Background This study researched the electric impedance properties of breast tissue and demonstrated the differentcharacteristic of electrical impedance scanning (EIS) images.Methods The impedance character of 40 malignant tumors, 34 benign tumors and some normal breast tissue from 69patients undergoing breast surgery was examined by EIS in vivo measurement and mammography screening, with aseries of frequencies set between 100 Hz-100 kHz in the ex vivo spectroscopy measurement.Results Of the 39 patients with 40 malignant tumors, 24 showed bright spots, 11 showed dark areas in EIS and 5showed no specific image. Of the 30 patients with 34 benign tumors there were almost no specific abnormality shown inthe EIS results. Primary ex vivo spectroscopy experiments showed that the resistivity of various breast tissue take thefollowing pattern: adipose tissue>cancerous tissue>mammary gland and benign tumor tissue.Conclusions There are significant differences in the electrical impedance properties between cancerous tissue andhealthy tissue. The impedivity of benign tumor is lower, and is at the same level with that of the mammary glandulartissue. 目的:评价电阻抗扫描成像(electrical impedance scanning,EIS)和高频超声对小乳腺癌的诊断价值.方法:乳腺肿块(直径≤2cm)54例病理证实,行EIS和高频超声检查,时间间隔不超过3d.结果:乳腺癌23例,EIS诊断小乳腺癌17例,敏感性73.9%,特异性80.6%,准确性77.8%,阳性预测值73.9%,阴性预测值80.6%;高频超声诊断小乳腺癌20例,敏感性87.0%,特异性87.1%,准确性87.0%,阳性预测值83.3%,阴性预测值90.0%.二联合诊断,敏感性95.6%,特异性90.3%,准确性92.6%,阳性预测值88.0%,阴性预测值96.6%.结论:EIS对小乳腺癌的诊断有较大实用价值,且与高频超声结合应用,可以提高小乳腺癌的诊断准确奎.  相似文献

目的探讨自动乳腺全容积成像(ABVS)系统检查在鉴别乳腺良恶性肿瘤中的临床价值。方法使用西门子Acuson S2000的ABVS系统扫查84例乳腺肿瘤患者的双侧乳腺,获取双侧乳腺横切面图像上的基本数据,系统自动进行三维数据重建,然后获得乳腺冠状面和矢状面图像。与常规二维超声检查相对比,并与术后或穿刺病理结果相对照,评价ABVS系统对乳腺良恶性肿瘤的鉴别诊断价值。结果 ABVS系统诊断的灵敏度为90%,特异度为90%,准确度为90%;常规超声诊断的灵敏度为70%,特异度为90%,准确度为81%。结论 ABVS系统作为一种新的超声检查途径,提高了超声对乳腺良恶性肿瘤鉴别诊断的准确度,为临床提供更加准确、完整的诊断信息。  相似文献

Background  As an uncommon presentation, occult primary breast cancer remains a diagnostic and therapeutic challenge in clinical practice. This study aimed to retrospectively assess the feasibility of breast magnetic resonance imaging (MRI) in patients with malignant axillary lymphadenopathy and unknown primary malignancy, and correlation with histopathological characteristics.
Methods  A total of 35 women with occult breast carcinoma were evaluated with dynamic contrast-enhanced breast MRI. Whole seriate section was used in all cases. MRI performance was assessed and correlated with histopathological findings.
Results  Twenty-one of 35 patients were found to have primary breast carcinoma histologically. Twenty of the 21 patients had abnormal MR findings and 1 patient had a normal MRI study. Of the remaining 14 patients, 10 were negative on both MRI and surgery. Four had suspicious enhancement on MRI and no corresponding tumor was found. Lesions with mass enhancement were found in 55% (11/20) and ductual and segmental enhancement in 45%. The average diameter of the primary tumors was 15 mm. Invasive ductal carcinomas were found in 81% (17/21). One of 17 invasive ductual carcinomas was too small to be graded. Fourteen of the remaining 16 were classified as grade II and 2 as grade I. Thirty-two of the 35 patients had received estrogen receptor, progesterone receptor and human epidermal growth factor receptor 2 examinations and the 12 of 32 were triple-negative breast carcinoma.
Conclusions  Mass lesions with small size and lesions with ductal or segment enhancement are common MRI features in patients with occult breast cancer. The dominant types of primary tumors are invasive ductal carcinoma with moderate histopathological grade. The rate of triple-negative breast carcinoma may be higher in occult breast cancer.

目的探讨原癌基因c-met在乳腺癌血管生成中的作用及其临床意义,为寻求抗人乳腺癌血管生成药物提供依据。方法应用SP免疫组化和计算机图像分析技术,对80例乳腺癌、20例乳腺纤维腺瘤的微血管密度和c-met蛋白表达进行定量测定。结果乳腺癌组织微血管计数、c-met阳性表达与组织学分级、淋巴结转移、肿瘤分期显著相关(P<0.01)。高微血管计数与c-net阳性表达显著相关(P<0.01)。结论原癌基因c-met激活参与调控乳腺癌的肿瘤血管生成,是一重要的预后因素。  相似文献

目的探讨乳腺分叶状肿瘤的临床病理特点、治疗原则和影响预后的因素。方法回顾性分析2001年1月~2005年8月收治的13例乳腺分叶状肿瘤患者的临床资料。结果13例患者中8例行局部切除术,4例行乳房单纯切除术,1例行乳腺癌改良根治术,全部患者术后随访,随访时间4~47个月,中位时间13个月,1例行乳腺癌改良根治术后3年死于远处转移,2例行局部切除术后复发。结论局部扩大切除术治疗乳腺分叶状肿瘤是可行的,而对肿瘤体积较大、浸润胸肌或局部切除术后复发患者则宜行乳房切除术。  相似文献

目的:探讨钼靶下导丝定位活检技术对触诊阴性乳腺可疑病灶的诊断价值。方法:选取52例钼靶检查发现可疑病变而临床触诊阴性、彩超无特异发现的乳腺病灶进行钼靶下导丝定位后手术活检并送病理检查。结果:52例患者行钼靶引导下导丝定位均一次操作成功,术后病理报告:恶性病变16例,其中浸润性导管癌7例,导管原位癌6例,导管内癌早期浸润2例,浸润性小叶癌1例;良性病变36例,其中乳腺增生症15例,纤维腺瘤7例,轻~中度非典型增生5例,炎症性病变4例,导管内乳头状瘤3例,乳腺导管扩张症2例。乳腺癌检出率为30.8%,诊断准确率为100%。结论:钼靶下导丝定位乳腺活检定位准确,安全可行,可有效提高不可触及的乳腺病变切除活检的精确性,利于早期乳腺癌的发现。  相似文献

目的:评价声脉冲辐射力成像(ARFI)技术对乳腺良恶性病变的鉴别诊断价值。方法选取84例患者共87个肿块进行常规超声、声触诊组织成像(VTI)及声触诊组织量化(VTQ)检查,得出剪切波速度值,并运用image J软件对VTI图像进行后处理得出肿块的平均光密度值,以病理结果为金标准,绘制受试者工作特征(ROC)曲线,评价ARFI的意义。结果经手术病理证实,87个乳腺肿块中良性病灶56个、恶性病灶31个。恶性病灶V T Q≥3.49 m/s (临界值)的病灶数为21个,<3.49 m/s为10个。良性病灶V T Q≥3.49 m/s病灶数为3个,<3.49 m/s为53个。恶性病灶平均光密度值大于或等于202.25(临界值)的病灶数为28个,<202.25为3个。良性病变平均光密度值大于或等于202.25的病灶数6个,<202.25为50个。乳腺良恶性病灶VTQ剪切波速度及VTI平均光密度值比较,差异均有统计学意义(P=0.000)。关于乳腺肿瘤的诊断灵敏度、特异度、阳性预测值、阴性预测值、准确率及约登指数,常规超声分别为96.8%、67.8%、62.5%、97.4%、78.2%、0.645;ARFI分别为93.5%、98.2%、96.7%、96.5%、96.6%、0.917。结论 ARFI成像对乳腺肿瘤诊断的特异度、准确率优于常规超声。  相似文献

目的:探讨多层螺旋CT(MSCT)多种成像技术及摸拟手术定位在乳腺疾病中的作用。方法:对69例经临床触诊检查怀疑有乳腺肿块的患者进行平扫加增强的MSCT扫描,并应用图像后处理工作站Advantaged—workstation4.0对原始图像进行处理。图像重建采用3D、表面遮盖技术(SSD)、多平面容积重建(MPR)、最大密度透影(MIP)、摸拟手术刀技术(CUT)。结果:69例经MSCT检查及应用后处理技术能很好地显示肿块边缘有毛刺、细小的钙化,病灶明显强化,皮肤增厚,乳头凹陷,是乳腺疾病诊断的重要依据。结论:MSCT图像后处理技术对乳腺疾病的诊断和手术定位具有很高的临床使用价值。  相似文献

目的:分析乳腺自动全容积成像联合超声造影对乳腺导管内病变诊断价值。方法:前瞻性研究选取2021年8月-2023年3月于本院接受治疗的110例乳腺疾病患者为本次研究对象,计划110例患者作研究对象(考虑有10%的脱落率,选取样本由100例提至110例)。由于部分患者的双侧乳房存在病变的状况,此次研究按病理学检验结果、研究目的将其分成导管内病变组(60例)与非导管内病变组(50例),同时对于患者双侧同时伴有病变,只要一侧诊断为导管内病变即可作为导管内病变组。结果:经分析显示,超声造影对患者乳腺导管内病变的检测结果与年龄、边缘、病灶增强特征、回声变化无关(P>0.05);而与病灶达峰强度、病灶形态、直径、周边放射状增强、达峰增强使扩大径线变化有关(P<0.05)。乳腺自动全容积成像检测乳腺导管内病变患者的结果与年龄、肿块形态、回声、边缘变化无关(P>0.05);而与微钙化、肿块位置、伴有结构变化有关(P<0.05)。在乳腺导管内阴、阳性病变诊断中二者联合检测明显高于超声造影、乳腺自动全容积成像单一检测(P<0.05)。与超声造影、乳腺自动全容积成像单项诊断相比,两者联合诊断乳腺导管内阳性病变的敏感度、特异度、准确性均较优(P<0.05)结论:乳腺自动全容积成像、超声造影联合诊断乳腺导管内病变具有较高的敏感度、特异度及准确度,可及时明确病变的类型及发展情况,为临床制定有效治疗措施提供了参考。  相似文献
