Purpose

Hallux valgus is a complex deformity of the first metatarsophalangeal joint, with varus angulation of the first metatarsal, valgus deviation of the great toe and lateral displacement of the sesamoids and the extensor tendons. The aim of the surgery is to achieve correction of the varus deviation of the 1st metatarsal which is considered by some as the primary intrinsic predisposing factor to hallux valgus deformity.

Methods

We retrospectively reviewed 85 patients (107 feet) who underwent an opening wedge osteotomy of the 1st metatarsal for correction of moderate to severe hallux valgus and metatarsus primus varus. A medially applied anatomic pre-contoured locking plate was used for fixation of the osteotomy.

Results

The mean IMA was decreased from 15.8 (range 12–22) degrees to 7.8 (range 0–12) degrees. The mean pre-operative HVA was 39 (range 21–52) degrees and the mean postoperative HVA was 11.8 (6–19) degrees. The pre-operative AOFAS score was 52 (SD 3.1) and the postoperative score was 85 (SD 5.2).

Conclusion

The proximal opening wedge metatarsal osteotomy is a safe, effective and reproducible technique for correction of moderate to severe hallux valgus deformity. The use of a locking plate provides enough control at the fragments, enhancing healing of osteotomy and maintenance of the correction even with a violated proximal lateral cortex.  相似文献

 目的 探讨第一跖骨双截骨矫正重度外翻畸形的手术疗效。方法 回顾性分析2008年1月至2011年12月,采用第一跖骨双截骨术治疗并获得随访的62例(87足)第一跖骨远端关节面角(distal metatarsal articular angle, DMAA)增大的重度外翻患者资料,男9例(14足),女53例(73足);年龄28~70岁,平均56岁。术前X线片示外翻角(hallux valgus angle,HVA)平均为48.6°,第一、二跖骨间角(intermetatarsal angle,IMA)平均为19.8°,DMAA平均为22.1°。术前根据患者畸形情况制定手术方案,截骨部位、角度、截骨量均依据术前测量值操作。比较术前、术后及取内固定前HVA、IMA、DMAA的变化,同时采用美国足踝外科协会(AOFAS)趾-跖趾-趾间关节功能评分标准评价疗效。结果 62例(87足)获得随访,随访时间为10~57个月,平均21个月。术后6个月, HVA 14.6°±1.2°, IMA 7.9°±0.7°,DMAA 7.7°±0.9°,矫形效果均满意。术后出现僵硬2例,皮神经损伤2例,转移性跖痛1例,无一例发生畸形明显复发、骨不愈合及跖骨坏死。术后AOFAS评分由术前平均(28.4±9.1)分达到术后1年的(91.8±1.8)分;49足为优,31足为良,7足为可,优良率为92.0%(80/87)。结论 第一跖骨双截骨术可有效矫正DMAA增大的重度外翻畸形,患者术后可早期部分负重,并发症少,手术疗效佳。  相似文献

During a 12-year period in which 878 hallux valgus corrections were performed, 18 patients (21 feet) with symptomatic hallux valgus deformity and an increased distal metatarsal articular angle (DMAA) underwent periarticular osteotomies (double or triple first ray osteotomies). They were studied retrospectively at an average follow-up of 33 months. The surgical technique comprised a closing wedge distal first metatarsal osteotomy combined with either a proximal first metatarsal osteotomy or an opening wedge cuneiform osteotomy (double osteotomy). When a phalangeal osteotomy was added, the procedure was termed a "triple osteotomy." The average age of the patients at the time of surgery was 26 years. At final follow-up, the average hallux valgus correction measured 23 degrees and the average 1-2 intermetatarsal angle correction was 9 degrees. The DMAA averaged 23 degrees preoperatively and was corrected to an average of 9 degrees postoperatively. One patient developed a postoperative hallux varus deformity, and one patient developed a malunion, both of which required a second surgery. A hallux valgus deformity with an increased DMAA can be successfully treated with multiple first ray osteotomies that maintain articular congruity of the first metatarsophalangeal joint.  相似文献

BACKGROUND: Symptomatic large hallux valgus deformities commonly require surgical intervention with a proximal metatarsal osteotomy. A number of fixation methods have been described for proximal chevron osteotomies; one of the most recent is locking plates. METHODS: We retrospectively reviewed the records of 16 consecutive patients (20 feet) with severe bunion deformities who had locking-plate fixation of proximal chevron osteotomies. Clinical evaluation focused on osteotomy healing, transfer lesions, and hardware-related complications. Preoperative and postoperative radiographic evaluation included the hallux valgus angle (HVA), 1-2 intermetatarsal angle (IMA), medial 1-2 intermetatarsal distance (MIMD; the amount of narrowing of the foot), sesamoid position, first metatarsal elevation, and metatarsal length change. A postoperative American Orthopaedic Foot and Ankle Society (AOFAS) score was obtained in all patients. RESULTS: The average radiographic improvements were HVA, 16.0 degrees, IMA, 7.6 degrees, and MIMD, 9.0 mm. Sesamoid position improved in 16 of 20 feet. First metatarsal elevation averaged 0.8 degrees, and the average metatarsal shortening was less than 1 mm. The AOFAS score averaged 94.1 points. Two complications were unrelated to plate fixation. CONCLUSIONS: The locking plate held alignment and position of the first ray after chevron osteotomy without clinical evidence of transfer lesions or hardware-related symptoms. Locking plates may improve stability of the proximal metatarsal after a chevron osteotomy for correction of hallux valgus.  相似文献

The recognition, definition, and management of the congruent hallux valgus deformity continue to evolve. To correct the skeletal deformity and maintain joint congruity, many authors have emphasized the importance of extra-articular procedures. One such procedure is a distal medial closing wedge osteotomy of the first metatarsal. Unfortunately, there are few guidelines to help determine the pre- and intraoperative size of the medial wedge to obtain the desired correction of the distal metatarsal articular angle (DMAA). The purpose of this study was to quantify the effects of increasing distal medial closing wedge osteotomies on the DMAA in an in vitro cadaver model. In this study, a closing wedge osteotomy was performed 2 cm proximal to the articular surface, removing wedges measuring 2 mm, 4 mm, and 6 mm in width. The mean preoperative DMAA was 8.5 degrees, and the mean postoperative DMAAs after 2-mm, 4-mm, and 6-mm closing wedge osteotomies were -2.6 degrees, -10.2 degrees, and -20.2 degrees, respectively. The data showed that for every 1 mm of closing wedge osteotomy, the DMAA decreased by 4.7 degrees +/- 0.6 degrees. These results can be used for pre- and intraoperative planning when surgically correcting a congruent hallux valgus deformity with a distal medial closing wedge osteotomy of the first metatarsal. Additional information obtained from this cadaver study includes (1) increased shortening of the first metatarsal and (2) incongruity produced at the joint after the medial-based osteotomy. The amount of shortening of the first metatarsal correlated directly with the size of the medial-based wedge. The second point indicates that a lateral soft-tissue release may still be required when using this method of reorienting the DMAA.  相似文献

BackgroundScarf osteotomy is a frequently used technique to correct moderate to severe hallux valgus deformities. Recurrence of a deformity is a commonly reported complication after surgery. The aim of our study was to evaluate the impact of preoperative deformity on radiological outcome in terms of postoperative loss of correction after scarf osteotomy.Methods102 patients, in which a hallux valgus deformity was corrected with an isolated scarf osteotomy were included. Weightbearing radiographs were analyzed preoperatively, postoperatively, after 6 weeks and after three months (mean 10.9 months SD 17.2 months). The following radiological parameters were used for analysis: the intermetatarsal angle (IMA), the hallux valgus angle (HVA), the distal metatarsal articular angle (DMAA), position of the sesamoids, first metatarsal length, and first metatarsophalangeal joint congruity.ResultsSignificant correction of IMA, HVA, DMAA, sesamoid position and joint congruity was achieved (p < 0.001). The IMA improved from 15.8 ± 2.3 to 4.3 ± 2.8°, the HVA from 32.6 ± 6.8 to 9.1 ± 7.2, and the DMAA from 11.4 ± 6.9 to 8.4 ± 5.2°, respectively. In contrast to DMAA, throughout followup we could detect loss of correction for HVA and for IMA amounting 6.3° ± 5.8 and 3.8° ± 2.8 respectively. Loss of HVA correction revealed a significant correlation with preoperative DMAA, but not with the other preoperative radiological parameters.ConclusionsPreoperative deformity does not correlate with postoperative loss of correction after scarf osteotomy, except DMAA.Clinical relevanceOur results may be helpful in counseling patients regarding recurrence of hallux valgus deformity after scarf osteotomy.Level of evidenceTherapeutic, Level IV, retrospective case series.  相似文献

Introduction Distal metatarsal osteotomies have been described for surgical treatment of hallux valgus with good results. The aim of this study is to review the results of 299 consecutive hallux valgus cases treated by minimally invasive distal metatarsal osteotomy, S.E.R.I. (Simple, Effective, Rapid, Inexpensive). Materials and methods 299 feet in 190 patients (109 bilateral), aged between 35 and 70 years (mean age: 53 years) affected by hallux valgus were studied. A 1 cm medial incision at the metatarsal neck, and a complete osteotomy, using an oscillating saw were performed. With the naked eye all characteristics of the deformity were corrected by displacement of the metatarsal head (HVA, IMA, DMAA, dorsal or plantar displacement). The osteotomy was stabilized by a 2 mm Kirschner wire. Immediate weight bearing was allowed with gauze bandage and Talus shoes for 4 weeks. All patients were checked at an average follow-up of 4 years. Results All osteotomies healed no avascular necrosis of the metatarsal head or pseudoarthrosis of the osteotomy was observed. Mean preoperative AOFAS score was 43 and 88 at follow-up. The mean preoperative HVA was 33, while at follow-up it was 16 (P < 0.05), mean preoperative IMA was 13, while at follow-up it was 7 (P < 0.05), mean preoperative DMAA was 20, while at follow-up it was 8 (P < 0.05). Conclusion S.E.R.I. osteotomy has been simple, effective, rapid and inexpensive in correcting hallux valgus deformity. Clinical and radiographic findings showed an adequate correction of the deformity.  相似文献

Acquired hallux valgus deformity arises from progressively changing relationships of the bones in the first ray of the foot. Ligamentous laxity develops at the medial capsule of the first metatarsophalangeal (MTP) joint and the lateral capsule of the first metatarsocuneiform joint. The adductor hallucis tendons and the transverse metatarsal ligament tether the sesamoid bones and the base of the proximal phalanx of the great toe while the intermetatarsal angle (IMA) increases. These deforming forces are addressed by the distal soft tissue release in the first web space and the soft tissue plication at the medial aspect of the first MTP joint. The addition of the proximal chevron osteotomy of the first metatarsal shaft permits complete correction of the increased hallux valgus and intermetatarsal angles. The improved stability of the proximal chevron osteotomy over other types of osteotomies theoretically reduces the incidence of delayed transfer metatarsalgia. This article will show the anatomy of acquired hallux valgus, as well as the theories and techniques behind the authors' method of surgical correction.  相似文献

目的总结双平面截骨术治疗合并跖骨远端关节面角(distal metatarsal articular angle,DMAA)增大的重度[足母]外翻疗效。方法回顾性分析2014年6月-2017年12月收治并获完整随访的64例(94足)合并DMAA增大的重度[足母]外翻患者临床资料。患者均接受双平面截骨术(跖骨远端Reverdin截骨术+跖骨近端开放楔形截骨术)联合Akin截骨术及软组织手术。男10例(15足),女54例(79足);年龄26~66岁,平均44.5岁。单侧34例,双侧30例。参照美国矫形足踝协会(AOFAS)Maryland跖趾关节评分系统评分为(54.3±7.4)分,疼痛视觉模拟评分(VAS)为(6.0±2.0)分。比较手术前后AOFAS Maryland跖趾关节评分系统评分及VAS评分,以及[足母]外翻角(hallux valgus angle,HVA)、第1-2跖骨间角(first-second intermetatarsal angle,1-2IMA)、DMAA、第1跖骨长度(first metatarsal length,FML)。结果术后切口均Ⅰ期愈合。患者均获随访,随访时间12~15个月,平均13.2个月。4足发生并发症,其中[足母]僵硬、内侧切口边缘皮肤感觉麻木、转移性跖痛、第1跖骨头坏死各1足。术后1年AOFAS Maryland跖趾关节评分为(89.2±7.4)分,与术前比较差异有统计学意义(t=18.427,P=0.000);其中优78足、良12足、中3足、差1足,优良率为95.7%。VAS评分为(1.5±2.0)分,较术前明显改善(t=10.238,P=0.000)。X线片复查显示术后3个月截骨均达骨性愈合。术后6个月及1年HVA、1-2IMA、DMAA与术前比较,差异均有统计学意义(P<0.05);术后1年FML与术前比较,差异无统计学意义(t=0.136,P=0.863)。结论双平面截骨术可以显著改善合并DMAA增大的重度[足母]外翻患者临床症状以及影像学参数,术后并发症少。  相似文献

BackgroundWe aimed to compare the clinical and radiographic outcomes of minimally invasive surgery (MIS) and distal chevron metatarsal osteotomy (DCMO) for young female patients with mild-to-moderate juvenile hallux valgus deformity.MethodsWe retrospectively reviewed the radiographs and clinical findings of young female patients with mild-to-moderate juvenile hallux valgus who underwent MIS (25 feet) or DCMO (30 feet). In 12 of 25 MIS feet, 2.0-mm bio-absorbable pins were used as an additional fixation device crossing the osteotomy site, and 1.4-mm Kirschner wires were used in the remaining 13 feet.ResultsRadiographic and clinical parameters preoperatively and at the final follow-up were not significantly different between the 2 groups. There were no significant differences in the increments of hallux valgus angle (HVA), distal metatarsal articular angle, medial sesamoid position, first metatarsal length, metatarsal length index, or relative second metatarsal length. Two MIS subgroups according to the additional fixation device showed no significant differences in HVA, the first to second intermetatarsal angle lateral translation ratio, or plantar offset at the final follow-up.ConclusionsMIS for young female patients with mild-to-moderate juvenile hallux valgus deformity had similar radiographic and clinical outcomes compared to DCMO. Regarding additional fixation crossing the osteotomy site, both temporary Kirschner wires and absorbable pins showed no radiographic differences in terms of correction maintenance.Level of evidence: 3.  相似文献

目的 :介绍一种伴有足横弓塌陷的严重外翻的手术方法。方法 :12例伴有足横弓塌陷的严重的外翻足首先经X线、CT检测其三维畸形程度 ,而后手术中采用第一跖骨基底截骨 ,远端外推、下压、外旋以纠正第一跖骨的三维畸形 ,恢复足横弓正常形态 ,矫形位置以克氏针固定 :对外翻角 (HVA)的纠正采用常规第一跖趾关节软组织手术。结果 :术后 3~ 12月随访 ,优 6例、良 4例、中 2例 ,优良率 83 % ,其中 ,前足底中部痛性胼胝均有明显减轻。结论 :足横弓重建手术是严重外翻较好的选择术式。  相似文献

Moderate and severe hallux valgus usually consists of metatarsus varus and incongruency of the joints. Basal osteotomy and distal soft tissue release provides adequate correction of intermetatarsal angle (IMA) and joint alignment.This is a retrospective study of 26 feet in 20 patients. American Orthopaedics Foot and Ankle Society Score (AOFAS) and subjective grading system for patient's satisfaction were used for assessment. The surgical technique consisted of crescentic basal osteotomy, lateral distal soft tissue release and medial capsular plication according to Roger Mann. Fourteen osteotomies were fixed with K wires and Barouk Screws were used in 12 feet. Patients were mobilised in high heeled shoe post-operatively.Average age was 55.2 years and average follow-up was 25.8 months. The average preoperative hallux valgus angle (HVA) and IMA were 37.38 and 17.27°, respectively. The average post-operative HVA and IMA were 13.3 and 6.4°, respectively. All incongruent joints became congruent after surgery. Sesamoid position improved in 25 feet. Average AOFAS score was 88.8. Ninety-four percent patients were highly satisfied.Complications included breakage of K wire in one leading to change in practice, decreased sensations over medial side of toe in three and mild metatarsalgia in one foot.Basal osteotomy with distal soft tissue release provides good correction of moderate to severe deformity and has high patient satisfaction. 目的探讨第1跖列三平面截骨联合其余跖骨基底截骨治疗中重度跖内收型[足母]外翻的早期疗效。方法2012年2月-2016年9月,收治10例(12足)中重度跖内收型[足母]外翻患者。男1例(2足),女9例(10足);年龄21~55岁,平均34.5岁。病程3~10年,平均5.8年。根据改良Sgarlato测量法对跖内收分度:中度4足、重度8足。术前美国矫形足踝协会(AOFAS)评分为(46.4±9.3)分;跖内收角(metatarsus adductus angle,MAA)为(25.41±3.66)°,[足母]外翻角(hallux valgus angle,HVA)为(41.42±9.67)°,第1-2跖骨间角(first-second intermetatarsal angle,1-2IMA)为(10.72±2.26)°。采用第1跖列三平面截骨联合其余跖骨基底截骨以及外侧软组织松解术治疗。结果术后切口均Ⅰ期愈合。患者均获随访,随访时间18~24个月,平均21.4个月。1例(1足)术后出现转移性跖痛,对症处理后症状消失。X线片复查示截骨部位均愈合,愈合时间为2.4~3.2个月,平均2.8个月。末次随访时,MAA为(8.42±0.71)°、HVA为(13.29±1.03)°、1-2IMA为(4.41±0.48)°,AOFAS评分为(89.8±5.9)分,均较术前明显改善(P<0.05)。结论第1跖列三平面截骨联合其余跖骨基底截骨治疗中重度跖内收型[足母]外翻,可获得较好早期疗效。  相似文献

BackgroundPercutaneous, implantless basal closing wedge osteotomy (PIBCO) is a new technique for hallux valgus (HV) deformity correction that does not need internal fixation. We present this technique and its short-term clinical outcomes for moderate-to-severe HV deformities.MethodsA retrospective review of 162 feet in 114 patients who underwent PIBCO of the first metatarsal (MT1) was conducted. Follow-up averaged 18.5 months. Outcomes were assessed using the visual analog scale (VAS), hallux valgus angle (HVA), intermetatarsal angle (IMA), and radiologic union. Complications were also assessed.ResultsAverage VAS score improved from 6.2 to 1.0 (P < 0.001). HVA improved from 29.8° to 7.6° (P < 0.001). IMA improved from 14.5° to 6.3° (P < 0.001). A total of 8 major complications were noted (4.9%).ConclusionsSatisfactory HV deformity correction was achieved. However, some instances may require internal fixation in order to avoid complications related to instability and noncompliance of postoperative rehabilitation protocols.  相似文献

目的 探讨第1跖骨基底截骨结合Chevron-Gerbert手术治疗严重(足母)外翻畸形的临床疗效.方法 2004年6月至2008年8月利用第1跖骨基底截骨结合Chevron-Gerbert手术治疗重度(足母)外翻患者37例(66足),男性5例(10足),女性21例(38足);年龄21~76岁,平均58岁.术前、术后、随访时均拍摄足部负重位正、侧位X线片,测量(足母)外翻角、第1、2跖骨间角、近侧关节固定角.比较术前、术后6周、末次随访时(足母)外翻相关各角度的X线测量值,并结合美国足踝外科协会(AOFAS)的Maryland评分法观察疗效.结果 本组患者26例(48足)得到随访,随访时间1~4年,平均2.3年.末次随访时(足母)外翻角较术前纠正25.6°±3.8°,第1、2跖骨间角较术前纠正8.6°±2.4°,近侧关节固定角较术前纠正4.7°±4.2°.根据AOFAS百分评分法评定,优15例(28足)、良8例(16足)、可3例(4足),优良率为91.7%.结论 利用第1跖骨基底截骨结合Chevron-Gerbert手术治疗严重(足母)外翻畸形的临床疗效良好,但有手术相对复杂、需行多段截骨及内固定等缺点.  相似文献

目的探讨改良Scarf截骨术治疗拇外翻的临床疗效。方法应用改良Scarf截骨术治疗拇外翻患者68例(92足),比较手术前后拇外翻角(HVA)、第1、2跖骨间角(IMA)及远端关节面固有角(DMAA)的变化。末次随访时,采用AOFAS评分标准评价疗效。结果68例患者均获得随访,时间9~12个月。末次随访时,HVA、IMA、DMAA均较术前显著减小(P<0.01),AOFAS评分较术前显著升高(P<0.01),疗效优、良、可分别为82、5、5足,治疗优良率为94.6%。结论改良Scarf截骨术是矫正拇外翻畸形的可靠技术,纠正畸形效果满意,能有效缓解疼痛。  相似文献

目的:探讨经皮微创截骨术联合“8”字绷带和分趾垫外固定治疗中度拇外翻的临床疗效。方法:自2019年8月至2021年1月采用经皮微创截骨术联合“8”字绷带和分趾垫外固定治疗中度拇外翻患者23例,失访1例,最终纳入22例(30足),男4例6足,女18例24足,年龄27~66 (50.59±11.95)岁。比较术前和术后6个月患足拇外翻角(hallux valgus angle,HVA),第1、2跖骨间角(intermetatarsal angle,IMA),跖骨跨度(第1、5跖骨头之间的距离)和软组织宽度变化情况,并采用美国足踝外科协会踝-后足评分标准(American Orthopaedic Foot and Ankle Society,AOFAS)进行临床疗效评价,采用视觉模拟评分(visual analogue scale,VAS)评价足部自觉疼痛情况。结果:22例患者获得随访,时间5.7~6.4(6.13±0.85)个月。患者第1跖骨截骨处均获得骨性愈合,足趾外观畸形得到纠正,术后患足未出现跖骨头缺血性坏死、转移性跖骨痛等并发症。术后6个月患足HVA、IMA、跖骨跨度、软组织宽度、...  相似文献

BACKGROUND: Distal osteotomy of the first metatarsal is indicated for the surgical treatment of mild-to-moderate hallux valgus deformity. The aim of this study was to evaluate the results of a subcapital distal osteotomy of the first metatarsal with use of a percutaneous technique. METHODS: From 1996 to 2001, 118 consecutive percutaneous distal osteotomies of the first metatarsal were performed for the treatment of painful mild-to-moderate hallux valgus in eighty-two patients. The patients were assessed with a clinical and radiographic protocol at a mean of 35.9 months postoperatively. The American Orthopaedic Foot and Ankle Society (AOFAS) hallux-metatarsophalangeal-interphalangeal scale was used for the clinical assessment. RESULTS: The patients were satisfied following 107 (91%) of the 118 procedures. The mean score on the AOFAS scale was 88.2 +/- 12.9 points. The postoperative radiographic assessments showed a significant change (p < 0.05), compared with the preoperative values, in the mean hallux valgus angle, first intermetatarsal angle, distal metatarsal articular angle, and sesamoid position. The valgus deformity recurred after three procedures (2.5%), the first metatarsophalangeal joint was stiff but not painful after eight (6.8%), and a deep infection developed after one (0.8%). The infection resolved with antibiotic therapy. CONCLUSIONS: The percutaneous technique proved to be reliable for the correct execution of a distal linear osteotomy of the first metatarsal for the correction of a painful mild-to-moderate hallux valgus deformity. The clinical results appear to be comparable with those obtainable with traditional open techniques, with the additional advantages of a minimally invasive procedure, a substantially shorter operating time, and a reduced risk of complications related to surgical exposure.  相似文献

目的探讨距足母囊近端2.0 cm处Z型截骨并T型微型钛板内固定治疗中重度足母外翻的临床疗效。方法2009年11月至2013年6月,对50例(81足)中重度足母外翻患者行第一跖骨远端Z型截骨后T型微型钛板内固定治疗。结果术后随访3~36个月,患者足母外翻角(halluxv algus angle,HVA)减少了11°~23°,平均13.1°,跖骨间角(intermetatarsal angle,IMA)减少了5°~10°,平均6.6°。采用美国足踝矫形学会Maryland足功能评分系统评分:90~100分,79足,80~89分2足,优良率100%,未见感染,截骨处均获骨性愈合,畸形无复发。结论距足母囊近端截骨并微型钛板内固定治疗中重度足母外翻疗效可靠,值得推广。  相似文献

We compared the results of proximal chevron osteotomy and double metatarsal osteotomy for hallux valgus with an increased distal metatarsal articular angle (DMAA). From October 2008 to December 2012, first metatarsal osteotomies were performed in 64 patients (69 feet) with symptomatic hallux valgus associated with an increased DMAA. Proximal chevron with Akin osteotomy and lateral soft tissue release was performed in 46 feet (PCO group); double metatarsal osteotomy and Akin osteotomy without lateral soft tissue release was performed in 23 feet (DMO group). Clinical assessments were performed using the American Orthopaedic Foot and Ankle Society (AOFAS) scale and visual analog scale (VAS). The hallux valgus angles, intermetatarsal angles, sesamoid positions, metatarsus adductus angles, and DMAAs were compared at different postoperative times. Postoperative shortening of first the metatarsal and complications were compared. The mean AOFAS scale and VAS scores showed significant improvement in both groups after surgery; however, no significant difference was observed between the 2 groups. The immediate postoperative hallux valgus angle and sesamoid position were significantly larger in DMO group; however, no intergroup difference was observed at the last follow-up visit, with the hallux valgus angle gradually increasing in the PCO group. The postoperative DMAA was significantly smaller in the DMO group. The mean shortening of the first metatarsal after surgery was significantly larger in the DMO group than in the PCO group. Transfer metatarsalgia developed in 1 foot (2.2%) in the PCO group and 2 feet (8.7%) in the DMO group. Partial avascular necrosis of the metatarsal head with advanced arthritis of the first metatarsophalangeal joint developed in 1 foot (4.3%) in the DMO group. In conclusion, no differences in the clinical and radiographic results were observed between the 2 groups for hallux valgus deformity with an increased DMAA.  相似文献
