[Purpose] This study attempted to identify how genu varum or valgum affects the electromyographic activities of the vastus medialis, vastus lateralis, and rectus femoris during knee isometric contraction. [Subjects] Fifty-two healthy young adults were enrolled in this study. They were enrolled and classified into three groups by knee alignment conditions: the genu varum, genu valgum, and control groups. [Methods] The electromyographic activity ratio of the vastus medialis to the vastus lateralis and rectus femoris were calculated using the percentage of maximum voluntary contraction. The participants contracted their quadriceps during isometric contraction at 30 and 60° of knee flexion. [Results] The genu varum group had more activity in the vastus medialis than in the vastus lateralis and rectus femoris, whereas the genu valgum group had more activity in the vastus lateralis and rectus femoris than in the vastus medialis. There was a significant difference in the muscle activity ratio between the vastus medialis and vastus lateralis by angle of knee flexion degree only in the genu valgum. There were no significant differences in any of the three groups in terms of the muscle activity ratio of the vastus medialis to the rectus femoris by angle of knee flexion. [Conclusion] The quadriceps femoris was used for different strategies according to knee alignment during isometric contraction at 30 and 60°. This study suggests that rehabilitation training programs used to strengthen the quadriceps should consider the knee alignment conditions of the target subjects.Key words: Electromyography, Quadriceps muscle, Isometric contraction  相似文献

[Purpose] This investigation evaluated the effect of real-time feedback on electrical activation of the quadriceps during 3 weight-bearing tasks of the Wii Fit Plus®. [Subjects] Thirty male healthy volunteers were recruited. [Methods] Activation of the vastus medialis, vastus lateralis, and rectus femoris muscles was recorded during virtual lunge, single leg extension, and single leg reach exercises. Each exercise was performed twice in 3 randomized experimental conditions (with visual feedback, with auditory feedback, and with no feedback). The normalized electromyographic data (using maximum voluntary isometric contraction) were analyzed using repeated measures analysis of variance and Tukey’s test. [Results] No significant difference was found in the muscles among the feedback conditions during the 3 exercises. However, the variation in the muscle activity of the vastus medialis and vastus lateralis (18.23–29.20% of maximum voluntary isometric contraction) was higher (47–62%) than that in the rectus femoris (7.35–12.98% of maximum voluntary isometric contraction). [Conclusion] Real-time feedback did not alter quadriceps activation during the Wii tasks. Additionally, these games showed electromyographic activation levels similar to those for the same tasks outside the virtual environment. The Wii weight-bearing tasks could therefore constitute a physical activity program but without the additional benefit of feedback.Key words: Electromyography, Functional exercises, Exergames  相似文献

[Purpose] To investigate the effects of a seven-week quadriceps stretching program on the muscle fibre orientation of the vastus medialis oblique and vastus lateralis in the lower limbs by ultrasound imaging. [Participants and Methods] Twenty-seven healthy, physically fit, asymptomatic females and males (age 21.5 ± 1.3, Tegner activity level score ≥4) were recruited. Their initial vastus medialis oblique and vastus lateralis fibre angles were determined using ultrasound. They then undertook a seven-week quadriceps stretching program, 3 sets of stretches to be performed on both lower limbs, 3 times a week on 3 separate days. One volunteer was assigned as an intra-rater control and did not take part in the stretching program. The vastus lateralis and vastus medialis oblique fibre angles were measured again on completion of the exercise regime. [Results] A statistically significant decrease in muscle fibre angle was observed in both the right and left vastus medialis oblique, and the right and left vastus lateralis. [Conclusion] A 7-week stretching program can result in a significant decrease in muscle fibre angle in both the vastus medialis oblique and the vastus lateralis. This can help in understanding the effects of prescribed stretching exercises on athletic patients with PFP.  相似文献

[Purpose] We investigated the difference in onset time between the vastus medialis and lateralis according to knee alignment during stair ascent and descent to examine the effects of knee alignment on the quadriceps during stair stepping. [Subjects] Fifty-two adults (20 with genu varum, 12 with genu valgum, and 20 controls) were enrolled. Subjects with > 4 cm between the medial epicondyles of the knees were placed in the genu varum group, whereas subjects with > 4 cm between the medial malleolus of the ankle were placed in the genu valgum group. [Methods] Surface electromyography was used to measure the onset times of the vastus medialis and vastus lateralis during stair ascent and descent. [Results] The vastus lateralis showed more delayed firing than the vastus medialis in the genu varum group, whereas vastus medialis firing was more delayed than vastus lateralis firing in the genu valgum group. Significant differences in onset time were detected between stair ascent and descent in the genu varum and valgum groups. [Conclusion] Genu varum and valgum affect quadriceps firing during stair stepping. Therefore, selective rehabilitation training of the quadriceps femoris should be considered to prevent pain or knee malalignment deformities.Key words: Quadriceps muscle, Genu varum, Genu valgum  相似文献

[Purpose] The purpose of this study was to examine the vastus medialis oblique to vastus lateralis ratio in two pelvic tilt positions while performing the sit-to-stand task. [Subjects and Methods] Activation of the vastus medialis oblique and the vastus lateralis muscles of 46 healthy subjects (25 males, 21 females) were recorded by surface electromyography during the STS task with anterior pelvic tilt (sit with thoracolumbar spine extended and pelvis in an anterior tilt) and neutral pelvic tilt (sit with thoracolumbar spine relaxed and pelvis in the neutral tilt position) positions. Changes in vastus medialis oblique, vastus lateralis activation and the vastus medialis oblique/vastus lateralis ratio were analyzed. [Results] Vastus medialis oblique and vastus lateralis muscle activation significantly increased in neutral pelvic tilt position, but the vastus medialis oblique/vastus lateralis ratio was not statistically different. [Conclusion] The sit-to-stand procedure with neutral pelvic tilt position increased activation of the vastus medialis oblique and vastus lateralis, usefully strengthening the quadriceps, but did not selectively activate the vastus medialis oblique muscle.Key words: Anterior pelvic tilt, Neutral pelvic tilt, Sit-to-stand task  相似文献

[Purpose] The purpose of the present study was to examine the effects of squat exercises performed on different surfaces on the activity of the quadriceps femoris muscle in order to provide information on support surfaces for effective squat exercises. [Subjects and Method] Fourteen healthy subjects performed squat exercises for five seconds each on three different support surfaces: hard plates, foam, and rubber air discs. Their performance was measured using electromyography. As the subjects performed the squat exercises on each surface, data on the activity of the vastus medialis oblique and the vastus lateralis, and the vastus medials oblique/vastus lateralis ratio, were collected. [Results] The activity of the vastus medialis oblique and the vastus medialis oblique/vastus lateralis ratio were found to be statistically significantly higher on rubber air discs than when the squats were performed on hard plates or foam. [Conclusion] To activate the vastus medialis obilique, and to enhance the vastus medialis oblique/vastus lateralis ratio, unstable surfaces that are highly unstable should be selected.Key words: Support surfaces, Vastus medialis obilique, Vastus lateralis  相似文献

[Purpose] The muscle strength of the quadriceps muscle is critical in patellofemoral pain syndrome. The quadriceps muscle supplies the power for dynamic patellar movement, and the vastus medialis oblique (VMO) and vastus lateralis (VL) enable the patella to stabilize during tracking. We followed the theories about open and closed kinetic chain exercises to design two exercises, sling open chain knee extension (SOCKE) exercise and sling closed chain knee extension (SCCKE) exercise. The purpose of our study was to research the changes in quadriceps muscle activity during both exercises. [Methods] Electromyographic analysis was used to explore the different effects of the two exercises. The MVC% was calculated for the VMO and VL during exercise for analysis. [Results] We found that the mean MVC% values of the VMO and VL during the SOCKE exercise were higher than those during the SCCKE exercise. The ratio of the VMO to VL was 1.0 ± 0.19 during the SOCKE exercise and 1.11 ± 0.15 during the SCCKE exercise. [Conclusions] The SOCKE exercise is targeted at quadriceps muscle training and has a recruitment effect on the VMO. OBJECTIVE: To determine the effects of a weight-bearing rehabilitation program on quadriceps and gluteus medius electromyographic activity, pain, and function in subjects diagnosed with patellofemoral pain syndrome (PFPS). DESIGN: Pretest and posttest 6-week intervention study. SETTING: Musculoskeletal research laboratory. PARTICIPANTS: Fourteen subjects diagnosed with PFPS and 14 healthy control subjects volunteered to participate in this study. No subjects withdrew from the study because of adverse effects. INTERVENTION: Subjects diagnosed with PFPS participated in a 6-week rehabilitation program. The rehabilitation program consisted of weight-bearing exercises that focused on strengthening the quadriceps and hip abductor musculature. MAIN OUTCOME MEASURES: Electromyographic onsets of the vastus medialis oblique (VMO) and vastus lateralis and onset and duration of the gluteus medius were collected during a stair-stepping task that was performed during the pretest and posttest. A visual analog scale (VAS) and Functional Index Questionnaire (FIQ) were administered at pretest and posttest and each week of the intervention. RESULTS: Vastus lateralis and VMO onset timing differences (vastus lateralis electromyographic onset minus VMO electromyographic onset) and VAS and FIQ scores significantly improved for patients diagnosed with PFPS. Vastus lateralis and VMO onset timing in the PFPS group were significantly different from those in the control group at baseline and were not significantly different from the control group after the intervention. We did not find differences in gluteus medius onsets or duration of activity. CONCLUSIONS: Subjects diagnosed with PFPS responded favorably and quickly to a therapeutic exercise program that incorporated quadriceps and hip musculature strengthening. The efficacy of the therapeutic exercise program used in this study should be further investigated in a larger subject population.  相似文献

OBJECTIVE: To evaluate whether using surface electromyography to assess skeletal muscle fatigue during an isometric exercise has the potential to be clinically useful in patients with coronary artery disease (CAD). DESIGN: Double sample comparative study. SETTING: Cardiac rehabilitation service in France. PARTICIPANTS: Sixteen men with documented CAD and 9 age-matched healthy men. INTERVENTIONS: Assessment of quadriceps skeletal muscle fatigue on an isokinetic apparatus with surface electromyography measurements and a symptom-limited exercise test in a laboratory. MAIN OUTCOME MEASURES: The maximal voluntary isometric force (MVIF) of the quadriceps was quantified as a measure of muscle strength and isometric endurance was defined as the time required to sustain a contraction at 50% of MVIF until exhaustion. Surface electromyography signals were recorded from the vastus lateralis, rectus femoris, and vastus medialis during isometric endurance. The root mean square (RMS) and the median frequency (MF) were directly calculated on a computer and then normalized (as a percentage of the initial value). RESULTS: Muscle strength did not differ significantly between the patients with CAD and the healthy subjects (229+/-21N/m vs 228+/-52N/m), but isometric endurance was reduced (64+/-17s vs 90+/-7s, P <.01). The RMS values showed a significantly higher increase in the healthy subjects versus the patients with CAD for the vastus lateralis and vastus medialis ( P <.001). The MF values were significantly lower for the vastus lateralis, rectus femoris ( P <.01), and vastus medialis ( P <.05) in patients with CAD compared with the healthy subjects. CONCLUSIONS: Skeletal muscle fatigue occurs sooner in men with CAD relative to matched healthy men, despite similar muscle strength. This finding may be the result of an abnormality of skeletal muscle function and may play an important role in measuring functional capacity. In addition, it may be a useful tool to assess the efficacy of cardiac rehabilitation interventions.  相似文献

[Purpose] This study evaluated the effective selective activation method of the vastus medialis oblique for knee joint stabilization in patients with patellofemoral pain syndrome. [Subjects and Methods] Fifteen healthy college students (9 males, 6 females); mean age, height, and weight: 22.2 years, 167.8 cm, and 61.4 kg, respectively) participated. The knee angle was held at 60°. Muscle activities were measured once each during an ordinary squat and a squat accompanied by hip joint adduction. The muscle activities of the vastus medialis oblique and vastus lateralis were measured by electromyography for five seconds while maintaining 60° knee flexion. Electromyography signals were obtained at a sampling rate of 1,000 Hz and band pass filtering at 20–50 Hz. The obtained raw root mean square was divided by the maximal voluntary isometric contraction and expressed as a percentage. The selective activity of the vastus medialis oblique was assessed according to the muscle activity ratio of the vastus medialis oblique to the vastus lateralis. [Results] The activity ratio of the vastus medialis oblique was higher during a squat with hip joint adduction than without. [Conclusion] A squat accompanied by hip joint adduction is effective for the selective activation of the vastus medialis oblique.Key words: Patellofemoral pain syndrome, Vastus medialis oblique, Squat  相似文献

OBJECTIVE: This study examined the immediate effect of patellar taping with a standard force on the onset of vastus medialis obliquus and vastus lateralis activities before and after muscle fatigue in able-bodied subjects. DESIGN: This study tested 29 mature able-bodied subjects. The surface electromyographic onset time of their vastus medialis obliquus and vastus lateralis was measured after a posteroanterior perturbation at the knee in a single-legged standing position. The measurements were taken under three conditions in random order of true patellar taping, sham patellar taping, and no patellar taping. Afterward, subjects performed a knee-extension exercise until their quadriceps fatigued, and the above tests were repeated to test the effect of fatigue on the outcome. RESULTS: There was no statistically significant difference with patellar taping on the electromyographic onset time of the vastus medialis obliquus and vastus lateralis compared with the placebo-taping and no-taping conditions (P = 0.455). There was no statistically significant difference in onset of vastus medialis obliquus and vastus lateralis muscles before and after muscle fatigue (P = 0.304). CONCLUSIONS: The present study suggests that patellar taping does not enhance the temporal activation of vastus medialis obliquus in both fatigue and nonfatigue conditions on able-bodied subjects.  相似文献

BackgroundTo investigate the extent to which quadriceps muscle activation and strength are responsible for patellofemoral pain.MethodsA pain on–off switch system synchronized with a force transducer and surface electromyography was utilized on 32 volunteer patellofemoral pain patients during maximal isometric and squat exercises.FindingsThere were 26 patients out of the 32 tested who complained of pain during the squat or isometric test, of these 20 subjects presented a significant advantage for the vastus lateralis compared to the vastus medialis obliquis activation and 12 patients had decreased quadriceps strength of the symptomatic compared to the non symptomatic leg. All patients who demonstrated weak vastus medialis obliquis activation during the isometric exercise possessed the same symptoms during the squat. On the other hand, 9 patients who showed diminished vastus medialis obliquis activation during the squat displayed equal activation between the vastus medialis obliquis and the vastus lateralis during the isometric task. With regard to the timing for the onset of muscle activation, there were only 4 patients who had a difference (P = 0.03) between the symptomatic (0.042 s) and non-symptomatic legs (0.011 s).InterpretationCauses for patellofemoral pain vary and are not necessarily a result of quadriceps strength deficit or vastus medialis obliquis activation weakness. Patellofemoral pain patients who possess lower vastus medialis obliquis activation compared to the vastus lateralis do not necessarily have quadriceps weakness while patients presenting with quadriceps strength deficits do not always have an imbalance between vastus medialis obliquis and vastus lateralis activation.  相似文献

目的:研究肌发电量在膝关节骨关节炎患者股四头肌肌力训练中的变化情况及股四头肌的详细变化情况。方法:采用股四头肌的等长收缩运动训练方法和其他疗法进行比较,用肌电图进行股直肌、股内侧肌、股外侧肌的肌发电量测定,同时用“膝关节功能评定表”进行功能的辅助评定。结果:两种方法治疗后,股直肌、股内侧肌的肌发电量都有显著的变化,尤其以股内侧肌的变化极显著,股外侧的肌发电量无显著变化。两种方法中肌力训练较其他治疗方法在提高肌发电量方面更为有效。结论:股内侧肌在膝关节肌力锻炼中的变化最大,说明股内侧肌比股直肌和股外侧肌在稳定膝关节方面更重要。股外侧肌在训练中的变化最小,在维护膝关节的稳定性方面起的作用比股内侧肌和股直肌小;肌力训练比其他方法在增加肌发电量方面更有效,更能提高膝关节的稳定性。  相似文献

[Purpose] This study investigated the effects of the slow speed-targeting squat exercise on the vastus medialis oblique/vastus lateralis ratio. [Subjects] Ten asymptomatic men were recruited. [Methods] The EMG activities of the vastus medialis oblique and vastus lateralis muscles were recorded using surface electrodes. The subject performed the squat exercise under 3 different conditions. [Results] The vastus medialis oblique/vastus lateralis ratio in condition 2 (1.5 ± 0.7) was significantly higher than that in conditions 1 and 3 (1.0 ± 0.5, 1.1 ± 0.8, respectively) [Conclusion] Therefore, an effectively slow movement speed is recommended for selective strengthening of vastus medialis oblique using a slow speed-targeting device that provides biofeedback.Key words: Slow movement speed, Squat exercise, VMO  相似文献

Both knee extension and straight leg raises have been used to rehabilitate atrophied quadriceps musculature following knee immobilization. A comparison of the tension developed during these two exercises was made in each of the three surface quadriceps muscles (quadriceps femoris, vastus lateralis, vastus medialis) at three different levels of activity (20%, 50%, and 80% of 1RM). Integrated EMG (m Volt-seconds) was used to measure tension developed during a seven second contraction. Tension developed was significantly greater during a seven second contraction. Tension developed was significantly greater during knee extension than during straight leg raises at each exercise intensity. For both exercises and at all three exercise levels, the vastus lateralis was the most active; the vastus medialis was the least active. The difference between knee extension and straight leg raises became greater with increasing levels of activity. Therefore, unless contraindicated by chondromalacia of lack of knee flexibility, knee extensions should be used for post-surgical knee rehabilitation. The greater tension development would lead to a more rapid return of strength, and therefore quicker rehabilitation.  相似文献

The effects of high frequency alternating knee flexion-extension on muscle activity of the quadriceps and hamstring muscle groups has been investigated. Standard loads were used for each subject. The muscle activity in vastus medialis, vastus lateralis, rectus femoris and the lateral hamstrings were recorded by electromyography during increasing velocities. Rectus femoris and hamstrings were found to increase their activities significantly with increasing speed while vastus medialis and vastus lateralis showed no such change. The individual thigh muscles thus differ in function in relation to the velocity of movement.  相似文献

背景:髌腱末端病是腱止点部位的微细损伤,股四头肌群中的股内侧肌与股外侧肌之间的力量平衡发生变化,可直接导致髌骨产生异常运动从而对腱止点部位产生影响。目的:分析髌腱末端病发生机制和髌腱末端病对膝关节及其周围肌肉活动产生的影响。方法:选择10名患有髌骨肌腱病的男运动员及10名运动项目、身高和年龄匹配的正常男运动员,采用瑞士CON-TREX等速肌力测试与训练系统,分别进行膝关节力量和表面肌电图的测试,比较与分析膝关节屈肌力矩、伸肌力矩和股四头肌表面肌电图的变化。结果与结论:发现在等长运动时,末端病组的伸肌伸肌峰力矩明显小于对照组、峰值转矩比比值明显大于对照组;在等速运动时,末端病组的伸肌伸肌峰力矩明显小于对照组,并随着运动角速度增加而变化显著,而峰值转矩比比值均不同程高于对照组;末端病组的股内侧肌/股外侧肌的整合肌电图比值在等长运动和等速运动时,均明显低于对照组。提示髌腱末端病运动员膝关节屈肌与伸肌的力量差距较为突出,存在股内侧肌活动低下和股内侧肌与股外侧肌之间不平衡的现象。  相似文献

OBJECTIVE: To evaluate the effect of inhibition and facilitation taping techniques on the activity of vastus lateralis and vastus medialis obliquus. DESIGN: Repeated-measures design. SETTING: Laboratory in an educational institution. SUBJECTS: Thirty healthy females aged between 18 and 23 years. INTERVENTION: Subjects were tested during the application of inhibition tape, facilitation tape and no tape to vastus lateralis in random order. MAIN OUTCOME MEASURES: Electromyographic activity of vastus lateralis and vastus medialis obliquus during stair descent was recorded using bipolar surface electrodes. RESULTS: Electromyographic activity of vastus lateralis and vastus medialis obliquus during the application of inhibition tape or facilitation tape did not alter significantly compared with no tape condition. CONCLUSIONS: Taping designed to inhibit or facilitate muscle activity via sensory stimulation does not affect vastus lateralis or vastus medialis obliquus activity in healthy subjects. Further investigation is required to find a means of muscle taping that could be recommended as an intervention for patients with patellofemoral pain syndrome.  相似文献
