目的采用超声射频信号血管分析技术对妊娠高血压综合征(PIH)孕妇颈动脉内中膜厚度及弹性进行综合评估,并与正常妊娠孕妇比较,观察PIH孕妇颈动脉血管功能的变化.方法PIH孕妇27例(PIH组),平均动脉压108 mm Hg(1 mm Hg=0.133 kPa),采用年龄及孕周匹配的30名正常妊娠孕妇作为健康对照组.彩色多普勒超声诊断仪分别启用超声射频信号技术中的血管内-中膜定量技术(QIMT)及血管硬度定量分析技术(QAS)测量两组孕妇颈动脉内-中膜厚度IMT及弹性.结果 PIH组孕妇颈总动脉IMT、脉搏波传播速度(PWV)、等容收缩期到射血期转折点压力(PT1)、动脉增强压(AP)及动脉压增强指数(AIx)分别为(466.84±118.50)μm、(7.09±1.97)m/s、(127.50±14.29)mm Hg、(5.14±3.39) mm Hg及(7.58±8.73)%,健康对照组分别为(386.58±125.79)μm、(5.95±1.11) m/s、(105.89±11.02)mm Hg、(1.98±2.19)mm Hg及(-4.79±7.92)%.两组比较差异有统计学意义(t值分别为2.660、2.660、3.460、3.460、3.460,P值均<0.01).结论 与正常妊娠孕妇比较,PIH孕妇颈动脉内中膜明显增厚,弹性减低.超声射频信号血管分析技术可敏感地反映弹性血管结构和功能变化.  相似文献

目的 探讨内中膜厚度(QIMT)及动脉僵硬度(QAS)测量技术评价高血压患者颈动脉弹性功能的临床价值.方法 原发性高血压患者137例,按有无斑块形成分为高血压无斑块组(63例)及高血压斑块组(74例),并与87例正常人相对照.应用QIMT软件描记颈动脉内中膜厚度(IMT),QAS软件测量颈动脉内径(d)、扩张度(e)、弹性系数(α)、弹性系数(β)、僵硬度系数(CC)、脉搏波传播速度(PWV)及压力局部收缩压(LOCPsys)等,分析三组上述指标的差别、与血压的关系及弹性指标之间的相关性.结果 随颈动脉壁病变的加重,高血压组颈动脉IMT、d、α、β、PWV、LOCPsys逐渐升高,CC逐渐降低;两侧颈动脉弹性指标之间存在差异.α、β、PWV OR值较大,β OR值最大.IMT、e、α、β、PWV、LOCPsys同收缩压和脉压呈正相关,CC与之呈负相关;颈动脉弹性指标间相关性很大.结论 随高血压颈动脉壁病变的加重,颈动脉IMT逐渐增厚,弹性减低;应用QIMT和QAS技术能准确评价颈动脉弹性.
Abstract:
Objective To explore the value of quantivtative intima-media thickness(QIMT) and quantitative arterial stiffness (QAS) technique on evaluating the elasticity of carotid artery in patients with essential hypertension.Methods One hundred and thirty-seven patients with primary hypertension were divided into two groups according with or without atherosclerotic plaques:hypertensive group without plaque (63 cases) and hypertensive group with plaque (74 cases),and 87 healthy aged subjects were selected as the control group.The intima media thickness (IMT) was tracted with QIMT software.The internal diameter (d),expansion (e),elastic coefficient α and β,compliance coefficient (CC),pulse wave velocity (PWV) and local systolic pressure (LOCPsys) were measured with QAS software.The differences of the parameters among the groups,and the relation with pressure and the correlation between the elastic indexs were analyzed.Results IMT,d,α,β,PWV and LOCPsys in hypertensive group increased,and CC decreased gradually with the lesion development of carotid aterial wall.There was difference in elastic indexs between the left and right carotid.The OR of α,β,PWV was comparatively large and the OR of β was the largest.There was positive relevance between the indexs of IMT,e,α,β,PWV,LOCPsys and systolic blood pressure and pulse pressure,and there was negative correlation between CC and systolic blood pressure and pulse pressure.The relevance between the elastic indexs was large.Conclusions The intima-media thickness of carotid increased gradually,and the elasticity decreased with the lesion development of carotid aterial wall in patients with hypertension.It can evaluate the arterial elasticity accurately and quantitavely using the QIMT and QAS technique.  相似文献

目的:应用超声射频信号血管内中膜分析(quality intima-media thickness,QIMT)技术测量肥胖儿童的颈动脉内中膜厚度(intima-media thickness,IMT),探讨肥胖儿童代谢异常状况与颈动脉IMT间的关系。方法:由1名高年资超声医师应用QIMT技术,测量32例肥胖儿童(肥胖组)和20名正常对照儿童(对照组)的双侧颈总动脉IMT。结果:肥胖组与对照组儿童的空腹血糖(FBG)、总胆固醇(TCH)及低密度脂蛋白(LDL)差异均无统计学意义(P>0.05);肥胖组儿童的三酰甘油(TG)高于对照组,差异有统计学意义(P<0.01),高密度脂蛋白(HDL)则明显低于对照组,差异有统计学意义(P<0.05);肥胖组儿童的颈总动脉IMT明显高于对照组,差异亦有统计学意义(P<0.01)。结论:肥胖儿童的颈动脉IMT已开始增加,并可合并高脂血症,提示动脉硬化(AS)是可在小儿时期得病、成人发病。超声检查颈动脉IMT简便、安全无创,可作为早期发现AS的一个良好的预报因子。  相似文献

Large artery stiffness and small artery structural changes are both cardiovascular risk factors. Arterial stiffness increases with age and blood pressure (BP), but it is unclear in which way large artery pulse wave velocity (PWV) and peripheral vascular resistance are related and whether age has any influence. In a cross‐sectional study, PWV and forearm minimum vascular resistance (Rmin) was compared with emphasis on the impact of age. Normotensive (n = 53) and untreated hypertensive (n = 23) subjects were included based on 24‐h BP measurements. Age ranged from 21 to 79 years with an even distribution from each age decade. PWV was assessed using tonometry. Forearm Rmin was measured by venous occlusion plethysmography at maximal vasodilatation induced by 10 min of ischaemia in combination with skin heating and hand grip exercise. In both normotensive and hypertensive subjects, PWV correlated significantly with age and BP. Based on median age, both groups were assigned into two equally large subgroups. Normotensive older (66 ± 7 years) and younger (35 ± 10 years) persons had different carotid‐femoral PWV (7·9 ± 1·8 versus 5·7 ± 0·9 m/s, P<0·01), but similar Rmin values (3·7 ± 0·9 versus 3·6 ± 1·2 mmHg/ml/min/100 ml). Hypertensive older (63 ± 6 years) and younger (40 ± 10 years) also had different PWV (8·0 ± 1·5 versus 6·7 ± 1·1 m/s, P<0·05), but the older had lower Rmin (3·1 ± 0·8 versus 4·7 ± 2·2 mmHg/ml/min/100 ml, P<0·05). In a regression analysis adjusting for age, BP, gender and heart rate, no correlation was seen between PWV and Rmin. The data suggest that age differentially affects PWV and Rmin and that BP can increase in older persons without affecting Rmin.  相似文献

Our objective was to study the interrelationships between longitudinal movement of the wall of the common carotid artery and the conventional measures of arteriosclerosis in a large and well-characterized study population. Successful longitudinal movement analyses were performed on 292 subjects. The peak-to-peak and retrograde amplitudes of the longitudinal movement were directly correlated with carotid artery distensibility (r = 0·21, P<0·001 and r = 0·23, P<0·001, respectively) and inversely correlated with pulse wave velocity (r = −0·14, P<0·05 and r = −0·17, P<0·01, respectively). All longitudinal motion parameters were independent of brachial flow-mediated dilatation and intima-media thickness. Our findings indicate that arterial stiffening modulates longitudinal movement and, therefore, measurement of longitudinal movement can be of value in the assessment of vascular health.  相似文献

目的探讨高血压患者室间隔厚度与血管内皮功能及颈动脉内膜中层厚度的关系。方法选择中老年高血压患者85例,应用彩色多普勒超声检测心脏室间隔厚度、肱动脉内皮依赖性舒张功能(FMD)及颈动脉内膜中层厚度(IMT),根据室间隔厚度分为室间隔肥厚组和室间隔正常组,并测定一氧化氮(NO)、内皮素-1(ET-1)、空腹血糖、总胆固醇、低密度脂蛋白等生化指标,同时选择我院体检中心健康人群30名作为健康对照组。结果室间隔肥厚组FMD、NO含量较室间隔正常组及对照组显著降低,颈动脉IMT明显增厚,ET-1浓度明显增高,差异有统计学意义(P0.05)。相关性分析提示室间隔厚度与IMT(r=0.533)、ET-1(r=0.673)呈正相关,与FMD(-0.628)、NO(r=-0.629)呈负相关,颈动脉IMT与肱动脉FMD呈负相关(r=-0.706)。结论高血压患者存在血管功能障碍,且室间隔肥厚患者血管内皮功能及颈动脉损害程度更严重。  相似文献

目的应用极速脉搏波技术(UFPWV)评价单纯收缩期高血压(ISH)患者、双期高血压(SDH)患者颈总动脉弹性情况;比较UFPWV技术所检测的颈总动脉前壁心脏收缩起始脉搏波速度(BS)、心脏收缩末期脉搏波速度(ES)值与传统颈总动脉内中膜厚度(IMT)值在早期动脉硬化评估方面的优越性。 方法选取于2017年12月至2018年9月就诊于华北理工大学附属医院的住院患者125例:包括ISH患者42例[(56.69±6.16)岁]、SDH患者39例[(55.97±7.23)岁]、正常血压人群44名为健康对照组[(55.18±8.20)岁]。常规二维测量IMT,采用UFPWV检测BS、ES。对上述各组间的BS、ES及IMT值进行单因素方差分析,组内采用LSD-t检验行两两比较。 结果SDH组IMT、BS值明显高于健康对照组,差异均有统计学意义(t=-2.302、-3.143,P均<0.05)。ISH组IMT、BS、ES值均明显高于健康对照组,差异均有统计学意义(t=-5.024、-3.961、-4.010,P均<0.05)。ISH组BS、ES值均明显高于SDH组,差异均有统计学意义(t=2.596、-2.801,P均<0.05)。 结论ISH患者较SDH患者血管弹性减低明显,更易发生动脉硬化。在评价早期评价动脉硬化方面,UFPWV较常规二维IMT测量更为敏感。  相似文献

The purpose of the study was to describe the relation between wall elasticity and intima-media thickness in the human carotid artery prior to the development of atherosclerotic plaques. Fifty-eight apparently healthy men, aged 42-65 years (mean 55 years), without symptoms of cardiovascular disease were studied. Thickness and elastic properties of the common carotid artery wall were assessed using ultrasonography and non-invasive arterial pressure measurements. The relation between the calculated intima-media area and the pressure strain elastic modulus was positive and statistically significant on the right but not on the left side. No statistically significant relations were found between the calculated intima-media area and the stiffness or between the intima-media thickness and the elastic modulus or stiffness on either side. Thus, the relations between the common carotid artery intima-media thickness/calculated intima-media area and the common carotid artery elastic modulus/stiffness are weak. In regions without atherosclerotic plaques, the elastic properties of the human carotid artery wall do not seem to be influenced by the wall thickness in an important way.  相似文献

目的 应用高频超声射频数据处理(ultrasound radio-frequency data,RF-data)技术定量检测健康人颈总动脉内-中膜厚度(intima-media thickness,IMT)和血管弹性,确定相关参数的正常值范围.方法 对160例健康人根据年龄分为4组(30~39岁,40~49岁,50~59岁,60~69岁),每组40例,男性20例,女性20例;RF-data技术获取健康者双侧颈总动脉IMT、颈总动脉扩张性(CD)、单点脉搏波传播速度(PWVβ)、僵硬度(β),并分析各参数之间及与年龄、性别的关系.结果 ①IMT、PWVβ和β与年龄呈显著正相关,CD与年龄负相关;IMT、CD、PWVβ和β与性别相关性不显著;②IMT与PWVβ、β呈显著正相关,而与CD相关性不显著;PWVβ与β显著正相关,而与CD显著负相关;③随着年龄的增加,IMT亦随之增高(P<0.05).在30~39岁、40~49岁组中,随年龄增加PWVβ增高,而CD减低明显(P<0.05);50~59岁组PWVβ和β增高显著(P<0.05);在60~69岁组,随年龄增加CD、PWVβ、β差异无统计学意义.④CD、PWVβ和β的正常值在60~69岁组分别为(329.39±102.06)μm、(7.22±1.91)m/s和10.04±4.91,而在<60岁者随年龄不同而异.结论 RF-data技术能够准确检测颈总动脉IMT和血管弹性,根据年龄确立正常值范围,为临床定量评估血管性能提供新的手段.  相似文献

目的探讨辛伐他汀对高血压合并颈动脉硬化患者肱踝脉搏波传导速度(baPWV)及颈动脉内膜中层厚度(IMT)的影响。方法 120例原发性高血压合并颈动脉硬化患者,随机分为辛伐他汀组和对照组,各60例。2组患者均维持原有降压方案不变,治疗组在原有降压方案基础上给予辛伐他汀治疗。观察2组患者治疗前后的血脂、IMT及baPWV变化情况。结果与治疗前比较,辛伐他汀组治疗后血清TC、TG、LDL-C水平明显降低(P<0.01),血清HDL水平明显升高(P<0.01);治疗后,辛伐他汀组血清TC、TG、LDL-C水平明显低于对照组(P<0.01),血清HDL水平明显高于对照组(P<0.01)。与治疗前比较,2组治疗后baPWV明显降低(P<0.01),辛伐他汀组治疗后IMT明显减少(P<0.01);与对照组治疗后比较,辛伐他汀组IMT、baPWV水平明显增加(P<0.01)。结论辛伐他汀可有效降低血脂水平,通过降低IMT及baPWV防止动脉粥样硬化形成。  相似文献

目的:应用超声观察原发性高血压患者颈动脉内膜-中层厚度(intima-mediathickness,IMT)并探讨其血管内皮功能及两者的相关性为早期干预、防治心脑血管疾病提供有力证据。方法:高血压组:选择山东省泰山疗养院126例原发性高血压患者,平均年龄(64±8)岁。另选30例健康人为对照组。测定126例高血压患者颈动脉内膜-中层厚度及反应性充血时和舌下含服硝酸甘油后肱动脉内径的变化,并与对照组进行比较,分析各指标相关性。结果:高血压患者IMT犤(1.06±0.05)mm犦明显比正常对照组犤(0.73±0.02)mm犦增厚(t=57.29,P<0.001)。反应性充血时肱动脉内径的扩张程度明显低于对照组(t=25.8P<0.001)。颈动脉内膜厚度与反应性充血时和舌下含服硝酸甘油后肱动脉内径变化呈显著负相关(r=-0.538,-0.46,P<0.001)。结论:原发性高血压患者存在明显的血管内皮依赖性及非依赖性舒张功能的损伤;内皮功能紊乱引起血管重构、肥厚、斑块形成。原发性高血压的内皮功能损伤促进了动脉粥样硬化的发生并加速了动脉粥样硬化的进程。  相似文献

目的 利用血管回声跟踪技术(ET)测量高血压病患者的颈动脉弹性功能,探讨管径增大指数(AI)、单点脉搏波传导速度(ewv[3)与动脉弹性功能的关系。方法高血压病患者90例,均分为三组:A组肱动脉脉压差〈60mmHg,B组脉压差为60~80mmHg,C组脉压差≥80mmHg。测量患者颈动脉ET参数:硬化指数(B)、压力一应变弹性系数(Ep)、顺应性(AC)、AI、PWV[3及肱动脉脉压差,比较不同脉压差组问颈动脉弹性功能参数的差异。结果A、B、C三组间收缩压依次增高,B、C组收缩期颈动脉内径(Ds)、舒张期颈动脉内径(Dd)较A组增高,差异均有统计学意义(P〈0.05)。B组和C组β、Ep、AC、AI较A组增高,差异有统计学意义(P〈0.05)。AI与年龄、IMT、脉压差呈正相关,与Ds—Dd呈负相关;PwVB与年龄、收缩压、脉压差、Dd呈正相关,与Ds—Dd呈负相关。结论AI、PWVβ随脉压差增大而增大,参数异常早于脉压差增大,二者结合能较敏感地早期反应动脉弹性功能。  相似文献

目的应用实时剪切波弹性成像及彩色脉搏波成像技术分析高血压患者颈总动脉纵向及环向血管壁硬度,比较不同级别高血压患者颈总动脉血管壁硬度差异,探讨2种技术在高血压分级诊断中的应用价值。 方法选取2016年2至12月云南省第二人民医院收治的临床确诊的高血压患者120例。根据中国高血压防治指南(2010年版)分级分为1、2、3级高血压组3组各40例。另选取同期云南省第二人民医院体检无高血压病史的健康志愿者40名作为健康对照组。应用实时剪切波弹性成像技术获得所有研究对象颈总动脉前壁及后壁的弹性模量值(Mean、Max、Min),得到颈动脉的纵向血管壁硬度;应用彩色脉搏波成像技术获得所有研究对象颈总动脉壁的脉搏波速度,包括收缩期开始时的脉搏波速度(PWVBS)、收缩期结束时的脉搏波速度(PWVES),得到颈动脉的环向血管壁硬度。采用单因素方差分析比较不同级别高血压组患者与健康对照组受试者颈总动脉壁硬度值(Mean、Max、Min、PWVBS、PWVES),进一步组间两两比较采用LSD-t检验;采用Pearson相关分析法分析高血压患者颈动脉壁硬度值(Mean、Max、Min、PWVBS、PWVES)与高血压级数的相关性。 结果颈总动脉(前壁及后壁)的弹性模量值(Mean、Max、Min)均为高血压3级组>高血压2级组>高血压1级组>健康对照组,且任意2组间比较差异均有统计学意义。Pearson相关分析结果显示,颈总动脉(前壁及后壁)的弹性模量值(Mean、Max、Min)与高血压级数均有相关性(r=0.817、0.767、0.796,P均<0.05)。随着高血压级数的增高,高血压患者颈总动脉(前壁及后壁)的弹性模量值(Mean、Max、Min)呈增高趋势。颈总动脉脉搏波速度(PWVBS、PWVES)均为高血压3组>高血压2组>高血压1组>健康对照组,且任意2组间比较差异均有统计学意义。Pearson相关分析结果显示,脉搏波速度(PWVES、PWVBS)与高血压级数均有相关性(r=0.650、0.585,P<0.05)。随着高血压级数的增高,高血压患者脉搏波速度(PWVBS、PWVES)呈增高趋势。 结论实时剪切波弹性成像及彩色脉搏波成像技术为高血压分级诊断提供了一种有价值的参考。随着高血压级数的增高,高血压患者颈总动脉壁的纵向及环向血管壁硬度均呈增高趋势,且两者具有一致性。  相似文献

Isolated systolic hypertension is the predominant type of hypertension in the elderly and is associated with cardiovascular complications such as stroke, coronary heart disease and heart failure. In this review, the role of arterial stiffness, endothelial function, atherosclerosis and oxidative stress in the pathogenesis of isolated systolic hypertension is extensively discussed. Placebo-controlled intervention trials such as the Systolic Hypertension in Europe Trial and the Systolic Hypertension in the Elderly Program have clearly shown that pharmacological treatment of isolated systolic hypertension improves outcome in the elderly. Nevertheless, isolated systolic hypertension remains the major subtype of untreated and uncontrolled hypertension.  相似文献

目的 应用射频超声血管定量技术检测乙肝患者的颈总动脉弹性变化,评价其血管功能。 方法 选取40例乙肝病毒(HBV)表面抗原阳性的肝病患者和34例年龄、性别匹配、无肝病病史的健康人,对以上两组研究对象分别进行肝脏声辐射力脉冲成像检查和颈总动脉射频超声检查,记录颈总动脉弹性参数和肝脏硬度参数,前者包括血管内中膜厚度(IMT)、脉搏波传导速度(PWV)、动脉管壁扩张性系数(DC)和顺应性系数(CC)、硬度指数、硬度指数,转折点压力P(T1),后者为剪切波传导速度ARFI,比较两组各项参数,并行统计学分析。 结果 乙肝患者组ARFI值明显高于对照组(2.07±0.67 m/s vs 1.03±0.10 m/s, P<0.001)。乙肝患者组颈总动脉的QIMT高于对照组(581.8±115.1 um vs 523.4±72.2 um, P<0.05)。PWV(7.52±1.71 m/s vs 7.83±1.63 m/s)、CC(0.90±0.37 /kPa vs 0.78±0.30 /kPa)、(5.19±2.48 vs 5.66±2.31)、DC、、P(T1)等弹性参数两组间未见显著性差异(P>0.05)。结论 乙肝患者的肝脏硬度和颈总动脉内中膜厚度高于健康对照组,PWV、CC、等功能性参数没有显著性差异。利用超声检测技术对乙肝患者颈总动脉的监测,将有益于对其血管功能和结构的关注。  相似文献

目的 探讨血管回声跟踪(ET)技术检测正常高值血压者颈总动脉弹性功能的变化情况及其临床意义.方法 应用ET技术检测63例正常高值血压者(HNBP组)与50例健康成人(正常对照组)双侧颈总动脉的弹性参数,包括弹性系数(β)、僵硬度(Eρ)、顺应性(AC)、增大指数(AI)及单点脉搏波传导速度(PWVβ),并使用常规血管超声测量颈动脉内-中膜厚度(IMT),结果进行对比分析.结果 组内双侧颈总动脉的IMT及其各弹性参数比较差异均无统计学意义;两组间上述各指标比较,HNBP组的β、Eρ和PWVβ增高,AC降低,差异均有统计学意义(P 〈 0.01),AI和IMT无明显变化,差异无统计学意义.结论 正常高值血压者颈总动脉Eρ增加,ET技术能较早发现颈总动脉弹性的改变,对临床早期预防心脑血管疾病的发生具有重要意义.  相似文献

目的:探讨老年高血压患者动脉脉搏波速率与血管危险因子同型半胱氨酸浓度的关系。方法选取2012年2月-2013年2月在本院老年科住院的老年高血压患者52例,在空腹状态下测定血浆同型半胱氨酸浓度及动脉脉搏波速率。结果血浆同型半胱氨酸浓度与脉搏波速率具有显著相关性(r=0.8854,P<0.001)。结论在血压控制的情况下,老年高血压患者血浆同型半胱氨酸浓度与脉搏波速率呈正相关。  相似文献

目的应用彩色多普勒超声分析颈动脉内-中膜厚度(IMT)及血流动力学变化。方法超声检测111例糖尿病患者和97例健康者为对照组的双侧颈总动脉IMT、收缩期峰值血流速度(PSV)、舒张期末血流速度(EDV)、阻力指数及搏动指数。结果糖尿病组颈总动脉、分叉处及颈内动脉平均IMT均高于对照组(P〈0.05),糖尿病组颈总动脉及颈内动脉(PSV)、EDV低于对照组(P〈0.05);两组阻力指数、搏动指数比较,没有统计学差异(P〉0.05)。结论彩色多普勤招声可直接反应颈动脉IMT及血流动力学改变,对于糖尿病患者并发症的防治具有重要的意义。  相似文献
