Effect of Yiqi Huoxue Recipe (益气活血方) on cardiac function and ultrastructure in regression of pressure overload-induced myocardial hypertrophy in rats

Objective:To investigate the effect of Yiqi Huoxue Recipe (YHR,益气活血方)on the cardiac function and ultrastructure during the regression of myocardial hypertrophy induced by pressure overload in rats.Methods:The model of myocardial hypertrophy was established by abdominal aortic banding.Eighty male Wistar rats were divided into six groups,the normal control groupⅠ(n=20),the normal control groupⅡ(n=12),the hypertension model groupⅠ(n=12),the hypertension model groupⅡ(n=12),the YHR group(n=12)and the Captopril group(n=12).The observation was carried out in the normal control groupⅠand the hypertension model groupⅠafter 4 weeks of modeling,and the other four groups were observed after 16 weeks of modeling(12 weeks of administration).The cardiac function was measured with a multichannel biological signal analysis system,and the myocardium ultrastructure was observed by a transmission electron microscope. Results:(1)Compared with the normal control groupⅠ,the systolic blood pressure and cardiac coefficient(left ventricular weight/body weight)in the modelⅠgroup was higher(P<0.05,P<0.01). (2)In the YHR group,cardiac coefficient and-dp/dt_(max)were lower,left ventricular systolic pressure and dp/dt_(min)were higher when compared with the model groupⅡand the Captopril group(P<0.05 or P<0.01).In the Captopril group,only cardiac coefficient was lower when compared with the mode groupⅡ(P<0.05).(3)Compared with the normal control groupⅡ, dp/dt_(max)was higher(P<0.01), -dp/dt_(max)and isovolumetric contraction time(ICT)was lower(P<0.05,P<0.01)in both the YHR group and the Captopril group.(4)Results of the myocardium ultrastructure showed edema under myocardium plasmalemma,enlarged sarcoplasmic reticulum and T tube,and significantly enlarged intercalated disc of the cardiac muscle in the model groups.In the Captopril group,the extension of sarcoplasmic reticulum and T tube as well as the pathological changes of intercalated disc were lighter,with slight edema under the myocardium plasmalemma.In the YHR group,the expansion of the sarcoplasmic reticulum was less than in the Captopril group,part of the pathological changes of intercalated discs was slightly more severe than that in the Captopril group,the dissolution of nuclear chromatin was not found,which was similar to that of the Captopril group,and no injury of the nucleus was found,either.Conclusion:YHR could reverse myocardial hypertrophy in rats with abdominal aortic banding and improve the systolic and diastolic function of the left ventricle.The ultrastructure of the myocardium such as arcoplasmic reticulum,intercalated disc,and cell nucleus in abdominal aortic banding rats could be partly reversed by the recipe.

Changes of Intestinal Microecology in Patients with Primary Sjogren's Syndrome after Therapy of Yangyin Yiqi Huoxue Recipe(养阴益气活血方)

Objective: To explore the change of intestinal microecology in patients with primary Sjogren's syndrome(p SS) and correlation with disease activity, and also discuss the therapy effect of Yangyin Yiqi Huoxue Recipe(养阴益气活血方, YYHD). Methods: Sixteen p SS patients were enrolled in the present study, who received 3-month treatment of YYHR, 200 mL orally twice daily. Their pre-and post-test ESSDAI scores, erythrocyte sedimentation rate(ESR) and serum immunoglobulin G(IgG) levels were measured respectively. The 16 Sr DNA metagenomic sequencing was used to detect and analyze the abundance and diversity of intestinal bacteria flora and the proportion of bacteria at the levels of phylum, family, and genus, in comparision with those of 6 healthy subjects in the control group. Results: The abundance and diversity of intestinal bacteria flora in pSS patients were lower than those of healthy subjects(P0.05). After the treatment with YYHD, patients' ESSDAI score and levels of IgG and ESR have decreased significantly(P0.05). At the phylum level, the proportions of Actinobacteria, Firmicutes, Fusobacteria and Proteobacteria have reduced sharply, while the proportions of Bacteroidetes, Teneriquetes and Candidate-division-TM7 have increased significantly by treatment(all P0.05). At the classification level, such treatment has caused a significant decrease in the proportions of Bacteroidaceae, Ruminococcaceae, Veillonellaceae, and Enterobacteriacea(all P0.05), but a significant increase in the proportion of Lachnospiraceae(P0.05). At the genus level, the treatment has significantly decreased the proportions of Bifidobacterium, Bacteroides, Escherichia-Shigella, Faecalibacterium and Prevotella(all P0.05), but significantly increased the proportion of Clostridia(P0.05), close to the levels of healthy subjects(P0.05). Conclusions: There exists an imbalance of intestinal microecology in p SS patients, which can be improved through the treatment with YYHD. Besides, such treatment can also improve the disease activity and adjust the diversity of intestinal bacteria flora, the composition and the abundance of intestinal flora.

Effect of Yiqi Huoxue（益气活血） Herbs on Vascular Endothelial Cells and Platelets in Patients with Chronic Cor Pulmonale

Objective: To observe the clinical effect of Yiqi Huoxue (益气活血, YQHX) herbs in treating the patients with chronic cor pulmonale and to explore its mechanism by determining the relationship of oxidation/antioxidation system and how such herbs change on the function of endothelial cells and platelets. Methods: Fifty-eight patients were divided into two groups: conventional therapy group (control group, 28 paventional management. The treated group were treated with YQHX 150 ml, twice a day, plus the conventional treatment, and the clinical efficacy was recorded. The lipid peroxidation (LPO), superoxide dismutase (SOD), α-granule membrane protein (GMP140), partial pressure of arterial oxygen (PaO2), partial pressure of carbon dioxide in artery (PaCO2) and circulating endothelial cells (CEC) were measured respectively before and after treatment, and the relationship between various parameters were analyzed. The results were compared with those of 10 healthy subjects got at the same period. Results: (1) The effective rate and PaO2 of the treated group was higher than that of the control group and there were no difference in PaCO2 between the two groups. (2) The levels of LPO, GMP140, CEC in all the patients before therapy were significantly higher than those of the healthy group, and there were marked decrease in the levels of those after treatment (all P＜0. 01). On the contrary, the levels of SOD in all the patients before therapy were markedly lower than those in the healthy subjects and increased after treatment, P＜0.01. (3) The increase of SOD in the treated group was significantly more obvious than that of the control group. In the treated group, the decrease of LPO, GMP140, CEC were markedly more obvious than those in the control group (all P＜0.01).(4) The number of CEC, as well as GMP140, was negatively correlated to PaO2 (P＜0.01) and SOD (P＜0.01), which was positively correlated to LPO (P＜0.01). There was a positive correlation between CEC and GMP140 (P＜0.05). Conclusion: YQHX herbs in treating chronic cor pulmonale proved to be effective by balancing the oxidation and antioxidation, protecting the pulmonary endothelial cells and activated platelets and helpful in treating respiratory failure.     

Effect of Bushen Yiqi Hexue Recipe (补肾益气和血方) on Pinopodes Expression on Endometrial Surface in Embryo Implantation Dysfunctional Mice

Objective: To explore the therapeutic mechanism of Bushen Yiqi Hexue recipe (补肾益气和embryo implantation dysfunctional mice. Methods: Pregnant Kunming mice were randomly divided into the normal group, the control group and the treated group. Mice in the treated group were administered with BYHR, and those in the normal group and the control group were administered with normal saline, starting from the very first day of pregnancy (Pdl). On the Pd4, mifepristone (RU486) was subcutaneously injected into mice in the control group and the treated group. Pinopodes expression on endometrial surface at 09:30-10:00 pm on Pd4 (regarded as time point 1, T1) and at 09:30-10:00 am on Pd5 (as time point 2, T2) was determined by scanning electron microscopy. Pregnancy rate and average implanted embryos were observed on Pd7. Results: Pregnancy rate and average implanted embryos in the control group were obviously lower than those in the normal group (all P＜0.01), while the two parameters were significantly higher in the treated group ( P＜0.05, P＜0.01, respectively) than those in the control group. In the normal group, abundant developing pinopodes were distributed over the whole endometrial surface in T1, and they were altered to a great deal of fully developed pinopodes in T2. But in the control group, only a few pinopodes were expressed locally on endometrial surface in T1, showing a nonsynchronous figure of development, and they completely disappeared in T2. In the treated group, a lot of developing pinopodes were expressed like those in the normal group but somewhat lagged behind, whereas many fully developed pinopodes were expressed in T2.Conclusion: The decrease of the pinopodes in T1 and the cleaning up of them in T2 is possibly the partial mechanism of mifepristone in inducing embryo implantation dysfunction. It is indicated that BYHR could improve pinopodes expression on endometrial surface, eventually better the uterine receptivity and improve the embryo implantation.

Clinical Observation on Effect of Xuesaitong Injection (血塞通注射液) as Auxiliary Treatment of Severe Craniocerebral Injury

Objective To evaluate the effect of Xuesai-tong injection (XSTI, 血塞通注射液 , a preparation of Panax Notoginseng) as auxilliary treatment of severe craniocerebral injury. Methods Eighty-seven patients with severe craniocerebral injury were selected and randomly divided into the treated group (n=44) and the control group (n = 43), they were treated with conventional treatment, and XSTI was given additionally to the treated group. Intracranial pressure (ICP) and Glasgow coma score (GCS) of all patients were measuredafter 1 or 2-week treatment, and Glasgow outcome scale (GOS) of them was determined 3 months later. Then the therapeutic effect in the two groups were compared. Results After treatment, the ICP was lower, GCS higher and GOS better in the treated group than those in the control group significantly, all showing statistical significance (P<0.05). Clinical Observation on Acupuncture Treatment in 35 Cases of Diabetic Gastroparesis

In order to observe the therapeutic effects of acupuncture on diabetic gastroparesis, 80 cases were dividedrandomly into 3 groups. In addition to the routine treatment for diabetes, 35 cases in the treatment groupwere additionally treated with acupuncture, and 25 cases in the control group 1 with domperidone, while20 cases in the control group 2 with nothing. The three groups were all observed for 25 days. The resultsturned out to be that the total effective rate in the treatment group reached 94.2%, that in the control group1, 72%, and in the control group 2, 40%, the treatment group showing a better therapeutic effect than theother two control groups (P<0.05, P<0.01). It can be concluded that in addition to the routine treatment forcontrolling blood sugar, acupuncture can give quite good therapeutic effects on diabetic gastroparesis.

We hypothesize that irbesartan, an angiotensin [I receptor antagonist, can be used to treat VVS with no limitation especially in small dosage.     

Clinical Observation on Treatment of Acute Gouty Arthritis byTongfengkang(痛风康)

Objective To objectively evaluate the clinical therapeutic effect of Tongfengkang (TFK) in treating acute gouty arthritis. Methods: Adopting randomized single blinded controlled trial, the 40 patients were equally divided into two groups. The tested group was treated with TFK, the control group was treated with indomethacin and allopurinol, the therapeutic course for both groups was 10 days. Results: The clinical cure rate in the tested group and the control group was 30% and 35% respectively, and the total effective rate 90% and 95% respectively, with no significant difference between the two groups (P>0.05).The scores of blood uric acid and symptom significantly lowered in both groups after treatment (P <0.01), but showed no significant difference between them (P>0.05).
