目的 探讨细胞外基质蛋白诱导因子(CD147)和细胞角蛋白19(CK19)在肝细胞癌(HCC)中的表达及临床意义。方法 采用组织芯片技术和免疫组织化学法检测CD147和CK19在272例HCC组织和81例癌旁组织中的表达情况。结果 CD147在HCC中的阳性表达率为73.53%(200/272),在癌旁组织中的阳性表达率为13.58%(11/81),差异有统计学意义(P<0.05)。CK19在HCC组织中的阳性表达率为14.34%(39/272),CK19在癌旁组织中无表达。在HCC中,CD147的表达与组织学分级、临床分期、术后无瘤生存时间、肿瘤直径、脉管或门静脉癌栓相关,与性别、年龄、肝硬化、AFP水平、HBV感染、淋巴结转移、病灶数目、肝被膜浸润及卫星灶无关(P>0.05);CK19在HCC中的表达与术后无瘤生存时间、组织学分级、肿瘤直径、肝硬化、卫星灶、淋巴结转移及临床分期有关,与性别、年龄、病灶数目、肝被膜浸润、AFP水平、HBV感染及脉管或门静脉癌栓无关(P>0.05)。在HCC中,CD147 阳性表达者与阴性表达者的中位复发时间分别为13个月和48个月(P<0.05),中位生存时间分别为24个月和60个月(P<0.05);CK19阳性表达者与阴性表达者的中位复发时间分别为7个月和31个月(P<0.05),中位生存时间分别为13个月和42个月(P<0.05)。 CD147的表达和CK19的表达无明显相关性(r=0.061,P=0.317)。结论 HCC中CD147和CK19的表达与预后密切相关,且两者均可作为HCC预后不良的判断指标。  相似文献

应用免疫酶标染色法检测了59例急性髓系白血病(AML)患者的白血病细胞免疫表型,结果表明CD2、CD5、CD7、CD10、CD19、CD22淋系抗原的表达率分别为16.9%(10/59)、119%(7/59)、16.9%(10/59)、15.3%(9/59)、102%(6/59)和6.8%(4/59)。进一步分析结果表明,在M3病例细胞中,CD2、CD10和CD19抗原表达阳性率明显高于M5组,而CD7抗原表达阳性率则明显低于M5组。结合临床,CD2、CD19阳性的AML病例对化疗治疗及应优于CD2、CD19阴性的AML病例;CD7阳性的AML病例的疗效与预后则比CD7阴性的AML病例差。提示部分AML病例的白血病细胞存在不同程度异常免疫表型的表达,且与疗效及预后有一定关系。  相似文献

观察肝动脉导管灌注CD3AK细胞联合TAE治疗晚期HCC的效果。方法:40例晚期HCC分为两组,实验组20例,采用肝动脉导管灌注CD3AK细胞联合TAE治疗;对照组20例,单纯TAE治疗。并检测两组AFP及T细胞亚群的变化。结果:实验组与对照组有效率分别为45%、30%(P<0.01),治疗后实验组CD4、CD4/CD8比值明显升高(P<0.01),而对照组则轻度下降。AFP两组均明显下降,以实验组为著(P<0.005)。结论:肝动脉导管灌注CD3AK细胞联合TAE治疗晚期HCC,是一种最有希望的方法。  相似文献

目的:检测非小细胞肺癌患者CK19mRNA和LUNXmRNA的表达情况,探讨其作为微转移检测分子标志物的可行性.方法:选择初治的非小细胞肺癌(NSCLC)患者62例,以肺部良性疾病10例作为对照组,采用实时荧光定量逆转录聚合酶链反应(FQ—RT—PCR)和普通RT—PCR技术,检测肺癌、良性病变肺组织和骨髓的CK19mRNA和LUNXmRNA的表达。结果:肺癌和良性病变肺组织RT—PCR检测结果显示CK19mRNA和LUNXmRNA表达阳性率为100%:62例NSCLC患者骨髓标本FQ—RT—PCR检测结果显示,NSCLC患者中骨髓CK19mRNA表达阳性率45.2%(28/62),明显高于肺良性病变组10%(1/10)(P=0.037),NSCLC组骨髓LUNXmRNA表达阳性率38.7%(24/62)明显高于肺良性病变组0%(0/10)(P=0.017);骨髓CK19mRNA表达阳性率随病理分期升高,接近统计学意义(P=0.06),而与病理类型、肿瘤分化程度无关;骨髓LUNXmRNA表达阳性率随分期升高且有统计学意义(P=0.02),而与病理类型、肿瘤分化程度无关:骨髓CK19mRNA和LUNXmRNA表达明显相关(P〈0.001)结论:CK19mRNA和LUNXmRNA可作为检测非小细胞肺癌患者微转移特异、敏感的分子标志物,可能有助于早期诊断肺癌转移,从而指导临床分期和治疗、  相似文献

目的:研究成人急性髓系白血病(acute myeloid leukemia,AML)中淋系抗原的表达及其与预后的关系。方法:对275例成人AML进行细胞形态学、流式细胞免疫表型分析和染色体细胞遗传学(MIC)分型实验研究并对其中的64例M2进行临床治疗结果观察研究。结果:①AML淋系抗原阳性率31.3%,其中t(8;21)者为48.3%,无t(8;21)者为26,5%(P〈0.001)。②CD19^+在t(8;21)中占38.3%,无t(8;21)中占1.9%(P〈0.001);而其他淋系抗原两组比较均为P〉0.05.③治疗结果(CR率):t(8;21)和无t(8;21)M2分别为66.7%和41.9%,CD19^+和CD19^+ M2分别为76.5%和46.8%,CD19^+/t(8;21)和CD1^+9/无t(8;21)分别为80.0%和41.4%,以上均为P〈0.05。而CD19^+/t(8;21)和CD19^+/无t(8;21)分别为55.6%和50.0%(P〉0.05)。结论:成人AML中淋系抗原表达与核型密切相关,t(8;21)AML高表达CD19。t(8;21)和CD19都是M2预后好的标志之一。  相似文献

[目的]探讨原发性肝细胞肝癌(HCC)中乙型肝炎病毒(HBV)的感染与肝癌生物学行为及其新生血管表达的关系。[方法]利用血清学指标检测56例HCC的HBV感染情况,并采用快速免疫组化MaxVisionTM法检测56例HCC组织中VEGF和CD105的表达并计数微血管密度(CD105-MVD)。[结果]HBV感染与肝癌的Edmondson分级、AFP值增高、门静脉癌栓形成、肝内转移相关,有统计学意义(P〈0.05)。38例HBV感染型HCC中VEGF的表达率为78.9%(30/38).CD105-MVD值为45.40±4.45,18例非HBV感染型HCC中VEGF的表达率为50.0%(9/18),CD105-MVD值为35.30±3.21,差异均有统计学意义(P〈0.05)。[结论]HBV感染可能通过上调VEGF的表达,促进了肿瘤血管的生成.从而促进HCC的生长、浸润和转移。  相似文献

应用免疫组化S-P法,研究47例原发性肺癌手术标本中nm23-H1,PCNA,CD15的表达及其与区域性淋巴结转移的关系。结果显示:在原发性肺癌中nm23-H1,PC-NA,CD15的阳性表达分别为51.06%,65.96%,51.06%。其中nm23-H1在鳞癌中阳性表达43.3%(13/30),腺癌64.71%(11/17),腺瘤表达较鳞癌高。在鳞癌中无肺门及纵隔淋巴结转移者阳性62.50%(10/16),有转移者阳性21.41%(3/14)(P<0.01),显示nm23-H1表达与鳞癌的肺门及纵隔淋巴结转移呈负相关。在PCNA表达中,有区域淋巴结转移的阳性表达为82.6%(19/23),无淋巴结转移为54.16(13/24)(P<0.01),低分化癌阳性表达88.8%(8/9),明显高于高分化癌43.75%(7/16),(P<0.01)。在CD15表达中,有淋巴结转移组中阳性表达为78.26%(18/23),无区域性淋巴结转移组中阳性表达25%(P<0.01)。而nm23-H1,PCNA,CD15表达之间无显著相关性。本研究提示nm23-H1,PCNA,CD15的表达对肺癌预后评估有重要意义。  相似文献

目的研究浸润性T淋巴细胞和Th细胞因子在不同类型乳腺癌中表达的意义。方法利用免疫组织化学链霉亲和素生物素法(labelled streptavidin-biotin method,LSAB)方法检测61例浸润性微乳头状癌(invasive micropapillary carcinoma,IMPC)、71例浸润性导管癌(invasive ductal carcinoma,IDC)和27例髓样癌(medullary carcinoma,MC)3种乳腺癌中浸润T淋巴细胞亚群分布以及Th细胞因子的表达。T淋巴细胞组间比较采用Kruskal-Wallis Test,组内比较采用Witcoxon Signed Ranks Test;组间Th细胞因子的比较采用x^2检验。结果CD3、CD8和CD4均表达于淋巴细胞胞膜。CD3^+T细胞在3组间差异无统计学意义(P=0.735)。CD8^+T和CD4^+T细胞在3组间差异均有统计学意义(P=0.000,P=0.012)。Th细胞因子主要表达于肿瘤细胞和淋巴细胞的胞质。IL-2表达在IMPC组最低(25/61,41.0%),其次是IDC组(33/71,46.5%),最高是MC组(20/27,74.1%),3组间差异有统计学意义(P=0.014);IFN-γ的表达类似于IL-2,IMPC组最低(39/61,63.9%),其次是IDC组(57/71,80.3%),最高是MC组(24/27,88.9%),3组间差异有统计学意义(P=0.019);IL-4的表达在IDC组最高(42/71,59.2%),其次是IMPC组(19/61,31.1%),最低是MC组(5/27,18.5%),3组间差异有统计学意义(P=0.000);IL-10的阳性率在IMPC组是87.8%(47/61),在IDC组是87.3%(62/71),在MC组是88.9%(24/27),3组间差异无统计学意义(P〉0.050)。结论浸润性T淋巴细胞和Th细胞因子在不同类型乳腺癌中表达不同。  相似文献

目的:探讨人血清磷脂酸肌醇蛋白聚糖-3(GlyPican-3)和甲胎蛋白(a—fetoprotein,AFP)联合检测对肝细胞癌(hepatocellularcarcinoma,HCC)诊断的临床价值。方法:收集2011—1l—02-2013—01—13遵义市第一人民医院174例和青岛市城阳区人民医院44例,门诊和肝胆外科人外周血血清218例,其中HCC54例,肝硬化36例,肝炎40例,肝良性肿瘤18例,其他恶性肿瘤40例,健康志愿者30名。双抗体夹心酶联免疫定量测定方法和电化学发光法检测血清中GlyPican-3和AFP的表达水平,结果以中位数(M)表示。制作受试者工作特征(receiveroperatingcharacteristic,ROC)曲线,以曲线下面积(areaundercurve,AUC)反映诊断的准确性。结果:HCC组血清GlyPican-3的表达水平为654.67ρg/mL,显著高于肝硬化组的32.83ρg/mL、肝炎组的33.04ρg/mL、肝良性肿瘤组的15.14ρg/mL、其他恶性肿瘤组的25.82ρg/mL和健康志愿者组的3.90ρg/mL,P值均〈0.001;GlyPican-3组AUC为0.883,95%CI:0.833~0.923;AFP组为0.802,95%CI:0.743~0.853;GlyPican-3和AFP联合检测为0.945,95%CI:0.905~O.971。GlyPican-3和AFP联合检测显著高于单个检测指标,Z值分别为5.944和3.409,P值分别为〈0.001和0.007。GlyPican-3取最佳临界值为60ρg/mL时,GlyPican-3诊断HCC的灵敏度和特异度分别为85.18%(46/54)和73.78%(121/164);GlyPican-3联合AFP检测诊断HCC的灵敏度可达96.29%(52/54)。血清GlyPican-3的表达水平随HCC的临床分期增加而升高,H=4.571,P=0.025。结论:血清GlyPican-3的表达水平对于HCC具有较高的诊断价值,血清GlyPican-3联合AFP检测可提高HCC的诊断率。血清GlyPican-3的表达水平与,临床分期有关。  相似文献

目的:探讨食管癌患者外周血和骨髓中CK18mRNA基因表达及临床意义。方法:采用巢式RT—PCR方法检测50例M0、10例M0的食管癌患者及10例食管良I生病变患者外周血及肋骨骨髓中细胞角蛋白CK18mRNA基因的表达。结果:50例M0食管癌患者外周血、骨髓中CK18mRNA阳性表达率分别为38.0%(19/50)和42.0%(21/50);10例M,的食管癌患者外周血、骨髓中CK18mRNA阳性表达率分别为90.0%(9/10)、100%(10/10);食管良性病变患者的外周血和骨髓中K18mRNA阳性表达率均为0%(0/10),CK18mRNA阳性表达在三组间差异有显著性(P〈0.01)。CK18mRNA在Ⅳ期食管癌患者外周血和骨髓中阳性表达率与III期以前患者相比,有显著性差异(P〈0.05)。CK18mRNA在外周血和骨髓中表达无差异(P〉0.05),但呈正相关性(r=0.704,P〈0.01)。结论:应用RT—PCR法检测食管癌患者外周血或骨髓中CK18mRNA微转移有助于食管癌的分期和预后的判断。  相似文献

Objective

The aim of this study was to investigate the expressions of CD147 and CK19 in hepatocellular carcinoma (HCC) and their clinical significance.

Methods

The expressions of CD147 and CK19 were determined by tissue microarray and immunohistochemistry (IHC) in 272 cases of HCC and 81 cases of adjacent tumorous tissue.

Results

The positive expression of CD147 in HCC and adjacent tumorous tissue was 73.53% (200/272) and 13.58% (11/81) with significant difference (P < 0.05). The positive expression of CK19 in HCC and adjacent tumorous tissue was 14.34% (39/272) and 0 (0/81) with significant difference (P < 0.05). The positive expression of CD147 were closely correlated to the histological grade, clinical stage, tumor-free survival, diameter of tumor and embolus of cancer in aqueduct or portal vein; but not to the patients’ sex, age, liver cirrhosis, AFP level, infection of HBV, lymph node metastasis, number of tumor, invasion liver involucrum and the micro-satellites (P > 0.05). The expression of CK19 in HCC were closely correlate to the tumor-free survival, histological grade, diameter of tumor, liver cirrhosis, micro-satellites, lymph node metastasis and clinical stage; but not to patients’ sex, age, number of tumor, invasion liver involucrum, AFP level, infection of HBV and embolus of cancer in aqueduct or portal vein (P > 0.05). Among the patients of positive expression of CD147, the median replacing time and overall survival were 13 and 24 months, lower than 48 and 60 months in the patients of negative expression (P < 0.05). Among the patients of positive expression of CK19, the median replacing time and overall survival were 7 and 13 months, lower than 31 and 42 months in the patients of negative expression (P < 0.05). The expression of CD147 had no correlation with the expression of CK19 (r = 0.061, P = 0.317).

Conclusion

The positive of CD147 and CK19 closely correlate with the clinical prognosis of HCC, it may indicate poor prognosis of HCC.  相似文献

目的:探讨鉴别肝细胞癌(HCC)、肝内胆管癌(ICC)和肝转移性腺癌(MAC)的免疫组化诊断谱特点。方法:对手术切除的300例HCC、35例ICC和30例MAC分别进行AFP、Hep Pau 1、CK18、CK19、CA19-9、CD34和pCEA等7种免疫组化染色,将特异性和敏感性的综合性能计分(CCS)≥8分的抗体定为具有高度诊断价值。结果:CCS≥8分的抗体在HCC中为Hep Par 1和CD34,在ICC中为CK19,在MAC中无。Hep Par 1的CCS(9分)明显高于AFP(7分),其对HCC的敏感性达到83.7%,特异性达到96.7%。结论:HCC的一线抗体由Hep Par 1和CD34组成,二线抗体由pCEA和AFP组成:ICC的一线抗体为CK19,二线抗体为CA19-9。由3种一线抗体组合成肝癌的核心免疫组化谱,酌情使用二线抗体,可以较好地解决对HCC、ICC和MAC之间的免疫病理诊断和鉴别诊断。  相似文献

目的:观察 CK7、CK15、CK20、CD10、Bcl -2、Nestin 在基底细胞癌(basal cell carcinoma,BCC)和毛母细胞瘤(trichoblastoma,TB)中的表达,探讨其在两种肿瘤诊断和鉴别诊断中的作用。方法:对确诊的35例BCC 和7例 TB 石蜡包埋标本行 CK7、CK15、CK20、CD10、Bcl -2、Nestin 免疫组化染色,观察它们在肿瘤细胞及其周围间质中的表达。结果:对各种抗体在基底细胞癌与毛母细胞瘤表达分布比较,CK7、CK15、CK20、Nestin 表达分布无明显差异。Bcl -2在基底细胞癌中仅癌巢周边细胞阳性,间质部分细胞灶状阳性,而毛母细胞瘤中上皮细胞均弥漫弱阳性。CD10在两种肿瘤中周边细胞均阳性表达,但基底细胞癌中间质阴性,而毛母细胞瘤中间质阳性表达。结论:分析 CD10和 Bcl -2在肿瘤中表达分布的不同,有助于这两种肿瘤的鉴别诊断。  相似文献

BACKGROUND:

CK19 and CD10 are useful markers in the differential diagnosis of pancreatic tumors. The authors evaluated CK19 and CD10 expression in pancreatic neuroendocrine tumors (NETs) obtained by endoscopic ultrasound (EUS)‐guided fine‐needle aspiration (FNA).

METHODS:

Twenty‐eight patients diagnosed with pancreatic NETs based on EUS‐FNA cytology were studied retrospectively (2004‐2007) for immunohistochemical expression of CK19 and CD10. Immunohistochemistry was performed on cell blocks for each case. The pattern of expression for CD10 (cytoplasmic or membranous) and its intensity (0‐2) were noted. The staining of the stromal elements for CD10 was recorded as negative. Cytoplasmic staining in tumor cells and percentage distribution (1+ to 4+) for CK19 were regarded as positive.

RESULTS:

Twenty‐three of 28 (82.14%) NETs showed positive cytoplasmic and/or membranous staining for CD10, and 25 of 28 (89.29%) cases were positive for CK19.

CONCLUSIONS:

The findings demonstrate the high expression of CD10 and CK19 in pancreatic NETs. This indicates that CD10 and CK19 cannot reliably differentiate NETs from other tumors with similar cytomorphologic features (solid pseudopapillary tumors, which frequently stain with CD10, and pancreatic adenocarcinoma, which stains with CK19). AIM: Serum alpha-fetoprotein (AFP) is the most important tumor marker for hepatocellular carcinoma (HCC). Previous reports indicated that HCC was also associated with increased levels of interleukin (IL)-6, IL-10 and hepatocyte growth factor (HGF). This study investigated the role of these cytokines as tumor markers for HCC. METHOD: A total of 128 adults were prospectively enrolled and categorized into four groups: normal subjects (n=29), chronic hepatitis B or C (n=50), non-HCC tumors (n=23) and HCC (n=26). Serum AFP, IL-6, IL-10 and HGF levels were determined in all subjects. RESULTS: The expression of IL-6 or IL-10 (> or =3 pg/ml), or high level of HGF (>1000 pg/ml) or AFP (>20 ng/ml) was observed in only 0-3% of normal subjects. Patients with HCC more frequently had higher IL-6 and IL-10 levels (p<0.05), whereas HGF levels in HCC patients were not significantly elevated compared to patients with chronic hepatitis or non-HCC tumors. Among patients with low (<20 ng/ml) AFP level, IL-6 or IL-10 expression was significantly associated with the existence of HCC (p<0.05). Patients with large (>5 cm) HCC more often had increased IL-6, IL-10 or AFP levels (p values all <0.05). CONCLUSIONS: Serum levels of IL-6 and IL-10 are frequently elevated in patients with HCC but not in benign liver disease or non-HCC tumors. IL-6 and IL-10 may help identify a subset of HCC patients with low AFP level, and may serve as complementary tumor markers in these patients.  相似文献

 目的 研究流式细胞术(FCM)检测在急性髓系白血病(AML)及混合型白血病(MAL)诊断中的意义。方法 采用四色FCM对52例AML和7例MAL患者进行免疫表型检测。结果 52 例AML患者以系列专一表达为主,主要表达 CD13(94.2 %),CD117(90.4 %),cMPO(90.4 %)CD33(86.5 %),CD34(57.7 %),HLA-DR(53.8 %)。7例MAL中髓系/B系(M/B)混合3例,均表达CD13,CD34,CD117,CD10,CD19,cMPO,cCD79a;髓系/T系(M/T)混合2例,均表达CD13,CD117,CD33,CD34,CD5,CD7,cMPO;B系/T系(B/T)混合2例,均表达CD5,CD7,CD10,CD19,CD20,CD34。且与形态学和组织化学诊断具有高符合率。结论 FCM能够提高AML和MAL的确诊率,对该类白血病的明确诊断及分型具有重要的临床指导意义。  相似文献

 目的 研究双克隆型多发性骨髓瘤(MM)患者的临床特点及治疗。方法 分析该科自1996年1月至2010年6月收治的5例双克隆型MM患者,男性2例,女性3例,年龄59~76岁,中位年龄72岁。其中IgG+IgA型3例,IgM+IgG/A型2例。结果 本组患者中诊断时均为DS分期Ⅲ期,治疗效果为2/4达到部分缓解(PR),1/4为稳定(SD),1/4为进展(PD),无达到完全缓解(CR)的病例。结论 双克隆型MM较为罕见,目前对此型MM的诊断和治疗尚缺乏更深刻的认识,需要更多病例的分析。  相似文献

The distribution of alpha-tetoprotein (AFP) and immunoreactivecarcinoembryonic antigen (CEA) in 62 hepatocellular carcinomas(HCC) and five hepatoblastomas (HBL) all surgically removedwas studied by an immuno-histochemical method, and the resultswere compared with the levels of the antigens in the patients'serum. AFP was present as coarse granules in the cytoplasm of immaturetumor cells. AFP-positive tumor cells were detected in 34/43cases (79.1%) with serum AFP levels higher than 400 ng/ml andin 1/24 (4.2%) of the remaining cases. They were present in8/32 (25%) cases of relatively well-differentiated HCC (Edmondson'sgrade I or II), 22/32 (68.8%) of relatively poorly differentiatedHCC (Edmondson's grade III or IV), and 5/5 of HBL. An antigen immunoreactive with a conventional rabbit anti-CEAim-munoglobulin (DAKO) but not with a murine monoclonal anti-CEAantibody (Hybritech) was present on the surface of bile canaliculiof both non-neo-plastic and neoplastic hepatocytes. This CEAcross-reactive differentiation antigen was more often foundin relatively well-differentiated tumors, and the presence ofthis antigen in tumors did not correlate with patients' serumCEA levels. In conclusion, CEA of an oncofetal nature was notproduced by either HCC or HBL.  相似文献
