We describe two patients who presented with a history of recurrent palpitations on swallowing of solid food. The event-recorder and Holter monitoring documented episodic supraventricular tachycardia (SVT) initiated by atrial premature contractions (APCs). During electrophysiological study (EPS), swallowing of solid food consistently induced APCs and their activation sequence, morphology of P wave were suggestive of their right atrial origin in them. Drug challenge did not affect the APC onset during the swallowing. During EPS, slow-fast variety of atrioventricular nodal reentrant tachycardia (AVNRT) was induced and successful radiofrequency (RF) catheter ablation of slow pathway resulted in total relief of their symptoms.  相似文献

This case report describes pacemaker-like syndrome after successful slow pathway ablation for atrioventricular (AV) nodal reentrant tachycardia due to recurrence of single AV nodal echo beats during sinus rhythm. The resultant AV dyssynchrony was responsible for the symptom complex. Following ablation of retrograde ventriculoatrial conduction, the AV nodal echo beats were eliminated and the pacemaker-like syndrome resolved.  相似文献

A 29-year-old woman was referred for electrophysiological testing and radiofrequency ablation because of repeated episodes of palpitation of a 8-year duration. The 12-lead ECG during palpitations showed narrow QRS tachycardia at a rate of 160 beats/min. Dual AVN physiology according to electrophysiological criteria was not shown by single atrial extrastimulation and the tachycardia could not be induced. Slow/fast atrioventricular nodal reentrant tachycardia (AVNRT) was induced once by double atrial extrastimuli, but it was not reproducible. However, intravenous bolus injection of adenosine triphosphate (12.5 mg) during sinus rhythm led to reproducible initiation of slow/fast AVNRT.  相似文献

Background: Atypical atrioventricular (AV) nodal reentrant tachycardias (AVNRT) usually exhibit the earliest retrograde atrial activation (ERAA) at the right inferoseptum (Rt-IS) or proximal coronary sinus (PCS). The purpose of this study was to characterize atypical AVNRT with the ERAA at the right superoseptum (Rt-SS).
Methods: Seventy-three atypical AVNRTs induced in 63 cases were classified into the superior type with the ERAA at the Rt-SS and inferior type with the ERAA at the Rt-IS or PCS.
Results: There were nine superior (12%) and 64 inferior types of atypical AVNRT (88%) in seven and 56 cases, respectively. The superior type exhibited a short atrial-His interval during the tachycardia (166 ± 41 ms), long His-atrial interval during the tachycardia (H-At:156 ± 38 ms), and ventricular pacing at the tachycardia cycle length (TCL) (H-Ap:201 ± 36 ms), and evidence for a lower common pathway, including second-degree AV block (four tachycardias) and an H-Ap being longer than the H-At (nine tachycardias). The TCL was shorter in the superior type than in the inferior type (322 ± 35 vs 404 ± 110 ms; P < 0.02). In the inferior type, all tachycardias were eliminated after the ablation at the Rt-IS (44 tachycardias) or PCS (20 tachycardias) where an ERAA was recorded. In the superior type, ablation at the Rt-IS was ineffective; however, ablation at the right midseptum eliminated seven (78%) of the nine tachycardias.
Conclusions: The tachycardia circuit of the superior type might have deviated to a more superior part of Koch's triangle than that of the inferior type.  相似文献

目的探讨射频消融慢径路治疗阵发性房室结折返性心动过速(AVNRT)的临床效果。方法回顾性分析425例采用射频消融治疗的AVNRT患者的临床资料。结果417例手术成功,成功率98.12%。4例(0.94%)出现高度房宣传导阻滞,2例(0.47%)因心室率较慢而安装永久性起搏器。随访6个月内有8例复发,复发率1.9%。结论选择性慢径路消融是成功治愈AVNRT安全有效的方法。  相似文献

Narrow QRS tachycardia with atrial activation occurring before ventricular activation was induced in a 34-year-old woman with dilated cardiomyopathy. During tachycardia late ventricular extrastimulus delivered when His bundle was refractory failed to reset the tachycardia while early ventricular extrastimulus caused paradoxical delay of the subsequent atrial response and terminated the tachycardia with a QRS not being followed by an atrial response. This is a rare but specific sign for excluding atrial reentry as the mechanism of tachycardia when P wave or atrial activation is registered before QRS response.  相似文献

The determinants of slow pathway conduction in patients with AV nodal reentrant tachycardia (AVNRT) are still unknown, and great differences in the AH interval during slow pathway conduction are observed between patients. In 35 patients with typical AVNRT who underwent successful slow pathway ablation (defined as complete elimination of dual pathway physiology), the A2H2 interval at the "jump" during programmed atrial stimulation and the AH interval during AVNRT (as a reflection of slow pathway conduction time) and the fluoroscopic distance between the successful ablation site and the His-bundle recording site and between the coronary sinus ostium (CSO) and the His-bundle recording site were determined. The mean (+/- SEM) AH interval during slow pathway conduction was 323 +/- 12 ms with programmed stimulation and 310 +/- 10 ms during AVNRT. The mean number of energy applications was 8 +/- 1 (range 1-21). The mean distances between (1) the successful ablation site and the His bundle recording site and (2) between the CSO and the His-bundle recording site were 24 +/- 1 and 28 +/- 1 mm in the RAO and 23 +/- 1 and 28 +/- 1 mm in the LAO projections, respectively. The AH interval during slow pathway conduction correlated significantly with the distance between the successful ablation site and the His-bundle (P < 0.001) but not with the distance between CSO and His-bundle recording site. There is a significant correlation between the AH interval during slow pathway conduction and the distance of the successful ablation site from the His bundle. This relationship (1) suggests that, in addition to functional factors, anatomic factors influence slow pathway conduction and (2) may be helpful in determining the initial energy application site during slow pathway ablation.  相似文献

An association between atrial flutter and atrioventricular nodal reentrant tachycardia (AVNRT) has been observed, but the underlying mechanisms are poorly defined. This issue was therefore investigated by comparing the electrophysiological properties of AVNRT patients with and without inducible atrial flutter and those of patients with a history of flutter. Twenty-nine patients with clinically documented atrial flutter and 104 with AVNRT were studied. Atrial flutter was induced in 38 (37%) AVNRT patients during standardized electrophysiological testing before radiofrequency ablation. The atrial relative refractory periods in AVNRT patients with inducible flutter (260 +/- 30 ms) were significantly shorter than those of either patients with a history of flutter (282 +/- 30 ms; P = 0.02) or AVNRT patients without inducible flutter (284 +/- 38 ms; P = 0.006). The atrial effective refractory periods in AVNRT patients with inducible flutter (205 +/- 31 ms) were shorter than in AVNRT patients without inducible flutter (227 +/- 40 ms; P = 0.01). The maximum AH interval during premature atrial stimulation in patients with clinical flutter (239 +/- 94 ms) was shorter than in AVNRT patients either with (290 +/- 91 ms; P = 0.04) or without inducible flutter (313 +/- 101 ms; P = 0.002). However, no significant differences were found in the maximum AH interval achieved during incremental atrial pacing among different groups. Our data show that a non-clinical flutter could more often be induced in those who had short atrial refractoriness. Despite their anatomical proximity, the slow pathway conduction of AVNRT and the isthmus slow conduction of flutter may be related to different mechanisms.  相似文献

A 32-week, premature neonate with incessant supraventricular tachycardia and hemodynamic compromise who failed to respond to antenatal and postnatal antiarrhythmic therapy underwent successful radiofrequency catheter ablation (RCA) of a concealed left free-wall accessory pathway when the infant was 4-days-old and weighed only 1,840 grams. At follow-up performed 60 days after the procedure, the infant remained free of any drug, in sinus rhythm, and in normal hemodynamic condition.  相似文献

BACKGROUND: Previous studies in adults have shown a significant shortening of the fast pathway effective refractory period (ERP) after successful slow pathway ablation. However, information on atrioventricular nodal reentrant tachycardia (AVNRT) in children is limited. The purpose of this retrospective study was to investigate the different effects of radiofrequency (RF) catheter ablation in pediatric AVNRT patients between those with and without dual atrioventricular (AV) nodal pathways. METHODS: From January 1992 to August 2004, a total 67 pediatric patients with AVNRT underwent an electrophysiologic study and RF catheter ablation at our institution. We compared the electrophysiologic characteristics between those obtained before and after ablation in the children with AVNRT with and without dual AV nodal pathways. RESULTS: Dual AV nodal pathways were found in 37 (55%) of 67 children, including 36 (54%) with antegrade and 10 (15%) with retrograde dual AV nodal pathways. The antegrade and retrograde fast pathway ERPs in children with dual AV nodal pathways were both longer than the antegrade and retrograde ERPs in children without dual AV nodal pathways (300 +/- 68 vs 264 +/- 58 ms, P = 0.004; 415 +/- 70 vs 250 +/- 45 ms, P < 0.001) before ablation. In children with antegrade dual AV nodal pathways, the antegrade fast pathway ERP decreased from 300 +/- 68 ms to 258 +/- 62 ms (P = 0.008). The retrograde fast pathway ERP also decreased after successful ablation in the children with retrograde dual AV nodal pathways (415 +/- 70 vs. 358 +/- 72 ms, P = 0.026). CONCLUSION: The dual AV nodal physiology could not be commonly demonstrated in pediatric patients with inducible AVNRT. After a successful slow pathway ablation, the fast pathway ERP shortened significantly in the children with dual AV nodal pathways.  相似文献

BACKGROUNDS: Radiofrequency (RF) catheter ablation represents a major advance in the management of children with cardiac arrhythmias and has rapidly become the standard of care for the first-ling therapy of supraventricular tachycardias (SVTs). The purpose of this study was to investigate the results of the RF catheter ablation of SVTs in pediatric patients. METHODS: From December 1989 to August 2005, a total of 228 pediatric patients (age: 9 +/- 7 years, range: 5-18 years; male:female = 117:111) with clinically documented SVT underwent an electrophysiologic study and RF catheter ablation at our institution. RESULTS: The arrhythmias included atrioventricular reentrant tachycardia (AVRT; n = 140, 61%), atrioventricular nodal reentrant tachycardia (AVNRT; n = 66, 29%), atrial tachycardia (AT; n = 11, 5%), and atrial flutter (AFL; n = 11, 5%). The success rate of the RF catheter ablation was 92% for AVRT, 97% for AVNRT, 82% for AT, and 91% for AFL, respectively. Procedure-related complications were infrequent (8.7%; major complications: high grade AV block (2/231, 0.9%); minor complications: first degree AV block (6/231, 2.6%), reversible brachial plexus injury (2/231, 0.9%), and local hematomas or bruises (10/231, 4.3%)). The recurrence rate was 4.7% (10/212) during a follow-up period of 86 +/- 38 months (0.5-185 months). CONCLUSIONS: The RF catheter ablation was a safe and effective method to manage children with paroxysmal and incessant tachycardia. The substrates of the arrhythmias differed between the pediatric and adult patients. However, the success rate of the ablation, complications, and recurrence during childhood were similar to those of adults.  相似文献

The acute and chronic effects of selective AV nodal artery ethanol infusion on AV nodal function was studied in 9 closed chest anesthetized dogs. Using standard percutaneous techniques of arterial catheterization, a 2.2 French infusion catheter was positioned in the AV nodal artery. Ten minute infusions into the AV nodal artery of 25%, 50%, or 100% ethanol in normal saline at rates of 0.5 mL/min acutely resulted in complete AV nodal block (AVB) in 5 dogs, 2:1 AV nodal block in 1 dog, and prolongation of AV nodal effective refractory period and/or Wenckebach cycle length in the remaining 3 dogs. One dog died with persistent complete AV block 1 week after the ethanol infusion. When restudied 4 weeks later, 7 of the 8 surviving dogs had persistent modification of AV nodal function, including complete AV block in 5 dogs and lengthening of AV nodal effective refractory period and/or Wenckebach cycle length without AV block in 2 dogs. Pathologic examination of the animals exhibiting chronic modification or ablation of AV nodal function revealed healing infarction of the AV node or its approaches. Distant myocardial necrosis was not observed and left ventricular function was normal. Slow infusion of low concentrations of ethanol into the AV nodal artery results in AV nodal modification or ablation due to localized necrosis in or around the AV node. This technique may have a role in AV nodal modification or ablation, particularly in patients who have failed DC shock or radiofrequency ablation.  相似文献

HAISAGUERRE, M., ET AL.: Fulguration for AV Nodal Tachycardia: Results in 42 Patients with a Mean Follow-Up of 23 Months. This report describes a catheter ablation technique to treat atrioventricular nodal reentrant tachycardia while preserving anterograde conduction, and its application in 42 patients with drug-refractory repetitive episodes of tachycardia. One of these patients had common and reverse forms of tachycardia. Using atrial activation in the His-bundle lead as a reference, the optimal ablation site was selected by positioning an electrode catheter to obtain a synchronous or earlier atrial activation than the reference during tachycardia. At this site, His-bundle deflection was completely absent, or was present at a low amplitude (< 0.1 mV). In the majority of patients, these criteria were found in the immediate vicinity of the site of proximal His-bundle recording (adjacent to the reference catheter). Shocks of 160 or 240 joules (J) were delivered at this site (mean ± SD = 518 ± 392 J/session) with a resulting preferential abolition of impairment of fast retrograde conduction. Anterograde conduction, though modified, was preserved in all patients, except for four (10%) patients who remained in complete heart block. Thirty patients (70%) remained free of arrhythmia without medication or pacemaker for a mean follow-up period of 23 ± 13 (2–63) months. Six other patients (15%) were controlled with a previously ineffective medication.  相似文献

We describe a case of ablation of atrioventricular nodal reentrant tachycardia in a patient with tricuspid atresia and L-malposition of great vessels using an electroanatomical mapping system integrated with cardiac magnetic resonance imaging. Atrial activation mapping during tachycardia identified the retrograde fast pathway proximal to the His bundle, observed in the left interatrial septum. Ablation was successfully completed below this area. Map integration with the patient's anatomy allowed a safe, individualized procedure.  相似文献

Clinically important supraventricular arrhythmias are occasionally encountered in patients following cardiac transplantation and the use of catheter ablation as a treatment has been reported. The following three cases are described: (1) atrial flutter, including electroanatomic mapping of the donor and recipient components of the right atrium, (2) a mid-septal accessory pathway, and (3) atrioventricular nodal reentrant tachycardia (AVNRT). A Medline database search was performed and articles addressing catheter ablation following cardiac transplantation were reviewed. The efficacy of RFA for treating various arrhythmia mechanisms was evaluated based on a summary of published case reports.  相似文献

Background: Recurrence rates of atrioventricular nodal reentry tachycardia (AVNRT) after cryoablation continue to remain high despite excellent initial success rates. Our objective was to evaluate the clinical outcomes of cryoablation for AVNRT with the 4-mm and 6-mm tip cryoablation catheters in a young population and to elicit predictors of arrhythmia recurrence.
Methods: We retrospectively reviewed all patients who underwent cryoablation for AVNRT at the UCSF/Stanford Pediatric Arrhythmia Center from January 2004 to February 2007.
Results: One hundred fifty-four patients (age 13.7 years (3.2–24.4)) underwent cryoablation for AVNRT of which 144 patients had inducible AVNRT (123 sustained and 21 nonsustained) and 10 had presumed AVNRT. Initial success was achieved in 95% (146/154), with no difference between the 4-mm (93%) and 6-mm (98%) cryoablation catheter tips (P = 0.15). There was no permanent atrioventricular (AV) block. Transient third-degree AV block occurred in nine patients (6%), with no difference between the 4-mm (4%) and 6-mm (9%) tip (P = 0.13). AVNRT recurrence was documented in 14% in a median time of 2.5 months (0.25–20). Recurrences were lower with the 6-mm (9%) versus the 4-mm (18%) tip, but this did not reach statistical significance (P = 0.16). With univariate analysis, a longer fluoroscopy time was the only significant factor associated with recurrence. Multivariate analysis failed to identify any significant predictor of AVNRT recurrence.
Conclusion: Outcomes of cryoablation for AVNRT continue to be good without the complication of AV block. We could not identify any specific parameter associated with AVNRT recurrence. Further improvements in cryoablation technique will be necessary to reduce recurrences.  相似文献
