BACKGROUND: Nowadays, endoscopic management of ureteropelvic junction (UPJ) obstruction is the treatment of choice. However, in the presence of crossing vessels, the success rate of endoscopic management decreases and the risk of hemorrhagic and vascular complications rises. The purpose of this study is to evaluate patients with UPJ obstruction using contrast enhanced multidetector computed tomography (CT) angiography to aid in surgical planning and management. PATIENTS AND METHODS: Between 2001 and 2005, 27 patients (mean age: 43 years; age range: 17-75 years) with UPJ obstruction were studied with multidetector CT angiography. Identification and characterization of crossing vessels was performed with multidirectional images and three-directional reconstructions. RESULTS: 12 patients (44%) were found to have 16 crossing vessels (vessels in contact with the UPJ or within a vicinity of less than 2 mm). Nine of these vessels were arteries and seven were veins. Nine vessels crossed anteriorly, two posteriorly, and one anteromedially. Endopyelotomy was contraindicated in these 12 patients due to the presence of crossing vessels. Eleven out of the 12 patients underwent a pyeloplasty by open surgery or laparoscopy, where the presence of crossing vessels was confirmed. One of the 12 patients did not undergo surgery. CONCLUSION: Multidetector CT angiography permits an adequate preoperative assessment of patients with UPJ obstruction as it is able to identify the presence and location of crossing vessels. Furthermore, it allows to study in detail the anatomy of the renal area and its vascular variants.  相似文献

Helical computed tomography (CT) was used to demonstrate the distribution of crossing vessels in patients with ureteropelvic junction (UPJ) obstruction for planning surgical management. Twenty patients with symptomatic UPJ obstruction were evaluated with dual-phase contrast material-enhanced helical CT. In addition to axial images, coronal, sagittal, and curved paracoronal images along the crossing vessels or the UPJ were obtained by means of multiplanar reconstruction. Crossing vessels were evaluated according to type, position, and association with UPJ obstruction. Fifteen vessels in 12 of the 20 patients were found to cross the UPJ at helical CT. Nine vessels were arteries and six were veins; seven vessels crossed anteriorly and eight crossed posteriorly. In 11 patients, crossing vessels were thought to be associated with UPJ obstruction at helical CT; retroperitoneoscopic repair was performed, and the diagnosis was found to have been accurate in all except one. In the eight patients in whom no significant vessels were seen and the one patient in whom the crossing vessel was not associated with UPJ obstruction at helical CT, endopyelotomy was performed and UPJ obstruction was relieved, with no complications. Helical CT is useful for evaluation of vessels crossing the UPJ and for planning surgical management.  相似文献

Purpose The purpose of this study was to clarify and compare the accuracy of contrast-enhanced computed tomography (CT) angiography using multidetector-row helical CT (MDCT angiography) and gadolinium-enhanced MR angiography using three-dimensional Fourier transformation gradient-echo sequence (3D MR angiography) for preoperative evaluation of renal arteries in living renal donors. Materials and methods A total of 42 living renal donor candidates underwent both MDCT angiography and 3D MR angiography before digital subtraction angiography (DSA). Each MDCT angiogram and 3D MR angiogram was prospectively interpreted, and the findings were compared with the DSA results. Results MDCT angiography identified all of the 12 supernumerary arteries detected by DSA, whereas 3D MR angiography identified only 8. MDCT angiography identified all of the 19 proximal arterial branches detected by DSA, whereas 3D MR angiography identified only 16. Conclusion A more accurate depiction of renal arteries in living renal donors can be achieved with MDCT angiography than with 3D MR angiography.  相似文献

PURPOSE: To assess the usefulness of intraarterial computed tomographic (CT) angiography in conjunction with digital subtraction angiography (DSA) by using a combined CT and angiographic unit in the preoperative evaluation of intracranial aneurysms. MATERIALS AND METHODS: Prospectively, 22 patients with or without subarachnoid hemorrhage underwent CT angiography in conjunction with DSA. Two radiologists independently evaluated DSA and CT angiographic images. Referring neurosurgeons were questioned as to how the additional information provided by CT angiography changed patient treatment. RESULTS: Intraarterial CT angiography was superior to DSA for use in aneurysm detection in three (12%) of 26 aneurysms and for delineation of aneurysm shape, neck, and location in more than half. In 14 (64%) of 22 patients, CT angiography demonstrated 18 additional findings: a very small aneurysm (n = 2), aneurysm shape and neck (n = 6), relationship of the aneurysm to adjacent arteries or bone structure (n = 8), and branches deriving from the aneurysm (n = 2). In four (27%) of 15 patients who underwent surgery or embolization, additional information obtained at CT angiography affected the treatment. CT angiography failed to clearly demonstrate an aneurysm adjacent to bone structures and small perforators, which were derived from the parent artery. CONCLUSION: Intraarterial CT angiography is useful for preoperative evaluation of intracranial aneurysms as a supplement to DSA.  相似文献

目的:客观评价螺旋CT肾动脉成像的临床应用价值.材料和方法:从本院螺旋CT肾动脉成像(CTRA)资料中选取31例曾做过DSA肾动脉造影(含动脉成形和支架放置术)者,CTRA和DSA图像分别先由两位医师独立观察.分析内容包括解剖变异、有无狭窄及进行狭窄程度分级.最后将结果进行对比分析.结果:CTRA发现狭窄血管33支,其中31支经DSA证实(2支假阳性).符合率为94%.清楚显示5支副肾动脉(经DSA证实).结论:CTRA是肾性高血压筛选的可靠方法并可取代DSA作为首选检查方法.  相似文献

PURPOSE: To compare the costs of performing helical computed tomographic (CT) angiography with three-dimensional rendering versus intraarterial digital subtraction angiography (DSA) for preoperative imaging of abdominal aortic aneurysms (AAAs). MATERIALS AND METHODS: A single observer determined the variable direct costs of performing nine intraarterial DSA and 10 CT angiographic examinations in age- and general health-matched patients with AAA by using time and motion analyses. All personnel directly involved in the cases were tracked, and the involvement times were recorded to the nearest minute. All material items used during the procedures were recorded. The cost of labor was determined from personnel reimbursement data, and the cost of materials, from vendor pricing. The variable direct costs of laboratory tests and using the ambulatory treatment unit for postprocedural monitoring, as well as all fixed direct costs, were assessed from hospital accounting records. The total costs were determined for each procedure and compared by using the Student t test and calculating the CIs. RESULTS: The mean total direct cost of intraarterial DSA (+/- SD) was $1,052 +/- 71, and that of CT angiography was $300 +/- 30, which are significantly different (P < 4.1 x 10(-11)). With 95% confidence, intraarterial DSA cost 3.2-3.7 times more than CT angiography for the assessment of AAA. CONCLUSION: Assuming equal diagnostic utility and procedure-related morbidity, institutions may have substantial cost savings whenever CT angiography can replace intraarterial DSA for imaging AAAs.  相似文献

PURPOSE: To assess the luminal morphology of the extracranial internal carotid artery at three-dimensional (3D) computed tomographic (CT) angiography and how this factor affects measurement of maximum carotid arterial stenoses at conventional intraarterial digital subtraction angiography (DSA). MATERIALS AND METHODS: Prospectively, conventional intraarterial DSA and 3D CT angiography were performed in 42 carotid arteries in 21 patients with suspected carotid artery disease. The longest axis length-perpendicular axis length (L/P) ratios of the arterial lumen on the cross-sectional images at the most stenotic area and distal nonstenotic area were analyzed by acquiring multiplanar reconstruction (MPR) images at 3D CT angiography. The maximum stenosis was measured at each modality with North American Symptomatic Carotid Endarterectomy Trial criteria. RESULTS: The L/P ratios in the most stenotic areas ranged from 1.0 to 3.2 (mean, 1.5 +/- 0.5 [SD]). The mean difference in maximum percentage of stenosis between the two modalities for L/P ratios of 2.0 or greater was significantly greater than that for L/P ratios of less than 1.5 (P < .05). Three carotid arteries with 70%-99% stenosis, with grades determined only with 3D CT angiography, had L/P ratios of 2.0 or greater. CONCLUSION: On MPR images at 3D CT angiography, the lumen of extracranial internal carotid artery stenosis showed a wide range of shapes. When a carotid artery has a high L/P ratio, the luminal morphology of the carotid artery stenosis may affect the assessment of maximum stenosis of the internal carotid artery at conventional DSA.  相似文献

Purpose: To evaluate the diagnostic accuracy of virtual vascular endoscopy (VVE) in the detection of accessory renal arteries. Methods: We retrospectively reviewed the CT angiography data sets of 67 patients (29 male and 38 female; age range 17–72 years, mean age 53 years) imaged for the study of the renal arteries, and affected by renovascular hypertension. All patients also had intraarterial digital subtraction angiography (DSA). CT angiography data sets were processed to obtain maximum intensity projection (MIP) and surface-rendered VVE of the aorta. Axial images, MIP, and VVE were evaluated separately and in combination in the detection of accessory renal arteries. Their results in terms of sensitivity and specificity were then compared with DSA. Results: Axial images had a sensitivity of 88% and specificity of 94% for accessory renal artery detection, MIP had a sensitivity of 88% and specificity of 98%, and VVE had a sensitivity of 63% and a specificity of 88% (p < 0.05 vs DSA), but these increased to 88% and 98% respectively if endoscopic views were integrated with the other display techniques. Conclusion: VVE based on surface rendering does not add substantial benefits to CTY angiography; by contrast MIP is the most accurate display technique for the detection of accessory renal arteries.  相似文献

OBJECTIVE: The aim of our study was to evaluate a three-dimensional gadolinium-enhanced breath-hold MR angiography sequence using standard MR gradients in detecting renal artery stenosis. SUBJECTS AND METHODS: Forty-two patients referred for angiography for suspected renal artery stenosis underwent both conventional digital subtraction angiography (DSA) and MR angiography. MR angiography was performed on a 1.5-T scanner with standard gradients. A fast multiplanar spoiled gradient-echo sequence was used with the following parameters: TR/TE, 10.3/1.9; flip angle, 45 degrees; field of view, 36 x 32 cm; matrix size, 256 x 128; one excitation; volume thickness, 70 mm; and partitions, 28. Gadolinium was administered IV as a dynamic bolus of 30-40 ml. Conventional and MR angiographic images were interpreted by two radiologists in consensus. RESULTS: DSA revealed 87 renal arteries, of which 79 were in 35 patients with native kidneys and eight arteries were in seven patients with transplanted kidneys. Gadolinium-enhanced MR angiography showed 85 (98%) of 87 renal arteries. Seventeen patients had 20 significant (>50% stenosis) renal artery stenoses and five patients had five occluded renal arteries revealed by DSA. MR angiography revealed 85 renal arteries (98%), 20 stenoses (100%), and five occlusions (100%). Gadolinium-enhanced MR angiography led to one false-positive interpretation for renal artery stenosis and no false-negative interpretations. Thus, the sensitivity, specificity, and accuracy of MR angiography for renal artery stenosis were 100%, 98%, and 99%, respectively. CONCLUSION: The MR angiography pulse sequence we used was an effective and reliable technique for the diagnosis of renal artery stenosis. The sequence can be performed on widely available MR equipment that does not require fast gradient hardware.  相似文献

目的:探讨64层螺旋CT血管成像术在下肢动脉疾病中的应用价值及技术优势。方法:对21例疑有下肢动脉疾病患者行64层螺旋CT增强扫描,扫描范围从肾动脉水平达足底。薄层重建横断面图像传入Wizard工作站,进行血管三维重建。其中21例结合常规血管造影评价64层螺旋CT血管成像术(CTA)的准确性。结果:在441个动脉节段中,435个节段在CTA与DSA均可以显示,在DSA图像上,狭窄闭塞的节段共130个(轻度狭窄16段,中度狭窄12段,重度狭窄22段,闭塞80段),最大密度投影(MIP)显示中度以上狭窄的敏感性、特异性及准确性分别为:99.1%、99.7%、99.5%。结论:64层CT血管成像是一种高度准确、非侵袭性的成像技术,在评估下肢动脉疾病方面与常规血管造影结果无明显差别,是下肢动脉疾病较好的影像学检查手段。  相似文献

PURPOSE: To compare results of helical computed tomographic (CT) angiography with real-time interactive volume rendering (VR) to CT angiography with maximum intensity projection (MIP) for the detection of renal artery stenosis. MATERIALS AND METHODS: Twenty-five patients underwent both conventional and CT angiography of the renal arteries. Images were blindly reviewed after rendering with MIP and VR algorithms. MIP images were viewed in conjunction with axial CT images; VR models were evaluated in real time at the workstation without CT images. Findings in 50 main and 11 accessory renal arteries were categorized as normal or by degree of stenosis. RESULTS: All arteries depicted on conventional angiograms were visualized on MIP and VR images. Receiver operating characteristic (ROC) analysis for MIP and VIR images demonstrated excellent discrimination for the diagnosis of stenosis of at least 50% (area under the ROC curve, 0.96-0.99). Although sensitivity was not significantly different for VR and MIP (89% vs 94%, P > .1), specificity was greater with VR (99% vs 87%, P = .008 to .08). Stenosis of at least 50% was overestimated with CT angiography in four accessory renal arteries, but three accessory renal arteries not depicted at conventional angiography were depicted at CT angiography. CONCLUSION: In the evaluation of renal artery stenosis, CT angiography with VR is faster and more accurate than CT angiography with MIP. Accessory arteries not depicted with conventional angiography were depicted with both CT angiographic algorithms.  相似文献

PURPOSE: To evaluate in living renal donors the usefulness of multi-detector row computed tomography (CT) in the assessment of renal vasculature and the upper urinary tract. MATERIALS AND METHODS: Four-channel multi-detector row CT scans were obtained in 77 patients. Vascular phase scans were used for CT angiography; excretory phase scans, for CT urography. At CT angiography, two independent observers evaluated the number of arteries and veins and the presence of early-branching arteries. CT urographic images were evaluated with regard to the opacification of the urinary tract and for abnormalities. Findings of CT angiography and urography were compared with surgical findings. Interobserver agreement between CT angiographic and surgical findings was quantified with weighted kappa statistics. Sensitivity and specificity of CT angiography in identifying supernumerary vessels and early-branching arteries were also evaluated. To evaluate the radiation dose to patients, weighted CT dose index (DI) was assessed for each scan. RESULTS: Agreement between CT angiographic and surgical findings was excellent for the number of renal arteries (kappa = 0.896) and veins (kappa = 0.843). Detection rate of CT angiography was 98% (89 of 91) for arteries and 98% (83 of 85) for veins. The respective sensitivity and specificity of CT angiography were 86% (12 of 14) and 100% (65 of 65) for supernumerary arteries, 100% (11 of 11) and 100% (66 of 66) for early-branching arteries, and 75% (six of eight) and 100% (69 of 69) for supernumerary veins. At CT urography, collecting systems and proximal ureters were well opacified in all patients; two patients had underrotated kidneys without obstruction. The weighted CT DI was 10.19 mGy for unenhanced and excretory phase scans and 12.88 mGy for the vascular phase scan. CONCLUSION: Multi-detector row CT can help assess well the renal vasculature and the urinary tract of living renal donors.  相似文献

Our aim was to investigate the usefulness of helical CT during selective angiography (CT arteriography) in pretreatment assessment of unruptured intracranial aneurysms. We studied 47 unruptured aneurysms in 34 prospectively recruited patients for whom endovascular embolisation was initially considered. As pretreatment assessment, we performed rotational digital subtraction angiography (DSA) followed by CT arteriography. The findings on axial source images (axial images) and reconstructed three-dimensional CT angiography (3D-CTA) of CT arteriography were compared to those of rotational DSA, with particular attention to the neck of the aneurysm and arterial branches adjacent to it. Information provided by CT arteriography was more useful than that of rotational DSA as regards the neck in 25 (53 %) of 47 cases and as regards branches in 18 (49 %) of 37 aneurysms. On axial images, small arteries such as the anterior choroidal artery were seen in some cases. CT arteriography can provide valuable additional information about unruptured aneurysms, which cannot be obtained by rotational DSA alone. This technique is useful for obtaining anatomical information about aneurysm anatomy and for deciding the therapeutic strategy. Received: 22 May 2000/Accepted: 25 September 2000  相似文献

PURPOSE: To compare computed tomographic (CT) angiography and magnetic resonance (MR) angiography for preoperative evaluation of living renal donors. MATERIALS AND METHODS: Thirty-five living renal donors underwent preoperative contrast material-enhanced CT angiography and gadolinium-enhanced MR angiography. Each study was interpreted by two independent radiologists blinded to all other studies and to interpretations provided by other reviewers. Eighteen kidneys had surgical correlation. RESULTS: CT demonstrated 33 supernumerary arteries in 19 patients, bilateral solitary arteries in 16 patients, and 18 proximal arterial branches in 16 patients. MR demonstrated 26 supernumerary arteries in 15 patients, bilateral solitary renal arteries in 20 patients, and 21 proximal arterial branches in 16 patients. Interobserver agreements for MR (kappa = 0. 74) and CT (kappa = 0.73) were similar to the agreement between MR and CT (kappa = 0.74). Among the kidneys chosen for nephrectomy, one small accessory artery and one proximal arterial branch were missed with CT and MR. Two of the accessory arteries suggested at CT were not found at nephrectomy. By averaging data for both modalities, supernumerary arteries were present in 49% of kidney donors and were bilateral in approximately 17%. Proximal arterial branches were present in 46% of kidney donors. CONCLUSION: Preoperative CT and MR angiography of the renal arteries in renal donors demonstrate substantial agreement. Interobserver disagreement in the interpretation of CT and MR angiograms is related to 1-2-mm-diameter vessels.  相似文献

OBJECTIVE: The purpose of this study was to determine the accuracy of multidetector CT (MDCT) angiography as the primary imaging technique in the evaluation of living kidney donors. SUBJECTS AND METHODS: Seventy-four consecutive living kidney donors (30 men, 44 women; mean age, 41.7 years) who underwent MDCT were evaluated. CT examination was performed with 120 mL of IV contrast material at an injection rate of 3 mL/sec and a pitch of 6. In every case, arterial and venous phase volumetric data sets were acquired at 25 and 55 sec, respectively. Scans were reconstructed at 1-mm intervals for three-dimensional (3D) imaging using a volume-rendering technique. Axial CT images and 3D CT angiography were evaluated prospectively by one reviewer and retrospectively by two reviewers who had no knowledge of surgical results. Surgical correlation for the location of primary and accessory renal arteries, early branching of the renal arteries, and renal vein anomalies was made. RESULTS: Seventy-two subjects underwent left nephrectomy, and two subjects underwent right nephrectomy because supernumerary left renal arteries were detected on preoperative CT angiography. Eighteen supernumerary renal arteries (two arteries to 16 kidneys and three arteries to one kidney) to 74 kidneys underwent nephrectomy. CT and surgical findings agreed in 93% of subjects (the average of three reviewers; range, 89-97%). Two small accessory renal arteries were missed by all three reviewers. Those arteries were diminutive and were thought to be insignificant by the surgeons. Early branching of the renal arteries was shown in 14 arteries, and CT and surgical findings agreed in 96% (the average of three reviewers; range, 93-97%). Renal vein anomalies were present in eight subjects, and CT and surgical findings agreed in 99% of the cases (range, 96-100%). CONCLUSION: MDCT angiography is highly accurate for detecting vascular anomalies and providing anatomic information for laparoscopic living donor nephrectomy.  相似文献

A new technique for hepatic intraarterial digital subtraction angiography (DSA)--hepatoportal subtraction angiography (HPSA)--that emphasizes the enhancement of tumors by suppressing liver parenchymal enhancement has been developed. The tumor detection sensitivity of HPSA was evaluated prospectively in comparison with that of conventional hepatic intraarterial DSA, unenhanced computed tomography (CT) or CT enhanced with iodine-containing contrast material (conventional CT), CT with iodized oil, and ultrasound (US). For 84 detected lesions of hepatoma, the sensitivity of HPSA was 92%, statistically superior to the 71% of hepatic intraarterial DSA (P less than .002), the 42% of US (P less than .001), and the 39% of conventional CT (P less than .001) and nearly the same as the 90% of CT with iodized oil. Among 33 lesions of metastasis, HPSA depicted 91%; conventional CT, 93%; US, 87%; and hepatic intraarterial DSA, 76%. HPSA was more sensitive than hepatic intraarterial DSA, but the difference was not statistically significant. Compared with hepatic intraarterial DSA, HPSA enabled detection of more faintly enhanced tumors, and this technique was considered useful for detecting small hypervascular hepatic tumors.  相似文献

PURPOSE: To prospectively evaluate the accuracy of intraarterial magnetic resonance (MR) angiography in the depiction of significant stenoses and occlusions, with intraarterial digital subtraction angiography (DSA) serving as the reference standard. MATERIALS AND METHODS: Approval of the local ethics committee and informed consent were obtained. Twenty patients (11 men; nine women; age range, 48-86 years; mean age, 69.5 years+/-11.2 [standard deviation]) with symptomatic peripheral arterial occlusive disease (PAOD) were prospectively enrolled. After percutaneous transluminal angioplasty (PTA), intraarterial MR angiography was performed in the thigh and the calf with a 1.5-T MR imager in two consecutive runs. Intraarterial MR angiography was performed with a low-dose injection protocol (ie, two 20-mL injections of a 50-mmol gadolinium-based contrast agent). Moderate stenoses (luminal narrowing50%) or vessel occlusions; 95% confidence intervals (CIs) were calculated for sensitivity and specificity. RESULTS: Intraarterial DSA revealed 78 moderate stenoses, 57 significant stenoses, and 28 occlusions. Sensitivity, specificity, and accuracy of intraarterial MR angiography in the characterization of significant stenoses or occlusions were 92% (95% CI: 72%, 99%), 94% (95% CI: 82%, 98%), and 93%, respectively, in femoropopliteal arteries and 93% (95% CI: 83%, 98%), 71% (95% CI: 51%, 86%), and 86%, respectively, in infrapopliteal arteries. The main artifact observed with intraarterial MR angiography was venous contamination (12%). CONCLUSION: Intraarterial MR angiography is an accurate method used to depict significant stenoses and occlusions in lower extremity arteries with a low-dose injection protocol.  相似文献

OBJECTIVE: Catheter-based intraarterial injections of gadolinium are useful during MR imaging-guided endovascular procedures to generate rapid vascular road maps. Using an animal model of renal artery stenosis, we tested the hypothesis that intraarterial gadolinium-enhanced MR angiography is as accurate as IV gadolinium-enhanced MR angiography and digital subtraction angiography (DSA). We also tested the hypothesis that intraarterial MR angiography uses less gadolinium than IV MR angiography. MATERIALS AND METHODS: We induced bilateral renal artery stenosis in five pigs. All pigs underwent comparative imaging with DSA, IV MR angiography, and aortic catheter-directed intraarterial MR angiography. For IV and intraarterial MR angiography, we used the same three-dimensional acquisition. We assessed differences in quantitative stenosis measurements among DSA, IV MR angiography, and intraarterial MR angiography using the Wilcoxon's signed rank test. RESULTS: Mean stenosis measurements (+/-SD) were as follows: DSA, 58% +/- 12%; IV MR angiography, 63% +/- 9.3%; and intraarterial MR angiography, 64% +/- 11%. There were no statistically significant differences in accuracy between DSA and IV MR angiography (p = 0.06), DSA and intraarterial MR angiography (p = 0.16), or IV and intraarterial MR angiography (p = 0.70). Intraarterial MR angiography used a mean gadolinium dose of 5.6 mL, compared with 9 mL for IV MR angiography. CONCLUSION: In swine, IV and intraarterial MR angiography have a similar accuracy for detecting renal artery stenosis. Intraarterial MR angiography uses smaller doses of injected gadolinium.  相似文献

PURPOSE: To determine whether intraarterial digital subtraction angiography (DSA) can be replaced by contrast material-enhanced magnetic resonance (MR) angiography in the assessment of patency or thrombosis of the portal venous system in patients with portal hypertension. MATERIALS AND METHODS: Thirty-six patients with portal hypertension underwent contrast-enhanced MR angiography and intraarterial DSA for assessment of the portal venous system. The images were evaluated for vessel patency or thrombosis of the portal, splenic, or superior mesenteric vein. RESULTS: Of the 101 vessels evaluated, 42 were thrombosed. Overall sensitivity, specificity, and accuracy for the detection of thrombosis were 100%, 98%, and 99%, respectively, for MR angiography and 91%, 100%, and 96%, respectively, for DSA; differences between the imaging methods were not statistically significant. Only in four patients with six vessels (6%) were there discordant findings between MR angiography and DSA. CONCLUSION: Noninvasive contrast-enhanced MR angiography has the potential to replace intraarterial DSA as the standard method to assess the whole portal venous system.  相似文献

PURPOSE: To compare a multislab balanced turbo field-echo magnetic resonance (MR) angiographic technique, without the use of a contrast agent, with digital subtraction angiography (DSA) for imaging of the renal arteries. MATERIALS AND METHODS: Twenty-five randomly selected patients (eight women and 17 men; age range, 27-88 years; mean age, 72 years) suspected of having renal artery stenosis underwent both DSA and balanced turbo field-echo MR angiography. A consensus result was obtained among three radiologists in evaluation of main renal arteries on balanced turbo field-echo images and DSA images. Sensitivity, specificity, and negative and positive predictive values of the balanced turbo field-echo technique were calculated, and receiver operating characteristic analysis was performed for depiction of hemodynamically significant stenosis. Cohen kappa analysis was used to assess agreement between the two imaging methods in grading of stenoses and depiction of significant stenosis. Accessory renal arteries also were evaluated. RESULTS: Fifty main renal arteries and 11 accessory arteries were fully depicted with DSA. DSA depicted 11 stenotic lesions in the main renal arteries. In comparison, balanced turbo field-echo MR angiography enabled visualization of 46 of 50 main renal arteries to their first branching points and depicted 10 of 11 accessory arteries. Sensitivity, specificity, negative predictive value, and positive predictive value of this technique for depiction of significant stenosis were 100% (four of four), 98% (41 of 42), 100% (41 of 41), and 80% (four of five), respectively. The area under the receiver operating characteristic curve was 0.988. kappa was 0.782 for grading of stenoses and 0.877 for depiction of significant stenosis. CONCLUSION: Multislab balanced turbo field-echo imaging has potential as an MR angiography technique for depiction of normal and diseased renal arteries.  相似文献
