目的利用锥形柬计算机断层扫描(CBCT)研究上颌第一磨牙近中颊根第二根管(MB2)发生率、根管解剖形态以及根管口位置分布。方法随机选取2011-2013年航天中心医院口腔科20~30岁患者CBCT影像160张(男80张、女80张)。平行近中颊根长轴平面观察其根管形态,并按照Vertucci分类统计。水平面观察根管口的位置,记录MB、MB2及腭根根管口间距离以及MB、MB2根管口分别到腭根根管口连线夹角,并比较不同性别间差异。结果本研究观察了279颗上颌第一磨牙,其中存在MB2牙齿233颗,检出率为83.5%,不同性别间差异无统计学意义(P〉0.05)。不同类型的根管出现概率不同,各类型出现率男女间无差异。MB与MB2根管口间距男性为(2.195±0.504)mm,女性为(1.966±0.419)mm,差异有统计学意义(P〈0.05)。结论上颌第一磨牙MB2发生率高,不同类型MB2发生概率不同,男性与女性之间无差异。MB与MB2根管口间距在两性之间存在统计学差异。  相似文献

??Objective    To investigate the oral health status of 6??20-year old visually impaired students in Shenyang and analyze the risk factors of caries. Methods    Oral health status??oral health knowledge and related behavioral habits of 103 students were surveyed by questionaires. The risk factors of caries were analyzed through logistic regression analysis. Results    Caries prevalence rate was 78.64%??and the total mean DMFT was 2.43 ± 2.75. The rate of gingival bleeding and dental calculus was 44.66% and 67.96% respectively. Malocclusion rate was 49.51%. Female students had significantly higher dental caries experience than the male ones??P??0.05??. Logistic regression analysis indicated that caries experience was related to the education degree of their mothers and the experience of toothache. Brushing teeth properly with fluoride toothpaste could reduce the incidence of dental caries. Conclusion    Oral health of visually impaired students is poor. Oral health education and promotion measures should be emphasized among disabled students.  相似文献

目的应用聚合酶链式反应-变形梯度凝胶电泳(PCR—DGGE)技术检测慢性根尖周炎患牙根管内中间普菌和链球菌定植情况,分析根管内细菌与患牙症状间关系。方法2011年12月至2013年5月于北京大学深圳医院口腔科就诊的27例慢性根尖周炎患牙根管内细菌样本,提取DNA,利用16SrDNA引物进行PCR—DGGE技术分析。结果27例共检出细菌菌属17种。中间普菌在17例有症状组中检测出16例(94.1%),在10例无症状组中检测出6例(60.0%),两组检出率差异有统计学意义(P〈0.05)。链球菌在17例有症状组中均未检测出,在10例无症状组中检测出4例(40.0%),两组检出率差异有统计学意义(P〈0.05)。结论慢性根尖周炎患牙以厌氧菌感染为主,根管内中间普菌、链球菌与临床症状相关。  相似文献

??Objective    To evaluate and compare the amount of debris and irrigant extrusion quantitatively by using rotary nickel-titanium ??Ni-Ti?? instrument and sonic handpiece in root canal preparation. Methods    Forty freshly extracted human teeth were selected??which were with single canals??having similar canal length and curvature??due to orthodontics therapy and periodontitis in Department of Oral and Maxillofacial Surgery of China Medical University Stomatology Hospital during October 2012 to January 2014. They were separated into two groups based on Ni-Ti instruments ProTaper and sonic handpiece and mounted in a debris collection apparatus. After each instrument change??1 mL of NaClO was used as the irrigant and the amount of irrigant extruded was measured by EP tube. The EP tube was weighed using an electronic microbalance to determine its weight. After drying in an incubator for five days??the EP tube was weighed again. Compare the amount of debris and irrigant extrusion quantitatively. Results    The median of the amount of irrigant??in the group of ProTaper it was 116.24 mg??in the group of sonic handpiece it was 13.76 mg. The rotary Ni-Ti instrumentation techniques extruded significantly more irrigant than sonic handpiece??P < 0.05??. The median of the amount of debris??in the group of ProTaper it was 4.87 mg??in the group of sonic handpiece it was 0.63 mg. The rotary Ni-Ti instrumentation techniques extruded significantly more debris than sonic handpiece??P < 0.05??. Conclusion    The sonic handpiece extrudes significantly less irrigant and debris than the rotary Ni-Ti instrumentation techniques. It has a good clinical application prospects.  相似文献

??To compare the amount of apically extruded debris from root canals when different nickel-titanium instruments were used. Methods    Sixty-eight healthy single-rooted premolar teeth extracted for orthodontic purpose were induded in the study. The teeth were randomly divided into the control group??hand ProTaper??and 3 experimental groups. Three kinds of nickel-titanium instruments were used??rotary ProTaper??Waveone and Mtwo??n = 17??. Debris extruded from the apical foramen after root canal preparation was collected into tubes and the amount of apically extruded debris was determined and analyzed. Results    The OD values of apically extruded debris??in the control group it was 68.65 ± 11.47??in rotary ProTaper group it was 62.66 ± 11.17?? in Waveone group it was 58.49 ± 12.96??in Mtwo group it was 47.70 ± 10.28. Waveone group and Mtwo group had less extruded debris??which was significantly different from the control group??P < 0.05??. Mtwo group had the least of extruded debris??which was significantly different from the other groups??P < 0.05??.  Conclusion    Mtwo nickel-titanium instruments produce obviously less apically extruded debris than ProTaper and Waveone.  相似文献

目的探讨开窗减压术治疗颌骨巨大囊性病变的临床应用价值。方法对2009年5月至2010年11月南方医科大学附属南海医院口腔科收治的8例颌骨囊性病变患者(包括1例根尖囊肿,1例含牙囊肿、5例角化囊肿、1例囊性成釉细胞瘤),经口内进行开窗减压,术后定期复诊,3个月复查曲面断层摄影,1年进行三维CT重建,测量术后囊腔缩小情况。结果 8例患者,最长的已观察18个月,最短的观察3个月,2例患者(包括1例根尖囊肿、1例成釉细胞瘤)囊腔完全消失,6例患者囊腔越来越缩小。结论开窗减压术是一种简便、经济、效果良好的治疗颌骨囊性病变的方法。  相似文献

下颌骨巨大囊肿,其骨质破坏严重,通常的治疗手段是将受累的下颌骨完全切除后,利用骨质移植或人工材料修复。但是此方法难度大,对患者创伤大,经济负担重,基层医院难以推广。2005—2007年我科采用开窗减压术治疗下颌骨巨大囊肿13例,避免了常规手术的一些缺点,手术效果满意。现报告如下。  相似文献

目的调查分析特殊教育学校学生的牙周健康状况。方法2010年1-3月从江西省吉安市特殊教育学校中选择3所学校,根据年级分层抽取不同班级,再从各班级中随机抽样7-18岁学生共157名;同时从该市普通中小学中采用同种抽样方法抽取112名学生为对照。采用视诊和探诊相结合方法检查全口牙周,检查的内容包括牙周健康状况、牙结石、牙龈出血、牙周袋和牙周附着丧失,并对两组学生的牙周检查情况进行对比。结果特殊教育学校学生牙结石检出率、牙龈出血栓出率、牙周袋检出率和附着丧失检出率均高于普通学校(P〈0.05)。结论特殊教育学校学生口腔卫生状况较差,应得到更多的关心和重视。  相似文献

上颌磨牙近中颊根根管截面形态的研究   总被引:2,自引:0,他引:2  
目的:研究国人离体上颌第一、二磨牙近中颊根根管的内径和截面形态,为临床进行上颌磨牙根管治疗提供理论参考。方法:收集离体上颌第一磨牙108个,上颌第二磨牙167个。自根管口水平截取近中颊根,包埋。自根尖面开始每隔1mm横切牙根至距根尖6mm处,第7个横切面平近中颊根的根管口水平面。根管显微镜(DOM)下观察、拍摄每个牙根的7个切片的冠面,记录每个冠面含有的根管数,测量各截面各根管的颊舌径和近远中径,并按标准记录根管截面形状。结果:上颌第一磨牙共截取756个切片,均见近中颊根主根管(MB),464个切片中可观察到近中颊根第二根管(MB2);上颌第二磨牙共截取1169个切片,含MB共1169个,MB2共442个;含MB2的标本数在上颌第一磨牙和第二磨牙分别为96个和90个,占88.89%和53.89%。近中颊根主根管(MB)的颊舌径多大于近远中径,截面形状以扁形和椭圆形多见,但愈近根尖,两径间的差距愈小,即MB愈近根尖,愈趋向成圆形:近中颊根第二根管(MB2)的颊舌径和近远中径间的差距变化不大,截面形状以圆形最多,椭圆形次之,扁形最少。结论:上颌第一、二磨牙近中颊根的根管截面形态复杂,提示临床上需配合有效的清理和成形技术以提高该牙位的根管治疗成功率。  相似文献

目的:探讨运用根管显微镜对经治的上颌第一磨牙遗漏近颊第二根管(MB2)的诊治方法。方法:选择需行根管再治疗的上颌第一磨牙64个。依据x线片特征,牙冠与髓底的形态特点判断是否存在MB2。采用斜方形开髓入口,小号K锉探查,必要时使用根管手术显微镜定位MB2根管。镍钛机用根管锉冠向下法预备根管,1%NaOCL冲洗根管系统,冷牙胶侧方加压与热牙胶垂直加压充填根管。记录近颊根根管分型、根管预备过程中并发症的情况,评价疗效。结果:再治疗中共发现MB2根管55个(85.81%),疏通45个(80%)。近颊根双根管以2—1型、2—2型较常见(76.56%)。治疗过程中未发生器械折断、管壁侧穿等并发症,治疗成功率为95.55%。结论:上颌第一磨牙MB2发生率高,通常狭窄、弯曲、隐蔽,是临床治疗的难点;临床医生有意识地寻找和在显微镜下的可视化操作能提高MB2根管的发现率和治疗的成功率。  相似文献

目的 探讨上颌第一磨牙近烦第二根管(second mesiobuccal canal,MB2)的特征及临床治疗方法。方法 选取患牙髓炎或根尖用炎的上颌第一磨牙95颗,拍摄术前X线片,术中采用小号K锉配合15%EDTA溶液探查根管,记录根管数目、形态和类型;采用机动镍钛器械Hero642进行根管预备,侧向加压充填技术充填根管。结果上颌第一磨牙MB2的发现率为81.1%,扩通的MB2占72.6%,多数MB2根管细小弯曲,需用小号锉配合使用EDTA进行疏通,机动镍钛器械预备根管无明显并发症。结论上颌第一磨牙MB2的发生率高,多数根管细小弯曲,小号K锉配合EDTA疏通根管,机动镍钛器械预备可获得良好的治疗效果。  相似文献

The aim of this study was to investigate whether the use of operating microscope in combination with ultrasonics increased the rate of second mesiobuccal (MB2) canal detection in permanent maxillary first molar teeth. A hundred extracted human maxillary first molars were assessed. After location of the main canals, the MB2 canal was sought in all teeth first without microscopy, then with the aid of the operating microscope and finally with the combined use of the operating microscope and ultrasonics. With these techniques, the MB2 canal was detected in 62%, 67% and 74% of the teeth, respectively. The combination of the operating microscope and ultrasonics detected significantly more MB2 canals than when no microscopy was utilized (P < 0.05). Sectioning of the roots disclosed the presence of the MB2 canal in 82% of the teeth. Twenty‐nine per cent of the teeth had a separate MB2 canal orifice and separate apical foramina. The results of this study suggested that the combined use of the operating microscope and ultrasonics increased the detection of MB2 canals in maxillary first permanent molars.  相似文献

目的:评价改进开髓口和应用根管显微镜(DOM)定位离体上颌第一、二磨牙近颊根第二根管(MB2)的能力.方法:收集离体上颌第一、二磨牙共550颗.在肉眼、传统三角形开髓口(N.E-1),肉眼、改进开髓口(N.E-2)和根管显微镜辅助(DOM)三种不同条件下分别探查根管,记录MB2数目.结果:N.E-1、N.E-2和DOM三种不同条件下,MB2发现率在上颌第一磨牙分别为34.26%、51.85%和78.24%;第二磨牙分别为16.17%、23.65%和41.32%.改进开髓口和使用DOM可显著提高MB2发现率,与传统开髓口间的差别有统计学意义,P<0.05.结论:建议将上颌磨牙开髓口由传统的三角形改为斜四边形,并使用合适的放大技术和增强照明,以提高MB2发现率和治疗率.  相似文献

目的:运用透明标本法观察离体上颌第一、二磨牙近颊根根管形态,并进行分类和讨论其临床意义。方法:收集离体上颌第一、二磨牙共275颗,制成透明标本。根管显微镜下观察、记录近颊根的根管数目、走向、融合和分支情况,并进行分类。结果:①上颌磨牙近颊根管形态各异,可概括为八种类型,双根管中以2-1型和2-2型多见;双根管率在第一磨牙和第二磨牙分别为81.48%和49.7%。②近颊根主根管(MB)和近颊根第二根管(MB2)的尖1/3多弯向远中,MB2在近根管口1mm-3mm处有一斜向近中的弯曲点。结论:掌握上颌磨牙近颊根1-2型、2-1-2型和1-2-1型根管形态,是提高上颌磨牙根管治疗技巧和成功率的关键;有效去除根管近中壁部分突起的牙本质,以减小冠1/3处的弯曲度,可望提高MB2根管的探通率。  相似文献

探讨上颌第二乳磨牙近颊第二根管(MB2)的特征及临床治疗方法。选取患牙髓炎或根尖周炎的上颌第二乳磨牙95颗,拍摄术前及根充后 X 线片,采用耳鼻喉内窥镜观察测量髓室底各根管口位置关系;术中采用10号锉结合17%EDTA 凝胶探查根管,手动器械进行根管预备,螺旋输送器导入 Vitapex 糊剂充填根管。在95颗上颌第二乳磨牙中,有14颗存在 MB2,发现率为14.7%,MB2位于 MB-P 根管口之间假想连线的近中。  相似文献

目的:寻找并尽量扩通上颌磨牙近中颊根第二根管,统计其发生率及通畅率。方法:离体上颌磨牙11颗,开髓、拔髓,寻找并尽量扩通近中颊根第二根管即MB2,根管预备、充填,拍片。统计其发生及发现率。临床上对146颗上颌磨牙开髓、拔髓,寻找并尽量扩通近中颊根第二根管即MB2,根管预备,充填,拍片。结果:离体牙10颗,上颌第一磨牙近中颊根第二根管发现率为60%,通畅率为50%。94颗上颌第一磨牙,近中颊根第二根管发现率为44.68%.通畅率为29.78%,52颗上颌第二磨牙,发现率为11.53%,通畅率为7.69%。结论:上颌第一磨牙近中颊根第二根管的发生率较高,其寻找、定位及扩通尽管有一定的困难,临床医生仍应注意寻找,以提高上颌磨牙根管治疗的成功率。  相似文献

目的:探讨提高老年人上颌第一磨牙近颊第二根管(MB2)发现率的临床方法.方法:按入选条件随机选取65岁以上老年人需要根管治疗的上颌第一磨牙80颗,分为两组:试验组45颗,用头灯放大镜寻找MB2并记录其发现率和根管找寻时间;对照组35颗,在常规光源下寻找MB2并记录发生率和根管找寻时间,与试验组进行比较.结果:试验组MB2发现率为86.67%、总根管找寻时间为20.13±0.42(分钟);对照组MB2发现率40%、总根管找寻时间31.21±0.72(min),两组间MB2发现率及总根管找寻时间存在显著性差异(P<0.05).结论:寻找老年人上颌第一磨牙MB2时,头灯放大镜明显优于常规光源,经济实惠、适合临床推广.  相似文献

目的研究上颌第二磨牙近中颊根第二根管的临床发现率。方法采用改良髓腔入口和探查近中颊根根管口与腭根根管口之间发育沟或暗线的方法研究上颌第二磨牙近中颊根第二根管的肉眼发现率。结果60例患者的64颗上颌第二磨牙中有19颗存在近中颊根第二根管,发现率为29.7%。结论改良髓腔入口和探查近中颊根根管口与腭根根管口之间发育沟或暗线的方法,有利于发现和治疗上颌第二磨牙近中颊根第二根管。  相似文献
