The purpose of this study was to assess the feasibility of high spatial resolution, selective arterial phase, 3D contrast-enhanced
(CE) MR angiography with first pass bolus, software-trigger, elliptical centric view ordering in the detection of intracranial
aneurysms. Our study included nine consecutive patients with ten intracranial aneurysms. 3D TOF MR angiography and 3D CE MR
angiography were carried out with a 1.5-T MR scanner. 3D CE MR angiography was performed with an automated bolus detection
algorithm and elliptical centric view order using ultrafast SPGR with a spatial resolution of 0.63×0.83×0.5 mm and imaging
time of 55 s. Observers detected seven of ten aneurysms on 3D TOF MR angiograms and nine of ten aneurysms on 3D CE MR angiograms.
3D CE MR angiography clearly revealed an IC-PC aneurysm with a relatively smaller neck, a broad-based small aneurysm originating
from tortuous and dilated MCA bifurcation, and a residual aneurysm and parent vessels adjacent to metallic aneurysmal clips,
which had relatively low signal intensities on 3D TOF MR angiograms. 3D CE MR angiography was found to be a good and promising
technique for detecting intracranial aneurysms with small necks and slow flow, vasculature with aneurysmal clips and tortuous

HASTE (HAlf fourier Single-shot Turbo spin-Echo) is a single-section T2-weighted sequence that acquires images in less than 1 second. Images are breathing independent and possess a variety of other features useful for imaging the abdomen. The design of this technique is described. Clinical studies of 38 consecutive patients were performed using this technique. HASTE images were considered good in 28 and fair in 10 patients, including five patients who could not suspend respiration. Definition of liver and bowel was particularly clear. 相似文献

The purpose of this paper was to develop and evaluate a fast inversion recovery (FIR) technique for T1-weighted MR imaging of contrast-enhancing brain pathology. The FIR technique was developed, capable of imaging 24 sections in approximately 7 minutes using two echoes per repetition and an alternating echo phase encoding assignment. Resulting images were compared with conventional T1-weighted spin echo (T1SE) images in 18 consecutive patients. Compared with corresponding T1SE images, FIR images were quantitatively comparable or superior for lesion-to-background contrast and contrast-to-noise ratio (CNR). Gray-to-white matter and cerebrospinal fluid (CSF)-to-white matter contrast and CNR were statistically superior in FIR images. Qualitatively, the FIR technique provided comparable lesion detection, improved lesion conspicuity, and superior image contrast compared with T1SE images. Although FIR images had greater amounts of image artifacts, there was not a statistically increased amount of interpretation-interfering image artifact. FIR provides T1-weighted images that are superior to T1SE images for a number of image quality criteria. 相似文献

RATIONALE AND OBJECTIVES: The purpose of the study was to determine the dose and echo time dependence of abdominal vessel enhancement at magnetic resonance (MR) imaging after injection of a blood pool contrast agent at two field strengths. MATERIALS AND METHODS: Sixteen healthy volunteers received NC100150 Injection at three dose levels (1.0 mg, 2.5 mg, and 4.0 mg of iron per kilogram of body weight). Images of the aorta and inferior vena cava (IVC) were obtained at 0.5 or 1.5 T. Four sequences with varying echo times were used with each subject. Signal intensities were recorded from the aorta, IVC, vessel vicinity, air, and a marker outside the patient. Contrast-to-noise ratios (CNRs) were calculated for the vessels. Aortic delineation was subjectively evaluated. RESULTS: Images with the highest mean vessel signal intensities, subjectively assessed as satisfactory for aortic delineation, were obtained with 2.5-4.0 mg of iron per kilogram of body weight at both field strengths. The highest CNR was found with 4.0 mg of iron per kilogram of body weight at 1.5 T. An increase in echo time caused larger signal intensity loss at larger dose levels. The signal intensity from the IVC was higher than that of the aorta at all dose levels, echo times, and field strengths. CONCLUSION: NC100150 Injection is an efficient T1-reducing agent at both 0.5 and 1.5 T. A positive dose response for CNR of the aorta and IVC was seen at 1.5 T. 相似文献

AIM: To evaluate the feasibility of performing in vivo proton magnetic resonance spectroscopy ((1)H-MRS) of cervical lymph nodes, and the clinical usefulness of the technique in the characterization of cervical lymphadenopathy. MATERIALS AND METHODS: Cervical lymphadenopathy was examined in 20 individuals with malignant disease, i.e. 10 with squamous cell carcinoma (SCC), 6 with undifferentiated carcinoma (UDC) and 4 with non-Hodgkin's lymphoma (NHL). Cervical lymphadenopathy was also examined in 4 individuals with benign disease, i.e. 3 with tuberculosis (TB) and 1 with Castleman's disease. A point-resolved spectroscopic sequence with echo times (TE) of 136 and 272 ms and a time-domain spectral fitting procedure were used to estimate peak amplitude of choline (Cho), creatine (Cr) and unsuppressed water. Cho/Cr and Cho/water ratios were measured for each lesion. The mean ratio for each lesion group was obtained and results were compared statistically. RESULTS: At TE of 136 ms, spectra were interpretable in all 24 cases and a Cr peak was identified with post-processing in 15 cases. The Cho/Cr and Cho/water ratios for NHL were 9.1 +/- 5.2 and 1.7 +/- 0.2 x 10(-3), for UDC were 4.4 +/- 0.9 and 1.2 +/- 0.4 x 10(-3), and for SCC were 2.1 +/- 0.6 and 0.5 +/- 0.3 x 10(-3), respectively. Both Cho/Cr and Cho/water ratios for UDC were significantly higher than SCC (p = 0.002 and 0.026, respectively). At TE of 272 ms, spectra were interpretable in 22 of 24 cases and a Cr peak was identified with post-processing in 11 cases. Cho/Cr and Cho/water ratios for NHL were 5.4 and 4.6 +/- 1.4 x 10(-3), for UDC were 4.2 +/- 1.5 and 2.6 +/- 1.0 x 10(-3) and for SCC were 2.5 +/- 1.1 and 1.3 +/- 0.6 x 10(-3), respectively. The Cho/water ratio for UDC was significantly higher than for SCC (p = 0.04). The Cho/Cr ratio for UDC was also higher than for SCC, but this difference was not statistically significant (p = 0.07). Neither Cho nor Cr was detected in the 3 cases of TB. CONCLUSION: In vivo (1)H-MRS is a feasible technique for the evaluation of cervical lymph nodes and it offers potential as a clinical tool in the investigation of cervical lymphadenopathy. However, further studies with larger patient cohorts are needed to validate the findings of this initial report. 相似文献

Objective
This study was designed to determine whether diffusion-weighted imaging (DWI) with sensitivity encoding (SENSE) could detect bone marrow involvement in patients with cranial bone marrow (CBM) metastases. DWI results obtained were compared with T1-weighted imaging (T1WI) findings.Materials and Methods
DWI with sensitivity encoding (SENSE; b value = 1,000) was performed consecutively in 13 patients with CBM metastases diagnosed pathologically and radiologically. CBM lesions were dichotomized according to the involved site, i.e., skull base or calvarium. Two radiologists qualitatively evaluated the relative conspicuousness of CBM lesions and image qualities in B0 and in isotropic DWI and in T1WI. According to region of interest analysis of normal and pathologic marrow for these three sequences, absolute signal difference percentages (SD%) were calculated to quantitatively analyze lesion contrast.Results
All 20 lesions in 13 patients with CBM metastases revealed abnormal DWI signals in areas corresponding to T1WI abnormalities. Both skull base and calvarial lesions provided better lesion conspicuousness than T1WI and B0 images. Although the image quality of DWI was less satisfactory than that of T1WI, relatively good image qualities were obtained. Quantitatively, B0 images (SD%, 82.1 ±7.9%) showed better lesion contrast than isotropic DWI (SD%, 71.4 ±13.7%) and T1WI (SD%, 65.7 ±9.3%) images.Conclusion
For scan times of less than 30 seconds, DWI with SENSE was able to detect bone marrow involvement, and was superior to T1WI in terms of lesion conspicuity. DWI with SENSE may be helpful for the detection of cranial bone/bone marrow metastases when used in conjunction with conventional MR sequences. 相似文献7.

The aim of the study was to apply time-of-flight MR angiography to renal transplant arteries with comparison of two- and three-dimensional (2D and 3D) sequences and to correlate the findings with colour flow sonography (CFS) and digital subtraction angiography (DSA). A total of 102 MR studies were performed in 101 patients: 87 with the 2D-FLASH sequence (18 repeated after Gd-DOTA administration), 49 with the 3D-FISP (both in 34). All patients were also studied with CFS and 15 with intra-arterial DSA. The 3D sequence produced good-quality MR angiograms in 94% of cases (82% in 2D). Gd-DOTA infusion improved the quality of the 2D angiograms in 7 of 18 cases. Only these patients were included in the remainder of the evaluation (90 patients with 103 arteries). CFS showed 72 normal and 10 abnormal arteries. In this group, the 2D sequence led to 7 (12%) false positives of stenosis and the 3D sequence yielded 1 (3%). Correlation between MR angiography and DSA was obtained for 21 arteries (15 patients) with suspicion of arterial complications. The 2D-FLASH (n = 13) and the 3D-FISP (n = 12) MR sequences allowed the correct diagnosis of all main artery complications (14 stenoses and 4 thromboses) without any false negatives and without discordance when both sequences were performed (n = 4). In the 3 other cases with a normal main artery, 2 segmental thromboses were correctly identified by both sequences and 1 stenosis of a segmental branch was correctly identified by the 2D sequence only but misdiagnosed as a thrombosis with the 3D sequence. Grading of the severity of stenoses was inaccurate with both sequences. It is concluded that the 3D time-of-flight MR sequence provides better MR angiograms than the 2D, with fewer false positives for stenosis. No false-negative arterial complications were noted.

The purpose of this study was to prospectively investigate the extent to which reduced portal blood flow in patients with hepatic cirrhosis and portal hypertension affects hepatic parenchymal enhancement during gadolinium-chelate-enhanced dynamic MR imaging. Breath-hold three-dimensional (3D) spoiled gradientrecalled echo (GRE) MR imaging technique obtained after intravenous administration of a gadolinium chelate was used to measure hepatic parenchymal enhancement and time to peak enhancement in 20 patients with hepatic cirrhosis and clinical evidence of portal hypertension (group 1) and in 20 control subjects without portal hypertension (group 2) who were matched for age, sex, and body weight. Mean peak hepatic enhancement values ± SD and times to peak enhancement ± SD were determined for both groups of patients. Mean peak enhancement value (±SD) was 78.7% ± 36.2 in group 1 and 91.6% ± 46.2 in group 2 (not significant). However, in the nine patients in group 1 with splenomegaly, mean peak enhancement value was 61.3% ± 14.4, whereas it was 93.0% ± 42.7 in the 11 patients without splenomegaly (P < .05). Mean time to peak enhancement was 84 seconds ± 23 in group 1 and 54.0 sec ± 25.0 in group 2 (P < .01). Our results show that mean peak enhancement value of hepatic parenchyma after intravenous administration of a gadolinium chelate is significantly altered for patients with portal hypertension and splenomegaly. In addition, the time to peak enhancement is delayed significantly when portal hypertension is present. Thus, it is possible that the optimal time for imaging the liver during the portal phase must be tailored to the status of the portal system of the patient. 相似文献

RATIONALE AND OBJECTIVES: The morbidity associated with a diagnostic test can influence its cost-effectiveness, but the quantification of that morbidity is controversial. Accounting for pain and invasiveness requires the measurement of "process utility" in addition to the expected value of testing. An original time trade-off variant was applied to the imaging evaluation of cerebrovascular disease, for which differences in morbidity are important to patients. MATERIALS AND METHODS: A "waiting trade-off" (WTO) was used to evaluate the preferences of 89 patients for magnetic resonance (MR) angiography and conventional x-ray angiography. Patients were experienced with both tests. A weighted difference was calculated between the period a patient was willing to wait for a test result and treatment after a hypothetical "ideal" test and the choice to undergo conventional angiography or MR angiography with immediate treatment. A rating scale was used to check the convergent validity of the WTO. RESULTS: Paired data showed a highly significant difference (P = .0001) between the mean preference for conventional and MR angiography, favoring the latter and translating into a difference of 5 quality-adjusted life days. The more negatively patients judged their conventional angiographic experience, the longer they were willing to wait for the ideal test result. CONCLUSION: The WTO provides a reasonable estimate of the relative morbidity of more invasive conventional angiographic procedures and provides a quality-adjustment term for economic analysis. Such an approach may enable more complete evaluation of the effects of other processes on medical care. 相似文献

PURPOSE: To validate partial Fourier phase contrast magnetic resonance (PC MR) with full number of excitation (NEX) PC MR measurements in vitro and in vivo. MATERIALS AND METHODS: MR flow measurements were performed using a partial Fourier and a full NEX PC MR sequence in a flow phantom and in 10 popliteal and renal arteries of 10 different healthy volunteers. Average velocity, peak velocity, and flow results were calculated and compared with regression analysis. RESULTS: Excellent correlations in average velocities (r = 0.99, P < 0.001), peak velocities (r = 0.99, P < 0.001), and flow rates (r = 0.98, P < 0.001) were demonstrated in vitro between the two different acquisitions. For the popliteal arteries there was excellent correlation between peak velocities for both acquisitions (r = 0.98, P < 0.0001); the correlation of average velocity measurements when using all data points in the cardiac cycle for all volunteers was 0.96 (P < 0.001). For the renal arteries the same comparison resulted in a good correlation for average velocity (0.93, P < 0.001) and peak velocity measurements (r = 0.91, P = 0.002), although the correlation coefficient for flow rates was 0.88 (P = 0.004). Blurring of the vessel margins was consistently observed on magnitude images acquired with the partial Fourier method, causing overestimation of the vessel area and some error in the flow measurements. CONCLUSION: Partial Fourier PC MR is able to provide comparable average and peak velocity values when using 1 NEX PC MRI as a reference. 相似文献

RATIONALE AND OBJECTIVES: This study was performed to evaluate the relationship between dose levels of contrast medium and image quality in magnetic resonance (MR) angiography of the carotid arteries with fluoroscopically monitored, manually triggered, elliptically ordered image acquisitions. MATERIALS AND METHODS: Twenty-five patients with clinical indications for angiography of the carotid arteries were examined with MR at 1.5 T by using a fluoroscopically monitored, manually triggered, elliptically ordered pulse sequence with the administration of one of three different volumes of gadolinium-based contrast medium. The signal intensities of the vessel lumen and the surrounding tissues were measured in single partitions at the origin of the common carotid artery, the carotid bifurcation, and the intracranial internal carotid arteries. The contrast-to-noise ratio in these regions of interest also was measured. Maximum intensity projection image quality was appraised for blurring, artifacts, venous enhancement, background suppression, and contrast medium distribution. RESULTS: No artifacts or venous enhancement was observed. The position of the fluoroscopic section affected the distribution of contrast medium along the vessel, as evidenced by the difference between the contrast-to-noise ratio at the origin of the common carotid artery and the ratio at the carotid bifurcation and the intracranial internal carotid arteries (P < .01). The contrast medium dose administered was strongly correlated with image quality (r = 0.90). CONCLUSION: Contrast medium dose is related to image quality in MR angiography of the carotid arteries performed with elliptical ordering, fluoroscopic monitoring, and manual triggering. 相似文献

A FLASH technique was used, which encompassed the entire thorax in the transverse plane, before and after dynamic intravenous injection of godalinium DTPA (Gd-DTPA) to study 7 patients with normal lungs, 12 patients with interstitial lung disease (ILD), and 11 patients with pulmonary nodules. Comparative CT studies were obtained within 2 weeks of the MRI study in the patients with lung disease. Quantitative signal intensity (SI) measurements were performed. Qualitative evaluation of lung parenchyma was determined in a prospective blinded fashion, and in the normal group comparison was made with the CT images. In normal patients, SI of lung parenchyma increased by 7.7 ± 1.3%. On precontrast images, second-order pulmonary branchings were visible while post-contrast, fifth-to sixth-order branches were apparent. In patients with ILD, interstitial changes enhanced to a variable ] substantial (308.4%). Detection of pulmonary nodules improved following contrast injection. The minimum lesion size detectable decreased from 8 mm precontrast to 5 mm post-contrast. The lack of a naturally occurring background signal from fluorine in magnetic resonance (MR) imaging makes fluorinated compounds potentially attractive candidates for tissue-specific MR contrast agents. Problems associated with the in vivo use of fluorinated compounds are toxicity, which limits the amount of agent that can be used; multiple resonance lines; and an excessively long T1, which leads to long sequence TRs and consequently long imaging times. Many fluorinated agents also possess complex MR spectra that result in chemical shift artifacts if not corrected. The authors demonstrate the use of an extracellular fluorinated agent with a single MR peak for selective imaging of a brain abscess in an animal model and show that the image signal per unit of acquisition time can be enhanced through the use of a T1 relaxation agent, gadolinium diethylenetriamine-pentaacetic acid (DTPA). Trifluoromethylsulfonate was administered at a fluorine-19 dose of 4 mmol/kg, and fluorine images of the induced abscess were acquired before and after the injection of a standard dose of Gd-DTPA (0.1 mmol/kg); non—section-selected projection images were used. Typical imaging times were less than 5 minutes. The signal enhancement factor achieved was approximately four (4.0 ± 0.8) with use of a 500/12 (TR msec/TE msec) spinecho sequence. 相似文献

Ultrafast gradient systems and hybrid imaging sequences offer the opportunity to acquire phase contrast flow data in real time. In a 1.5-Tesla magnetic resonance (MR)-tomograph, peak velocity and volume flow were assessed in 36 large vessels (aorta) and 33 medium-sized vessels (carotid and iliac artery) using a real-time (segmented k-space turbo gradient-echo planar imaging sequence) in comparison with a gradient-echo technique. With the real-time technique, the matrix was reduced from 116 to 64, and temporal resolution changed from 30 msec to 124 msec. Measurements of peak velocity correlated in large (r = 0.88) and medium-sized vessels (r = 0.81). Volume flow measurements correlated in large vessels (r = 0.87), however, a poor correlation (r = 0.64) was found in medium-sized vessels. Thus, scan time can be significantly reduced and images acquired without electrocardiogram (ECG)-triggering. Flow volume can only be determined in large vessels with sufficient accuracy, mainly due to reduced spatial resolution in smaller vessels. 相似文献

OBJECTIVE: To evaluate the evolution of a metastatic bone tumor model with MRI-pathology correlation. MATERIALS AND METHODS: VX2 carcinoma was implanted into the tibiae of 20 rabbits. The rabbits were divided into four groups of five (Groups I-IV). MRI was repeated at 1-week interval up to the fourth week, including sagittal T1-weighted image (T1WI), T2-weighted image (T2WI), gadolinium-enhanced fat-suppressed T1WI (GdT1WI), and diffusion-weighted image (DWI). Each group was sacrificed after the imaging, then histological examination for the tibiae with an implanted tumor was performed and MRI-pathologic correlation was done. RESULTS: On MRI-pathology correlation, the corresponding findings were as follows; low SI on T1WI, T2WI-tumor cells, fibrosis (1 week); central low SI on T1WI, T2WI, GdT1WI -tumor cells with fibrosis and necrosis; peripheral high SI on T2WI, DWI, GdT1WI-edema, fibrosis (2 weeks); heterogeneous SI with central low SI on T2WI, DWI-tumor cell nests with extensive necrosis, fibrosis; high SI on T2WI along periosteum-periosteal reaction; high SI around low SI and in bone marrow on T2WI, DWI, GdT1WI-edema, fibrosis; low SI on T1WI in surrounding bone marrow-tumor extension (3-4 weeks). CONCLUSION: The evolution of VX2 carcinoma model was well depicted on MR imaging. Necrosis and extent of tumor were best depicted on enhanced, fat-suppressed T1-weighted images. Heterogeneity of the tumor, peripheral edema, and fibrosis were represented well on T2-weighted images. Diffusion-weighted imaging could have a role in depicting necrosis in the evaluation of bone tumor. 相似文献

In vivo sodium-23 and hydrogen-1 magnetic resonance (MR) imaging and spectroscopy of the rat brain during infusion of the shift reagent thulium DOTP5- (1,4,7,10-tetraazacyclododecane-1,4,7,10-tetra[methylene phosphonate] was performed to assign the various peaks observed during infusion and to evaluate the shift reagent in discriminating tissue compartments. Na-23 spectra collected during the infusion showed two shifted peaks that were assigned to intravascular Na+ and extracellular muscle Na+, respectively, and one unshifted peak assigned to intra- and extracellular brain Na+ and cerebrospinal fluid Na+. These assignments were validated with H-1 and Na-23 MR imaging and Na-23 chemical shift imaging (CSI). The H-1 and Na-23 images showed that a surface coil placed on a rat head can detect a substantial amount of signal from muscle surrounding the skull. Na-23 CSI spectra from successive 1-mm-thick coronal sections indicated that the shift reagent did not cross the blood-brain barrier. The study also showed that bulk susceptibility shifts are quite small with Tm-DOTP5-. This reagent may be useful in determining compartmental Na+ concentrations and blood flow kinetics in brain and in examining the integrity of the blood-brain barrier. 相似文献

Summary A case of CPM/ODS with mesencephalic involvement is presented. The lesions were nonenhancing on CT and were homogeneous and well-defined on MR with prolonged T1- and T2-relaxation times. MR is recommended for imaging the pontomesencephalic demyelinating lesions associated with this disease. 相似文献
