The Importance of Different Atrioventricular Delay for Left Ventricular Filling in Sequential Pacing: Clinical Implications 总被引:1,自引:0,他引:1

We assessed the influence and clinical consequences of different AV delay on ventricular filling in 30 patients (mean age 60 ± 5 years) who had ODD pacemakers for AV block. All 30 patients presented a normal ejection fraction, but in 18 cases (Group I), an echo-Doppler examination revealed ventricular hypertrophy (mean end-diastolic wall thickness of 1.4 ± 0.16 cm. LV mass index 155 ± 17g/m2 ), and an abnormal relaxation pattern (isovoiumetric relaxation time = 124.72 ± 11.82; early to late peak velocity = 0.6 ± 0.03; deceleration time - 296.83 ± 34.02 ms). Group II included the remaining 12 patients who had a normal filling pattern. In all 30 patients, the pattern was reassessed following modification of the AV delay (200, 150, 100. and 75 ms). Patients at baseline (AV delay of 200 ms) also underwent an exercise test with determination of respiratory gas exchange. In Group I, 13 (72.5%) patients were classified as Weber class B (VO2 Max 16.8 ± 1.7mL/min per kg); and 5 (27.5%) were Class A (VO2 Max 22.5 ± 1.4 mL/min per kg). In Group II, all 12 patients were classified as Weber Class A. In Group II, changes in AV delay caused no consistent variations in filling pattern, and therefore AV delay was not modified. In Group I patients, since reduction to 100 ms resulted in normalization of the filling pattern, the AV delay was programmed to 100 ms. A graded exercise test repeated after 6 months follow-up showed an improved Weber class in 13 patients (from B to A) and greater VO2 Max in the remaining five already in Class A. We concluded that, in sequential paced patients with normal ejection fraction but abnormal relaxation pattern, modification in AV delay can induce normalization of filling and improvement in cardiac functional capacity. 相似文献

Our study included eight patients with physiological pacemakers programmed to the VDD mode. The blood velocity in the aortic root was determined by Doppler echocardiography. Changes in the integral of maximum velocity reflect stroke volume changes. The Doppler probe was placed and held in the suprasternal position while programming the pacemakers from the VVI to all available AV delays in the VDD mode; the heart rates were nearly constant. Stroke volumes were highly dependent on changes of the AV delay; the optimum AV delay varied considerably from patient to patient. These changes were more pronounced in the patients with left ventricular strain due to aortic valve disease than in the patients with isolated conduction defects. 相似文献

Atrial synchronous pacing with short, nonphysiologicai atrioventricular (AV) intervals has been reported to increase cardiac output in selected patients with severe dilated heart failure. The aim of this study was to determine the acute effect of atrial synchronous pacing with short AV intervals in a consecutive series of patients with dilated heart failure. Twelve patients with a mean ejection fraction of 21 %± standard error 2.5% were studied. Pacing catheters were placed in the high right atrium and right ventricular apex and a balloon flotation catheter in the pulmonary artery for measurement of cardiac output. Simultaneous transthoracic echocardiography was performed for measurement of left ventricular filling time and mitral regurgitation. In a randomized crossover design, measurements were made during VDD pacing at programmed AV intervals of 100 and 60 msec and during a control period in sinus rhythm. Left ventricular filling time increased at AV intervals of 100 and 60 msec (mean difference 37 ± 9 and 34 ± 11 msec, respectively, both P < 0.01 compared to control). Despite increases in ventricular filling time, stroke, and cardiac index declined with short atrioventricular intervals (at an AV interval of 60 msec, stroke index fell by 2.1 ± 0.5 mL/m2, P < 0.05 and cardiac index by 125 ± 45 mL/m2; P = NS). Heart rate was unchanged at both AV intervals (78 ± 4.9 at control, 78 ± 5.2 at 100 msec and 79 ± 4.9 beats/min at 60 msec; P = NS). The decrease in stroke index at an AV Interval of 60 msec was inversely related to control left ventricular filling time (r = 0.74; P = 0.01) and ejection fraction (r = 0.69; P = 0.02) and directly related to heart rate (r = 0.77; P < 0.01). The change in stroke index at an AV delay of 60 msec was also inversely related to the change in mitral regurgitation induced by pacing (r = 0.84; P = 0.01). Thus, in a group of patients with stable dilated heart failure, atrial synchronous pacing with short AV intervals did not improve cardiac output. The change in cardiac output with pacing was inversely related to baseline left ventricular function and to the change in mitral regurgitation induced by pacing. 相似文献

Dual chamber pacing (DDD) maintains atrioventricular (AV) sequence; AV delay programmability modifies the relationship between atrial and ventricular contraction. To evaluate the hemodynamic effects of such a modification, ten patients with a DDD unit for complete AV block were studied by time-motion (M-mode) and Doppler echocardiography during inhibited ventricular pacing (VVI), atrial-triggered ventricular pacing (VDD) and atrioventricular sequential pacing (DVI) at different AV delay (90, 140, 190, 240 msec). A significant improvement in stroke volume (SV) (15%-20%, P less than 0.05) was seen during DDD versus VVI pacing; no changes, however, were observed in the same patient with different AV delay or during DVI versus VDD pacing. These data suggest that programming of AV delay does not affect systolic performance at rest; longer diastolic filling times recorded during DDD pacing with "short" AV delay (90-140 msec) do not seem to be a hemodynamically relevant epi-phenomenon of PM programming. 相似文献

The effect of this change on cardiac output [CO) was studied in closed chest, AV blocked canine preparations (N: 10), during AV sequential pacing (80 bpm). CO (thermodilution technique) and transmitral flow velocity (TMFV, pulsed-wave Doppler) were measured and compared (paired t-test) on the basis of TMFV pattern, when atrial contraction (A wave) started just after early diastolic transmitral flow deceleration [PR:219 ± 25 ms, mean ± SD) and when A wave occurred at the end of late diastole and shortened due to the next ventricular contraction (PR: 56 ± 11 ms). The short AV delay resulted in 12.0 ± 5.9% decrease of CO, reflecting the interrupted late diastolic atrial transport. Background: Prolonged right ventricle (RV) apical pacing is associated with left ventricle (LV) dysfunction due to dysynchronous ventricular activation and contraction. Alternative RV pacing sites with a narrower QRS compared to RV pacing might reflect a more physiological and synchronous LV activation. The purpose of this study was to compare the QRS morphology, duration, and suitability of RV outflow tract (RVOT) septal and mid‐RV septal pacing. Methods: Seventeen consecutive patients with indication for dual‐chamber pacing were enrolled in the study. Two standard 58‐cm active fixation leads were passed to the RV and positioned in the RVOT septum and mid‐RV septum using a commercially available septal stylet (model 4140, St. Jude Medical, St. Paul, MN, USA). QRS duration, morphology, and pacing parameters were compared at the two sites. The RV lead with less‐satisfactory electrical parameters was withdrawn and deployed in the right atrium. Results: Successful positioning of the pacing leads at the RVOT septum and mid‐RV septum was achieved in 15 patients (88.2%). There were no significant differences in the mean stimulation threshold, R‐wave sensing, and lead impedance between the two sites. The QRS duration in the RVOT septum was 151 ± 14 ms and in the mid‐RV septum 145 ± 13 ms (P = 0.150). Conclusions: This prospective observational study shows that septal pacing can be reliably achieved both in the RVOT and mid‐RV with active fixation leads using a specifically shaped stylet. Background: The detrimental effects of right ventricular apical pacing on left ventricular function has driven interest in selective site pacing, predominantly on the right ventricular outflow tract (RVOT) septum. There is currently no information on long-term ventricular lead electrical performance from this site.
Methods: A total of 100 patients with ventricular lead placement on the RVOT septum undergoing pacemaker implantation for bradycardia indications were analyzed retrospectively. Lead positioning was confirmed with the use of fluoroscopy. Long-term (1 year) follow-up was obtained in 92 patients. Information on stimulation threshold, R-wave sensing, lead impedance, and lead complications were collected.
Results: Lead performance at the RVOT septal position was stable in the long term. Ventricular electrical parameters were acceptable with stable long-term stimulation thresholds, sensing, and impedance for all lead types. One-year results demonstrated mean stimulation threshold of 0.71 ± 0.25 V, mean R wave of 12.4 ± 6.05 mV, and mean impedance values of 520 ± 127 Ω. There were no cases of high pacing thresholds or inadequate sensing.
Conclusions: This study confirms satisfactory long-term performance with leads placed on the RVOT septum, comparable to traditional pacing sites. It is now time to undertake studies to examine the long-term hemodynamic effects of RVOT septal pacing. 相似文献

Acute Hemodynamic Improvement by Pacing in Patients with Severe Congestive Heart Failure 总被引:9,自引:0,他引:9
Since the first report on dual chamber pacing for congestive heart failure (CHF) in 1991, a number of investigators have explored the topic with conflicting results. These conflicts may arise from an incomplete understanding of the mechanisms by which pacing improves cardiac function. Potential mechanisms include: (1) increase in filling time: (2) decrease in mitral regurgitation: (3) optimization of left heart mechanical atrioventricular delay (left heart MAVD); and (4) normalization of ventricular activation. One or more of these mechanisms may be operative in an individual patient, implying that patients may require individuol optimization. Acute pacing studies were conducted on nine CHF patients, NYHA Class II-III to Class IV. Measurements of conduction times in sinus rhythm revealed: (1) normal interatrial conduction times (59 ± 5 ms) in all patients, with wide variations in interventricular conduction times (range, ?15–105 ms); and (2) a wide range of left heart MAVD (range, 97–388 ms). While pacing the right, left, or both ventricles, measurement of high fidelity aortic pressure and mitral and aortic velocities revealed the following: (1) 6 of 9 patients increased mean pulse pressure over sinus value during RV orLV pacing at an optimal A V delay: (2) the maximum aortic pulse pressure was achieved when the atrium was not paced: an 8% increase over sinus pulse pressure with paced RV versus a 5% decrease for paced atrium and RV at optimum AV delay (paired Student's t-test, P = 0.01), and a 0% increase over sinus with paced LV versus 7% decrease for paced atrium and LV at optimum AV delay, P < 0.05: (3) significant dependence on pacing site was noted, with 4 patients doing best with RV pacing. 3 patients achieving a maximum with LV pacing, and 2 patients showing no preference; and (4) 2 of 4 patients with restrictive filling patterns were converted to nonrestrictive patterns with optimum pacing. Patient hemodynamics appear to benefit acutely from individually optimized pacing. Increases in filling time, optimization of left heart MAVD, and normalization of intraventricular activation are the most significant mechanisms. This study compares LV performance during high right ventricular septal (RVS) and apical (RVA) pacing in patients with LV dysfunction who underwent His-bundle ablation for chronic AF. We inserted a passive fixation pacing electrode into the RVA and an active fixation electrode in the RVS. A dual chamber, rate responsive pulse generator stimulated the RVA through the ventricular port and the RVS via the atrial port. Patients were randomized to initial RVA (VVIR) or RVS (AAIR) pacing for 2 months. The pacing site was reversed during the next 2 months. At the 2 and 4 month follow-up visit, each patient underwent a transthoracic echocardiographical study and a rest/exercise first pass radionuclide ventriculogram. We studied nine men and three women (mean age of 68 +/- 7 years) with congestive heart failure functional Class (NYHA Classification): I (3 patients), II (7 patients), and III (2 patients). The QRS duration was shorter during RVS stimulation (158 +/- 10 vs 170 +/- 11 ms, P < 0.001). Chronic capture threshold and lead impedance did not significantly differ. LV fractional shortening improved during RVS pacing (0.31 +/- 0.05 vs 0.26 +/- 0.07, P < 0.01). RVS activation increased the resting first pass LV ejection fraction (0.51 +/- 0.14 vs 0.43 +/- 0.10, P < 0.01). No significant difference was observed during RVS and RVA pacing in the exercise time (5.6 +/- 3.2 vs 5.4 +/- 3.1, P = 0.6) or the exercise first pass LV ejection fraction (0.58 +/- 0.15 vs 0.55 +/- 0.16, P = 0.2). The relative changes in QRS duration and LV ejection fraction at both pacing sites showed a significant correlation (P < 0.01). We conclude that RVS pacing produces shorter QRS duration and better chronic LV function than RVA pacing in patients with mild to moderate LV dysfunction and chronic AF after His-bundle ablation. 相似文献

The effect of atrialventricular versus ventricular pacing and contraction were studied in seven open-chest dogs. Cardiac output, left ventricular, left atrial, right atrial and pulmonary artery pressures were recorded. The right or left ventricular apical areas were consistently superior as ventricular pacing sites.
Appearance of cannon A waves within the pre- or ejection period produced a significant decrease in left ventricular and systemic blood pressure, and cardiac output with a concomitant increase in right atrial, ventricular and pulmonary pressures. Prominent "v" waves were also observed during these periods.
Reducing the basic driving cycle length from 400 to 300 msec caused a marlted deterioration of all hemodynamic parameters with the appearance of mechanical alternans. Random VA conduction or ventricular pacing in the presence of com-plete AV and VA heart block appeared to offer a more favorable hemodynamic result than constant 1:1 VA conduction. Background: Right ventricular septal pacing has been proposed as an alternative to apical pacing. However, data concerning thresholds and requirement for lead repositioning with this technique are scant.
Methods: We reviewed data at implantation and follow-up of 362 consecutive recipients of the same model of active fixation lead (Medtronic 5076-58, Minneapolis, MN, USA) to avoid differences due to lead characteristics. Patients were divided into two groups according to whether the lead was positioned on the interventricular septum (n = 157) or at the right ventricular apex (n = 205). Thresholds, lead impedance, and requirement for lead repositioning were compared between groups at implantation and follow-up.
Results: There were no differences between the septal and apical groups in sensing and pacing thresholds or lead impedance, either at implantation or during a 24-month follow-up. In the septal group, the lead had to be repositioned in four patients (2.5%) due to lead dislodgement in two patients, acute threshold rise in one patient, and pericardial effusion in another patient (the lead had unintentionally been positioned on the anterior free wall in these last two patients). In the apical group, the lead had to be repositioned in eight patients (3.9%, P = 0.56) due to lead dislodgement in three patients and acute threshold rise in five patients.
Conclusions: Acute and chronic thresholds associated with septal pacing are similar to those observed with apical pacing, and risk of lead dislodgement is low. However, multiple radioscopic views must be used to avoid inadvertent positioning of the lead on the anterior free wall . 相似文献

Background: Right ventricular (RV) apical pacing deteriorates left ventricular (LV) function. RV nonoutflow (low) septal pacing may better preserve ventricular performance, but this has not been systematically tested. Our aim was to assess (1) whether long‐term RV lower septal pacing is superior to RV apical pacing regarding LV volumes and ejection fraction (EF), and (2) if the changes in LV dyssynchrony imposed by pacing are related to the long‐term changes in LV volumes and EF. Methods: In thirty‐six patients with atrioventricular (AV) block, a dual‐chamber pacemaker was implanted. The ventricular electrode was placed either at the apex or at the lower septum, in a randomized sequence. Twenty‐four to 48 hours following implantation, we measured LV volumes, EF, and LV dyssynchrony (by tissue Doppler imaging), both with and without pacing. Patients were reassessed echocardiographically after 12 months. Results: Lower septal pacing induced a more synchronized pattern of LV contraction changes (P < 0.05). Following 12 months, differences were observed between groups regarding LV volumes and EF. EF increased within the septal group (from 52 ± 3.3% to 59 ± 3.0%, P < 0.05). A significant inverse relation was documented between changes in LV dyssynchrony and changes in EF (r =?0.64, P < 0.05). Conclusions: In patients with AV block, RV nonoutflow septal pacing represents an attractive alternative, since it preserves better and may even improve LV volumes and EF. Atrioventricular Sequential Pacing: Comparison with Ventricular Pacing Using Systolic Time Intervals

Nine patients with programmable atrioventricular sequential pacers were studied using systolic time intervals (QS2—the total electrical and mechanical systole, left ventricular ejection time, and pre-ejeclion period). These measurements were obtained by simultaneous recording of the electrocardiogram, phonocardiogram. and carotid pulse tracing. There was a dramatic fall of left ventricular ejection time (LVET) and an increase of the pre-ejection time (PEP) in all patients when the pacers were programmed from the atrioventricular to the ventricular mode at constant heart rate. This resulted in an increase of the ratio PEP/LVET from 428 to 574 suggesting loss of ventricular function. These measurements all reversed to baseline values when the pacers were reprogrammed back to the atrioventricular mode. This study suggests systolic time intervals might be useful to select non-invasively pacer parameters such as mode, rate, and effective PR interval in order to provide the best hemodynamics in a given patient. 相似文献

The hemodynamic responses of atrial lAF], atrioventricu-lar sequential (AVP) and ventricuJar pacing (VP) were compared to sinus rhythm (SfiJ in seventeen anesthetized dogs with intact AV conduction. The atrium and/or ventricle were paced at fixed rates above the control sinus rate. An AV interval shorter than normal conduction was selected to capture the ventricle. Background: The detrimental effects of right ventricular (RV) apical pacing on left ventricular function has driven interest in alternative pacing sites and in particular the mid RV septum and RV outflow tract (RVOT). RV septal lead positioning can be successfully achieved with a specifically shaped stylet and confirmed by the left anterior oblique (LAO) fluoroscopic projection. Such a projection is neither always used nor available during pacemaker implantation. The aim of this study was to evaluate how effective is the stylet-driven technique in septal lead placement guided only by posterior-anterior (PA) fluoroscopic view.
Methods: One hundred consecutive patients with an indication for single- or dual-chamber pacing were enrolled. RV septal lead positioning was attempted in the PA projection only and confirmed by the LAO projection at the end of the procedure.
Results: The RV lead position was septal in 90% of the patients. This included mid RV in 56 and RVOT in 34 patients. There were no significant differences in the mean stimulation threshold, R-wave sensing, and lead impedance between the two sites . In the RVOT, 97% (34/35) of leads were placed on the septum, whereas in the mid RV the value was 89% (56/63).
Conclusions: The study confirms that conventional active-fixation pacing leads can be successfully and safely deployed onto the RV septum using a purposely-shaped stylet guided only by the PA fluoroscopic projection. (PACE 2010; 49–53) 相似文献

目的 应用定量组织速度成像 (QTVI)评价不同房室 (AV)间期对DDD双腔起搏患者的左室收缩功能的影响以探求最佳AV间期。方法 3 0例植入DDD双腔起搏器的患者 ,在DDD起搏方式下将AV间期程控为 10 0ms、13 0ms、15 0ms、170ms、2 0 0ms ,测定后间隔、侧壁、前壁、下壁、前间隔和后壁六个部位二尖瓣环的收缩期平均峰值速度(Vs)及从QTVI曲线上A波结束到S波开始的时间 (TA -S) ,并将Vs与二维法所测的心输出量 (CO )作相关分析。结果 QTVI所测定的二尖瓣环的Vs与CO呈正相关 (r =0 .62 ,P <0 .0 1)。不同的AV间期明显影响Vs的值 ,TA -S则随AV间期的延长而增大。以Vs为标准 ,DDD起搏方式下最佳AV间期为 ( 15 4± 2 4)ms ,相应的TA -S 的平均值为 ( 7.6±2 .3 )s- 2 。结论 DDD双腔起搏时房室 (AV)间期影响左室收缩功能。QTVI可用于选择DDD双腔起搏的最佳AV间期。 相似文献

We hypothesized that pacing, which provided a rapid uniform contraction of the ventricles with a narrower QRS, would produce a better stroke volume and cardiac output (CO). We sought to study whether pacing simultaneously at two sites in the right ventricle (right ventricular apex and outflow tract) would provide a narrower QRS and improved CO in 11 patients undergoing elective electrophysiology studies. Patients were studied by transthoracic echocardiography measurement of CO using the Doppler flow velocity method in normal sinus rhythm, AOO pacing (rate 80), DOO pacing in the right ventricular apex (AV delay 100 ms). DOO pacing in the right ventricular outflow tract, and DOO pacing at both right ventricular sites simultaneously in random order. The COs were 5.42 ± 1.83, 5.61 ± 1.97. 5.67 ± 1.6. 5.84 ± 1.68. and 5.86 ± 1.52 L/min, respectively (no significant difference by repeated measures analysis of variance [ANOVA]). The QRS durations were 0.09 ± 0.02, 0.09 ± 0.02. 0.13 ± 0.027, 0.13 ± 0.03, and 0.11 ± 0.03 sees respectively. Repeated measures ANOVA showed that the QRS duration significantly increased with right ventricular apex or right ventricluar outflow tract pacing compared to sinus rhythm and AOO pacing (P < 0.001) but then diminished with pacing at both sites (P < 0.01). QRS duration was not correlated with CO, however the change in QRS duration correlated significantly with the change in CO when pacing was performed at the two right ventricular sites simultaneoasly. In conclusion, during DOO pacing, there was a trend for pacing in the right ventricular outflow tract or both sites to improve the CO compared to the right ventricular apex. With simultaneous pacing at both ventricular sites, the QRS narrowed. Further studies will be required to see if this approach has value in patients with poor left ventricular function or congestive heart failure. 相似文献

Animal data and recent findings in humans have questioned the appropriateness of pacing the heart from the right ventricular apex. Numerous, mostly small sized, studies have evaluated alternative sites within the right ventricle. There is now sufficient evidence that right ventricular apical pacing in patients with left ventricular dysfunction with or without heart failure is detrimental. Pacing from the right side of the heart as an attempt at nonpharmacological therapy for heart failure, turns out to be obsolete. In antibradycardia pacing with the need for continuous ventricular support, the interest in preserving left ventricular function drives the ongoing search for the most favorable pacing site within the right ventricle. Results, so far, are conflicting which may be attributed to the inhomogeneity of patient groups, the small cohorts studied, the differing protocols used, and the lack of accepted definitions of right ventricular lead positions. Larger studies are needed to evaluate intraoperative criteria for optimal lead placement and the potential benefit of nonapical right ventricular pacing. (PACE 2004; 27[Pt. II]:855–861) 相似文献
