Objective  

In clinical positron emission tomography (PET) studies for oncology, it is occasionally required to differentiate a diffuse increase in bone marrow (BM) F-18 fluoro-2-deoxyglucose (FDG) uptake due to the involvement of malignancy or hematopoietic disease and that due to the administration of hematopoietic cytokines, an inflammation reaction, or stimulation by some types of malignancy. The objectives of this study were to clarify the relationships between BM F-18 FDG uptake and blood parameters as well as age, and also to determine the degree of F-18 FDG accumulation that constitutes an abnormal level referring to blood parameters.  相似文献

Whole-body fluorine-18 fluorodeoxyglucose (F-18 FDG) positron emission tomography (PET) has been useful in the management of a variety of malignancies. In patients with chemotherapy followed by bone marrow stimulants such as granulocyte colony-stimulating factor and granulocyte-macrophage colony-stimulating factor, the bone marrow will have diffuse, increased FDG accumulation. Therefore, diffuse bone marrow FDG uptake is commonly attributable to the effect of hematopoietic cytokines. However, diffuse bone marrow FDG uptake can also be caused by bone marrow involvement by malignancy. The authors report a patient with diffuse bone marrow involvement of Hodgkin disease that appears indistinguishable from hematopoietic cytokine-mediated FDG bone marrow uptake.  相似文献

Alveolar rhabdomyosarcoma (RMS) accounts for 20% to 30% of childhood RMS and is associated with a prognosis worse than embryonal RMS. Disseminated RMS can present with extensive bone marrow involvement. Assessing the extent of the tumor is critical, because therapy and prognosis depend on the degree to which the mass has spread beyond the primary site. The value of F-18 FDG PET in patients with RMS has been reported in some series but none specifically involving bone marrow. Children have a highly cellular hematopoietic bone marrow and differentiation of a highly cellular marrow from neoplastic infiltration may be difficult. Various other conditions associated with diffuse FDG uptake in the bone marrow include marrow hyperplasia resulting from hemolytic/iron-deficiency/blood-loss anemia, after chemotherapy with granulocyte/macrophage colony-stimulating factor and recombinant erythropoietin treatment, leukemia, non-Hodgkin lymphoma, and myelodysplasia. It is therefore important to consider the above differential diagnoses in mind when evaluating cases of unexpected marrow uptake in F-18 FDG PET studies. We report here a case of RMS with diffuse bone marrow involvement detected on F-18 FDG PET wherein FDG PET was useful in determining the true extent (primary and metastases) of RMS before definitive therapy and the valuable adjunct role it has with structural imaging in management.  相似文献

A 60-year-old male was referred for a positron emission tomography (PET) scan using F-18 fluoro-2-deoxyglucose (F-18 FDG) for evaluation of a right lung opacity identified on a computed tomography (CT) scan. The patient also had a history of idiopathic myelofibrosis. The PET scan revealed markedly increased uptake throughout the spleen and liver, which were massively enlarged. There was also significantly increased uptake diffusely throughout the bone marrow. These findings are a reflection of the patient's myelofibrosis.  相似文献

A patient with a history of colon carcinoma and computed tomography (CT) evidence suggestive of a liver metastasis underwent an F-18 FDG PET scan. The PET scan confirmed a metastasis in the left lobe of the liver. The study also demonstrated bilateral symmetric uptake involving the inferior subscapular regions. Correlation with the previous CT showed this to correspond to bilateral elastofibroma dorsi, which had been prospectively reported on the CT. Elastofibroma dorsi is a rare benign mass consisting of fibroelastic tissue and fat. It has been previously described on PET, CT, ultrasound, and magnetic resonance imaging (MRI). On CT, the masses are of soft tissue density, similar to adjacent musculature, and may contain linear areas of low density secondary to fat. On MRI, elastofibroma dorsi has been shown to have variable enhancement, which may reflect increased vascularity of the lesion. In this case, the masses demonstrated low-grade diffuse F-18 FDG uptake.  相似文献

Endometrial cancer, which is one of the most common malignant gynecologic diseases, was detected by F-18 fluorodeoxyglucose (FDG) positron emission tomography (PET) in a 60-year-old woman with abdominal distention. FDG PET revealed heterogeneous and marked accumulation in the endometrium, which was thought to represent endometrial cancer. In addition, focal intense accumulation of FDG in both lungs suggestive of lung metastases were noted. Endometrial cancer and lung metastases were confirmed by endometrial biopsy and computed tomography of the chest, respectively.  相似文献
