尿液核基质蛋白22在膀胱移行上皮癌筛检中的价值   总被引:1,自引:0,他引:1  
目的:评价尿核基质蛋白22(NMP22)在膀胱移行上皮癌筛检中的意义。方法:采用酶联免疫法检测28例膀胱移行上皮癌、25例泌尿系良性疾病和10例泌尿系非移行上皮癌患者尿NMP22水平,并与尿脱落细胞学检查进行比较。结果:膀胱移行上皮癌患者尿NMP,,中位值为66.5u/ml,明显高于泌尿系良性疾病和非移行上皮癌,与肿瘤分期、分级、数目无关,以10u/ml为临界,诊断膀胱癌敏感性为85.7%,特异性为60%,尿脱落细胞学检查敏感性为32.1%,特异性为100%。结论:尿NMP22检测膀胱移行上皮癌敏感性、特异性均较高,可用于膀胱移行上皮癌的筛选和术后随访。  相似文献

目的:探讨LewisX抗原在膀胱尿路上皮癌非侵袭性诊断中的应用价值。方法:采用EnVision免疫细胞化学方法,检测52例膀胱尿路上皮癌和16例非肿瘤患者尿脱落细胞标本中LewisX抗原的表达情况,并与细胞病理学检测结果相比较。结果:尿路上皮癌诊断的敏感性和特异性分别为84.6%和87.5%,其敏感性显著高于细胞病理学。结论:尿脱落细胞LewisX抗原免疫染色,是检测膀胱尿路上皮癌可行的较敏感的非侵袭性方法。  相似文献

目的:评估细胞角蛋白20(CK20)标志物作为膀胱癌早期诊断及临床监测的价值。方法:对62例患者的尿液进行尿脱落细胞学及CK20标志物免疫荧光检测的前瞻性研究。分析参数包括肿瘤数目、大小及WHO分级,术前或活检前尿脱落细胞学和CK20标志物。结果:病理活检证实15例移行细胞癌中,CK20为13例阳性,2例阴性;47例非膀胱癌中,CK20标志物2例假阳性。与尿脱落细胞学比较,CK20标志物对膀胱移行细胞癌诊断的特异性和阳性预报值更高,分别为96.0%比82.5%(U=2.18,P<0.05),86.7%比52.4%(U=2.16,P<0.05);但两者在敏感性和阴性预报值间无显著性差异(U值分别为:0.91和1.02,P>0.05)。CK20表达与肿瘤分级间无明显相关性。结论:CK20是诊断膀胱移行细胞癌的一种良好标志物,其特异性明显优于尿脱落细胞学。  相似文献

 目的 探讨COX-2在膀胱癌组织中的表达,了解尿脱落细胞COX-2表达在膀胱癌早期诊断中的价值。方法 应用免疫组化技术检测48例膀胱移行细胞癌组织、免疫细胞化学技术检测40例膀胱移行细胞癌患者和30例非肿瘤患者尿脱落细胞COX-2的表达。结果 膀胱移行细胞癌组织COX-2阳性表达率为72.9%,对照组正常膀胱黏膜无表达。COX-2的表达与膀胱癌临床分期显著相关(P<0.05),不同病理分级膀胱癌的表达差别无显著性意义。非肿瘤患者尿脱落细胞无COx-2表达,膀胱癌尿脱落细胞COX-2免疫细胞化学检测阳性率为67.5%,明显高于常规尿细胞学的37.5%(P<0.05),尤其对于G1级和Ta~T1期的低级、早期肿瘤,尿脱落细胞COX-2免疫细胞化学检测与常规尿细胞学检查相比,具有显著性意义(P<0.05)。结论 COX-2在膀胱移行细胞癌的发生发展中起重要作用,与肿瘤的浸润、转移相关。尿脱落细胞COX-2表达检测特异性高,可作为早期诊断膀胱癌的一种标志物。  相似文献

目的:探讨尿细胞角蛋白检测与尿脱落细胞学检查在膀胱移行细胞癌诊断中的价值。方法:136例怀疑膀胱癌者,进行尿细胞角蛋白8和18的含量(UBC值)。检测与尿细胞学检查,其中87例经组织学证实为膀胱移行细胞癌。比较两者诊断膀胱癌的敏感性和特异性。结果:尿细胞角蛋白的敏感性为70.1%,特异性为73.3%;尿细胞学的敏感性为42.5%,特异性为83.7%。尿细胞角蛋白在膀胱癌不同分级和分期中的敏感性优于尿细胞学(P<0.05)。结论:尿细胞角蛋白的检测在早期诊断膀胱癌方面优于尿细胞学检查,可作为膀胱癌的早期检测指标。  相似文献

目的:探讨检测端粒酶活性在膀胱癌早期诊断中的临床意义。方法:采用TRAP-PCR-ELISA法检测32例膀胱癌患者尿液和膀胱冲洗液中脱落细胞、膀胱癌组织、20例正常膀胱组织及14例非膀胱肿瘤患者尿液脱落细胞中端粒酶活性,并行尿脱落细胞学检查。结果:32例膀胱癌患者尿液脱落细胞、膀胱冲洗液脱落细胞、膀胱癌组织中端粒酶活性阳性率分别为65.6%(21/32)、71.9%(23/32)和84.0%(27/32),20例正常膀胱组织端粒酶活性均为阴性,14例非膀胱肿瘤患者尿液中1例端粒酶活性阳性。端粒酶活性阳性表达与肿瘤的分级、分期之间差异无明显相关性(P〉0.05),敏感性明显高于脱落细胞病理学检查。结论:尿液、膀胱冲洗液脱落细胞端粒酶活性测定敏感性较高.可用于膀胱癌的早期诊断。  相似文献

目的:探讨通过检测尿脱落细胞端粒酶活性早期诊断膀胱癌。方法:采用改进的银染-TRAP(端粒重复序列扩增)法,分别对45例膀胱移行细胞癌患者,10例膀胱良性病变患者及8例正常人尿脱落细胞进行了端粒酶活性检测。结果:24例G1级(分化良好)的膀胱癌患者,端粒酶活性强阳性12例,弱阳性6例(阳性率75.0%),21例G2,G3级膀胱癌患者,强阳性16例,弱阳性3例(阳性率94.5%),膀胱良性病变患者及正常人尿脱落细胞只有1例端粒酶活性呈弱阳性,其余全部阴性。结论:尿脱落细胞端粒酶活性检测特异性强,敏感性高,与病理分类,组织分级等无明显关系,提示有可能成为膀胱癌临床早期诊断指标之一。  相似文献

BACKGROUND: Mutations in the p53 tumor suppressor gene may correlate with an increased risk of recurrence and disease progression in patients with bladder carcinoma. The ability to accurately and sensitively detect p53 mutations in cytology specimens may be of benefit in the treatment of bladder carcinoma patients with superficial, minimally invasive disease. METHODS: Genomic DNA was isolated from 49 cases, each of which was comprised of matched bladder tumor tissue, bladder wash, and voided urine specimens obtained concurrently at a single institution. The genomic DNA was analyzed for mutations in the p53 tumor suppressor gene using a p53 mutation detection assay. Automated dideoxy sequencing of mutant specimens also was performed. RESULTS: Of the 49 cases, 29 (59%) showed no evidence of p53 mutations in the tumor, bladder wash, or voided urine specimens. Of the remaining 20 cases, 19 showed evidence of mutations in the tumor. Of these 19 p53 mutant bladder tumors, 16 (84%) were detected in the matched bladder wash and 16 (84%) were detected in the matched voided urine specimens. One case resulted in the detection of mutant p53 in the voided urine and the bladder wash, but not in the tumor. Analysis of the results between tumor tissue and bladder wash or tumor and voided urine showed 84.2% sensitivity, 96.8% specificity, and 91.8% accuracy. Sequence analysis of the mutant cases showed that the mutations detected in the tumor tissue were the same mutations detected in the bladder wash and the voided urine specimens. CONCLUSIONS: Both voided urine and bladder wash specimens from patients with bladder carcinoma were found to provide a high rate of clinical accuracy for the determination of the p53 gene status in patients with bladder tumors.  相似文献

目的应用荧光原位杂交((fluorescence in situ hybridization,FISH)技术检测膀胱癌患者尿液脱落细胞中染色体异常,评估FISH在中国人群中诊断膀胱癌的作用。方法2007年1月至2008年8月,随机留取20例良性前列腺增生症患者的新鲜尿液,用3号和7号、17号及p16位两组混合探针,通过在尿液脱落细胞标本上进行FISH检测,建立正常人群的阈值;其后随机留取30例门诊膀胱镜活检证实的膀胱癌患者的尿液,同时进行尿液脱落细胞的细胞形态学分析及FISH检测,对比检查结果。结果3号、7号和17号染色体非整倍性改变及p16位点异常正常阈值分别为8.5%、7.1%、6.8%和9.2%,FISH与细胞学检查总敏感性分别为76.6%和43.3%(P<0.05)。T_(is)及T_a、T_1患者FISH检测的敏感性分别为80.0%和64.2%,脱落细胞组织学检测显示敏感性分别为40.0%和35.7%;T_(2-3)患者FISH的敏感性为90.9%,而脱落细胞组织学检测为54.7%(P<0.05),低级别尿路上皮癌FISH及细胞学敏感性分别为68.4%和31.6%;高级别分别为90.9%和63.6%。结论与尿液脱落细胞组织学检测相比,对尿液脱落细胞进行FISH检测可以提高膀胱癌的诊断率,FISH可以作为诊断膀胱癌的一种无创伤的新方法。  相似文献

The expression of variant isoforms of the adhesion molecule CD44 is correlated with the onset of neoplasia in many carcinomas. We have previously shown that noninvasive detection of bladder carcinoma is possible by analysis of anomalous CD44 expression in exfoliated urothelia. Although the sensitivity and specificity values obtained for the detection of bladder tumors using RT-PCR and Western blotting methods were superior to those obtained using urine cytology, the application of such techniques is inconvenient for routine diagnostic use. We now report the design and development of a sandwich-ELISA system for the reliable detection of CD44 protein extracted from sedimented urothelial cells in voided urine. Naturally micturated urine samples were obtained from 53 patients with newly diagnosed bladder cancer and from 65 subjects with no evidence of disease; patients with gross hematuria were excluded because of interference with the assay. To demonstrate the diagnostic potential of the system, a "gate" was imposed at N (max), i.e., the highest absorbance value obtained from a sample known to be tumor free. All values above this value were assumed to be indicative of the presence of a tumor. Using this parameter, 42 of 53 (81.1%) patients with histologically confirmed bladder tumors were correctly diagnosed. Correspondingly, under these conditions, the assay is 100% specific for tumor detection, with a sensitivity of 81.1%, which equates to a positive predictive value of 100% and a negative predictive value of 81.1%. A further 54 patients who had previously received treatment for bladder cancer but were currently clinically disease-free were also investigated. Of these, 47 of 54 (87%) were correctly diagnosed to be tumor-free, which in this group equates to a positive predictive value of 87% and a negative predictive value of 100%. The data presented demonstrate that the rapid and accurate detection of elevated levels of CD44 protein isoforms in exfoliated urothelial cells is applicable to the identification and monitoring of primary and recurrent bladder cancer.  相似文献

BACKGROUND: Cystoscopy and histologic examination remain the standard methods for initial tumor diagnosis and monitoring for early detection of recurrences, since the sensitivity of conventional urinary cytology for the detection of urothelial tumors in urinary specimens is low. DNA image cytometry (ICM) and fluorescence in situ hybridization (FISH) have been suggested as ancillary tools. The goal of the current study was to compare the diagnostic value of DNA image cytometry and FISH for the noninvasive detection of urothelial tumors in voided urine. METHODS: Cytospin preparations were prepared from voided urine collected prior to the resection of 26 noninvasive (pTa) and 11 invasive (pT1-2) tumors. Specimens from 14 patients with benign prostatic hyperplasia were used as negative controls. DNA ICM was performed using the AUTOCYTE trade mark cell analytical system on Feulgen-stained cytospin specimens. The commercially available UroVysion trade mark FISH multiprobe was used to analyze chromosomes 3, 7, and 17, and 9p21. RESULTS: The overall sensitivity of cytology improved from 24% to 54% and to 78% if supplemented by ICM or FISH, respectively. Image cytometry detected all invasive tumors (pT1-2), while FISH missed one; FISH identified 19 of 26 (73%) pTa tumors, while only 9 (35%) of these tumors were aneuploid by ICM. The results of ICM and FISH were concordant in 37 of 51 (72%) cases. CONCLUSIONS: The current study shows that both FISH and ICM can successfully be used as supplementary methods to detect the clinically most relevant group of invasive bladder carcinomas. However, UroVysion FISH is more sensitive in the detection of pTa tumors than ICM, as it recognizes individual chromosomal alterations that frequently prevail in urothelial tumors.  相似文献

BACKGROUND: Urine cytology plays an important role in monitoring patients with a history of urothelial carcinoma. Because it is difficult to reliably discriminate artifacts induced by instrumentation, inflammation, or therapy those of from malignant cells, many of these specimens are categorized as atypical. The objective of the current study was to study the prevalence and significance of atypical urine cytology with regard to the effect of instrumentation and prior biopsy. METHODS: All urine cytology cases seen during a 4-year period (2001-2004) with a diagnosis of atypical urothelial cells (AU) were obtained from the cytopathology computer database. In all cases with available surgical follow-up, the following data were extracted: total number and type of urine specimen, the primary histologic diagnosis, and follow-up histologic diagnosis. RESULTS: In all, 1653 voided and 3502 instrumented urine specimens were examined. A diagnosis of AU was rendered in 115 (6.9%) of the voided urine specimens and in 277 (7.9%) of the instrumented specimens. Follow-up histology was available in 70 cases, including 55 instrumented and 15 voided urine specimens. A nonbenign follow-up diagnosis was observed in 18 of 55 (32.7%) cases in the instrumented group and in 7 of 15 (46.6%) cases in the voided group. Voided urine was marginally associated with a worse subsequent biopsy diagnosis (Pexact Monte Carlo = .09) CONCLUSIONS: An AU diagnosis is more predictive of a subsequent adverse biopsy diagnosis in voided urine specimens compared with instrumented urines. In the absence of a benchmark for the atypia rate, it is prudent to keep the atypia rate low to keep it more meaningful. This important category should be used by the pathologist to convey concern and recognize the difficulty in interpretation of specimens that may require close follow-up.  相似文献

BACKGROUND:

ImmunoCyt (uCyt) and UroVysion are ancillary studies that may aid in the detection of urothelial carcinoma in urine specimens. We compared ImmunoCyt and UroVysion to urine cytology in the ability to detect recurrent urothelial carcinoma.

METHODS:

Single voided urine samples were obtained from 100 patients who had a previous history of bladder cancer. All patients underwent cystoscopy immediately after urine sample collection. Forty‐one cystoscopically suspicious lesions were biopsied. Urine samples were divided and processed blindly and independently in 3 different laboratories for ImmunoCyt, UroVysion, and urine cytology (ThinPrep method).

RESULTS:

Of the 41 cystoscopically positive cases, most cystoscopy findings showed multiple tumors that were papillary and less than 1 cm. Biopsies showed many low‐grade tumors (54%). Overall sensitivity of cytology for low‐ and high‐grade urothelial cell carcinoma was 15% and 27%, whereas ImmunoCyt was 62% and 91% and UroVysion was 8% and 18%, respectively. Overall specificity of cytology was 97%, whereas ImmunoCyt was 63% and UroVysion was 90%.

CONCLUSIONS:

ImmunoCyt is more sensitive than either cytology or UroVysion in detecting low‐grade tumors. Both cytology and UroVysion have comparable specificity in cystoscopically negative cases. Whereas ImmunoCyt may improve the cytological detection of recurrent bladder cancer, UroVysion may be used as a confirmatory test for either cytology or ImmunoCyt. Urinary cells obtained from voided urine specimens of 46 patients with urothelial carcinomas (UCs) and 10normal individuals were analyzed with 3 different centromeric fluorescence in situ hybridization (FISH) probes.The overall sensitivity of cytology was 48.9% compared to 95.7%with the FISH technique. The minimumvalues were found for stage Ta and grade 1 (90.5 and 89.4) and sensitivity of FISH in other stages and gradeswas 100%. Chromosome 3 demonstrated the most frequent chromosomal abnormality in all samples (43%),followed by chromosome 17 (32%) and chromosome 7 (25%). There was a statistically significant associationbetween the number of cell abnormalities in chromosome 17 and the tumour stage (p value=0.02). No relationshipwas found between the type of chromosomal abnormality and grade. Thus feasibility and reliability of a FISHbased approach was confirmed for detection of UC in urine samples.  相似文献
