From mid-June through early August 1980, an outbreak of gastrointestinal illness in Red Lodge, Montana affected approximately 780 persons, as estimated from attack rates of 33 per cent and 15 per cent in urban and rural residents, respectively. Giardia lamblia was identified in stool specimens from 51 per cent of 47 persons with a history of untreated gastrointestinal illness and in 13 per cent of 24 specimens from asymptomatic persons (p = .00045, Fisher's Exact Test). The epidemic curve was bimodal with peaks in mid-June and mid-July. Each peak occurred about three weeks after an episode of very heavy water runoff resulting from warm sunny weather and snow darkened by ashfall from the Mt. St. Helens volcanic eruption of May 18, 1980. Unfiltered and inadequately chlorinated surface water was supplied by the city water system, which was implicated as the vehicle of transmission in the outbreak. Water systems providing unfiltered surface water are more likely to become contaminated during periods of heavy water runoff.  相似文献

In June 1982, an outbreak of gastrointestinal illness caused by Shigella sonnei occurred among residents of two counties in Oklahoma. A case-control study of cases and age and sex-matched controls showed an association with attendance at a southern Oklahoma lake (14/17 cases vs 3/17 controls, matched pair odds ratio [OR] 9/0, confidence interval [CI] 2.4-infinity). A survey of 85 persons who had visited the lake area showed that persons who had swum were more likely to have been ill with a gastrointestinal illness (50 per cent) than persons who had not swum (0 per cent); among those who had swum, illness was more frequent among those who reported having water in their mouths while swimming (62 per cent) than those who did not (19 per cent) (OR = 6.9, 95% CI = 2.2-21.5). No further primary lake-associated cases had onset of symptoms beyond two days of closing the reservoir. Swimming should be considered as a potential source of enteric infections.  相似文献

An outbreak of gastro-enteritis occurred in La Neuveville, township with 3358 inhabitants. A retrospective cohort study of 1915 participants showed that 1607 (84%) had been ill. Campylobacter jejuni was isolated from 28 patient faecal samples, Shigella sonnei from 21 patients and small round structured viruses (SRSV) from 6 patients. More than one pathogen was identified in eight persons. The epidemic curve was characteristic of a point-source outbreak. The risk for illness was significantly higher among persons who had drank unboiled drinking water than among those who had not (1290 [80.3%] of 1607 vs. 86 [27.9%] of 308; RR = 2.87; 95% CI 2.40-3.45). Risk increased significantly with the quantity of water consumed (P < 0.00 x 10(-6)). An SRSV isolate from water and one human faeces had an identical DNA sequence. The outbreak was due to a pump failure producing a spill of sewage into the groundwater. We conclude that transmission was waterborne and that measures including early warning, basic hygiene and sanitation improvements controlled this epidemic.  相似文献

Abstract: During the 1989 Christmas holiday period, a large outbreak of gastroenteritis occurred among persons staying at a caravan park in southern New South Wales. Review of local hospital records found that 77 per cent of patients presenting with infective diarrhoea between 29 December and 3 January had stayed at the caravan park. In a retrospective cohort study we compared rates of illness among caravan park patrons exposed to different water sources. Stools were tested for pathogens and convalescent sera for viral antibodies. Rain and reticulated river water sampled from the caravan park were tested for bacteria and viruses. Of 351 persons interviewed at the caravan park, 305 (87 per cent) reported an illness characterised by diarrhoea, vomiting and abdominal pain. Of 196 persons who used reticulated river water for drinking or ablutions, 175 (89 per cent) became ill compared with 47 of 72 persons (65 per cent) who did not use this water (relative risk 1.4, 95 per cent confidence interval 1.2 to 1.6). The outbreak was probably caused by a 27–28 nm small round structured virus found in the stool from one ill person. High levels of faecal coliforms in the reticulated river water and enterovirus in sediment samples suggest that the outbreak was caused by sewage contaminating the reticulated river water through a break in the pipe directly over the underground water tanks. To prevent such outbreaks, poor water and sewerage system layouts should be avoided and nonpotable water should be clearly labelled. Where feasible, all camping-ground water should stem from town supplies.  相似文献

An outbreak of gastroenteritis occurred in March-April 1981, in Wichita, Kansas, and involved more than 250 persons who drank raw milk from a single local dairy. Campylobacter jejuni was recovered from 60 of 116 (52%) persons in households that had one or more ill family members. A cohort study of families that belonged to a food cooperative that purchased raw milk from the implicated dairy showed a significant association between illness and having drunk raw milk. Thirty-nine of 55 (71%) persons who drank raw milk became ill compared with four of 36 (11%) persons who did not drink raw milk (p less than 0.01, t test, accounting for clustering). Peak (convalescent) antibody titers to C. jejuni, determined by indirect immunofluorescence, in 20 raw-milk drinkers showed a geometric mean of 1:27 in contrast to geometric mean titer of 1:6 in 10 well persons from the cohort who did not drink raw milk (p less than 0.002, t test). C. jejuni was recovered from 21 of 34 (66%) raw-milk drinkers, versus none of 26 people who did not drink raw milk (p less than 0.001, Fisher's exact test, one tailed). C. jejuni of the same serotype was isolated from the case-patients and from rectal swabs of cows in the dairy. These findings indicate that raw milk contaminated by Campylobacter was the vehicle for this outbreak.  相似文献

OBJECTIVES: A 1993 large water-borne outbreak of Salmonella typhimurium infections in Gideon, Mo, a city of 1100 with an unchlorinated community water supply, was investigated to determine the source of contamination and the effectiveness of an order to boil water. METHODS: A survey of household members in Gideon and the surrounding township produced information on diarrheal illness, water consumption, and compliance with the boil water order. RESULTS: More than 650 persons were ill; 15 were hospitalized, and 7 died. Persons consuming city water were more likely to be ill (relative risk [RR] = 9.1, 95% confidence interval [CI] = 2.9, 28.4), and the attack rate increased with increased water consumption. S. typhimurium was recovered from samples taken from a city fire hydrant and a water storage tower. Persons in 31% (30/ 98) of city households had drunk unboiled water after being informed about the boil water order, including 14 individuals who subsequently became ill. Reasons for noncompliance included "not remembering" (44%) and "disbelieving" (25%) the order. CONCLUSIONS: Communities with deteriorating water systems risk widespread illness unless water supplies are properly operated and maintained. Effective education to improve compliance during boil water orders is needed.  相似文献

An outbreak of Campylobacter enteritis occurred in 1982 among 11 of 15 members and friends of an extended family gathering in Colorado. Median onset for illness, characterized by diarrhea, abdominal cramps, fever and headache, was four days after the party. Illness was associated with eating undercooked chicken. Two ill persons had stool specimens positive for C. jejuni. Eight of ten ill persons tested had Immunoglobulin-M-specific indirect fluorescent antibody (IFA) titers to C. jejuni greater than or equal to 32. IFA titers to C. jejuni peaked within three weeks of exposure and decreased to control levels within three months after exposure. Inadequate cooking of poultry may increase the risk of these infections.  相似文献

An outbreak of Campylobacter jejuni enteritis occurred among people who had attended a meal where raw milk was served. A case control study was conducted using instances of illness as cases; those who attended the event but did not become ill served as controls. Thirteen of 20 people who had attended the meal became ill. C. jejuni was cultured from five of six stools that were submitted. Raw milk consumption was strongly associated with illness (p = .0072, Fisher exact test). Although C. jejuni outbreaks associated with milk can be prevented with pasteurization, they still occur in association with raw milk consumption.  相似文献

An explosive outbreak of gastrointestinal illness clinically compatible with infection by an agent serologically related to Norwalk virus agent occurred in an elementary school in May 1978. Seroconversion by radioimmunoassay to the Norwalk antigen was noted in two of three ill persons, but no viral particles were identified in stool. Illness developed in 72% of students and teachers at the school, and 32% of household contacts of these ill persons. Of household contacts of persons exposed at school but not clinically ill, 11% developed illness. This value, however, was not statistically different from the level of illness observed concurrently in household contacts of students at an unaffected school nearby. Epidemiologic investigation implicated water as the mode of transmission. Average consumption of one or more glasses per day was strongly associated with illness (p less than 0.00000001). Among soccer team members with limited school contact, water consumption at the school was associated with a 14-fold greater risk of illness (p less than 0.000001). Drinking water was most likely contaminated by back-siphonage through a cross-connection between the school's well and septic tank. This contamination occurred approximately 24 to 36 hours before the outbreak developed.  相似文献

Campylobacter infection is one of the most commonly reported foodborne diseases in Australia however, reported Campylobacter outbreaks are rare. This report describes such an outbreak among delegates attending a 10 day international academic meeting in South Australia during May 2001. A retrospective cohort study of the 29 delegates who attended the conference was conducted. A questionnaire was sent by email with a response rate of 93 per cent. Ten cases (onset of diarrhoea while attending the conference) were identified. Two were culture positive for Campylobacter jejuni. There was a significant association between the illness and eating a number of food items from two restaurants however, environmental investigation of the two venues did not identify a definitive source for the outbreak. This investigation demonstrates the usefulness of email in the distribution of questionnaires among specific cohorts.  相似文献

In November 1981, an outbreak of waterborne giardiasis occurred at a popular ski resort in Colorado. Stratification of illness by consumption of municipal tap water showed a striking dose-response, with an attack rate of 42 per cent among persons who drank six or more glasses of water per day. Filtered water samples revealed Giardia cysts in specimens both before and after treatment, and several deficiencies were found in the water treatment facility. Residents who had lived in the area greater than 2 years had a lower attack rate for illness than short-term residents.  相似文献

In June 1987, an outbreak of acute enteritis occurred over a period of 2 weeks in a small rural community where the potable water supply is unfiltrated and unchlorinated. Campylobacter jejuni was isolated from 6 patients. A case-control study performed in a local fiberglass shop where workers drink a lot of water, showed an association between the occurrence of enteritis and the consumption of 10 or more 8-ounce glasses of water per day (Odds Ratio = 6, p = 0.04). The attack rate of enteritis was 23.2% (13/56) in that industry where a dose response relationship was also noted (p = 0.05). C. jejuni was not recovered from the local water supply, which had temporarily been chlorinated the day before the samples were drawn. However, samples taken just before the outbreak showed high coliform counts. This episode suggests that unprotected water systems may be contaminated by C. jejuni.  相似文献

An operating error in a sewage treatment plant led to severe drinking water contamination in a well-defined district of a suburban municipality of Zurich, Switzerland. Despite the alert issued to the local population on the same day advising people not to consume the contaminated water, cases of acute gastroenteric diseases were subsequently observed. Considerable faecal contamination was detected the day after the incident in water samples taken up to 500 m from the sewage plant. In a retrospective epidemiological study involving 240 persons living in the affected area, 126 cases of acute gastrointestinal illness were documented. The epidemic curve revealed a peak incidence two days after the event. Stool samples from 11 of 20 patients were positive for noroviruses or Campylobacter jejuni. Although these microorganisms were not detected in the contaminated water, the subsequently conducted case–control study among the surveyed population showed that consumption of contaminated drinking water was associated with gastrointestinal illness (odds ratio 29.1; 95% confidence interval: 9.8–86.4; p = 0.001). The study also revealed the very probable time period of infection. We present the dimension and chronology of this outbreak and discuss the reasons for its localised and temporary spread.  相似文献

In August 1998, an outbreak of campylobacteriosis occurred in one municipality in northern Finland. A 10% random sample of residents (population 15 000) was selected through the National Population Registry for a survey conducted by using postal questionnaires. Cases were defined as residents of the municipality with onset of acute gastroenteritis from 1 to 20 August 1998. Of 1167 respondents (response rate 78%), 218 (18.7%) met the case definition. Drinking non-chlorinated municipal tap water was strongly associated with illness (OR 34.4). The estimated total number of ill persons was 2700. Campylobacter jejuni was isolated from stool samples of 45 (61%) out of 74 patients tested. All five isolates tested had indistinguishable PFGE patterns. Water samples were negative for campylobacter and coliforms. Epidemiological and environmental evidence suggested mains repair as the source of contamination. Non-chlorinated ground-water systems may be susceptible to contamination and can cause large outbreaks.  相似文献

An outbreak of Campylobacter jejuni infection occurred in a South Wales Valleys housing estate. Illness in estate residents was associated with tap water consumption [population attributable risk (PAR) 50%, relative risk (RR) 2.53, 95% confidence interval (CI) 1.9-3.37] and residence in the upper estate (PAR 49%, RR 2.44, 95% CI 1.83-3.24). Amongst upper estate residents, rates of diarrhoeal illness increased with rates of water consumption (OR 18, 95% CI 3.5-92.4 for heaviest consumers, chi2 trend P<0.0001). The upper estate received mains water via a covered holding reservoir. A crack in the wall of the holding reservoir was identified. Contamination with surface water from nearby pasture land was the likely cause of this outbreak. Service reservoirs are common in rural communities and need regular maintenance and inspection. The role of water in sporadic cases of campylobacter enteritis may be underestimated.  相似文献

STUDY OBJECTIVE: To determine the source and the extent of a community wide outbreak of gastroenteritis. DESIGN: A matched case-control study with postal questionnaires. Subtyping of campylobacter strains by pulsed field gel electrophoresis (PFGE). SETTING: A rural municipality with a population of 8600 in southern Finland, August 2000. Two thirds of the population receive non-chlorinated ground water from the municipal water supply. PARTICIPANTS: Cases were randomly selected among residents of the municipality who contacted the municipal health centre because of gastroenteritis and had illness onset between 31 July and 20 August 2000. Community controls were identified from the population registry and matched according to sex, year of birth, and postal code. Main results: Four hundred and sixty three persons contacted the municipal health centre because of gastroenteritis. Campylobacter jejuni was isolated from stool samples of 24 persons. One hundred and thirty seven cases and 388 controls were enrolled in the case-control study. In multivariate analysis, drinking unboiled water from the municipal supply was significantly associated with illness (odds ratio 11.1, 95% confidence interval 1.4 to 90.2). C jejuni was isolated from one tap water sample. The water isolate and all but one of the patient isolates were indistinguishable by PFGE. CONCLUSIONS: Combining epidemiological investigation with molecular subtyping methods provided strong evidence that water was the source of the outbreak. Non-chlorinated small ground water systems may be susceptible to waterborne outbreaks and constitute a risk to rural populations.  相似文献

During November and December 1981 more than 50 residents in a village in Derbyshire had an acute gastrointestinal illness. One month later a second outbreak occurred affecting another 22 people. Campylobacter jejuni was isolated from 12 patients; no other gastrointestinal pathogens were identified. A case-control study showed an association with the consumption of unpasteurized milk from one particular farm. No new cases were identified for 6 months following the application of a Pasteurization Order from 26 January to 23 February 1982.  相似文献

Cryptosporidiosis is a common gastrointestinal illness that is transmitted from infected persons, animals, or contaminated water or food. This article reports on an outbreak of cryptosporidiosis associated with an animal nursery at an agricultural show held in northern Tasmania during October 2001. Eighty-one per cent of cases (38/47) notified to the Tasmanian Department of Health and Human Services over a 35 day period were interviewed to determine potential sources of infection. Eighty-one per cent of interviewed cases (29/36) reported that they had attended the agricultural show, and 75 per cent (27/36) reported contact with animals in the animal nursery. Cases occurring more than one incubation period after the agricultural show were significantly more likely to have had contact with someone else with diarrhoea (p<0.01). This is the first reported outbreak of cryptosporidiosis associated with an animal nursery in Australia. The outbreak demonstrates the importance of infection control policies and hygiene measures in the animal nursery setting.  相似文献

An outbreak of gastroenteritis occurred at a Pennsylvania summer camp in July 1978. Symptoms included abdominal pain (81 per cent), nausea (72 per cent), and vomiting (53 per cent); upper respiratory infection symptoms occurred in 35 per cent of the campers. Illness was associated with consumption of five or more glasses of water or water-containing beverages. Stool cultures from affected persons were negative for bacterial pathogens; however, a fourfold or greater rise to the Norwalk agent was demonstrated in serum samples of three of three ill persons tested and in none of eight controls (p < .02). After a nursery school trip to a dairy farm, 20 (53%) of 38 children and 3 (23%) of 13 adult helpers developed gastrointestinal infection. Campylobacter jejuni was isolated from 15 primary cases and from 3 of 9 secondary household cases. A cohort study of the school party found illness to be associated with drinking raw milk (relative risk 5.4, 95% confidence interval 1.4-20.4, P = 0.001). There was a significant dose response relationship between amount of raw milk consumed and risk of illness (chi 2-test for linear trend 12.1, P = 0.0005) but not with incubation period, severity of symptoms or duration of illness. All 18 human campylobacter isolates were C. jejuni resistotype 02 and either biotype I (number 16) or biotype II (number 2). Campylobacter was also isolated from samples of dairy cattle and bird faeces obtained at the farm but these were of different resisto/biotypes. Educational farm visits have become increasingly popular in recent years and this outbreak illustrates the hazard of exposure to raw milk in this setting.  相似文献
