AimTo investigate the root and canal morphology of maxillary and mandibular permanent molar teeth in a Caucasian population by using Cone Beam Computed Tomography (CBCT).MethodsA total of 596 permanent molars were included. The number of roots, the number of canals per root and the root canal configurations according to the method of Vertucci were recorded.ResultsAlmost all of maxillary first molars (95.7%) had three separate roots; however, 40.3% of mesiobuccal roots had two canals (MB2). Of 157 maxillary second molars, 88.5% had three roots. Among the mesiobuccal roots, 15.1% had two canals. The majority of mandibular molars (100% of first molars, 89.4% of second molars) had two separate roots. Most distal roots had a simple type I configuration, whereas mesial roots had more complex canal systems, with more than one canal.ConclusionsCBCT is an efficient method of studying root canal systems.  相似文献

This in vitro study investigated the presence of second (MB2) root canals and the number of apical foramina found in mesiobuccal roots of the permanent maxillary first molars; in addition, the study evaluated the effectiveness of magnification to detect MB2 canals. One hundred extracted maxillary first molars were collected from Jordanian patients. The number of roots (as well as their morphology) was investigated. The number of canal orifices in mesiobuccal root was assessed, with and without magnification, and findings were compared. The number of apical foramina and the distance between mesiobuccal and second canal orifices were investigated under magnification. Of the 97 specimens that were subject to additional investigation, all teeth had three separated roots, except for three specimens that had either mesiobuccal or distobuccal roots fused with the palatal root. The number of MB2 canals that were detected increased from 55 (56.7%) to 61 (62.9%) teeth when magnification was used. Within the limits of this in vitro study, it was concluded that the Jordanian population had a high percentage of MB2 canals in the mesial buccal roots. Moreover, the use of clinically used magnifying devices increased the number of MB2 canals detected.  相似文献

Aim To identify the number of roots and canal configurations in permanent first molars of the indigenous Kuwaiti sub‐population and compare them against a similar group of non‐Kuwaiti population in different age groups and gender, and to determine the frequency of a second canal in the mesiobuccal root of maxillary first molars and distal root of mandibular first molars that could be located and treated in routine practice without using magnification or special lighting conditions. Methodology A total of 220 permanent first molar teeth of patients scheduled for root canal treatment over a period of 1 year were included. Patients were identified and grouped according to their nationality as Kuwaiti and non‐Kuwaiti (which included Filipinos, Indonesian Indians, Srilankans, Egyptians, Saudi Arabians and Syrians). In both nationality groups, patients were successively distributed into four groups based on their age. The first age group included patients below 20 years, the second 21–30 years, the third 31–40 years and the fourth were over 40 years. The first three groups comprised of 15 male and 15 female patients whilst the fourth group had 10 patients of each gender. Teeth with open apices, resorptions and calcification were excluded from the study. The teeth included were both clinically and radiographically examined for number of roots, the canal configuration and the presence of the additional mesiobuccal and distal canals and recorded. The simplified classification of canal configurations proposed by Weine was utilized. Results The incidence of a second canal in the mesiobuccal (MB) root of maxillary first molars and the distal root of mandibular first molars was not influenced significantly by nationality and gender. All the 110 maxillary first molars treated had three roots; 58% of MB root had one canal and 42% had two canals. The majority of the mesial roots had Weine type II canal configuration when the second mesiobuccal canal was present. All distobuccal and palatal roots had a single or type I canal configuration. Of the 110 mandibular first molars that were treated, 96% had two roots and 4% had three roots. When present, the third root was located either buccal or lingual to the main distal root. Overall 51% of the single distal roots had one canal whilst 49% had two canals. The mesial root frequently had a Weine type II canal configuration as did the distal root when a second canal was present. Pearson’s correlation analysis of both molars revealed a significant (P < 0.05) negative correlation (r = ?0.274, r = ?0.144) between age and number of canals as well as the type of canal. Conclusions The incidence of a second canal in distal roots of permanent mandibular first molars was 49% in the Kuwaiti population and this was similar to other Asian ethnic populations. Adopting modified access and troughing procedures revealed a 42% frequency of MB2 canals in maxillary first molars. The incidence of a second canal in both mesiobuccal roots of maxillary molars and distal roots of mandibular molars decreased significantly with age; no differences were noticed amongst the nationalities and gender studied. The possibility of extra roots should be anticipated in mandibular molars.  相似文献

ObjectivesDentin thickness in concave areas of the root creates risk for complications such as strip perforation during endodontic treatment. The study aims to examine dentin thickness of the danger (DZ) and safety zone (SZ), canal configuration, and the presence of isthmus in the mesiobuccal root of maxillary molars.Material and MethodsCone-beam Computed Tomography (CBCT) images of 1251 teeth belonging to 642 patients were retrospectively reviewed. The dentin thicknesses at DZ and SZ in maxillary molars with one (MB) or two mesiobuccal canals (MB1, MB2) were measured at the 3 mm apical to the furcation level. Vertucci''s canal configurations and the isthmus rate were recorded. The Chi-square test andThe Student’s t-test were performed.ResultsMB2 rate was higher in maxillary first molars (61.68%) than second molars (39.36%). Isthmus rates were 27.3% and 44.11% in first and second molars. DZ thickness was thinner than the dentin thickness in the SZ in both first and second molars with one or two mesial canals (p< 0.05). In teeth with single canal, the mean DZ thickness was 0.88mm. In teeth with two canals, the mean DZ thicknesses were 0.83mm and 0.80mm for MB1 and MB2 canals, respectively.ConclusionMB2 rate was higher in the first molar (61.68%), and the isthmus rate was higher in the second molar (44.11%). DZ and SZ were thinner in MB2 than in MB1 at the maxillary molars with two mesial canals. The results indicated that more conservative preparation must be applied to the MB2 canal in the maxillary molars.  相似文献

目的:通过锥体束CT(cone--beam computed tomography,CBCT)成像系统分析上颌第一磨牙近中颊根根管形态及第二(second mesiobuccal,MB2)根管的发生率。方法:选择95名进行CBCT扫描的患者,共计180颗上颌第一磨牙,观察近颊根根管形态、MB2根管的发生率、左右侧发生比率及男女发生比率差异。结果:上颌第一磨牙近颊根管形态以Ⅲ型为主,MB2根管发生率为79.4%,左右侧发生比率无统计学差异(P=0.846),男性发生率明显高于女性(P=0.043)。结论:上颌第一磨牙近颊根MB2根管的发生率较高,可通过CBCT有效的发现MB2根管,为临床诊疗提供依据。  相似文献

目的研究上颌第二恒磨牙近中颊根第二根管(MB2)的发生情况,为提高上颌第二恒磨牙根管治疗的成功率提供解剖学基础。方法于山东地区收集离体上颌第二恒磨牙118颗,采用斜方型开髓洞型开髓,利用小号C型锉探查并疏通根管,记录MB2的发现率及扩通率;拍摄X线牙片,记录近中颊根的根管数目、形态和类型;在根管显微镜下观察并应用数显卡尺测量近中颊根主根管和MB2根管口之间的距离,确定根管口的位置;记录近中颊根的根尖孔数目,并测量解剖根尖孔至解剖根尖的距离。结果118颗上颌第二恒磨牙中,有58颗发现MB2,发现率为49.15%;其中48颗牙齿的MB2被扩通,扩通率为82.76%。108颗3根牙中,近中颊根的根管形态为Ⅰ型者有50颗,占46.30%;Ⅱ型及Ⅲ型者分别为14和34颗,占12.96%和31.48%。近中颊根主根管口与MB2根管口的平均距离为1.26 mm;近中颊根解剖根尖孔至解剖根尖的距离平均值为1.13 mm。结论山东地区上颌第二恒磨牙MB2的发现率较高,临床治疗中采用改良的斜方型开髓孔有利于发现MB2,用X线片确定工作长度时需要结合临床综合判断根尖孔的位置。  相似文献

目的 探讨上颌第一磨牙近烦第二根管(second mesiobuccal canal,MB2)的特征及临床治疗方法。方法 选取患牙髓炎或根尖用炎的上颌第一磨牙95颗,拍摄术前X线片,术中采用小号K锉配合15%EDTA溶液探查根管,记录根管数目、形态和类型;采用机动镍钛器械Hero642进行根管预备,侧向加压充填技术充填根管。结果上颌第一磨牙MB2的发现率为81.1%,扩通的MB2占72.6%,多数MB2根管细小弯曲,需用小号锉配合使用EDTA进行疏通,机动镍钛器械预备根管无明显并发症。结论上颌第一磨牙MB2的发生率高,多数根管细小弯曲,小号K锉配合EDTA疏通根管,机动镍钛器械预备可获得良好的治疗效果。  相似文献

目的 观察上颌第一磨牙的根管系统。方法 制备50个上颌第一磨牙透明牙标本,显露根管系统的空间构型,在体视显微镜下观察并记录根管系统的形态,结合应用加以统计。结果 ①上颌第一磨牙近中颊根单根管率为66%,其中Ⅰ型占38%,双根管率为34%,远中颊根Ⅰ型占74%,腭根Ⅰ型占94%。②根管侧支、根尖分歧、根尖分叉多见于近颊根,在近颊根的发生率为40%, 远颊根的发生率为24%,腭根的发生率为6%,发生部位多见于根尖1/3。结论 上颌第一磨牙近颊根的根管系统最为复杂,根管变异较远颊根多,腭根变异最少。  相似文献

目的 观察临床治疗中上颌第二乳磨牙根管数目.方法 选取需行上颌第二乳磨牙根管治疗的3~8岁患儿204例,共216颗患牙.常规方法开髓、制备开髓洞型,使髓室底完全暴露,用小号锉结合17%乙二胺四乙酸(ethylene diamine tetraacetic acid,EDTA)溶液探查各根管口的位置,确定并记录根管数目,根管预备完成后采用碘仿氧化锌糊剂进行根管充填.结果 216颗上颌第二乳磨牙中,122颗为3根管(占56.5%);94颗为4根管(占43.5%),根管数目变异以近中颊根第二根管(second mesiobuccal canal,MB2)出现频率较高,占比41.2%.上颌第二乳磨牙MB2的发现率在性别(X2=1.004,P=0.316)及牙位方面的差异(X2=0.072,P=0.788)没有统计学意义.结论 上颌第二乳磨牙根管数目存在较多变异,MB2发现率较高.  相似文献

目的:探讨运用根管显微镜对经治的上颌第一磨牙遗漏近颊第二根管(MB2)的诊治方法。方法:选择需行根管再治疗的上颌第一磨牙64个。依据x线片特征,牙冠与髓底的形态特点判断是否存在MB2。采用斜方形开髓入口,小号K锉探查,必要时使用根管手术显微镜定位MB2根管。镍钛机用根管锉冠向下法预备根管,1%NaOCL冲洗根管系统,冷牙胶侧方加压与热牙胶垂直加压充填根管。记录近颊根根管分型、根管预备过程中并发症的情况,评价疗效。结果:再治疗中共发现MB2根管55个(85.81%),疏通45个(80%)。近颊根双根管以2—1型、2—2型较常见(76.56%)。治疗过程中未发生器械折断、管壁侧穿等并发症,治疗成功率为95.55%。结论:上颌第一磨牙MB2发生率高,通常狭窄、弯曲、隐蔽,是临床治疗的难点;临床医生有意识地寻找和在显微镜下的可视化操作能提高MB2根管的发现率和治疗的成功率。  相似文献

目的:观察山东地区人上颌第二恒磨牙牙根及根管形态的解剖特点。方法:于山东地区收集离体上颌第二恒磨牙118个,观察牙根形态及类型;应用透明牙标本法观察根管形态并按Vertucci八分类法对其进行分类。结果:118个上颌第二恒磨牙中以3根分离者为主,占91.52%,牙根融合者共10例,占8.47%,其中融合成2根者为5.08%,主要为近中颊根与远中颊根融合;融合成单根者为3.39%。透明牙标本法下观察根管系统,上颌第二恒磨牙远中颊根及腭根根管以Ⅰ型为主;近颊根根管仅44.95%为Ⅰ型,近颊根第二根管(the second mesiobuccal canal,MB2)的发生率达51.38%。结论:山东地区上颌第二恒磨牙的牙根具有多样性,以3根分离根形态为主,同时可发生各种融合根;其根管形态复杂,MB2的发现率较高。  相似文献

目的:评价改进开髓口和应用根管显微镜(DOM)定位离体上颌第一、二磨牙近颊根第二根管(MB2)的能力.方法:收集离体上颌第一、二磨牙共550颗.在肉眼、传统三角形开髓口(N.E-1),肉眼、改进开髓口(N.E-2)和根管显微镜辅助(DOM)三种不同条件下分别探查根管,记录MB2数目.结果:N.E-1、N.E-2和DOM三种不同条件下,MB2发现率在上颌第一磨牙分别为34.26%、51.85%和78.24%;第二磨牙分别为16.17%、23.65%和41.32%.改进开髓口和使用DOM可显著提高MB2发现率,与传统开髓口间的差别有统计学意义,P<0.05.结论:建议将上颌磨牙开髓口由传统的三角形改为斜四边形,并使用合适的放大技术和增强照明,以提高MB2发现率和治疗率.  相似文献

目的运用透明标本观察法、螺旋CT薄层扫描技术和牙根硬组织切片观察法研究中国人离体上颌第一、二磨牙近中颊根第二根管(MB2)的出现率。方法分别收集离体上颌第一磨牙216例、第二磨牙334例,均随机分为两组:A组制成透明标本;B组标本经螺旋CT扫描后,自根尖开始每隔1mm横切牙根。所有离体牙标本均在牙科手术显微镜下观察并记录近中颊根的根管数目。结果透明标本观察法所得的上颌第一、第二磨牙MB2出现率分别为81.48%和49.70%;螺旋CT扫描技术所得的结果分别为77.78%和47.31%;牙根硬组织切片法所得的结果分别为88.89%和53.89%。上颌第一磨牙MB2出现率高于第二磨牙,其差异有统计学意义(P〈0.05)。同一牙位不同实验方法间的结果差异均无统计学意义(P〉0.05)。结论上颌第一、二磨牙MB2出现率高,提示临床医师应努力探查该根管,以提高上颌磨牙根管治疗的成功率。  相似文献

AIM: The purpose of the present ex vivo study was to investigate variations in the root canal systems of mandibular and maxillary first permanent molar teeth of South Asian Pakistanis. METHODOLOGY: The root canal systems of a sample of 30 mandibular and 30 maxillary first permanent molar teeth extracted from South Asian Pakistanis were studied using a clearing technique. RESULTS: The mesial roots of the mandibular molar teeth typically presented with two canals (97%) of type II, IV or VI configuration. The distal roots of these teeth presented with a single canal (50%) of type I or V configuration or with two canals (50%) of type II, IV or VI. The prevalence of four root canals in two-rooted mandibular first permanent molar teeth was 47%. In maxillary molar teeth the mesial roots with a single canal (47%) were type I or type V; those with two canals (53%) were type II, IV or type VI. The distal and palatal roots that presented as a single canal (100%) were type I or type V configuration. The prevalence of four root canals in three-rooted maxillary first permanent molar teeth was 53%. CONCLUSION: It is concluded that four root canals in mandibular and maxillary first permanent molar teeth of South Asian Pakistanis is a common occurrence. The distribution of the different configurations of root canal systems in this population differed from that in Caucasian groups, suggesting that variations in root canal systems may be attributed to racial divergence.  相似文献

The purpose of this in vitro study was to investigate the canal configuration types, and the prevalence and location of anatomical variations in the mesiobuccal (MB) and mesial roots of permanent maxillary and mandibular first molars after instrumentation. The number and the type of canals were determined before instrumentation using conventional methods. All root canals from the 47 MB roots and 42 mesial roots were then instrumented to size #30 with ProFile .04 taper rotary instruments in a crown-down method and then filled with a single gutta-percha cone and sealer. Transverse 1 mm-thick cross-sections at 2, 3, 4, and 5 mm from the apex were obtained, stained and examined using a stereomicroscope. The canal configuration types and the prevalence and location of isthmi and accessory canals in roots with two canals were evaluated. The prevalence of two canals was 80.8% in the maxillary MB roots and 95.2% in the mandibular mesial roots. There were six types of canal configurations in the instrumented root apices. The prevalence of anatomical variations was highest at the apical 4 mm level, and was more frequent in mandibular first molars, and in roots with Weine type III canal. chi test showed that the prevalence of the anatomical variations was statistically higher in the maxillary MB roots with Weine type III canals than in those with Weine type II canals (p < 0.05). Different canal configurations were often found at different levels in the same root. The results indicate that anatomical variations persist following instrumentation of roots with two canals in first molars. These anatomical variations should be considered during surgical or nonsurgical endodontic procedures of the permanent first molars.  相似文献

Introduction

The aim of this study was to analyze and characterize root canal morphology of maxillary molars of the Brazilian population using cone-beam computed tomographic (CBCT) imaging.

Methods

Patients referred for a CBCT radiographic examination for accurate diagnosis and treatment planning were enrolled in the study. A total of 620 healthy, untreated, fully developed maxillary first and second molars were included (314 first molars and 306 second molars). The following observations were recorded: (1) number of roots and their morphology, (2) number of canals per root, (3) fused roots, and (4) primary variations in the morphology of the root canal systems.

Results

First and second molars showed a higher prevalence of 3 separate roots, mesiobuccal, distobuccal, and palatal, with 1 canal in each root (52.87% and 45.09%, respectively). Two canals in the mesiobuccal roots represented 42.63% of teeth, whereas mesiobuccal roots of second molars presented 2 canals in 34.32%. The most common anatomic variation in the maxillary first molar was related to the root canal configuration of the mesiobuccal root, whereas the root canal system of the maxillary second molar teeth showed more anatomic variables.

Conclusions

Mesiobuccal roots of maxillary molar teeth had more variation in their canal system than the distobuccal or palatal roots. The root canal configuration of the maxillary second molars was more variable than the first molars in a Brazilian population. CBCT imaging is a clinically useful tool for endodontic diagnosis and treatment planning.  相似文献

AIM: The aim of this investigation was to study the root canal anatomy of maxillary first and second molar teeth from an Irish population sample using a clearing technique. METHODOLOGY: Eighty-three extracted permanent maxillary right first molars and 40 permanent right maxillary second molars were included in this investigation. The specimens were demineralized and then cleared using methyl salicylate. The following observations were made: number of roots, prevalence of fusion, types of root canals using Vertucci's classification, presence and position of lateral canals, presence and position of transverse anastomoses, number and position of apical foramina and the frequency of occurrence of apical deltas. RESULTS: Eleven per cent of maxillary first molars and 43% of maxillary second molars had fused roots. A total of 78% of mesiobuccal roots in maxillary first and 58% in maxillary second molars had two canals. Sixty-two per cent of maxillary first and 50% of maxillary second molars had two apical foramina. There was a significant inverse relationship between age and the occurrence of two canals and between age and the occurrence of transverse anastomoses in both tooth morphotypes (P < or = 0.05). CONCLUSIONS: It is concluded that a significant proportion of the first and second molar specimens studied had two canals in the mesiobuccal root (78% and 58%, respectively) and that the occurrence of two canals and transverse anastomoses decreased significantly with increasing age.  相似文献

AIM: To investigate the root and canal morphology of permanent maxillary molar teeth from a Ugandan population. METHODOLOGY: Maxillary first (n = 221) and second molar (n = 221) teeth were collected from patients attending dental clinics in Kampala. Teeth were prepared using a clearing technique: the pulp chambers were accessed and the teeth placed consecutively into 5% sodium hypochlorite, 10% nitric acid, then methyl salicylate. Indian ink was injected into the pulp chambers to demonstrate the canal system. RESULTS: In the first molars, 95.9% of the teeth had separate roots. The mesiobuccal root was fused with the palatal root in 3% of specimens and with the distobuccal root in 0.5% of teeth. In the second molars, 86% of the teeth had separate roots. The mesiobuccal root was fused with the palatal root in 6.3% of specimens and with the distobuccal root in 6.8% of teeth. Apical deltas were more frequent in the mesiobuccal root when compared with distobuccal and palatal roots of both the first and second molars. A type I canal configuration (>75%) was the most frequent in all the roots of both the first and second molars. Canal intercommunications and lateral canals were more frequent in the mesiobuccal root when compared with other roots. CONCLUSIONS: The mesiobuccal root tended to have more variations in the canal system followed by the distobuccal root, whereas the palatal root had the least. The findings in root and canal morphology of this Ugandan population were different from previous studies, which may partly be attributed to racial differences.  相似文献

Occurrence of the second mesiobuccal canal (MB2) is a frequent finding. Literary reports have shown it to be found more in the cases of the maxillary first molar. However the maxillary second molars have also been found with this variation in a number of canals.
