Thirteen out of 223 consecutive cardiac transplant patients required permanent pacemaker implantation; 11 for sinus node dysfunction and 2 for complete AV block. Patients with sinus node dysfunction were considered for AAIB mode pacemakers if they had intact AV conduction defined as a Wenckebach point of > 120 beats/min. Ten of 11 patients with sinus node dysfunction had a single atrial lead placed. Atrial lead placement was most easily accomplished with a straight, active fixation lead and the use of manually curved stylets to find an optimal position in the donor atrium, although active fixation leads with a preformed atrial J were used as well. Two leads dislodged requiring revision. In contrast, only 1 of 250 atrial leads implanted in nontranspianted hearts dislodged (P < 0.0001). Transvenous endomyocardial biopsies have not caused atrial lead dislodgment. Most transplant recipients requiring permanent pacing have intact AV nodal function and require only atrial pacing. Atrial lead dislodgment requiring lead revision occurs more frequently in heart transplant recipients than in native hearts. Use of a straight active fixation lead with a munually formed curve in the stylet is useful in order to find the optimal position for pacing.  相似文献

Single chamber rate responsive pacing offers many potential advantages over the more complex dual chamber atrial tracking pacing mode in children, and the preservation of atrioventricular synchrony could be unnecessary in selected groups of pediatric patients. Twenty-two pediatric patients (age range 9 months to 12 years; mean 6.5 years) had implantation of ventricular rate responsive (VVIR) pacemakers over a 2-year period. All patients had chronic third-degree atrioventricular block, and a normal ventricular function at rest. During the follow-up each patient underwent a 24-hour Holter monitoring, and ten performed a graded treadmill test in both ventricular fixed rate (VVI) and rate responsive (VVIR) pacing mode. Paced ventricular rates were found to be normal for age in all 22 patients; maximum rate did not reach the higher programmed rate during daily activities in any patient. Comparing the mean paced ventricular rate to the mean rates of blocked P waves, six patients showed a difference of more than 20 beats/min, which induced the pacemaker parameters to be reprogrammed. In all patients a significant correlation was found between variations of paced ventricular rate and variations of spontaneous blocked atrial rhythm (P < 0.05); this correlation persisted in the subsequent Holter controls in the ten patients with longer follow-up. Exercise tolerance resulted normal in the ten patients who performed a treadmill test either in VVIR or VVI mode, with increased maximal heart rates and maximal systolic blood pressure in VVIR mode (P < 0.0013). Rate responsive ventricular pacemakers seem to adequately respond to the physiological needs of daily life of this selected group of children requiring permanent pacing.  相似文献

Following the Fontan operation for definitive palliation of the univentricular heart, sinus node dysfunction, and/or atrioventricular block requiring pacemaker therapy is common. In previous studies ventricular rate responsive pacing (VVI, R) resulted in improved exercise performance over VVI pacing in anatomically normal hearts with either sinus node disease or atrioventricular block. In this study, the usefulness of both VVI, R and DDD, R pacing are evaluated in the postoperative univentricular heart following the Fontan operation. Eight postoperative Fontan patients with sinus node disease or atrioventricular block underwent exercise testing using a treadmill protocol. Six patients had single chamber ventricular pacemakers and two patients had dual chambered rate responsive pacemakers. Median age at exercise testing was 14 years. Patients were tested in the VVI, VVI, R, and DDD, R modes acting as their own controls. Heart rate, work rate, oxygen consumption, and respiratory exchange ratio were monitored continuously. Heart rate was significantly increased in the rate responsive modes compared to the VVI mode. In spite of the significant increase in heart rate, there was no change in maximal work rate or oxygen consumption. There was also no significant change in oxygen consumption at ventilatory anaerobic threshold. From these data we would conclude that VVI, R pacing in postoperative univentricular hearts does not result in improved exercise performance and that further study with DDD, R pacing is needed to determine its usefulness in this group of patients.  相似文献

Temperature responsive pacemakers were implanted in 45 patients (ages 44 to 90); 31 patients were evaluated by randomized, paired treadmill exercise tests 1 month postimplant. Of 28 males and 17 females, 19 had coronary artery disease; 8 had congestive heart failure. Pacing indications included sinus node disease (26), atrial fibrillation (15), AV block (10), and brady/tachy syndrome (10); some had multiple indications. Blood temperature (every 10 seconds, resolution = 0.004 degrees C) and pacing rate (every minute) were telemetered from the pacemaker. Average heart rate, exercise duration (5.7 min VVI; 6.7 min VVIR), VVIR response time (22 sec), initial temperature drop (0.23 degrees C) and maximum rate of drop (0.65 degrees C/min), temperature rise (0.31 degrees C VVI; 0.38 degrees C VVIR) and rate of rise (0.27 degrees C/min) were studied in a subset of patients. In pacer-dependent patients, average paired increases in exercise duration and heart rate was 56% and 34%, respectively. Including all (31) patients, some with intermittent sinus rhythm, increases were 28% and 9%, respectively. Because exercise duration increased, temperature rise was higher with rate adaptation. Rate adaptation was obtainable in all patients and patients averaged 99 +/- 48 increases above basic pacing rate per day at nominal temperature sensitivity. Conclusion: Beneficial rate adaptation is achievable using blood temperature to modify rate in a sensor based system.  相似文献

BONGIORNI, M.G., ET AL.: Evaluation of Rate-Responsive Pacemakers by Transesophageal Holter Monitoring of Spontaneous Atrial Rate. One of the most important problems in rate responsive (RR) pacing is the clinical experimental evaluation of the reliability of various sensors. In particular, it is difficult to test their sensitivity and specificity during daily activity of the patients. Atrial rate, when present and normal, is the most physiological marker of metabolic requirements, but sometimes it is impossible to analyze the P wave in ventricular paced rhythm during routinely performed tests (e.g., ergometric test and 24-hour Holter monitoring). During various physical activities, we monitored atrial electrograms on an esophageal lead on the first channel of a standard Holter tape recorder; on the second channel a surface ECG lead was recorded. We selected 10 patients with high grade heart block and normal sinus node function paced in RR-VVI mode. RR pacing was obtained using various sensors (body activity, blood temperature, spike-T interval, minute ventilation). The good quality of recording allowed an easy evaluation of atrial and ventricular rates. In four cases an appropriate increase in heart rate was documented; sensitivity threshold and/or rate response slope were reprogrammed when indicated. The pacing rate of one patient did not parallel the atrial rate during walking only. In three cases, we observed a delay in the ventricular rate increase, with ventricular rate decreasing at peak exercise despite further atrial rate increase. In the last two patients, we observed inappropriate pacing response; pacing rate increased later and to a lower level than the atrial one. This new method is applied easily and appears reliable to evaluate the response of RR pacemakers to individual metabolic needs. Its applicability is, however, limited by the need for a normal sinus node function. In conclusion, transesophageal atrial rate recording is a useful tool for the clinical evaluation of RR pacemakers, and it can be proposed as a new method for testing new sensors.  相似文献

Ejects of extracorporeal shockwave lithotripsy (ESWL) were studied on 15 pacemakers (standard single chamber n = 5, dual chamber n = 6, rate responsive single chamber [Activitrax] n = 4). In-vitro testing involved suspending the pacemakers in a bath of degassified, deionized water firmly taped to a platform at the point of maximal pressure, i.e., second focal point (F2), where they received pressure shocks (x?= 1300) from the HM3 Dornier lithotriptor. The pacemakers, programmed to their most sensitive setting, were continuously pacing at nominal outputs (atria) and ventricular pacing in the DDD mode). All units were assessed by a pacing system analyzer before and after the study, then underwent destructive analysis. During standard single chamber pacing (VVI) the pacing stimulus triggered ESWL. For dual chamber devices, ESWL was triggered by the atrial paced event which induced inhibition of the ventricular output in two pacemaker. This was eliminated by reprogramming to a less sensitive setting. The pacemaker can, hermetic seal and internal circuitry were undamaged in all units. Two rate responsive single chamber pacemakers had their activity sensing piezoelectric elements shattered when placed at F2. Two other units placed 5 cm from F2 were stimulated to their maximum upper programmed pacing rate with ESWL therapy, but were otherwise unaffected. Subsequent to this study, six patients with pacemakers programmed to the VVI (five), DDD (one) modes implanted in the thorax underwent successful ESWL without pacemaker or arrhythmic event. Conclusions: (A) It is generally safe for patients implanted with standard single chamber devices in a ventricular application to undergo ESWL without modifying the pacing/sensing parameters. (B) Patients implanted with dual chamber devices who pace in the atrium should be reprogrammed to the VVI mode during ESWL. (C) Patients with piezoelectric activity sensing rate responsive single chamber pacemakers should have this feature programmed off during ESWL and, if implanted in the abdomen, probably should not undergo ESWL.  相似文献

We compared the clinical course of patients paced in VVIR versus DDDR mode to determine the most appropriate method of pacing following cardiac transplantation. Pacemaker implantation was required in 9 of 90 orthotopic cardiac transplants (10%). Indications included sinus bradycardia or sinus arrest (8 patients) and AV node dysfunction (1 patient). VVIR pacemakers were implanted in four patients and DDDR in five patients. DDDR patients : The mean P wave was 1.7 mV and the mean atrial stimulation threshold was 0.8 V (at 0.5 msec). During follow-up of 20 months, two atrial lead complications developed (29% of leads in 33% of patients). No lead complications were directly related to endomyocardial biopsy. VVIR patients : All four patients developed VA conduction with mean VA time 180 msec (160–240 msec). Two patients developed pacemaker syndrome. Conclusions : VA conduction and pacemaker syndrome may develop in cardiac transplant recipients paced in the VVIR mode. Dual chamber pacing is technically feasible and preferable following cardiac transplantation.  相似文献

Thirteen patients who all had previously inserted temporary or permanent pacemakers (6, VVI; 7, A-V sequential) were studied by two-dimensional echocardiography and radionuclide gated blood pool ventriculography (RVG) for non-invasive evaluation of cardiac performance. Patients were paced in both the VVI mode and during sinus/atrial or A-V sequential pacing. Although there was no objective change of the ejection fraction during V-pacing and atrial/A-V sequential pacing or sinus rhythm, as has been previously reported, A-V sequential pacing did result in significant improvement in overall cardiac function and output as judged by radionuclide ventriculography and blood pressure response in most of our patients. An appropriately timed atrial contribution to ventricular systole resulted in improved ventricular function in those individuals with pre-existing systolic or diastolic myocardial dysfunction and/or sick sinus syndrome in whom pacemaker therapy was indicated. Radionuclide ventriculography appears to be a reliable, accurate, non-invasive method that can be used to evaluate patients before implantation of a permanent ventricular or atrioventricular pacemaker in order to decide which pacing mode is best for that particular individual.  相似文献

A consecutive series of 66 patients (males = 32; mean age +/- SD = 71 +/- 9 years) given atrial inhibited pacemakers for sick sinus nodes were followed to study the incidence of lead failures, chronic atrial tachyarrhythmias, and atrioventricular conduction disturbances. The need for rate responsive pacing was also assessed. Pre and postoperative investigation could include carotid sinus massage, Holter monitoring, exercise testing, and invasive electrophysiology. The mean follow-up time +/- SD was 32 +/- 29 months (median = 26 months). Three patients (5%) had their pacemakers replaced due to lead failures (loss of sensing = 2; exit block = 1). Two pacemakers (3%) were replaced after 5 and 22 months due to atrial fibrillation. Four patients (6%) received new pacemakers because of development of second-degree or complete atrioventricular block after 1, 6, 12, and 31 months, respectively. During exercise, most patients (76%) responded with an increase in sinus rate at least as marked as that achievable with the currently available rate responsive pacemakers. Assuming careful patient selection, atrial inhibited pacing is well suited for many patients with sinus node dysfunction and preserved atrioventricular conduction. There is a limited need for rate responsive pacemakers in these patients.  相似文献

Eleven patients with rate responsive pacemakers (7 men, 4 women, mean age 41 years with a range of 23-60) were randomly assigned to a cross-over study in order to assess their overall exercise capacity and quality-of-life (QOL) scores. All of the pacemakers were implanted for complete AV block or sick sinus syndrome. The pacemakers were randomly programmed into VVI or rate responsive (VVIR) pacing modes for 3-week study periods in each mode. At the end of each period, an exercise test was performed and the QOL was evaluated by the "Hacettepe Quality-of-Life Questionnaire". All patients exercised longer in the VVIR mode (mean 10.54 ± 0,73 min) than in the VVI mode (mean 7.81 ± 0.62 min) (P < 0.05). QOL scores were also found to be significantly higher in the VVIR mode (mean 173.81 ± 16.22 points) compared to the VVI mode (mean 156.27 ± 21.22 points) (P < 0.01). In conclusion, our results suggest that VVIR pacing offers a better QOL in addition to an improved exercise capacity, compared to the single chamber nonrate modulated pacing (VVI).  相似文献

The purpose of this paper is the assessment of sinus node competence over time in patients with isolated atrioventricular block (AV block). Patients implanted with AV synchronous pacemakers for isolated A V block between December 1993 and June 1995 were prospectively evaluated at predischarge, 6 weeks, and subsequent 6 months follow-up with respect to atrial rate monitors/24-hour Holter and modified exercise test. Patients unable to maintain AV synchronous pacing or complete a modified exercise test were excluded. Sinus node competency is interpreted as: (1) absence of atrial brady- or tach-yarrhythmia, (2) ability to achieve a minimum heart rate of 100 beats/min with modified exercise test or during daily activities. There were 58 patients (22 women), mean age 71.0 ± 13.8 with an average follow-up of 30.4 months (11–40). Three patients did not complete a modified exercise test, 4 patients were lost to follow-up, and 2 patients were unable to maintain AV synchronous pacing. Of the remaining 49 patients, 3 developed chronic or paroxysmal atrial fibrillation. No patient developed significant bradyarrhythmias. All patients achieved a heart rate of ≥ 100 beats/min modified exercise test. In our group of patients with isolated AV block within a moderate follow-up period, development of Sinoatrial dysfunction was rare (6%). A longer follow-up is required to delineate the natural history of Sinoatrial dysfunction in patients with isolated AV block.  相似文献

We used a cardiopulmonary test to assess the physiological benefit of single lead VDD pacing in ten patients (six men, four women; aged 32-84 years, mean 69 years) with atrioventricular block. Maximal symptom-limited treadmill exercise test using a ramp protocol was performed under VDD and VVIR or VVI pacing (VVI) in random sequence. The pacemaker was then programmed to the VDD mode, and Holter ECG was recorded in nine patients. Compared with findings during the VVI, the VDD mode had a greater chronotropic response (mean maximal heart rate, VDD 106 +/- 17 beats/min vs VVI 79 +/- 19 beats/min, P = 0.03), and was associated with prolongation of exercise duration (VDD 11.2 +/- 2.9 minute vs VVI 10.5 +/- 3.1 minute; P = 0.01), and the onset of anaerobic threshold at a higher oxygen uptake (VDD 12.4 +/- 3.4 mL/min per kilogram vs VVI 10.0 +/- 2.1 mL/min per kilogram; P < 0.01). Atrial sensing was recognized in almost all normal sinus P waves for all cases examined using Holter ECG. Thus, chronotropic response during exercise by VDD pacemaker improved exercise tolerance, indicating that a VDD pacemaker might be useful for patients requiring physical activity.  相似文献

Methods and devices for permanent cardiac pacing remained relatively stable for over two decades with use of the single-chamber ventricular demand (VVI) pacemaker. However, changes have occurred in the 1980s and are expected to continue with the availability of more advanced technology and with increasing knowledge about cardiac pacing. The physiologic benefit of the newer dual-chamber atrial synchronous (VDD) and fully automatic, universal (DDD) pacemakers over the VVI pacemaker in patients with permanent complete heart block and normal sinus node function has been established. These newer units not only reestablish atrioventricular synchrony but also are physiologically rate-responsive. The VDD pacemaker is expected to be phased out in favor of the DDD pacemaker. When the atrial rate or interval is lower than the lower rate limit, the VDD pacemaker functions as a VVI, whereas the DDD pacemaker functions as an atrioventricular sequential (DVI) pacemaker to maintain continuous atrioventricular synchrony. Contrary to general belief, patients with complete heart block and normal sinus node function may gain very little physiologic benefit, if any, from DVI pacing. The sinus node will compete with the pacemaker's atrial stimulation when the sinus rate is faster than the DVI pacemaker rate (which usually occurs during activity). Also, the ventricular pacing rate will not vary with physiologic change. The DVI and atrial demand (AAI) pacemakers have been used in some patients with sinus node dysfunction. Increasing exercise tolerance should not be expected in the majority of patients because they are not pacemaker-dependent during activity, ie, their heart rate is higher than the pacemaker rate. However, these pacemakers appear to help in eliminating pacemaker syndrome, which does not infrequently occur with VVI pacemakers. Patients with sinus node dysfunction but without atrioventricular block do not gain more physiologic benefit with a DDD than with a DVI pacemaker. Whether these patients have severe sinus node dysfunction all the time or adequate sinus node function most of the time during follow-up, the DDD pacemaker will function as a noncommitted DVI with atrial sensing (DDI). The early report of DVI pacemaker-induced atrial fibrillation during follow-up has been refuted by more recent works. If the DDD pacemaker is significantly more expensive than the DVI pacemaker, the latter type may be a good alternative for this condition.  相似文献

RF current delivery may cause acute and chronic dysfunction of previously implanted pacemakers. The aim of this study was to assess prospectively the effects of RF energy on Thera I and Kappa pacemakers in 70 consecutive patients (mean age 70 ± 11 years, mean left ventricular ejection fraction 48 ± 15%) who underwent RF ablation of the AV junction for antiarrhythmic drug refractory atrial fibrillation (permanent in 42 patients, paroxysmal in 28). These pacing systems incorporate protection elements to avoid electromagnetic interference. The pacemakers (Thera DR 7960 I in 20 patients, Thera SR 8960 1 in 30, Kappa DR 600–601 in 8, Kappa SR 700–701 in 12) were implanted prior to RF ablation in a single session procedure and were transiently programmed to VVI mode at a rate of 30 beats/min. Capsure SP and Z unibipolar leads were used. During RF application there was continuous monitoring of three ECG leads, endocavitary electrograms, and event markers. Complete AV block was achieved in all cases after 3.6 ± 2.9 RF pulses and 100 ± 75 seconds of RF energy delivery. The mean time of pacemaker implantation and RF ablation was 60 ± 20 minutes. Transient or permanent pacemaker dysfunction including under/oversensing, reversion to a "noise-mode" pacing, pacing inhibition, reprogramming, or recycling were not observed. Leads impedance, sensing, and pacing thresholds remained in the normal range in the acute and long-term phase (average follow-up 18 ± 12 months). In conclusion, Thera I and Kappa pacemakers exhibit excellent protection against interference produced by RF current. The functional integrity of the pacemakers and Capsure leads was observed in the acute and chronic phases. Thus, the implantation of these pacing systems prior to RF ablation of the AV junction can be recommended.  相似文献

Atrioventricular synchronous pacing offers advantages over fixed-rate ventricular (VVI) pacing both at rest and during exercise. This study compared the hemodynamic effects at rest and exercise of ventricular pacing at a rate of 70 beats/min, ventricular pacing where the rate was increased during exercise and dual chamber pacing. Ten patients, age 63 +/- 8 years, with multiprogrammable DDD pacemakers were studied using supine bicycle radionuclide ventriculography. Radionuclide data during dual chamber pacing was acquired at rest and during a submaximal workload of 200-400 kpm/min. The pacemakers were then programmed to VVI pacing at a rate of 70 beats/min, and 1 week later, studies were repeated in the VVI mode at rest, during exercise at a rate of 70 beats/min, and during exercise with the VVI pacemaker programmed to a rate adapted to the DDD pacing exercise rate. At rest, the cardiac output was lower in the VVI compared with the AV sequential mode (4.1 +/- 1.1 vs 5.7 +/- 1.1 1/min, P less than 0.01). During exercise, the cardiac output increased from resting values in the DDD and VVI pacing modes, however cardiac output in the rate-adapted VVI mode was higher than in the VVI mode with the rate maintained at 70 beats/min (8.1 +/- 1.5 vs 6.3 +/- 1.1 1/min, P = 0.02). Three patients completed lower workloads with VVI pacing at 70 beats/min compared with AV synchronous pacing. At rest, AV sequential pacing was superior to VVI pacing, suggesting the importance of the atrial contribution to ventricular filling. With VVI pacing during exercise, cardiac output was improved with an increased pacemaker rate, suggesting that the heart rate response during exercise was the major determinant of the higher cardiac output.  相似文献

To assess the variation in paced rate during everyday activity and the importance of atrioventricular synchronization (AV synchrony) for submaximal exercise tolerance, atrial synchronous (DDD) and activity rate modulated ventricular (VVI,R) pacing were compared in 17 patients with high degree AV block. The patients were randomly assigned to either mode and evaluated by treadmill exercise to moderate exertion and by 24-hour Holter monitoring after 2 months in the DDD and VVI,R modes, respectively. At the end of the study, the patients were programmed to the pacing mode corresponding to the preferred study period. During the treadmill test, the mean exercise time to submaximal exertion (Borg 5/10), exertion ratings and respiratory rate did not differ between pacing modes despite a significantly lower ventricular rate in the VVI,R mode. The atrial rate during VVI,R pacing was significantly higher than the ventricular rate, but did not differ from the ventricular rate during DDD pacing. There was a diurnal variation in paced rate in both pacing modes. Paced ventricular rate was, however, higher and variation in paced rate greater in DDD compared to VVI,R pacing. Nine patients preferred the DDD mode, three patients preferred the VVI,R mode, while five subjects did not express any preference. The results from this study indicate that the variation in paced rate during activity sensor-driven VVI,R pacing does not match that during DDD pacing neither during everyday activities nor during submaximal treadmill exercise. Nevertheless, no differences in exercise time, Borg ratings, and respiratory rate during submaximal exercise were found. Thus, for most patients with high degree AV block, DDD and VVI,R pacing seem equally satisfactory for submaximal exercise.  相似文献

Current rate responsive pacemakers incorporate sensors such as minute ventilation (MV) for adapting to changing patient conditions during exercise and periods of exertion. However, for sleep and/or rest periods, the only pacemakers currently on the market that slow the pacing rate utilize an internal timer to determine a decrease in pacing rate. It would be advantageous if the pacing rate could be automatically lowered during periods of sleep or rest. This study utilized a rate responsive sensor, MV, to track the patient's sleeping and resting periods and to decrease the pacing rate at such times. A total of eight patients implanted with Sentri 1210 single chamber MV sensor pacemakers were studied. A sleep rate (SH) of 45 beats/min was selected. A sleep rate response function, which indicated the relationship between changes in MV and corresponding heart rate, was initially set at a value of 16 and continually and automatically updated in a 3-month study. Adaptation was based on the premise that 3 hours per day should be spent at the SR. The average decrease in pacing rates from onset to 3 month for the eight patients was 12.4%± 5.3%. Correspondingly, the histograms of the lowest datalog histogram (40–59 beats/min) increased from 0% to 15.4%± 0.9% of paced beats. Correlation between the patients' 24-hour diary and Holter recordings showed that the pacing rates during sleep were consistently lower than when the patients were awake and active. This was also the case with a patient whose nocturnal and daily routine was intentionally altered. In conclusion, the MV sensor has shown to be an accurate sensor for the automatic determination of periods of sleep or rest. Data demonstrates that the decreased average pacing rate will increase pacer longevity as well as potentially provide more physiological pacing rates at night.  相似文献

Rate adaptive pacing has been shown to improve hemodynamic performance and exercise tolerance during acute testing. However, there remain concerns about its benefit in daily life and possible complications incurred by unnecessary pacing. This double-blind crossover study compared the benefit of rate adaptive (SSIR) versus fixed rate (SSI) pacing under laboratory and daily life conditions in 20 rate incompetent patients with minute ventilation single chamber pacemakers (META II). The heart rate (HR) response during three different exercise tests (treadmill, bicycle ergomctry, walking test) was correlated with the Holler findings during daily life in either pacing mode. The maximal HR was significantly higher in the SSIR-mode compared to the SSI-mode, both during laboratory testing (treadmill: 123 ± 15 vs 93 ± 29 beats/min: ergometry: 118 ± 15 vs 89 ± 27 beats/min; walking test: 127 ± 9 vs 95 ± 26 beats/min, all P values < 0.01) as well as during daily life (Holter: 126 ± 13 vs 103 ± 24 beats/min, P < 0.01). On Holter, the average HR (71 ± 14 vs 71 ± 8 beats/min) and the percentage of paced rhythm (54 % vs 62%, SSI- vs SSIR-mode, P = NS) were not different in either mode. However, despite a 30% rate gain in the SSIR-mode, the exercise capacity remained unchanged, and only 38% of patients preferred the SSIR-mode. Minute ventilation pacemakers provide a physiological rate response to exercise. Irrespective of the protocol used, the findings of laboratory testing are comparable to those during daily life. However, patient selection for rate adaptive single chamber pacing should be made with caution, since the objective benefit of restoring normal chronotropy may subjectively be negligible for most patients.  相似文献

In 14 patients with symptomatic sinus node dysfunction—sinus bradycardia, sino-atrial exit block, or sinus arrest—electrophysiological studies were performed before implantation of a pacemaker. In 8 patients incremented high right atrial pacing showed AV-nodal Wenckebach at pacing rates equal to or above 130/min (group I); in 6 patients AV-nodal Wenckebach was reached at pacing rates Jess than 130/min (group II). During ventricular pacing at a rate 10–15% faster than the existing sinus rate, ventriculo-atrial (VA) conduction was present in all patients of group I, while VA conduction was present in only 2 patients of group II (p < 0.05). Patients with symptomatic sinus node dysfunction but with intact AV conduction frequency show VA conduction during ventricular pacing and thus are particularly at risk for developing a pacemaker syndrome when a ventricular demand (VVI) pacemaker is implanted. This complication can be avoided by atrial demand (AAI) pacing or A V sequential (DVI) pacing. When adequate experience has been gathered with A V universel (DDD) pacemakers, the indications for selection of a pacemaker in patients with symptomatic sinus node dysfunction will probably change.  相似文献

Exercise capacity and general well-being are improved by appropriately programmed rate responsive pacemakers when compared to fixed rate units. Ten patients had activity sensing DDDR units implanted for combined AV block and sinus node incompetence. Ten patients had Sensolog activity sensing VVIR units implanted for complete heart block. The effects of over and under programming of rate response in both dual and single chamber activity sensor rate adaptive pacemakers has been assessed subjectively by visual analog scales and specific activity questionnaires and objectively by graded treadmill testing and the performance of standardized daily activities. Patients were randomly programmed to absent rate response (VVI in the Sensolog group), hyporesponsive (DDD in the dual chamber group), appropriate response (VVIR, DDDR according to Manufacturer's instructions) and over responsive (VVIR+, DDDR+) in a double-blind crossover design. Thirty percent of patients demanded early crossover from VVI, 30% from DDDR+ and 50% from VVIR+. Perception of Exercise Capability was similar to objective exercise treadmill times which were shorter in VVI than in VVIR or VVIR+ (P less than 0.05) or control subjects (P less than 0.001). There was no difference between any dual chamber mode or control subjects. General well-being was poorest in DDDR+ and VVIR+ modes despite objective improvement in exercise capacity. Symptoms were least in VVIR and DDDR and all but one patient chose appropriate programming as their overall preferred mode. Thus, even inaccurate rate response programming results in similar and improved exercise capacity compared to absent rate response but overprogramming is unacceptable to most patients, confirming that appropriate programming and sensor specificity is critical in rate responsive pacing.  相似文献
