In the primary dentition, traumatic injuries affecting the tooth-supporting structures are common due to increased bone resilience in children. Crown-root fracture, defined as a fracture involving enamel, dentin, and cementum, is uncommon in the primary dentition, comprising only 2% of dental traumas. This article reports the treatment and follow-up of a 2-year-old boy who suffered a traumatic crown-root fracture involving a primary anterior incisor that was fused to a supernumerary tooth.  相似文献

Vertical root fractures (VRFs) in teeth without endodontic treatment are uncommon. A clinical study was done on 64 cases of VRFs in 61 Chinese patients to define better the clinical characteristics, diagnostic signs, and possible causes. Results showed that fractures occurred most often in first molars of patients between 40 and 69 yr of age, and the incidence was two times higher in males than in females. Flat roots with smaller mesiodistal diameters were more prone to fracture. The majority (79%) of patients had intact dentition with less than four teeth missing. Fractured teeth were frequently attrited with no restorations. VRFs may result from excessive or repetitive masticatory force due to damaging chewing habits exerted on flat roots of smaller mesiodistal diameter. Nonendodontic VRFs seem to occur more frequently than earlier believed and may represent an underdiagnosed clinical entity deserving of our attention.  相似文献

Abstract –  The increased incidence of traumatic injuries to anterior teeth is a consequence of leisure activities, where the most common injuries are crown fractures. Treatment of the dental trauma is complex and requires a comprehensive and accurate diagnostic and treatment plan. It is also important to consider the biological, functional, esthetic and economic aspects, as well as the patient's desire. The purpose of this article is to report a case that shows the multidisciplinary approach required to successfully manage the rehabilitation of a maxillary central incisor with a complex crown fracture and a maxillary lateral incisor, that at first presented an oblique crown-root fracture, and after the orthodontic extrusion, suffered a more apical new crown-root fracture.  相似文献

In the pre-adhesive era, fractured teeth needed to be restored either with pin-retained inlays or cast restorations that sacrificed healthy tooth structure and were a challenge for clinicians to match with adjacent teeth. The development of adhesive dentistry has allowed dentists to use the patient's own fragment to restore the fractured tooth. Three cases are presented here with complicated crown fracture of maxillary central incisors; the reattachment of the fractured tooth segment in this case has been performed using different combinations of techniques, viz. simple reattachment, circumferential bevel and internal dentinal groove.  相似文献

Traumatic tooth injuries are common in children. When permanent teeth are involved, it can be a challenge to save these teeth. This clinical case study describes the multidisciplinary treatment of a complex crown fracture and luxation of a right maxillary incisor along with esthetic management. After periodontal surgery including guided bone regeneration and endodontic treatment, we used a glass-fibre-reinforced composite post to increase retention and distribute stress along the root. The restoration was completed using composite in an incremental technique. During follow-up appointments, clinical and radiographic examinations revealed no root canal or periodontal problems, suggesting the efficacy of the treatment in retaining the fractured tooth. Periodontal surgery with endodontic treatment is an alternative treatment for severe trauma in permanent teeth.  相似文献

Crown-root fractures are complex traumatic injuries that require multidisciplinary management and afford uncertain prognosis. The purpose of this clinical article was to report the case of crown-root fracture where a multidisciplinary approach was successfully executed. A 10-year-old male patient who suffered a complicated crown-root fracture on a permanent maxillary central incisor was treated using an intentional 180°-rotation replantation technique, followed by endodontic therapy and restoration with resin composite. Two years after the initial treatment, the case was stable and the tooth was restored with a resin core and a ceramic crown. This article describes how adequate multidisciplinary treatment planning, coordination, and execution are necessary for similar cases to be properly managed. It can be concluded that the intentional replantation with 180°-rotation allowed a more conservative approach, showing, in this case, a clinical success without resorption.  相似文献

Splinting teeth to each other allows weakened teeth to be supported by neighbouring teeth, although the procedure can make oral hygiene procedures difficult. Several methods for splinting teeth, both extracoronal and intracoronal, as well as the materials commonly used for splinting, are described and illustrated. The purpose of this paper was to present a case of surgical exposure of a horizontally impacted permanent maxillary left central incisor followed by fixed orthodontic treatment to eruption in a 9-year-old boy with a 2-mm overjet and 2-mm overbite who had sustained a traumatic injury to his primary maxillary central incisors when he was 6-years-old. Panoramic, occlusal, and cepholometric radiographs showed the affected tooth's crown to be positioned horizontally, and a CT scan showed the impacted central incisor's crown to be located below the anterior nasal spine and its apex to be located parallel to the palatal plane. A treatment plan was devised that included surgically exposing the impacted teeth and applying extrusive forces in the direction of the occlusal plane to bring about eruption. After 22 months of treatment, the central incisor erupted into the oral cavity. The patient now possesses the confidence to smile and enhanced self-esteem.  相似文献

Abstract –  This clinical case study describes a multidisciplinary modified technique for the treatment of an oblique root fracture. A-38-year old woman with a history of trauma and a broken tooth was referred to our clinic. There was an oblique crown fracture extending the coronal third of the root just underneath the cingulum of the crown. The patient was willing to keep her tooth in function by any means. Two weeks after root canal therapy the flap was raised. The coronal two third of the root canal was enlarged. An impression of the enlarged root canal and tooth surface with surrounding bone was taken. The flap was sutured and the impression was sent to the laboratory for the post-casting preparation. After 7 days, the flap was opened again and casting was cemented. The flap was sutured. Ten days after the second surgery, three anterior teeth were prepared and a final impression was taken for the preparation of porcelain crowns. Restoration was finished 15 days after the second operation. The patient is still under maintenance therapy and the 12 month results are presented in this report. Extraction may not be the only alternative for the root fractures. Even for fractures under the alveolar margin, alternative multidisciplinary approaches can be used to restore and allow the tooth to survive.  相似文献

A previous attempt to locate the root canal system in the maxillary left central incisor with a bur created a perforating root defect at the level of the alveolar crest. The patient received root canal therapy. Next, vertical root extrusion was chosen to expose sound tooth structure apical to the defect so that a crown could be constructed. With an anterior open bite, the natural crown was left intact and it was possible to position the anchor bar in an incisal and lingual position, maintaining esthetics for the patient during both the extrusive and stabilization phases of treatment.  相似文献

Abstract – Treatment of crown fractures often requires a multidisciplinary approach. In the anterior teeth, reestablishment of proper esthetics and function is quite important for the patient. However, crown‐root fractures with fracture line below the gingival attachment or alveolar bone crest presents restorative difficulties. This case report presents a cervical tooth fracture that had been treated with minimal invasive approach with different disciplines. The tooth had endodontic treatment and a glass‐fiber post, and a composite core was accomplished. Then, the tooth was extruded to the desired level with orthodontic forced eruption before definitive restoration.  相似文献

Extensive restorative dentistry or so-called mouth rehabilitation is merely a correlation of required and indicated dental restorations and treatment for a particular patient, the procedures of which are governed and controlled by that patient's limitations. No set rule can be recommended which will apply to the treatment of all patients. The correct and proper method of approach depends upon the foresight, the ability, and practical experience of the practitioner and his technician. An 11-year-old boy was diagnosed with a horizontal root fracture in the maxillary right lateral incisor and a crown fracture in the maxillary right central incisor. This patient was treated endodontically and conservatively. At a follow-up examination 36 months after treatment, the teeth were functionally and esthetically normal. Radiographically, no pathosis was evident in periapical or fracture areas.  相似文献
