BackgroundConventional total knee arthroplasty (CONV-TKA) inevitably perturbs femoral medullary canal, disturbs medullary micro-architecture and increases blood loss and inflammatory responses. We hypothesized that avoidance of intramedullary violation may lower the incidence of periprosthetic joint infection (PJI). The aim of this study was to verify whether computer-assisted total knee arthroplasty (CAS-TKA) lowers the incidence of PJI as compared with CONV-TKA.MethodsA propensity score matching study of 5342 patients who underwent CAS-TKA (n = 1085) or CONV-TKA (n = 4257) for primary osteoarthritis of the knee from 2007 to 2015 in our institute was performed. Patients who underwent CAS-TKA were matched to those who received CONV-TKA at a 1:2 ratio according to demographics and comorbidities. PJI was defined according to the Musculoskeletal Infection Society diagnostic criteria from the 2013 International Consensus Meeting.ResultsAfter controlling potential risk factors, the use of CAS-TKA resulted in a lower incidence of PJI as compared with CONV-TKA [adjusted hazard ratio (HR), 0.42; 95% confidence interval (CI), 0.18–0.99]. The same trend in PJI reduction was observed with the usage of CAS-TKA under sensitivity testing [HR, 0.33; 95% CI, 0.12–0.95]. The cumulative incidence of PJI was lower in the CAS-TKA group than the CONV-TKA group (log-rank test, p = 0.013).ConclusionAvoidance of intramedullary violation during TKA may play a pivotal role in lowering the incidence of PJI. The use of CAS-TKA can reduce the incidence of PJI, with a better survival rate in terms of being free of PJI, as compared with CONV-TKA.Level of evidence III.  相似文献

Background

The optimal type, characteristics, and success rates of articulating antibiotic spacers used during total knee arthroplasty (TKA) periprosthetic joint infection (PJI) have not been well defined in a single series. We sought to (1) determine the success rate for three unique spacer constructs and (2) evaluate any microbiological, surgical, or patient characteristics that would influence the success rate.

Methods

We retrospectively reviewed patients who underwent a two-stage exchange for a TKA PJI with a prefabricated spacer (PREFAB), home-made mold (MOLD), or autoclaved femoral component (AUTOCL). Patient demographics, microbiology data, amount of antibiotic in each spacer construct, postoperative course, and infection cure outcomes were evaluated.

Results

The success rate for being infection free at final follow-up without the need for further reoperation for infection was 82.7% in the PREFAB group, 88.4% in the MOLD group, and 79.4% in the AUTOCL group (p = 0.54). There was no clear statistical link between raw quantities of vancomycin and aminoglycoside in the spacer and a successful outcome. The surgeon's own intraoperatively created mold group had the lowest construct cost at a mean $1341.00 ± 889.10 (p < 0.0001) per construct, while the commercial cement molds had the highest mean cost at $5439.00 ± 657.80 (p < 0.0001).

Conclusions

There was no statistically significant difference in the success rates between the antibiotic spacer types. The surgeon's own intraoperative mold had the least overall associated cost.  相似文献

BackgroundThis study aimed to clarify the association between types of knee arthroplasty (KA) (total knee arthroplasty (TKA) or unicompatmental knee arthroplasty (UKA)) and surgical site infection (SSI) with adjustment for various factors, using a Japanese national database.MethodsData on 181,608 patients who underwent unilateral primary KA for osteoarthritis from 2010 to 2017 were obtained from the Japanese Diagnosis Procedure Combination database. SSI was identified based on International Classification of Diseases 10th Revision codes. Deep SSI (i.e. periprosthetic joint infection (PJI)) was identified as SSI treated with surgical procedures. Multivariable logistic regression analyses for SSI and PJI were performed, in which dependent variables included types of KA, patient backgrounds (sex, age, body mass index (BMI), smoking status, comorbidities), and seasonality.ResultsEight percent of analyzed patients underwent UKA, while 92% underwent TKA. The proportions of SSI and PJI after UKA were 0.9% and 0.3%, respectively, both of which were lower than those after TKA (1.9% and 0.6%) (P < 0.001). Multivariable analyses showed lower proportions of SSI for UKA (adjusted odds ratio, 0.47; 95% confidence interval, 0.37–0.60; P < 0.001) and PJI (adjusted odds ratio, 0.47; 95% confidence interval, 0.34–0.65; P < 0.001) than TKA. Other factors associated with both SSI and PJI included male sex, BMI >30 kg/m2, renal dysfunction and summer season.ConclusionUKA was associated with lower proportions of SSI and PJI than TKA. Surgeons should carefully consider the indication of UKA before performing TKA, especially in patients with knee unicompartmental osteoarthritis who are at a high risk for SSI or PJI.  相似文献

目的探讨旋转平台全膝置换治疗重症膝关节骨性关节炎的临床疗效。方法对2007年1月~2009年10月我院34例(41膝)人工旋转平台全膝置换术进行临床分析和总结,行HSS[5,6]膝关节评分系统进行分析。结果术后平均随访17个月(5~31个月),手术前HSS(thehospitalforspecialsurgery)评分平均51分,手术后评分平均92分,手术优良率为91%。病人术后在膝关节疼痛,功能和活动度方面有明显改善。结论旋转平台全膝置换术对治疗重症膝关节炎有良好的疗效。  相似文献

目的 比较膝关节单髁置换术(unicompartmental knee arthroplasty, UKA)与全膝人工关节置换术(total knee arthroplasty, TKA)治疗膝内侧单间室骨性关节炎的早期膝关节功能评分与关节遗忘度评分,评价并分析两类术式在早期关节遗忘度评分方面的差异。方法 回顾性分析上海中医药大学附属光华医院关节外科2016年10月至2020年10月期间行全膝关节置换术及膝关节单髁置换术患者共296例,根据不同手术方式进行分组,其中TKA组共144例,UKA组共152例。出院后对患者术后1、3、6、12、24个月进行门诊复查,进行关节遗忘度评分、膝关节功能HSS评分、目测类比评分(VAS评分)。结果 (1)296例患者获得24~37个月的随访,平均随访(30.41±6.93)个月;(2)术后24个月TKA组与UKA组的HSS评分与VAS评分均明显好于术前,组内比较差异具有统计学意义(P<0.05);(3)两组患者术后24个月膝关节VAS评分的改善程度差异无统计学意义(P>0.05),UKA组的HSS评分改善程度好于TKA组,且差异有统计学意...  相似文献

目的 依据髌周解剖学特点,探讨全膝关节置换术(total knee arthroplasty,TKA)中应用髌周电灼去神经化的临床效果。 方法 纳入82名诊断为骨性关节炎的患者(91膝),予行双侧或单侧不置换髌骨的TKA,按随机对照原则将病人分为两组,共有41名实验组患者(45膝)在TKA中接受了髌周去神经化处理,41名对照组患者(46膝)未做该处理。手术主刀为同一骨科医师,均使用相同的膝关节假体系统。主要评价项目包括膝关节KSS评分、Western Ontario and McMaster Universities(WOMAC)、Feller髌骨评分及VAS评分。 结果 82名患者术后均获随访,平均随访时间为12个月,两组病人的膝关节KSS评分、WOMAC、Feller髌骨评分及VAS评分均无显著统计学差异(P>0.05)。 结论 在TKA中行髌周电灼去神经化,不能显著改善病人的预后。  相似文献

We report a series of 16 consecutive total knee arthroplasty (TKA) revision procedures for deep infection, treated with a newly developed intraoperatively moulded PMMA cement-prostheses-like spacer (CPLS). The standard treatment consisted of a two-stage protocol with initial explantation of the infected components combined with radical debridement, followed by implantation of a temporary cement spacer and final reimplantation of a new TKA. A sterilizeable Teflon tapered aluminium mould was developed for production of a custom made CPLS during the intervention. Stable implantation of the CPLS was achieved with a second cementation, allowing for correct alignment and ligament balancing. The spacer remained 3.5 months on average until reimplantation of a TKA occurred. At time of reimplantation, patients had an average KSS score of 84.44 points with an average flexion capacity of 102°. There was no recurrent infection during the study period of minimum 2 years. With this new technique, a low friction articulation with good stability, high comfort and a better range of motion compared to handcrafted spacers was achieved. The use of this spacer is a time sparing, cheap and convenient option in 2-stage TKA revision.  相似文献

BackgroundHemarthrosis after total knee arthroplasty (TKA) is a relatively rare complication. Although most cases are effectively treated with conservative therapy, some cases require angiographic embolization or surgical intervention. Angiosarcoma is a rare malignant tumor derived from the vascular endothelium with neovascular hyperplasia and mainly arises in the skin and superficial soft tissue, and less frequently in deep soft tissue and bone. Although malignant neoplasms such as angiosarcoma in the vicinity of orthopedic implants were reported, the causal relationship between development of the malignant tumor and the orthopedic implant is widely debated in the literature.Case presentationWe report the case of a 68-year-old female with angiosarcoma that developed in the knee joint 2 years after revision TKA. The patient exhibited severe persistent bleeding, which reached 1000–1400 ml per day for 4 months. Histological analysis of the synovial tissue in the knee joint showed large cells with nuclear atypia. Immunohistochemical staining showed cells that were positive for CD31, CD34, and D2-40, and she was diagnosed with angiosarcoma. The patient underwent an amputation at the level of the thigh, and her general condition immediately improved after the operation. The patient did not exhibit bleeding from the site of amputation, and no local recurrence or distant metastases were detected 1 year after the amputation.ConclusionsTo the best of our knowledge, this represents the first report of angiosarcoma 2 years after revision TKA. Further careful follow up is needed, given the high-grade malignancy.  相似文献

BACKGROUND: Periprosthetic infection of the knee is one of the most seriouscomplications after total knee arthroplasty. Second-stage revision is recognizedas the “gold standard” of treatment. The placement of antibiotic cement spacerduring the interval of the second operation not only increases theconcentration of antibiotics, but also increases the joint activity ofpatients. At present, the fabrication methods of spacer are different. Thereare some shortcomings such as low articular surface fit, low fabricationaccuracy, unstable structure and low strength, which cannot well fit thepatient’s physiological structure. ObjectivesKnowledge of the microbiological aetiology of periprosthetic joint infection (PJI) is essential to its management. Contemporary literature from the United States on this topic is lacking. This study aimed to identify the most common microorganisms associated with types of arthroplasty, the timing of infection, and clues to polymicrobial infection.MethodsWe performed an analytical cross-sectional study of patients 18 years of age or older with hip or knee PJI diagnosed at our institution between 2010 and 2019. PJI was defined using the criteria adapted from those of the Musculoskeletal Infection Society. Cases included PJI associated with primary or revision arthroplasty and arthroplasty performed at our institution or elsewhere.ResultsA total of 2067 episodes of PJI in 1651 patients were included. Monomicrobial infections represented 70% of episodes (n = 1448), with 25% being polymicrobial (n = 508) and the rest (5%, n = 111) culture-negative. The most common group causing PJI was coagulase-negative Staphylococcus species (other than S. ludgunensis) (37%, n = 761). The distribution of most common organisms was similar regardless of arthroplasty type. The S. aureus complex, Gram-negative bacteria, and anaerobic bacteria (other than Cutibacterium species) were more likely to be isolated than other organisms in the first year following index arthroplasty (OR 1.7, 95%CI 1.4–2.2; OR 1.5, 95%CI 1.1–2.0; and OR 1.5, 95%CI 1.0–2.2, respectively). The proportion of culture-negative PJIs was higher in primary than revision arthroplasty (6.5% versus 3%, p 0.0005). The presence of a sinus tract increased the probability of the isolation of more than one microorganism by almost three-fold (OR 2.6, 95%CI 2.0–3.3).ConclusionsJoint age, presence of a sinus tract, and revision arthroplasties influenced PJI microbiology.  相似文献

Background

There are many uncertainties about the advantages and disadvantages of using unicompartmental (UKA) versus total knee arthroplasty (TKA) to treat patients with knee osteoarthritis. It is important to have sufficient early postoperative quadriceps strength for long-term, self-reported and gait-related outcomes after knee arthroplasty, but very limited comparative data exist regarding UKA and TKA patients.

Methods

This study assessed isometric quadriceps strength, spatio-temporal gait parameters (walking speed, step length, single-limb support phase) and self-reported outcomes (pain, function, stiffness) in 18 TKA and 18 UKA patients six months after surgery, as well as in 18 healthy controls.

Results

Quadriceps strength of TKA, but not of UKA patients, was lower than that of controls (P?<?0.05). UKA patients demonstrated better gait function in terms of a longer single-limb support phase than TKA patients (P?<?0.01), which agreed with better self-reported pain (P?<?0.05), function (P?<?0.01) and stiffness (P?<?0.05) scores compared to TKA patients.

Conclusions

Six months after surgery, UKA patients showed better short-term quadriceps strength and gait function compared to TKA patients, together with less self-reported knee pain and stiffness. Patients eligible for UKA may experience less functional impairments compared to those who require TKA.  相似文献

The records of 26 patients (27 knees) who sustained a fracture of a total condylar design total knee prosthesis were reviewed. Twenty-five fractures involved the tibial component and two involved the femoral component. Mean time to failure was 8.2 years for tibial components and 7.2 years for femoral components. The overall fracture prevalence rates were 0.33% and 0.03% for tibial and femoral components, respectively. Specific designs had increased fracture rates and revision arthroplasty increased the relative risk. Anatomic limb alignment exceeded 5 of varus or valgus in 24 of 25 knees with tibial component fractures. Lucencies were noted beneath all but one fractured tibial tray. However, 22 of 23 tibial components that had a keel or stem were well fixed distally and subsidence occurred only beneath the fractured portion of the prosthesis. Tibial component fractures are related to revision arthroplasty, prosthesis design, malalignment, and inadequate support beneath the tibial tray with good distal fixation.  相似文献

BackgroundNo data have demonstrated how joint gap measured under a distraction force is actually associated with mediolateral laxity evaluated under a varus–valgus force during total knee arthroplasty (TKA). This study aimed to investigate the correlations between them using a navigation system.MethodsA total of 113 primary navigated TKAs were included. After bone resection and soft-tissue balancing, the component gap was measured with a distraction force of 60 N and 80 N for both the medial and lateral compartment (i.e. a total of 120 N and 160 N) at 0°, 10°, 30°, 60°, 90°, and 120° knee flexion. After the final prosthetic implantation and capsule closure, mediolateral laxity under a maximum varus–valgus stress was recorded with image-free navigation at each knee flexion angle. The correlation between joint gap laxity (total differences between component gap and insert thickness in the medial and lateral compartment) and mediolateral laxity was analyzed using Spearman’s rank correlation coefficient.ResultsThe joint gap laxity under both distraction forces showed significant positive correlations with mediolateral laxity at 10°, 30°, 60°, and 90° flexion, whereas no correlation was observed at extension and 120° flexion. The correlations were stronger in gap measurement under 80 N than 60 N at all examined ranges. In patients with body mass indexes (BMIs) ≥ 30 kg/m2, the correlation became non-significant.ConclusionIntraoperative joint gap laxity was associated with mediolateral laxity after TKA, especially at mid-flexion angles. The factors weakening the correlations were a lower applied distraction force for gap measurement and a larger BMI.  相似文献

BackgroundFemoral component fracture is a rare complication of total knee arthroplasty (TKA).CaseWe report a case of oxidized zirconium (Oxinium) femoral component fracture after total knee arthroplasty. The fracture site was the junction of the central and medial flanges. The patellar component and polyethylene insert had delamination at the contact point of the fracture line, and the tibial tray had loosening at the medial side. There was no cement adherence at the component fracture site, suggesting that debonding had occurred at the cement-implant interface in this area. Examination with a scanning electron microscope revealed beach marks, which are characteristic findings of metal fatigue.ConclusionWe considered that the cause of femoral component fracture was a fatigue fracture due to poor fixation of the component to the bone caused by poor osteotomy technique or poor cementing technique. To our knowledge, this is the first case of Oxinium femoral component fracture.  相似文献

The aim of this retrospective study was to identify and evaluate complications after hip spacer implantation other than reinfection and/or infection persistence.Between 1999 and 2008, 88 hip spacer implantations in 82 patients have been performed. There were 43 male and 39 female patients at a mean age of 70 [43 - 89] years. The mean spacer implantation time was 90 [14-1460] days. The mean follow-up was 54 [7-96] months. The most common identified organisms were S. aureus and S. epidermidis. In most cases, the spacers were impregnated with 1 g gentamicin and 4 g vancomycin / 80 g bone cement.The overall complication rate was 58.5 % (48/82 cases). A spacer dislocation occurred in 15 cases (17 %). Spacer fractures could be noticed in 9 cases (10.2 %). Femoral fractures occurred in 12 cases (13.6 %). After prosthesis reimplantation, 16 patients suffered from a prosthesis dislocation (23 %). 2 patients (2.4 %) showed allergic reactions against the intravenous antibiotic therapy. An acute renal failure occurred in 5 cases (6 %). No cases of hepatic failure or ototoxicity could be observed in our collective. General complications (consisting mostly of draining sinus, pneumonia, cardiopulmonary decompensation, lower urinary tract infections) occurred in 38 patients (46.3 %).Despite the retrospective study design and the limited possibility of interpreting these findings and their causes, this rate indicates that patients suffering from late hip joint infections and being treated with a two-stage protocol are prone to having complications. Orthopaedic surgeons should be aware of these complications and their treatment options and focus on the early diagnosis for prevention of further complications. Between stages, an interdisciplinary cooperation with other facilities (internal medicine, microbiologists) should be aimed for patients with several comorbidities for optimizing their general medical condition.  相似文献

Background

The goals of this study were (1) to document the gait pattern of patients with unilateral knee osteoarthritis (OA), (2) to determine the knee adduction moment (KAM) changes in the non-operated knee, and (3) to identify the predictors of change in KAM in the non-operated knee.

Methods

The study recruited 23 patients with advanced unilateral knee OA. The preoperative Kellgren–Lawrence (KL) grade of the non-operated knee was one or two; non-operated knee pain, rated using a numeric rating scale (NRS), was less than three out of 10 points. We used a commercial gait analysis system to evaluate kinetics and kinematics. Radiological and gait measurements at the two-year follow-up were compared with baseline data.

Results

The preoperative asymmetrical gait cycle characterized by elongation of the stance phase of the non-operated knee became symmetrical after TKA. The average KAM of the non-operated knee increased (P = 0.010) and it was best predicted by the baseline mechanical axis of the non-operated knee. If the baseline mechanical axis was varus four degrees or above (varus group), the average KAM increased by 0.64 (% body weight × height, P = 0.015), while for varus less than four degrees (non-varus group), KAM was unchanged. The KL grade was increased in the varus group (P = 0.020) but it was unchanged in the non-varus group. Average pain NRS score was also higher (P = 0.044) in the varus group.

Conclusions

Close follow-up is necessary for patients with a baseline varus deformity of the non-operated knee because of the higher possibility of developing pain, subsequent arthritis and increased joint loading of the non-operated knee.

BackgroundPostoperative radiographs are commonly ordered after primary total knee arthroplasty (TKA), however, there is limited data on how often these films change management over the entire postoperative time course, and what should prompt imaging to maximize clinical utility.MethodsA retrospective cohort study was conducted of patients ≥ 18 years old who underwent a primary TKA at two level one trauma centers. Postoperative data were collected to determine the frequency of postoperative radiograph series, radiograph findings that did not suggest normal healing or alignment to radiologist and orthopedists, and changes in postoperative management. The total cost and radiation exposure values were calculated for all patient radiographs using estimates from previous literature.ResultsFrom the 1258 patients included, 3831 postoperative radiographs were taken (mean ± 95% confidence interval [CI]: 3.05 ± 0.11 radiographs per patient). Of these 3831 radiographs, 44 (1.1%) contained a positive radiographic finding. Only 13 (0.3% of radiographs) of these positive radiographic findings were positive orthopaedic findings, 11 of which led to changes in management. For all but 1 of these patients (10/11, 91%), these radiographs were taken during a non-routine postoperative visit. Routine postoperative radiographs that did not change management cost $1,008,480 and administered 22.92 mSV of radiation to patients within this study.ConclusionPostoperative radiography obtained after primary TKA were of low clinical utility yet resulted in considerable healthcare costs and unnecessary radiation burden. Radiographs ordered during a non-routine visit, however, were a reliable indicator of when this imaging provided clinical utility.  相似文献
