Plattenosteosynthese oder Marknagelung bei proximaler Humerusfraktur – Retrospektive Analyse von 156 Fällen über 3 Jahre mit einem 2-Jahres-Follow-up

The ideal implant for the treatment of proximal humeral fractures remains controversial. In a single center study, we retrospectively analyzed a cohort of 156 cases over a period of 3 years. Fractures where classified according to Neer. In 86 cases a plate osteosynthesis was performed and in 70 cases a nail osteosynthesis. Treatment algorithm was standardized including radiographs pre- and postoperatively. In the follow up examination, age and gender, postoperative complications and body mass index (BMI) were recorded. The Constant-Murley, Neer and DASH (disabilities of the arm, shoulder and hand) scores where used. According to Neer, 156 patients were classified as follows: 34 patients with two-part fractures, 80 patients with three-part fractures, and 42 patients with four-part fractures. The postoperative analyses of complications showed 33 surgical events in 31 patients (19.7?%). Follow up examination using the adapted Constant-Murley score showed a mean score of 67 points in the nailing group and 61 points in the plate group (p?=?0.032). Examination of the DASH core showed a trend for a better result in the nail group without statistical significance. Slightly better function was achieved in the nail group, again without statistical significance. A complication rate of 19.7?% showed no statistical significant difference between the two groups.     

210. Entscheidungshilfen bei der Indikationsstellung anhand von Ergebnissen nach Lyse und Thrombekomie

Zusammenfassung Zwischen 1/83 und 3/86 wurden 128 Patienten wegen einer tiefen Thrombose behandelt. 68 Patienten mit reiner US-Venenthrombose wurden gelyst bzw. heparinisiert. Für 60 Patienten mit Becken- und/oder Beinvenenthrombose konnte somit alternativ Lyse oder Thrombektomie in Erwägung gezogen werden. 37 Patienten wurden operiert, 23 gelyst. Aufgrund der Ergebnisse sehen wir als Indikation zur Lyse die reine US-Thrombose sowie die Becken- bzw. Beinthrombose bis distal des Kniegelenkes. Indikationen zur OP sind die reine Beckenthrombose sowie Thrombosen bei Schwangeren und in der postoperativen Phase.     

Local injection of bleomycin A 5 in children with hemangiomas. Analysis of 210 cases]

210 children with hemangiomas were treated by local injection of Bleomycin A5. Bleomycin A5 was effective in all patients with strawberry and mixed hemangiomas, 91.2% of patients with cavernous hemangiomas, 44.4% of patients with port-wine stain. There was no response in pampiniform hemangioma. The therapeutic mechanism, indications and complications of the new method for treatment of hemangioma are discussed.

Mummy 1911-210-1

An ancient Egyptian mummy from the collections of the National Museums of Scotland was examined using computerised tomography (CT) scanning as part of the NMS mummy project. A facial reconstruction was produced from the CT scans for comparison with a painted 'portrait' which covers the face of the wrappings. The scans indicated the mummified wrapped body of an adult male 1.65 m tall with excellent preservation of the body. An exact replica, translucent model of the skull was created from the CT scans and standard cephalometric analysis undertaken. The facial features were reconstructed onto a plaster model of the skull from known tissue depths using terracotta clay. The resultant face was compared with the portrait and an extremely close match obtained, suggesting this was an individual portrait painted around the time of death.     

胰十二指肠切除术210例临床分析

目的 探讨胰十二指肠切除(PD)术后并发症及死亡率发生、预防及处理的问题。方法 回顾性分析1977～2002年间210例胰十二指肠切除术的并发癌及死亡率，根据手术年代分为三组，比较各组的术后并发症和死亡率。结果 本组术后出现并发症72例(34．3%)，严重并发症55例(26．2％)，死亡17例(8．1％)；近2年的并发症发生率为17．5%，严重并发症发生率为14％，死亡率1．8％，均比以前明显下降。结论 PD术是需要经过一定时间外科训练才能完成的手术，手术经验、精细操作和围手术监护治疗是减少并发症及死亡率的关键。     

5. Radikalitätsprinzipien bei der Behandlung von Weichteiltumoren

Zusammenfassung Jeder Weichteiltumor ist verdächtig auf Malignität;die histologische Untersuchung des gesamten Präparats ist daher unerläßlich. Die Amputation ist bei lokal nicht operablen Tumoren und vor allem Rezidiven als kurative, bei Exulceration und Functio laesa auch bei Fernmetastasen als palliative Maßnahme indiziert. Die adjuvante Radio- und Chemotherapie wird in Abhängigkeit vom Tumortyp dargestellt. Wichtig ist die interdisziplinäre Zusammenarbeit zwischen Chirurg, Pathologe, Radio- und Chemotherapeut. Dringend anzustreben ist ein zentrales Tumorregister unter pathologischeranatomischer Leitung.     

Augmentation Mammaplasty: Retrospective Analysis of 210 cases

Since its introduction in 1895, augmentation mammaplasty has gained widespread acceptance. The choice of breast augmentation procedure is determined almost entirely by three variables: the selection of incision location, the pocket plane for implant placement (either subpectoral or completely subglandular), and the appropriate implant. The current study evaluated 210 cases of augmentation mammaplasty retrospectively. A capsular contracture rate of 8% was found. Rupture and gel bleeding were observed in eight cases (4%). For various reasons, such as capsular contracture or implant rupture, the prostheses were renewed once in 16 patients (8%) and twice in 5 patients (2%). Submammary incision was used in 42 cases (20%). The patients in 23 cases responded that they had implant folds or edges they could feel (11%). Only 4 of these 23 patients stated that feeling the edge of the implant was a concern for them. Of the 210 augmented breasts, 5 had diminished sensation postoperatively (2%), as interpreted by the patient.     

210. Humanlebertransplantationen. Komplikationen und ihre Diagnose

Zusammenfassung Es wurden in der Chirurgischen Universitätsklinik Bonn 3 Humanlebertransplantationen durchgeführt.Als Frühkomplikationen wurden ischämische Schädigung der Spenderleber, rechtsseitige Zwerchfell-Lähmung sowie Thrombose der Arteria hepatica beobachtet, die klinisch, biochemisch und radiologisch leicht diagnostiziert werden konnten. Diese Komplikationen waren operativ bedingt und können in Zukunft vermieden werden.Als Spätkomplikationen waren die chronische Abstoßungsreaktion und die üchen Nebenwirkungen der Immunosuppressiva z. B. Steroidulcus, Steroiddiabetes, Infektionen u. a. auffällig. Diese Komplikationen können nur durch Verbesserung der Immunosuppression bzw. Erzeugung einer biologischen Immuntoleranz verhindert werden.

Human liver transplantations. Complications and their diagnosis

Summary Three human liver transplantations were carried out at the Surgical University Clinic, Bonn.Early complications were ischaemic damage to the donorliver, right-sided paralysis of the diaphragm and thrombosis of the hepatic artery, which could be diagnosed fairly easily on clinical, biochemical and radiological grounds. These complications were caused by the operation and can be avoided in the future.Late complications were the chronic rejection reaction and the usual side effects of immunosuppressioe drugs, e. g. steroid ulcers, steroid diabetes, infections, etc. These complications can only be prevented by better immunosuppression, that is, by the creation of a biological immune-tolerance.

     

Cauda equina hemangioblastoma at L5 vertebral level related to von Hippel-Lindau disease

Hemangioblastoma (HB) is uncommon, with only a few cases of hemangioblastoma with von Hippel-Lindau disease (VHL) located in cauda equina previously reported. We describe a case of hemangioblastoma misdiagnosed as nerve sheath tumour in the cauda equina.     

Zur AO-Osteosynthese von Knöchelbrüchen: Ergebnisse nach 5 Jahren

Zusammenfassung Der konservativen Behandlung der Luxationsfrakturen des oberen Sprunggelenkes sind Grenzen gesetzt, da es oft nicht gelingt, im geschlossenen Raum der Weichteilbedeckung alle Teilläsionen des Knochens und der Bänder anatomisch exakt zur Ausheilung zu bringen.Im Bestreben nach besserer anatomischer Wiederherstellung am Knochen und Bandapparat sind am Kantonsspital St. Gallen seit Ende 1960 Sprunggelenksfrakturen nahezu ausschließlich operativ-funktionell im Sinne der AO behandelt worden.1966 ist von Weber das Ergebnis dieser Methode an Hand einer geschlossenen Kontrollserie von 150 Fällen durchschnittlich 14,4 Monate nach Unfall und Operation überprüft worden. Die hier vorgelegten Ergebnisse der erneuten Kontrolle dieser Serie nach durchschnittlich 67,5 Monaten ergeben insgesamt eine zwischenzeitliche Verschlechterung von 9,5%. Mit 77,10% Restitutio ad integrum nach 51/2 Jahren sind aber die Behandlungsergebnisse immer noch so gut, wie es bis jetzt noch keine andere Behandlungsart erbringen konnte.

Summary The results of conservative treatment of ankle fractures prove often to be unsatisfactory; lesions of bone and ligaments are not restored anatomically, resulting in incongruency, provoking secondary arthrosis.To obtain an anatomical accurate restoration of bone and ligaments is the endeavour of the orthopaedic department at the Kantonsspital St. Gallen. Therefore since 1960 the ankle fractures have been operated as a routine.In 1966 Weber published a follow-up of 150 cases on an average 14.4 months after operation according to the ASIF-methods.In this paper we report the results of the same series on an average 67.5 months after fracture. Meanwhile in 9.5% the recorded results showed to be worse. However, restitutio ad integrum in 77.1% after more than 5 years prove the benefit of internal fixation and post-operative functional after-treatment.

Résumé Le traitement conservateur des fractures-luxations de la cheville ne permet pas la réduction anatomique parfaite de toutes les lésions ligamentaires et osseuses.Désirant une réduction anatomique parfaite de l'os et des ligaments, nous avons traité à l'hôpital cantonal de St. Gall depuis 1960 presque toutes les fractures de la cheville par ostéosynthèse selon l'AO.Weber a présenté en 1966 le résultat de cette méthode après avoir contrôlé une série de 150 cas, 14,4 mois après l'opération.Les résultats d'un nouveau contrôle 67,5 mois après l'opération, sujet de ce présent travail, mettent en évidence pour la même série une dépréciation intermédiaire de 9,5%.Toutefois, le traitement opératoire avec 77,10% de restitutio ad integrum après 67,5 mois donne des résultats nettement supérieurs à ceux des autres techniques.
