Voice quality after narrow-margin laser cordectomy compared with laryngeal irradiation.

Voice quality remains the issue often used to support preference for radiotherapy in treatment of early glottic cancer. We therefore conducted a perceptual voice study in 2 groups, one treated with radiotherapy for malignant disease and the other with narrow-margin laser cordectomy for either malignant or extensive benign lesions. Sequential patients, 12 treated with radiotherapy and 30 with CO(2) laser excision, were included. Voice samples were recorded before and at intervals after surgery. Ratings of validated judges were used for statistical analysis of various voice characteristics at each time point. Voice deteriorated temporarily after surgery as compared with the radiated group; however, at 6 and 24 months no significant differences were found between the groups. Preferential use of narrow-margin laser cordectomy for appropriate early glottic tumors can be supported not only for oncologic reasons but also on the basis of voice results, cost, and efficiency considerations.     

Voice restoration after laryngopharyngoesophagectomy

Although various methods of alaryngeal communication after laryngectomy have been described, little attention has been given to voice restoration after laryngopharyngoesophagectomy. This article deals with our experience reconstructing the hypopharynx and cervical esophagus in five patients by the use of microvascular free tissue grafts. Voice restoration procedures with use of a Blom-Singer prostheses were used uniformly and resulted in the attainment of functional voice in each patient. It is our feeling that this method should be used equally in patients undergoing both traditional laryngectomy and laryngopharyngoesophagectomy.

Voice target time in Parkinson's disease: A preliminary report.

A target-matching paradigm was developed to assess the vocal equivalents of reaction and movement time in Parkinson's disease. Six patients with Parkinson's disease and 6 age- and gender-matched control subjects were asked to enunciate /pa/ to reach a target frequency and intensity level in response to a light stimulus. The stimulus and acoustic responses were simultaneously recorded. Measures included laryngeal reaction time, time between stimulus and phonation onset; frequency voice target time, time from phonation onset to target level of frequency; and amplitude voice target time, time from phonation onset to target level of intensity. The 2 subject groups were significantly differentiated by laryngeal reaction time (t = 299.67, df = 10, P = 0.005) and frequency voice target time (t = 148, df = 10, P = 0.014). These data suggest voice target time is a viable tool for assessing the effects of neurologic disorders on voice execution in Parkinson's disease.

Voice and vocal self-assessment after thyroidectomy

BACKGROUND: Voice alterations after thyroidectomy can be found even with preserved function of laryngeal nerves. The purpose of this study was to evaluate voice before and after thyroid surgery and the role of orotracheal intubation on voice changes. METHODS: We conducted a prospective nonrandomized study of patients who underwent thyroid surgery and compared the results with a control group of patients who underwent breast surgery. Subjects underwent a videolaryngoscopic exam, a subjective and an objective voice analysis and a Voice Handicap Index (VHI) questionnaire before and after surgery. RESULTS: A total of 100 patients who underwent thyroidectomy and 30 who underwent breast surgery were studied. Both groups were similar in demographic, clinical, and surgical variables. Postoperative videolaryngoscopy showed larynx alterations in 28% of the thyroidectomized patients, without significant alterations in the control group. There were subjective voice changes in 29.7% of the patients without vocal fold immobility after thyroid surgery and no statistically significant changes after breast surgery. Acoustic analysis showed significant increased values in the voice turbulence index (VTI) parameter in both groups, although higher in the thyroid one. In the VHI assessment, voice complaints were more frequently registered in the thyroid group rather than in the control group. CONCLUSIONS: Voice alterations are frequent after thyroidectomy even with preserved vocal fold mobility. Such alterations were more frequently detected in that group than in patients who underwent breast surgery. Orotracheal intubation is just one of the multiple factors involved.

Sexual reassignment surgery for individuals with gender identity confusion has been highly successful, especially in the male-to-female transformation. A troublesome problem for some of these individuals is the retention of their male voice. Although castration and estrogen therapy may produce minor elevations of pitch and the assumption of a falsetto voice produces a more typical but contrived female tone, the basic male anatomic structure of the larynx must be altered to effect a physiological transformation. In three transsexuals, an attempt was made to effect a more female tonal quality by shortening the vibrating length of the vocal folds. Through a laryngofissure, the anterior one third of each cord was denuded and the raw edges approximated. This produced an anterior web that successfully resulted in pitch elevation in all three cases.
