In 1986 the Professional Fire Fighters of Florida (PFFF), a labor organization of 11,000 members, passed an unprecedented resolution to create a "smoke-free fire service." The PFFF's commitment arose from its (i) concern for the health of fire fighters, (ii) need to address the issue of smoking to protect the fire fighters' "Heart and Lung Law," and (iii) attempt to avert criticism of its proposed presumptive "Cancer Bill." In 1987 the PFFF gained support for its resolution from a council representing chiefs, fire instructors and inspectors, and volunteer fire fighters. Prior to the "smoke-free" resolution, one fire department in Florida required that new fire fighters be non-smokers. Since the resolution passed, 14 departments added this requirement. In 1989 the PFFF succeeded in getting a landmark bill passed requiring that new fire fighters be non-users of tobacco for at least one year prior to application. Important implications of the PFFF's efforts to create a "smoke-free fire service" are also discussed.  相似文献

Cohort mortality study of Seattle fire fighters: 1945-1983   总被引:2,自引:0,他引:2  
Fire fighters are known to be occupationally exposed to many toxic substances. However, the limited number of previous studies has not demonstrated any consistent excess mortality from diseases of a priori concern, such as lung cancer, non-malignant respiratory disease, and cardiovascular disease. We studied 2,289 Seattle fire fighters from 1945 through 1983, and observed 383 deaths. Excess mortality from leukemia (SMR = 503, n = 3) and multiple myeloma (SMR = 989, n = 2) was observed among fire fighters with 30 years or more fire combat duty. Lung cancer mortality was elevated (SMR = 177, n = 18) among fire fighters 65 years old or older. We also analyzed the data by considering fire fighters at risk only after 30 years from first exposure. In this analysis, a trend of increasing risk with increasing exposure was observed for diseases of the circulatory system. For this cause of death, fire fighters with 30 years or more fire combat duty had a relative risk of 1.84 compared to those with less than 15 years of fire combat duty.  相似文献

目的对1994~2005年西藏自治区火灾发生的趋势及流行病学特征进行分析,探讨其危险因素和防范措施,为预防和控制火灾发生提供科学依据。方法采用回顾性流行病学研究方法对西藏自治区1994~2005年的火灾资料进行描述性研究和统计分析。结果a994~2005年西藏自治区共发生火灾1662起,平均每年138.5起,死亡8.08人,受伤22.25人,直接经济损失830.5595万元,人均火灾发生数呈逐年上升趋势;春、冬季为火灾的高发季节,且呈逐年上升趋势;用火不慎和电气为主要火灾发生原因,分别占发生总数的41.4%和17.2%,且呈逐年上升的趋势;火灾发生总数及电气、用火不慎、玩火等引起的火灾数均随国内生产总值和居民年均生活消费支出的增加而增多,呈正相关关系;西藏火灾发生数也与私营企业数呈正相关关系;西藏火灾发生数和直接经济损失虽低于火灾高发城市,但伤亡率却相对较高,伤亡率与卫生资源规模有关。结论西藏自治区的火灾形势严峻,伤亡率严重,针对火灾的发生原因及特点,积极预防火灾的发生十分重要。  相似文献

This case-control study within a metropolitan fire department evaluated the effect of self-contained breathing apparatus (SCBA) and other risk factors on three types of injury at the scene of a fire (smoke inhalation, burns, and falls). Data on 75 injured fire fighters and 144 controls came from telephone interviews and department records. The two sets of uninjured fire-fighter controls were matched to cases on incident (n = 72) or on job position and fire type and size (n = 72). Smoke inhalation cases were not significantly different from controls in SCBA use, cigarette smoking, previous fires in the shift, or injury history. Jobs with high risk of burns included nozzle operator, engine officer, and forcible-entry person in first-due companies (OR = 20.1). Other risk factors for burns were: basement origin of fire (OR = 10.2); prior fire-fighting training outside the present department (same fire: OR = 11.2; similar fire: OR = 3.9); and on-duty injury in the prior 12 months (same fire: OR = 4.3; similar fire: OR = 3.5). When other risk factors were considered, consistent SCBA use was associated with falls (same fire: OR = 11.8; similar fire: OR = 4.3) but not with burns. Risk of falls also was elevated among members of truck companies (OR = 17.7) and fire fighters without children (same fire: OR = 8.4; similar fire: OR = 7.4). On-duty injury in the past 12 months was associated with falls when one compared cases with similar-fire controls (OR = 5.5), but not with controls attending the same fire. The National Institute for Occupational Safety and Health (NIOSH) was requested to conduct a health hazard evaluation (HHE) at a large metropolitan fire department. The request concerned the hearing levels and noise exposures of fire fighters who were assigned to two fire stations serving the international airport. There was concern that these fire fighters were at a greater risk of accruing hearing loss than fire fighters located at other fire stations because of the addition of aircraft noise to their occupational noise exposures. The city also requested that NIOSH investigate other fire stations, not influenced by the airport, for noise exposures and hearing ability among a larger population of the fire fighters. NIOSH investigators conducted noise surveys at five fire stations and examined the hearing ability of 197 fire fighters. The noise surveys consisted of personal noise dosimetry on fire fighters assigned to the fire station for the entire 24-hr tour of duty over 2 consecutive days at each of the five stations. A NIOSH investigator accompanied the fire fighters on their vehicle to log response times and activities. The audiometric examinations were pure-tone, air conduction tests administered according to the Occupational Safety and Health Administration's (OSHA's) hearing conservation amendment. The noise dosimetry results revealed time-weighted averages (TWAs) that ranged from 60 to 82 dBA. However, the levels encountered during Code 3 responses (warning lights, sirens, and air horns) reached 109 dBA for a 1-min time period. BACKGROUND: Homes in rural areas have a higher fire death rate. Although successful exit from a home fire could greatly reduce fire-related deaths and injuries, little is known about factors associated with behaviors of developing and practicing an escape plan. METHODS: Between July 2003 and June 2004, a baseline survey was administered, in person, to 691 rural households. Information collected included a history of previous home fire, perceived risk of home fire, existing smoke alarms and their working status, and home fire safety practices, as well as home and occupant characteristics. The association of residents' perceived risk of home fire and fire escape plans was assessed. RESULTS: Forty-two percent of rural households reported having a fire escape plan. Of the households with a plan, less than two thirds (56.9%) discussed or practiced the plan. Households with children were more likely to develop and practice a fire escape plan. Households with an elderly or disabled person were less likely to develop or practice the plan. Compared to respondents who perceived low or very low risk of home fire, those who perceived a high or very high risk had 3.5 times greater odds of having a fire escape plan and 5.5 times greater odds of discussion or practicing their plan. CONCLUSIONS: Increasing awareness of the potential risk of home fires may help occupants develop and practice home fire escape plans. In order to reduce fire deaths and injuries, different strategies need to be developed for those households in which the occupants lack the ability to escape.  相似文献

Impact of forest fires on PAH level and distribution in soils   总被引:3,自引:0,他引:3  
Surface (0–5 cm) and subsurface (−5 to 15 cm) soils from burned forest areas in South of France were analyzed to determine contents of 14 priority polycyclic aromatic hydrocarbons (PAHs) and their distribution profile. The sampling procedure allowed us to study the effect of the frequency of fire as well as the influence of the time elapsed since the last fire. The contribution of forest fires to the content of PAHs in soils was demonstrated, as well as the decrease of their total level with time. The hypothesis is that a natural remediation takes place a few years after the last fire event. The lowest molecular weight studied PAHs (naphthalene, acenaphtene, fluorene, phenanthrene, anthracene, fluoranthene and pyrene) appear to be the major ones produced by forest fire. Naphtalene levels are remarkably high in burned soils (more than 70 μg kg−1, i.e. more than 20 times higher than in the control soils) and still remain important years after the last fire event. The time elapsed since the last fire appears to be a more influencing factor than the fire frequency. The index defined from the PAH levels shows values reflecting the time elapsed since the last fire.  相似文献

Intense exertion is an occupational hazard inherent to fire fighting. This study was designed to look at the exertion levels that fire fighters attain during a fire fighting exercise when using (1) no self-contained breathing apparatus (SCBA), (2) light SCBA, and (3) heavy SCBA. Exertion levels were measured as a function of the heart rate increase relative to the maximum predicted heart rate determined by a standard treadmill exercise test. Five fire fighters wore electrocardiographic monitors during a routine fire fighting exercise. Heart rates increased rapidly to 70% to 80% of maximum within the first minute and then plateaued at 90% to 100% until the attack on the fire was completed. There was no significant difference between exertion levels when using no SCBA, light SCBA, and heavy SCBA (split-plot analysis of variance, p greater than .25). These results suggest that fire fighters attain an intense level of physical activity quickly and maintain that level as long as they are actively engaged in fighting fire. These results also suggest that regardless of the weight of the SCBA, if employed, fire fighters exert themselves from 85% to 100% of their maximum and adjust their work output to maintain that near-maximal level.  相似文献

The Chiado’s fire that affected the city centre of Lisbon (Portugal) occurred on 25th August 1988 and had a significant human and environmental impact. This fire was considered the most significant hazard to have occurred in Lisbon city centre after the major earthquake of 1755. A clear signature of this fire is found in the atmospheric electric field data recorded at Portela meteorological station about 8 km NE from the site where the fire started at Chiado. Measurements were made using a Benndorf electrograph with a probe at 1-m height. The atmospheric electric field reached 510 V/m when the wind direction was coming from SW to NE, favourable to the transport of the smoke plume from Chiado to Portela. Such observations agree with predictions using Hysplit air mass trajectory modelling and have been used to estimate the smoke concentration to be ~0.4 mg/m3. It is demonstrated that atmospheric electric field measurements were therefore extremely sensitive to Chiado’s fire. This result is of particular current interest in using networks of atmospheric electric field sensors to complement existing optical and meteorological observations for fire monitoring.  相似文献

From a cohort of 1,768 Boston fire fighters studied initially in 1970, pulmonary function measurements were repeated on 1, 146 active subjects in 1974. The mean time between examinations was 3.4 years. The data on 1,430 subjects after one year of follow-up had indicated excessive decrements in levels of forced vital capacity and one-second forced expiratory bolume which were related to the frequency of fire exposure. The annual decline over three years was less than that observed over one year and could not be related to the number of fires fought or to other indices of acute fire exposure. Selection factors within the fire department appear to be important in protecting fire fighter from continued loss ventilatory capacity.  相似文献

Fires and burns are the fifth most common cause of unintentional injury deaths in the United States. To address fires and fire deaths, the National Fire Data Center (NFDC) established the National Fire Incident Reporting System (NFIRS) as a surveillance system for fires. Delaware implemented NFIRS as the Delaware Fire Incident Reporting System (DFIRS), and is currently capturing all fires reported in the system. The objectives of this study are to: 1) understand the implementation of DFIRS; 2) analyze data from DFIRS to describe fire incidents; and 3) inform other states’ fire surveillance efforts. We interviewed Delaware State Fire Marshal’s Office personnel to understand the implementation of DFIRS and analyzed DFIRS data from May 2003 to December 2004 to examine data completeness, and characteristics of fires, smoke alarms, and fire injuries and deaths. DFIRS captures 100% of Delaware fires reported to fire departments. Data completeness for the fields examined ranged from 33% to 100%. Fires in which smoke alarms alerted occupants were significantly less likely to result in injury or death than fires in which smoke alarms did not. DFIRS has the potential to serve as a valuable fire prevention and fire analysis tool. For DFIRS to reach its full potential as a surveillance system, increased attention to data completeness is necessary. The authors Gwendolyn Bergen and Van M. Wildland fire smoke is inevitable. Size and intensity of wildland fires are increasing in the western USA. Smoke-free skies and public exposure to wildland fire smoke have effectively been postponed through suppression. The historic policy of suppression has systematically both instilled a public expectation of a smoke-free environment and deferred emissions through increased forest fuel loads that will lead to an eventual large spontaneous release. High intensity fire smoke is impacting a larger area including high density urban areas. Policy change has largely attempted to provide the avenue for increased use of ecologically beneficial fire but allows for continued reliance on suppression as a primary tool for a smoke averse population. While understanding the essential role of suppression in protection of life and property, we dispute the efficacy of attempting to eliminate smoke exposure through suppression in a fire prone area to protect human health at the population level. Sufficient consideration to future negative health outcomes needs to be considered in fire management decisions. It is likely that long term air quality is inextricably linked to ecosystem health in the Sierra Nevada. We contend that landscape use of ecological fire is essential to forest and human health. Radical change is needed where beneficial wildland fire smoke is treated as natural background and exempted from much of the regulation applied to anthropogenic sources. Tolerance of the measured release of routine smoke emissions from beneficial fire is needed. Using present air quality standards in the more remote areas will provide an opportunity to increase burning in many forests while protecting public health.  相似文献

The purpose of the study was to assess the state of fire prevention research, provide an updated synthesis of evaluated fire prevention programs, and discuss the role of fire fighters and data systems in prevention efforts. The review included all evaluations of U.S. based fire prevention interventions published between January 1998 and September 2004 and any earlier articles about U.S. fire prevention interventions not included in two prior review articles. We retrieved information from each identified study including evaluation findings, involvement of fire service personnel and use of existing data systems. We identified twelve articles: seven reported on smoke alarm interventions, three on multi-faceted programs, and two other programs. Five programs involved fire service personnel in the design, implementation, and/or evaluation, and three used existing data systems. Studies reviewed suggest that canvassing and smoke alarm installations are the most effective means of distributing alarms and increasing the functional status of distributed alarms. The functionality of smoke alarms, an issue noted in earlier reviews, remains a problem. Programs involving partnerships with fire departments have indicated success in preventing fires and deaths, improving smoke alarm ownership and functional status, and improving children's fire safety knowledge. Using existing data systems to target and to evaluate interventions was effective. In the years since prior reviews, some improvements in the rigor of evaluation designs have been made, but there is still a need for high quality evaluations that will inform fire injury prevention efforts.  相似文献

This article presents a qualitative evaluation of six fire safety education programs for older adults delivered by public fire educators. Our main aims were to explore how these programs are implemented and to determine important factors that may lead to program success, from the perspectives of the public fire educators and the older adults. For each program, we interviewed the public fire educator(s), observed the program in action, and conducted focus groups with older adults attending the program. Analysis revealed three factors that were believed to facilitate program success (established relationships with the older adult community, rapport with older adult audiences, and presentation relevance) as well as three challenges (lack of a standardized curriculum and program implementation strategies, attendance difficulties, and physical limitations due to age). More fire safety education should be developed for older adult populations. For successful programs, public fire educators should address the specific needs of their local older adult community.  相似文献

OBJECTIVES: This study was conducted to estimate (1) the proportion of U.S. homes with installed smoke alarms and fire escape plans, and (2) the frequency of testing home smoke alarms and of practicing the fire escape plans. METHODS: The authors analyzed data on smoke alarms and fire escape plans from a national cross-sectional random-digit dialed telephone survey of 9,684 households. RESULTS: Ninety-five percent of surveyed households reported at least one installed smoke alarm and 52% had a fire escape plan. The prevalence of alarms varied by educational level, income, and the presence of a child in the home. Only 15% tested their alarms once a month and only 16% of homes with an escape plan reported practicing it every six months. CONCLUSION: While smoke alarm prevalence in U.S. homes is high, only half of homes have a fire escape plan. Additional emphasis is needed on testing of installed smoke alarms and on preparedness for fire escape plans.  相似文献

Is testicular cancer an occupational disease of fire fighters?*   总被引:2,自引:0,他引:2  
BACKGROUND: A previous investigation showed an increased risk of testicular cancer among fire fighters in Wellington City, New Zealand, during the 1980s. Other studies of fire fighters had not identified testicular cancer as an occupational disease. METHODS: This was an historical cohort study of mortality and cancer incidence in all paid New Zealand fire fighters, from 1977 to 1995. RESULTS: The only cancer for which this study provided evidence of an increased risk was testicular cancer, even after excluding cases from the previous investigation. The standardized incidence ratio for 1990-96 was 3.0 (95% confidence interval: 1.3-5.90). There was no evidence that fire fighters were at increased risk from any particular cause of death. CONCLUSIONS: This study confirmed that New Zealand fire fighters are at increased risk of testicular cancer, although the reason is unknown. Other incidence studies of cancer in fire fighters are needed to confirm this finding.  相似文献
