Adhesive cementation is a useful procedure not only for cementing restorations onto teeth but also for attaching new ceramic restorations to existing restorations to improve their esthetics and/or function. This clinical report presents a technique for modifying an existing nonsatisfactory implant-supported fixed dental prosthesis by means of adhesively cemented veneers and onlays bonded to the pre-existing metal-ceramic surface with resin cement. This modification may avoid the replacement of the fixed dental prosthesis, reducing both treatment costs and time while fulfilling the patient’s demands.  相似文献

Statement of problem

The color of an esthetic restoration and its color stability are important for long-term success. However, the impact of common beverages on lithium disilicate ceramic is not well known.

Purpose

The purpose of this in vitro study was to investigate color variations of lithium disilicate ceramics after thermocycling (TC) and immersion in commonly consumed beverages.

Material and methods

A total of 288 specimens (1×10×10 mm) were fabricated from IPS e-max computer-aided design (CAD) (n=72), IPS e-max CERAM ([CER] n=72), IPS e-max Press with glazing ([PG] n=72), and IPS e-max Press without glazing ([PNG] n=72) according to the manufacturer's instructions. Each group was divided into 4 subgroups (n=18): TC, coffee, black tea, and red wine. Thermocycling was performed at 21?900 cycles at 5°C, 37°C, 55°C, and 37°C (3 years’ clinical simulation), whereas the specimens were soaked in the staining solutions for up to 54 hours (3 years’ clinical simulation). Color parameters L*, a*, and b* were assessed with an ultraviolet-visible spectroscopy recording spectrophotometer. Color difference (ΔE) was calculated using the equation [ΔΕ= [(ΔL*)2 + (Δa*)2 + (Δb*)2]½]. Intraexaminer reliability was assessed by using the intraclass correlation coefficient. Two-way analysis of variance was used for the analysis of ΔΕ, and the parameters L*, a*, and b* were analyzed with linear mixed models for repeated measurements and the Bonferroni pair-wise comparison test (α=.05).

Results

Parameters ΔΕ, L*, a*, and b* were significantly affected by the interaction between material and treatment (P<.001). A ΔΕ >1 was recorded for PG with tea, wine, and coffee, PG after TC and CER after TC. For L*, greater reduction was observed for PNG with tea and CER after TC, whereas for a* significant changes were positive (to red shades), except for PNG with TC, where PNG with wine showed the greatest positive change. For b*, significant changes were negative (to blue shades) except for PNG with tea and coffee and CAD with tea.

Conclusions

All groups demonstrated color changes below the clinically perceptible level (ΔΕ<3.7), except PNG in tea which showed statistically significant color differences (ΔΕ>4). CAD presented higher color stability compared with the nonglazed Press specimens.  相似文献

Statement of problem

Complete dentures fabricated by computer-aided design and computer-aided manufacturing (CAD-CAM) techniques have become popular. The 2 principal CAD-CAM techniques, milling and rapid prototyping (3D printing), used in the fabrication of complete dentures have been reported to yield clinically acceptable results. However, clinical trials or in vitro studies that evaluated the accuracy of the 2 manufacturing techniques are lacking.

Purpose

The purpose of this in vitro study was to compare the differences in trueness between the CAD-CAM milled and 3D-printed complete dentures.

Material and methods

Two groups of identical maxillary complete dentures were fabricated. A 3D-printed denture group (3DPD) (n=10) and a milled denture group (MDG) (n=10) from a reference maxillary edentulous model. The intaglio surfaces of the fabricated complete dentures were scanned at baseline using a laboratory scanner. The complete dentures were then immersed in an artificial saliva solution for a period of 21 days, followed by a second scan (after immersion in saliva). A third scan (after the wet-dry cycle) was then made after 21 days, during which the complete dentures were maintained in the artificial saliva solution during the day and stored dry at night. A purpose-built 3D comparison software program was used to analyze the differences in the trueness of the complete dentures. The analyses were performed for the entire intaglio surface and specific regions of interest: posterior crest, palatal vault, posterior palatal seal area, tuberosity, anterior ridge, vestibular flange, and mid-palatal raphae. Independent t tests, ANOVA, and post hoc tests were used for statistical analyses (α=.05).

Results

The trueness of the milled prostheses was significantly better than that of the rapid prototyping group with regard to the entire intaglio surface (P<.001), posterior crest (P<.001), palatal vault (P<.001), posterior palatal seal area (P<.001), tuberosity (P<.001), anterior ridge (baseline: P<.001; after immersion in saliva: P=.001; after the wet-dry cycle: P=.011), vestibular flange (P<.001), and mid-palatal raphae (P<.001).

Conclusions

The CAD-CAM, milled complete dentures, under the present manufacturing standards, were superior to the rapidly prototyped complete dentures in terms of trueness of the intaglio surfaces. However, further research is needed on the biomechanical, clinical, and patient-centered outcome measures to determine the true superiority of one technique over the other with regard to fabricating complete dentures by CAD-CAM techniques.  相似文献

Statement of problem

The effect of thermal aging on the color stability of monolithic zirconia has not been thoroughly investigated.

Purpose

The purpose of this in vitro study was to evaluate the color stability before and after thermocycling of monolithic zirconia specimens, which were either preshaded or characterized in the laboratory through the assessment of color parameters L*, a*, and b*.

Material and methods

A total of 80 specimens (10×10×15 mm) were prepared from zirconia disks (BruxZir) and divided into 2 groups. The first group consisted of preshaded specimens from disks 100, 200, 300, and 400, and the second group consisted of white specimens characterized before the sintering stage with coloring liquids in shades A2, B2, C2, and D2. Ten specimens of each shade were used. A double-beam ultraviolet-visible light recording spectrophotometer was used to assess the color parameters L*, a*, and b* before and after thermocycling. The color difference ΔE was calculated based on ΔΕ=[(ΔL*)2+(Δa*)2+(Δb*)2]½. The reliability of the examiner was investigated by MedCalc v12.5 software, and the tests of normality, homogeneity of variances, and 1-way ANOVA statistical analysis of variance were performed by IBM Statistics SPSS v20.0 software (α=.05).

Results

No statistically significant ΔΕ (P>.05) was observed among the different groups after thermocycling. All ΔΕ values were below the limit of the 3.7 that an untrained observer can perceive. Only for the specimens in shades 200 and 400 was ΔΕ above 1, revealing a visible color difference, which, however, was still clinically acceptable. The L*, a*, and b* values for both liquid-shaded and preshaded zirconia specimens were not significantly affected by thermocycling. A higher color stability for laboratory-characterized compared with preshaded monolithic zirconia was observed irrespective of the color shade.

Conclusions

Within the limitations of this in vitro study, monolithic zirconia specimens of any shade, either preshaded or characterized in the laboratory, can be considered as color-stable materials after thermocycling.  相似文献

Statement of problem

Ceramic restorations are popular because of their excellent optical properties. However, failures are still a major concern, and dentists are confronted with the following question: is sleep bruxism (SB) associated with an increased frequency of ceramic restoration failures?

Purpose

The purpose of this systematic review and meta-analysis was to assess whether the presence of SB is associated with increased ceramic restoration failure.

Material and methods

Observational studies and clinical trials that evaluated the short- and long-term survival rate of ceramic restorations in SB participants were selected. Sleep bruxism diagnostic criteria must have included at least 1 of the following: questionnaire, clinical evaluation, or polysomnography. Seven databases, in addition to 3 nonpeer-reviewed literature databases, were searched. The risk of bias was assessed by using the meta-analysis of statistics assessment and review instrument (MAStARI) checklist.

Results

Eight studies were included for qualitative synthesis, but only 5 for the meta-analysis. Three studies were categorized as moderate risk and 5 as high risk of bias. Clinical and methodological heterogeneity across studies were considered high. Increased hazard ratio (HR=7.74; 95% confidence interval [CI]=2.50 to 23.95) and odds ratio (OR=2.52; 95% CI=1.24 to 5.12) were observed considering only anterior ceramic veneers. Nevertheless, limited data from the meta-analysis and from the restricted number of included studies suggested that differences in the overall odds of failure concerning SB and other types of ceramic restorations did not favor or disfavor any association (OR=1.10; 95% CI=0.43 to 2.8). The overall quality of evidence was considered very low according to the GRADE criteria.

Conclusions

Within the limitations of this systematic review, the overall result from the meta-analysis did not favor any association between SB and increased odds of failure for ceramic restorations.  相似文献

Statement of problem

Whether masking or translucency should be the goal for achieving an esthetic restoration with ceramic materials is unknown.

Purpose

The purpose of this in vitro study was to examine the effect of the number of firings on the color and translucency of ceramic core material with different thicknesses of veneer ceramic.

Material and methods

Three different ceramic systems were used: IPS Empress (IE), IPS e.max Press (IEP), and Turkom Cera (TC). Ninety disk-shaped cores, 10 mm in diameter and 0.8 mm in thickness were prepared using each system (n=30). The specimens were divided into 3 subgroups (n=10) according to the thickness of the veneering ceramic: 0.5, 1.0, and 1.5 mm. The specimens were fired 1, 3, and 5 times, and the color parameters of the specimens were measured. The color measurements were repeated, and the data were statistically evaluated using 2-way and multifactorial analysis of variance, the Duncan test, and the Bonferroni post hoc test.

Results

The number of firings affected the translucency parameter and CIELab values. Increased porcelain thickness led to decreased translucency parameter and L* values and increased a* and b* values (P<.05). The IE system was the most translucent (P<.001). The changes with increased firing periods were not statistically significant (P>.05). The number of firings affected the color change of the IE and IEP systems but not of the translucency parameter system.

Conclusions

Repeated firings and porcelain thickness affected the final color and translucency of the ceramic systems tested. These 2 factors adversely affected the esthetic result and should be considered in both the preparation and the laboratory phase.  相似文献

Statement of problem

Clinical data regarding newly introduced laser-sintered removable partial dentures (RPDs) are needed before this technique can be recommended. Currently, only a few clinical reports have been published, with no clinical studies.

Purpose

This clinical trial compared short-term satisfaction in patients wearing RPDs fabricated with conventional or computer-aided design and computer-aided manufacturing (CAD-CAM) laser-sintering technology.

Material and methods

Twelve participants with partial edentulism were enrolled in this pilot crossover double-blinded clinical trial. Participants were randomly assigned to wear cast or CAD-CAM laser-sintered RPDs for alternate periods of 30 days. The outcome of interest was patient satisfaction as measured using the McGill Denture Satisfaction Instrument. Assessments was conducted at 1, 2, and 4 weeks. The participant’s preference in regard to the type of prosthesis was assessed at the final evaluation. The linear mixed effects regression models for repeated measures were used to analyze the data, using the intention-to-treat principle. To assess the robustness of potential, incomplete adherence, sensitivity analyses were conducted.

Results

Statistically significant differences were found in patients’ satisfaction between the 2 methods of RPD fabrication. Participants were significantly more satisfied with laser-sintered prostheses than cast prostheses in regard to general satisfaction, ability to speak, ability to clean, comfort, ability to masticate, masticatory efficiency, and oral condition (P<.05). At the end of the study, 5 participants preferred the laser-sintered, 1 preferred the cast RPD, and 3 had no preference.

Conclusions

The use of CAD-CAM laser-sintering technology in the fabrication of removable partial dentures may lead to better outcomes in terms of patient satisfaction in the short term. PurposeThis systematic review aimed to investigate the prevalence of odontogenic cysts and tumors associated with impacted third molars (ITM).MethodsOnly studies that performed histopathological diagnosis of lesions were eligible for inclusion. Five main electronic and three grey literature databases were searched. Risk of bias (RoB) of included articles was assessed using the Joanna Briggs Institute Critical Appraisal Checklist for Studies Reporting Prevalence Data.ResultsFrom 1,300 studies identified, 16 met the inclusion criteria. Seven studies were classified as high, seven as moderate, and two as low RoB. The prevalence of odontogenic cysts and tumors associated with ITM was 5.3% (95%CI: 3.1%–8.1%) of ITM. Odontogenic cysts in particular were found in 4.4% (95%CI: 2.5–6.8%) of the extracted ITM, whilst odontogenic tumors in 0.5% (95%CI: 0.2–0.9%). The dentigerous cyst was mentioned in eleven studies with a pooled prevalence of 2.1% (95%CI: 1.4–3.1%). The odontogenic keratocyst was cited by nine studies and had a prevalence of 0.5% (95%CI: 0.2–0.7%). The radicular cyst was mentioned only in three articles and the pooled prevalence was 4.7% (95%CI: 0.0–19.4%)ConclusionOdontogenic cysts and tumors were found in 5.3% of ITM extracted. The most common lesions were the radicular cyst, dentigerous cyst, and odontogenic keratocyst.  相似文献

ObjectivesThe aim of this study was to assess and compare the crystalline structures of both powder and bulk human dental enamel by X-ray diffraction analysis (XRD).MethodsThe buccal surfaces of 60 selected noncarious third molars were divided into two groups of powdered and bulk enamel specimens. To prepare enamel powders, the enamel tissues were ground and powdered manually using a mortar and pestle. For bulk samples, the enamel slabs were sectioned using a low-speed diamond saw. The crystalline structures of samples were analyzed by XRD, and the obtained data were analyzed. The values of the lattice parameters were obtained using the equation of hexagonal crystalline structures. The crystal sizes and microstrains of crystallites were calculated using both the Scherrer and Williamson–Hall (W–H) methods. The total area under the peaks was considered as the criterion for assessing crystallinity. The data were analyzed by a parametric independent t-test and nonparametric Mann–Whitney test using SPSS for Windows at a significance level of 0.05.ResultsThe results of the current study revealed significant differences between the crystallinity values and crystal sizes of the two study groups (p ≤ 0.001). The powdered samples showed higher crystallinity and smaller crystal sizes than those of the bulk samples. The obtained strain values of the powder samples were nearly twice those of the bulk samples. This difference was not statistically significant (p = 0.76).ConclusionsThe results of this study revealed that the sample preparation method for human tooth enamel can affect the crystalline data obtained from XRD analysis.  相似文献

This article describes a procedure in which articulating paper is modified for the intraoral assessment of static occlusal contacts. The rectangular-shaped articulating paper is modified by creating parallel cuts at 2- to 3-mm intervals perpendicular to its long side without completely separating the paper. Statement of problem

Although newly developed photoinitiators can make up for the limitations of camphorquinone, their absorption spectra are different. However, little attention has been paid to the compatibility of available wavelength spectra of light-polymerization units (LPUs) and photoinitiators within resin cements.

Purpose

The purpose of this in vitro study was to evaluate the effect of monowave and polywave light-emitting diode (LED) units on the adhesion of dual-polymerizing self-adhesive resin cements to monolithic zirconia.

Material and methods

Monowave LPUs and polywave LPUs were chosen to polymerize 2 dual-polymerizing self-adhesive resin cements. Ninety-six zirconia disks were randomly divided into 4 groups as different combinations of LPUs and resin cements, namely ES-U200, BS-U200, ES-SC, and BS-SC. Resin cements were adhered to zirconia disks, and the microshear bond strength (μSBS) test was conducted after 24 hours of H2O storage (24 h) and 10?000 thermocycles (10k/TC). Failure modes were examined by stereomicroscopy and scanning electron microscopy (SEM). The degree of conversion (DC) was tested both immediately and 24 hours later. In statistical analyses, 2-way ANOVA and the Tukey honestly significant difference post hoc test were performed for μSBS and DC results, and chi-square test was performed for failure mode analysis (α=.05 for all tests).

Results

The 2-way ANOVA demonstrated that different combinations of LPUs and resin cements, as well as different artificial aging levels, significantly influenced microshear bond strength values (P<.001). The interactions between 2 factors were also significant (P<.001). The BS-SC group possessed relatively high bond strength in both 24-hour and 10k/TC aging levels. For the same resin cement, no significant difference was found in the immediate DC (P=.405 for U200 and P=.708 for SC). At 24 hours, DC and BS-U200 values were significantly higher than ES-U200 values (P=.002), whereas BS-SC values were not significantly different from ES-SC values (P=.284).

Conclusions

Within the limitation of this in vitro study, the emission spectra of LED units significantly influenced the bond strengths, DC, and failure mode of dual-polymerizing self-adhesive resin cements to zirconia at both immediate and artificial aging levels. The LPU should provide light energy to match the absorption wavelengths of photoinitiators in the resin cements.  相似文献

Statement of problem

Visual shade matching is subjective and a cause of concern for clinicians. Different measurement devices have been developed to assist in tooth color selection and to achieve better esthetic results. However, consensus is lacking as to which method of tooth shade selection provides more predictable results.

Purpose

The purpose of this clinical study was to compare the reliability of different visual and instrumental methods for dental shade matching.

Material and methods

Visual shade matching was performed by 3 experienced clinicians using 2 different shade guides (VITA Classical A1-D4 and VITA Toothguide 3D-MASTER with 29 tabs; VITA Zahnfabrik) with and without the aid of a light-correcting device (Smile Lite; Smile Line). An intraoral scanner (TRIOS; 3Shape A/S) and a spectrophotometer (VITA Easyshade Advance 4.0; VITA Zahnfabrik) were also used for color shade matching. The instrumental methods were repeated 3 times to determine repeatability. Shade-matching sessions for each method were performed under controlled lighting on the middle third of the maxillary right central incisor of 28 participants. The Fleiss' kappa statistical test was used to assess the reliability of each method. The weighted kappa statistical test was used to assess the agreement between the shades matched by different methods (α=.05).

Results

Instrumental methods were more accurate than visual methods. The best performance was found for the intraoral scanner configured for the 3D-MASTER scale (Fleiss' kappa value of .874) and for the spectrophotometer configured for the VITA Classical scale (Fleiss' kappa value of .805). The best visual shade-matching method was the VITA Classical scale associated with the light-correcting device (Fleiss' kappa value of .322). The Classical scale without the light-correcting device showed the poorest reliability (Fleiss' kappa value of .177) (P<.05).

Conclusions

Instrumental methods for color shade matching were more reliable than the visual methods tested.  相似文献

PurposeTo test three potential prosthetic material options for zirconia implants in regard to their mechanical properties, loading and retention capacity as well as to record abrasion after chewing simulation followed by thermocyclic aging.MethodsMolar crowns (n = 96) of three different computer-aided design/computer-aided manufacturing (CAD/CAM) materials were produced and cemented on zirconia implants (ceramic.implant, Vita) with a diameter of 4.5 mm. Monolithic zirconia (Vita YZ [YZ] with RelyX Unicem 2 Automix [RUN], polymer-infiltrated ceramic (Vita Enamic [VE]) with Vita Adiva F-Cem [VAF] and acrylate polymer (CAD Temp [CT]) with RelyX Ultimate [RUL]. Fracture load and retentive force of the crowns were measured after 24 h water storage at 37 °C and after a chewing simulation followed by thermocyclic aging. Abrasion was recorded by matching stereolithography-data of the crowns obtained before and after chewing simulation. Additionally, the mechanical properties and bonding capabilities of the crown and cement materials were assessed.ResultsFracture load values were significantly highest for YZ > VE = CT. Retention force values did not differ significantly between the materials. The aging procedure did not affect the fracture load values nor the retention force significantly. Abrasion depth of the crowns was lowest for YZ followed by VE and CT. On unpolished crowns, abrasion of YZ and VE tended to be higher than on polished specimens.ConclusionsBased on the obtained in-vitro results, all tested materials can be recommended for the use on zirconia implants, although CT is only approved for temporary crowns. The loading and retention capacity of the materials were not significantly affected by aging.  相似文献

Fractured implant abutment screws can be retrieved with repair or rescue devices. However, whether the screw was completely retrieved or the inner implant body was damaged may be uncertain. A silicone replica technique was recently implemented in Bern and may be the most predictable method available at the moment for clinically assessing the internal implant body. Demineralized teeth, which were cleared and hardened in xylene, were used in the teaching of preclinical endodontics. The teeth appeared well suited for the purpose.
