Objective: Immediate and early loading of dental implants can simplify treatment and increase overall patient satisfaction. The purpose of this 3‐year prospective randomized‐controlled multicenter study was to assess the differences in survival rates and bone level changes between immediately and early‐loaded implants with a new chemically modified surface (SLActive). This investigation shows interim results obtained after 5 months. Material and methods: Patients ≥18 years of age missing at least one tooth in the posterior maxilla or mandible were enrolled in the study. Following implant placement, patients received a temporary restoration either on the day of surgery (immediate loading) or 28–34 days after surgery (early loading); restorations consisted of single crowns or two to four unit fixed dental prostheses. Permanent restorations were placed 20–23 weeks following surgery. The primary efficacy variable was change in bone level (assessed by standardized radiographs) from baseline to 5 months; secondary variables included implant survival and success rates. Results: A total of 266 patients were enrolled (118 males and 148 females), and a total of 383 implants were placed (197 and 186 in the immediate and early loading groups, respectively). Mean patient age was 46.3±12.8 years. After 5 months, implant survival rates were 98% in the immediate group and 97% in the early group. Mean bone level change from baseline was 0.81±0.89 mm in the immediate group and 0.56±0.73 mm in the early group (P<0.05). Statistical analysis revealed a significant center effect (P<0.0001) and a significant treatment × center interaction (P=0.008). Conclusions: The results suggested that Straumann implants with an SLActive can be used predictably in time‐critical (early or immediate) loading treatment protocols when appropriate patient selection criteria are observed. The mean bone level changes observed from baseline to 5 months (0.56 and 0.81 mm) corresponded to physiological observations from other studies, i.e., were not clinically significant. The presence of a significant center effect and treatment × center interaction indicated that the differences in bone level changes between the two groups were center dependent.  相似文献

Objective: Immediate and early loading of implants can simplify treatment and increase patient satisfaction. This 3‐year randomized‐controlled trial will therefore evaluate survival rates and bone‐level changes with immediately and early loaded Straumann implants with the SLActive surface. Material and methods: Partially edentulous patients ≥18 years of age were enrolled. Patients received a temporary restoration (single crown or two to four unit fixed partial denture) out of occlusal contact either immediately (immediate loading) or 28–34 days later (early loading group), with permanent restorations placed 20–23 weeks after surgery. The primary endpoint was change in crestal bone level from baseline (implant placement) to 12 months; the secondary variables were implant survival and success rates. Results: A total of 383 implants (197 immediate and 186 early) were placed in 266 patients; 41.8% were placed in type III and IV bone. The mean patient age was 46.3±12.8 years. Four implants failed in the immediate loading group and six in the early loading group, giving implant survival rates of 98% and 97%, respectively (P=NS). There were no implant failures in type IV bone. The overall mean bone level change from baseline to 12 months was 0.77±0.93 mm (0.90±0.90 and 0.63±0.95 mm in the immediate and early groups, respectively; P<0.001). However, a significant difference in implantation depth between the two groups (P<0.0001) was found. After adjusting for this slight difference in initial surgical placement depth, time to loading no longer had a significant influence on bone‐level change. Significant influence was found for: center (P<0.0001), implant length (P<0.05) and implant position (P<0.0001). Bone gain was observed in approximately 16% of implants. Conclusions: The results demonstrated that Straumann implants with the SLActive surface are safe and predictable when used in immediate and early loading procedures. Even in poor‐quality bone, survival rates were comparable with those from conventional or delayed loading. The mean bone‐level change was not deemed to be clinically significant and compared well with the typical bone resorption observed in conventional implant loading.  相似文献

Abstract: In this prospective study, the clinical outcome of 2‐months loaded implants placed in the posterior jaws was evaluated after up to 3 years of functional loading. 405 Osseotite® implants with microtextured acid‐etched surface were consecutively placed in 11 fully and 164 partially edentulous patients (mean age 53.5± 15.3 (S.D.) years) using a one‐stage technique. 282 implants, supporting 154 restorations, were placed in the mandible, while 123 implants, supporting 75 restorations, were placed in the maxilla. A total of 99 single‐tooth restorations, 119 short‐span fixed bridges and 11 full‐arch prostheses were applied. The mean time interval from surgery to connection of provisional restoration was 2.0±0.7 months. 9 failures were reported up to 3 years of follow‐up; of these, 4 mandibular and 2 maxillary implants failed during the initial healing period (before prosthesis connection), while 3 mandibular implants failed after loading. No other complications occurred throughout the study. Life table analysis showed a cumulative survival rate of 97.5% for the mandible and 98.4% for the maxilla. Post‐loading implant survival rate was 98.9% for the mandible and 100% for the maxilla. The excellent outcome of this interim report suggests that microtextured Osseotite® implants in the posterior jaws may safely bear a functional load applied 2 months after insertion. The results of the present study are particularly encouraging for implants placed in the maxilla, since both a significant reduction of the healing period compared to the 6 months suggested by the classical Brånemark protocol, and an excellent post‐loading function may be achieved also in low quality bone.  相似文献

Background: Results from some studies clearly suggest that immediate loading can achieve equal success rates as those found in delayed or unloaded implants. There is still a lack of knowledge about the role of surface oxide properties during the peri‐implant bone healing processes. Purpose: The aim of this study was a clinical follow‐up study of immediately loaded implants with a porous anodized surface. Materials and Methods: A total of 142 TiUniteTM (Nobel Biocare, Göteborg, Sweden) implants were inserted from January to September 2001. All implants have been used in fixed restorations. Fifty implants were inserted in completely edentulous mandibles, and 69 implants were inserted in completely edentulous maxillae. All 119 implants were subjected to immediate functional loading (IFL) (immediate restoration with full occlusal contact). The other 23 implants, inserted in 12 patients, underwent immediate nonfunctional loading (INFL) (immediate restoration without occlusal contact) in different anatomical configurations (single tooth, small bridges in the anterior mandible, anterior maxilla, and posterior maxilla). All 142 implants have been followed for at least 3 years. Results: All implants appeared to be osseointegrated from a clinical and radiographic point of view. No failures were observed in the IFL and INFL groups. The implant success was 100%. The mean marginal bone loss was 0.8 and 1.0 mm at 12 and 36 months, respectively. Conclusion: Implants with a porous anodized surface appear to work well under an immediate loading state in the long term.  相似文献

Objectives: To evaluate and compare the outcome of dental implants placed using a flapless protocol and immediate loading with a conventional protocol and loading after 6 weeks. Materials and methods: Fourteen patients with bilateral maxillary edentulous areas were treated using Straumann SLA‐implants. Using a randomized split‐mouth design, implants were placed in one side of the maxilla using a stereolithographic surgical guide for flapless surgery and immediately loaded on temporary abutments with a bridge (test). Implants in the other side were placed using the conventional protocol and loaded after 6 weeks of healing (control). Clinical and radiographic evaluation of peri‐implant tissues was performed at time of implant surgery, and after 1 week, 6 weeks, 3, 6, 12 and 18 months. Results: A total of 70 implants were placed (36 test and 34 control). One implant (test) was lost after 3 months, resulting in a survival rate of 97.3% for the test implants and 100% for the control implants. Marginal bone levels were not statistically significantly different between the test and control implants but at baseline the marginal bone level was significantly lower compared to the other evaluation periods (P < 0.05). The mean bone level for test and control implants was 1.95 mm ± 0.70 and 1.93 mm ±0.42 after 18 months, respectively. There was a significant change in height of the attached mucosa at implants placed with a conventional flap between post‐operative and 1 week and between 1 week and 6 weeks. Statistically significant differences were found between the test side and the control side for opinion about speech, function, aesthetics, self‐confidence and overall appreciation the first 6 weeks. Conclusion: Implants can successfully integrate in the posterior maxilla using a flapless approach with immediate loading similar to a conventional protocol. The mucosal tissues around implants placed with a conventional flap changed significantly compared with flapless placed implants. To cite this article:

Objective: The functional, esthetic, and psychological problems of 3–6 months of edentulousness and also discomfort of two‐stage surgery might cause many patients to avoid implant‐supported restorations. Therefore, the immediate non‐functional loading protocol might be of value. Methods: Twenty Astra Tech implants, 11–13 mm in length and 3.5–4 mm in diameter, were placed in maxillary and mandibular posterior sites of 10 healthy patients (two for each) with adequate alveolar bone. Temporary prostheses were relieved of all centric and eccentric contacts. Functional permanent prostheses were fabricated 13 weeks postoperatively. Clinical and radiographic examinations (including the implant stability quotient [ISQ]) were carried out immediately after implantation and 3, 6, and 12 months postoperatively. Results: A 100% success rate was observed within 1 year. The plaque and bleeding indices, and pocket‐probing depths did not show a significant change throughout the study. The ISQ values after implant insertion, and 3, 6, and 12 months later were respectively 76.6 ± 6.57, 77.6 ± 10.54, 81.8 ± 6.8, and 82.43 ± 3.7. The increase was statistically significant (P=0.004) according to the Freidman test. The ISQ values measured after the implant insertion were significantly different from those measured in the sixth and 12th postoperative months (P<0.008) according to the Wilcoxon signed‐ranks test. The mean crestal alveolar bone resorption values in the third, sixth, and 12th postoperative months were 0.4 ± 0.55, 0.48 ± 0.37, and 0.48 ± 0.21 mm, respectively; and the change in marginal alveolar bone loss was not significant according to the Freidman test. Conclusion: Within the limitations of this pilot study, non‐functional immediate loading might meet the expectations of both the patients and clinicians. To cite this article:

Background

Few prospective studies about early loading of short implant have been available and very little evidence exists on the outcomes longer than 3 years.

Purpose

To assess clinical and radiographic outcomes of 6 mm‐short implants placed in the posterior maxilla and mandible applying an early loading protocol.

Materials and methods

Ninety‐five short implants (6 mm‐short, Ø 4 mm) were placed in 45 subjects at 3 study sites, 2 or 3 implants per subject, using a one‐stage surgical procedure and loaded with a screw‐retained splinted fixed prosthesis 6 weeks later. Follow‐up took place at 6, 12, 24, and 36 months after loading. Marginal bone level changes, implant survival, clinical variables, and adverse events were assessed.

Results

The survival rate for all implants placed was 95.8%. From implant loading to 3 years follow‐up, mean marginal bone level changes were minimal (0.07 ± 0.49 mm) and the peri‐implant soft tissue status was healthy. No major technical or biological complications occurred except for the 4 early implant losses.

Conclusion

Three‐year data indicates that the use of splinted 6 mm‐short implants is a viable treatment in posterior regions with low marginal bone resorption. Early loading after 6 weeks should be taken cautiously in patients with known risk factors.  相似文献

Purpose: The aim of this two‐center study was to evaluate screw‐type titanium implants with a chemically modified, sandblasted and acid‐etched surface when placed in the posterior maxilla or mandible, and loaded 21 days after placement. Material and Methods: All 56 patients met strict inclusion criteria and provided informed consent. Each patient displayed either a single‐tooth gap, an extended edentulous space, or a distal extension situation in the posterior mandible or maxilla. Eighty‐nine dental implants (SLActive®, Institut Straumann AG, Basel, Switzerland) were inserted according to an established nonsubmerged protocol and underwent undisturbed healing for a period of 21 days. Where appropriate, the implants were loaded after 21 days of healing with provisional restorations in full occlusion. Definitive metal ceramic restorations were fabricated and positioned on each implant after 6 months of healing. Clinical measurements regarding soft tissue parameters and radiographs were obtained at different time points up to 24 months after implant placement. Results: Of the 89 inserted implants, two (2.2%) implants failed to integrate and were removed during healing, and two (2.2%) additional implants required a prolonged healing time. A total of 85 (95.6%) implants were therefore loaded without incident after 21 days of healing. No additional implant was lost throughout the study period, whereas one implant was lost to follow‐up and therefore left unaccounted for further analysis. The remaining 86 implants all exhibited favorable radiographic and clinical findings. Based on strict success criteria, these implants were considered successfully integrated 2 years after insertion, resulting in a 2‐year success rate of 97.7%. Conclusion: The results of this prospective two‐center study demonstrate that titanium implants with a modified SLA surface can predictably achieve successful tissue integration when loaded in full occlusion 21 days after placement. Integration could be maintained without incident for at least 2 years of follow‐up.  相似文献

PURPOSE: The aim of the present study was to evaluate the concept of an immediate loading protocol in the posterior maxilla and mandible through analysis of implant survival at 1 year. MATERIALS AND METHODS: One year follow-up data of a multicenter study are reported. Eighty-two ITI sandblasted, acid-etched (SLA) implants in 40 patients were loaded between 0 and 11 days after implant placement (mean 4.3 +/- 2.8 days). The restorations consisted of either 2 splinted crowns or a 3-unit fixed prosthesis. All restorations were put into full functional occlusion. Periapical radiographs were evaluated for changes in crestal bone level from baseline to 1 year postloading. Primary stability of the implants was checked initially and before the fitting of the definitive prosthesis. The restorations were evaluated by the practitioners for retention, stability, and esthetics. RESULTS: Three patients' implants were not loaded because of lack of primary stability, and a fourth patient was excluded from the study because of a protocol violation (more than 4 implants were used). All 4 patients were successfully treated outside the protocol. The overall survival rate of the remaining implants at 1 year was 98.8%. The mean bone loss at 1 year was 0.52 +/- 0.98 mm, which is within the reported limits of less than 1 mm (range 0.4 to 1.4 mm) loss in the first year. DISCUSSION AND CONCLUSION: The early results from this study indicate that early and immediate loading of 2 implants in the posterior maxilla and mandible may be suitable in selected patients. On the basis of 1 year of observation, the results appear similar to those achieved with a delayed procedure.  相似文献

Background: A minimal marginal bone loss around implants during early healing has been considered acceptable. However, the preservation of the marginal bone is related to soft tissue stability and esthetics. Implant designs and surfaces were evaluated to determine their impact on the behavior of the crestal bone. The purpose of this study is to evaluate histologic marginal bone level changes around early loaded, chemically modified, sandblasted acid‐etched–surfaced implants with a machined collar (MC) or no MC (NMC). Methods: Three months after a tooth extraction, 72 sandblasted acid‐etched chemically modified implants were placed in six dogs. Thirty‐six implants had NMC, and 36 implants had a 2.8‐mm MC. All implants were loaded 21 days after placement. For histologic analyses, specimens were obtained at 3 and 12 months. Assessments of the percentage of the total bone‐to‐implant contact and linear measurements of the distance from the shoulder of the implant to the first bone‐to‐implant contact (fBIC) were performed. Based on fBIC measurements, estimates of bone loss were obtained for each implant. A mixed‐model analysis of variance was used to assess the effects of implant type and sacrifice time. Results: All implants achieved osseointegration. The mean bone gain observed around NMC early loaded implants (at 3 months: 0.13 ± 0.37 mm; at 12 months: 0.13 ± 0.44 mm) was significantly different from the mean bone loss for MC early loaded implants (at 3 months: ?0.32 ± 0.70 mm; at 12 months: ?0.79 ± 0.35 mm) at 3 months (P = 0.003) and 12 months (P <0.001). No infrabony component was present at the marginal fBIC around NMC implants in most cases. There were no statistically significant differences among the means of total bone contact for implant types. Conclusions: Chemically modified, sandblasted acid‐etched–surfaced implants with NMC presented crestal bone gain after 3 and 12 months under loading conditions in the canine mandible. The implant design and surface were determinants in the marginal bone level preservation.  相似文献

PURPOSE: The authors' objective was to measure crestal bone level change in subjects with immediately loaded implants and to identify risk factors associated with changes in bone level. MATERIALS AND METHODS: A retrospective cohort study design was used. The sample comprised subjects who had had endosseous implants placed and immediately loaded between July 2001 and July 2003. Demographic, health status-related, anatomic, implant-specific, prosthetic, and surgical variables were examined. The primary outcome variable was change in crestal bone level over time. Appropriate uni-, bi-, and multivariate statistics were computed. RESULTS: The sample comprised 174 subjects who received 347 immediately loaded implants. The mean duration of radiographic follow-up was 6.9 +/- 4.0 months, respectively. Mean changes in radiographic bone level were -0.5 mm and -0.6 mm on the mesial and distal surfaces, respectively, after a mean of 6.9 months of radiographic follow-up. Using least squares methods, it was estimated that radiographic bone levels would be -1.0 mm and -0.8 mm on the mesial and distal surfaces, respectively, at 12 months. The multivariate model revealed that radiolucency at or adjacent to implant site was associated with an increased risk of crestal bone loss (odds ratio, 1.88; 95% CI, 1.00 to 3.60). Twelve months after placement, 92.5% of implants had had < or = 1.5 mm of crestal bone loss. DISCUSSION: The results of this study were comparable to the results of other studies comparing immediate loading to delayed loading. Further research to estimate long-term changes in crestal bone loss and to identify risk factors for bone loss with immediate loading is recommended. CONCLUSION: This study suggests that crestal bone level changes with immediately loaded implants were within the recommended range for 92.5% of the evaluated implants. The mandible showed a higher risk for crestal bone loss compared to the maxilla.  相似文献

Purpose: The aim of this prospective study was to assess clinical outcomes and peri‐implant bone level changes around tilted and axial implants supporting full‐arch fixed immediate rehabilitations up to 60 months of loading. Material and Methods: Forty‐seven patients (22 women and 25 men) were included in the study. Each patient received a full‐arch fixed bridge supported by two axial and two distal tilted implants. Loading was applied within 48 hours of surgery. Patients were scheduled for follow‐up at 6, 12, 18, 24 months, and annually up to 5 years. At each follow‐up, plaque level and bleeding scores were assessed and radiographic evaluation of marginal bone level change was performed. Periapical radiographs were taken using a paralleling technique, and subsequently scanned at 600 dpi. An image analysis software was used to assess bone level. Results: A total of 33 mandibles and 16 maxillae were rehabilitated (two patients received a fixed prosthesis in both arches). One hundred ninety‐six Nobel Biocare implants of 4 mm diameter were placed. The mean follow‐up duration was 52.8 months (range 30–66 months) in the mandible, and 33.8 months (range 22–40 months) in the maxilla. All subjects attended the scheduled follow‐up visits. No implant was lost. No significant difference in marginal bone loss was found between axial and tilted implants in both jaws, at each follow‐up. No significant difference in bone loss was found between mandible and maxilla, for both axial and tilted implants at each comparable time frame, although slightly higher mean values were always found for the mandible. Conclusion: The use of tilted implants in the immediate rehabilitation of fully edentulous jaws is safe and is not associated to a higher marginal bone loss as compared to axially placed implants.  相似文献

Background: Immediate or early loading of implants placed in maxillas and posterior mandibles has been a concern as bone density is often low in these areas, making it difficult to establish good initial implant stability. By adapting implant design and insertion protocols, however, high initial implant stability may be achieved in these regions. Further, a modified implant surface texture has been proved to help in maintaining stability during the initial healing period. Purpose: The aim of the present study was to investigate the clinical performance of oxidized titanium implants (TiUnite?, Nobel Biocare AB, Gothenburg, Sweden) when used for early function in the maxilla and in the posterior mandible, locations where the bone density often is low. A further aim was to evaluate the marginal bone level at oxidized implants and compare it with that of machined‐surface implants used in a previous study. Materials and Methods: Thirty‐one patients were consecutively included in the study, and 37 edentulous areas in maxillas and posterior mandibles were treated. Bruxism and uncontrolled periodontal disease were exclusion criteria. Temporary prostheses were generally placed within 9 days but not after 16 days from implant placement. A previous study applying the same study design and clinical protocol but using machined‐surface implants was used for comparisons. Results: Of the 111 implants installed, 1 failed, giving an overall survival rate of 99.1% after 18 months. The prosthesis survival was 100%. The marginal bone resorption was 0.8 mm (standard deviation ISD], 1.0), as opposed to 1.6 mm (SD, 1.3) in the previous study with machined‐surface implants, but was not statistically significantly different (p= .10). Conclusion: The present clinical protocol (aiming at high primary stability) and the use of oxidized titanium implants for early functional loading in the maxilla and the posterior mandible resulted in a high implant survival rate and a favorable marginal bone level during a follow‐up of 18 months. The difference in marginal bone resorption between the oxidized implants in the present study and the machined implants from a previous investigation with the same study design was not statistically significant.  相似文献

Objective: To evaluate the effect of the timing of loading on bone‐to‐implant contact (BIC) following immediate placement of implants with a hydrophilic sandblasted, large‐grit and acid‐etched surface (modSLA) into fresh extraction sockets in a minipig model. Material and methods: Six minipigs were used in this study. In each hemi‐mandible, two conical shape implants (TE, Straumann implants) with a hydrophilic surface (modSLA) were placed in fresh extraction sockets. In one side of the mandible (control), two implants were immediately placed in fresh extraction sockets. The implants were loaded after 4 weeks of healing. At the contralateral side (test), two implants were immediately placed and loaded. After 8 weeks of healing, the animals were sacrificed and histologically analysed. Results: During the experimental period, no implants were lost and all of them presented to be osseointegrated. The percentage of BIC was similar in both groups: 66.1% and 65.1% for the control and test group, respectively. Furthermore, the distance from the shoulder of the implant to bone crest and the distance from the shoulder to the first BIC were similar in both groups. Conclusion: Immediate implant placement and loading showed similar BIC with immediate placement and delayed loading when implants with a modSLA surface were used. Both procedures showed similar buccal bone crest levels, which presented some resorption irrespective of the treatment modality. To cite this article:

Introduction: The use of immediate implant loading protocols delivers obvious benefits to the patient. When applied in healed sites, this has not only been well documented in the totally edentolous mandible but has also been documented and reported to be predictable in the upper jaw, and in cases of partial edentoulism, as well. A further application of immediate loading protocol, although still controversial, especially when replacing single maxillary teeth in the anterior zone, is the immediate implant placement and provisionalization in postextractive sockets. In consideration of the oxidized surface promoting bone healing and the tapered shape of the implant body, the Replace Select Tapered TiUnite implants have been used for many years in our clinic when facing these clinical situations. This article will report about our long‐term clinical experience with such implants and the relevant role of a correct surgical and prosthetic treatment planning. Purpose: The aim of this retrospective study was to report on the 5‐year clinical and radiologic outcome of patients treated with Replace Select Tapered TiUnite implants when used according to an immediate loading protocol in postextraction sites. Method and Materials: In routine practice, 56 consecutive patients were treated with 79 implants. The patients, 23 males and 33 females, had a mean age of 50.9 years, range 21–76 years, at implant placement. Forty‐seven implants were placed in the maxilla and 32 implants were placed in the mandible. All implants were placed in postextraction sites and were immediately loaded. Provisional restorations were delivered within 2 hours from surgery and all were in occlusion. Forty‐three patients received a single implant while in the remaining 13 patients the implants were splinted. Definitive prosthetic restoration was delivered within 1 to 4 months following implant placement. Evaluations of soft tissue health and marginal bone remodeling were conducted. An independent radiologist performed the radiographic evaluation using the top of the implant as the reference point with negative values indicating a level below the reference point. Results: Forty‐eight patients, accounting for 66 implants, have passed the 5‐year follow‐up. No implants have failed resulting in a 5‐year cumulative implant survival rate of 100%. Three patients, with six implants, withdrew during the course of the follow‐up; one patient passed away and two patients moved. Five patients with seven implants did not show up at 5 years recall. At the 5‐year follow‐up, majority of the implants that were followed demonstrated normal periimplant mucosa and no visible plaque. The mean bone level at 5‐year follow‐up was ?2.45 mm (SD 1.29, n = 63) demonstrating a level in line with the first thread. Mean marginal bone loss from implant inserting to 5 years was 0.56 mm (SD 1.98, n = 63). In regard with complications, a fracture of the ceramic crown was reported 5 years after implant insertion in a patient who developed bruxism. No other biologic nor mechanical complications were reported. Conclusion: This retrospective 5‐year follow‐up study of 56 patients treated with implants immediately placed in postextraction sockets and immediately loaded demonstrates good treatment outcome with regard to implant survival, soft tissue condition, and marginal bone response.  相似文献

Objective: The aim of this study was to compare the survival rate, the bone loss and soft‐tissue healing patterns of immediately loaded and immediately restored implants in cases of partial posterior mandibular edentulism. Material and methods: Fifty patients with partial posterior mandibular edentulism were randomly selected for two treatments: 25 were included in the immediate loading group (test) and 25 in the immediate restoration group (control). All implants were placed in healed sites with a torque of >25 N cm. The temporary prosthesis of the immediate restoration group was placed so as to avoid occlusal contact in centric and lateral excursions. Both groups received fully occluding final restorations 6 months after surgery. Mean marginal bone loss was assessed at 6‐, 12‐, 24‐ and 36‐month follow‐up examinations by a blinded examiner. Results: A total of 100 implants were placed in the period between February 2004 and October 2006, of which 42 (42%) were for men and 58 (58%) for women. Five and 7 weeks after surgery, mobility of one implant was assessed in one (4%) patient in the test group and one (4%) patient in the control group, respectively. At the 36‐month follow‐up, the accumulated mean marginal bone loss was 0.987 mm (SD=0.375) for the immediate restoration group (n=48) and 0.947 mm (SD=0.323) for the immediate loading group (n=48). There was no statistically significant difference (P>0.05) for the tested outcome measures between the two procedures. Conclusions: This study was unable to detect any statistically significant difference in the survival rate, bone loss and soft tissue healing patterns between the immediately loaded and the immediately restored implants in cases of partial posterior mandibular edentulism. The immediate temporary rehabilitation of the partially edentulous posterior mandible is a predictable procedure using both procedures. To cite this article:

PURPOSE: The aim of the present prospective study was to evaluate the concept of early loading of rough-surfaced implants in the completely edentulous maxilla and in the edentulous posterior mandible and maxilla. MATERIALS AND METHODS: Fifty-four consecutive patients were treated. Twenty patients were completely edentulous in the maxilla (group A), 19 patients were edentulous in the posterior left and/or right maxilla (group B), and 15 patients were edentulous in the posterior left and/or right mandible (group C). One patient in group B and 5 in group C were bilaterally treated. Two hundred thirty-four solid screw-type, sandblasted, large-grit, acid-etched (SLA) ITI implants were placed, 58 (25%) immediately after tooth extraction. Mean placement torque and standard deviations were measured at all sites. Sixty fixed prostheses were delivered after a mean delay of 9 days (range, 4 to 22 days). Mean marginal bone reduction was measured after 1 year of loading. RESULTS: Two implants were lost (0.9%), 1 before functional loading and 1 after 1 year. All other implants were clinically stable, with a mean marginal bone loss of 0.75 mm (+/-1.3 mm). Marginal bone loss ranged from 0 to 3.5 mm. Mean placement torque on implants placed in healed bone or immediately after tooth extraction ranged from 29.1+/-9.3 Ncm to 35.5+/-5.8 Ncm. No statistical difference was found (P > .05) between implants placed in healed bone and those placed immediately after tooth extraction. DISCUSSION: There is little documentation for immediate or early loading in the areas studied. However, in this study, favorable results were obtained in 54 consecutive patients in these regions. CONCLUSION: In this study population, early loading protocols can be applied with predictable results using rough-surfaced implants for rehabilitation of the completely edentulous maxilla, posterior maxilla, and posterior mandible.  相似文献

The single-tooth implant: a viable alternative for single-tooth replacement   总被引:2,自引:0,他引:2  
BACKGROUND: The use of dental implants for single-tooth replacement cases introduces a challenge to the performance of these restorations. Clinical data suggest that single-tooth implant restorations do not benefit from lateral support from the surrounding dentition as do bridged implants, and that these restorations are subjected to greater masticatory forces. Therefore, single-tooth implants are exposed to an increased risk of failure. This prospective clinical trial was designed to evaluate the cumulative success rate of dual acid-etched single-tooth replacement implants in two clinical centers. METHODS: The study protocol included 59 patients between 19 and 73 years. Implants were evaluated for mobility, infection, and crestal bone loss from 30.9 to 60 months. RESULTS: Seventy-one (71) implants were placed in 59 patients, 20 male and 39 female. Forty-seven (47) were placed in the maxilla and 24 in the mandible. The percentages of anterior and posterior implants were 45.1% and 54.9%, respectively. A total of 13 (18.4%) implants were placed in soft, poor quality (Type IV) bone. Beginning 1 month post-implant placement, cases were followed for 30.9 to 60 months with a mean follow-up of 45.9 months. One implant failed (peri-implant infection) yielding an overall success rate of 98.6%. CONCLUSIONS: This success rate for single-tooth replacement dual acid-etched implants compares favorably with bridged implants and with success rates of other single-tooth implant studies. Dual acid-etched implants performed well even under conditions of poor quality bone, where concomitant bone augmentation was performed, and when used for immediate replacement of extracted teeth.  相似文献

Objectives: The purpose of this study was to prospectively evaluate the clinical and radiographic outcomes of immediately loaded full‐arch fixed prostheses supported by a combination of axially and non‐axially positioned implants in a large cohort of patients with completely edentulous jaws, up to 5 years of function. Materials and methods: One hundred and seventy‐three edentulous patients (80 males and 93 females) were enrolled according to specific selection criteria. Each patient received a full‐arch fixed prosthesis supported by two distal tilted implants and two anterior axially placed implants. The provisional functional acrylic prosthesis was delivered the same day as surgery in all cases. All cases were finalized 4–6 months later. The patients were scheduled for follow‐up at 6 and 12 months of function, and annually up to 5 years. At each follow‐up plaque and bleeding score was assessed and radiographic evaluation of marginal bone level was performed. Results: The overall follow‐up range was 4–59 months. A total of 154 immediately loaded prostheses (61 in the maxilla and 93 in the mandible) were in function for at least 1 year and were considered for the analysis. Four axially placed implants failed in the maxilla and one tilted implant in the mandible, all within 6 months of loading. No further implant failure occurred to date. Implant survival at 1 year was 98.36% and 99.73% for the maxilla and the mandible, respectively. Marginal bone loss at 1 year averaged 0.9±0.7 mm in the maxilla (204 implants) and 1.2±0.9 mm in the mandible (292 implants). No difference was found in marginal bone loss between axial and tilted implants. Plaque and bleeding scores progressively improved from 6 to 12 months. Fracture of the acrylic prosthesis occurred in 14% of total cases. Conclusions: The present preliminary results from a relatively large sample size suggest that the present technique can be considered a viable treatment option for the immediate rehabilitation of both mandible and maxilla. Purpose: The aim of this multicenter study was to evaluate success and restorative problems of ITI (ITI Dental Implant System®, Straumann AG, Basel, Switzerland) implant‐supported posterior partial prostheses in Jordan. Materials and Methods: One‐hundred forty‐one ITI implants were placed in the posterior region of the mandible or the maxilla in 66 patients at multiple clinical practices in Jordan. The age of the patients ranged from 17 to 85 years. The implants were retrospectively analyzed from the first date of placement in September 1999 until May 2006. Eighty‐three implants (58.87%) were placed in the mandible, and 58 (41.13%) in the maxilla. The implants were loaded with either cement‐retained single‐ or multiple‐tooth replacements. Results: Three maxillary implants of two male patients have been lost (2.13% of the total and 5.12% of the maxillary implants). Failed implants were of wide‐neck type with 6‐mm lengths. Moreover, in another two male patients, two single implants at the maxillary premolar region exhibited significant bone loss from the buccal side of the implant surface (2‐mm bone resorption). Those two implants are still functioning and were included in calculating the survival rate but not the success rate. Therefore, the cumulative survival rate for both arches and genders was 97.87% and that for male patients in the maxillary region was 94.88%. The cumulative success rate for both arches and genders was 96.45% and that for the maxillary region was 86.21%. The corresponding rates concerning implants in female patients and the posterior mandible of both genders were 100% for both survival and success rates. Only one crown (mandibular) and another two abutment bridge (maxillary) were decemented in different patients (2.13%). Conclusions: The survival and success rates of implants placed in male patients and in the maxilla were lower than that of implants placed in female patients and in the mandible. Cement‐retained restorations showed minimal complications.  相似文献
