 50 dB SPL, 65 dB SPL and 80 dB SPL. Nineteen people with mild to moderate hearing loss participated in the speech intelligibility experiment. Scores in% correct were arcsine-transformed and subjected to repeated measures ANOVA with pairwise comparisons of significant main effects using Bonferroni adjustments for multiple comparisons.ResultsThe LTASS for Japanese was slightly different from the values of previous reports. A comparison of LTASS values to ANSI S3.5 with values for Japanese showed that the Japanese amplification rationale for “moderate” speech levels required more gain in the low-frequency area, and less gain in the high-frequency area. There was no significant difference in the speech intelligibility level between the amplification characteristics of Western European languages and Japanese language at each presentation level.ConclusionIt was shown that for hearing-aid amplification for Japanese, adjustments based on LTASS differences for Western European Languages could be made. This preserved speech intelligibility at the same level as the original amplification rationale, suggesting that there was no need to consider differences in phonetics of Japanese to optimize speech understanding.  相似文献

The relation between rated intelligibility and correct key word repetitions of sentences was examined in listeners with normal hearing and listeners with hearing loss. Ten lists of 10 CID sentences were arranged in two randomly ordered groups. Listeners were asked to complete two tasks: (1) rate the intelligibility of 50 sentences on a scale of 0 to 100%, and (2) repeat each of the 50 sentences, which were scored as the number of key words repeated correctly. Sentences were presented at five signal-to-noise ratios. The start level for sentence presentation was established using the method of adjustment and the Revised Speech Intelligibility Rating passages. Correlations of rated intelligibility and correct repetition were 0.85 for listeners with normal hearing and 0.86 for listeners with hearing loss. This study confirms the earlier work of Speaks, Parker, Harris, and Kuhl (1972), and the findings demonstrate that listeners with hearing loss preserve the ability to rate the intelligibility of speech.  相似文献

Background: Sudden sensorineural hearing loss (SSNHL) is commonly encountered in clinical practice.

Aim/Objective: Determine if local administration of corticosteroids to the inner ear can improve hearing and speech intelligibility after the failure of conventional treatment for SSNHL loss when administered for 10 days after the onset of the hearing loss in a large cohort of 77 patients.

Materials and methods: A Silverstein MicroWick? was placed under local anesthesia and endoscopic control in the round window niche, allowing self-administration of methylprednisolone twice daily for four weeks.

Results: An improvement of the pure tone average was shown in 31% of patients. Speech intelligibility improved significantly in 55% of the total cohort and in 34% of the population with a stable pure tone average. Among the 77 patients, 22% used a hearing aid. Only 14% of the patients were hearing-aid users in the group with an improvement in speech intelligibility as opposed to 31% in the failure group.

Conclusion and significance: Local administration of steroids to the inner ear through the round window route improves hearing and speech intelligibility in patients after failure of conventional therapy. The use of a hearing aid was reduced by 50% when speech intelligibility was improved.  相似文献
