目的比较应用功能性印模与传统印模制作全口义齿的临床效果。方法选择2006年8月至2007年4月于中国医科大学附属第四医院口腔修复门诊就诊的全口牙列缺失患者30例,分别应用功能性印模(15例)和传统印模(15例)制作全口义齿,于患者试戴义齿3个月后测试其压痛点、咀嚼效率以及固位力和力并进行比较。结果功能性印模制取技术较传统印模法制作的全口义齿压痛点数量少,咀嚼效率高,固位力和力明显提高(P<0.05)。结论从咀嚼功能和戴牙舒适度上讲,采用功能性印模制作的全口义齿明显优于传统印模义齿。  相似文献

印模质量对全口义齿修复效果至关重要,除了材料本身的特性,临床实际操作情况(如牙弓外形,牙体解剖以及印模制取等)也对其精度产生一定影响。特别是无牙颌条件较差时,义齿边缘伸展、系带缓冲义齿组织面与组织的密合程度等都会直接影响义齿的最终修复效果。笔者对21例上下颌牙列缺失且有旧义齿的患者,利用旧义齿做个别托盘,采用闭口式印模方法制取印模,制做全口义齿。观察复诊次数、舒适度和固位力,对闭口式和开口式印模方法进行了比较。  相似文献

目的对全口义齿修复患者复诊原因进行分析。方法无旧义齿可利用患者应用传统方法制作全口义齿。原有旧义齿可以利用的患者者,将旧义齿作为个别托盘取闭口式印模,按旧义齿咬关系上架,去除旧义齿,常规完成全口义齿制作。结果首次全口义齿修复患者复诊率为75%。有全口义齿修复患者复诊率为17%。结论全口义齿修复应注意戴用后的组织疼痛以及患者的心理因素有关,固位稳定是全口义齿修复的基本条件。  相似文献

目的 评价末端游离缺失患者使用不同印模材制取功能性印模过程中以及义齿修复后的临床疗效.方法 选择2011年2-9月在山西医科大学口腔医院修复科就诊的末端游离缺失患者40例,随机分为A组和B组(各20例),分别使用印模膏加藻酸盐印模材和硅橡胶(重体加轻体)制取印模,常规方法制作义齿,比较制取印模过程中以及义齿戴用3个月后患者的满意度.结果 制取印模过程中,患者对硅橡胶印模材的满意度高于印模膏加藻酸盐印模材(P<0.05);义齿戴用3个月后,患者对印模膏加藻酸盐印模材组的满意度较硅橡胶印模材组高,差异均有统计学意义(P<0.05).结论 可摘局部义齿修复末端游离缺失时,使用印模膏加藻酸盐印模材制取功能性印模其义齿修复效果好,但制取印模过程中患者感觉用硅橡胶印模材较舒适.由此可见,不同的印模材具有各自的优越性,要根据临床需要进行选择.  相似文献

目的:观察改良型托盘与传统托盘对下颌全口义齿印模的影响,比较义齿修复后的临床效果。方法:对60例无牙颌牙槽嵴条件较差病例进行两种印模方法制取下颌全口义齿印模,义齿戴入后对义齿的固位功能、黏膜情况,舒适性、咀嚼功能、食物嵌塞5项内容进行评价。结果:两组病例在舒适程度、黏膜情况、食物嵌塞、咀嚼功能等方面无显著性差异,P>0.05;但在固位功能方面改良型托盘组明显优于传统托盘组,P<0.05。结论:改良型托盘制取的印模显著提高了下颌全口义齿的吸附力,可以普遍应用于牙槽嵴或黏膜条件差的无牙颌修复患者。  相似文献

目的:探讨模型置换印模技术对双侧下颌游离端缺牙患者的修复效果。方法:在解剖式印模上常规制作铸造支架,在铸造支架上制作自凝暂时基板,经口内肌功能修整并制取咬合记录后于口内取功能性印模。经模型置换后常规完成义齿。结果:90.2%的患者用模型置换印模技术制作的义齿疗效良好。结论:模型置换印模作为游离端可摘部分义齿的重要功能印模方法,可以有效改善义齿修复质量。  相似文献

目的:探讨下颌全口义齿改良式印模法的临床应用。方法:5例下颌牙槽嵴低平的无牙颌患者,口角炎反复发作,其旧义齿牙列磨损严重,戴用时稳定性差。本文根据日本学者阿部二郎提出的下颌吸附性义齿理论与临床操作新要求,来介绍改良印模法制作下颌全口义齿的过程。结果:下颌全口义齿可通过改良式印模法实现义齿边缘的完整封闭,增加义齿固位,提高患者咀嚼效率与生活质量。患者评价较满意。结论:改良印模法应用于下颌牙槽嵴低平无牙颌患者,可明显增加义齿固位力。  相似文献

目的 :评价对无牙颌患者运用二次功能印模法进行全口义齿修复的临床效果。方法 :对433例无牙颌患者运用二次功能印模法进行全口义齿修复,修复前后使用老年口腔健康评价指数(Geriatric Oral Health Assessment Index,GOHAI)评估患者口腔健康相关生活质量。根据患者是否有义齿修复史,分为无义齿修复史、局部可摘义齿修复史和全口义齿修复史3组。在全口义齿修复史组中,选取30例患者,使用修复前旧义齿及新义齿咀嚼双色口香糖,采用ViewGum分析计算口香糖的混合程度数据(SDHue值)。采用SPSS 26.0软件包对数据进行统计学分析。结果:3组患者修复后口腔健康相关生活质量均有提升,修复后1个月GOHAI指数均显著高于修复前(P<0.05)。在全口义齿修复史组中,30例患者修复后SDHue值显著低于修复前(P<0.05)。结论:二次功能性印模法操作简便快捷,能够提升患者对全口义齿修复后的满意度。  相似文献

This study examines the effects of denture adhesive on the retention and stability of complete dentures and the masticatory function. The authors estimated the stability of complete dentures from 3-dimensional (3-D) denture movement and rotational denture movement and additionally the masticatory function from cycle time and chewing time. Six edentulous subjects who participated in this study had old and newly fabricated complete dentures. Upper denture movement was recorded using a 3-D motion capture system while chewing 3 kinds of food (peanuts, fish paste, raisins). Both the new and old dentures showed that using a denture adhesive contributes to reducing 3-D denture movement, rotational denture movement and chewing time during chewing the various foods. The cycle time in new and old dentures was statistically unaffected by applying denture adhesive. There was no statistically significant difference in improvement in 3-D denture movement or rotational denture movement between the new and old dentures, for any of the foods. This study observes the overall effect of denture adhesive during use for both dentures. The results of this study suggest that denture adhesive contributes to reducing denture movement and so improves chewing function.  相似文献

目的:观察充分利用旧义齿信息在老人全口义齿再修复疑难病例中的应用效果。方法:选择132例戴用旧义齿10年以上的老年无牙颌患者,按照旧义齿使用年限分两组,实验组:旧义齿使用15年以上66例,利用超硬石膏翻制重衬后的旧义齿组织面模型,利用聚醚橡胶翻制修整后的旧义齿蜡型,将蜡型放在石膏模型上按照咬合关系上牙合架,再制作新义齿;对照组:旧义齿使用10~15年66例,按照常规方法制作新义齿。结果:与常规方法比较,充分利用旧义齿信息制作的全口义齿患者适应较快,满意度高。戴用新义齿后3 d,分别做咀嚼效率、舒适度满意度调查,均P<0.05,差异有统计学意义;2周做总体满意度比较(P<0.05),差异有统计学意义。结论:老年人全口义齿再修复的疑难病例,需要充分利用旧义齿信息制作新义齿才能提高满意度。  相似文献

??Objective    To compare the clinical effect of modified silicone rubber impression technology with that of traditional alginate impression technology in complete denture. Methods    Twenty-three complete dentures by modified silicone impression technology and twenty-three complete dentures by traditional alginate impression technology were separately made. Sore spots were measured in situ respectively. Results    The number of sore spots in modified silicone impression technology dentures were less than those in traditional ones. ABSTRACT: BACKGROUND: According to the UK Adult Dental Health Survey (2009) 15% of adults aged 65-74, 30% aged 75-84 and 47% aged >85 years are edentulous and require complete dentures. Patients' quality of life and nutrition status are affected by poor dentures. The quality of the dental impression is the most important issue for improving the fit and comfort of new dentures. There is paucity of RCT evidence for which impression material is best for complete dentures construction. This study aims to compare two impression materials for effectiveness and cost effectiveness. METHODS: IMPROVDENT is a double-blind crossover trial comparing the use of alginate and silicone, two commonly used denture impression materials, in terms of patient preference and cost-effectiveness. Eighty five edentulous patients will be recruited and provided with two sets of dentures, similar in all aspects except for the impression material used (alginate or silicone). Patients will try both sets of dentures for a two-week period, unadjusted, to become accustomed to the feel of the new dentures (habituation period). Patients will then wear each set of dentures for a period of 8 weeks (in random order) during which time the dentures will be adjusted for optimum comfort. Finally, patients will be given both sets of dentures for a further two weeks to wear whichever denture they prefer (confirmation period). Patients will be asked about quality of life and to rate dentures on function and comfort at the end of each trial period and asked which set they prefer at the end of the habituation period (unadjusted denture preference) and confirmation period (adjusted denture preference). A health economic evaluation will estimate incremental cost-effectiveness ratios of producing dentures from the two materials. A qualitative study will investigate the impact of dentures on behaviour and quality of life. Funding: IMPROVDENT is funded by NIHR RfPB (PB-PG-0408-16300). DISCUSSION: This trial aims to provide evidence on the costs and quality of dentures cast from two different commonly used impression materials; the intention is to significantly impact on the quality of denture production within NHS dentistry. Accurate impressions are important elements in both the fabrication and maintenance phases of complete denture therapy. For patients possessing nonrestorable, periodontally hopeless residual dentitions, immediate denture therapy is often the treatment of choice. An impression procedure capable of accurately registering functional vestibular anatomy facilitates successful therapy. For complete dentures currently in function, periodic assessment and correction of fit extends long-term prosthesis performance. To maintain optimal tissue-base relationships, use of specialized impressions, and subsequent laboratory reline procedures is often indicated. For both of these impression procedures (ie, immediate denture impressions and denture reline impressions), vinyl polysiloxane (VPS) impression material offers distinct advantages. After complete denture insertion, edentulous patients usually present transitory difficulties. This is one of the most critical stages of prosthetic treatment and it is strongly related to the prostheses acceptance. The aim of this study was to evaluate potential changes in the mandibular movement pattern related to insertion of the complete denture during the functional adaptation period. The kinesiograph instrument K6-I (Myotronics Research Inc., Seattle, WA) was used to evaluate eight edentulous patients that received conventional complete denture treatment. Recordings of opening and closure movement limits, movement velocity, postural rest position, chewing cycle and limits of eccentric movements were performed. Each patient was evaluated in four different stages: with the old dentures, immediately after insertion of the new dentures, and 30 days and 6 months after insertion of the dentures. The results showed that there were no remarkable changes in the mandibular motion patterns after insertion of the new dentures. It was suggested that the problems related to the process of utilization of new dentures are not strictly associated to the mandibular movement. Therefore, functional adaptation after denture insertion is not directly related to functional changes, but to the quality of the dentures and to individual features.  相似文献

PURPOSETo improve the clinical effects of complete denture use and simplify its clinical application, a digital complete denture restoration workflow (Functional Suitable Digital Complete Denture System, FSD) was proposed and preliminary clinical evaluation was done.MATERIALS AND METHODSForty edentulous patients were enrolled, of which half were treated by a prosthodontic chief physician, and the others were treated by a postgraduate student. Based on the primary impression and jaw relation obtained at the first visit, diagnostic denture was designed and printed to create a definitive impression, jaw relation, and esthetic confirmation at the second visit. A redesigned complete denture was printed as a mold to fabricate final denture that was delivered at the third visit. To evaluate accuracy of impression made by diagnostic denture, the final denture was used as a tray to make impression, and 3D comparison was used to analyze their difference. To evaluate the clinical effect of FSD, visual analogue scores (VAS) were determined by both dentists and patients.RESULTSTwo visits were reduced before denture delivery. The RMS values of 3D comparison between the impression made via diagnostic dentures and the final dentures were 0.165 ± 0.033 mm in the upper jaw and 0.139 ± 0.031 mm in the lower jaw. VAS ratings were between 8.5 and 9.6 in the chief physician group, while 7.7 and 9.5 in the student group; there was no statistical difference between the two groups.CONCLUSIONFSD can simplify the complete denture restoration process and reduce the number of visits. The accuracy of impressions made by diagnostic dentures was acceptable in clinic. The VASs of both dentists and patients were satisfied.  相似文献
